Removed some unused headers, replaced some headers with forward class declarations.
Found using simple scripts like this one:

Add header guards to files that were missing guards. Remove #endif comments
as they don't seem common in LLVM (we can easily add them back if we decide
they're useful)
Changes made by clang-tidy with minor tweaks.

Again, tools are trickier to pick the main module header for than
library source files. I've started to follow the pattern of using
LLVMContext.h when it is included as a stub for program source files.

There are some that I didn't remove this round because they looked like
obvious stubs. There are dead variables in gtest too, they should be
fixed upstream.
