Heiko Carstens 097a116c7e s390/cpuinfo: show dynamic and static cpu mhz
Show the dynamic and static cpu mhz of each cpu. Since these values
are per cpu this requires a fundamental extension of the format of
/proc/cpuinfo.

Historically we had only a single line per cpu and a summary at the
top of the file. This format is hardly extendible if we want to add
more per cpu information.

Therefore this patch adds per cpu blocks at the end of /proc/cpuinfo:

cpu             : 0
cpu Mhz dynamic : 5504
cpu Mhz static  : 5504

cpu             : 1
cpu Mhz dynamic : 5504
cpu Mhz static  : 5504

cpu             : 2
cpu Mhz dynamic : 5504

cpu Mhz static  : 5504

Right now each block contains only the dynamic and static cpu mhz,
but it can be easily extended like on every other architecture.

This extension is supposed to be compatible with the old format.

Heiko Carstens 37cd944c8d s390/pgtable: add mapping statistics
Add statistics that show how memory is mapped within the kernel
identity mapping. This is more or less the same like git
commit ce0c0e50f9 ("x86, generic: CPA add statistics about state
of direct mapping v4") for x86.

I also intentionally copied the lower case "k" within DirectMap4k vs
the upper case "M" and "G" within the two other lines. Let's have
consistent inconsistencies across architectures.

The output of /proc/meminfo now contains these additional lines:

DirectMap4k:        2048 kB
DirectMap1M:     3991552 kB
DirectMap2G:     4194304 kB

The implementation on s390 is lockless unlike the x86 version, since I
assume changes to the kernel mapping are a very rare event. Therefore
it really doesn't matter if these statistics could potentially be
inconsistent if read while kernel pages tables are being changed.

Heiko Carstens e8a97e42dc s390/pageattr: allow kernel page table splitting
set_memory_ro() and set_memory_rw() currently only work on 4k
mappings, which is good enough for module code aka the vmalloc area.

However we stumbled already twice into the need to make this also work
on larger mappings:
- the ro after init patch set
- the crash kernel resize code

Therefore this patch implements automatic kernel page table splitting
if e.g. set_memory_ro() would be called on parts of a 2G mapping.
This works quite the same as the x86 code, but is much simpler.

In order to make this work and to be architecturally compliant we now
always use the csp, cspg or crdte instructions to replace valid page
table entries. This means that set_memory_ro() and set_memory_rw()
will be much more expensive than before. In order to avoid huge
latencies the code contains a couple of cond_resched() calls.

The current code only splits page tables, but does not merge them if
it would be possible.  The reason for this is that currently there is
no real life scenarion where this would really happen. All current use
cases that I know of only change access rights once during the life
time. If that should change we can still implement kernel page table
merging at a later time.

Heiko Carstens 9e20b4dac1 s390/pgtable: make pmd and pud helper functions available
Make pmd_wrprotect() and pmd_mkwrite() available independently from
CONFIG_TRANSPARENT_HUGEPAGE and CONFIG_HUGETLB_PAGE so these can be
used on the kernel mapping.

Also introduce a couple of pud helper functions, namely pud_pfn(),
pud_wrprotect(), pud_mkwrite(), pud_mkdirty() and pud_mkclean()
which only work on the kernel mapping.

Always use PAGE_KERNEL when re-enabling pages within the kernel
mapping due to debug pagealloc. Without using this pgprot value

Heiko Carstens 2dffdcbac9 s390/vmem: introduce and use SEGMENT_KERNEL and REGION3_KERNEL
Instead of open-coded SEGMENT_KERNEL and REGION3_KERNEL assignments use
defines.  Also to make e.g. pmd_wrprotect() work on the kernel mapping
a couple more flags must be set. Therefore add the missing flags also.

In order to make everything symmetrical this patch also adds software
dirty, young, read and write bits for region 3 table entries.

Heiko Carstens 2e9996fcf8 s390/vmem: align segment and region tables to 16k
Usually segment and region tables are 16k aligned due to the way the
buddy allocator works.  This is not true for the vmem code which only
asks for a 4k alignment. In order to be consistent enforce a 16k
alignment here as well.

This alignment will be assumed and therefore is required by the
pageattr code.

Christian Borntraeger 1c343f7b0e KVM: s390/mm: Fix CMMA reset during reboot
commit 1e133ab296 ("s390/mm: split arch/s390/mm/pgtable.c") factored
out the page table handling code from __gmap_zap and  __s390_reset_cmma
into ptep_zap_unused and added a simple flag that tells which one of the
function (reset or not) is to be made. This also changed the behaviour,
as it also zaps unused page table entries on reset.
Turns out that this is wrong as s390_reset_cmma uses the page walker,
which DOES NOT take the ptl lock.

The most simple fix is to not do the zapping part on reset (which uses
the walker)

We already indicate that facility but don't implement it in our pfmf
interception handler. Let's add a new storage key handling function for
conditionally setting the guest storage key.

As we will reuse this function later on, let's directly implement returning
the old key via parameter and indicating if any change happened via rc.

The current calculation is wrong if absolute != real address. Let's just
calculate the start address for 4k frames upfront. Otherwise, the
calculated end address will be wrong, resulting in wrong memory
location/storage keys getting touched.

To keep low-address protection working (using the effective address),
we have to move the check.

Christian Borntraeger dcc98ea614 KVM: s390: fixup I/O interrupt traces
We currently have two issues with the I/O  interrupt injection logging:
1. All QEMU versions up to 2.6 have a wrong encoding of device numbers
etc for the I/O interrupt type, so the inject VM_EVENT will have wrong
data. Let's fix this by using the interrupt parameters and not the
interrupt type number.
2. We only log in kvm_s390_inject_vm, but not when coming from
kvm_s390_reinject_io_int or from flic. Let's move the logging to the
common __inject_io function.

