Convert magic values 1 and -1 to NETDEV_TX_BUSY and NETDEV_TX_LOCKED respectively.
0 (NETDEV_TX_OK) is not changed to keep the noise down, except in very few cases
where its in direct proximity to one of the other values.

Don't access the block layer request, get the payload length instead
from the FC job. Simplify access to the zfcp_port, only the d_id is
required, if the port is no longer accessed later. This is possible
when the els_handler does not access the port pointer from the ELS
request.

If a DASD requests is started with dasd_sleep_on and fails, then the
calling function may need to know the reason for the failure.
In cases of hardware errors it can inspect the sense data in the irb,
but when the reason is internal (e.g. start_IO failed) then it needs
a meaningfull return code.

Some functions called as a late_initcall depend on completely
initialized devices. Since commit
f3445a1a65 the dasd driver uses the
new async framework and relies on the fact that synchronization is
done in prepare_namespace which is called after the late_initcalls.
Fix this by calling async_synchronize_full at the end of the related
init functions.

Lockdep found a problem with the lock order of the view lock and the
ccw device lock. raw3270_activate_view/raw3270_deactivate_view first
take the ccw device lock then call the activate/deactivate functions
of the view which take view lock. The update functions of the
con3270/tty3270 view will first take the view lock, then take the
ccw device lock. To fix this the activate/deactivate functions are
changed to avoid taking the view lock by moving the functions calls
that modify the 3270 output buffer to the update function which is
called by a timer.

The tty3270_notifier that calls tty_register_device / tty_unregister_device
is harmful in two ways:
1) the device node that is create is wrong because the minor numbers for
3270 tty start with 1 and tty_notifier passes the minor as index.
2) If 1) is corrected you'll get a warning:

The 227:1 link is already created by raw3270_create_attributes to refer

This patch converts unicast address list to standard list_head using
previously introduced struct netdev_hw_addr. It also relaxes the
locking. Original spinlock (still used for multicast addresses) is not
needed and is no longer used for a protection of this list. All
reading and writing takes place under rtnl (with no changes).
I also removed a possibility to specify the length of the address
while adding or deleting unicast address. It's always dev->addr_len.
The convertion touched especially e1000 and ixgbe codes when the
change is not so trivial.

Keep the information about the device and model id in zfcp_ccw. This
requires an additional helper function to check for the privileged
cfdc subchannel, but it allows the removal of the redundant defines
from the zfcp_def header file.

If the destination ID (D_ID) of a remote storage port changed, e.g.
re-plugged cable on the switch in a different switch port, the port
was never (re-)attached within Linux. This patch fixes the broken
mapping between the WWPN and the D_ID.

In rare cases, open port request might timeout, erp calls
zfcp_port_put, port gets dequeued. Now, the late returning (or
dismissed) fsf-port-open calls the fsf_port_open_handler that tries to
reference the port data structure leading to a kernel oops.

enum dma_data_direction only has the 4 values DMA_BIDIRECTIONAL,
DMA_TO_DEVICE, DMA_FROM_DEVICE and DMA_NONE. No need to have the
default case. While changing this, setup sbtype in one place to make
sparse happy.
The default value of retval is already -EIO, so remove the
additional assignment for these two cases.

zfcp did always set the queue_depth for SCSI devices to 32, not
allowing to change this. Introduce a kernel parameter zfcp.queue_depth
and the change_queue_depth callback to allow changing the queue_depth
when it is required.

Until now we have had a 1:1 mapping between storage device physical
block size and the logical block sized used when addressing the device.
With SATA 4KB drives coming out that will no longer be the case. The
sector size will be 4KB but the logical block size will remain
512-bytes. Hence we need to distinguish between the physical block size
and the logical ditto.
This patch renames hardsect_size to logical_block_size.

Function qeth_clear_output_buffer for HiperSockets may not clear
all 16 SBALEs, but only the used ones. The error flag in SBALF15

For HiperSocket devices receive-path checksumming is not required.
Thus NO_CHECKSUMMING is used as default for HiperSocket interfaces.
For layer3 devices configured with NO_CHECKSUMMING received skbs
should have set their ip_summed field to CHECKSUM_UNNECESSARY.

z/VM Virtual Switch Port Isolation allows guests on a VLAN UNAWARE
virtual switch to be isolated from other guests on the VSWITCH.
(See z/VM Apars VM64281 and VM64463).
The Linux qeth driver is affected, because it has to handle new
error codes introduced with the z/VM VSWITCH Port Isolation support.

If a qeth device is plugged off, setting the device online stops in
state HARDSETUP and a failure is reported to the base cio-layer
causing halt/clear to be invoked. Replugging the device again triggers
a qeth recovery without notification of the cio-layer. If a device
is ungrouped in this state, the qeth set_offline function is not
invoked, because the corresponding ccwgroup device is not in state
ONLINE. Then incoming traffic is still handled by the qdio layer
resulting in a crash in qeth_l<x>_qdio_input_handler, because (part
of) the qeth data structures for this device are already removed.
Solution: After replugging the device qeth recovery should lead to a
working net device. Thus a "LAN offline" result when setting a qeth
device online must not report a failure to the base cio-layer.

Till now block layer allowed two separate modes of request execution.
A request is always acquired from the request queue via
elv_next_request(). After that, drivers are free to either dequeue it
or process it without dequeueing. Dequeue allows elv_next_request()
to return the next request so that multiple requests can be in flight.
Executing requests without dequeueing has its merits mostly in
allowing drivers for simpler devices which can't do sg to deal with
segments only without considering request boundary. However, the
benefit this brings is dubious and declining while the cost of the API
ambiguity is increasing. Segment based drivers are usually for very
old or limited devices and as converting to dequeueing model isn't
difficult, it doesn't justify the API overhead it puts on block layer
and its more modern users.
Previous patches converted all block low level drivers to dequeueing
model. This patch completes the API transition by...
* renaming elv_next_request() to blk_peek_request()
* renaming blkdev_dequeue_request() to blk_start_request()
* adding blk_fetch_request() which is combination of peek and start
* disallowing completion of queued (not started) requests
* applying new API to all LLDs
Renamings are for consistency and to break out of tree code so that
it's apparent that out of tree drivers need updating.

