When a PCI device is transferred to another domain and it is still
in usage (from the internal perspective), mention which other
domain is using it to aid in debugging.

The full warning is:
"pciback 0000:05:00.0: device has been assigned to 2 domain! Over-writting the ownership, but beware."
which is correct - the previous domain that was using the device
forgot to unregister the ownership. This patch fixes this by
calling the unregister ownership function when the PCI device is
relinquished from the guest domain.

this we could have potentially unloaded the vf driver from the
xen pciback control even if the driver was binded to the xen-pciback.

This resolves the conflict in the arch/arm/mach-s3c64xx/s3c6400.c file,
and it fixes the build error in the arch/x86/kernel/microcode_core.c
file, that the merge did not catch.
The microcode_core.c patch was provided by Stephen Rothwell
<sfr@canb.auug.org.au> who was invaluable in the merge issues involved
with the large sysdev removal process in the driver-core tree.

Use this now that it is defined even though it happens to be == PAGE_SIZE.
The code which takes requests from userspace already validates against the size
of this buffer so no further checks are required to ensure that userspace
requests comply with the protocol in this respect.

Haogang Chen found out that:
There is a potential integer overflow in process_msg() that could result
in cross-domain attack.
body = kmalloc(msg->hdr.len + 1, GFP_NOIO | __GFP_HIGH);
When a malicious guest passes 0xffffffff in msg->hdr.len, the subsequent
call to xb_read() would write to a zero-length buffer.
The other end of this connection is always the xenstore backend daemon
so there is no guest (malicious or otherwise) which can do this. The
xenstore daemon is a trusted component in the system.
However this seem like a reasonable robustness improvement so we should
have it.
And Ian when read the API docs found that:
The payload length (len field of the header) is limited to 4096
(XENSTORE_PAYLOAD_MAX) in both directions. If a client exceeds the
limit, its xenstored connection will be immediately killed by
xenstored, which is usually catastrophic from the client's point of
view. Clients (particularly domains, which cannot just reconnect)
should avoid this.
so this patch checks against that instead.
This also avoids a potential integer overflow pointed out by Haogang Chen.

The 'name', 'owner', and 'mod_name' members are redundant with the
identically named fields in the 'driver' sub-structure. Rather than
switching each instance to specify these fields explicitly, introduce
a macro to simplify this.
Eliminate further redundancy by allowing the drvname argument to
DEFINE_XENBUS_DRIVER() to be blank (in which case the first entry from
the ID table will be used for .driver.name).
Also eliminate the questionable xenbus_register_{back,front}end()
wrappers - their sole remaining purpose was the checking of the
'owner' field, proper setting of which shouldn't be an issue anymore
when the macro gets used.

Error handling code following a kmalloc should free the allocated data.
Out_unlock is used on both success and failure, so free vm_priv before
jumping to that label.

Add support for mappings without GNTMAP_contains_pte. This was not
supported because the unmap operation assumed that this flag was being
used; adding a parameter to the unmap operation to allow the PTE
clearing to be disabled is sufficient to make unmap capable of
supporting either mapping type.

For xenbus_{map,unmap}_ring to work on HVM, the grant table operations
must be set up using the gnttab_set_{map,unmap}_op functions instead of
directly populating the fields of gnttab_map_grant_ref. These functions
simply populate the structure on paravirtualized Xen; however, on HVM
they must call __pa() on vaddr when populating op->host_addr because the
hypervisor cannot directly interpret guest-virtual addresses.

Add HVM implementations of xenbus_(map,unmap)_ring_v(alloc,free) so
that ring mappings can be done without using GNTMAP_contains_pte which
is not supported on HVM. This also removes the need to use vmlist_lock
on PV by tracking the allocated xenbus rings.

When a multi-page mapping of gntalloc is created, the reference counts
of all pages in the vma are incremented. However, the vma open/close
operations only adjusted the reference count of the first page in the
mapping, leaking the other pages. Store a struct in the vm_private_data
to track the original page count to properly free the pages when the
last reference to the vma is closed.

gnttab_end_foreign_access_ref does not return the grant reference it is
passed to the free list; gnttab_free_grant_reference needs to be
explicitly called. While gnttab_end_foreign_access provides a wrapper
for this, it is unsuitable because it does not return errors.

These allow a domain A which has been granted access on a page of domain B's
memory to issue domain C with a copy-grant on the same page. This is useful
e.g. for forwarding packets between domains.

