Varun Prakash 1a40f0a36f iscsi-target: fix iscsi cmd leak
If iscsi-target receives NOP OUT with ITT and TTT
set to 0xffffffff it allocates iscsi_cmd but
does not free the cmd, so free iscsi_cmd in this case.

Andy Grover 554617b2bb target/user: Return an error if cmd data size is too large
Userspace should be implementing VPD B0 (Block Limits) to inform the
initiator of max data size, but just in case we do get a too-large request,
do what the spec says and return INVALID_CDB_FIELD.

Make sure to unlock udev->cmdr_lock before returning.

Allocate resources dynamically to cxgb4's Upper layer driver's(ULD) like
cxgbit, iw_cxgb4 and cxgb4i. Allocate resources when they register with
cxgb4 driver and free them while unregistering. All the queues and the
interrupts for them will be allocated during ULD probe only and freed
during remove.

Varun Prakash b8b9d81b36 libcxgb: add library module for Chelsio drivers
Add common library module(libcxgb.ko) for
Chelsio drivers to remove duplicate code.

Code for iSCSI DDP Page Pod Manager is moved
from cxgb4.ko to libcxgb.ko. Earlier only cxgbit.ko
was using this code, now cxgb3i and cxgb4i will
also use common Page Pod manager code.

In future this module will have common connection
management and hardware specific code that can be
shared by multiple Chelsio drivers.

Sumit Rai c1ccbfe031 target: Fix residual overflow handling in target_complete_cmd_with_length
This patch fixes residual overflow handling to correctly set the
residual_count using SPDTL, instead of SCSI Allocation Length.

Allocation Length is the maximum value of the SPDTL and not substitute
for it, hence it shouldn’t be used to calculate ResidualCount except for
cases where SPDTL > Allocation Length and Data is truncated (in that
case both Alloc Len and SPDTL are same). (SPC 5r01 Section 4.2.5.6).

Feng Li 8abc718de6 iscsi-target: Fix panic when adding second TCP connection to iSCSI session
In MC/S scenario, the conn->sess has been set NULL in
iscsi_login_non_zero_tsih_s1 when the second connection comes here,
then kernel panic.

The conn->sess will be assigned in iscsi_login_non_zero_tsih_s2. So
we should check whether it's NULL before calling.

Nicholas Bellinger 410c29dfbf target: Fix ordered task CHECK_CONDITION early exception handling
If a Simple command is sent with a failure, target_setup_cmd_from_cdb
returns with TCM_UNSUPPORTED_SCSI_OPCODE or TCM_INVALID_CDB_FIELD.

So in the cases where target_setup_cmd_from_cdb returns an error, we
never get far enough to call target_execute_cmd to increment simple_cmds.
Since simple_cmds isn't incremented, the result of the failure from
target_setup_cmd_from_cdb causes transport_generic_request_failure to
decrement simple_cmds, due to call to transport_complete_task_attr.

With this dev->simple_cmds or dev->dev_ordered_sync is now -1, not 0.
So when a subsequent command with an Ordered Task is sent, it causes
a hang, since dev->simple_cmds is at -1.

Nicholas Bellinger dff0ca9ea7 target: Fix ordered task target_setup_cmd_from_cdb exception hang
If a command with a Simple task attribute is failed due to a Unit
Attention, then a subsequent command with an Ordered task attribute
will hang forever.  The reason for this is that the Unit Attention
status is checked for in target_setup_cmd_from_cdb, before the call
to target_execute_cmd, which calls target_handle_task_attr, which
in turn increments dev->simple_cmds.

However, transport_generic_request_failure still calls
transport_complete_task_attr, which will decrement dev->simple_cmds.
In this case, simple_cmds is now -1.  So when a command with the
Ordered task attribute is sent, target_handle_task_attr sees that
dev->simple_cmds is not 0, so it decides it can't execute the
command until all the (nonexistent) Simple commands have completed.

