Christoph Hellwig 17d5363b83 scsi: introduce a result field in struct scsi_request
This passes on the scsi_cmnd result field to users of passthrough
requests. Currently we abuse req->errors for this purpose, but that
field will go away in its current form.

Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it. I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

Christoph Hellwig aebf526b53 block: fold cmd_type into the REQ_OP_ space
Instead of keeping two levels of indirection for requests types, fold it
all into the operations. The little caveat here is that previously
cmd_type only applied to struct request, while the request and bio op
fields were set to plain REQ_OP_READ/WRITE even for passthrough
operations.

Instead this patch adds new REQ_OP_* for SCSI passthrough and driver
private requests, althought it has to add two for each so that we
can communicate the data in/out nature of the request.

Linus Torvalds 1289ace5b4 SCSI misc on 20160113
This pull includes driver updates from the usual suspects (bfa, arcmsr,
scsi_dh_alua, lpfc, storvsc, cxlflash). The major change is the addition of
the hisi_sas driver, which is an ARM platform device for SAS. The other
change of note is an enormous style transformation to the atp870u driver
(which is our worst written SCSI driver).

Mike Miller e7b18ede44 cciss: set max scatter gather entries to 32 on P600
At one time we used to set the maximum number of scatter gather elements
on all Smart Array controllers to 32. At some point in time the
firmware began to write the "appropriate" value for each controller into
the config table. The cciss driver would then read that and set
h->maxsgentries.

        h->maxsgentries = readl(&(h->cfgtable->MaxSGElements);

On the P600 that value is 544. Under some workloads a significant
performance reduction may result. This patch forces the P600 to use
only 32 scatter gather elements. Other controllers are not affected.

Mike Miller 0821e90405 cciss: bug fix to prevent cciss from loading in kdump crash kernel
By default the cciss driver supports all "older" HP Smart Array
controllers and hpsa supports all controllers starting with the G6 family.
There are module parameters that allow a user to override those defaults
and use hpsa for any HP Smart Array controller.

If the user does override the default behavior and uses hpsa for older
controllers it is possible that cciss may try to load in a kdump crash
kernel. This may happen if cciss is loaded first from the kdump initrd
image. If cciss does load rather than hpsa and reset_devices is true we
immediately call cciss_hard_reset_controller. This will result in a
kernel panic and the core file cannot be created. This patch prevents
cciss from trying to load in this scenario.

Linus Torvalds 9228ff9038 Merge branch 'for-3.8/drivers' of git://git.kernel.dk/linux-block
Pull block driver update from Jens Axboe:
"Now that the core bits are in, here are the driver bits for 3.8. The
  branch contains:

   - A huge pile of drbd bits that were dumped from the 3.7 merge
window. Following that, it was both made perfectly clear that
there is going to be no more over-the-wall pulls and how the
situation on individual pulls can be improved.

   - A few cleanups from Akinobu Mita for drbd and cciss.

   - Queue improvement for loop from Lukas. This grew into adding a
generic interface for waiting/checking an even with a specific
lock, allowing this to be pulled out of md and now loop and drbd is
     also using it.

   - A few fixes for xen back/front block driver from Roger Pau Monne.

   - Partition improvements from Stephen Warren, allowing partiion UUID
to be used as an identifier."

* 'for-3.8/drivers' of git://git.kernel.dk/linux-block: (609 commits)
drbd: update Kconfig to match current dependencies
drbd: Fix drbdsetup wait-connect, wait-sync etc... commands

  drbd: Remove obsolete check
drbd: fixup after wait_even_lock_irq() addition to generic code
loop: Limit the number of requests in the bio list
  wait: add wait_event_lock_irq() interface
  xen-blkfront: free allocated page

Paolo Bonzini 577ebb374c block: add and use scsi_blk_cmd_ioctl
Introduce a wrapper around scsi_cmd_ioctl that takes a block device.

The function will then be enhanced to detect partition block devices
and, in that case, subject the ioctls to whitelisting.

Stephen M. Cameron 0007a4c90a cciss: auto engage SCSI mid layer at driver load time
A long time ago, probably in 2002, one of the distros, or maybe more than
one, loaded block drivers prior to loading the SCSI mid layer. This meant
that the cciss driver, being a block driver, could not engage the SCSI mid
layer at init time without panicking, and relied on being poked by a
userland program after the system was up (and the SCSI mid layer was
therefore present) to engage the SCSI mid layer.

This is no longer the case, and cciss can safely rely on the SCSI mid
layer being present at init time and engage the SCSI mid layer straight
away. This means that users will see their tape drives and medium
changers at driver load time without need for a script in /etc/rc.d that
does this:

for x in /proc/driver/cciss/cciss*
do
	echo "engage scsi" > $x
done

However, if no tape drives or medium changers are detected, the SCSI mid
layer will not be engaged. If a tape drive or medium change is later
hot-added to the system it will then be necessary to use the above script
or similar for the device(s) to be acceesible.

Stephen M. Cameron 8a4ec67bd5 cciss: add cciss_tape_cmds module paramter
This is to allow number of commands reserved for use by SCSI tape drives
and medium changers to be adjusted at driver load time via the kernel
parameter cciss_tape_cmds, with a default value of 6, and a range
of 2 - 16 inclusive. Previously, the driver limited the number of
commands which could be queued to the SCSI half of the the driver
to only 2. This is to fix the problem that if you had more than
two tape drives, you couldn't, for example, erase or rewind them all
at the same time.

Stephen M. Cameron 5afe278114 cciss: do soft reset if hard reset is broken
on driver load, if reset_devices is set, and the hard reset
attempts fail, try to bring up the controller to the point that
a command can be sent, and send it a soft reset command, then
after the reset undo whatever driver initialization was done to get
it to the point to take a command, and re-do it after the reset.

This is to get kdump to work on all the "non-resettable" controllers
(except 64xx controllers which can't be reset due to the potentially
shared cache module.)
