The tty3270_resize_work functions acquires a reference to the tty
to call tty_do_resize but fails to release the reference again.

Replace ASYNC_INITIALIZED bit in the tty_port::flags field with
TTY_PORT_INITIALIZED bit in the tty_port::iflags field. Introduce helpers
tty_port_set_initialized() and tty_port_initialized() to abstract
atomic bit ops.
Note: the transforms for test_and_set_bit() and test_and_clear_bit()
are unnecessary as the state transitions are already mutually exclusive;
the tty lock prevents concurrent open/close/hangup.

The sclp_ctl_ioctl_sccb function uses two copy_from_user calls to
retrieve the sclp request from user space. The first copy_from_user
fetches the length of the request which is stored in the first two
bytes of the request. The second copy_from_user gets the complete
sclp request, but this copies the length field a second time.
A malicious user may have changed the length in the meantime.

Sort the sclp event type defines and use a macro to create the
corresponding event type masks. In addition to that one unused
type/mask pair is removed and another previously unused define
is used now (it was probably unused/unknown because it didn't

dcssblk_remove_store() holds the dcssblk_devices_sem semaphore while
calling device_unregister(), which in turn tries to acquire the kernfs
kn->dev_map rwsem for the device sysfs subtree. The same rwsem is also
acquired when using the per-device sysfs attributes in the device sub-tree,
and the attribute handlers then also acquire the dcssblk_devices_sem.
This can lead to a deadlock when removing a DCSS while concurrently
reading from / writing to one of its sysfs attributes. The following

On larger installations it is useful to disable automatic LUN scanning,
and only add the required LUNs via udev rules. This can speed up bootup
dramatically.
This patch introduces a new scan module parameter value 'manual', which
works like 'none', but can be overridden by setting the 'rescan' value
from scsi_scan_target to 'SCSI_SCAN_MANUAL'. And it updates all
relevant callers to set the 'rescan' value to 'SCSI_SCAN_MANUAL' if
invoked via the 'scan' option in sysfs.

Reorder lcu and device lock to get rid of the error-prone trylock
mechanism.
The locking order is lcu lock -> device lock.
This protects against changes to the lcu device lists and enables us
to iterate over the devices, take the cdev lock and make changes to
the device structures.
The complicated part is the summary unit check handler that gets an
interrupt on one device of the lcu that leads to structural changes of
the whole lcu itself. This work needs to be done even if devices on
the lcu disappear. So a device independent worker is used.
The old approach tried to update some lcu structures and set up the
lcu worker in the interrupt context with the device lock held.
But this forced the lock order "cdev lock -> lcu lock" that made it
hard to have the lcu lock held and iterate over all devices and change
them.
The new approach is to schedule a device specific worker that gets
out of the interrupt context and rid of the device lock for summary
unit checks. This worker is able to take the lcu lock and schedule the
lcu worker that updates all devices. The time between interrupt and
worker execution is no problem because the devices in the lcu reject
all I/O in this time with an appropriate error. The dasd driver can
deal with this situation and re-drive the I/O later on.

Prepare for new format checking function by renaming functions and
moving reusable code to separate functions:
- Move sanity checks into a new function and make it reusable.
- Move common format code to a new function called
dasd_eckd_format_process_data.
- Create the generic function dasd_eckd_format_build_ccw_req, which
itself will then decide what ccw request is being built according to
the input data. (with upcoming functionality).

Currently, dasd_format is calling the format logic of a DASD discipline
with PAV enabled. If that fails with an error code of -EAGAIN the value
of retries is decremented and the discipline function is called with PAV
turned off.
The loop is supposed to try this up to 255 times until success. However,
-EAGAIN can only occur once here and therefore the loop will never reach
the 255 retries.
So, replace the unnecessarily complicated loop logic and simply try again
without PAV enabled in case of an -EAGAIN error.

SET_IND takes as a payload the _address_ of the indicators, meaning
that we have one of the rare cases where kmalloc(sizeof(&...)) is
actually correct. Let's clarify that with a comment.
The count for the ccw, however, was only correct because the
indicators are 64 bit. Let's use the correct value.

git commit 1ec2772e0c ("s390/diag: add a statistic for diagnose
calls") added function calls to gather diagnose statistics.
In case of the dasd diag driver the function call was added between a
register asm statement which initialized register r2 and the inline
assembly itself. The function call clobbers the contents of register
r2 and therefore the diag 0x250 call behaves in a more or less random
way.
Fix this by extracting the function call into a separate function like
we do everywhere else.

The correct lock order for LCU lock and cdev lock is to take the cdev
lock first and afterwards the LCU lock. This is caused by the fact
that LCU functions are called in an interrupt context with the cdev
lock implicitly hold by CIO.
To assure the right locking order but also be able to iterate over
devices in a LCU introduce a trylock block that can be called with
the device lock for one device hold and then takes the LCU lock and
try to lock all devices accounted to this LCU. Afterwards all devices
and the LCU itself are locked.

Commit ca369d51b ("sd: Fix device-imposed transfer length limits")
introduced a new queue limit max_dev_sectors which limits the maximum
sectors for requests. The default value leads to small dasd requests
and therefor to a performance drop.
Set the max_dev_sectors value to the same value as the max_hw_sectors
to use the maximum available request size for DASD devices.

The channel checks the specified length and the provided amount of
data for CCWs and provides an incorrect length error if the size does
not match. Under z/VM with simulation activated the length may get
changed. Having the suppress length indication bit set is stated as
good CCW coding practice and avoids errors under z/VM.

The tty core no longer provides nor uses ASYNC_CLOSING; remove from
tty_port_close_start() and tty_port_close_end() as well as tty drivers
which open-code these state changes. Unfortunately, even though the
bit is masked from userspace, its inclusion in a uapi header precludes
removing the macro.

