Benjamin Kramer	a7c40ef022	Canonicalize header guards into a common format. Add header guards to files that were missing guards. Jim Grosbach	a85a4e21c9	Re-apply r115363 and r115366 now that r115525 has removed the un-needed header that caused the circular dependencies on Linux.