We also enhance the logging for delivery to match the data.

David Hildenbrand f9cbd9b025 KVM: s390: provide CMMA attributes only if available
Let's not provide the device attribute for cmma enabling and clearing
if the hardware doesn't support it.

This also helps getting rid of the undocumented return value "-EINVAL"
in case CMMA is not available when trying to enable it.

David Hildenbrand 0a763c780b KVM: s390: interface to query and configure cpu subfunctions
We have certain instructions that indicate available subfunctions via
a query subfunction (crypto functions and ptff), or via a test bit
function (plo).

By exposing these "subfunction blocks" to user space, we allow user space
to
1) query available subfunctions and make sure subfunctions won't get lost
   during migration - e.g. properly indicate them via a CPU model
2) change the subfunctions to be reported to the guest (even adding
   unavailable ones)

This mechanism works just like the way we indicate the stfl(e) list to
user space.

This way, user space could even emulate some subfunctions in QEMU in the
future. If this is ever applicable, we have to make sure later on, that
unsupported subfunctions result in an intercept to QEMU.

Please note that support to indicate them to the guest is still missing
and requires hardware support. Usually, the IBC takes already care of these
subfunctions for migration safety. QEMU should make sure to always set
these bits properly according to the machine generation to be emulated.

Available subfunctions are only valid in combination with STFLE bits
retrieved via KVM_S390_VM_CPU_MACHINE and enabled via
KVM_S390_VM_CPU_PROCESSOR. If the applicable bits are available, the
indicated subfunctions are guaranteed to be correct.

David Hildenbrand 22be5a1331 KVM: s390: forward ESOP if available
ESOP guarantees that during a protection exception, bit 61 of real location
168-175 will only be set to 1 if it was because of ALCP or DATP. If the
exception is due to LAP or KCP, the bit will always be set to 0.

The old SOP definition allowed bit 61 to be unpredictable in case of LAP
or KCP in some conditions. So ESOP replaces this unpredictability by
a guarantee.

Therefore, we can directly forward ESOP if it is available on our machine.
We don't have to do anything when ESOP is disabled - the guest will simply
expect unpredictable values. Our guest access functions are already
handling ESOP properly.

Please note that future functionality in KVM will require knowledge about
ESOP being enabled for a guest or not.

David Hildenbrand 15c9705f0c KVM: s390: interface to query and configure cpu features
For now, we only have an interface to query and configure facilities
indicated via STFL(E). However, we also have features indicated via
SCLP, that have to be indicated to the guest by user space and usually
require KVM support.

This patch allows user space to query and configure available cpu features
for the guest.

Please note that disabling a feature doesn't necessarily mean that it is
completely disabled (e.g. ESOP is mostly handled by the SIE). We will try
our best to disable it.

Most features (e.g. SCLP) can't directly be forwarded, as most of them need
in addition to hardware support, support in KVM. As we later on want to
turn these features in KVM explicitly on/off (to simulate different
behavior), we have to filter all features provided by the hardware and
make them configurable.

Store hypervisor information is a valid instruction not only in
supervisor state but also in problem state, i.e. the guest's
userspace. Its execution is not only computational and memory
intensive, but also has to get hold of the ipte lock to write to the
guest's memory.

This lock is not intended to be held often and long, especially not
from the untrusted guest userspace. Therefore we apply rate limiting
of sthyi executions per VM.

Janosch Frank 95ca2cb579 KVM: s390: Add sthyi emulation
Store Hypervisor Information is an emulated z/VM instruction that
provides a guest with basic information about the layers it is running
on. This includes information about the cpu configuration of both the
machine and the lpar, as well as their names, machine model and
machine type. This information enables an application to determine the
maximum capacity of CPs and IFLs available to software.

The instruction is available whenever the facility bit 74 is set,
otherwise executing it results in an operation exception.

It is important to check the validity flags in the sections before
using data from any structure member. It is not guaranteed that all
members will be valid on all machines / machine configurations.

Janosch Frank e435dc3139 s390: Make cpc_name accessible
sclp_ocf.c is the only way to get the cpc name, as it registers the
sole event handler for the ocf event. By creating a new global
function that copies that name, we make it accessible to the world
which longs to retrieve it.

Additionally we now also store the cpc name as EBCDIC, so we don't
have to convert it to and from ASCII if it is requested in native
encoding.

Michal Hocko 6904817607 vdso: make arch_setup_additional_pages wait for mmap_sem for write killable
most architectures are relying on mmap_sem for write in their
arch_setup_additional_pages.  If the waiting task gets killed by the oom
killer it would block oom_reaper from asynchronous address space reclaim
and reduce the chances of timely OOM resolving.  Wait for the lock in
the killable mode and return with EINTR if the task got killed while
waiting.

Xunlei Pang 7a0058ec78 s390/kexec: consolidate crash_map/unmap_reserved_pages() and arch_kexec_protect(unprotect)_crashkres()
Commit 3f625002581b ("kexec: introduce a protection mechanism for the
crashkernel reserved memory") is a similar mechanism for protecting the
crash kernel reserved memory to previous crash_map/unmap_reserved_pages()
implementation, the new one is more generic in name and cleaner in code
(besides, some arch may not be allowed to unmap the pgtable).

Therefore, this patch consolidates them, and uses the new
arch_kexec_protect(unprotect)_crashkres() to replace former
crash_map/unmap_reserved_pages() which by now has been only used by
S390.