Per channel path measurement characteristics are obtained during channel
path registration. However if some properties of a channel path change
we don't update the measurement characteristics.
Make sure to update the characteristics when we change the properties of
a channel path or receive a notification from FW about such a change.

Make sure that in all cases where we could not obtain measurement
characteristics the associated fields are set to invalid values.
Note: without this change the "shared" capability of a channel path
for which we could not obtain the measurement characteristics was
incorrectly displayed as 0 (not shared). We will now correctly
report "unknown" in this case.

Measurement characteristics are allocated during channel path
registration but not freed during deregistration. Fix this by
embedding these characteristics inside struct channel_path.

Currently, on card response failures a combination of card domain and
domain id is recorded in the kernel messages.
According to the message description only the card id will be recorded.
The domain id is not relevant, since the whole card including all domains
is set offline.

For the purpose of communicating the optional presence of a 'struct
page' for the pfn returned from ->direct_access(), introduce a type that
encapsulates a page-frame-number plus flags. These flags contain the
historical "page_link" encoding for a scatterlist entry, but can also
denote "device memory". Where "device memory" is a set of pfns that are
not part of the kernel's linear mapping by default, but are accessed via
the same memory controller as ram.
The motivation for this new type is large capacity persistent memory
that needs struct page entries in the 'memmap' to support 3rd party DMA
(i.e. O_DIRECT I/O with a persistent memory source/target). However,
we also need it in support of maintaining a list of mapped inodes which
need to be unmapped at driver teardown or freeze_bdev() time.

with that. I really should've asked for moratorium on lookup-related

and get conversion of i_mutex to rwsem with ->lookup() done under lock
taken shared.
There will be a patch closer to the end of the window, along the lines

checkpatch.pl wants arrays of strings declared as follows:

Currently the find_vqs() function takes a const char *names[] argument
so passing checkpatch.pl's const char * const names[] results in a
compiler error due to losing the second const.
This patch adjusts the find_vqs() prototype and updates all virtio
transports. This makes it possible for virtio_balloon.c, virtio_input.c,
virtgpu_kms.c, and virtio_rpmsg_bus.c to use the checkpatch.pl-friendly
type.

sclp_sync_wait() disables all external interrupt classes except for
the service signal subclass.
The static mask used for that however is wrong. It clears a couple of
bits which shouldn't be cleared and on the other hand potentially does
not clear bits which should be cleared.
Fix this by using the same generic mask like we do it in our delay
implementation.

Enabling failfast should let request fail immediately if either an
error occurred or the device gets disconnected.
For disconnected devices new requests are not fetches from the block
queue and therefore failfast is not triggered.
Fix by letting the DASD driver fetch requests for disconnected devices
with failfast active.

Adding tracepoints to inline functions adds tracepoint invocation code
for each instance where the function is inlined. The resulting increase
in kernel code size can have negative impact:
- More cache misses in instruction cache

to normal functions.

Replace the current semi-arbitrary distribution of inline assemblies:
- Inline assemblies used by CIO go into ioasm.h
- Data definitions used by inline assemblies go into cio.h
Beyond cleaning up the current structure this is also required for

The numeric representation of the xsch instruction was incorrectly
specified, resulting in reserved fields of the instruction opcode
potentially being set to a non-zero value. While this doesn't currently
cause any problem, a future architecture might make use of these fields
so that the current specification could result in an exception or
unwanted side-effects.
Fix this by using the xsch instruction code for which support in
binutils was added in 2003.

There is no longer a need to maintain two versions of the same inline
assembly - one with exception handling, and one without - so get rid of
the duplicates and adjust names accordingly. This applies to stsch_err
and msch_err which are now renamed to stsch and msch respectively,
while the original msch function is removed.

The common I/O layer may pass an error value as the irb in the device's
interrupt handler (for classic channel I/O). This won't happen in
current virtio-ccw implementations, but it's better to be safe than
sorry.
Let's just return the error conveyed by the irb and clear any possible
pending I/O indications.

When sending skbs, qeth determines the number of qdio SBAL elements
required. If qeth sends a fragmented skb, the SBAL element number
calculation is wrong, because the fragmented data part is added
twice in qeth_l3_tso_elements(). This patch makes sure fragmented
data is handled in qeth_elements_for_frags() only, while
qeth_l3_tso_elements() starts calculation of qdio SBAL elements just
with the linear data part of the skb.

/sys/class/net/<interface>/operstate for an active qeth network
interface offen shows "unknown", which translates to "state UNKNOWN
in output of "ip link show". It is caused by a missing initialization
of the __LINK_STATE_NOCARRIER bit in the net_device state field.
This patch adds a netif_carrier_off() invocation when creating the
net_device for a qeth device.

Provide a user space interface and an enhancement to the sclp device driver
which allows to send an 'Open for Business' event from the operating system
to the Support Element. The 'Open for Business' event is used to signal the
Support Element that the operating system (or an application running on top
of it) is up and running.

Since commit c05ffc4f2b ("[S390] sclp: sysfs interface for
SCLP cpi"), which was made 2008 the user can specify a system
and sysplex name through the /sys/firmware/cpi interface. In
addition to sysplex and system name, the user can also override
the system type and system version.
Because the syfs interface is easier to use and allows the
settings to be updated, the sclp_cpi module becomes obsolete
and can be removed.

Introduce save_area_alloc(), save_area_boot_cpu(), save_area_add_regs()
and save_area_add_vxrs to deal with storing the CPU state in case of a
system dump. Remove struct save_area and save_area_ext, and create a new
struct save_area as a local definition to arch/s390/kernel/crash_dump.c.
Copy each individual field from the hardware status area to the save area,
storing the minimum of required data.

To collect the CPU registers of the crashed system allocated a single
page with memblock_alloc_base and use it as a copy buffer. Replace the
stop-and-store-status sigp with a store-status-at-address sigp in
smp_save_dump_cpus() and smp_store_status(). In both cases the target
CPU is already stopped and store-status-at-address avoids the detour
via the absolute zero page.
For kexec simplify s390_reset_system and call store_status() before
the prefix register of the boot CPU has been set to zero. Use STPX
to store the prefix register and remove dump_prefix_page.

