Heiko Carstens 8e1023016c [S390] prevent unneccesary loops_per_jiffy recalculation
When the seqfile /proc/cpuinfo gets accesses for each possible cpu
loops_per_jiffy gets recalculated. However its value is only needed
on first access.
In addition loops_per_jiffy should be recalculated when the machine
reports a capability change.

Peter Oberparleiter c03017544e [S390] cio: fix ccwgroup unregistration race condition
A race condition exists in the ccwgroup device unregistration code
which can cause a kernel panic due to a use-after-free bug. This
race condition might be triggered when all ccw devices associated with
a ccwgroup device are removed at the same time (e.g. because the
corresponding channel path becomes no longer available).

Fix this race condition by clearing the references from the associated
ccw devices to the ccw group device during unregistration of the
ccw group device.

is decreased twice.

Stefan Haberland e4dbb0f2b5 [S390] dasd: Add support for raw ECKD access.
Normal I/O operations through the DASD device driver give only access
to the data fields of an ECKD device even for track based I/O.
This patch extends the DASD device driver to give access to whole
ECKD tracks including count, key and data fields.

Stefan Haberland 6f272b9cec [S390] dasd: Prevent deadlock during suspend/resume.
The freeze callback may set a stop bit so that a worker thread could
not start I/O. The discipline specific freeze function waits for the
worker to be completed.
Set the stop_bit after the discipline specific freeze function has
returned and no worker is running.

Stefan Weinhuber 5a27e60dec [S390] dasd: Improve handling of stolen DASD reservation
If a DASD device has been reserved by a Linux system, and later
this reservation is ‘stolen’ by a second system by means of an
unconditional reserve, then the first system receives a
notification about this fact. With this patch such an event can
be either ignored, as before, or it can be used to let the device
fail all I/O request, so that the device will not block anymore.

Stefan Weinhuber a4d26c6aec [S390] dasd: do path verification for paths added at runtime
When a new path is added at runtime, the CIO layer will call the drivers
path_event callback. The DASD device driver uses this callback to trigger
a path verification for the new path. The driver will use only those
paths for I/O, which have been successfully verified.

Stefan Weinhuber 9062014cb6 [S390] cio: reduce memory consumption of itcw structures
Any list of indirect data adresses (TIDAL) used by a TCW must not
cross a page boundary. The original itcw implementation complies with
this restriction by allocating allmost twice as much memory as
actually needed, so that in any case there is enough room for the full
TIDAL, either above or below the page boundary.
This patch implements an alternative method, by using a TTIC TIDAW to
connect TIDAL parts below and above a page boundary.

Jan Glauber 110da31709 [S390] qdio: remove enhanced SIGA
HiperSocket devices only use one SBAL per qdio call without the enhanced SIGA
feature. Since that feature is currently not used remove it from the qdio code
so the compiler can generate better code for the HiperSocket outbound path.
While at it mark the SIGA error conditions as unlikely.

If QIOASSIST is enabled for a qdio device the SIGA instruction requires
a modified function code. This function code modifier was missing for
SIGA-R and SIGA-S which can lead to a kernel panic caused by an
operand exception.

Jan Glauber 3d6c76ff32 [S390] qdio: outbound tasklet scan threshold
Introduce a scan treshold for the qdio outbound queues. By setting the
threshold the driver can tell qdio after how much used SBALs qdio
should schedule the outbound tasklet that scans the queue for finished
SBALs. The threshold is specific by the drivers because a
Hipersockets device is much faster in utilizing outbound buffers than a
ZFCP or OSA device.

The default values after how many used SBALs the tasklet should run are:

OSA:          > 31 SBALs
Hipersockets: > 7 SBALs
zfcp:         > 55 SBALs

If the shared indicator is used the following race leads to
an inbound stall:

shared DSCI => 1
ALSI => 1

                        shared DSCI => 0

                                                ALSI => 0
                        ALSI was set,
                        shared DSCI => 1

After that no more interrupts occur because the DSCI is still set.
Fix that race by only resetting the shared DSCI if it was actually
set so the tasklets for all shared devices are scheduled and will
run after the interrupt.

Felix Beck c2567f8ffa [S390] zcrypt: cope with cca restriction of cex3
The cca on the crypto adapter has a restriction in the size of the
exponent if a key with a modulus bigger than 2048 bit is used. Thus
in that case we have to avoid that the crypto device driver thinks
the adapter is defect and sets it offline. Therfore a new member for
the zcrypt_device struct called max_exp_bit_length is introduced. This
will be set the first time the cca returns the error code function
not implemented. If this is done with an adapter twice it will return
-EINVAL.