The consolidation work needs the crash memory to be mapped initially,
this is done in machine_kdump_pm_init() which is after
reserve_crashkernel().  Once kdump kernel is loaded, the new
arch_kexec_protect_crashkres() implemented for S390 will actually
unmap the pgtable like before.

The reason seems to be that do_sigsegv is not initializing siginfo
structure defined on the stack completely so it will leak 4B of
the previous stack content. Fix it simply by initializing si_errno
to 0 (same as do_sigbus does already).

Zhaoxiu Zeng fff7fb0b2d lib/GCD.c: use binary GCD algorithm instead of Euclidean
The binary GCD algorithm is based on the following facts:
	1. If a and b are all evens, then gcd(a,b) = 2 * gcd(a/2, b/2)
	2. If a is even and b is odd, then gcd(a,b) = gcd(a/2, b)
	3. If a and b are all odds, then gcd(a,b) = gcd((a-b)/2, b) = gcd((a+b)/2, b)

Even on x86 machines with reasonable division hardware, the binary
algorithm runs about 25% faster (80% the execution time) than the
division-based Euclidian algorithm.

On platforms like Alpha and ARMv6 where division is a function call to
emulation code, it's even more significant.

There are two variants of the code here, depending on whether a fast
__ffs (find least significant set bit) instruction is available.  This
allows the unpredictable branches in the bit-at-a-time shifting loop to
be eliminated.

If fast __ffs is not available, the "even/odd" GCD variant is used.

Petr Mladek 42a0bb3f71 printk/nmi: generic solution for safe printk in NMI
printk() takes some locks and could not be used a safe way in NMI
context.

The chance of a deadlock is real especially when printing stacks from
all CPUs.  This particular problem has been addressed on x86 by the
commit a9edc88093 ("x86/nmi: Perform a safe NMI stack trace on all
CPUs").

The patchset brings two big advantages.  First, it makes the NMI
backtraces safe on all architectures for free.  Second, it makes all NMI
messages almost safe on all architectures (the temporary buffer is
limited.  We still should keep the number of messages in NMI context at
minimum).

Note that there already are several messages printed in NMI context:
WARN_ON(in_nmi()), BUG_ON(in_nmi()), anything being printed out from MCE
handlers.  These are not easy to avoid.

This patch reuses most of the code and makes it generic.  It is useful
for all messages and architectures that support NMI.

The alternative printk_func is set when entering and is reseted when
leaving NMI context.  It queues IRQ work to copy the messages into the
main ring buffer in a safe context.

__printk_nmi_flush() copies all available messages and reset the buffer.
Then we could use a simple cmpxchg operations to get synchronized with
writers.  There is also used a spinlock to get synchronized with other
flushers.

We do not longer use seq_buf because it depends on external lock.  It
would be hard to make all supported operations safe for a lockless use.
It would be confusing and error prone to make only some operations safe.

The code is put into separate printk/nmi.c as suggested by Steven
Rostedt.  It needs a per-CPU buffer and is compiled only on
architectures that call nmi_enter().  This is achieved by the new
HAVE_NMI Kconfig flag.

The are MN10300 and Xtensa architectures.  We need to clean up NMI
handling there first.  Let's do it separately.

Hugh Dickins fd8cfd3000 arch: fix has_transparent_hugepage()
I've just discovered that the useful-sounding has_transparent_hugepage()
is actually an architecture-dependent minefield: on some arches it only
builds if CONFIG_TRANSPARENT_HUGEPAGE=y, on others it's also there when
not, but on some of those (arm and arm64) it then gives the wrong
answer; and on mips alone it's marked __init, which would crash if
called later (but so far it has not been called later).

Straighten this out: make it available to all configs, with a sensible
default in asm-generic/pgtable.h, removing its definitions from those
arches (arc, arm, arm64, sparc, tile) which are served by the default,
adding #define has_transparent_hugepage has_transparent_hugepage to
those (mips, powerpc, s390, x86) which need to override the default at
runtime, and removing the __init from mips (but maybe that kind of code
should be avoided after init: set a static variable the first time it's
called).

Michael Holzheu 6edf0aa4f8 s390/bpf: fix recache skb->data/hlen for skb_vlan_push/pop
In case of usage of skb_vlan_push/pop, in the prologue we store
the SKB pointer on the stack and restore it after BPF_JMP_CALL
to skb_vlan_push/pop.

Unfortunately currently there are two bugs in the code:

 1) The wrong stack slot (offset 170 instead of 176) is used
 2) The wrong register (W1 instead of B1) is saved

So fix this and use correct stack slot and register.

Michael Holzheu 0fa963553a s390/bpf: reduce maximum program size to 64 KB
The s390 BFP compiler currently uses relative branch instructions
that only support jumps up to 64 KB. Examples are "j", "jnz", "cgrj",
etc.  Currently the maximum size of s390 BPF programs is set
to 0x7ffff.  If branches over 64 KB are generated the, kernel can
crash due to incorrect code.

So fix this an reduce the maximum size to 64 KB. Programs larger than
that will be interpreted.

Daniel Borkmann d93a47f735 bpf, s390: add support for constant blinding
This patch adds recently added constant blinding helpers into the
s390 eBPF JIT. In the bpf_int_jit_compile() path, requirements are
to utilize bpf_jit_blind_constants()/bpf_jit_prog_release_other()
pair for rewriting the program into a blinded one, and to map the
BPF_REG_AX register to a CPU register. The mapping of BPF_REG_AX
is at r12 and similarly like in x86 case performs reloading when
ld_abs/ind is used. When blinding is not used, there's no additional
overhead in the generated image.

