Currently, we assert that `CXXCtorInitializer`s are field initializers. Replace
the assertion with an early return. Otherwise, we crash every time we process a
constructor with a non-field (e.g. base class) initializer.

The API for `AggregateStorageLocation` does not allow for missing fields (it asserts). Therefore, it is incorrect to filter out any fields at location-creation time which may be accessed by the code. Currently, we limit filtering to private, base-calss fields on the assumption that those can never be accessed. However, `friend` declarations can invalidate that assumption, thereby breaking our invariants.
This patch removes said field filtering to avoid violating the invariant of "no missing fields" for `AggregateStorageLocation`.

In case where we have removed all declarations for a given declaration name
entry we should remove the whole StoredDeclsMap entry.

This is bringing the following benefits:
* Full access to the preprocessor state during dependency scanning. E.g. a component can see what includes were taken and where they were located in the actual sources.
* Improved performance for dependency scanning. Measurements with a release+thin-LTO build shows ~ -11% reduction in wall time.
* Opportunity to use dependency scanning lexing to speed-up skipping of excluded conditional blocks during normal preprocessing (as follow-up, not part of this patch).
For normal preprocessing measurements show differences are below the noise level.
Since, after this change, we don't minimize sources and pass them in place of the real sources, `DependencyScanningFilesystem` is not technically necessary, but it has valuable performance benefits for caching file `stat`s along with the results of scanning the sources. So the setup of using the `DependencyScanningFilesystem` during a dependency scan remains.

When constructing the `Environment`, the `this` pointee is established
for a `CXXMethodDecl` by looking at its parent. However, inside of
lambdas, a `CXXThisExpr` refers to the captured `this` coming from the
enclosing member function.
When establishing the `this` pointee for a function, we check whether
the function is a lambda, and check for an enclosing member function
to establish the `this` pointee storage location.

Support for unions is incomplete (per 99f7d55e) and the `this` pointee
storage location is not set for unions. The assert in
`VisitCXXThisExpr` is then guaranteed to trigger when analyzing member
functions of a union.
This commit changes the assert to an early-return. Any expression may
be undefined, and so having a value for the `CXXThisExpr` is not a
postcondition of the transfer function.

which where awkwardly broken up like this:

WG14 DR423 (https://www.open-std.org/jtc1/sc22/wg14/www/docs/n2148.htm#dr_423),
resolved during the C11 time frame, changed the way qualifiers are
handled on function return types and in cast expressions after it was
noted that these types are now directly observable via generic
selection expressions. In C, the function declarator is adjusted to
ignore all qualifiers (including _Atomic qualifiers).
Clang already handles the cast expression case correctly (by performing
the lvalue conversion, which drops the qualifiers as well), but with
these changes it will now also handle function declarations
appropriately.

Much of the size of PCH/PCM files comes from stored SourceLocations.
These are encoded using (almost) their raw value, VBR-encoded. Absolute
SourceLocations can be relatively large numbers, so this commonly takes
20-30 bits per location.
We can reduce this by exploiting redundancy: many "nearby" SourceLocations are
stored differing only slightly and can be delta-encoded.
Randam-access loading of AST nodes constrains how long these sequences
can be, but we can do it at least within a node that always gets
deserialized as an atomic unit.
TypeLoc is implemented in this patch as it's a relatively small change
that shows most of the API.
This saves ~3.5% of PCH size, I have local changes applying this technique
further that save another 3%, I think it's possible to get to 10% total.

Sub-expressions that are logical operators are not spelled out
separately in basic blocks, so we need to manually visit them when we
encounter them. We do this in both the `TerminatorVisitor`
(conditionally) and the `TransferVisitor` (unconditionally), which can
cause cause an expression to be visited twice when the binary
operators are nested 2+ times.
This changes the visit in `TransferVisitor` to check if it has been
evaluated before trying to visit the sub-expression.

The combination of
- AlignConsecutiveAssignments.Enabled = true
- BinPackArguments = false
would result in the first continuation line of a braced-init-list being
improperly indented (missing a shift) when in a continued function call.
Indentation was also wrong for braced-init-lists continuing a
direct-list-initialization. Check for opening braced lists in
continuation and ensure that the correct shift occurs.

Due to how parseBracedList always stopped on the first closing angle
bracket and was used in parsing angle bracketed expression inside concept
definition, nested brackets inside concepts were parsed incorrectly.
nextToken() call before calling parseBracedList is required because
we were processing opening angle bracket inside parseBracedList second
time leading to incorrect logic after my fix.

This is generally a better default for tools other than the compiler, which
shouldn't assume a PCH file on disk is something they can consume.
Preserve the old behavior in places associated with libclang/c-index-test
(including ASTUnit) as there are tests relying on it and most important
consumers are out-of-tree. It's unclear whether the tests are specifically
trying to test this functionality, and what the downstream implications of
removing it are. Hopefully someone more familiar can clean this up in future.

This fixes regression introduced in 5e5efd8a91.
Before the culprit commit, macros in WhitespaceSensitiveMacros were correctly formatted even if their closing parenthesis weren't followed by semicolon (or, to be precise, when they were followed by a newline).
That commit changed the type of the macro token type from TT_UntouchableMacroFunc to TT_FunctionLikeOrFreestandingMacro.
Correct formatting (with `WhitespaceSensitiveMacros = ['FOO']`):

The ShouldShiftBeAdded lambda checks if extra space should be
added before the wrapped part of a braced list. If the first
element of the list is wrapped, no extra space should be added.

If clang is passed "-include foo.h", it will rewrite to "-include-pch foo.h.pch"
before passing it to cc1, if foo.h.pch exists.
Existence is checked, but validity is not. This is probably a reasonable
assumption for the compiler itself, but not for clang-based tools where the
actual compiler may be a different version of clang, or even GCC.
In the end, we lose our -include, we gain a -include-pch that can't be used,
and the file often fails to parse.
I would like to turn this off for all non-clang invocations (i.e.
createInvocationFromCommandLine), but we have explicit tests of this behavior
for libclang and I can't work out the implications of changing it.
Instead this patch:
- makes it optional in the driver, default on (no change)
- makes it optional in createInvocationFromCommandLine, default on (no change)
- changes driver to do IO through the VFS so it can be tested
- tests the option
- turns the option off in clangd where the problem was reported
Subsequent patches should make libclang opt in explicitly and flip the default
for all other tools. It's probably also time to extract an options struct
for createInvocationFromCommandLine.
