fix array form parameters in live api docs 

test plan:
 - go to /doc/api/live
 - enter an API token and click "Save token"
 - expand "Courses" and "Update courses"
 - enter an account_id and one or more course_ids
   separated by commas
 - click "Try it out"
 - the request should succeed

fixes ADMIN-1124
