For the "struct tcmu_cmd_entry" in cmd area, the minimum size
will be sizeof(struct tcmu_cmd_entry) == 112 Bytes. And it could
fill about (sizeof(struct rsp) - sizeof(struct req)) /
sizeof(struct iovec) == 68 / 16 ~= 4 data regions(iov[4]) by
default.
For most tcmu_cmds, the data block indexes allocated from the
data area will be continuous. And for the continuous blocks they
will be merged into the same region using only one iovec. For
the current code, it will always allocates the same number of
iovecs with blocks for each tcmu_cmd, and it will wastes much
memories.
For example, when the block size is 4K and the DATA_OUT buffer
size is 64K, and the regions needed is less than 5(on my
environment is almost 99.7%). The current code will allocate
about 16 iovecs, and there will be (16 - 4) * sizeof(struct
iovec) = 192 Bytes cmd area memories wasted.
Here adds two helpers to calculate the base size and full size
of the tcmu_cmd. And will recalculate them again when it make sure
how many iovs is needed before insert it to cmd area.

For each target there will be one ring, when the target number
grows larger and larger, it could eventually runs out of the
system memories.
In this patch for each target ring, currently for the cmd area
the size will be fixed to 8MB and for the data area the size
will grow from 0 to max 256K * PAGE_SIZE(1G for 4K page size).
For all the targets' data areas, they will get empty blocks
from the "global data block pool", which has limited to 512K *
PAGE_SIZE(2G for 4K page size) for now.
When the "global data block pool" has been used up, then any
target could wake up the unmap thread routine to shrink other
targets' data area memories. And the unmap thread routine will
always try to truncate the ring vma from the last using block
offset.
When user space has touched the data blocks out of tcmu_cmd
iov[], the tcmu_page_fault() will try to return one zeroed blocks.
Here we move the timeout's tcmu_handle_completions() into unmap
thread routine, that's to say when the timeout fired, it will
only do the tcmu_check_expired_cmd() and then wake up the unmap
thread to do the completions() and then try to shrink its idle
memories. Then the cmdr_lock could be a mutex and could simplify
this patch because the unmap_mapping_range() or zap_* may go to
sleep.

Currently for the TCMU, the ring buffer size is fixed to 64K cmd
area + 1M data area, and this will be bottlenecks for high iops.
The struct tcmu_cmd_entry {} size is fixed about 112 bytes with
iovec[N] & N <= 4, and the size of struct iovec is about 16 bytes.
If N == 0, the ratio will be sizeof(cmd entry) : sizeof(datas) ==
112Bytes : (N * 4096)Bytes = 28 : 0, no data area is need.
If 0 < N <=4, the ratio will be sizeof(cmd entry) : sizeof(datas)
== 112Bytes : (N * 4096)Bytes = 28 : (N * 1024), so the max will
be 28 : 1024.
If N > 4, the sizeof(cmd entry) will be [(N - 4) *16 + 112] bytes,
and its corresponding data size will be [N * 4096], so the ratio
of sizeof(cmd entry) : sizeof(datas) == [(N - 4) * 16 + 112)Bytes
: (N * 4096)Bytes == 4/1024 - 12/(N * 1024), so the max is about
4 : 1024.
When N is bigger, the ratio will be smaller.
As the initial patch, we will set the cmd area size to 2M, and
the cmd area size to 32M. The TCMU will dynamically grows the data
area from 0 to max 32M size as needed.
The cmd area memory will be allocated through vmalloc(), and the
data area's blocks will be allocated individually later when needed.
The allocated data area block memory will be managed via radix tree.
For now the bitmap still be the most efficient way to search and
manage the block index, this could be update later.

When setting up an ALUA target port group with an invalid ID the
error message
kstrtoul() returned -22 for tg_pt_gp_id
is displayed, which is not really helpful.
Convert it to something sane.

This adds initial PGR support for just TCMU, since tcmu doesn't
have the necessary IT_NEXUS info to process PGR in userspace,
so have those commands be processed in kernel.
HA support is not available yet, we will work on it if this patch
is acceptable.

Currently ramdisk and fileio always perform PI verification
before and after backend IO. This approach is not very flexible.
Because some one may want to postpone this work to other layers in
IO stack. For example if we want to test blk_integrity_profile

If we failed to read data from backing file (probably because some one
truncate file under us), we must zerofill cmd's data, otherwise it will
be returned as is. Most likely cmd's data are unitialized pages from
page cache. This result in information leak.

Use the value of the BYTCHK field to determine the size of the
Data-Out buffer. For VERIFY, honor the VRPROTECT, DPO and FUA
fields. This patch avoids that LIO complains about a mismatch
between the expected transfer length and the SCSI CDB length
if the value of the BYTCHK field is 0.

refcount_t type and corresponding API should be
used instead of atomic_t when the variable is used as

Pull block layer updates from Jens Axboe:
- Add BFQ IO scheduler under the new blk-mq scheduling framework. BFQ
was initially a fork of CFQ, but subsequently changed to implement
fairness based on B-WF2Q+, a modified variant of WF2Q. BFQ is meant
to be used on desktop type single drives, providing good fairness.