When BPF_REG_AX is used, we don't need to emit skb->data reload when
helper function changed skb->data, as this will be reloaded later
on anyway from stack on ld_abs/ind, where skb->data is needed. s390
allows for this w/o much additional complexity unlike f.e. x86.

Daniel Borkmann d1c55ab5e4 bpf: prepare bpf_int_jit_compile/bpf_prog_select_runtime apis
Since the blinding is strictly only called from inside eBPF JITs,
we need to change signatures for bpf_int_jit_compile() and
bpf_prog_select_runtime() first in order to prepare that the
eBPF program we're dealing with can change underneath. Hence,
for call sites, we need to return the latest prog. No functional
change in this patch.

Christian Borntraeger c4a8de357e KVM: s390: set halt polling to 80 microseconds
on s390 we disabled the halt polling with commit 920552b213
("KVM: disable halt_poll_ns as default for s390x"), as floating
interrupts would let all CPUs have a successful poll, resulting
in much higher CPU usage (on otherwise idle systems).

With the improved selection of polls we can now retry halt polling.
Performance measurements with different choices like 25,50,80,100,200
microseconds showed that 80 microseconds seems to improve several cases
without increasing the CPU costs too much. Higher values would improve
the performance even more but increased the cpu time as well.
So let's start small and use this value of 80 microseconds on s390 until
we have a better understanding of cost/benefit of higher values.

Christian Borntraeger 3491caf275 KVM: halt_polling: provide a way to qualify wakeups during poll
Some wakeups should not be considered a sucessful poll. For example on
s390 I/O interrupts are usually floating, which means that _ALL_ CPUs
would be considered runnable - letting all vCPUs poll all the time for
transactional like workload, even if one vCPU would be enough.
This can result in huge CPU usage for large guests.
This patch lets architectures provide a way to qualify wakeups if they
should be considered a good/bad wakeups in regard to polls.

For s390 the implementation will fence of halt polling for anything but
known good, single vCPU events. The s390 implementation for floating
interrupts does a wakeup for one vCPU, but the interrupt will be delivered
by whatever CPU checks first for a pending interrupt. We prefer the
woken up CPU by marking the poll of this CPU as "good" poll.
This code will also mark several other wakeup reasons like IPI or
expired timers as "good". This will of course also mark some events as
not sucessful. As  KVM on z runs always as a 2nd level hypervisor,
we prefer to not poll, unless we are really sure, though.

This patch successfully limits the CPU usage for cases like uperf 1byte
transactional ping pong workload or wakeup heavy workload like OLTP
while still providing a proper speedup.

This also introduced a new vcpu stat "halt_poll_no_tuning" that marks
wakeups that are considered not good for polling.

Heiko Carstens c34a69059d s390/vmem: fix identity mapping
The identity mapping is suboptimal for the last 2GB frame. The mapping
will be established with a mix of 4KB and 1MB mappings instead of a
single 2GB mapping.

This happens because of a off-by-one bug introduced with
commit 50be634507 ("s390/mm: Convert bootmem to memblock").

Currently the identity mapping looks like this:

Alexander Yarygin 60a37709ce KVM: s390: Populate mask of non-hypervisor managed facility bits
When a guest is initializing, KVM provides facility bits that can be
successfully used by the guest. It's done by applying
kvm_s390_fac_list_mask mask on host facility bits stored by the STFLE
instruction. Facility bits can be one of two kinds: it's either a
hypervisor managed bit or non-hypervisor managed.

The hardware provides information which bits need special handling.
Let's automatically passthrough to guests new facility bits, that
don't require hypervisor support.

Peter Zijlstra (Intel) e9d867a67f sched: Allow per-cpu kernel threads to run on online && !active
In order to enable symmetric hotplug, we must mirror the online &&
!active state of cpu-down on the cpu-up side.

However, to retain sanity, limit this state to per-cpu kthreads.

Aside from the change to set_cpus_allowed_ptr(), which allow moving
the per-cpu kthreads on, the other critical piece is the cpu selection
for pinned tasks in select_task_rq(). This avoids dropping into
select_fallback_rq().

select_fallback_rq() cannot be allowed to select !active cpus because
its used to migrate user tasks away. And we do not want to move user
tasks onto cpus that are in transition.

Heiko Carstens 5606330627 s390/dumpstack: implement and use return_address()
Implement return_address() and use it instead of __builtin_return_address(n).

__builtin_return_address(n) is not guaranteed to work for n > 0,
therefore implement a private return_address() function which walks
the stack frames and returns the proper return address.

This way we get also rid of a compile warning which gcc 6.1 emits and
look like all other architectures.

We should never inject an exception after we manually rewound the PSW
(to retry the ESSA instruction in this case). This will mess up the PSW.
So this never worked and therefore never really triggered.

Looking at the details, we don't even have to perform any validity checks.
1. Bits 52-63 of an entry are stored as 0 by the hardware.
2. We are dealing with absolute addresses but only check for the prefix
   starting at address 0. This isn't correct and doesn't make much sense,
   cpus could still zap the prefix of other cpus. But as prefix pages
   cannot be swapped out without a notifier being called for the affected
   VCPU, a zap can never remove a protected prefix.

Martin Schwidefsky 3f6813b9a5 s390/fpu: allocate 'struct fpu' with the task_struct
Analog to git commit 0c8c0f03e3
"x86/fpu, sched: Dynamically allocate 'struct fpu'"
move the struct fpu to the end of the struct thread_struct,
set CONFIG_ARCH_WANTS_DYNAMIC_TASK_STRUCT and add the
setup_task_size() function to calculate the correct size
fo the task struct.