With recent cleanups, there is no place where low level driver
directly manipulates request fields. This means that the 'hard'
request fields always equal the !hard fields. Convert all
rq->sectors, nr_sectors and current_nr_sectors references to
accessors.
While at it, drop superflous blk_rq_pos() < 0 test in swim.c.

There are many [__]blk_end_request() call sites which call it with
full request length and expect full completion. Many of them ensure
that the request actually completes by doing BUG_ON() the return
value, which is awkward and error-prone.
This patch adds [__]blk_end_request_all() which takes @rq and @error
and fully completes the request. BUG_ON() is added to to ensure that
this actually happens.
Most conversions are simple but there are a few noteworthy ones.
* cdrom/viocd: viocd_end_request() replaced with direct calls to
__blk_end_request_all().
* s390/block/dasd: dasd_end_request() replaced with direct calls to
__blk_end_request_all().
* s390/char/tape_block: tapeblock_end_request() replaced with direct
calls to blk_end_request_all().

The zfcp_port might have been removed, while the FC fast_io_fail timer
is still running and could trigger the terminate_rport_io callback.
Set the pointer to the zfcp_port to NULL and check accordingly
before using it.

If this problem appears zfcp ports cannot be de-queued since it is
checked for a zero refcount. The port reference counting is wrong for
existing zfcp ports when e.g. an adapter gets on-line again. During
port scanning the reference counting for existing ports should not be
changed.

The current sbal counting can be wrong if a fsf request is
waiting for free sbals and at the same time qdio request queue
is shutdown and re-opened. Revering a previous patch fixes this
issue.

When the abort handler cannot find a pending FSF request, the request
completion could just be running. This means we cannot return SUCCESS,
since this would lead to call to scsi_done after exiting the SCSI
error handler which is not allowed.

A remote port remains in error state even if we receive a RSCN
stating that the connection is re-established. The port recovery
is not started due to a flag which is not reset.
The solution is to clear the flag in question before we trigger a ERP.

The remote port remains in error state even if the connection
is re-established. A wrong precondition check was performed on
the port status leading to a cancellation of the port reopen.
Remove the pre-req check because it's not required and better
handled within the ERP.

The ERP thread is performing a task before it is executing the
corresponding down on the semaphore. The response handler of the
just started exchange config should wait for the completion by
performing a down on this semaphore. Since this semaphore is still
positive from the ERP enqueue the handler won't wait and therefore
the exchange config will always fail leaving the adapter in error.
The problem can be solved by performing the down on the semaphore
before starting an ERP task. This is the logically correct order.
Only walk the ERP loop if there is a task to perform.

The nameserver port might be in state online when the adapter is
offlined. On adapter reactivation the nameserver port is not
re-opened due to the PORT_ONLINE status. This results in an
unsuccessful recovery. In forcing the nameserver port status
to offline on all adapter offline events this issue is prevented.
Waiting for the reference count to drop to zero in
zfcp_wka_port_offline is not required, so remove it.

When running the scsi_scan from the zfcp workqueue and the target
device does not respond, the zfcp workqueue can block until the
scsi_scan hits a timeout. Move the work to the scsi host workqueue,
since this one is also used for the scan from the SCSI midlayer.

A ccw command that reads or writes several records at once will
usually transfer more data then fits into one page and needs to
address memory areas using a list of indirect data address words
(idaw). All but the first of these areas must start on a 4KB or 2KB
block boundary (depending on the idaw format).
A check for this restriction was missing and has been added with
this patch.

The dasd driver can automatically online detected dasds, which
especially important for finding the root device. Currently,
it will wait for each online operation to finish individually,
which may take long if many dasds need to be onlined. When using
the new async framework, these onlining operations can run in
parallel and presence of the root device is ensured by the fact
that prepare_namespace() waits for all async threads to finish.

The QDIO ccw devices are started by ccw_device_start so no timeout
can occur for the interrupt handler. Remove the dead code.
In case of an I/O error set the device state to error and wake up
a possibly running qdio_shutdown waiter.

It is a fairly common operation to have a pointer to a work and to need a
pointer to the delayed work it is contained in. In particular, all
delayed works which want to rearm themselves will have to do that. So it
would seem fair to offer a helper function for this operation.

If a ccw device did not respond in time during internal io, we set it
into boxed state. With this patch we have the following behaviour:
* the ccw driver will get a notification if the device was online and
goes into the boxed state
* if the device was disconnected and got boxed nothing special is to be
done (it will be handled in reprobing later)
* if the device got boxed while initial sensing it will be unregistered

Wake up even on failed device recognition, since this may be triggered
from a user trying to force a device online. With this patch a write
to the online sysfs attribute will not block for ever but return with
-EAGAIN in this case.

Setting ->owner as done currently (pde->owner = THIS_MODULE) is racy
as correctly noted at bug #12454. Someone can lookup entry with NULL
->owner, thus not pinning enything, and release it later resulting
in module refcount underflow.
We can keep ->owner and supply it at registration time like ->proc_fops
and ->data.
But this leaves ->owner as easy-manipulative field (just one C assignment)
and somebody will forget to unpin previous/pin current module when
switching ->owner. ->proc_fops is declared as "const" which should give
some thoughts.
->read_proc/->write_proc were just fixed to not require ->owner for
protection.
rmmod'ed directories will be empty and return "." and ".." -- no harm.
And directories with tricky enough readdir and lookup shouldn't be modular.
We definitely don't want such modular code.
Removing ->owner will also make PDE smaller.
So, let's nuke it.