Implemented an asm in the ap bus and made it accessible for the card
specific parts of the zcrypt driver. Thus when a cex3a is recognized
a check can be performed to dermine whether the card supports 4096 bit
RSA keys.

Up to now /proc/interrupts only has statistics for external and i/o
interrupts but doesn't split up them any further.
This patch adds a line for every single interrupt source so that it
is possible to easier tell what the machine is/was doing.
Part of the output now looks like this;

Christof Schmitt 0d81b4e8dc [SCSI] zfcp: Add allow_lun_scan module parameter
The zfcpdump tool requires a method to attach exactly one LUN. The
easiest way to achieve this is to add a new zfcp module parameter.
When allow_lun_scan is set to "false", zfcp only accepts LUNs that
have been configured through the unit_add sysfs interface.

Initialization of the qdio waitqueue should happen when the qdio data
is initialized and the QDIOUP flag should be handled in the qdio code
as well. Adjust the code accordingly and remove the superfluos

The tracing environment of the zfcp LLD has become very bulky and hard
to maintain. Small changes involve a large modification process which
is error-prone and not effective.  This patch is the first of a set to
redesign the zfcp tracing to a more straight-forward and easy to
extend scheme.  It removes all interpretation and visualization parts
and focuses on bare logging of the information.

This patch deals with all trace records of the zfcp error recovery.

Christoph Lameter 4a6f4fe837 drivers: Replace __get_cpu_var with __this_cpu_read if not used for an address.
__get_cpu_var() can be replaced with this_cpu_read and will then use a single
read instruction with implied address calculation to access the correct per cpu
instance.

However, the address of a per cpu variable passed to __this_cpu_read() cannot be
determed (since its an implied address conversion through segment prefixes).
Therefore apply this only to uses of __get_cpu_var where the addres of the
variable is not used.

V3->V4:
	- Move one instance of this_cpu_inc_return to a later patch
	  so that this one can go in without percpu infrastructrure
	  changes.

Swen Schillig d3e1088d68 [SCSI] zfcp: No ERP escalation on gpn_ft eval
If the evaluation of GPN_FT requests wants to remove an invalid port
from the system the zfcp_erp_port_shutdown function is triggered.
Depending on the system status a superior action (e.g. adapter reopen)
is required. This can lead to an invalid mem access of the port struct
which might be freed at the time since the superior action is not
holding a reference of the port which triggered this ERP action.

Ursula Braun 2b6203bb7d qeth: enable interface setup if LAN is offline
Device initialization of a qeth device contains a STARTLAN step.
This step may fail, if cable is not yet plugged in. The qeth device
stays in state HARDSETUP until cable is plugged in. This prevents
further preparational initialization steps of the qeth device and
its network interface. This patch makes sure initialization of qeth
device continues, even though cable is not yet plugged in.
Once carrier is available, qeth is notified, triggers a recovery
which results in a working network interface.

Jeff Garzik f281233d3e SCSI host lock push-down
Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.

The patch below presents a simple SCSI host lock push-down as an
equivalent transformation.  No locking or other behavior should change
with this patch.  All existing bugs and locking orders are preserved.

Additionally, add one parameter to queuecommand,
	struct Scsi_Host *
and remove one parameter from queuecommand,
	void (*done)(struct scsi_cmnd *)

Scsi_Host* is a convenient pointer that most host drivers need anyway,
and 'done' is redundant to struct scsi_cmnd->scsi_done.

Minimal code disturbance was attempted with this change.  Most drivers
needed only two one-line modifications for their host lock push-down.

Tejun Heo e525fd89d3 block: make blkdev_get/put() handle exclusive access
Over time, block layer has accumulated a set of APIs dealing with bdev
open, close, claim and release.

* blkdev_get/put() are the primary open and close functions.

* bd_claim/release() deal with exclusive open.

* open/close_bdev_exclusive() are combination of open and claim and
  the other way around, respectively.

* bd_link/unlink_disk_holder() to create and remove holder/slave
  symlinks.

* open_by_devnum() wraps bdget() + blkdev_get().

The interface is a bit confusing and the decoupling of open and claim
makes it impossible to properly guarantee exclusive access as
in-kernel open + claim sequence can disturb the existing exclusive
open even before the block layer knows the current open if for another
exclusive access.  Reorganize the interface such that,

* blkdev_get() is extended to include exclusive access management.
  @holder argument is added and, if is @FMODE_EXCL specified, it will
  gain exclusive access atomically w.r.t. other exclusive accesses.