- They can't be used to map the page (so can only be used in a GNTTABOP_copy
hypercall).
- It's possible to grant access with a finer granularity than whole pages.
- Xen guarantees that they can be revoked quickly (a normal map grant can
only be revoked with the cooperation of the domain which has been granted
access).

This fixes an odd bug found on a Dell PowerEdge 1850/0RC130
(BIOS A05 01/09/2006) where all of the modules doing pci_set_dma_mask
would fail with:
ata_piix 0000:00:1f.1: enabling device (0005 -> 0007)
ata_piix 0000:00:1f.1: can't derive routing for PCI INT A
ata_piix 0000:00:1f.1: BMDMA: failed to set dma mask, falling back to PIO
The issue was the Xen-SWIOTLB was allocated such as that the end of
buffer was stradling a page (and also above 4GB). The fix was
spotted by Kalev Leonid which was to piggyback on git commit
e79f86b2ef "swiotlb: Use page alignment
for early buffer allocation" which:
We could call free_bootmem_late() if swiotlb is not used, and
it will shrink to page alignment.
So alloc them with page alignment at first, to avoid lose two pages
And doing that fixes the outstanding issue.

Receiver-side copying of packets is based on this implementation, it gives
better performance and better CPU accounting. It totally supports three types:
full-page, sub-page and transitive grants.
However this patch does not cover sub-page and transitive grants, it mainly
focus on Full-page part and implements grant table V2 interfaces corresponding
to what already exists in grant table V1, such as: grant table V2
initialization, mapping, releasing and exported interfaces.
Each guest can only supports one type of grant table type, every entry in grant
table should be the same version. It is necessary to set V1 or V2 version before
initializing the grant table.
Grant table exported interfaces of V2 are same with those of V1, Xen is
responsible to judge what grant table version guests are using in every grant
operation.
V2 fulfills the same role of V1, and it is totally backwards compitable with V1.
If dom0 support grant table V2, the guests runing on it can run with either V1
or V2.

This patch introduces new structures of grant table V2, grant table V2 is an
extension from V1. Grant table is shared between guest and Xen, and Xen is
responsible to do corresponding work for grant operations, such as: figure
out guest's grant table version, perform different actions based on
different grant table version, etc. Although full-page structure of V2
is different from V1, it play the same role as V1.

Event channels exposed to userspace by the evtchn module may be used by
other modules in an asynchronous manner, which requires that reference
counting be used to prevent the event channel from being closed before
the signals are delivered.
The reference count on new event channels defaults to -1 which indicates
the event channel is not referenced outside the kernel; evtchn_get fails
if called on such an event channel. The event channels made visible to
userspace by evtchn have a normal reference count.

gref->gref_id is unsigned so the error handling didn't work.
gnttab_grant_foreign_access() returns an int type, so we can add a
cast here, and it doesn't cause any problems.
gnttab_grant_foreign_access() can return a variety of errors
including -ENOSPC, -ENOSYS and -ENOMEM.

On 32 bit systems a high value of op.count could lead to an integer
overflow in the kzalloc() and gref_ids would be smaller than
expected. If the you triggered another integer overflow in
"if (gref_size + op.count > limit)" then you'd probably get memory
corruption inside add_grefs().

If highmem pages are requested from the balloon on a system without
highmem, the implementation of alloc_xenballooned_pages will allocate
all available memory trying to find highmem pages to return. Allow
low memory to be returned when highmem pages are requested to avoid
this loop.

When mapping a foreign page with xenbus_map_ring_valloc() with the
GNTTABOP_map_grant_ref hypercall, set the GNTMAP_contains_pte flag and
pass a pointer to the PTE (in init_mm).
After the page is mapped, the usual fault mechanism can be used to
update additional MMs. This allows the vmalloc_sync_all() to be
removed from alloc_vm_area().

.. we check whether 'xdev' is NULL - but there is no need for
it as the 'dev' check is done before. The 'dev' is embedded in
the 'xdev' so having xdev != NULL with dev being being checked
is not going to happen.

There are three different modes: PV, HVM, and initial domain 0. In all
the cases we would return -1 for failure instead of a proper error code.
Fix this by propagating the error code from the generic IRQ code.

The list operation checks whether the 'info' structure that is
retrieved from the list is NULL (otherwise it would not been able
to retrieve it). This check is not neccessary.

This adds a mechanism to resume selected IRQs during syscore_resume
instead of dpm_resume_noirq.
Under Xen we need to resume IRQs associated with IPIs early enough
that the resched IPI is unmasked and we can therefore schedule
ourselves out of the stop_machine where the suspend/resume takes
place.
This issue was introduced by 676dc3cf5b "xen: Use IRQF_FORCE_RESUME".