Errors from SIGA instructions are stored in the per queue qdio_error
and reported back when the queue handler is called. That opens a race
when multiple error conditions occur simultanously.
Report SIGA errors immediately in the return value of do_QDIO so the
upper layer can react and SIGA errors no longer interfere with other
errors.
Move the SIGA error handling in qeth from the outbound handler to
qeth_flush_buffers.

If the qdio module is unloaded the tiqdio tasklet must be terminated
by tasklet_kill. Move the tasklet_kill after the unregistration of
the adapter interrupt so the tiqdio tasklet will not be scheduled
anymore before calling tasklet_kill.

The index value that indicated that the input queue moved was also used to
store the index of the first acknowledged buffer. For non-qebsm only the
newest buffer is acknowledged which may be different from the last move index
so two seperate values are needed to track the input queue.

qdio_cleanup is a wrapper function that should call qdio_shutdown and
qdio_free. qdio_free was not called if an error occured in qdio_shutdown
resulting in a missing free of allocated resources.
Call qdio_free regardless of the return value of qdio_shutdown.

The queue tasklets were stopped with tasklet_disable. Although tasklet_disable
prevents the tasklet from beeing executed it is still possible that a tasklet
is scheduled on a CPU at that point. A following qdio_establish calls
tasklet_init which clears the tasklet count and the tasklet state leading to

Use tasklet_kill instead of tasklet_disbale. Since tasklet_schedule must not be
called after tasklet_kill use the QDIO_IRQ_STATE_STOPPED to inidicate that a

Add a mutex to protect the tiq_list. Although reading the list is done

Subchannel reprobing can block the kslowcrw workqueue indefinitely
while waiting for device recognition to finish which is also scheduled
to run on kslowcrw. Prevent this deadlock by moving the waiting
portion of subchannel reprobing to the cio workqueue.

The crw_unregister_handler uses xchg + synchronize_sched when
unregistering a crw_handler.
This doesn't protect crw_collect_info to potentially jump to NULL since
it has unlocked code like this:
if (crw_handlers[i])
crw_handlers[i](NULL, NULL, 1);
So add a mutex which protects the crw handler array for changes.

Use ccw_device_set_notoper() (which also deletes the device
timer and disables the subchannel) instead of simply setting
the state to DEV_STATE_NOT_OPER in the generic not operational
handling code. This prevents unexpected interrupts popping up
for devices that are deemed not operational.

Acting upon the assumption that cio_disable_subchannel()
is only called when we really want to disable the subchannel
(a) remove the check for activity (it is already done in
ccw_device_offline(), which is the place where it matters)
(b) collect pending status via tsch() and ignore it (it
can't matter anymore since the subchannel will be disabled).

The common I/O layer may encounter a situation where the
device number of a ccw device has changed or a device
driver doesn't want to keep a formerly disconnected device
becoming operational again. Instead of using device_del()/
device_add() as now, we can just unbind the driver from the
device and rebind it to get the desired effect (rebinding)
with less overhead.

The dasd device driver will now support ECKD devices with more then
65520 cylinders.
In the traditional ECKD adressing scheme each track is addressed
by a 16-bit cylinder and 16-bit head number. The new addressing
scheme makes use of the fact that the actual number of heads is
never larger then 15, so 12 bits of the head number can be redefined
to be part of the cylinder address.

When a recovery is started for a qeth device, additional invocations
to change a mac address, to configure a VLAN interface on top, or to
add multicast addresses should wait till recovery is finished,
otherwise recovery might fail.

The only way for a sysfs attribute to remove itself (without
deadlock) is to use the sysfs_schedule_callback() interface.
Vegard Nossum discovered that a poorly written sysfs ->store
callback can repeatedly schedule remove callbacks on the same
device over and over, e.g.
$ while true ; do echo 1 > /sys/devices/.../remove ; done
If the 'remove' attribute uses the sysfs_schedule_callback API
and also does not protect itself from concurrent accesses, its
callback handler will be called multiple times, and will
eventually attempt to perform operations on a freed kobject,
leading to many problems.
Instead of requiring all callers of sysfs_schedule_callback to
implement their own synchronization, provide the protection in
the infrastructure.
Now, sysfs_schedule_callback will only allow one scheduled
callback per kobject. On subsequent calls with the same kobject,
return -EAGAIN.
This is a short term fix. The long term fix is to allow sysfs
attributes to remove themselves directly, without any of this
callback hokey pokey.

dpm_list currently relies on the fact that child devices will
be registered after their parents to get a correct suspend
order. Using device_move() however destroys this assumption, as
an already registered device may be moved under a newly registered
one.
This patch adds a new argument to device_move(), allowing callers
to specify how dpm_list should be adapted.

This patch implements uevent suppress in kobject and removes it
from struct device, based on the following ideas:
1,Uevent sending should be one attribute of kobject, so suppressing it
in kobject layer is more natural than in device layer. By this way,
we can do it for other objects embedded with kobject.
2,It may save several bytes for each instance of struct device.(On my
omap3(32bit ARM) based box, can save 8bytes per device object)
This patch also introduces dev_set|get_uevent_suppress() helpers to
set and query uevent_suppress attribute in case to help kobject
as private part of struct device in future.

Since the receive code should tolerate any incoming garbage, it
should be protected against a potential wraparound when manipulating
length values within incoming data.
block_len is unsigned, so a too large subtraction will cause a
wraparound.

A scheduled work might still be pending, running while the adapter is
in progress to get dequeued from the system. This can lead to an
invalid pointer dereference (Oops). Once the adpater is set online
again, ensure the nameserver environment is initialized to the
appropriate values again.