* blkdev_put() is similarly extended.  It now takes @mode argument and
  if @FMODE_EXCL is set, it releases an exclusive access.  Also, when
  the last exclusive claim is released, the holder/slave symlinks are
  removed automatically.

* bd_claim/release() and close_bdev_exclusive() are no longer
  necessary and either made static or removed.

* bd_link_disk_holder() remains the same but bd_unlink_disk_holder()
  is no longer necessary and removed.

* open_bdev_exclusive() becomes a simple wrapper around lookup_bdev()
  and blkdev_get().  It also has an unexpected extra bdev_read_only()
  test which probably should be moved into blkdev_get().

* open_by_devnum() is modified to take @holder argument and pass it to
  blkdev_get().

Most of bdev open/close operations are unified into blkdev_get/put()
and most exclusive accesses are tested atomically at the open time (as
it should).  This cleans up code and removes some, both valid and
invalid, but unnecessary all the same, corner cases.

open_bdev_exclusive() and open_by_devnum() can use further cleanup -
rename to blkdev_get_by_path() and blkdev_get_by_devt() and drop
special features.  Well, let's leave them for another day.

Most conversions are straight-forward.  drbd conversion is a bit more
involved as there was some reordering, but the logic should stay the
same.

Stefan Weinhuber ca768b6631 [S390] vmlogrdr: purge after recording is switched off
If automatic purge is enabled for a vmlogrdr device, old records are
purged before an IUCV recording service is switched on or off. If z/VM
generates a large number of records between purging and switching the
recording service off, these records remain queued, and may have a
negative performance impact on the z/VM system. To avoid this problem,
we need to purge the records after recording is switched off.

Ursula Braun b67d801f92 qeth: remove dev_queue_xmit invocation
For a certain Hipersockets specific error code in the xmit path, the
qeth driver tries to invoke dev_queue_xmit again.
Commit 79640a4ca6 introduces a busylock
causing locking problems in case of re-invoked dev_queue_xmit by qeth.
This patch removes the attempts to retry packet sending with
dev_queue_xmit from the qeth driver.

Swen Schillig 0df138476c [SCSI] zfcp: Fix adapter activation on link down
If an exchange config is executed while the local link is down, the
request succeeds but the returned data is incomplete.  Proceeding with
the adapter activation is leading to an unpredictable behaviour (e.g.
kernel panic) caused by invalid values.  In such a scenario the
recommended ERP is to retry the action and wait for a link up event.
If the issue persists the activation has to fail.

Stefan Haberland 56b86b615b [S390] dasd: ignore unsolicited interrupts for DIAG
For the DASD DIAG discipline IO is started through special diagnose
calls. Unsolicited interrupts may contain information about the device
itself. But this information is not needed because the device is not
used directly.
Fix the case that an unimplemented dicipline function may be called
by ignoring unsolicited interrupts for the DIAG disciplin.

Stefan Weinhuber a5a0061fb3 [S390] dasd: fix unsolicited interrupt recognition
The dasd interrupt handler needs to distinguish solicited from
unsolicited interrupts, as unsolicited interrupts may require special
handling (e.g. summary unit checks) and solicited interrupts require
proper error recovery for the failed I/O request.
The interrupt handler needs to check several bit fields in the
interrupt response block (irb) to make this distinction.
So far our check of the status control bits has not been specific
enough, which may lead to a failed request getting just retried
instead of the necessary error recovery.

Sebastian Ott 26cffecf84 [S390] dasd: fix use after free in dbf
Writing to /proc/dasd/statistics while the debug level of the
generic dasd debug entry is set to DBF_DEBUG will lead to an
use after free when accessing the debug entry later.
Since for the format string "%s" in the s390 dbf only a pointer
to the string is stored in the debug feature and the buffer used
here is freed afterwards.

To fix this just remove the debug message.

Peter Oberparleiter 376ae4752e [S390] cio: fix I/O cancel function
Function ccw_device_cancel_halt_clear may cause an unexpected kernel
panic if a clear function is currently active at the subchannel for
which it is called. In that case, the iretry counter used to determine
the number of retries is never initialized, leading to an immediate
failure of the function which results in a kernel panic.

Fix this by initializing the iretry counter when the function is
first called. Also replace the kernel panic with a return code: a
single malfunctioning I/O device should not automatically cause a
system-wide kernel panic.

