This patch changes several mempool users, all of which are basically just
wrappers around kmalloc(), to use the common mempool_kmalloc/kfree, rather
than their own wrapper function, removing a bunch of duplicated code.

Undetected edge case for CRT messages to CEX2A caused length to be too short,
thus truncating the message. The solution was to check a different variable
which actually determines which key type is being used.
Increment version number in z90main.c to correct level of 1.3.3, fix copyright
year and add comment about bitlength limit of CEX2A.

If a tape device is assigned to another host, the interrupt for the assign
operation comes back with deferred condition code 1. Under some conditions
this can lead to an endless loop of retries. Check if the current request is
still in IO in deferred condition code handling and prevent retries when the
request has already been cancelled.

When a request is aborted because of a signal, we currently stop the request
via csh, but we do not wait for the interrupt of csh in any case. We free the
request structure and therefore when the interrupt for the csh operation is
presented, the request object is no longer valid and an invalid callback
pointer is used.
To fix this wait until the interrupt for csh arrives and until
wait_event_interruptible() does not return -ERESTARTSYS.

If a deferred CC happens there will be lots of messages, because the retry is
done immediatly in the interrupt handler which can be too fast. To avoid this
requeue the request and schedule the queue to be processed.

The DASD extended error reporting is a facility that allows to get detailed
information about certain problems in the DASD I/O. This information can be
used to implement fail-over applications that can recover these problems.

dasd_cmd just implements three ioctls which are wrappers around functionality
in the core kernel or other modules. When merging those into dasd_mod they
just add 22 lines of code which is far less than the amount of code removed in
the last two patches, and which doesn't spill into another 4k pages when build
modular, while removing a 128lines module.

Add an ->ioctl method to the dasd_discipline structure. This allows to apply
the same kind of cleanups the last patch applied to dasd_ioctl.c to
dasd_eckd.c (the only dasd discipline with special ioctls) aswell.
Again lots of code removed. During auditing the ioctls I found two fishy
return value propagations from copy_{from,to}_user, maintainers please check
those, I've marked them with XXX comments.

Handle ioctls implemented in dasd_ioctl through the normal switch statement
that most drivers use instead of the awkward dasd_ioctl_no_register routine.
This avoids searching a linear list on every call to dasd_ioctl(), and allows
to give the various ioctl implementation functions sane prototypes, aswell as
moving the check for bdev->bd_disk->private_data from the individual functions
to dasd_ioctl. (I think it can't actually every be NULL, but let's keep that
for later)

When cio waits for the interrupt for a basic sense, interrupts for hsch() or
csch() issued in the meantime are wrongly counted as interrupts for the basic
sense and the accumulated irb is passed to the device driver. In
ccw_device_w4sense(), check for clear or halt function in the irb and pass the
irb for the csch() or hsch() to the device driver.

The patch fixes following issues:
(1) Replace scsi_add_device with scsi_scan_target.
(Thus the rport instead of the scsi_host becomes parent of a
scsi_target again.)
(2) Avoid scsi_device allocation during registration of an remote port.
(Would be done during fc_scsi_scan_rport.)
(3) Fix queuecommand behaviour when an zfcp unit is blocked.
(Call scsi_done with DID_NO_CONNECT instead of returning
SCSI_MLQUEUE_DEVICE_BUSY otherwise we might end up waiting
for completion in blk_execute_rq for ever.)

DASD allows to open a device as soon as gendisk is registered, which means the
device is a fake device (capacity=0) and we do know nothing about blocksize
and partitions at that point of time. In case the device is opened by
someone, the bdev and inode creation is done with the fake device info and the
following partition detection code is just using the wrong data.
To avoid this modify the DASD state machine to make sure that the open is
rejected until the device analysis is either finished or an unformatted device
was detected.

It fixes a bug in zfcp which provokes a race
in scsi_scan.c. Finally this can lead to an Oops like:
kernel BUG at fs/sysfs/symlink.c:87!
Correctly set this_id for the host. Otherwise we provoke
a race between scsi_target_reap_work and concurrent
scsi_add_device.

When using the dasd diag discipline, the base discipline module (eckd or fba)
can be unloaded, even though the dasd driver requires both discipline modules
(base and diag) to work correctly.
Implement reference counting for both base and diag discipline modules in
order to fix this.

Using FCP devices with V=V support, the input queue stalled when CCQ 97 had
been returned in qdio_do_eqbs. When this happen we have to reissue the eqbs
instruction.
Another bug was when V=V was enabled we checked if hardware has SIGA-sync
support. If not we returned with 0 from tiqdio_is_inbound_q_done. Thus qdio
lost initiative on FCP devices and input queue stalled. Running devices in
V=V there is no SIGA-sync support but nevertheless we have to process
tiqdio_is_inbound_q_done either.