Ensure the refcounting is correct even if we were not able to

It will not be necessary to set the erp failed status bit
in case a SCSI device is removed by the SCSI mid layer.
In the case a SCSI device is unavailable for a short time
(15 to 20 seconds) a FCP unit will not get on-line again.

Use the I/O blocking mechanism in the FC transport class to allow
faster failovers for multipathing:
- Call fc_remote_port_delete early to set the rport to BLOCKED.
- Check the rport status in queuecommand with fc_remote_portchkready
to no longer accept new I/O for this port and fail the I/O with the
appropriate scsi_cmnd result.
- Implement the terminate_rport_io handler to abort all pending I/O
requests
- Return SCSI commands with DID_TRANSPORT_DISRUPTED while erp is
running.
- When updating the remote port status, check for late changes and
update the remote ports status accordingly.

The usage of the PCI flag to trigger interrupts is optional. Even
without setting the flag, qdio still receives interrupts to continue
working on the queue. Remove the PCI flag from zfcp, it is not
necessary.

The current number based id ERP logging is replaced by a string
based tag version. The benefit is an easier location of the code in
question and the removal of the lengthy array referencing the
individual messages.
The string (7 bytes) based version does not use more space since those
bytes were "used" anyway due to the alignment of the structure.
The encoding of the 7 byte string is as follows
[0-1] = filename
[2-5] = task/function
[6] = section
Due to the character of this string (fixed length) a string
termination is not required here.

(e.g. in an error scenario) or not (e.g. initial open).

Issue ELS ADISC requests from workqueue. This allows the link test
request to be sent when the request queue is full due to I/O load for
other remote ports. It also simplifies request queue locking,
zfcp_fsf_send_fcp_command_task is now the only function that has
interrupts disabled from the caller. This is also a prereq for the FC
passthrough support that issues ELS requests from userspace.

When the SCSI midlayer is running error recovery, the low-level error
recovery in zfcp could be running and preventing the SCSI midlayer to
issue error recovery requests. To avoid unnecessary error recovery
escalation, wait for the zfcp erp to finish and retry if necessary.
While reworking the SCSI eh handlers, alsa cleanup the code and
simplify the interface from zfcp_scsi to the fsf layer.

Provide measurement data for the utilisation of the QDIO outbound queue.
The additional value allows to calculate an average queue utilisation
by looking at the deltas per time unit. Needed for capacity planning.
It is up to user space to handle wrap-arounds of the 64 bit value.
The new counter neatly complements the existing counter for queue full
conditions. That is why, both statistics counter have been integrated.

PORT_PHYS_CLOSING is only set and cleared, but not actually used
for status checking.
PORT_INVALID_WWPN is set when the GID_PN request does not return
a d_id for a remote port, e.g. when a remote port has been
unplugged. For this case, the d_id is zero. In the erp we can
check the d_id and use the normal escalation procedure that gives
up after three retries and remove the special case.
PORT_NO_WWPN is unused: Each port in the remote port list has a
valid wwpn. The WKA ports are now tracked outside the port
list. Remove the PORT_NO_WWPN flag, since this is no longer set
for any port.

Standby memory detected with the sclp interface gets always registered
with add_memory calls without considering the limitationt that the
"mem=" kernel paramater implies.
So fix this and only register standby memory that is below the specified
limit.
This fixes zfcpdump since it uses "mem=32M". In case there is appr.
2GB standby memory present all of usable memory would be used for the
struct pages needed for standby memory.

In dasd_device_set_timer and dasd_block_set_timer we interpret the
return value of mod_timer in a wrong way. If the timer expires in
the small window between our check of timer_pending and the call to
mod_timer, then the timer will be set, mod_timer returns zero and
we will call add_timer for a timer that is already pending.
As del_timer and mod_timer do all the necessary checking themselves,
we can simplify our code and remove the race a the same time.

If CONFIG_DEBUG_FS is disabled debugfs_create_file returns -ENODEV instead

Add a check of the debugfs_create_file return value and in case of an error
set the entry to NULL so it will be ignored at shutdown.

When the connection between host and storage server is lost, the
dasd device driver usually blocks all I/O on affected devices and
waits for them to reappear. In some setups however it would be
better if the I/O is returned as error so that device can be
recovered by some other means, eg. in a raid or multipath setup.

When a DASD device enters or leaves the 'online' state we need to
trigger change events for the respective disk and partitions.
These extra events are needed because when disk and partitions are
first added, udev rules that try to read disk labels or other data
may fail as the disk may not yet be ready.

The ip assist hw command for setting an IP address last unacceptable
long so we can not spin while we waiting for the irq. Since we can
ensure process context for all occurrences of this command we can use
wait.

For z/VM GuestLAN or VSWITCH devices the transport layer is
configured in z/VM. The layer2 attribute of a participating Linux
device has to match the z/VM definition. In case of a mismatch
Linux currently crashes in qeth recovery due to a reference to the
not yet existing net_device.
Solution: add a check for existence of net_device and add a message
pointing to the mismatch of layer definitions in Linux and z/VM.

OSA-devices operating in layer3 mode offer adding of the source MAC
address to the QDIO header of inbound packets. The qeth driver can
exploit this functionality to replace FAKELL-entries in the ethernet
header of received packets.

Distinguish the cputime of the idle process where idle is actually using
cpu cycles from the cputime where idle is sleeping on an enabled wait psw.
The former is accounted as system time, the later as idle time.

This patch implements config_changed for the s390 virtio transport.
We use the least significant bit of the interrupt parameter field
to decide, if this interrupt should call the virtio virtqueue callback
or the config_changed callback.
This method is compatible with old host and guest code. Old 64 bit guests
will not check the bit and trigger a harmless additional vring_interrupt
call. Old host code will never set this bit, this is also safe.
This patch also takes care of a potential future 31 bit virtio transport
for s390. On 31 bit _LC_PFAULT_INTPARM and __LC_EXT_PARAMS are identical.
We exploit the alignment of the token and fold the change bit into the
lsb of the token itself.