- Add Kyber IO scheduler. This is a full multiqueue aware scheduler,
using a scalable token based algorithm that throttles IO based on
live completion IO stats, similary to blk-wbt. From Omar.
- A series from Jan, moving users to separately allocated backing
devices. This continues the work of separating backing device life
times, solving various problems with hot removal.
- A series of updates for lightnvm, mostly from Javier. Includes a
'pblk' target that exposes an open channel SSD as a physical block
device.
- A series of fixes and improvements for nbd from Josef.
- A series from Omar, removing queue sharing between devices on mostly
legacy drivers. This helps us clean up other bits, if we know that a
queue only has a single device backing. This has been overdue for
more than a decade.
- Fixes for the blk-stats, and improvements to unify the stats and user
windows. This both improves blk-wbt, and enables other users to
register a need to receive IO stats for a device. From Omar.
- blk-throttle improvements from Shaohua. This provides a scalable
framework for implementing scalable priotization - particularly for
blk-mq, but applicable to any type of block device. The interface is
marked experimental for now.
- Bucketized IO stats for IO polling from Stephen Bates. This improves
efficiency of polled workloads in the presence of mixed block size
IO.
- A few fixes for opal, from Scott.
- A few pulls for NVMe, including a lot of fixes for NVMe-over-fabrics.

- A series from Bart, improving our exposed info and capabilities from
the blk-mq debugfs support.

This passes on the scsi_cmnd result field to users of passthrough
requests. Currently we abuse req->errors for this purpose, but that
field will go away in its current form.
Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it. I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

For the bidirectional case, the Data-Out buffer blocks will always at
the head of the tcmu_cmd's bitmap, and before gathering the Data-In
buffer, first of all it should skip the Data-Out ones, or the device
supporting BIDI commands won't work.

Once upon a time back in 2009, a work-around was added to support
the GlobalSAN iSCSI initiator v3.3 for MacOSX, which during login
did not propose nor respond to MaxBurstLength, FirstBurstLength,
DefaultTime2Wait and DefaultTime2Retain keys.
The work-around in iscsi_check_proposer_for_optional_reply()
allowed the missing keys to be proposed, but did not require
waiting for a response before moving to full feature phase
operation. This allowed GlobalSAN v3.3 to work out-of-the
box, and for many years we didn't run into login interopt
issues with any other initiators..
Until recently, when Martin tried a QLogic 57840S iSCSI Offload
HBA on Windows 2016 which completed login, but subsequently
failed with:
Got unknown iSCSI OpCode: 0x43
The issue was QLogic MSFT side did not propose DefaultTime2Wait +
DefaultTime2Retain, so LIO proposes them itself, and immediately
transitions to full feature phase because of the GlobalSAN hack.
However, the QLogic MSFT side still attempts to respond to
DefaultTime2Retain + DefaultTime2Wait, even though LIO has set
ISCSI_FLAG_LOGIN_NEXT_STAGE3 + ISCSI_FLAG_LOGIN_TRANSIT
in last login response.
So while the QLogic MSFT side should have been proposing these
two keys to start, it was doing the correct thing per RFC-3720
attempting to respond to proposed keys before transitioning to
full feature phase.
All that said, recent versions of GlobalSAN iSCSI (v5.3.0.541)
does correctly propose the four keys during login, making the
original work-around moot.
So in order to allow QLogic MSFT to run unmodified as-is, go
ahead and drop this long standing work-around.

Multiple threads could be writing to alua_access_state at
the same time, or there could be multiple STPGs in flight
(different initiators sending them or one initiator sending
them to different ports), or a combo of both and the
core_alua_do_transition_tg_pt calls will race with each other.
Because from the last patches we no longer delay running
core_alua_do_transition_tg_pt_work, there does not seem to be
any point in running that in a workqueue. And, we always
wait for it to complete one way or another, so we can sleep
in this code path. So, this patch made over target-pending just adds a
mutex and does the work core_alua_do_transition_tg_pt_work was doing in
core_alua_do_transition_tg_pt.
There is also no need to use an atomic for the
tg_pt_gp_alua_access_state. In core_alua_do_transition_tg_pt we will
test and set it under the transition mutex. And, it is a int/32 bits
so in the other places where it is read, we will never see it partially
updated.

This patch fixes a set of queue-full response handling
bugs, where outgoing responses are leaked when a fabric
driver is propagating non -EAGAIN or -ENOMEM errors
to target-core.
It introduces TRANSPORT_COMPLETE_QF_ERR state used to
signal when CHECK_CONDITION status should be generated,
when fabric driver ->write_pending(), ->queue_data_in(),
or ->queue_status() callbacks fail with non -EAGAIN or
-ENOMEM errors, and data-transfer should not be retried.
Note all fabric driver -EAGAIN and -ENOMEM errors are
still retried indefinately with associated data-transfer
callbacks, following existing queue-full logic.
Also fix two missing ->queue_status() queue-full cases
related to CMD_T_ABORTED w/ TAS status handling.

The t_data_nents and t_bidi_data_nents are the numbers of the
segments, but it couldn't be sure the block size equals to size
of the segment.
For the worst case, all the blocks are discontiguous and there
will need the same number of iovecs, that's to say: blocks == iovs.
So here just set the number of iovs to block count needed by tcmu
cmd.

If there has BIDI data, its first iov[] will overwrite the last
iov[] for se_cmd->t_data_sg.
To fix this, we can just increase the iov pointer, but this may
introuduce a new memory leakage bug: If the se_cmd->data_length
and se_cmd->t_bidi_data_sg->length are all not aligned up to the
DATA_BLOCK_SIZE, the actual length needed maybe larger than just
sum of them.
So, this could be avoided by rounding all the data lengthes up
to DATA_BLOCK_SIZE.