If __ccw_device_disband_start() fails to initiate disbanding, it should finish
with ccw_device_disband_done() (which leaves the device in offline state)
instead of ccw_device_verify_done() (which leaves the device in online state).

When the sclp interface takes very long to serve a request, the sclp core
driver will report a failed request to the sclp tty driver even though the
request is still being processed by the sclp interface. Eventually the sclp
interface completes the request and updates some fields in the request buffer
which leads to a corrupted tty pages list. The next time function
sclp_tty_write_room is called, the corrupted list will be traversed, resulting
in an oops.
To avoid this remove the busy retry limit and increase retry intervals.

The sleep_on function clears a running cqr without waiting for the related
interrupt. This can lead to a panic at the time the interrupt is processed
because the related memory might already be freed. Wait for clear-interrupt
and de-queue cqr prior to return.

The open_count is increased for every opener, that includes the blkdev_get in
dasd_scan_partitions. This tampers the open_count in BIODASDINFO. Hide the
internal open from user-space.

Remove useless s390 define from hangcheck-timer, remove wrong definition of a
TOD second and other s390 ifdefs. Use monotonic_clock instead.
Add hangcheck-timer option, copied from drivers/char/Kconfig. This is ugly
but unless we have a big Kconfig cleanup we cannot include
drivers/char/Kconfig...

The chps[] array in struct channel_subsystem is one too short; therefore the
code doesn't realize the chpid ff is already known. When several devices on
chpid ff become available, the message "new_channel_path: could not register
ff" is displayed for every device but the first one.

Added host stats, removed superfluous get_starget_ functions,
removed some attributes from zfcp specific sysfs tree (e.g.
scsi_host_no, scsi_lun, wwnn and d_id).
Host stats are given for the physical adapter port not for the
virtual adapter. Reset stats is implemented in the device driver.

The API and code have been through various bits of initial review by
serial driver people but they definitely need to live somewhere for a
while so the unconverted drivers can get knocked into shape, existing
drivers that have been updated can be better tuned and bugs whacked out.
This replaces the tty flip buffers with kmalloc objects in rings. In the
normal situation for an IRQ driven serial port at typical speeds the
behaviour is pretty much the same, two buffers end up allocated and the
kernel cycles between them as before.
When there are delays or at high speed we now behave far better as the
buffer pool can grow a bit rather than lose characters. This also means
that we can operate at higher speeds reliably.
For drivers that receive characters in blocks (DMA based, USB and
especially virtualisation) the layer allows a lot of driver specific
code that works around the tty layer with private secondary queues to be
removed. The IBM folks need this sort of layer, the smart serial port
people do, the virtualisers do (because a virtualised tty typically
operates at infinite speed rather than emulating 9600 baud).
Finally many drivers had invalid and unsafe attempts to avoid buffer
overflows by directly invoking tty methods extracted out of the innards
of work queue structs. These are no longer needed and all go away. That
fixes various random hangs with serial ports on overflow.
The other change in here is to optimise the receive_room path that is
used by some callers. It turns out that only one ldisc uses receive room
except asa constant and it updates it far far less than the value is
read. We thus make it a variable not a function call.
I expect the code to contain bugs due to the size alone but I'll be
watching and squashing them and feeding out new patches as it goes.
Because the buffers now dynamically expand you should only run out of
buffering when the kernel runs out of memory for real. That means a lot of
the horrible hacks high performance drivers used to do just aren't needed any
more.
Description:
tty_insert_flip_char is an old API and continues to work as before, as does
tty_flip_buffer_push() [this is why many drivers dont need modification]. It
does now also return the number of chars inserted
There are also
tty_buffer_request_room(tty, len)
which asks for a buffer block of the length requested and returns the space
found. This improves efficiency with hardware that knows how much to
transfer.
and tty_insert_flip_string_flags(tty, str, flags, len)
to insert a string of characters and flags
For a smart interface the usual code is
len = tty_request_buffer_room(tty, amount_hardware_says);
tty_insert_flip_string(tty, buffer_from_card, len);