Replace the SAVE_AREA_BASE offset calculations in reipl.S with the
assembler constant for the location of each register status area.
Use __LC_FPREGS_SAVE_AREA instead of SAVE_AREA_BASE in the three
remaining code locations and remove the definition of SAVE_AREA_BASE.

The /sys/kernel/debug/zcore/mem interface delivers the memory of the
old system with the CPU registers stored to the assigned locations in
each prefix page.
For the vector registers the prefix page of each CPU has an address of
a 1024 byte save area at 0x11b0. But the /sys/kernel/debug/zcore/mem
interface fails copy the vector registers saved at boot of the zfcpdump
kernel into the dump image.
Copy the saved vector registers of a CPU to the outout buffer if the
memory area that is read via /sys/kernel/debug/zcore/mem intersects
with the vector register save area of this CPU.

When the AP queue depth of requests was reached additional requests
have been ignored. These request are stuck in the request queue.
The AP queue handling now push the next waiting request into the
queue after fetching a previous serviced and finished reply.

ap_bus and zcrypt_api assumed module information to always be present
and initialisation to be done in module loading order (symbol
dependencies). These assumptions don't hold if zcrypt is built-in;
THIS_MODULE will be NULL in this case and init call order is linker
order, i.e. Makefile order.
Fix initialisation order by ordering the object files in the Makefile
according to their dependencies, like the module loader would do.
Fix message type registration by using a dedicated "name" field rather
than piggy-backing on the module ("owner") information. There's no
change to the requirement that module name and msgtype name are
identical. The existing name macros are used.
We don't need any special code for dealing with the drivers being
built-in; the generic module support code already does the right
thing.
Test results:
1. CONFIG_MODULES=y, CONFIG_ZCRYPT=y
KVM: boots, no /sys/bus/ap (expected)

When zcrypt is built as a module, the module loader ensures that the
driver modules cannot be loaded if the AP bus module returns an error
during initialisation. But if zcrypt and the driver are built-in, the
driver is getting initialised even if the AP bus initialisation
failed. The driver invokes ap_driver_register() during initialisation,
which then causes operations on uninitialised data structures to be
performed.
Explicitly protect ap_driver_register() by introducing an
"initialised" flag that gets set iff the AP bus initialisation was
successful. When the AP bus initialisation failed,
ap_driver_register() will error out with -ENODEV, causing the driver
initialisation to fail as well.
Test results:
1. Inside KVM (no AP bus), zcrypt built-in
Boots. /sys/bus/ap not present (expected).
2. Inside KVM (no AP bus), zcrypt as module
Boots. Loading zcrypt_cex4 fails because loading ap_bus fails
(expected).
3. On LPAR with CEX5, zcrypt built-in
Boots. /sys/bus/ap/devices/card* present but .../card*/type missing

Simplify the css_generate_pgid code by using stap() independent of
CONFIG_SMP. For !CONFIG_SMP builds stap() will deliver the address
of the cpu we IPL'ed from (which can be != 0).
Note: the ifdef was likely added to be compatible with _very_ old
machines which we don't support anyway.

- Add the s390 backend for the software dirty bit tracking. This

Path verification is either done via dasd_eckd_read_conf() which is
triggered during online processing and resume or via
do_path_verification_work() which is triggered after path events.
The dasd_eckd_read_conf() version added paths unconditionally and did
not check if the path mask was empty. This led to devices having the
disconnected stop flag set but a valid path mask. So they where not
working although they had paths validated successfully. After a resume
this state could even not be solved with additional paths added.
Fix by checking for an empty path mask in dasd_eckd_read_conf() and
clearing the device stop bits for a newly added channel path.

For a valid PAV assignment the DASD driver needs to notice possibly
changed configuration data. Thus the failing of read configuration
data should also fail the device restore to prevent invalid PAV
assignment. The failed device may get restored after additional paths
get available later on.
If the restore fails after the device was added to the lcu alias
handling it needs to be removed from the alias handling before exiting
the restore function.

The configuration data is stored per path and also the first valid
configuration data per device. When dasd_eckd_read_conf is called
again after a path got lost the device configuration data is cleared
but possibly not the per path configuration data. This might lead to a
double free when the lost path gets operational again.
Fix by clearing all per path configuration data when the first valid
configuration data is received and stored.

Remove all the casts to and from the machine check interruption code.
This patch changes struct mci to a union, which contains an anonymous
structure with the already known bits and in addition an unsigned
long field, which contains the raw machine check interruption code.
This allows to simply assign and decoce the interruption code value
without the need for all those casts we had all the time.

A summary unit check occurs when the lcu updates the PAV configuration
e.g. base PAV assignment or PAV mode at all. This requires the reset
of the drivers internal pavgroups. Therefore the alias devices are
flushed and moved via a temporary list to the active_devices list
where they are not associated with a pavgroup. In conjunction with
updates to the base device the pavgroup may be removed since both
base_list and alias_list are empty. Unfortunately during alias flush
and move to the active_device list from alias_list the pavgroup
pointer is not deleted in the device private structure. This leads to
a list del_corruption if another lcu_update tries to move the device
in the non existent pavgroup.
Fix by removing the pavgroup pointer after the alias device was moved
to the active_devices list.

Replace the two fields 'unregistered' and 'reset' with a device
state with 5 possible values. Introduce two events for the AP devices,
device poll and device timeout. With the state machine it is easier
to deal with device initialization and suspend/resume. Device polling
is simpler as well, the arkane 'flags' passing is gone.