Remove a message that was emitted for a port that could not initially
be opened. This is a rare case when the port discovery hits an
initiator port and only confuses the user with an initator port logged
in the message. Remove the whole special case: The failed "open port"
request triggers required follow-up actions anyway.

Add the support to send CT and ELS requests as unchained FSF requests. This is
required for older hardware and was somehow omitted during the cleanup of the
FSF layer. The req_count and resp_count attributes are unused, so remove them
instead of adding a special case for setting them. Also add debug data and a
warning, when the ct request hits a limit.

The port flag DID_DID indicates whether we know the current id of the
port. This is always set in parallel. Since the id 0 is invalid
(because the port id 0 is invalid) we can remove the DID_DID flag:
d_id of 0 indicates an invalid d_id != 0 is a valid one.

The information from the kernel parameter is only needed during init.
Keep the three pieces (busid, wwpn and lun) local to simplify the
global zfcp_data structures. While at it, also remove the unused
loglevel variable and give the module parameter variable a better
name.

After the latest changes, the list of FCP devices is only used to
lookup the adapter for requests from the actcli tool. Change this to
use the lookup function in the cio layer. Now we can remove the
adapter list and have one place less to use the global config_lock.

To check if multipath is available we count the bits set in lpm,
which could change over time (via configure [on|off] of a path).

Sometimes we change the pmcw configuration but don't call msch
to transmit these changes to the channel subsystem.

To change the configuration of a subchannel we alter the modifiable
bits of the subchannel's schib field and issue a modify subchannel.
There can be the case that not all changes were applied -or worse-
quietly overwritten by the hardware. With the next store subchannel
we obtain the current state of the hardware but lose our target
configuration.
With this patch we introduce a subchannel_config structure which
contains the target subchannel configuration. Additionally the msch
wrapper cio_modify is replaced with cio_commit_config which
copies the desired changes to a temporary schib. msch is then
called with the temporary schib. This schib is only written back
to the subchannel if all changes were applied.

There is the chance that we get condition code 0 for a stsch but
the resulting schib is not vaild. In the current code there are
2 cases:
* we do a check for validity of the schib after stsch, but at this
time we have already stored the invaild schib in the subchannel
structure. This may lead to problems.
* we don't do a check for validity, which is not that good either.
The patch addresses both issues by introducing the stsch wrapper
cio_update_schib which performs stsch on a local schib. This schib
is only written back to the subchannel if it's valid.
side note: For some functions (chp_events) the return codes are
different now (-ENXIO vs -ENODEV) but this shouldn't do harm
since the caller doesn't check for _specific_ errors.

If we fail the probe for an I/O subchannel, we won't be able
to unregister it again since there are no sch_event()
callbacks for unbound subchannels. Just succeed the probe in
any case and schedule unregistering the subchannel.

There is a race between io_subchannel_register() and
io_subchannel_sch_event() which may cause a subchannel to be
unregistered because it is no longer operational before
io_subchannel_register() had run. We need to check whether the
subchannel is still registered before the ccw device can be
registered and just bail out if it is not.

Subchannel refcounting was incorrect in some places, especially
a refcount was missing when ccw_device_call_sch_unregister()
was called and the refcount was not correctly switched after
moving devices.
Fix this by establishing the following rules:
- The ccw_device obtains a reference on its parent subchannel
when dev.parent is set and gives it up in its release
function. This is needed because we need a parent reference
for correct refcounting even before the ccw device is (if at
all) registered.
- When calling device_move(), obtain a reference on the new
subchannel before moving the ccw device and give up the
reference on the old parent after moving. This brings the
refcount in line with the first rule.

The current code attempts to get an extra reference count
for online devices by doing a get_device() in ccw_device_online()
and a put_device() in ccw_device_done(). However, this
- incorrectly obtains an extra reference for disconnected
devices becoming available again (since they are already
online)
- needs special checks for css_init_done in order to handle
the console device
- is not obvious and
- may incorretly drop a reference count in ccw_device_done() if
that function is called after path verification for a device
that just became not operational.
So let's just get the reference in ccw_device_set_online() and
drop it in ccw_device_set_offline(). (Unfortunately, we still
need the special case in io_subchannel_probe().)

For a large number of I/O requests the values were shifted binary.
The shift was not transparent for the user because the shift value
was not displayed. To make this interface more human readable the
values are shifted decimal and the scale factor is displayed.

Register zfcp with the new /proc/service_level interface to report the
FCP microcode level. When the adapter goes offline or a channel path
disappears, zfcp unregisters, since the microcode version might change
and zfcp does not know about it.

to report a relevant service level, e.g. the microcode level of
devices, the service level of the hypervisor, etc.

Hipersocket connections can encounter temporary busy conditions.
In case of the busy bit set we retry the SIGA operation immediatelly.
If the busy condition still persists after 100 ms we fail and report
the error to the upper layer. The second stage retry logic is removed.
In case of ongoing busy conditions the upper layer needs to reset the
connection.
The reporting of a SIGA error is now done synchronously to allow the
network driver to requeue the buffers. Also no error trace is created
for the temporary SIGA errors so the error message view is not flooded.