The balloon driver's "current_pages" is very different from
totalram_pages. Self-ballooning needs to be driven by
the latter. Also, Committed_AS doesn't account for pages
used by the kernel so:
1) Add totalreserve_pages to Committed_AS for the normal target.
2) Enforce a floor for when there are little or no user-space threads
using memory (e.g. single-user mode) to avoid OOMs. The floor
function includes a "min_usable_mb" tuneable in case we discover
later that the floor function is still too aggressive in some
workloads, though likely it will not be needed.
Changes since version 4:
- change floor calculation so that it is not as aggressive; this version
uses a piecewise linear function similar to minimum_target in the 2.6.18
balloon driver, but modified to add to totalreserve_pages instead of
subtract from max_pfn, the 2.6.18 version causes OOMs on recent kernels
because the kernel has expanded over time
- change safety_margin to min_usable_mb and comment on its use
- since committed_as does NOT include kernel space (and other reserved
pages), totalreserve_pages is now added to committed_as. The result is
less aggressive self-ballooning, but theoretically more appropriate.

gntdev_put_map tries to acquire a mutex when freeing pages back to the
xenballoon pool, so it cannot be called with a spinlock held. In
gntdev_release, the spinlock is not needed as we are freeing the
structure later; in the ioctl, only the list manipulation needs to be
under the lock.

The xenstore daemon does not have to run in the xen initial domain;
however, Linux currently uses xen_initial_domain to test if a loopback
event channel should be used instead of the event channel provided in
Xen's start_info structure. Instead, if the event channel passed in the
start_info structure is not valid, assume that this domain will run
xenstored locally and set up the event channel.

The xenbus event channel established in xenbus_init is intended to be a
loopback channel, but the remote domain was hardcoded to 0; this will
cause the channel to be unusable when xenstore is not being run in
domain 0.

Allow the xen balloon driver to populate its list of extra pages from
more than one region of memory. This will allow platforms to provide
(for example) a region of low memory and a region of high memory.
The maximum possible number of extra regions is 128 (== E820MAX) which
is quite large so xen_extra_mem is placed in __initdata. This is safe
as both xen_memory_setup() and balloon_init() are in __init.
The balloon regions themselves are not altered (i.e., there is still
only the one region).

In xen_memory_setup() pages that occur in gaps in the memory map are
released back to Xen. This reduces the domain's current page count in
the hypervisor. The Xen balloon driver does not correctly decrease
its initial current_pages count to reflect this. If 'delta' pages are
released and the target is adjusted the resulting reservation is
always 'delta' less than the requested target.
This affects dom0 if the initial allocation of pages overlaps the PCI
memory region but won't affect most domU guests that have been setup
with pseudo-physical memory maps that don't have gaps.
Fix this by accouting for the released pages when starting the balloon
driver.
If the domain's targets are managed by xapi, the domain may eventually
run out of memory and die because xapi currently gets its target
calculations wrong and whenever it is restarted it always reduces the
target by 'delta'.

Xen PVHVM needs xen-platform-pci, on the other hand xen-platform-pci is
useless in any other cases.
Therefore remove the XEN_PLATFORM_PCI config option and compile
xen-platform-pci built-in if XEN_PVHVM is selected.

If we want to use granted pages for AIO, changing the mappings of a user
vma and the corresponding p2m is not enough, we also need to update the
kernel mappings accordingly.
Currently this is only needed for pages that are created for user usages
through /dev/xen/gntdev. As in, pages that have been in use by the
kernel and use the P2M will not need this special mapping.
However there are no guarantees that in the future the kernel won't
start accessing pages through the 1:1 even for internal usage.
In order to avoid the complexity of dealing with highmem, we allocated
the pages lowmem.
We issue a HYPERVISOR_grant_table_op right away in
m2p_add_override and we remove the mappings using another
HYPERVISOR_grant_table_op in m2p_remove_override.
Considering that m2p_add_override and m2p_remove_override are called
once per page we use multicalls and hypercall batching.
Use the kmap_op pointer directly as argument to do the mapping as it is
guaranteed to be present up until the unmapping is done.
Before issuing any unmapping multicalls, we need to make sure that the
mapping has already being done, because we need the kmap->handle to be
set correctly.