If a AP device is removed while messages are still pending, the requests
are cancelled by calling the message receive function with an error pointer
for the reply. The message type receive handler recognize this and create
a fake hardware error TYPE82_RSP_CODE / REP82_ERROR_MACHINE_FAILURE.
The message with the hardware error then causes a printk and a return
code of -EAGAIN.
Replace the intricate scheme with an explicit return code for this sitation
and avoid the error message.

Set the configuration timer at the end of the ap_scan_bus function.
Make use of setup_timer and remove some unnecessary add_timer, mod_timer
and del_timer_sync calls. Replace the complicated timer_pending, mod_timer
and add_timer code in ap_config_time_store with a simple mod_timer.

If there are no devices on the AP bus there will not be a single
call to the per-device ap_bus_suspend function. Even worse,
there will not be a call to the per-device ap_bus_resume either
and the AP will fail so resume correctly.
Introduce a bus specific dev_pm_ops to suspend / resume the AP
bus related things. While we are at it, simplify the power management
code of the AP bus.

The ap_queue_messsage function will call device_unregister if the
unregistered field of the device has been set while trying to queue
a message. This races with other device_unregister calls, e.g. from
the ap_scan_bus. Remove the call to device_unregister from
ap_queue_message and let ap_scan_bus deal with it.

The ap_query_configuration function allocates the ap_config_info
structure, but there is no code to free the structure.
Allocate the structure in the module_init function and free it
again in module_exit.
While we are at it simplify a few functions in regard to the
ap configuration data.

ap_test_queue, ap_query_facilities, __ap_query_functions all use
the same PQAP(TAPQ) command. Consolidate the three into a single
ap_test_queue function that returns the AP status and the 64-bit
result. The exception table entry for PQAP(TAPQ) can be avoided
if the T bit for the APFT facility is set only if test_facility(15)
indicated that the facility is present.
Integrate ap_query_function into ap_query queue to avoid calling
PQAP(TAPQ) twice.

The sclp console and tty code currently uses several message text
objects in a single message event to print several lines with one

During resume from hibernate we already reenable measurement block

Extended measurement blocks need to be 64 byte aligned. To achieve that
128 bytes for each measurement block are allocated and an align callback
returns a 64 byte aligned address inside this area.
Replace this code with kmem_cache allocations.

During allocation of extended measurement blocks we check if the device is
already active for channel measurement and add the device to a list of
devices with active channel measurement. The check is done under ccwlock
protection and the list modification is guarded by a different lock.
To guarantee that both states are in sync make sure that both locks
are held during the allocation process (like it's already done for the
"normal" measurement block allocation).

Devices with active channel measurement are included in a list. When a
device is removed without deactivating channel measurement first the
list_head is freed but still used. Fix this by making sure that
channel measurement is deactivated during device deregistration.
For devices that we deregister because they are no longer accessible
deactivating channel measurement will fail. In this case we can report
success because the FW will no longer access the measurement block.
In addition to these steps keep an extra device reference while
channel measurement is active.

We were able to reduce the CPU overhead of big paging scenarios
when announcing our paging disks as non-rotational.
Almost all dasd devices are implemented in storage servers with
cache, raid, striping and lots of magic. There is no point in
optimizing the disk schedulers and swap code for a single platter
moving arm rotational disks. Given the complexity of the setup
and the fact that this change is mostly to disable the additional
overhead in swap code, lets keep the other functionality unchanged
and do not disable the this device as entropy source - unlike other
non-rotational devices.

In the past only even modulus sizes were allowed for RSA keys in
CRT format. This restriction was based on limited RSA key generation
on older crypto adapters that provides only even modulus sizes. This
restriction is not valid any more.
Revoke restrictions that crypto requests can be serviced with odd
RSA modulus length in CRT format.

In layer2 mode of the qeth driver, MAC address lists
from struct net_device require mapping to the OSA-card.
The existing implementation is inefficient for lists with
more than several MAC addresses, since for every
ndo_set_rx_mode callback it removes all MAC addresses first,
and then registers the current MAC address list.
This patch changes implementation of ndo_set_rx_mode callback
in qeth, only performing hardware registration/removal for
new/deleted addresses. To shorten lookup of MAC addresses
registered addresses are kept in a hashtable instead of a
linear list.

The iucv code uses arrays as arguments. Even though this does not
really cause a problem, it could be misleading, since the compiler
turns array arguments into just a pointer argument. To be more

Checksum offloading for send and receive is already
supported for layer 3 (IP layer). This patch
adds support for RX and TX hardware checksum offloading
for layer 2 (MAC layer). The hardware calculates the checksum
for IP UDP and TCP packets.
This patch moves the hardware checksum offloading setup
to the set of common functions in qeth_core_main.c.
Layer 2 and layer 3 now simply call the same common functions.
Also note that TX checksum offloading is always enabled.
The device driver relies on the TCP/IP stack to make use of
this feature.

1/ Introduce ZONE_DEVICE and devm_memremap_pages() as a generic
mechanism for adding device-driver-discovered memory regions to the
kernel's direct map. This facility is used by the pmem driver to
enable pfn_to_page() operations on the page frames returned by DAX
('direct_access' in 'struct block_device_operations'). For now, the
'memmap' allocation for these "device" pages comes from "System
RAM". Support for allocating the memmap from device memory will
arrive in a later kernel.
2/ Introduce memremap() to replace usages of ioremap_cache() and
ioremap_wt(). memremap() drops the __iomem annotation for these
mappings to memory that do not have i/o side effects. The
replacement of ioremap_cache() with memremap() is limited to the

Implement magic sysrequest handling for the VT220 terminal (also known as
the Integrated ASCII console on the HMC/SE).
To invoke a "magic sysrequest" function, press "Ctrl+o" followed by a
second character that designates the debugging function.
The handling of the sysrq is scheduled away from the SCLP IRQ context;
because large amounts of sysrq output might fill up the console buffers.
The console might deadlock because it cannot empty the buffers while still
in the receiving IRQ context. This behavior is the same as for the SCLP
console.

function that can be directly used by other users.
Introduce a new sysrq_work structure to embed the work_struct and to
specify the magic sysrq function to be invoked.