For the performance_defconfig this increases the size of
struct task_struct from 7424 bytes to 7936 bytes (MACHINE_HAS_VX==1)
or 7552 bytes (MACHINE_HAS_VX==0). The dynamic allocation of the
struct fpu is removed. The slab cache uses an 8KB block for the
task struct in all cases, there is enough room for the struct fpu.
For MACHINE_HAS_VX==1 each task now needs 512 bytes less memory.

Gerald Schaefer 723cacbd9d s390/mm: fix asce_bits handling with dynamic pagetable levels
There is a race with multi-threaded applications between context switch and
pagetable upgrade. In switch_mm() a new user_asce is built from mm->pgd and
mm->context.asce_bits, w/o holding any locks. A concurrent mmap with a
pagetable upgrade on another thread in crst_table_upgrade() could already
have set new asce_bits, but not yet the new mm->pgd. This would result in a
corrupt user_asce in switch_mm(), and eventually in a kernel panic from a
translation exception.

Fix this by storing the complete asce instead of just the asce_bits, which
can then be read atomically from switch_mm(), so that it either sees the
old value or the new value, but no mixture. Both cases are OK. Having the
old value would result in a page fault on access to the higher level memory,
but the fault handler would see the new mm->pgd, if it was a valid access
after the mmap on the other thread has completed. So as worst-case scenario
we would have a page fault loop for the racing thread until the next time
slice.

Also remove dead code and simplify the upgrade/downgrade path, there are no
upgrades from 2 levels, and only downgrades from 3 levels for compat tasks.
There are also no concurrent upgrades, because the mmap_sem is held with
down_write() in do_mmap, so the flush and table checks during upgrade can
be removed.

Sebastian Ott dba599091c s390/pci: fix use after free in dma_init
After a failure during registration of the dma_table (because of the
function being in error state) we free its memory but don't reset the
associated pointer to zero.

When we then receive a notification from firmware (about the function
being in error state) we'll try to walk and free the dma_table again.

Fix this by resetting the dma_table pointer. In addition to that make
sure that we free the iommu_bitmap when appropriate.

Halil Pasic 6d28f789bf KVM: s390: add clear I/O irq operation for FLIC
Introduce a FLIC operation for clearing I/O interrupts for a subchannel.

Rationale: According to the platform specification, pending I/O
interruption requests have to be revoked in certain situations. For
instance, according to the Principles of Operation (page 17-27), a
subchannel put into the installed parameters initialized state is in the
same state as after an I/O system reset (just parameters possibly changed).
This implies that any I/O interrupts for that subchannel are no longer
pending (as I/O system resets clear I/O interrupts). Therefore, we need an
interface to clear pending I/O interrupts.

Martin Schwidefsky c7d4d259b7 s390/crypto: cleanup and move the header with the cpacf definitions
The CPACF instructions are going be used in KVM as well, move the
defines and the inline functions from arch/s390/crypt/crypt_s390.h
to arch/s390/include/asm. Rename the header to cpacf.h and replace
the crypt_s390_xxx names with cpacf_xxx.

While we are at it, cleanup the header as well. The encoding for
the CPACF operations is odd, there is an enum for each of the CPACF
instructions with the hardware function code in the lower 8 bits of
each entry and a software defined number for the CPACF instruction
in the upper 8 bits. Remove the superfluous software number and
replace the enums with simple defines.

The crypt_s390_func_available() function tests for the presence
of a specific CPACF operations. The new name of the function is
cpacf_query and it works slightly different than before. It gets
passed an opcode of an CPACF instruction and a function code for
this instruction. The facility_mask parameter is gone, the opcode
is used to find the correct MSA facility bit to check if the CPACF
instruction itself is available. If it is the query function of the
given instruction is used to test if the requested CPACF operation
is present.

Heiko Carstens 2fd9227364 s390: add CPU_BIG_ENDIAN config option
Make sure that s390 appears to be a big endian machine by defining
this config option.

Without this s390 appears to be little endian as seen by e.g. the
recordmount script: "perl ./scripts/recordmcount.pl "s390" "little"
"64""
This has no practical impact within the script since the endian
variable is only evaluated for mips. However there are already a
couple of common code places which evaluate this config option. None
of them is relevant for s390 currently though.

To avoid any issues in the future (and fix the recordmcount oddity)
add the new config option.

Heiko Carstens 8497695243 s390/spinlock: avoid yield to non existent cpu
arch_spin_lock_wait_flags() checks if a spinlock is not held before
trying a compare and swap instruction. If the lock is unlocked it
tries the compare and swap instruction, however if a different cpu
grabbed the lock in the meantime the instruction will fail as
expected.

Subsequently the arch_spin_lock_wait_flags() incorrectly tries to
figure out if the cpu that holds the lock is running. However it is
using the wrong cpu number for this (-1) and then will also yield the
current cpu to the wrong cpu.

Fix this by adding a missing continue statement.

breakage to be doable.

Let's stop pretending that pages in page cache are special.  They are

Jessica Yu 425595a7fc livepatch: reuse module loader code to write relocations
Reuse module loader code to write relocations, thereby eliminating the need
for architecture specific relocation code in livepatch. Specifically, reuse
the apply_relocate_add() function in the module loader to write relocations
instead of duplicating functionality in livepatch's arch-dependent
klp_write_module_reloc() function.