Sebastian Ott 906c9768c7 [S390] chsc: use the global page to determine the chp desriptor
chsc_determine_channel_path_desc is called by a wrapper
who allocates a response struct. The response data
is then memcpy'ed to this response struct by
chsc_determine_channel_path_desc.

Change chsc_determine_base_channel_path_desc to use the
global chsc_page and deliver it to the function doing
the actual chsc call. The channel path desriptor is
then directly read from the response data.

As a result we get rid of the additional allocation
for the response struct.

Sebastian Ott 34196f82b1 [S390] chsc: consolidate memory allocations
Most wrappers around the channel subsystem call have their own logic
to allocate memory (with proper alignment) or use preallocated or
static memory. This patch converts most users of the channel
subsystem call to use the same preallocated page (proteced by a
spinlock).

Note: The sei_page which is used in our crw handler to call
"store event information" has to coexist, since
a) in crw context, while accessing the sei_page, sleeping is allowed
   (which will conflict with the spinlock protection of the chsc_page)
b) in crw context, while accessing the sei_page, channel subsystem
   calls are allowed (which itself would require the page).

Sebastian Ott 74b6127e6c [S390] cio: fix memleak in resume path
If a ccwdevice is lost during hibernation and a different
ccwdevice is attached to the same subchannel, we will
deregister the old ccw device and register the new one.

Since deregistration is not allowed in this context, we
handle this action later. However, some parts of the
registration process for the new device were started anyway,
so that the old device structure is no longer accessible.

Fix this by deferring both actions to the afterwards
scheduled subchannel event.

Stefan Weinhuber 7ea8d32199 [S390] dasd: let recovery cqr inherit flags from failed cqr
The usual way to recover a failed DASD ECKD request (cqr) is to create
a new request with an appropriate recovery CCW program.  Certain
features, e.g. failfast, can be enabled per request and are stored in
the requests flags. These flags have to be copied from the failed to
the recovery request, to let the recovery request use the same
features as the original one.

Alexander Graf cefa33e2f8 KVM: S390: Add virtio hotplug add support
The one big missing feature in s390-virtio was hotplugging. This is no more.
This patch implements hotplug add support, so you can on the fly add new devices
in the guest.

Keep in mind that this needs a patch for qemu to actually leverage the
functionality.

Jesse Gross b738127dfb vlan: Rename VLAN_GROUP_ARRAY_LEN to VLAN_N_VID.
VLAN_GROUP_ARRAY_LEN is simply the number of possible vlan VIDs.
Since vlan groups will soon be more of an implementation detail
for vlan devices, rename the constant to be descriptive of its
actual purpose.

Arnd Bergmann 6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

Christof Schmitt bf5eefb007 [SCSI] zfcp: Remove scsi_cmnd->serial_number from debug traces
With the change that drivers have to explicitly request the serial
number for SCSI commands, this field should not be part of the zfcp
traces. It is not worth the effort to request the serial number only
for tracing purposes, so simply remove this field from the debug
traces.

Fix the IRQ flag handling naming.  In linux/irqflags.h under one configuration,
it maps:

	local_irq_enable() -> raw_local_irq_enable()
	local_irq_disable() -> raw_local_irq_disable()
	local_irq_save() -> raw_local_irq_save()
	...

and under the other configuration, it maps:

	raw_local_irq_enable() -> local_irq_enable()
	raw_local_irq_disable() -> local_irq_disable()
	raw_local_irq_save() -> local_irq_save()
	...

This is quite confusing.  There should be one set of names expected of the
arch, and this should be wrapped to give another set of names that are expected
by users of this facility.

Change this to have the arch provide:

	flags = arch_local_save_flags()
	flags = arch_local_irq_save()
	arch_local_irq_restore(flags)
	arch_local_irq_disable()
	arch_local_irq_enable()
	arch_irqs_disabled_flags(flags)
	arch_irqs_disabled()

Then linux/irqflags.h wraps these to provide:

	raw_local_irq_save(flags)

Christof Schmitt f8210e3488 [SCSI] zfcp: Allow midlayer to scan for LUNs when running in NPIV mode
Enable the LUN scanning mechanism in the SCSI midlayer:
- Do not set the disable_target_scan bit in the FC transport class.
- Set max_lun to 0xFFFFFFFF to allow the midlayer scan to include
  the two-level hierachical LUNs (like 0x40XX40XX00000000, but in SCSI
  midlayer LUN format).
- Set max_id to a high value to allow triggering the SCSI device
  rescan from sysfs.

When running in NPIV mode, zfcp accepts all LUNs in slave_attach. When
running in non-NPIV mode, the list of zfcp_unit structs determines
which SCSI devices are allowed on the current system.

