Remove a line that logs whether confChange was non-null. This was previously a pointer, and the logging indicated whether the caller expected a result. It is now a reference type, and we can't determine what the caller expects.

Move code in ApplyMetadataMutation from .h to .cpp
...
Not only can we now compile this giant code block in parallel with the
rest of the code, we also no longer compile this giant function twice!
(Once for the master, once for the proxy.)