In order to accomplish this, livepatch modules manage their own relocation
sections (marked with the SHF_RELA_LIVEPATCH section flag) and
livepatch-specific symbols (marked with SHN_LIVEPATCH symbol section
index). To apply livepatch relocation sections, livepatch symbols
referenced by relocs are resolved and then apply_relocate_add() is called
to apply those relocations.

In addition, remove x86 livepatch relocation code and the s390
klp_write_module_reloc() function stub. They are no longer needed since
relocation work has been offloaded to module loader.

Lastly, mark the module as a livepatch module so that the module loader
canappropriately identify and initialize it.

Jessica Yu f31e0960f3 module: s390: keep mod_arch_specific for livepatch modules
Livepatch needs to utilize the symbol information contained in the
mod_arch_specific struct in order to be able to call the s390
apply_relocate_add() function to apply relocations. Keep a reference to
syminfo if the module is a livepatch module. Remove the redundant vfree()
in module_finalize() since module_arch_freeing_init() (which also frees
those structures) is called in do_init_module(). If the module isn't a
livepatch module, we free the structures in module_arch_freeing_init() as
usual.

Alexander Potapenko be7635e728 arch, ftrace: for KASAN put hard/soft IRQ entries into separate sections
KASAN needs to know whether the allocation happens in an IRQ handler.
This lets us strip everything below the IRQ entry point to reduce the
number of unique stack traces needed to be stored.

Move the definition of __irq_entry to <linux/interrupt.h> so that the
users don't need to pull in <linux/ftrace.h>.  Also introduce the
__softirq_entry macro which is similar to __irq_entry, but puts the
corresponding functions to the .softirqentry.text section.

  fixed protection mask in the pte (which needs a system call to change
  and works on a per page basis), the user can map a (handful of)

  cheaply, without having to change every single page in the affected
  virtual memory range.

  This allows the dynamic switching of the protection bits of large
  amounts of virtual memory, via user-space instructions.  It also
  allows more precise control of MMU permission bits: for example the
  executable bit is separate from the read bit (see more about that
  below).

  This tree adds the MM infrastructure and low level x86 glue needed for
  that, plus it adds a high level API to make use of protection keys -
  if a user-space application calls:

        mmap(..., PROT_EXEC);

  or

        mprotect(ptr, sz, PROT_EXEC);

  (note PROT_EXEC-only, without PROT_READ/WRITE), the kernel will notice
  this special case, and will set a special protection key on this
  memory range.  It also sets the appropriate bits in the Protection
  Keys User Rights (PKRU) register so that the memory becomes unreadable
  and unwritable.

  So using protection keys the kernel is able to implement 'true'
  PROT_EXEC on x86 CPUs: without protection keys PROT_EXEC implies
  PROT_READ as well.  Unreadable executable mappings have security
  advantages: they cannot be read via information leaks to figure out

Gerald Schaefer fc897c95e9 s390/mm: handle PTE-mapped tail pages in fast gup
With the THP refcounting rework it is possible to see THP compound tail
pages mapped with PTEs during a THP split. This needs to be considered
when using page_cache_get_speculative(), which will always fail on tail
pages because ->_count is always zero. commit 7aef4172 "mm: handle
PTE-mapped tail pages in gerneric fast gup implementaiton" fixed it for
the generic fast gup code by using compound_head(page) instead of page,
but not for s390.

This patch is a 1:1 adaption of commit 7aef4172 for the s390 fast gup
code. Without this fix, gup will fall back to the slow path or fail
in the unlikely scenario that we hit a THP under splitting in-between
the page table split and the compound page split.

Heiko Carstens 91d3721176 s390: add DEBUG_RODATA support
git commit d2aa1acad2 ("mm/init: Add 'rodata=off' boot cmdline
parameter to disable read-only kernel mappings") adds a bogus warning
to the console which states that s390 does not support kernel memory
protection.

This however is not true. We do support that since a couple of years
however in a different way than the author of the above named patch
expected.

To get rid of the misleading message implement the mark_rodata_ro
function and emit a message which states the amount of memory which
was write protected already earlier.

This is the same what parisc currently does.

We currently do not support the kernel parameter "rodata=off" which
would allow to write to the rodata section again. However since we
have this feature since years without any problems there is no reason
to add support for this.

The cpumf_pmu_notfier() hotplug callback lacks handling of the
CPU_DOWN_FAILED case. That means, if CPU_DOWN_PREPARE failes, the PMC
of the CPU is not setup again. Furthermore the CPU_ONLINE_FROZEN case
will never be processed because of masking the switch expression with
CPU_TASKS_FROZEN.

Add handling for CPU_DOWN_FAILED transition to setup the PMC of the
CPU. Remove CPU_ONLINE_FROZEN case.

 "What's wrong with the current cpu hotplug infrastructure?

   - Asymmetry

     The hotplug notifier mechanism is asymmetric versus the bringup and
     teardown.  This is mostly caused by the notifier mechanism.

   - Largely undocumented dependencies

     While some notifiers use explicitely defined notifier priorities,
     we have quite some notifiers which use numerical priorities to
     express dependencies without any documentation why.

   - Control processor driven

     Most of the bringup/teardown of a cpu is driven by a control
     processor.  While it is understandable, that preperatory steps,
     like idle thread creation, memory allocation for and initialization
     of essential facilities needs to be done before a cpu can boot,
     there is no reason why everything else must run on a control
     processor.  Before this patch series, bringup looks like this:

       Control CPU                     Booting CPU

       do preparatory steps
       kick cpu into life

                                       do low level init

       sync with booting cpu           sync with control cpu

       bring the rest up

   - All or nothing approach

     There is no way to do partial bringups.  That's something which is
     really desired because we waste e.g.  at boot substantial amount of
     time just busy waiting that the cpu comes to life.  That's stupid
     as we could very well do preparatory steps and the initial IPI for
     other cpus and then go back and do the necessary low level
     synchronization with the freshly booted cpu.