This patch closes a race between se_lun deletion during configfs
unlink in target_fabric_port_unlink() -> core_dev_del_lun()
-> core_tpg_remove_lun(), when transport_clear_lun_ref() blocks
waiting for percpu_ref RCU grace period to finish, but a new
NodeACL mappedlun is added before the RCU grace period has
completed.
This can happen in target_fabric_mappedlun_link() because it
only checks for se_lun->lun_se_dev, which is not cleared until
after transport_clear_lun_ref() percpu_ref RCU grace period
finishes.
This bug originally manifested as NULL pointer dereference
OOPsen in target_stat_scsi_att_intr_port_show_attr_dev() on
v4.1.y code, because it dereferences lun->lun_se_dev without
a explicit NULL pointer check.
In post v4.1 code with target-core RCU conversion, the code
in target_stat_scsi_att_intr_port_show_attr_dev() no longer
uses se_lun->lun_se_dev, but the same race still exists.
To address the bug, go ahead and set se_lun>lun_shutdown as
early as possible in core_tpg_remove_lun(), and ensure new
NodeACL mappedlun creation in target_fabric_mappedlun_link()
fails during se_lun shutdown.

This patch fixes a iscsi-target specific TMR reference leak
during session shutdown, that could occur when a TMR was
quiesced before the hand-off back to iscsi-target code
via transport_cmd_check_stop_to_fabric().
The reference leak happens because iscsit_free_cmd() was
incorrectly skipping the final target_put_sess_cmd() for
TMRs when transport_generic_free_cmd() returned zero because
the se_cmd->cmd_kref did not reach zero, due to the missing
se_cmd assignment in original code.
The result was iscsi_cmd and it's associated se_cmd memory
would be freed once se_sess->sess_cmd_map where released,
but the associated se_tmr_req was leaked and remained part
of se_device->dev_tmr_list.
This bug would manfiest itself as kernel paging request
OOPsen in core_tmr_lun_reset(), when a left-over se_tmr_req
attempted to dereference it's se_cmd pointer that had
already been released during normal session shutdown.
To address this bug, go ahead and treat ISCSI_OP_SCSI_CMD
and ISCSI_OP_SCSI_TMFUNC the same when there is an extra
se_cmd->cmd_kref to drop in iscsit_free_cmd(), and use
op_scsi to signal __iscsit_free_cmd() when the former

Instead of putting cmd_time_out under ../target/core/user_0/foo/control,
which has historically been used by parameters needed for initial
backend device configuration, go ahead and move cmd_time_out into
a backend device attribute.
In order to do this, tcmu_module_init() has been updated to create
a local struct configfs_attribute **tcmu_attrs, that is based upon
the existing passthrough_attrib_attrs along with the new cmd_time_out
attribute. Once **tcm_attrs has been setup, go ahead and point
it at tcmu_ops->tb_dev_attrib_attrs so it's picked up by target-core.
Also following MNC's previous change, ->cmd_time_out is stored in
milliseconds but exposed via configfs in seconds. Also, note this
patch restricts the modification of ->cmd_time_out to before +
after the TCMU device has been configured, but not while it has
active fabric exports.

A single daemon could implement multiple types of devices
using multuple types of real devices that may not support
restarting from crashes and/or handling tcmu timeouts. This
makes the cmd timeout configurable, so handlers that do not
support it can turn if off for now.

This fixes the following races:
1. core_alua_do_transition_tg_pt could have read
tg_pt_gp_alua_access_state and gone into this if chunk:
if (!explicit &&

ALUA_ACCESS_STATE_TRANSITION) {
and then core_alua_do_transition_tg_pt_work could update the
state. core_alua_do_transition_tg_pt would then only set
tg_pt_gp_alua_pending_state and the tg_pt_gp_alua_access_state would
not get updated with the second calls state.
2. core_alua_do_transition_tg_pt could be setting
tg_pt_gp_transition_complete while the tg_pt_gp_transition_work
is already completing. core_alua_do_transition_tg_pt then waits on the
completion that will never be called.
To handle these issues, we just call flush_work which will return when
core_alua_do_transition_tg_pt_work has completed so there is no need
to do the complete/wait. And, if core_alua_do_transition_tg_pt_work
was running, instead of trying to sneak in the state change, we just
schedule up another core_alua_do_transition_tg_pt_work call.
Note that this does not handle a possible race where there are multiple
threads call core_alua_do_transition_tg_pt at the same time. I think
we need a mutex in target_tg_pt_gp_alua_access_state_store.

The implicit transition time tells initiators the min time
to wait before timing out a transition. We currently schedule
the transition to occur in tg_pt_gp_implicit_trans_secs
seconds so there is no room for delays. If
core_alua_do_transition_tg_pt_work->core_alua_update_tpg_primary_metadata
needs to write out info to a remote file, then the initiator can
easily time out the operation.

If tcmu-runner is processing a STPG and needs to change the kernel's
ALUA state then we cannot use the same work queue for task management
requests and ALUA transitions, because we could deadlock. The problem
occurs when a STPG times out before tcmu-runner is able to
call into target_tg_pt_gp_alua_access_state_store->
core_alua_do_port_transition -> core_alua_do_transition_tg_pt ->
queue_work. In this case, the tmr is on the work queue waiting for
the STPG to complete, but the STPG transition is now queued behind
the waiting tmr.
Note:
This bug will also be fixed by this patch:

which switches the tmr code to use the system workqueues.
For both, I am not sure if we need a dedicated workqueue since
it is not a performance path and I do not think we need WQ_MEM_RECLAIM
to make forward progress to free up memory like the block layer does.

We do not setup the LU group for pscsi devices, so if you write
a state to alua_access_state that will cause a transition you will
get a NULL pointer dereference.
This patch will fail attempts to try and transition the path
for backend devices that set the TRANSPORT_FLAG_PASSTHROUGH_ALUA
flag.

