Therefore use the language version instead of the feature macro.

Some headers which require the version header depend on other headers to
provide it. Include the version header in all top-level headers to make
sure a header cleanup can't remove the version header.
Note this doesn't add the version header to the c headers.

We've stopped doing it in libc++ for a while now because these names
would end up rotting as we move things around and copy/paste stuff.
This cleans up all the existing files so as to stop the spreading
as people copy-paste headers around.
`is_error_condition_enum_v` and `is_error_code_enum_v` are currently of
type `size_t`, but the standard mandates they are of type `bool`.
This is an ABI break technically since the size of these variable
templates has changed. Document it as such in the release notes.

All supported compilers provide support for inline variables in C++17 now.
Also, as a fly-by fix, replace some uses of _LIBCPP_CONSTEXPR by just
constexpr.
The only exception in this patch is `std::ignore`, which is provided
prior to C++17. Since it is defined in an anonymous namespace, it always
has internal linkage anyway, so using an inline variable there doesn't
provide any benefit. Instead, `inline` was removed entirely on `std::ignore`.

C++03 didn't support `explicit` conversion operators;
but Clang's C++03 mode does, as an extension, so we can use it.
This lets us make the conversion explicit in `std::function` (even in '03),
and remove some silly metaprogramming in `std::basic_ios`.
Drive-by improvements to the tests for these operators, in addition
to making sure all these tests also run in `c++03` mode.

Searching the TeX source for '#include' is a good way to find all of these
mandates.
The new tests are all autogenerated by utils/generate_header_inclusion_tests.py.
I was SHOCKED by how many mandates there are, and how many of them
libc++ wasn't conforming with.

Mostly, *don't* include <experimental/__config> from C++17 <any>,
because that doesn't make any sense. I think it was just a cut-and-paste
typo when this header moved from experimental/.

defeated my regular expressions.
We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

of symbols. This is better described by _LIBCPP_INLINE_VISIBILITY, which
is actually always defined the same.
This change is orthogonal to the decision of what we're actually going
to do with _LIBCPP_INLINE_VISIBILITY -- it just simplifies things by
having one canonical way of doing things.
Note that this commit had originally been applied in r336369 and then
reverted in r336382 because of unforeseen problems. Both of these problems
have now been fixed.

This reverts commit r336369. The commit had two problems:
1. __pbump was marked as _LIBCPP_EXTERN_TEMPLATE_INLINE_VISIBILITY instead of
_LIBCPP_INLINE_VISIBILITY, which lead to two symbols being added in the
dylib and the check-cxx-abilist failing.
2. The LLDB tests started failing because they undefine
`_LIBCPP_INLINE_VISIBILITY`. I need to figure out why they do that and
fix the tests before we can go forward with this change.

In order to allow inlining of previously out-of-line functions without an ABI break
libc++ provides legacy definitions in the dylib that old programs can
continue to use. Unfortunatly Windows link.exe detects this hack and diagnoses the duplicate
definitions.
This patch disable the duplicate definitions on Windows by adding an ABI option
which disables all "legacy out-of-line symbols"

The name _LIBCPP_TYPE_VIS_ONLY is no longer accurate because both
_LIBCPP_TYPE_VIS and _LIBCPP_TYPE_VIS_ONLY expand to
__attribute__((__type_visibility__)) with Clang. The only remaining difference
is that _LIBCPP_TYPE_VIS_ONLY can be applied to templates whereas
_LIBCPP_TYPE_VIS cannot (due to dllimport/dllexport not being allowed on
templates).
This patch renames _LIBCPP_TYPE_VIS_ONLY to _LIBCPP_TEMPLATE_VIS.

Currently libc++ compiles a special version of error_category()
into the dylib. This definition is no longer needed, and doesn't
work on Windows due to dllimport/dllexport semantics.
For those reasons this patch introduces an option to
disable/enable this definition. By default the definition
is provided in ABI v1 except on windows. This patch
also addresses D28210.