- Use automatic acknowledgement of incoming buffers in QEBSM mode
- Move ACK for non-QEBSM mode always to the newest buffer to prevent
a race with qdio_stop_polling
- Remove the polling spinlock, the upper layer drivers return new buffers
in the same code path and could not run in parallel
- Don't flood the error log in case of no-target-buffer-empty
- In handle_inbound we check if we would overwrite an ACK'ed buffer, if so
advance the pointer to the oldest ACK'ed buffer so we don't overwrite an
empty buffer in qdio_stop_polling

The vmcp driver uses the session->mutex for concurrent access of the data
structures. Therefore, the BKL in vmcp_open does not protect against any
other function in the driver.
The BLK in vmcp_open would protect concurrent access to the module init
but all necessary steps ave finished before misc_register is called.
We can safely remove the lock_kernel from vcmp.

So just don't allocate private data memory when do alloc_netdev()

The zfcp_scsi_queuecommand was not acting according to the standard
when the respective unit was not available. In this case an -EBUSY was
returned, which is not valid in itself, and in addition scsi_done
was called. This combination is not allowed and was leading to a
double finish of the request and therefor double decrement of the
host_busy counter.

Waiting for the ERP to be finished in a task running in the global
kernel work-queue is a bad idea, especially if the ERP needs to run
another job in this work-queue before it can finish. -> deadlock.
This patch removes the necessity to wait for a finished ERP from the
scan task and moves the job scheduling to the end of the ERP.

The check of having a valid pointer was performed before the
processing was secured by the lock. Between those two steps the
pointer can turn invalid. During further processing another value is
used (referenced by the pointer described above) as a function pointer
which is never verified to be valid either, resulting under some
circumstances in an invalid function call. This patch is fixing both
issues.

Aborting a SCSI cmnd might requrie to send a abort_fsf_cmnd. If the
creation of this fsf_req fails an ERR_PTR is returned where a NULL
value would be expected as an error indicator. This ERR_PTR is
dereferenced as valid fsf_req in succeeding processing leading to
an error.

Running two wka_port_get calls in parallel could issue two open_port
requests, overwriting the port handle. Don't issue an open_port
for the state PORT_OPENING, and only read the data from GOOD
responses.

For an incoming RSCN it was checked by the ZFCP_STATUS_PORT_DID_DID
define to re-open a remote port or to test the connection. Since this
define was re-used it was also necessary to replace that define with
ZFCP_STATUS_PORT_PHYS_OPEN.

In ccw_device_move_to_orphanage(), a replacing ccw_device
is searched via get_{disc,orphaned}_ccwdev_by_dev_id()
which obtain a reference on the returned ccw_device.
This reference must be given up again after the device
has been moved to its new parent.

The current virtio model on s390 has the descriptor page above the main
memory. The guest virtio detection will oops if the mem= parameter is
used to reduce/change the memory size.
We have to use real_memory_size instead of max_pfn to detect the virtio
descriptor pages.

- FC payloads were dumped in different record than the output

If an open port fsf request times out (in erp) the
corresponding erp_action member of the fsf
request need to set to NULL. If the port structure
will be removed later-on there will be still a
reference in the fsf request to the non existing
erp_action otherwise.

Attaching a unit immediately after setting the adapter online should
be possible. The problem right now is that the port_scan runs from a
workqueue and has not finished when the set_online call returns and
the sysfs structures for the ports are not available yet. Fix that by
waiting for the port scan to complete.

Fix the handling of the request list in the error path:
- Use irqsave for the lock as in the good path.
- Before removing the request, check if it is still in the list, a
call to dismiss_all might have changed the list in between.
- zfcp_qdio_send does not change the queue counters on failure,
trying revert something is wrong, so remove this.

It is possible that a remote port has a problem, the SCSI device gets
deleted after the rport timeout and then the timeout for pending SCSI
commands trigger an abort. For this case, don't delete the reference
from the SCSI device to the zfcp unit, so that we can still have the
reference to issue an abort request.

In case of I/O errors on a qdio subchannel qdio_shutdown may be
called twice by the qdio driver and by zfcp. Remove the
superfluous shutdown from qdio and let the upper layer driver
handle the error condition.

To keep the size of changesets sane we split the switch by drivers;
to keep the damn thing bisectable we do the following:
1) rename the affected methods, add ones with correct
prototypes, make (few) callers handle both. That's this changeset.
2) for each driver convert to new methods. *ALL* drivers
are converted in this series.
3) kill the old (renamed) methods.
Note that it _is_ a flagday; all in-tree drivers are converted and by the
end of this series no trace of old methods remain. The only reason why
we do that this way is to keep the damn thing bisectable and allow per-driver
debugging if anything goes wrong.

Currently it is sometimes locked by the tty mutex and sometimes by the
sighand lock. The latter is in fact correct and now we can hand back referenced
objects we can fix this up without problems around sleeping functions.

We now return a kref covered tty reference. That ensures the tty structure
doesn't go away when you have a return from get_current_tty. This is not

Multipath is best at handling transport errors. If it gets a device
error then there is not much the multipath layer can do. It will just
access the same device but from a different path.
This patch breaks up failfast into device, transport and driver errors.
The multipath layers (md and dm mutlipath) only ask the lower levels to
fast fail transport errors. The user of failfast, read ahead, will ask
to fast fail on all errors.
Note that blk_noretry_request will return true if any failfast bit
is set. This allows drivers that do not support the multipath failfast
bits to continue to fail on any failfast error like before. Drivers
like scsi that are able to fail fast specific errors can check
for the specific fail fast type. In the next patch I will convert
scsi.

The 3215 console irq handler used to schedule a tasklet. However the
console irq handler also gets called from the infamous cio_tpi()
function. Which in turn does something like
local_bh_disable()
[call console irq handler]
_local_bh_enable()
_local_bh_enable() prevents execution of softirqs, which is intended
within cio_tpi(). However there might be a new softirq pending because
irq handler scheduled a tasklet.
In order to prevent this behaviour we just get rid of the tasklet.
It's not doing much anyway.