This patch allows passthrough backends to use the core/base LIO
ALUA setup and state checks, but still handle the execution of
commands.
This will allow the target_core_user module to execute STPG and RTPG
in userspace, and not have to duplicate the ALUA state checks, path
information (needed so we can check if command is executable on
specific paths) and setup (rtslib sets/updates the configfs ALUA
interface like it does for iblock or file).
For STPG, the target_core_user userspace daemon, tcmu-runner will
still execute the STPG, and to update the core/base LIO state it
will use the existing configfs interface. For RTPG, tcmu-runner
will loop over configfs and/or cache the state.

tcmu hard codes the hw_max_sectors to 128 which is a litle small.
Userspace uses the max_sectors to report the optimal IO size and
some initiators perform better with larger IOs (open-iscsi seems
to do better with 256 to 512 depending on the test).

All in-tree fabric drivers provide a tfo->check_stop_free(),
so there is no need to do the extra check within existing
transport_cmd_check_stop_to_fabric() code.
Just to be sure, add a check in target_fabric_tf_ops_check()
to notify any out-of-tree drivers that might be missing it.

As reported by Max, the Windows 2008 R2 chkdsk utility expects
VERIFY_16 to be supported, and does not handle the returned
CHECK_CONDITION properly, resulting in an infinite loop.
The kernel will log huge amounts of this error:

The following fixes a divide by zero OOPs with TYPE_TAPE
due to pscsi_tape_read_blocksize() failing causing a zero
sd->sector_size being propigated up via dev_attrib.hw_block_size.
It also fixes another long-standing bug where TYPE_TAPE and
TYPE_MEDIMUM_CHANGER where using pscsi_create_type_other(),
which does not call scsi_device_get() to take the device
reference. Instead, rename pscsi_create_type_rom() to
pscsi_create_type_nondisk() and use it for all cases.
Finally, also drop a dump_stack() in pscsi_get_blocks() for
non TYPE_DISK, which in modern target-core can get invoked
via target_sense_desc_format() during CHECK_CONDITION.

<linux/sched.h> header footprint, to speed up the kernel build and to
have a cleaner header structure.
After these changes the new <linux/sched.h>'s typical preprocessed
size goes down from a previous ~0.68 MB (~22K lines) to ~0.45 MB (~15K
lines), which is around 40% faster to build on typical configs.
Not much changed from the last version (-v2) posted three weeks ago: I
eliminated quirks, backmerged fixes plus I rebased it to an upstream
SHA1 from yesterday that includes most changes queued up in -next plus
all sched.h changes that were pending from Andrew.
I've re-tested the series both on x86 and on cross-arch defconfigs,
and did a bisectability test at a number of random points.
I tried to test as many build configurations as possible, but some
build breakage is probably still left - but it should be mostly
limited to architectures that have no cross-compiler binaries

Create a trivial placeholder <linux/sched/signal.h> file that just

This patch introduces two counters for ABORT_TASK success +
failure under:
/sys/kernel/config/target/core/$HBA/$DEV/statistics/scsi_tgt_dev/
that are useful for diagnosing various backend device latency
and front fabric issues.
Normally when folks see alot of aborts_complete happening,
it means the backend device I/O completion latency is high,
and not returning completions fast enough before host side
timeouts trigger.
And normally when folks see alot of aborts_no_task, it means
completions are being posted by target-core into fabric driver
code, but the responses aren't making it back to the host.

Due to the long standing checks in iscsit_snmp_get_tiqn()
that assume conn->sess->tpg dereference of tpg->tpg_tiqn
for iscsit_collect_login_stats() usage, some of the early
login failure cases like ISCSI_LOGIN_STATUS_TGT_FORBIDDEN
where not getting incremented, due to sess->tpg assignment
happening later in iscsi_login_zero_tsih_s2().
Instead, use the earlier conn->tpg assignment done by
iscsi_target_locate_portal() -> iscsit_get_tpg_from_np()
so the existing counters are incremented correctly for
the various early login failure cases.
Also, go ahead and drop the old rate limiting check in
iscsit_collect_login_stats(), so we get the true number
of failed login attempts in the existing statistics.

When transport_clear_lun_ref() is shutting down a se_lun via
configfs with new I/O in-flight, it's possible to trigger a
NULL pointer dereference in transport_lookup_cmd_lun() due
to the fact percpu_ref_get() doesn't do any __PERCPU_REF_DEAD
checking before incrementing lun->lun_ref.count after
lun->lun_ref has switched to atomic_t mode.
This results in a NULL pointer dereference as LUN shutdown
code in core_tpg_remove_lun() continues running after the
existing ->release() -> core_tpg_lun_ref_release() callback
completes, and clears the RCU protected se_lun->lun_se_dev
pointer.
During the OOPs, the state of lun->lun_ref in the process
which triggered the NULL pointer dereference looks like

To address this bug, use percpu_ref_tryget_live() to ensure
once __PERCPU_REF_DEAD is visable on all CPUs and ->lun_ref
has switched to atomic_t, all new I/Os will fail to obtain
a new lun->lun_ref reference.
Also use an explicit percpu_ref_kill_and_confirm() callback
to block on ->lun_ref_comp to allow the first stage and
associated RCU grace period to complete, and then block on
->lun_ref_shutdown waiting for the final percpu_ref_put()
to drop the last reference via transport_lun_remove_cmd()
before continuing with core_tpg_remove_lun() shutdown.

