The rxe driver works as follows:
* The send queue, receive queue and completion queues are implemented as
circular buffers.
* ib_post_send() and ib_post_recv() calls are serialized through a spinlock.
* Removing elements from various queues happens from tasklet
context. Tasklets are guaranteed to run on at most one CPU. This serializes
access to these queues. See also rxe_completer(), rxe_requester() and
rxe_responder().
* rxe_completer() processes the skbs queued onto qp->resp_pkts.
* rxe_requester() handles the send queue (qp->sq.queue).
* rxe_responder() processes the skbs queued onto qp->req_pkts.
Since rxe_drain_req_pkts() processes qp->req_pkts, calling
rxe_drain_req_pkts() from rxe_requester() is racy. Hence this patch.

The following sequence:
* Change queue pair state into IB_QPS_ERR.
* Post a work request on the queue pair.
Triggers the following race condition in the rdma_rxe driver:
* rxe_qp_error() triggers an asynchronous call of rxe_completer(), the function
that examines the QP send queue.
* rxe_post_send() posts a work request on the QP send queue.
If rxe_completer() runs prior to rxe_post_send(), it will drain the send
queue and the driver will assume no further action is necessary.
However, once we post the send to the send queue, because the queue is
in error, no send completion will ever happen and the send will get
stuck. In order to process the send, we need to make sure that
rxe_completer() gets run after a send is posted to a queue pair in an
error state. This patch ensures that happens.

In xmit, there is a skb_clone. This function copies the struct sk_buff.
And some parameters are changed to the new skb. Then the new skb is sent
while the old skb is freed.
While the function skb_clone is removed, the parameter changes are made on
the old skb, then the old skb is sent. It can also work well.
The following tests are made.
server client
--------- ---------
|1.1.1.1|<----rxe-channel--->|1.1.1.2|

On client: rping -c -a 1.1.1.1 -v -C 1000 -S 512
The kernel config CONFIG_DEBUG_KMEMLEAK is enabled on both server
and client.
This test runs for several hours. There is no memory leak and the whole
system can work well.
As the above network, the following tests are made.
Server: ibv_rc_pingpong -d rxe0 -g 1
Client: ibv_rc_pingpong -d rxe0 -g 1 1.1.1.1

This patch adds static trace for RNRNAK timer. Currently the output from
hrtimer static trace only shows the addresses of hrtimers in the system
and there is no easy way to correlate an RNRNAK timer with its entries in
the hrtimer trace. This patch adds the correlation among a QP, its RNRNAK
timer, and its entries in the hrtimer trace. This correlation will be
enormously helpful when debugging RNRNAK related issues. In addition, this
patch cleans up rvt_stop_rnr_timer() to be void while here.

The smp_read_barrier_depends() does nothing at all except on DEC Alpha,
and no current DEC Alpha systems use Infiniband:
lkml.kernel.org/r/20171023085921.jwbntptn6ictbnvj@tower
This commit therefore makes Infiniband depend on !ALPHA and removes

the InfiniBand driver.
Please note that this patch should not be construed as my saying that

Pull rdma updates from Doug Ledford:
"This is a fairly plain pull request. Lots of driver updates across the
stack, a huge number of static analysis cleanups including a close to
50 patch series from Bart Van Assche, and a number of new features
inside the stack such as general CQ moderation support.
Nothing really stands out, but there might be a few conflicts as you
take things in. In particular, the cleanups touched some of the same
lines as the new timer_setup changes.
Everything in this pull request has been through 0day and at least two
days of linux-next (since Stephen doesn't necessarily flag new
errors/warnings until day2). A few more items (about 30 patches) from
Intel and Mellanox showed up on the list on Tuesday. I've excluded
those from this pull request, and I'm sure some of them qualify as
fixes suitable to send any time, but I still have to review them
fully. If they contain mostly fixes and little or no new development,
then I will probably send them through by the end of the week just to
get them out of the way.
There was a break in my acceptance of patches which coincides with the
computer problems I had, and then when I got things mostly back under
control I had a backlog of patches to process, which I did mostly last
Friday and Monday. So there is a larger number of patches processed in
that timeframe than I was striving for.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 11139
and resulted in the first patch in this series.
If that file was a */uapi/* path one, it was "GPL-2.0 WITH
Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:
SPDX license identifier # files
---------------------------------------------------|-------
GPL-2.0 WITH Linux-syscall-note 930
and resulted in the second patch in this series.
- if a file had some form of licensing information in it, and was one
of the */uapi/* ones, it was denoted with the Linux-syscall-note if
any GPL family license was found in the file or had no licensing in
it (per prior point). Results summary:
SPDX license identifier # files
---------------------------------------------------|------
GPL-2.0 WITH Linux-syscall-note 270
GPL-2.0+ WITH Linux-syscall-note 169
((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21
((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17
LGPL-2.1+ WITH Linux-syscall-note 15
GPL-1.0+ WITH Linux-syscall-note 14
((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5
LGPL-2.0+ WITH Linux-syscall-note 4
LGPL-2.1 WITH Linux-syscall-note 3
((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3
((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1
and that resulted in the third patch in this series.
- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Please do not apply this to mainline directly, instead please re-run the
coccinelle script shown below and apply its output.
For several reasons, it is desirable to use {READ,WRITE}_ONCE() in
preference to ACCESS_ONCE(), and new code is expected to use one of the
former. So far, there's been no reason to change most existing uses of
ACCESS_ONCE(), as these aren't harmful, and changing them results in
churn.
However, for some features, the read/write distinction is critical to
correct operation. To distinguish these cases, separate read/write
accessors must be used. This patch migrates (most) remaining