At the moment tty buffers are attached directly to the tty. This is causing a
lot of the problems related to tty layer locking, also problems at high speed
and also with bursty data (such as occurs in virtualised environments)
I'm working on ripping out the flip buffers and replacing them with a pool of
dynamically allocated buffers. This allows both for old style "byte I/O"
devices and also helps virtualisation and smart devices where large blocks of
data suddenely materialise and need storing.
So far so good. Lots of drivers reference tty->flip.*. Several of them also
call directly and unsafely into function pointers it provides. This will all
break. Most drivers can use tty_insert_flip_char which can be kept as an API
but others need more.
At the moment I've added the following interfaces, if people think more will
be needed now is a good time to say
int tty_buffer_request_room(tty, size)
Try and ensure at least size bytes are available, returns actual room (may be
zero). At the moment it just uses the flipbuf space but that will change.
Repeated calls without characters being added are not cumulative. (ie if you
call it with 1, 1, 1, and then 4 you'll have four characters of space. The
other functions will also try and grow buffers in future but this will be a
more efficient way when you know block sizes.
int tty_insert_flip_char(tty, ch, flag)
As before insert a character if there is room. Now returns 1 for success, 0
for failure.
int tty_insert_flip_string(tty, str, len)
Insert a block of non error characters. Returns the number inserted.
int tty_prepare_flip_string(tty, strptr, len)
Adjust the buffer to allow len characters to be added. Returns a buffer
pointer in strptr and the length available. This allows for hardware that
needs to use functions like insl or mencpy_fromio.

Add a compat_ioctl method to the dasd driver so the last entries in
arch/s390/kernel/compat_ioctl.c can go away. Unlike the previous attempt this
one does not replace the ioctl method with an unlocked_ioctl method so that
the ioctl_by_bdev calls in s390 partition code continue to work.

HDIO_GETGEO is implemented in most block drivers, and all of them have to
duplicate the code to copy the structure to userspace, as well as getting
the start sector. This patch moves that to common code [1] and adds a
->getgeo method to fill out the raw kernel hd_geometry structure. For many
drivers this means ->ioctl can go away now.
[1] the s390 block drivers are odd in this respect. xpram sets ->start
to 4 always which seems more than odd, and the dasd driver shifts
the start offset around, probably because of it's non-standard
sector size.

When the sclp_cpi module is loaded on a system which does not support the
required SCLP call (e.g. on z/VM), ENOSUPP is returned to user space. The

Add support for multiple subchannel sets. Works with arbitrary devices in
subchannel set 1 and is transparent to device drivers. Although currently
only two subchannel sets are available, this will work with the architectured
maximum number of subchannel sets as well.

This patch introduces a struct subchannel_id containing the subchannel number
(formerly referred to as "irq") and switches code formerly relying on the
subchannel number over to it.

New feature V=V qdio pass-through.
QDIO and HiperSockets processing in z/VM V=V guest environments (as well as
V=R with z/VM running in LPAR mode) requires shadowing of all QDIO
architecture queue elements. Especially the shadowing of SBALs and SLSBs
structures in the hypervisor, and the need to issue SIGA SYNC operations to
observe state changes, eventually causes significant CPU processing overhead
in the hypervisor.
The QDIO pass-through support for V=V guests avoids the shadowing of SBALs and
SLSBs. This significantly reduces the hypervisor overhead for QDIO based I/O.

To properly support multipath-failover handling, the linux block layer has
introduced a special request flag, 'REQ_FAILFAST'. This flag is now used to
return requests immediately in case the device is not operational.

If we receive path not operational indications (pnom in pmcw nonzero), we
switch off those paths. To catch them becoming available again, we have to
recalculate the lpm from the pmcw each time we start path verification.

Fix the broken atomic_cmpxchg primitive. Add atomic_sub_and_test,
atomic64_sub_return, atomic64_sub_and_test, atomic64_cmpxchg,
atomic64_add_unless and atomic64_inc_not_zero. Replace old style
atomic_compare_and_swap by atomic_cmpxchg. Shorten the whole header by
defining most primitives with the two inline functions atomic_add_return and
atomic_sub_return.
In addition this patch contains the s390 related fixes of Hugh's "mm: fill
arch atomic64 gaps" patch.

add @uptodate argument to end_that_request_last() and @error
to rq_end_io_fn(). there's no generic way to pass error code
to request completion function, making generic error handling
of non-fs request difficult (rq->errors is driver-specific and
each driver uses it differently). this patch adds @uptodate
to end_that_request_last() and @error to rq_end_io_fn().
for fs requests, this doesn't really matter, so just using the
same uptodate argument used in the last call to
end_that_request_first() should suffice. imho, this can also
help the generic command-carrying request jens is working on.

The first of these changes s/hotplug/uevent/ was needed to
compile sn2_defconfig (ia64/sn). The other three files
changed are blind changes of all remaining bus_type.hotplug
references I could find to bus_type.uevent.
This patch attempts to finish similar changes made in the