This patch does an explicit list_del_init(tmr->tmr_list) in
core_tmr_drain_tmr_list() before starting processing of
outstanding TMRs to abort, instead of explicitly checking

If a target driver (e.g. tcm_qla2xxx) calls
transport_generic_request_failure() to report that receiving data
has failed and that SCSI command has already been aborted by the
initiator, ensure that the SCSI status ABORTED is sent back to the
initiator instead of the sense code provided by the target driver.

I think the transport statistics device file was supposed
to show scsiTransportProtocolType. It instead shows the
fabric name which is normally closer to the driver name.
I was thinking I cannot change from fabric name to protocol
type name incase people are expecting the driver name, so
this patch adds another file proto_id that exports the SCSI
protocol identifier ID.

In case of unsolicited data for the first sequence
seq_end_offset must be set to minimum of total data length
and FirstBurstLength, so do not add cmd->write_data_done
to the min of total data length and FirstBurstLength.
This patch avoids that with ImmediateData=Yes, InitialR2T=No,
MaxXmitDataSegmentLength < FirstBurstLength that a WRITE command
with IO size above FirstBurstLength triggers sequence error
messages, for example

Chelsio T6 adapters reduce number of completion
to host by generating single completion for all
directly placed(DDP) iSCSI pdus in a sequence,
completion contains iSCSI hdr of the last pdu
in a sequence.
On receiving DDP completion cxgbit driver finds
iSCSI cmd using iscsit_find_cmd_from_itt_or_dump(),
then updates cmd->write_data_done, cmd->next_burst_len,
cmd->data_sn and calls __iscsit_check_dataout_hdr()
to validate iSCSI hdr.

Split iscsit_check_dataout_hdr() into two functions
1. __iscsit_check_dataout_hdr() - This function
validates data out hdr.
2. iscsit_check_dataout_hdr() - This function finds
iSCSI cmd using iscsit_find_cmd_from_itt_or_dump(),
then it calls __iscsit_check_dataout_hdr() to
validate iSCSI hdr.
This split is required to support Chelsio T6 iSCSI
DDP completion feature. T6 adapters reduce number of
completions to host by generating single completion
for all directly placed(DDP) iSCSI pdus in a sequence,
DDP completion contains iSCSI hdr of the last pdu in a
sequence.
On receiving DDP completion cxgbit driver will first
find iSCSI cmd using iscsit_find_cmd_from_itt_or_dump()

It is a relatively common idiom (8 instances) to first look up an IDR
entry, and then remove it from the tree if it is found, possibly doing
further operations upon the entry afterwards. If we change idr_remove()
to return the removed object, all of these users can save themselves a
walk of the IDR tree.

The patch that reworks task management function handling guarantees
that target_remove_from_state_list() is always called with CMD_T_BUSY
cleared. Since that function is the only function that tests that flag
this means that that flag is now superfluous. Hence remove that flag.

The function transport_cmd_check_stop() has two callers. These callers
invoke this function as follows:
* transport_cmd_check_stop(cmd, true, false)
* transport_cmd_check_stop(cmd, false, true)
Hence inline this function into its callers.
This patch does not change any functionality but improves source
code readability.

Stop execution if CMD_T_STOP has been set for a command just after
the command has been added to the device command list and before
.write_pending() is called. The following sequence can trigger this:
- transport_handle_cdb_direct() gets called. This function namely
sets CMD_T_ACTIVE before it calls transport_generic_new_cmd().
- __transport_wait_for_tasks() is called concurrently. This function
sets CMD_T_STOP for all active commands that have not been aborted.

transport_wait_for_tasks() not only waits for command completion
but also sets CMD_T_STOP. Additionally, this function is not only
called by frontend drivers but also by the target core. Update
the transport_wait_for_tasks() documentation to reflect this.

It is possible that two commands with the same tag are present on
sess_cmd_list because commands are removed from sess_cmd_list after
a response has been sent to the initiator. Hence continue searching
through sess_cmd_list even if a matching tag has already been found.

This patch addresses a long standing bug where the commit phase
of COMPARE_AND_WRITE would result in a se_cmd->cmd_kref reference
leak if se_cmd->scsi_status returned non SAM_STAT_GOOD.
This would manifest first as a lost SCSI response, and eventual
hung task during fabric driver logout or re-login, as existing
shutdown logic waited for the COMPARE_AND_WRITE se_cmd->cmd_kref
to reach zero.
To address this bug, compare_and_write_post() has been changed
to drop the incorrect !cmd->scsi_status conditional that was
preventing *post_ret = 1 for being set during non SAM_STAT_GOOD
status.
This patch has been tested with SAM_STAT_CHECK_CONDITION status
from normal target_complete_cmd() callback path, as well as the
incoming __target_execute_cmd() submission failure path when
se_cmd->execute_cmd() returns non zero status.

This patch addresses a long-standing bug with multi-session
(eg: iscsi-target + iser-target) se_node_acl dynamic free
withini transport_deregister_session().
This bug is caused when a storage endpoint is configured with
demo-mode (generate_node_acls = 1 + cache_dynamic_acls = 1)
initiators, and initiator login creates a new dynamic node acl
and attaches two sessions to it.
After that, demo-mode for the storage instance is disabled via
configfs (generate_node_acls = 0 + cache_dynamic_acls = 0) and
the existing dynamic acl is never converted to an explicit ACL.
The end result is dynamic acl resources are released twice when
the sessions are shutdown in transport_deregister_session().
If the storage instance is not changed to disable demo-mode,
or the dynamic acl is converted to an explict ACL, or there
is only a single session associated with the dynamic ACL,
the bug is not triggered.
To address this big, move the release of dynamic se_node_acl
memory into target_complete_nacl() so it's only freed once
when se_node_acl->acl_kref reaches zero.