Per the IBTA spec, QP destroy shall fail if the QP is attached
to multicast groups, although the spec is silent on modify_qp
to reset state. It implies that ULP must deregister QP from
all mcast groups for destroy to succeed.
The faulty patch "IB/ipoib: Update broadcast object if PKey value
was changed in index 0" exposed two issues in rdmavt:
1. Rvt QP reset waits for qp references to go to zero.
This will hang if QP is attached to multicast groups.
2. The mcast group detach will fail for a QP in reset state
therefore preventing ULP from correcting the issue.
This patch moves the reference count wait to the the destroy QP
path and allows a QP mcast detach to work in the reset state.

The current process is to first calculate the CRC and then copy the client
data into the packet. This leaves a window in which the packet contents and
CRC can get out of sync, if the client changes the data after the CRC is
calculated but before the data is copied.
By copying the data into the packet and then calculating the CRC directly
from the packet contents we eliminate the window.
This can be seen with qperf's ud_bi_bw test. This seems like very
strange/reckless client behavior, but whether the client has mangled its

This prevents the stack from accessing userspace objects while they
are being torn down.
One possible sequence of events:
- Userspace program exits
- ib_uverbs_cleanup_ucontext() runs, calling ib_destroy_qp(),
ib_destroy_cq(), etc. and releasing/freeing the UCQ
- The QP still has tasklets running, so it isn't destroyed yet
- The CQ is referenced by the QP, so the CQ isn't destroyed yet
- The UCQ is kfree()'d anyway
- A send work request completes
- rxe_send_complete() calls cq->ibcq.comp_handler()
- ib_uverbs_comp_handler() runs and crashes; the event queue is checked
for is_closed, but it has no way to check the ib_ucq_object before
accessing it
The reference counting on the CQ doesn't protect against this since the CQ
hasn't been destroyed yet.
There's no available interface to deregister the UCQ from the CQ, and it
didn't appear that attempting to add reference counting to the UCQ was
going to be a good way to go since this solution is much simpler.

Daniel Micay noted that struct rxe_pkt_info is 32 bytes on 32-bit
architectures, but skb->cb is still 64. The memcpy() over-reads 32
bytes. This fixes it by zeroing the unused bytes in skb->cb.

SGEs that are contiguous needlessly consume driver dependent TX resources.
The lkey validation logic is enhanced to compress the SGE that ends
up in the send wqe when consecutive addresses are detected.
The lkey validation API used to return 1 (success) or 0 (fail).
The return value is now an -errno, 0 (compressed), or 1 (uncompressed). A
additional argument is added to pass the last SQE for the compression.
Loopback callers always pass a NULL to last_sge since the optimization is
of little benefit in that situation.

Pull rdma fixes from Doug Ledford:
"I had thought at the time of the last pull request that there wouldn't
be much more to go, but several things just kept trickling in over the
last week.
Instead of just the six patches to bnxt_re that I had anticipated,
there are another five IPoIB patches, two qedr patches, and a few
other miscellaneous patches.
The bnxt_re patches are more lines of diff than I like to submit this
late in the game. That's mostly because of the first two patches in
the series of six. I almost dropped them just because of the lines of
churn, but on a close review, a lot of the churn came from removing
duplicated code sections and consolidating them into callable
routines. I felt like this made the number of lines of change more
acceptable, and they address problems, so I left them. The remainder
of the patches are all small, well contained, and well understood.
These have passed 0day testing, but have not been submitted to
linux-next (but a local merge test with your current master was
without any conflicts).