Mike Christie ea263c7fad target: Fix max_unmap_lba_count calc overflow
max_discard_sectors only 32bits, and some non scsi backend
devices will set this to the max 0xffffffff, so we can end up
overflowing during the max_unmap_lba_count calculation.

This fixes a regression caused by my patch:

Nicholas Bellinger 064cdd2d91 target: Fix race between iscsi-target connection shutdown + ABORT_TASK
This patch fixes a race in iscsit_release_commands_from_conn() ->
iscsit_free_cmd() -> transport_generic_free_cmd() + wait_for_tasks=1,
where CMD_T_FABRIC_STOP could end up being set after the final
kref_put() is called from core_tmr_abort_task() context.

This results in transport_generic_free_cmd() blocking indefinately
on se_cmd->cmd_wait_comp, because the target_release_cmd_kref()
check for CMD_T_FABRIC_STOP returns false.

To address this bug, make iscsit_release_commands_from_conn()
do list_splice and set CMD_T_FABRIC_STOP early while holding
iscsi_conn->cmd_lock.  Also make iscsit_aborted_task() only
remove iscsi_cmd_t if CMD_T_FABRIC_STOP has not already been
set.

Finally in target_release_cmd_kref(), only honor fabric_stop
if CMD_T_ABORTED has been set.

Nicholas Bellinger 5e2c956b8a target: Fix missing complete during ABORT_TASK + CMD_T_FABRIC_STOP
During transport_generic_free_cmd() with a concurrent TMR
ABORT_TASK and shutdown CMD_T_FABRIC_STOP bit set, the
caller will be blocked on se_cmd->cmd_wait_stop completion
until the final kref_put() -> target_release_cmd_kref()
has been invoked to call complete().

However, when ABORT_TASK is completed with FUNCTION_COMPLETE
in core_tmr_abort_task(), the aborted se_cmd will have already
been removed from se_sess->sess_cmd_list via list_del_init().

This results in target_release_cmd_kref() hitting the
legacy list_empty() == true check, invoking ->release_cmd()
but skipping complete() to wakeup se_cmd->cmd_wait_stop
blocked earlier in transport_generic_free_cmd() code.

To address this bug, it's safe to go ahead and drop the
original list_empty() check so that fabric_stop invokes
the complete() as expected, since list_del_init() can
safely be used on a empty list.

Nicholas Bellinger 8f0dfb3d8b iscsi-target: Fix early sk_data_ready LOGIN_FLAGS_READY race
There is a iscsi-target/tcp login race in LOGIN_FLAGS_READY
state assignment that can result in frequent errors during
iscsi discovery:

      "iSCSI Login negotiation failed."

To address this bug, move the initial LOGIN_FLAGS_READY
assignment ahead of iscsi_target_do_login() when handling
the initial iscsi_target_start_negotiation() request PDU
during connection login.

As iscsi_target_do_login_rx() work_struct callback is
clearing LOGIN_FLAGS_READ_ACTIVE after subsequent calls
to iscsi_target_do_login(), the early sk_data_ready
ahead of the first iscsi_target_do_login() expects
LOGIN_FLAGS_READY to also be set for the initial
login request PDU.

As reported by Maged, this was first obsered using an
MSFT initiator running across multiple VMWare host
virtual machines with iscsi-target/tcp.

Nicholas Bellinger d4b3fa4b08 iscsi-target: Make iscsi_tpg_np driver show/store use generic code
Go ahead and fold the duplicate iscsi_tpg_np driver attribute
show/store functions into generic callers, invoked by current
driver specific functions.

This allows for future v4.8+ changes to have iscsi_target_mod
use tpg_np driver attributes at runtime, instead of having
the defaults hardcoded in iscsi_target_configfs.c.

Also, drop the unused/legacy 'sctp' attribute for non standard
RFC-3720 operation with IPPROTO_SCTP.