This patch fixes a bug where incoming task management requests
can be explicitly aborted during an active LUN_RESET, but who's
struct work_struct are canceled in-flight before execution.
This occurs when core_tmr_drain_tmr_list() invokes cancel_work_sync()
for the incoming se_tmr_req->task_cmd->work, resulting in cmd->work
for target_tmr_work() never getting invoked and the aborted TMR
waiting indefinately within transport_wait_for_tasks().
To address this case, perform a CMD_T_ABORTED check early in
transport_generic_handle_tmr(), and invoke the normal path via
transport_cmd_check_stop_to_fabric() to complete any TMR kthreads
blocked waiting for CMD_T_STOP in transport_wait_for_tasks().
Also, move the TRANSPORT_ISTATE_PROCESSING assignment earlier
into transport_generic_handle_tmr() so the existing check in
core_tmr_drain_tmr_list() avoids attempting abort the incoming
se_tmr_req->task_cmd->work if it has already been queued into
se_device->tmr_wq.

This patch adds the missing target_complete_cmd() SCSI status
parameter change in target_xcopy_do_work(), that was originally
missing in commit 926317de33.
It correctly propigates up the correct SCSI status during
EXTENDED_COPY exception cases, instead of always using the
hardcoded SAM_STAT_CHECK_CONDITION from original code.
This is required for ESX host environments that expect to
hit SAM_STAT_RESERVATION_CONFLICT for certain scenarios,
and SAM_STAT_CHECK_CONDITION results in non-retriable
status for these cases.

After the v4.2+ RCU conversion to se_node_acl->lun_entry_hlist,
a BUG_ON() was added in core_enable_device_list_for_node() to
detect when the located orig->se_lun_acl contains an existing
se_lun_acl pointer reference.
However, this scenario can happen when a dynamically generated
NodeACL is being converted to an explicit NodeACL, when the
explicit NodeACL contains a different LUN mapping than the
default provided by the WWN endpoint.
So instead of triggering BUG_ON(), go ahead and fail instead
following the original pre RCU conversion logic.

Instead of keeping two levels of indirection for requests types, fold it
all into the operations. The little caveat here is that previously
cmd_type only applied to struct request, while the request and bio op
fields were set to plain REQ_OP_READ/WRITE even for passthrough
operations.
Instead this patch adds new REQ_OP_* for SCSI passthrough and driver
private requests, althought it has to add two for each so that we
can communicate the data in/out nature of the request.

We only need this code to support scsi, ide, cciss and virtio. And at
least for virtio it's a deprecated feature to start with.
This should shrink the kernel size for embedded device that only use,
say eMMC a bit.

Port and ACL information must be configured before an initiator
logs in. Make it possible to configure this information before
a subnet prefix has been assigned to a port by not only accepting
GIDs as target port and initiator port names but by also accepting
port GUIDs.
Add a 'priv' member to struct se_wwn to allow target drivers to
associate their own data with struct se_wwn.

The XCOPY specification in SPC4r37 states that the XCOPY source and
destination device(s) should be derived from the copy source and copy
destination (CSCD) descriptor IDs in the XCOPY segment descriptor.

Currently LIO completely ignores the src and dest CSCD IDs in the
Segment Descriptor List, and instead assumes that the first entry in the
CSCD list corresponds to the source, and the second to the destination.
This commit removes this assumption, by ensuring that the Segment
Descriptor List is parsed prior to processing the CSCD Descriptor List.
CSCD Descriptor List processing is modified to compare the current list
index with the previously obtained src and dest CSCD IDs.
Additionally, XCOPY requests where the src and dest CSCD IDs refer to
the CSCD Descriptor List entry can now be successfully processed.

If iscsit_tpg_add_network_portal() fails then
return error code instead of 0 to user space.
If iscsi-target returns 0 then user space keeps
on retrying same command infinitely, targetcli or
echo hangs till command completes with non zero
return value. In some cases it is possible that
add network portal command never completes with
success even after retrying multiple times,
for example - cxgbit_setup_np() always returns

This iscsit_tpg_add_portal_group() function is only called from
lio_target_tiqn_addtpg(). Both functions free the "tpg" pointer on
error so it's a double free bug. The memory is allocated in the caller
so it should be freed in the caller and not here.

Don't free the cmd in tcmu_check_expired_cmd, it's still referenced by
an entry in our cmd_id->cmd idr. If userspace ever resumes processing,
tcmu_handle_completions() will use the now-invalid cmd pointer.
Instead, don't free cmd. It will be freed by tcmu_handle_completion() if
userspace ever recovers, or tcmu_free_device if not.

Remove superfluous #include directives from the include/target/*.h
files. Add missing #include directives to other *.h and *.c files.
Use forward declarations for structures where possible. This
change reduces the build time for make M=drivers/target on my
laptop from 27.1s to 18.7s or by about 30%.

Documentation/filesystems/configfs/configfs.txt says:
"When unlink(2) is called on the symbolic link, the source item is
notified via the ->drop_link() method. Like the ->drop_item() method,
this is a void function and cannot return failure."
The ->drop_item() is indeed a void function, the ->drop_link() is
actually not. This, together with the fact that the value of ->drop_link()
is silently ignored suggests, that it is the ->drop_link() return
type that should be corrected and changed to void.
This patch changes drop_link() signature and all its users.