On sparc, if we have an alloca() like situation, as is the case with
SHASH_DESC_ON_STACK(), we can end up referencing deallocated stack
memory. The result can be that the value is clobbered if a trap
or interrupt arrives at just the right instruction.
It only occurs if the function ends returning a value from that
alloca() area and that value can be placed into the return value
register using a single instruction.
For example, in lib/libcrc32c.c:crc32c() we end up with a return
sequence like:
return %i7+8
lduw [%o5+16], %o0 ! MEM[(u32 *)__shash_desc.1_10 + 16B],
%o5 holds the base of the on-stack area allocated for the shash
descriptor. But the return released the stack frame and the
register window.
So if an intererupt arrives between 'return' and 'lduw', then
the value read at %o5+16 can be corrupted.
Add a data compiler barrier to work around this problem. This is
exactly what the gcc fix will end up doing as well, and it absolutely
should not change the code generated for other cpus (unless gcc
on them has the same bug :-)
With crucial insight from Eric Sandeen.

Callers of rxe_mem_copy() provide pointer to store updated CRC
value. That pointer was supposed to be updated, but the
commit cee2688e3c ("IB/rxe: Offload CRC calculation when possible")
mistakenly removed that assignment for RXE_MEM_TYPE_DMA memory type.
The code worked because there are no actual callers with
RXE_MEM_TYPE_DMA, who are interested in returned value of crcp.
The one caller in read_reply(), who uses the returned crcp didn't
set RXE_MEM_TYPE_DMA as mem->type.

When reading a RDMA WRITE FIRST packet we copy the DMA length from the RDMA
header into the qp->resp.resid variable for later use. Later in check_rkey()
we clamp it to the MTU if the packet is an RDMA WRITE packet and has a
residual length bigger than the MTU. Later in write_data_in() we subtract the
payload of the packet from the residual length. If the packet happens to have a
payload of exactly the MTU size we end up with a residual length of 0 despite
the packet not being the last in the conversation. When the next packet in the
conversation arrives, we don't have any residual length left and thus set the QP
into an error state.

The synchronize_rcu() call can be eliminated to improve memory deregistration
performance.
There are two key fields involved:
- The rcu pointer itself
- the lkey_published field
To close the window between the rcu read of the mregion pointer and the
reference count the code should:
1. To lkey/rkey validation (reader)
Read the rcu pointer. If the pointer is non-NULL, get a reference.
To the current validation tests use a READ_ONCE() on the lkey_published.
Upon any failure release the reference.
2. To the remove logic (delete)
Insure the published is zeroed prior to setting the pointer to NULL.
This requires using rcu_assign_pointer() to insure lkey_published
is written prior to the NULL.
3. To the insert logic (add)
Insure the published is set use an rcu_assign_pointer() to insure the
pointer is after all MR fields.

wqe operations to completion opcodes to rdmvat.
This precludes having driver dependent operations. Make the translation
driver dependent by doing the translation in the driver prior to the
rvt_qp_swqe_complete() call using restored translation tables.

According to C9-147, MSN should only be incremented when the last packet of
a multi packet request has been received.
"Logically, the requester associates a sequential Send Sequence Number
(SSN) with each WQE posted to the send queue. The SSN bears a one-
to-one relationship to the MSN returned by the responder in each re-
sponse packet. Therefore, when the requester receives a response, it in-
terprets the MSN as representing the SSN of the most recent request
completed by the responder to determine which send WQE(s) can be
completed."

When vmalloc_user is used to create memory that is supposed to be mmap'd
to user space, it is necessary for the mmap cookie (eg the offset) to be
aligned to SHMLBA.
This creates a situation where all virtual mappings of the same physical
page share the same virtual cache index and guarantees VIPT coherence.
Otherwise the cache is non-coherent and the kernel will not see writes
by userspace when reading the shared page (or vice-versa).

Bart Van Assche noted that the ib DMA mapping code was significantly
similar enough to the core DMA mapping code that with a few changes
it was possible to remove the IB DMA mapping code entirely and
switch the RDMA stack to use the core DMA mapping code. This resulted
in a nice set of cleanups, but touched the entire tree. This branch
will be submitted separately to Linus at the end of the merge window
as per normal practice for tree wide changes like this.