Device drivers that create and destroy SR-IOV virtual functions via
calls to pci_enable_sriov() and pci_disable_sriov can cause catastrophic
failures if they attempt to destroy VFs while they are assigned to
guest virtual machines. By adding a flag for use by the Xen PCI back
to indicate that a device is assigned a device driver can check that
flag and avoid destroying VFs while they are assigned and avoid system
failures.

Add new xs_reset_watches function to shutdown watches from old kernel after
kexec boot. The old kernel does not unregister all watches in the
shutdown path. They are still active, the double registration can not
be detected by the new kernel. When the watches fire, unexpected events
will arrive and the xenwatch thread will crash (jumps to NULL). An
orderly reboot of a hvm guest will destroy the entire guest with all its
resources (including the watches) before it is rebuilt from scratch, so
the missing unregister is not an issue in that case.
With this change the xenstored is instructed to wipe all active watches
for the guest. However, a patch for xenstored is required so that it
accepts the XS_RESET_WATCHES request from a client (see changeset
23839:42a45baf037d in xen-unstable.hg). Without the patch for xenstored
the registration of watches will fail and some features of a PVonHVM
guest are not available. The guest is still able to boot, but repeated
kexec boots will fail.

Now that the hypercall interface changes are in -unstable, make the
kernel side code not ignore the segment (aka domain) number anymore
(which results in pretty odd behavior on such systems). Rather, if
only the old interfaces are available, don't call them for devices on
non-zero segments at all.

The caller that orchestrates the state changes is xenwatch_thread

To accommodate the call to the callback function from
__xen_pcibk_publish_pci_roots(), which so far dropped and the re-
acquired the lock without checking that the list didn't actually
change, convert the code to use a mutex instead (observing that the
code taking the lock won't ever get called from non-sleepable
context).
As a result, drop the bogus use of list_for_each_entry_safe() and
remove the inappropriate dropping of the lock.

After triggering a crash dump in a HVM guest, the PV backend drivers
will remain in Connected state. When the kdump kernel starts the PV
drivers will skip such devices. As a result, no root device is found and
the vmcore cant be saved.
A similar situation happens after a kexec boot, here the devices will be
in the Closed state.
With this change all frontend devices with state XenbusStateConnected or
XenbusStateClosed will be reset by changing the state file to Closing ->
Closed -> Initializing. This will trigger a disconnect in the backend
drivers. Now the frontend drivers will find the backend drivers in state
Initwait and can connect.

During a kexec boot some virqs such as timer and debugirq were already
registered by the old kernel. The hypervisor will return -EEXISTS from
the new EVTCHNOP_bind_virq request and the BUG in bind_virq_to_irq()
triggers. Catch the -EEXISTS error and loop through all possible ports to find
what port belongs to the virq/cpu combo.

During repeated kexec boots xenwatch_thread() can crash because
xenbus_watch->callback is cleared by xenbus_watch_path() if a node/token
combo for a new watch happens to match an already registered watch from
an old kernel. In this case xs_watch returns -EEXISTS, then
register_xenbus_watch() does not remove the to-be-registered watch from
the list of active watches but returns the -EEXISTS to the caller
anyway.
Because the watch is still active in xenstored it will cause an event
which will arrive in the new kernel. process_msg() will find the
encapsulated struct xenbus_watch in its list of registered watches and
puts the "empty" watch handle in the queue for xenwatch_thread().
xenwatch_thread() then calls ->callback which was cleared earlier by
xenbus_watch_path().
To prevent that crash in a guest running on an old xen toolstack remove
the special -EEXIST handling.

The process to swizzle a Machine Frame Number (MFN) is not always
necessary. Especially if we know that we actually do not have to do it.
In this patch we check the MFN against the device's coherent
DMA mask and if the requested page(s) are contingous. If it all checks
out we will just return the bus addr without doing the memory
swizzle.

Notifier functions are expected to return NOTIFY_* codes, not -E...
ones. In particular, since the respective hypercalls failing is not
fatal to the operation of the Dom0 kernel, it must be avoided to
return negative values here as those would make it appear as if
NOTIFY_STOP_MASK wa set, suppressing further notification calls to
other interested parties (which is also why we don't want to use
notifier_from_errno() here).
While at it, also notify the user of a failed hypercall.