Allocate resources dynamically for Upper layer driver's (ULD) like
cxgbit, iw_cxgb4, cxgb4i and chcr. The resources allocated include Tx
queues which are allocated when ULD register with cxgb4 driver and freed
while un-registering. The Tx queues which are shared by ULD shall be
allocated by first registering driver and un-allocated by last
unregistering driver.

Now genl_register_family() is the only thing (other than the
users themselves, perhaps, but I didn't find any doing that)
writing to the family struct.
In all families that I found, genl_register_family() is only
called from __init functions (some indirectly, in which case
I've add __init annotations to clarifly things), so all can
actually be marked __ro_after_init.
This protects the data structure from accidental corruption.

Instead of providing macros/inline functions to initialize
the families, make all users initialize them statically and
get rid of the macros.
This reduces the kernel code size by about 1.6k on x86-64
(with allyesconfig).

Static family IDs have never really been used, the only
use case was the workaround I introduced for those users
that assumed their family ID was also their multicast
group ID.
Additionally, because static family IDs would never be
reserved by the generic netlink code, using a relatively
low ID would only work for built-in families that can be
registered immediately after generic netlink is started,
which is basically only the control family (apart from
the workaround code, which I also had to add code for so
it would reserve those IDs)
Thus, anything other than GENL_ID_GENERATE is flawed and
luckily not used except in the cases I mentioned. Move
those workarounds into a few lines of code, and then get
rid of GENL_ID_GENERATE entirely, making it more robust.

The libfc stack assigns exchange IDs based on the CPU the request
was received on, so we need to send the responses via the same CPU.
Otherwise the send logic gets confuses and responses will be delayed,
causing exchange timeouts on the initiator side.

target_sess_cmd_list_waiting() might hit on a condition where
the kref for the command is already 0, but the destructor has
not been called yet (or is stuck in waiting for a spin lock).
Rather than leaving the command on the list we should explicitly
remove it to avoid race issues later on.

Reverting this patch, as it incorrectly assumes the additional length
for INQUIRY in target_complete_cmd_with_length() is SCSI allocation
length, which breaks existing user-space code when SCSI allocation
length is smaller than additional length.

This patch addresses a bug where a local EXTENDED_COPY WRITE or READ
backend I/O request would always return SAM_STAT_CHECK_CONDITION,
even if underlying xcopy_pt_cmd->se_cmd generated a different
SCSI status code.
ESX host environments expect to hit SAM_STAT_RESERVATION_CONFLICT
for certain scenarios, and SAM_STAT_CHECK_CONDITION results in
non-retriable status for these cases.
Tested on v4.1.y with ESX v5.5u2+ with local IBLOCK backend copy.

This patch addresses a bug where EXTENDED_COPY across multiple LUNs
results in a CHECK_CONDITION when the source + destination are not
located on the same physical node.
ESX Host environments expect sense COPY_ABORTED w/ COPY TARGET DEVICE
NOT REACHABLE to be returned when this occurs, in order to signal
fallback to local copy method.
As described in section 6.3.3 of spc4r22:
"If it is not possible to complete processing of a segment because the
copy manager is unable to establish communications with a copy target
device, because the copy target device does not respond to INQUIRY,
or because the data returned in response to INQUIRY indicates
an unsupported logical unit, then the EXTENDED COPY command shall be
terminated with CHECK CONDITION status, with the sense key set to
COPY ABORTED, and the additional sense code set to COPY TARGET DEVICE
NOT REACHABLE."
Tested on v4.1.y with ESX v5.5u2+ with BlockCopy across multiple nodes.

This patch fixes a regression in >= v4.1.y code where the original
SCF_ACK_KREF assignment in target_get_sess_cmd() was dropped upstream
in commit 054922bb, but the series for addressing TMR ABORT_TASK +
LUN_RESET with fabric session reinstatement in commit febe562c20 still
depends on this code in transport_cmd_finish_abort().
The regression manifests itself as a se_cmd->cmd_kref +1 leak, where
ABORT_TASK + LUN_RESET can hang indefinately for a specific I_T session
for drivers using SCF_ACK_KREF, resulting in hung kthreads.
This patch has been verified with v4.1.y code.

If iscsi-target receives NOP OUT with ITT and TTT
set to 0xffffffff it allocates iscsi_cmd but

Userspace should be implementing VPD B0 (Block Limits) to inform the
initiator of max data size, but just in case we do get a too-large request,
do what the spec says and return INVALID_CDB_FIELD.
Make sure to unlock udev->cmdr_lock before returning.

Add common library module(libcxgb.ko) for
Chelsio drivers to remove duplicate code.
Code for iSCSI DDP Page Pod Manager is moved
from cxgb4.ko to libcxgb.ko. Earlier only cxgbit.ko
was using this code, now cxgb3i and cxgb4i will
also use common Page Pod manager code.
In future this module will have common connection
management and hardware specific code that can be
shared by multiple Chelsio drivers.

This patch fixes residual overflow handling to correctly set the
residual_count using SPDTL, instead of SCSI Allocation Length.
Allocation Length is the maximum value of the SPDTL and not substitute
for it, hence it shouldn’t be used to calculate ResidualCount except for
cases where SPDTL > Allocation Length and Data is truncated (in that
case both Alloc Len and SPDTL are same). (SPC 5r01 Section 4.2.5.6).

Using blk_rq_append_bio allows to append the bios to the request
directly instead of having to build up a list first, and also
allows to have a single code path for requests with or without
data attached to them.

In MC/S scenario, the conn->sess has been set NULL in
iscsi_login_non_zero_tsih_s1 when the second connection comes here,
then kernel panic.
The conn->sess will be assigned in iscsi_login_non_zero_tsih_s2. So
we should check whether it's NULL before calling.

