Summary:
Delete the BasicBlockPass and BasicBlockManager, all its dependencies and update documentation.
The BasicBlockManager was improperly tested and found to be potentially broken, and was deprecated as of rL373254.
In light of the switch to the new pass manager coming before the next release, this patch is a first cleanup of the LegacyPassManager.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

This is a minor cleanup in preparation for a change to export DI
statistics from -check-debugify. To do that, it would be cleaner to have
a dedicated header for the debugify interface.

This adds a -debugify-each mode to opt which, when enabled, wraps each
{Module,Function}Pass in a pipeline with logic to add, check, and strip
synthetic debug info for testing purposes.
This mode can be used to test complex pipelines for debug info bugs, or
to collect statistics about the number of debug values & locations lost
throughout various stages of a pipeline.

We've been running doxygen with the autobrief option for a couple of
years now. This makes the \brief markers into our comments
redundant. Since they are a visual distraction and we don't want to
encourage more \brief markers in new code either, this patch removes
them all.

These are self-contained in functionality so it makes sense to separate them,
as opt.cpp has grown quite big already.
Following Eric's suggestions, if this code is ever deemed useful outside of
tools/opt, it will make sense to move it to one of the LLVM libraries like IR.