The s390 console drivers use the unblank callback of the console
structure to flush the console buffer. In case of a panic or a
reboot the CPU doing the callback can block on the console i/o.
The other CPUs in the system continue to work. For panic this is
not a good idea.
Replace the unblank callback with proper panic/reboot notifier.
These get called after all but one CPU have been stopped.

specific parameter list sent to the array via a Perform Subsystem
Function CCW. The Symmetrix response is retrieved from the array
via a Read Subsystem Data CCW.

The xpram driver uses a single block device queue for all of its
devices so far. With recent kernels removing xpram module fails to
clean up all sysfs files. The next time the xpram module is loaded
you'll get warnings:

Followed by the usual WARN_ON output, followed by an error message

In the unsolicited interupt handler fake IRBs from CIO have to be
ignored because there is nothing to do.
The function dump_sense should not be called if there is no sense
data available.

allow a DCSS block device to map to a set of contiguous DCSSs. The

In commit e6bafba5b4, a bug was fixed that
involved converting !x & y to !(x & y). The code below shows the same
pattern, and thus should perhaps be fixed in the same way. In particular,
the result of !scsw_stctl(&request->irb.scsw) & SCSW_STCTL_STATUS_PEND is
always just !scsw_stctl(&request->irb.scsw).

If an asynchronous HiperSockets queue runs full, no further packet
can be sent. In this case the next initiative to give transmitted
skbs back to the stack is triggered only by a 10-seconds qdio timer.
This timer has been introduced for low multicast traffic scenarios
to guarantee freeing of skbs in a limited amount of time. For high
HiperSocket multicast traffic scenarios progress checking on the
outbound queue should be enforced by tasklet rescheduling.

* Implement disk_devt() and part_devt() and use them to directly
access devt instead of computing it from ->major and ->first_minor.
Note that all references to ->major and ->first_minor outside of
block layer is used to determine devt of the disk (the part0) and as
->major and ->first_minor will continue to represent devt for the
disk, converting these users aren't strictly necessary. However,
convert them for consistency.
* Implement disk_max_parts() to avoid directly deferencing
genhd->minors.
* Update bdget_disk() such that it doesn't assume consecutive minor
space.
* Move devt computation from register_disk() to add_disk() and make it
the only one (all other usages use the initially determined value).
These changes clean up the code and will help disk->part dereference

Due to the character of a scheduled work we cannot guarantee the
LUN register to be finished before an initial device tries to use it.
Therefor we have to wait for PENDING_SCSI_WORK flag to be cleared
before proceeding.

The zfcp_erp_thread was using the nolock version of the dbf function.
This resulted in a list access while other tasks could modifying the
list. The symptom was an erp thread running at 100% CPU and never
returning from the dbf function.

In case of an adapter reopen all rports have to be deleted from the
environment. This should only happen for already registered rports
otherwise fc_remote_port_delete is called with a NULL pointer.

Each adapter reopen trigger automatically a scan_port task which
is waiting for the ERP to be finished before further processing.
Since the initial device setup enqueues adapter, port and LUN which
are individual ERP actions, this process would start after
everything is done. Unfortunately the port_reopen requires another
scheduled work to be finished which is queued after the automatic
scan_port -> deadlock !
This fix creates an own work queue for ERP based nameserver requests.

Now that we removed the long messages for the bit error threshold
data, put the data in the hba trace. This way, we get a short warning
for the threshold event from the hardware and have the data in the
trace for further analysis.

Reduce the size of zfcp data structures by removing unused and
redundant members. scsi_lun is only the mangled version of the
fcp_lun. So, remove the redundant field and use the fcp_lun instead.
Since the queue lock and the pci_batch indicator are only used in the
request queue, move them from the common queue struct to the adapter
struct.

Changing the zfcp behaviour from always having the nameserver port
open to an on-demand strategy. This strategy reduces the use of
limited resources like port connections. The patch provides a common
infrastructure which could be used for all WKA ports in future.
Also reduce the number of nameserver lookups by changing the zfcp
behaviour of always querying the nameserver for the corresponding
destination ID of the remote port. If the destination ID has changed
during the reopen process we will be informed and then trigger a
nameserver query on demand.

- Remove unused references and declarations, including one instance
of the FC ls_adisc struct that has been defined twice.
- Also remove the flags COMMON_OPENING, COMMON_CLOSING,
ADAPTER_REGISTERED and XPORT_OK that are only set and cleared, but
not checked anywhere.
- Remove the zfcp specific atomic_test_mask makro. Simply use
atomic_read directly instead.
- Remove the zfcp internal sg helper functions and switch the places
where it is still used to call sg_virt directly.
- With the update of the QDIO code, the QDIO data structures no
longer use the volatile type qualifier. Now we can also remove the
volatile qualifiers from the zfcp code.

The problem is that dev->driver_data for a ccw device is NULL,
while it should point to the ccwgroup device it is a member of.
This happened due to incorrect cleanup if creating a ccwgroup
device failed because the ccw devices were already grouped.
Fix this by setting cdev[i] to NULL in the error handling of
ccwgroup_create_from_string() after we give up our reference and
by checking if the driver_data points to the ccwgroup device in
ccwgroup_release() just to be really sure.

When disconnected ccw devices are removed, the device has to be set
offline, otherwise there will be side effects including a reference
count imbalance. This patch modifies ccw_device_offline to work for
devices in disconnecte/not operational state. ccw_device_offline is
called by cio for devices which are online during device removal.

ssch() has two classes of return codes:
- condition codes (0-3) which need to be translated to Linux
error codes
- Linux error codes (-EIO on exceptions) which should be passed
to the caller (instead of erronously being handled like

The notify handler does not change the list of adapters, it only
triggers erp actions to open or shutdown the adapter. We don't
need to hold the config semaphore and wait for the erp to complete.
This is a prerequisite for a fix in the s390 common i/o code.

