Bob Wilson 05de0298d2 Rip out flags for controlling C++ "production mode" separately. 
This is old leftover cruft from the days when C++ was not yet ready
for prime time.

analyzer-plugin Remove EnhancedBugReport and RangedBugReport - pull all the extra functionality they provided into their parent BugReport.