If a 3270 terminal is disconnected and later reconnected again,
it gets an unsolicited device end. This is currently ignored and
you have to hit the clear key to get the screen redrawn.
Add an automatic full redraw of the screen for this case.

Update the annotation for the kaddr pointer returned by direct_access()
so that it is a __pmem pointer. This is consistent with the PMEM driver
and with how this direct_access() pointer is used in the DAX code.

There is no need to busy loop and monopolize a cpu for up to ~2 seconds.
The code in question that calls mdelay() is preemptible anyway, so better
let the kernel schedule different processes than just looping and causing
unnecessary delays.

The z/VM driver sets bit "63-22" in control register zero to one in order
to enable the CP Service interrupt (0x2603). However the irq subclass mask
that normally corresponds to the CP Service interrupt is
"63-54" (== "63-22-32").
So it looks like the author read the documentation with the 32 bit sized
cr0 register bit positions (== 22), but didn't realize that bit numbers
change, if applied to a 64 bit register (== 54) due to the numbering
scheme.
Also use irq_subclass_register() instead if ctl_set_bit() since multiple
services depend on the service signal subclass mask, which is the correct
bit. This also explains why nobody noticed the bug, since the bit is always
enabled anyway (e.g. pfault).

The way the block layer is currently written, it goes to great lengths
to avoid having to split bios; upper layer code (such as bio_add_page())
checks what the underlying device can handle and tries to always create
bios that don't need to be split.
But this approach becomes unwieldy and eventually breaks down with
stacked devices and devices with dynamic limits, and it adds a lot of
complexity. If the block layer could split bios as needed, we could
eliminate a lot of complexity elsewhere - particularly in stacked
drivers. Code that creates bios can then create whatever size bios are
convenient, and more importantly stacked drivers don't have to deal with
both their own bio size limitations and the limitations of the
(potentially multiple) devices underneath them. In the future this will
let us delete merge_bvec_fn and a bunch of other code.
We do this by adding calls to blk_queue_split() to the various
make_request functions that need it - a few can already handle arbitrary
size bios. Note that we add the call _after_ any call to
blk_queue_bounce(); this means that blk_queue_split() and
blk_recalc_rq_segments() don't need to be concerned with bouncing
affecting segment merging.
Some make_request_fn() callbacks were simple enough to audit and verify
they don't need blk_queue_split() calls. The skipped ones are:

This patch adds an enhanced detection for control unit initiated
reconfiguration request scope.
The first approach assumed the scope of the reconfiguration request
to be restricted to the path on which the message was received.
The enhanced approach determines the full scope of the reconfiguration
request by evaluating additional path and device selection information
contained in the reconfiguration message.

DASD path verification requires the usage of sleep_on_immediatly to
ensure that no other I/O request is blocking the recovery of
disconnected devices. But two concurrent path verification workers for
the same device may kill each others requests due to the usage of the
immediate sleep_on function. This may lead to unsuccessful path
verifications.
Prevent that two parallel path verification workers conflict with
each other by implementing a device flag signalling a already running
worker.

NUMA emulation (aka fake NUMA) distributes the available memory to nodes
without using real topology information about the physical memory of the
machine.
Splitting the system memory into nodes replicates the memory management
structures for each node. Particularly each node has its own "mm locks"
and its own "kswapd" task.
For large systems, under certain conditions, this results in improved
system performance and/or latency based on reduced pressure on the mm
locks and the kswapd tasks.
NUMA emulation distributes CPUs to nodes while respecting the original
machine topology information. This is done by trying to avoid to separate
CPUs which reside on the same book or even on the same MC. Because the
current Linux scheduler code requires a stable cpu to node mapping, cores
are pinned to nodes when the first CPU thread is set online.
This patch is based on the initial implementation from Philipp Hachtmann.

Currently we have two different ways to signal an I/O error on a BIO:
(1) by clearing the BIO_UPTODATE flag
(2) by returning a Linux errno value to the bi_end_io callback
The first one has the drawback of only communicating a single possible
error (-EIO), and the second one has the drawback of not beeing persistent
when bios are queued up, and are not passed along from child to parent
bio in the ever more popular chaining scenario. Having both mechanisms
available has the additional drawback of utterly confusing driver authors
and introducing bugs where various I/O submitters only deal with one of
them, and the others have to add boilerplate code to deal with both kinds
of error returns.
So add a new bi_error field to store an errno value directly in struct
bio and remove the existing mechanisms to clean all this up.

A hardware problem on a FICON link is reported by the Channel Subsystem
to the operating system via a Link Incident Record (LIR). In response,
the operating system should issue a message that enables hardware
service personnel to identify and repair the failing component.
Current Linux LIR handling is broken because LIR data is incorrectly
interpreted and no log message is generated.
This patch fixes Linux LIR handling by implementing a new log message
for LIRs indicating a degraded or non-operational link. Also LIRs are
no longer used to deactivate channel paths because the available data
does not reliably allow to determine the affected channel path.

The dasd device driver selects which (alias or base) device is used
for a given requests when the request is build. If the chosen alias
device is set offline before the request gets queued to the device
queue the starting function may use device structures that are
already freed. This might lead to a hanging offline process or a
kernel panic.
Add a check to the starting function that returns the request to the
upper layer if the device is already in offline processing.
In addition to that prevent that an alias device that's already in
offline processing gets chosen as start device.

Eric noticed problems with vhost-scsi and virtio-ccw: vhost-scsi
complained about overwriting values in the config space, which
was triggered by a broken implementation of virtio-ccw's config
get/set routines. It was probably sheer luck that we did not hit
this before.
When writing a value to the config space, the WRITE_CONF ccw will
always write from the beginning of the config space up to and
including the value to be set. If the config space up to the value
has not yet been retrieved from the device, however, we'll end up
overwriting values. Keep track of the known config space and update
if needed to avoid this.
Moreover, READ_CONF will only read the number of bytes it has been
instructed to retrieve, so we must not copy more than that to the
buffer, or we might overwrite trailing values.