Christoph Hellwig e64aa657c3 target: enhance and export target_alloc_sgl/target_free_sgl
The SRP target driver will need to allocate and chain it's own SGLs soon.
For this export target_alloc_sgl, and add a new argument to it so that it
can allocate an additional chain entry that doesn't point to a page.  Also
export transport_free_sgl after renaming it to target_free_sgl to free
these SGLs again.

Imran Haider 2b16509c5f iscsi-target: graceful disconnect on invalid mapping to iovec
Ensures the first page entry is within bounds. A failed check would
terminate the iSCSI connection instead of causing a NULL-dereference.

This violation seems to happen with certain iSCSI commands where the
computed CDB length is zero but the expected transfer length is
non-zero. The real problem is probably on the iSCSI initiator side
since there is a discrepancy between the iSCSI header and the
encapsulated CDB Opcode.

Colin Ian King cb79116947 target: need_to_release is always false, remove redundant check and kfree
The removal the #ifdef'd code on CONFIG_ARCH_HAS_SG_CHAIN in
commit 02c4de53ad ("target/rd: always chain S/G list")
removed the setting of need_to_release to true, so we are now left
with need_to_release always being false and hence the conditional
kfree is never reached.  Clean up the dead code by removing need_to_release
and the conditional kfree.

Christoph Hellwig 44f33d0fb5 iscsi-target: remove usage of ->shutdown_session
->shutdown session only decideѕ if the target core calls ->close_session
directly, or if the fabrics drivers calls it manually later through
target_put_session, which at this point will always close the session as
it has been removed from the lookup list and thus no new references will
be acquired from the core.

So instead remove ->shutdown and have the core call ->close_session
directly, and replace all calls to target_put_session in the iSCSI target
with direct calls to iscsit_close_session.

Christoph Hellwig 22d11759a4 target: make ->shutdown_session optional
Turns out the template and thus many drivers got the return value wrong:
0 means the fabrics driver needs to put a session reference, which no
driver except for the iSCSI target drivers did.  Fortunately none of these
drivers supports explicit Node ACLs, so the bug was harmless.

Even without that only qla2xxx and iscsi every did real work in
shutdown_session, so get rid of the boilerplate code in all other
drivers.

Lee Duncan a96e9783e0 target: make target db location configurable
This commit adds the read-write attribute "dbroot",
in the top-level CONFIGFS (core) target directory,
normally /sys/kernel/config/target. This attribute
defaults to "/var/target" but can be changed by
writing a new pathname string to it. Changing this
attribute is only allowed when no fabric drivers
are loaded and the supplied value specifies an
existing directory.

Target modules that care about the target database
root directory will be modified to use this
attribute in a future commit.

Himanshu Madhani 5e47f1985d target: Fix target_release_cmd_kref shutdown comp leak
This patch fixes an active I/O shutdown bug for fabric
drivers using target_wait_for_sess_cmds(), where se_cmd
descriptor shutdown would result in hung tasks waiting
indefinitely for se_cmd->cmd_wait_comp to complete().

To address this bug, drop the incorrect list_del_init()
usage in target_wait_for_sess_cmds() and always complete()
during se_cmd target_release_cmd_kref() put, in order to
let caller invoke the final fabric release callback
into se_cmd->se_tfo->release_cmd() code.

Nicholas Bellinger 4347ab5a08 target: Avoid DataIN transfers for non-GOOD SAM status
This patch modifies existing transport_complete_*() code
to avoid invoking target_core_fabric_ops->queue_data_in()
driver callbacks for I/O READs with non-GOOD SAM status.

Some initiators expect GOOD status when a DATA-IN payload
transfer is involved, so to be safe go ahead and always
invoke target_core_fabric_ops->queue_status() to generate
fabric responses instead.

Note this is a prerequisite for IBLOCK supporting retriable
status, so SAM_STAT_BUSY + SAM_STAT_TASK_SET_FULL always
generates fabric driver responses instead of initiating
DataIN payload transfer when non-GOOD status is present

Sheng Yang 26418649ee target/user: Introduce data_bitmap, replace data_length/data_head/data_tail
The data_bitmap was introduced to support asynchornization accessing of
data area.