When the last port entry in the gpn_ft response is already a known
port, the eval function would skip the end flag and try to attach
ports with wwpn 0. Fix this by checking the flag first.

The request queue lock can be acquired from softirq context when the
SCSI midlayer issues commands. Disable softirqs for this lock when
commands are issued from zfcp.

probe_error() frees memory only, if cgdev->dev.driver_data refers

Concurrently adding the same segment may lead to duplicate device_register()
calls, which will trigger an error in kobject code:
"... don't try to register things with the same name in the same directory".

Calling a ccw driver's notify function without the ccw device lock
held opens up a race window between discovery and handling of a change
in the device operational state. As a result, the device driver may
encounter unexpected device malfunction, leading to out-of-retry
situations or similar.
Remove race by extending the ccw device lock from state change
discovery to the calling of the notify function.

The Perform Subsystem Function/Prepare for Read Subsystem Data
command requires 12 bytes of parameter data, but the respective data
structure dasd_psf_prssd_data has a length of 16 bytes.
Current storage servers ignore the obsolete bytes, but older models
fail to execute the command and report an incorrect length error.
This causes the device initilization for these devices to fail.
To fix this problem we need to correct the dasd_psf_prssd_data
structure and shorten it to the correct length.

Fix ccw group device initialization: initialize device object before
using reference counting during cleanup.
Fixes the following message when group device initialization fails
(e.g. because too few devices where specified):
kobject: '<NULL>' (..): is not initialized, yet kobject_put() is
being called.

Increasing the qdio reference count for every used subchannel
is unnecessary since unloading qdio (if build as a module) is
only possible if other modules that use qdio are unloaded.
Unloading modules that use qdio in turn requires that these
modules shut down all qdio subchannels. Therefore the additional
module_get reference is not needed.

If qdio_establish fails we call qdio_shutdown to cleanup the
qdio subchannel. The tiq_list entry may not be valid at that
time, therefore we must ignore queues with an invalid list entry
in tiqdio_remove_input_queues.

In case of error, functions dasd_kmalloc_request and idal_buffer_alloc
return an ERR pointer, but never return the NULL pointer. So after a
call to one of these functions, a NULL test should be replaced by an
IS_ERR test.

sclp_sync_wait wait synchronously for an sclp interrupt and disables
timer interrupts. However on the irq enter paths there is an extra
check if a timer interrupt would be due and calls the timer callback.
This would schedule softirqs in the wrong context.
So introduce local_tick_enable/disable which prevents this.

When z/VM provides two virtual devices (minidisks) that reside on the
same real device, both will receive the configuration data from the
real device and thus get the same uid. To fix this problem, z/VM
provides an additional configuration data record that allows to
distinguish between minidisks.
z/VM APAR VM64273 needs be installed so this fix has an effect.

Cornelia Huck noticed that a modular virtio without kvm guest support
leads to a build error in the s390 virtio transport:

The virtio transport only works with kvm guest support and only as a
builtin. Lets change the build process of drivers/s390/kvm/kvm_virtio.c
to depend on kvm guest support, which is also a bool.
CONFIG_S390_GUEST already selects CONFIG_VIRTIO, that should prevent
CONFIG_S390_GUEST=y CONFIG_VIRTIO=n situations.

To prepare for virtio_ring transport feature bits, hook in a call in
all the users to manipulate them. This currently just clears all the
bits, since it doesn't understand any features.

Rather than explicitly handing the features to the lower-level, we just
hand the virtio_device and have it set the features. This make it clear
that it has the chance to manipulate the features of the device at this
point (and that all feature negotiation is already done).

This patch enables virtio_console as the default console on kvm for
s390. We currently use the same notify hack as lguest for early
console output. I will try to address this for lguest and s390 later.

Finally fixes a possible scheduling while in atomic context bug. The driver
used to wait on a waitqueue if no empty buffer was available. This could
lead to a deadlock if the driver was called from non-schedulable context.
So fix this. The write operation may fail now. It returns the number of
characters accepted. put_char will never fail, since it writes characters
to an intermediate buffer which gets flushed as soon as it is full.
That means the driver now can busy wait if something is in the intermediate
buffer and a write_string operation follows. Seems to be an acceptable
compromise, since that shouldn't happen too often.

After all we came to the conclusion that this interface doesn't make any
sense. Besides that the ioctl number used was never registered, the header
file isn't exported, and we doubt there is even a single user.
So remove this interface, since it eases maintenance.

Most likely it is broken anyway because of the changes in memory
detection. Since we can't test it and there are probably better ways

In some cases where the channel subsystem decides to drop a subchannel
device device_unregister may be called twice, which results in an oops.
The patch prevents this by only unregistering registered devices.

Add support for new micro code load of CEX2C and CEX2A adapters,
which uses different IDs. This patch just adds the IDs to the

The ap poll mechanism is converted to use a high-resolution timer for
polling. This allows more specific polling. With this a new sysfs
attribute is introduced to specify the polling rate in nanoseconds.

This patch adds a driver for subchannels of type chsc.
A device /dev/chsc is created which may be used to issue ioctls to:
- obtain information about the machine's I/O configuration
- dynamically change the machine's I/O configuration via
asynchronous chsc commands

Passing the affected chpid in chp_event() worked only by
chance since chpid is the first element in res_acc_data.
Make it work properly by generalizing res_acc_data as
chp_link and always passing around a properly filled out
chp_link structure in chp_event().

Enhance the adapter interruption API so that device drivers can
register a handler for a specific interruption subclass. This
will allow different device drivers to move to differently
prioritized subclasses in order to avoid congestion.

Replace the numeric values for I/O interruption subclass usage
with abstract definitions and collect them all in asm/isc.h.
This gives us a better overview of which iscs are actually used
and makes it possible to better spread out isc usage in the
future.
