Christoph Hellwig eb81cf9d0e iomap: renumber IOMAP_HOLE to 0 
Instead of keeping a separate unnamed state for uninitialized iomaps,
renumber IOMAP_HOLE to zero so that an uninitialized iomap is treated
as a hole.