   - Minimal debuggability

     Due to the notifier based design, it's impossible to switch between
     two stages of the bringup/teardown back and forth in order to test
     the correctness.  So in many hotplug notifiers the cancel
     mechanisms are either not existant or completely untested.

   - Notifier [un]registering is tedious

     To [un]register notifiers we need to protect against hotplug at
     every callsite.  There is no mechanism that bringup/teardown
     callbacks are issued on the online cpus, so every caller needs to
     do it itself.  That also includes error rollback.

  What's the new design?

     The base of the new design is a symmetric state machine, where both
     the control processor and the booting/dying cpu execute a well
     defined set of states.  Each state is symmetric in the end, except
     for some well defined exceptions, and the bringup/teardown can be
     stopped and reversed at almost all states.

     So the bringup of a cpu will look like this in the future:

       Control CPU                     Booting CPU

       do preparatory steps
       kick cpu into life

                                       do low level init

       sync with booting cpu           sync with control cpu

                                       bring itself up

     The synchronization step does not require the control cpu to wait.
     That mechanism can be done asynchronously via a worker or some
     other mechanism.

     The teardown can be made very similar, so that the dying cpu cleans
     up and brings itself down.  Cleanups which need to be done after
     the cpu is gone, can be scheduled asynchronously as well.

  There is a long way to this, as we need to refactor the notion when a
  cpu is available.  Today we set the cpu online right after it comes
  out of the low level bringup, which is not really correct.

  The proper mechanism is to set it to available, i.e. cpu local
  threads, like softirqd, hotplug thread etc. can be scheduled on that
  cpu, and once it finished all booting steps, it's set to online, so
  general workloads can be scheduled on it.  The reverse happens on
  teardown.  First thing to do is to forbid scheduling of general
  workloads, then teardown all the per cpu resources and finally shut it
  off completely.

  This patch series implements the basic infrastructure for this at the
  core level.  This includes the following:

   - Basic state machine implementation with well defined states, so
     ordering and prioritization can be expressed.

   - Interfaces to [un]register state callbacks

     This invokes the bringup/teardown callback on all online cpus with
     the proper protection in place and [un]installs the callbacks in
     the state machine array.

     For callbacks which have no particular ordering requirement we have
     a dynamic state space, so that drivers don't have to register an
     explicit hotplug state.

     If a callback fails, the code automatically does a rollback to the
     previous state.

   - Sysfs interface to drive the state machine to a particular step.

     This is only partially functional today.  Full functionality and
     therefor testability will be achieved once we converted all
     existing hotplug notifiers over to the new scheme.

   - Run all CPU_ONLINE/DOWN_PREPARE notifiers on the booting/dying
     processor:

     In a previous step of this work we've done a full tree mechanical
     conversion of all hotplug notifiers to the new scheme.  The balance
     is a net removal of about 4000 lines of code.

     This is not included in this series, as we decided to take a
     different approach.  Instead of mechanically converting everything
     over, we will do a proper overhaul of the usage sites one by one so
     they nicely fit into the symmetric callback scheme.

     I decided to do that after I looked at the ugliness of some of the
     converted sites and figured out that their hotplug mechanism is
     completely buggered anyway.  So there is no point to do a
     mechanical conversion first as we need to go through the usage
     sites one by one again in order to achieve a full symmetric and
     testable behaviour"

Alexander Duyck 01cfbad79a ipv4: Update parameters for csum_tcpudp_magic to their original types
This patch updates all instances of csum_tcpudp_magic and
csum_tcpudp_nofold to reflect the types that are usually used as the source
inputs.  For example the protocol field is populated based on nexthdr which
is actually an unsigned 8 bit value.  The length is usually populated based
on skb->len which is an unsigned integer.

This addresses an issue in which the IPv6 function csum_ipv6_magic was
generating a checksum using the full 32b of skb->len while
csum_tcpudp_magic was only using the lower 16 bits.  As a result we could
run into issues when attempting to adjust the checksum as there was no
protocol agnostic way to update it.

With this change the value is still truncated as many architectures use
"(len + proto) << 8", however this truncation only occurs for values
greater than 16776960 in length and as such is unlikely to occur as we stop
the inner headers at ~64K in size.

I did have to make a few minor changes in the arm, mn10300, nios2, and
score versions of the function in order to support these changes as they
were either using things such as an OR to combine the protocol and length,
or were using ntohs to convert the length which would have truncated the
value.

I also updated a few spots in terms of whitespace and type differences for
the addresses.  Most of this was just to make sure all of the definitions
were in sync going forward.

Martin Schwidefsky e370e47694 s390: fix floating pointer register corruption (again)
There is a tricky interaction between the machine check handler
and the critical sections of load_fpu_regs and save_fpu_regs
functions. If the machine check interrupts one of the two
functions the critical section cleanup will complete the function
before the machine check handler s390_do_machine_check is called.
Trouble is that the machine check handler needs to validate the
floating point registers *before* and not *after* the completion
of load_fpu_regs/save_fpu_regs.

The simplest solution is to rewind the PSW to the start of the
load_fpu_regs/save_fpu_regs and retry the function after the
return from the machine check handler.

Heiko Carstens 8f100bb1ff s390/cpumf: add missing lpp magic initialization
Add the missing lpp magic initialization for cpu 0. Without this all
samples on cpu 0 do not have the most significant bit set in the
program parameter field, which we use to distinguish between guest and
host samples if the pid is also 0.