Set the 'quality' property in the zcrypt rng device structure to enable the
zcrypt hwrng device to take part in the kernel entropy seeding process.
A module parameter named hwrng_seed will be introduced to disable the

It turned out that SIGP set-multi-threading can only be done once.
Therefore switching to a different MT level after switching to
sclp.mtid_prev in the dump case fails.
As a symptom specifying the "nosmt" parameter currently fails for
the kdump kernel and the kernel starts with multi-threading enabled.
So fix this and issue diag 308 subcode 1 call after collecting the
CPU states for the dump. Also enhance the diag308_reset() function to
be usable also with enabled lowcore protection and prefix register != 0.
After the reset it is possible to switch the MT level again. We have
to do the reset very early in order not to kill the already initialized
console. Therefore instead of kmalloc() the corresponding memblock
functions have to be used. To avoid copying the sclp cpu code into
sclp_early, we now use the simple sigp loop method for CPU detection.

The SCLP interface to query, configure and deconfigure CPUs actually
operates on cores. For a machine without the multi-threading faciltiy
a CPU and a core are equivalent but starting with System z13 a core
can have multiple hardware threads, also referred to as logical CPUs.
To avoid confusion replace the word 'cpu' with 'core' in the SCLP
interface. Also replace MAX_CPU_ADDRESS with SCLP_MAX_CORES.
The core-id is an 8-bit field, the maximum thread id is in the range
0-31. The theoretical limit for the CPU address is therefore 8191.

In case of request timeouts an AP queue reset will be triggered to
recover and reinitialize the AP queue. The previous behavior was an
immediate reset execution regardless of current/pending requests.
Due to newly changed firmware behavior the reset may be delayed, based
on the priority of pending request. The device driver's waiting time
frame was limited, hence it did not received the reset response. As a
consequence interrupts would not be enabled afterwards.
The RAPQ (queue reset) and AQIC (interrupt control) commands will be
treated fully asynchronous now. The device driver will check the reset and
interrupt states periodically, thus it can handle the reinitialization
properly.

schedule_timeout takes a timeout in jiffies but the code currently is
passing in a constant SDIAS_SLEEP_TICKS, as this is otherwise unused
it is replaced by 500ms as recommended by Michael Holzheu
<holzheu@linux.vnet.ibm.com> and converted with msecs_to_jiffies();
As SDIAS_SLEEP_TICKS was only used in this one location it is dropped.
Patch was compile tested with (s390) defconfig +
CONFIG_MARCH_Z10=y, (implies CONFIG_CRASH_DUMP=y)

An attempt to configure a CTC device as LCS results in the
following error message:
(null): Detecting a network adapter for LCS devices failed
with rc=-5 (0xfffffffb)
"(null)" results from access to &card->dev->dev in lcs_new_device()
which is only initialized later in the function. Fix this by using
&ccwgdev->dev instead which is initialized before lcs_new_device()
is called.

OSA Ethernet hardware is introducing BRIDGEPORT functionality
similar (but not identical) to HiperSockets BRIDGEPORT. This
patch makes HiperSockets BRIDGEPORT related sysfs attributes

OSA and HiperSocket devices do not support promiscuous mode proper,
but they support "BRIDGE PORT" mode that is functionally similar.
This update introduces sysfs attribute that, when set, makes the driver
try to "reflect" setting and resetting of the IFF_PROMISC flag on the
interface into setting and resetting PRIMARY or SECONDARY bridge port
role on the underlying OSA or HiperSocket device.

ethtool is used to change some device driver features
such as RX/TX hardware checksum offloading.
The qeth device driver callback function to
turn on/off RX hardware check sum handling never changes
the hardware configuration.
The NETIF_F_RXCSUM bit is cleared when the feature bitset
type netdev_features_t(64bit) is assigned to 32 a bit
variable.
This patch fixes the NETIF_F_RXCSUM handling.
Also there is no need to manipulate the device's features
bit set as this is done by the caller when no error occurs.

Enabling a DASD that was configured to use the DIAG250 access method
while the corresponding kernel module dasd_diag_mod has not been loaded
fails with an error message. To fix this, users need to manually load
the dasd_diag_mod module.
This procedure can be simplified by automatically loading the
dasd_diag_mod from within the kernel when a DASD configured for DIAG250
is set online.

Let's unify basic access to sclp fields by storing the data in an external
struct in asm/sclp.h.
The values can now directly be accessed by other components, so there is
no need for most accessor functions and external variables anymore.
The mtid, mtid_max and facility part will be cleaned up separately.

The DASD device driver prevents I/O from being started on stopped
devices. This also prevented channel paths to be verified and so
the device was unable to be resumed.
Fix by allowing path verification requests on stopped devices.

The DASD device driver only has a limited amount of memory to build
I/O requests.
This memory was used by blocklayer requests leading to an inability
to build needed internal requests to resume the device.
Fix by preventing the DASD driver to fetch requests for a stopped
device.

Fix ref counting for DASD devices leading to an inability to set a
DASD device offline.
Before a worker is scheduled the DASD device driver takes a reference
to the device. If the worker was already scheduled this reference was
never freed.
Fix by giving the reference to the DASD device free when
schedule_work() returns false.

