Fix incorrect merges of m0 inits in loops.
It was assumed that if a clobbering instruction appears in
the same block as an init and the clobbering instruction
does not dominate the init then it does not interfere with
init.
This does not work in the presence of loops, where in this
scenario, the clobbering instruction does interfere with
the init in another iteration.
To fix this, do not check for block equality and defer the
decision to the predecessor check.

This is the groundwork required to implement strictfp. For now, this
should be NFC for regular instructoins (many instructions just gain an
extra use of a reserved register). Regalloc won't rematerialize
instructions with reads of physical registers, but we were suffering
from that anyway with the exec reads.
Should add it for all the related FP uses (possibly with some
extras). I did not add it to either the gpr index mode instructions
(or every single VALU instruction) since it's a ridiculous feature
already modeled as an arbitrary side effect.
Also work towards marking instructions with FP exceptions. This
doesn't actually set the bit yet since this would start to change
codegen. It seems nofpexcept is currently not implied from the regular
IR FP operations. Add it to some MIR tests where I think it might
matter.
Summary:
After hoisting and merging m0 initializations schedule them as early as
possible in the MBB. This helps the scheduler avoid hazards in some
cases.

Summary:
Enable hoisting and merging m0 defs that are initialized with the same
immediate value. Fixes bug where removed instructions are not considered
to interfere with other inits, and make sure to not hoist inits before block
prologues.

In preparation for adding support for named vregs we are changing the sigil for
physical registers in MIR to '$' from '%'. This will prevent name clashes of
named physical register with named vregs.