Christof Schmitt b62a8d9b45 [SCSI] zfcp: Use SCSI device data zfcp_scsi_dev instead of zfcp_unit
This is the large change to switch from using the data in
zfcp_unit to zfcp_scsi_dev. Keeping everything working requires doing
the switch in one piece. To ensure that no code keeps using the data
in zfcp_unit, this patch also removes the data from zfcp_unit that is
now being replaced with zfcp_scsi_dev.

For zfcp, the scsi_device together with zfcp_scsi_dev exist from the
call of slave_alloc to the call of slave_destroy. The data in
zfcp_scsi_dev is initialized in zfcp_scsi_slave_alloc and the LUN is
opened; the final shutdown for the LUN is run from slave_destroy.

Where the scsi_device or zfcp_scsi_dev is needed, the pointer to the
scsi_device is passed as function argument and inside the function
converted to the pointer to zfcp_scsi_dev; this avoids back and forth
conversion betweeen scsi_device and zfcp_scsi_dev.

While changing the function arguments from zfcp_unit to scsi_device,
the functions names are renamed form "unit" to "lun". This is to have
a seperation between zfcp_scsi_dev/LUN and the zfcp_unit; only code
referring to the remaining configuration information in zfcp_unit
struct uses "unit".

Christof Schmitt fdbd1c5e27 [SCSI] zfcp: Allow running unit/LUN shutdown without acquiring reference
With the change for the LUN data to be part of the scsi_device, the
slave_destroy callback will be the final call to the
zfcp_erp_unit_shutdown function. The erp tries to acquire a reference
to run the action asynchronously and fail, if it cannot get the
reference. But calling scsi_device_get from slave_destroy will fail,
because the scsi_device is already in the state SDEV_DEL.

Introduce a new call into the zfcp erp to solve this: The function
zfcp_erp_unit_shutdown_wait will close the LUN and wait for the erp to
finish without acquiring an additional reference. The wait allows to
omit the reference; the caller waiting for the erp to finish already
has a reference that holds the struct in place.

Christof Schmitt 175b79f063 zfcp: Report scatter gather limit for DIX protection information
When sending DIX integrity segments with an I/O request, the
restriction for the maximum number of segments is still the same for
the zfcp hardware. Report the new sg_prot_tablesize for the SCSI host,
so that the number of integrity segments plus the number of data
segments is not larger than the hardware limit. This results in using
half of the hardware segments for integrity data and the other half
for regular data.

Tejun Heo 4913efe456 block: deprecate barrier and replace blk_queue_ordered() with blk_queue_flush()
Barrier is deemed too heavy and will soon be replaced by FLUSH/FUA
requests.  Deprecate barrier.  All REQ_HARDBARRIERs are failed with
-EOPNOTSUPP and blk_queue_ordered() is replaced with simpler
blk_queue_flush().

blk_queue_flush() takes combinations of REQ_FLUSH and FUA.  If a
device has write cache and can flush it, it should set REQ_FLUSH.  If
the device can handle FUA writes, it should also set REQ_FUA.

All blk_queue_ordered() users are converted.

* ORDERED_DRAIN is mapped to 0 which is the default value.
* ORDERED_DRAIN_FLUSH is mapped to REQ_FLUSH.
* ORDERED_DRAIN_FLUSH_FUA is mapped to REQ_FLUSH | REQ_FUA.

Frank Blaschka a1c3ed4c9c qeth: NAPI support for l2 and l3 discipline
This patch adds NAPI support to the qeth layer 2 and layer 3
discipline. It is important to understand that we can not enable/disable
IRQs as usual, we have to use the corresponding new QDIO interface.
Also to not overdraw the budget we have to stop and restart buffer
processing at any point during processing a bulk of QDIO buffers.
Having the driver NAPI enabled it is possible to turn on GRO for the
layer 3 discipline.

Jan Glauber d36deae750 qdio: extend API to allow polling
Extend the qdio API to allow polling in the upper-layer driver. This
is needed by qeth to use NAPI.

To use the new interface the upper-layer driver must specify the
queue_start_poll(). This callback is used to signal the upper-layer
driver that is has initiative and must process the inbound queue by
calling qdio_get_next_buffers(). If the upper-layer driver wants to
stop polling it calls qdio_start_irq().

Since adapter interrupts are not completely stoppable qdio implements
a software bit QDIO_QUEUE_IRQS_DISABLED to safely disable interrupts for an
input queue.

The old interface is preserved and will be used as is by zfcp.