Commit 27356f54c8 ("mm/hotplug: verify hotplug memory range")
introduced a check that makes add_memory() only accept section size
aligned memory.
Therefore on z/VM systems, where standby memory is not aligned, no
standby memory is registered at all.
Example:

For this setup the following error message is printed:
Section-unaligned hotplug range: start 0xdb000000, size 0x92000000
So fix this and register aligned memory in "sclp_cmd.c". This means
that for the corner cases where the standby memory is not aligned we
loose some memory.
In order to inform the user about the potential loss of standby memory,
we add a new message for each added standby block and print how
much of the standby memory is usable, for example:
sclp_cmd.4336b4: Standby memory at 0x50000000 (256M of 256M usable)
sclp_cmd.4336b4: Standby memory at 0xb0000000 (256M of 256M usable)
sclp_cmd.4336b4: Standby memory at 0xdb000000 (2048M of 2336M usable)
We also ensure that a potential memory block that contains both "assigned"
and "standby" memory cannot be setup offline.

Allocating a large number of elements in atomic context could quickly
deplete memory reserves, so just disallow atomic resizing entirely.
Nothing currently uses mempool_resize() with anything other than
GFP_KERNEL, so convert existing callers to drop the gfp_mask.

Remove the 31 bit support in order to reduce maintenance cost and
effectively remove dead code. Since a couple of years there is no
distribution left that comes with a 31 bit kernel.
The 31 bit kernel also has been broken since more than a year before
anybody noticed. In addition I added a removal warning to the kernel
shown at ipl for 5 minutes: a960062e58 ("s390: add 31 bit warning
message") which let everybody know about the plan to remove 31 bit
code. We didn't get any response.
Given that the last 31 bit only machine was introduced in 1999 let's
remove the code.
Anybody with 31 bit user space code can still use the compat mode.

We increase the msb_count after we're finished building the request.
That way we can always access the current request via
scmrq->request[msb_count] . But once the request is started we need
to make sure that the array index stays below msb_count.

Older machines with more then 16 domains need a special check before
PQAP instructions can be processed. With commit 5bc334bff9 this
check was reverted by accident. This patch re-establishes the additional
code needed for checking the extended domains for older machines.

The dasd driver has a lot of duplicated code to handle
dasd_global_profile. With this patch we use the same code for the
global and the per device profiling data. Note that dasd_stats_write
had to change slightly to maintain some odd differences between
A) per device and global profile and B) proc and sysfs interface
usage.

The device driver has plenty of command buffers available
per channel for channel command communication.
In the extremely rare case when there is no command buffer
available, return a NULL pointer and issue a warning
in the kernel log. The caller handles the case when
a NULL pointer is encountered and returns an error.
In the case the wait for command buffer is possible
(because no lock is held as in the OSN case), still wait
until a channel command buffer is available.

In the functions that are registering and unregistering MAC
addresses in the qeth-handled hardware, remove callback functions
that are unnesessary, as only the return code is analyzed.

Upcoming versions of secure key management facilities (CCA and
EP11) require information about the maximum number of supported
ap domains in order to service TKE requests properly. With IBM
z13 the number of available domains (so far 16) has increased up
to 85. This number varies depending on machine types and models.
Therefore the new sysfs attribute 'ap_max_domain_id' provides
this limit of supported ap domains. Upcoming releases for CCA
and EP11 will use this new information. Without this problem fix
it is not possible to retrieve reliable information about the
maximum number of supported ap domains. Thus, customers are not
able to perform key management for CCA and EP11 coprocessor
adapters.

The unit check handler for 3480 / 3490 tapes used to print a different
warning message for erpa code 0x57 dependent on the device type.
The warning messages have been remove in a cleanup, the if statement
is a left over. Remove it.

The content of a DCSS of type SN or EN cannot be saved. Issue a warning when
trying to save such a DCSS. Depending on the setup, this may be a user error
or intended behaviour e.g. with a multi-DCSS device.

The multi-threading facility is introduced with the z13 processor family.
This patch adds code to detect the multi-threading facility. With the
facility enabled each core will surface multiple hardware threads to the
system. Each hardware threads looks like a normal CPU to the operating
system with all its registers and properties.
The SCLP interface reports the SMT topology indirectly via the maximum
thread id. Each reported CPU in the result of a read-scp-information
is a core representing a number of hardware threads.
To reflect the reduced CPU capacity if two hardware threads run on a
single core the MT utilization counter set is used to normalize the
raw cputime obtained by the CPU timer deltas. This scaled cputime is
reported via the taskstats interface. The normal /proc/stat numbers
are based on the raw cputime and are not affected by the normalization.

This patch is concerned with migrating the time variables for the s390
network drivers. The changes handle the y2038 problem where timespec will
overflow in the year 2038. timespec was replaced by unsigned long and
all time variables get their values from the jiffies global variable.
This was done for the sake of speed and efficiency.

In order to support accesses to larger chunks of memory, pass in a
'size' parameter (counted in bytes), and return the amount available at
that address.
Add a new helper function, bdev_direct_access(), to handle common
functionality including partition handling, checking the length requested
is positive, checking for the sector being page-aligned, and checking
the length of the request does not pass the end of the partition.

Kernel oops caused by invalid parameter at TAPQ instruction:
On older systems where the QCI instruction is not available
all possible domains are probed via TAPQ instruction. The
range for the probe has been extended with the > 16 domain
support now leading to a possible specification exception
when this instruction is called for probing higher values
within the new range. This may happen during insmod and/or
ap bus reset only on machines without a QCI instruction (z10,
z196, z114), zEC12 and newer systems are not affected.
The fix modifies the domain checking function to limit the
allowed range if no QCI info is available.

the wireless maintainers as they relied on the debugfs changes.
Everything has been in linux-next for a while.

What does it mean if rev 1 device does not set
VIRTIO_F_VERSION_1? E.g. is it native endian?

Legacy balloon device doesn't pretend to support revision 1 or 64 bit
features.
But just in case someone implements a broken one that does, let's not
even try to drive legacy only devices using revision 1, and let's not
give them a chance to say they support VIRTIO_F_VERSION_1 by not reading
or writing high feature bits.