Propagate the baremetal git commit "swiotlb: fix wrong panic"
(fba99fa38b) in the Xen-SWIOTLB version.
wherein swiotlb's map_page wrongly calls panic() when it can't find
a buffer fit for device's dma mask. It should return an error instead.
Devices with an odd dma mask (i.e. under 4G) like b44 network card hit
this bug (the system crashes):

We can fail seting up Xen-SWIOTLB if:
- The host does not have enough contiguous DMA32 memory available
(can happen on a machine that has fragmented memory from starting,
stopping many guests).
- Not enough low memory (almost never happens).
We retry allocating and exchanging the swath of contiguous memory
up to three times. Each time we decrease the amount we need - the
minimum being of 2MB.
If we compleltly fail, we will print the reason for failure on the Xen
console on top of doing it to earlyprintk=xen console.

Memory hotplug support for Xen balloon driver. It should be mentioned
that hotplugged memory is not onlined automatically. It should be onlined
by user through standard sysfs interface.
Memory could be hotplugged in following steps:
1) dom0: xl mem-max <domU> <maxmem>
where <maxmem> is >= requested memory size,
2) dom0: xl mem-set <domU> <memory>
where <memory> is requested memory size; alternatively memory
could be added by writing proper value to
/sys/devices/system/xen_memory/xen_memory0/target or
/sys/devices/system/xen_memory/xen_memory0/target_kb on dumU,
3) domU: for i in /sys/devices/system/memory/memory*/state; do \

Memory could be onlined automatically on domU by adding following line to
udev rules:

- Remove the slot and controller controller backend as they
are not used.
- Document the find pciback_[read|write]_config_[byte|word|dword]
to make it easier to find.
- Collapse the code from conf_space_capability_msi into pciback_ops.c
- Collapse conf_space_capability_[pm|vpd].c in conf_space_capability.c
[and remove the conf_space_capability.h file]
- Rename all visible functions from pciback to xen_pcibk.
- Rename all the printk/pr_info, etc that use the "pciback" to say
"xen-pciback".
- Convert functions that are not referenced outside the code to be
static to save on name space.
- Do the same thing for structures that are internal to the driver.
- Run checkpatch.pl after the renames and fixup its warnings and

If we try to setup an fake IRQ handler for legacy interrupts
for devices that only have MSI-X (most if not all SR-IOV cards),
we will fail with this:
pciback[0000:01:10.0]: failed to install fake IRQ handler for IRQ 0! (rc:-38)
Since those cards don't have anything in dev->irq.

We were using coarse spinlocks that could end up with a deadlock.
This patch fixes that and makes the spinlocks much more fine-grained.
We also drop be->watchding state spinlocks as they are already
guarded by the xenwatch_thread against multiple customers. Without
that we would trigger the BUG: scheduling while atomic.

If the device that is to be shared with a guest is a level device and
the IRQ is shared with the initial domain we need to take actions.
Mainly we install a dummy IRQ handler that will ACK on the interrupt
line so as to not have the initial domain disable the interrupt line.
This dummy IRQ handler is not enabled when the device MSI/MSI-X lines
are set, nor for edge interrupts. And also not for level interrupts
that are not shared amongst devices. Lastly, if the user passes
to the guest all of the PCI devices on the shared line the we won't
install the dummy handler either.
There is also SysFS instrumentation to check its state and turn
IRQ ACKing on/off if necessary.

In cases where the guest is abruptly killed and has not disabled
MSI/MSI-X interrupts we want to do it for it.
Otherwise when the guest is started up and enables MSI, we would
get a WARN() that the device already had been enabled.

These changes are for PV guest to use Virtual Function. Because the VF's
vendor, device registers in cfg space are 0xffff, which are invalid and
ignored by PCI device scan. Values in 'struct pci_dev' are fixed up by
SR-IOV code, and using these values will present correct VID and DID to
PV guest kernel.
And command registers in the cfg space are read only 0, which means we
have to emulate MMIO enable bit (VF only uses MMIO resource) so PV
kernel can work properly.

When the front-end and back-end start negotiating we register
the domain that will use the PCI device. Furthermore during shutdown
of guest or unbinding of the PCI device (and unloading of module)
from pciback we unregister the domain owner.