Change the drivers to call ib_query_port in their get port
immutable handler instead of their own query port handler.
Doing this required to set the core cap flags of this device
before the ib_query_port call is made, since the IB core might
need these caps to serve the port query.
Drivers are ensured by the IB core that the port attributes passed
to the port query verb implementation are zero, and hence we
removed the zeroing from the drivers.

If the completer is in the middle of a large read operation, one
lost packet can cause havoc. Going to COMPST_ERROR_RETRY will
cause the requester to resend the request. After that, any packet
from the first attempt still in the receive queue will be
interpreted as an error, restarting the error/retry sequence.
The transfer will quickly exhaust its retries.
This behavior is very noticeable when doing 512KB reads on a
QEMU system configured with 1500B MTU.
Also, a resent request here will prompt the responder on the
other side to immediately start resending, but the resent
packets will get stuck in the already-loaded receive queue and
will never be processed.
Rather than erroring out every time an unexpected future packet
arrives, just drop it. Eventually the retry timer will send a
duplicate request; the completer will be able to make progress since
the queue will start relatively empty.

rvt_create_qp() creates qp->ip only when a qp creation request comes from
userspace (udata is not NULL). If we exceed the number of available
queue pairs however, the error path always attempts to put a kref to this
structure. If the requestor is inside the kernel, this leads to a crash.
We fix this by checking that qp->ip is not NULL before caling kref_put().

Use the new API to create and destroy the cq kthread worker.
The API hides some implementation details.
In particular, kthread_create_worker() allocates and initializes
struct kthread_worker. It runs the kthread the right way and stores
task_struct into the worker structure. In addition, the *on_cpu()
variant binds the kthread to the given cpu and the related memory
node.
kthread_destroy_worker() flushes all pending works, stops
the kthread and frees the structure.
This patch does not change the existing behavior. Note that we must
use the on_cpu() variant because the function starts the kthread
and it must bind it to the right CPU before waking. The numa node
is associated for given CPU as well.

The memory barrier is not enough to protect queuing works into
a destroyed cq kthread. Just imagine the following situation:
CPU1 CPU2
rvt_cq_enter()
worker = cq->rdi->worker;
rvt_cq_exit()
rdi->worker = NULL;
smp_wmb();
kthread_flush_worker(worker);
kthread_stop(worker->task);
kfree(worker);
// nothing queued yet =>
// nothing flushed and
// happily stopped and freed
if (likely(worker)) {
// true => read before CPU2 acted
cq->notify = RVT_CQ_NONE;
cq->triggered++;
kthread_queue_work(worker, &cq->comptask);
BANG: worker has been flushed/stopped/freed in the meantime.
This patch solves this by protecting the critical sections by
rdi->n_cqs_lock. It seems that this lock is not much contended
and looks reasonable for this purpose.
One catch is that rvt_cq_enter() might be called from IRQ context.
Therefore we must always take the lock with IRQs disabled to avoid
a possible deadlock.

It might be possible for all of a QP's references to be dropped
while one of that QP's tasklets is running.
For example, the completer might run during QP destroy.
If qp->valid is false, it will drop all of the packets on
the resp_pkts list, potentially removing the last reference.
Then it tries to advance the SQ consumer pointer. If the
SQ's buffer has already been destroyed, the system will
panic.
To be safe, hold a reference on the QP for the duration
of each tasklet.

The system may crash when a malformed request is received and
the error is detected by the responder.
NodeA: $ ibv_rc_pingpong -g 0 -d rxe0 -i 1 -n 1 -s 50000
NodeB: $ ibv_rc_pingpong -g 0 -d rxe0 -i 1 -n 1 -s 1024 <NodeA_ip>
The responder generates a receive error on node B since the incoming
SEND is oversized. If the client tears down the QP before the responder
or the completer finish running, a page fault may occur.
The fix makes the destroy operation spin until the tasks complete, which
appears to be original intent of the design.

Peek at the CQ after arming it so that we can return a hint.
This avoids missed completions due to a race between posting
CQEs and arming the CQ.
For example, CM teardown waits on MAD requests to complete with
ib_cq_poll_work(). Without this fix, the last completion might be
left on the CQ, hanging the kthread doing the teardown.