If a Simple command is sent with a failure, target_setup_cmd_from_cdb
returns with TCM_UNSUPPORTED_SCSI_OPCODE or TCM_INVALID_CDB_FIELD.
So in the cases where target_setup_cmd_from_cdb returns an error, we
never get far enough to call target_execute_cmd to increment simple_cmds.
Since simple_cmds isn't incremented, the result of the failure from
target_setup_cmd_from_cdb causes transport_generic_request_failure to
decrement simple_cmds, due to call to transport_complete_task_attr.
With this dev->simple_cmds or dev->dev_ordered_sync is now -1, not 0.
So when a subsequent command with an Ordered Task is sent, it causes
a hang, since dev->simple_cmds is at -1.

If a command with a Simple task attribute is failed due to a Unit
Attention, then a subsequent command with an Ordered task attribute
will hang forever. The reason for this is that the Unit Attention
status is checked for in target_setup_cmd_from_cdb, before the call
to target_execute_cmd, which calls target_handle_task_attr, which
in turn increments dev->simple_cmds.
However, transport_generic_request_failure still calls
transport_complete_task_attr, which will decrement dev->simple_cmds.
In this case, simple_cmds is now -1. So when a command with the
Ordered task attribute is sent, target_handle_task_attr sees that
dev->simple_cmds is not 0, so it decides it can't execute the
command until all the (nonexistent) Simple commands have completed.

max_discard_sectors only 32bits, and some non scsi backend
devices will set this to the max 0xffffffff, so we can end up
overflowing during the max_unmap_lba_count calculation.

This patch fixes a race in iscsit_release_commands_from_conn() ->
iscsit_free_cmd() -> transport_generic_free_cmd() + wait_for_tasks=1,
where CMD_T_FABRIC_STOP could end up being set after the final
kref_put() is called from core_tmr_abort_task() context.
This results in transport_generic_free_cmd() blocking indefinately
on se_cmd->cmd_wait_comp, because the target_release_cmd_kref()
check for CMD_T_FABRIC_STOP returns false.
To address this bug, make iscsit_release_commands_from_conn()
do list_splice and set CMD_T_FABRIC_STOP early while holding
iscsi_conn->cmd_lock. Also make iscsit_aborted_task() only
remove iscsi_cmd_t if CMD_T_FABRIC_STOP has not already been
set.
Finally in target_release_cmd_kref(), only honor fabric_stop
if CMD_T_ABORTED has been set.

During transport_generic_free_cmd() with a concurrent TMR
ABORT_TASK and shutdown CMD_T_FABRIC_STOP bit set, the
caller will be blocked on se_cmd->cmd_wait_stop completion
until the final kref_put() -> target_release_cmd_kref()
has been invoked to call complete().
However, when ABORT_TASK is completed with FUNCTION_COMPLETE
in core_tmr_abort_task(), the aborted se_cmd will have already
been removed from se_sess->sess_cmd_list via list_del_init().
This results in target_release_cmd_kref() hitting the
legacy list_empty() == true check, invoking ->release_cmd()
but skipping complete() to wakeup se_cmd->cmd_wait_stop
blocked earlier in transport_generic_free_cmd() code.
To address this bug, it's safe to go ahead and drop the
original list_empty() check so that fabric_stop invokes
the complete() as expected, since list_del_init() can
safely be used on a empty list.

There is a iscsi-target/tcp login race in LOGIN_FLAGS_READY
state assignment that can result in frequent errors during
iscsi discovery:
"iSCSI Login negotiation failed."
To address this bug, move the initial LOGIN_FLAGS_READY
assignment ahead of iscsi_target_do_login() when handling
the initial iscsi_target_start_negotiation() request PDU
during connection login.
As iscsi_target_do_login_rx() work_struct callback is
clearing LOGIN_FLAGS_READ_ACTIVE after subsequent calls
to iscsi_target_do_login(), the early sk_data_ready
ahead of the first iscsi_target_do_login() expects
LOGIN_FLAGS_READY to also be set for the initial
login request PDU.
As reported by Maged, this was first obsered using an
MSFT initiator running across multiple VMWare host
virtual machines with iscsi-target/tcp.

Instead of special casing the handful of callers that check for

Go ahead and fold the duplicate iscsi_tpg_np driver attribute
show/store functions into generic callers, invoked by current
driver specific functions.
This allows for future v4.8+ changes to have iscsi_target_mod
use tpg_np driver attributes at runtime, instead of having
the defaults hardcoded in iscsi_target_configfs.c.

The SRP target driver will need to allocate and chain it's own SGLs soon.
For this export target_alloc_sgl, and add a new argument to it so that it
can allocate an additional chain entry that doesn't point to a page. Also
export transport_free_sgl after renaming it to target_free_sgl to free
these SGLs again.

Ensures the first page entry is within bounds. A failed check would
terminate the iSCSI connection instead of causing a NULL-dereference.
This violation seems to happen with certain iSCSI commands where the
computed CDB length is zero but the expected transfer length is
non-zero. The real problem is probably on the iSCSI initiator side
since there is a discrepancy between the iSCSI header and the
encapsulated CDB Opcode.

The removal the #ifdef'd code on CONFIG_ARCH_HAS_SG_CHAIN in
commit 02c4de53ad ("target/rd: always chain S/G list")
removed the setting of need_to_release to true, so we are now left
with need_to_release always being false and hence the conditional
kfree is never reached. Clean up the dead code by removing need_to_release
and the conditional kfree.