This is the host side counterpart to the frontend driver in
drivers/pci/xen-pcifront.c. The PV protocol is also implemented by
frontend drivers in other OSes too, such as the BSDs.
The PV protocol is rather simple. There is page shared with the guest,
which has the 'struct xen_pci_sharedinfo' embossed in it. The backend
has a thread that is kicked every-time the structure is changed and
based on the operation field it performs specific tasks:
XEN_PCI_OP_conf_[read|write]:
Read/Write 0xCF8/0xCFC filtered data. (conf_space*.c)
Based on which field is probed, we either enable/disable the PCI
device, change power state, read VPD, etc. The major goal of this
call is to provide a Physical IRQ (PIRQ) to the guest.
The PIRQ is Xen hypervisor global IRQ value irrespective of the IRQ
is tied in to the IO-APIC, or is a vector. For GSI type
interrupts, the PIRQ==GSI holds. For MSI/MSI-X the
PIRQ value != Linux IRQ number (thought PIRQ==vector).
Please note, that with Xen, all interrupts (except those level shared ones)
are injected directly to the guest - there is no host interaction.
XEN_PCI_OP_[enable|disable]_msi[|x] (pciback_ops.c)
Enables/disables the MSI/MSI-X capability of the device. These operations
setup the MSI/MSI-X vectors for the guest and pass them to the frontend.
When the device is activated, the interrupts are directly injected in the
guest without involving the host.
XEN_PCI_OP_aer_[detected|resume|mmio|slotreset]: In case of failure,
perform the appropriate AER commands on the guest. Right now that is
a cop-out - we just kill the guest.
Besides implementing those commands, it can also
- hide a PCI device from the host. When booting up, the user can specify
xen-pciback.hide=(1:0:0)(BDF..) so that host does not try to use the
device.
The driver was lifted from linux-2.6.18.hg tree and fixed up
so that it could compile under v3.0. Per suggestion from Jesse Barnes
moved the driver to drivers/xen/xen-pciback.

In the past (2.6.38) the 'xen_allocate_pirq_gsi' would allocate
an entry in a Linux IRQ -> {XEN_IRQ, type, event, ..} array. All
of that has been removed in 2.6.39 and the Xen IRQ subsystem uses
an linked list that is populated when the call to
'xen_allocate_irq_gsi' (universally done from any of the xen_bind_*
calls) is done. The 'xen_allocate_pirq_gsi' is a NOP and there is
no need for it anymore so lets remove it.

This patch introduces two in-kernel drivers for Xen transcendent memory
("tmem") functionality that complement cleancache and frontswap. Both
use control theory to dynamically adjust and optimize memory utilization.
Selfballooning controls the in-kernel Xen balloon driver, targeting a goal
value (vm_committed_as), thus pushing less frequently used clean
page cache pages (through the cleancache code) into Xen tmem where
Xen can balance needs across all VMs residing on the physical machine.
Frontswap-selfshrinking controls the number of pages in frontswap,
driving it towards zero (effectively doing a partial swapoff) when
in-kernel memory pressure subsides, freeing up RAM for other VMs.

The xenbus bus type uses device_create_file to assign all used device
attributes. However it does not remove them when the device goes away.
This patch uses the dev_attrs field of the bus type to specify default
attributes for all devices.

the 'swiotlb=<nslabs>' the default value is ignored and
the value provided by the user is used... Except when the SWIOTLB
is used under Xen - there the default value of 64MB is used and
the Xen-SWIOTLB has no mechanism to get the 'io_tlb_nslabs' filled
out by setup_io_tlb_npages functions. This patch provides a function
for the Xen-SWIOTLB to call to see if the io_tlb_nslabs is set
and if so use that value.

Do not use pirq_needs_eoi to decide which irq handler to use because Xen
always returns true if the guest does not support pirq_eoi_map.
Use the trigger information we already have from MP-tables and ACPI.

This patch provides a shim between the kernel-internal cleancache
API (see Documentation/mm/cleancache.txt) and the Xen Transcendent
Memory ABI (see http://oss.oracle.com/projects/tmem).
Xen tmem provides "hypervisor RAM" as an ephemeral page-oriented
pseudo-RAM store for cleancache pages, shared cleancache pages,
and frontswap pages. Tmem provides enterprise-quality concurrency,
full save/restore and live migration support, compression
and deduplication.
A presentation showing up to 8% faster performance and up to 52%
reduction in sectors read on a kernel compile workload, despite
aggressive in-kernel page reclamation ("self-ballooning") can be

Since suspend, resume and shutdown operations in struct sysdev_class
and struct sysdev_driver are not used any more, remove them. Also
drop sysdev_suspend(), sysdev_resume() and sysdev_shutdown() used
for executing those operations and modify all of their users
accordingly. This reduces kernel code size quite a bit and reduces
its complexity.