We did initialize the lpp magic in the absolute zero lowcore but
forgot that when switching to the allocated lowcore on cpu 0 only.

Martin Schwidefsky 3446c13b26 s390/mm: four page table levels vs. fork
The fork of a process with four page table levels is broken since
git commit 6252d702c5 "[S390] dynamic page tables."

All new mm contexts are created with three page table levels and
an asce limit of 4TB. If the parent has four levels dup_mmap will
add vmas to the new context which are outside of the asce limit.
The subsequent call to copy_page_range will walk the three level
page table structure of the new process with non-zero pgd and pud
indexes. This leads to memory clobbers as the pgd_index *and* the
pud_index is added to the mm->pgd pointer without a pgd_deref
in between.

The init_new_context() function is selecting the number of page
table levels for a new context. The function is used by mm_init()
which in turn is called by dup_mm() and mm_alloc(). These two are
used by fork() and exec(). The init_new_context() function can
distinguish the two cases by looking at mm->context.asce_limit,
for fork() the mm struct has been copied and the number of page
table levels may not change. For exec() the mm_alloc() function
set the new mm structure to zero, in this case a three-level page
table is created as the temporary stack space is located at
STACK_TOP_MAX = 4TB.

This fixes CVE-2016-2143.

David Hildenbrand c54f0d6ae0 KVM: s390: allocate only one DMA page per VM
We can fit the 2k for the STFLE interpretation and the crypto
control block into one DMA page. As we now only have to allocate
one DMA page, we can clean up the code a bit.

As a nice side effect, this also fixes a problem with crycbd alignment in
case special allocation debug options are enabled, debugged by Sascha
Silbe.

David Hildenbrand b3c17f10fa KVM: s390: wake up when the VCPU cpu timer expires
When the VCPU cpu timer expires, we have to wake up just like when the ckc
triggers. For now, setting up a cpu timer in the guest and going into
enabled wait will never lead to a wakeup. This patch fixes this problem.
Just as for the ckc, we have to take care of waking up too early. We
have to recalculate the sleep time and go back to sleep.

Please note that the timer callback calls kvm_s390_get_cpu_timer() from
interrupt context. As the timer is canceled when leaving handle_wait(),
and we don't do any VCPU cpu timer writes/updates in that function, we can
be sure that we will never try to read the VCPU cpu timer from the same cpu
that is currentyl updating the timer (deadlock).

David Hildenbrand 9c23a1318e KVM: s390: protect VCPU cpu timer with a seqcount
For now, only the owning VCPU thread (that has loaded the VCPU) can get a
consistent cpu timer value when calculating the delta. However, other
threads might also be interested in a more recent, consistent value. Of
special interest will be the timer callback of a VCPU that executes without
having the VCPU loaded and could run in parallel with the VCPU thread.

The cpu timer has a nice property: it is only updated by the owning VCPU
thread. And speaking about accounting, a consistent value can only be
calculated by looking at cputm_start and the cpu timer itself in
one shot, otherwise the result might be wrong.

As we only have one writing thread at a time (owning VCPU thread), we can
use a seqcount instead of a seqlock and retry if the VCPU refreshed its
cpu timer. This avoids any heavy locking and only introduces a counter
update/check plus a handful of smp_wmb().

The owning VCPU thread should never have to retry on reads, and also for
other threads this might be a very rare scenario.

Please note that we have to use the raw_* variants for locking the seqcount
as lockdep will produce false warnings otherwise. The rq->lock held during
vcpu_load/put is also acquired from hardirq context. Lockdep cannot know
that we avoid potential deadlocks by disabling preemption and thereby
disable concurrent write locking attempts (via vcpu_put/load).

David Hildenbrand db0758b297 KVM: s390: step VCPU cpu timer during kvm_run ioctl
Architecturally we should only provide steal time if we are scheduled
away, and not if the host interprets a guest exit. We have to step
the guest CPU timer in these cases.

In the first shot, we will step the VCPU timer only during the kvm_run
ioctl. Therefore all time spent e.g. in interception handlers or on irq
delivery will be accounted for that VCPU.

We have to take care of a few special cases:
- Other VCPUs can test for pending irqs. We can only report a consistent
  value for the VCPU thread itself when adding the delta.
- We have to take care of STP sync, therefore we have to extend
  kvm_clock_sync() and disable preemption accordingly
- During any call to disable/enable/start/stop we could get premeempted
  and therefore get start/stop calls. Therefore we have to make sure we
  don't get into an inconsistent state.

Whenever a VCPU is scheduled out, sleeping, in user space or just about
to enter the SIE, the guest cpu timer isn't stepped.

Please note that all primitives are prepared to be called from both
environments (cpu timer accounting enabled or not), although not completely
used in this patch yet (e.g. kvm_s390_set_cpu_timer() will never be called
while cpu timer accounting is enabled).

David Hildenbrand 9522b37f5a KVM: s390: correct fprs on SIGP (STOP AND) STORE STATUS
With MACHINE_HAS_VX, we convert the floating point registers from the
vector registeres when storing the status. For other VCPUs, these are
stored to vcpu->run->s.regs.vrs, but we are using current->thread.fpu.vxrs,
which resolves to the currently loaded VCPU.

So kvm_s390_store_status_unloaded() currently writes the wrong floating
point registers (converted from the vector registers) when called from
another VCPU on a z13.

This is only the case for old user space not handling SIGP STORE STATUS and
SIGP STOP AND STORE STATUS, but relying on the kernel implementation. All
other calls come from the loaded VCPU via kvm_s390_store_status().