A client might post a read followed by a send. The partner receives

retries the read, the partner's responder processes the duplicate

To correctly handle a erroneous WR this fix does the following
1. Make sure the bad WQE causes a user completion event.
2. Call rxe_completer to handle the erred WQE.
Before the fix, when rxe_requester found a bad WQE, it changed its
status to IB_WC_LOC_PROT_ERR and exit with 0 for non RC QPs.
If this was the 1st WQE then there would be no ACK to invoke the
completer and this bad WQE would be stuck in the QP's send-q.
On top of that the requester exiting with 0 caused rxe_do_task to
endlessly invoke rxe_requester, resulting in a soft-lockup attached
below.
In case the WQE was not the 1st and rxe_completer did get a chance to
handle the bad WQE, it did not cause a complete event since the WQE's
IB_SEND_SIGNALED flag was not set.
Setting WQE status to IB_SEND_SIGNALED is subject to IBA spec
version 1.2.1, section 10.7.3.1 Signaled Completions.

Profiling shows that the key validation is susceptible
to cache line trading when accessing the lkey table.
Fix by separating out the read mostly fields from the write
fields. In addition the shift amount, which is function
of the lkey table size, is precomputed and stored with the
table pointer. Since both the shift and table pointer
are in the same read mostly cacheline, this saves a cache
line in this hot path.

The initial code for rdmavt carried with it a restriction that was a
vestige from the qib driver, that to dma map a page it had to be less
than a page size. This is not the case on modern hardware, both qib and
hfi1 will be just fine with unaligned map requests.
This fixes a 4.8 regression where by an IPoIB transfer of > PAGE_SIZE
will hang because the dma map page call always fails. This was
introduced after commit 5faba54695 ("IB/ipoib: Report SG feature
regardless of HW UD CSUM capability") added the capability to use SG by
default. Rather than override this, the HW supports it, so allow SG.

A good practice is to prefix the names of functions by the name
of the subsystem.
The kthread worker API is a mix of classic kthreads and workqueues. Each
worker has a dedicated kthread. It runs a generic function that process
queued works. It is implemented as part of the kthread subsystem.
This patch renames the existing kthread worker API to use
the corresponding name from the workqueues API prefixed by
kthread_:
__init_kthread_worker() -> __kthread_init_worker()
init_kthread_worker() -> kthread_init_worker()
init_kthread_work() -> kthread_init_work()
insert_kthread_work() -> kthread_insert_work()
queue_kthread_work() -> kthread_queue_work()
flush_kthread_work() -> kthread_flush_work()
flush_kthread_worker() -> kthread_flush_worker()
Note that the names of DEFINE_KTHREAD_WORK*() macros stay
as they are. It is common that the "DEFINE_" prefix has
precedence over the subsystem names.
Note that INIT() macros and init() functions use different
naming scheme. There is no good solution. There are several
reasons for this solution:
+ "init" in the function names stands for the verb "initialize"
aka "initialize worker". While "INIT" in the macro names
stands for the noun "INITIALIZER" aka "worker initializer".
+ INIT() macros are used only in DEFINE() macros
+ init() functions are used close to the other kthread()
functions. It looks much better if all the functions
use the same scheme.
+ There will be also kthread_destroy_worker() that will
be used close to kthread_cancel_work(). It is related
to the init() function. Again it looks better if all
functions use the same naming scheme.
+ there are several precedents for such init() function
names, e.g. amd_iommu_init_device(), free_area_init_node(),
jump_label_init_type(), regmap_init_mmio_clk(),
+ It is not an argument but it was inconsistent even before.

This patch adds qp number to important debug logs.

There is a problem when CONFIG_RDMA_RXE=y and CONFIG_IPV6=y. This
results in the rdma_rxe initialization occurring before the IPv6
services are ready. This patch delays the initialization of rdma_rxe
until after the IPv6 services are ready. This fix is based on one
proposed by Logan Gunthorpe on a much older code base.

This patch fixes below kernel crash on memory registration for rxe
and other transport drivers which has dma_ops extension.
IB/core invokes ib_map_sg_attrs() in generic manner with dma attributes
which is used by mlx5 and mthca adapters. However in doing so it
ignored honoring dma_ops extension of software based transports for
sg map/unmap operation. This results in calling dma_map_sg_attrs of
hardware virtual device resulting in crash for null reference.
We extend the core to support sg_map/unmap_attrs and transport drivers
to implement those dma_ops callback functions.
Verified usign perftest applications.

Both prepare4 and prepare6 sets loopback mask in pkt_info structure
instance of skb. The xmit_packet and other requester side functions
use a pkt_info struct from the stack without the proper mask. This
results in sending out the packet to the actual netdev device and
loopback functionality is broken.
Modify prepare() to pass its correctly marked pkt_info struct to
prepare4() and prepare6() instead of them using SKB_TO_PKT(skb) and
getting an incorrectly set mask.