Change the irq handler of evtchns and pirqs that don't need EOI (pirqs
that correspond to physical edge interrupts) to handle_edge_irq.
Use handle_fasteoi_irq for pirqs that need eoi (they generally
correspond to level triggered irqs), no risk in loosing interrupts
because we have to EOI the irq anyway.
This change has the following benefits:
- it uses the very same handlers that Linux would use on native for the
same irqs (handle_edge_irq for edge irqs and msis, and
handle_fasteoi_irq for everything else);
- it uses these handlers in the same way native code would use them: it
let Linux mask\unmask and ack the irq when Linux want to mask\unmask
and ack the irq;
- it fixes a problem occurring when a driver calls disable_irq() in its
handler: the old code was unconditionally unmasking the evtchn even if
the irq is disabled when irq_eoi was called.

Device suspend/resume infrastructure is used not only by the suspend
and hibernate code in kernel/power, but also by APM, Xen and the
kexec jump feature. However, commit 40dc166cb5
(PM / Core: Introduce struct syscore_ops for core subsystems PM)
failed to add syscore_suspend() and syscore_resume() calls to that
code, which generally leads to breakage when the features in question
are used.
To fix this problem, add the missing syscore_suspend() and
syscore_resume() calls to arch/x86/kernel/apm_32.c, kernel/kexec.c
and drivers/xen/manage.c.

We only supported the M2P (and P2M) override only for the
GNTMAP_contains_pte type mappings. Meaning that we grants
operations would "contain the machine address of the PTE to update"
If the flag is unset, then the grant operation is
"contains a host virtual address". The latter case means that
the Hypervisor takes care of updating our page table
(specifically the PTE entry) with the guest's MFN. As such we should
not try to do anything with the PTE. Previous to this patch
we would try to clear the PTE which resulted in Xen hypervisor
being upset with us:

This patch allows us to inhibit the PTE clearing in the PV guest
if the GNTMAP_contains_pte is not set.
On the m2p_remove_override path we provide the same parameter.
Sadly in the grant-table driver we do not have a mechanism to
tell m2p_remove_override whether to clear the PTE or not. Since
the grant-table driver is used by user-space, we can safely assume
that it operates only on PTE's. Hence the implementation for
it to work on !GNTMAP_contains_pte returns -EOPNOTSUPP. In the future
we can implement the support for this. It will require some extra
accounting structure to keep track of the page[i], and the flag.

We need this to find the real Xen PIRQ value for a device
that requests an MSI or MSI-X. In the past we used
'xen_gsi_from_irq' since that function would return
an Xen PIRQ or GSI depending on the provided IRQ. Now that
we have seperated that we need to use the correct
function.

We do this via the PHYSDEVOP_irq_status_query support hypervisor call.
We will get a positive value if another domain has binded its
PIRQ to the specified GSI (IRQ line).

Xen save/restore is going to use hibernate device callbacks for
quiescing devices and putting them back to normal operations and it
would need to select CONFIG_HIBERNATION for this purpose. However,
that also would cause the hibernate interfaces for user space to be
enabled, which might confuse user space, because the Xen kernels
don't support hibernation. Moreover, it would be wasteful, as it
would make the Xen kernels include a substantial amount of code that
they would never use.
To address this issue introduce new power management Kconfig option
CONFIG_HIBERNATE_CALLBACKS, such that it will only select the code
that is necessary for the hibernate device callbacks to work and make
CONFIG_HIBERNATION select it. Then, Xen save/restore will be able to
select CONFIG_HIBERNATE_CALLBACKS without dragging the entire
hibernate code along with it.

git commit 9be4d45759 (xen: add
extra pages to balloon) splited balloon_append() into two functions
(balloon_append() and __balloon_append()) and left decrementation
of totalram_pages counter in __balloon_append(). In this situation
if __balloon_append() is called on i386 with highmem page referenced
then totalhigh_pages is decremented, however, it should not. This
patch corrects that issue and moves dec_totalhigh_pages() from
__balloon_append() to balloon_append(). Now totalram_pages and
totalhigh_pages are decremented simultaneously only when
balloon_append() is called.

copy_to_user() returns the amount of data remaining to be copied. We
want to return a negative error code here. The upper layers just
call WARN_ON() if we return non-zero so this doesn't change the
behavior. But returning -EFAULT is still cleaner.

Fix up conflicts in drivers/staging/brcm80211/brcmsmac/wl_mac80211.c
with the staging updates.
Use PM_FREEZE, PM_THAW and PM_RESTORE power events for
suspend/resume/checkpoint functionality, instead of PM_SUSPEND
and PM_RESUME. Use of these pm events fixes the Xen Guest hangup
when taking checkpoints. When a suspend event is cancelled
(while taking checkpoints once/continuously), we use PM_THAW
instead of PM_RESUME. PM_RESTORE is used when suspend is not
cancelled. See Documentation/power/devices.txt and linux/pm.h
for more info about freeze, thaw and restore. The sequence of

Grant mappings cause the PFN<->MFN mapping to be lost on the pages used
for the mapping. Instead of leaking memory, use pages that have already
been ballooned out and so have no valid mapping. This removes the need
for the bad-page leak workaround as pages are repopulated by the balloon
driver.