The CCW_CMD_SET_VQ command has a different format for revision 1+
devices, allowing to specify a more complex virtqueue layout. For
now, we stay however with the old layout and simply use the new
command format for virtio-1 devices.

It seemed like a good idea to use bitmap for features
in struct virtio_device, but it's actually a pain,
and seems to become even more painful when we get more
than 32 feature bits. Just change it to a u32 for now.

Handle up to 8 block layer requests per HW request. These requests
can be processed in parallel on the device leading to better
throughput (and less interrupts). The overhead for additional
requests is small since we don't blindly allocate new aidaws but
try to use what's left of the previous one.

AOBs (the structure describing the HW request) need to be 4K
aligned but very little of that page is actually used. With
this patch we place aidaws at the end of the AOB page and only
allocate a separate page for aidaws when we have to (lists of
aidaws must not cross page boundaries).

We currently use one preallocated page per HW request to store
aidaws. With this patch we use mempool to allocate an aidaw page
whenever we need it.

Fix race for sleep_on requests leading to list corruption.
The SLEEP_ON_END_TAG is set during CQR clean up. Remove it from
interrupt handler to avoid the CQR from being cleared when it is
still in the device_queue.

During device activation all paths could be lost and since the device
is not active it has no indication of this fact - hence the CQR will
time-out. The following cancelation might fail with -EINVAL because
CIO took over control and started path verification. In this case mark
the CQR as being CLEARED since it could not be running any more.

All drivers use the implementation for ramping the queue up and down, so
instead of overloading the change_queue_depth method call the
implementation diretly if the driver opts into it by setting the
track_queue_depth flag in the host template.
Note that a few drivers validated the new queue depth in their
change_queue_depth method, but as we never go over the queue depth
set during slave_configure or the sysfs file this isn't nessecary
and can safely be removed.

This patch improves the Fibre Channel port scan behaviour of the zfcp lldd.
Without it the zfcp device driver may churn up the storage area network by
excessive scanning and scan bursts, particularly in big virtual server
environments, potentially resulting in interference of virtual servers and
reduced availability of storage connectivity.
The two main issues as to the zfcp device drivers automatic port scan in
virtual server environments are frequency and simultaneity.
On the one hand, there is no point in allowing lots of ports scans
in a row. It makes sense, though, to make sure that a scan is conducted
eventually if there has been any indication for potential SAN changes.
On the other hand, lots of virtual servers receiving the same indication
for a SAN change had better not attempt to conduct a scan instantly,
that is, at the same time.
Hence this patch has a two-fold approach for better port scanning:
the introduction of a rate limit to amend frequency issues, and the
introduction of a short random backoff to amend simultaneity issues.
Both approaches boil down to deferred port scans, with delays
comprising parts for both approaches.
The new port scan behaviour is summarised best by:

Implementation is straight-forward by converting an existing worker to
a delayed worker. But care is needed whenever that worker is going to be
flushed (in order to make sure work has been completed), since a flush
operation cancels the timer set up for deferred execution (see * above).
There is a small race window whenever a port scan work starts
running up to the point in time of storing the time stamp for that port
scan. The impact is negligible. Closing that gap isn't trivial, though, and
would the destroy the beauty of a simple work-to-delayed-work conversion.

Through sysfs attributes, zfcp unit objects
provide a trigger for manual LUN recovery
and export information for problem determination.
With commit

"[SCSI] zfcp: Allow midlayer to scan for LUNs when running in NPIV mode"
and when attaching SCSI devices through this new optional method,
no more zfcp unit objects are allocated for such SCSI devices.
Hence, the above-mentioned trigger and information were missing.

Hence, introduce the trigger and the information unconditionally
for all SCSI devices attached through zfcp.
We prefix the attribute names with 'zfcp_' to prevent collisions and
to avoid mix-ups such as with the common 'state' attribute.
Since some of the new attribute views do not need zfcp_port
in the ZFCP_DEFINE_SCSI_ATTR helper macro, remove zfcp_port
to avoid compiler warnings on unused variable.
It's easy to open code the conversion from zfcp_scsi_dev to zfcp_port
for the two already existing attributes hba_id and wwpn.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Unless we want to build a SPI tag message we should just check SCMD_TAGGED
instead of reverse engineering a tag type through the use of
scsi_populate_tag_msg.
Also rename the function to spi_populate_tag_msg, make it behave like the
other spi message helpers, and move it to the spi transport class.

This patch fixes trace formatting issues using the
QETH_CARD_TEXT_ macro. The total size of each trace entry
is 8 bytes. Some of the sprintf formats exceed these 8
bytes (for example using abcd:%d and the converted value
needs more than 3 bytes). The solution is to shorten the
text prepending the value or use a different format (%x).

Pull percpu consistent-ops changes from Tejun Heo:
"Way back, before the current percpu allocator was implemented, static
and dynamic percpu memory areas were allocated and handled separately
and had their own accessors. The distinction has been gone for many
years now; however, the now duplicate two sets of accessors remained
with the pointer based ones - this_cpu_*() - evolving various other
operations over time. During the process, we also accumulated other
inconsistent operations.
This pull request contains Christoph's patches to clean up the
duplicate accessor situation. __get_cpu_var() uses are replaced with
with this_cpu_ptr() and __this_cpu_ptr() with raw_cpu_ptr().
Unfortunately, the former sometimes is tricky thanks to C being a bit
messy with the distinction between lvalues and pointers, which led to
a rather ugly solution for cpumask_var_t involving the introduction of
this_cpu_cpumask_var_ptr().
This converts most of the uses but not all. Christoph will follow up
with the remaining conversions in this merge window and hopefully
remove the obsolete accessors"

Replace duplicated code in all transports with a single wrapper in
virtio.c.
The only functional change is in virtio_mmio.c: if a buggy device sends
us an interrupt before driver is set, we previously returned IRQ_NONE,
now we return IRQ_HANDLED.
As this must not happen in practice, this does not look like a big deal.