We divide mailbox data area into blocks, and use data_bitmap to track the
usage of data area. All the new command's data would start with a new block,
and may left unusable space after it end. But it's easy to track using
data_bitmap.

Now we can allocate data area for asynchronization accessing from userspace,
since we can track the allocation using data_bitmap. The userspace part would
be the same as Maxim's previous asynchronized implementation.

Andy Grover 03a68b44fa target: Remove enum transport_lunflags_table
se_dev_entry.lun_flags and se_lun.lun_access are only used for keeping
track of read-write vs. read-only state. Since this is an either/or thing
we can represent it as bool, and remove the unneeded enum
transport_lunflags_table, which is left over from when there were more
flags.

Change code that uses this enum to just use true/false, and make it clear
through variable and param names that true means read-only, false means
read-write.

done for UNMAP/discards, except that we export a large max write same
value to the initiator, and then rely on the block layer to

Nicholas Bellinger 7f54ab5ff5 target: Drop incorrect ABORT_TASK put for completed commands
This patch fixes a recent ABORT_TASK regression associated
with commit febe562c, where a left-over target_put_sess_cmd()
would still be called when __target_check_io_state() detected
a command has already been completed, and explicit ABORT must
be avoided.

Note commit febe562c dropped the local kref_get_unless_zero()
check in core_tmr_abort_task(), but did not drop this extra
corresponding target_put_sess_cmd() in the failure path.

So go ahead and drop this now bogus target_put_sess_cmd(),
and avoid this potential use-after-free.

Arnd Bergmann 0633e12346 target/user: Fix cast from pointer to phys_addr_t
The uio_mem structure has a member that is a phys_addr_t, but can
be a number of other types too. The target core driver attempts
to assign a pointer from vmalloc() to it, by casting it to
phys_addr_t, but that causes a warning when phys_addr_t is longer
than a pointer:

This adds another cast to uintptr_t to shut up the warning.
A nicer fix might be to have additional fields in uio_mem
for the different purposes, so we can assign a pointer directly.

Nicholas Bellinger 310d3d314b target: Fix race with SCF_SEND_DELAYED_TAS handling
This patch fixes a race between setting of SCF_SEND_DELAYED_TAS
in transport_send_task_abort(), and check of the same bit in
transport_check_aborted_status().

It adds a __transport_check_aborted_status() version that is
used by target_execute_cmd() when se_cmd->t_state_lock is
held, and a transport_check_aborted_status() wrapper for
all other existing callers.

Also, it handles the case where the check happens before
transport_send_task_abort() gets called.  For this, go
ahead and set SCF_SEND_DELAYED_TAS early when necessary,
and have transport_send_task_abort() send the abort.

Nicholas Bellinger 0f4a943168 target: Fix remote-port TMR ABORT + se_cmd fabric stop
To address the bug where fabric driver level shutdown
of se_cmd occurs at the same time when TMR CMD_T_ABORTED
is happening resulting in a -1 ->cmd_kref, this patch
adds a CMD_T_FABRIC_STOP bit that is used to determine
when TMR + driver I_T nexus shutdown is happening
concurrently.

It changes target_sess_cmd_list_set_waiting() to obtain
se_cmd->cmd_kref + set CMD_T_FABRIC_STOP, and drop local
reference in target_wait_for_sess_cmds() and invoke extra
target_put_sess_cmd() during Task Aborted Status (TAS)
when necessary.

Also, it adds a new target_wait_free_cmd() wrapper around
transport_wait_for_tasks() for the special case within
transport_generic_free_cmd() to set CMD_T_FABRIC_STOP,
and is now aware of CMD_T_ABORTED + CMD_T_TAS status
bits to know when an extra transport_put_cmd() during
TAS is required.

Note transport_generic_free_cmd() is expected to block on
cmd->cmd_wait_comp in order to follow what iscsi-target
expects during iscsi_conn context se_cmd shutdown.
