Fixes to AC::Streaming docs

* Unlink the Rails module automatically
* Inline the documentation links for unicorn and passenger
* Use RDoc fixed-width for passenger_buffer_response instead of markdown
* TIL: about linking to headings, so fixed that for "Middlewares" section
