On quiz pages:
* change background to light blue;
* replace right/wrong indicators with new arrows;
* change 'flag' question icon;
* update layout of questions;
* standardize question comment styles;
* display radio and checkbox buttons on results
* changed last-chance save to not be async to fix FF issue
Test plan:
* view all quiz pages (new/edit, take, review, history)
and verify that they work as expected.