Pages that have been ballooned are useful for other Xen drivers doing
grant table actions, because these pages have valid struct page/PFNs but
have no valid MFN so are available for remapping.

The basic functionality of ballooning pages is useful for Xen drivers in
general. Rather than require a dependency on the balloon module, split
the functionality that is reused into the core. The balloon module is
still required to follow ballooning requests from xenstore or to view
balloon statistics in sysfs.

In the past the retry_count (in other form of this code) was zero.
Meaning retry forever. Do the same thing here without changing it to
the value 16.
Also remove some of the pr_info as there is no need to spam
the user.

There isn't really much relationship between the two, other than
nr_irqs often being the larger of the two.
Allows us to remove a nr_irqs sized array, the only users of this
array are MSI setup and restore, neither of which are particularly
performance critical.

In a PVHVM kernel not all interrupts are Xen interrupts (APIC interrupts can also be present).
Currently we get away with walking over all interrupts because the
lookup in the irq_info array simply returns IRQT_UNBOUND and we ignore
it. However this array will be going away in a future patch so we need
to manually track which interrupts have been allocated by the Xen
events infrastructure.

Following the example set by xen_allocate_pirq_msi and
xen_bind_pirq_msi_to_irq:
xen_allocate_pirq becomes xen_allocate_pirq_gsi and now only allocates
a pirq number and does not bind it.
xen_map_pirq_gsi becomes xen_bind_pirq_gsi_to_irq and binds an
existing pirq.

I was unable to find any user of these functions in either the
functionality pending for 2.6.39 or the xen/next-2.6.32 branch of
xen.git
An exception to this was xen_gsi_from_irq which did appear to be used
in xen/next-2.6.32's pciback. However in the 2.6.39 version of pciback
xen_pirq_from_irq is, correctly AFAICT, used instead.
Only a minority of functions in events.h use "extern" so drop it from
those places for consistency.

Split the binding aspect of xen_allocate_pirq_msi out into a new
xen_bind_pirq_to_irq function.
In xen_hvm_setup_msi_irq when allocating a pirq write the MSI message
to signal the PIRQ as soon as the pirq is obtained. There is no way to
free the pirq back so if the subsequent binding to an IRQ fails we
want to ensure that we will reuse the PIRQ next time rather than leak
it.

The find_unbound_pirq is called only from xen_allocate_pirq_msi and
only if alloc_pirq is true. The only caller which does this is
xen_hvm_setup_msi_irqs. The use of this function is gated, in
pci_xen_hvm_init, on XENFEAT_hvm_pirqs.
The PHYSDEVOP_get_free_pirq interfaces was added to the hypervisor in
22410:be96f6058c05 while XENFEAT_hvm_pirqs was added a couple of
minutes prior in 22409:6663214f06ac. Therefore we do not need to
concern ourselves with hypervisors which support XENFEAT_hvm_pirqs but
not PHYSDEVOP_get_free_pirq.
This eliminates the fallback path in find_unbound_pirq which walks to
pirq_to_irq array looking for a free pirq. Unlike the
PHYSDEVOP_get_free_pirq interface this fallback only looks up a free
pirq but does not reserve it. Removing this fallback will simplify
locking in the future.

The only time when granted pages need to be treated specially is when
using Xen's PTE modification for grant mappings owned by another domain
(that is, only gntdev on PV guests). Otherwise, the area does not
require VM_DONTCOPY and VM_PFNMAP, since it can be accessed just like
any other page of RAM.
Since the vm_operations_struct close operations decrement reference
counts, a corresponding open function that increments them is required
now that it is possible to have multiple references to a single area.
We are careful in the gntdev to check if we can remove those flags. The
reason that we need to be careful in gntdev on PV guests is because we are
not changing the PFN/MFN mapping on PV; instead, we change the application's
page tables to point to the other domain's memory. This means that the vma
cannot be copied without using another grant mapping hypercall; it also
requires special handling on unmap, which is the reason for gntdev's
dependency on the MMU notifier.
For gntalloc, this is not a concern - the pages are owned by the domain
using the gntalloc device, and can be mapped and unmapped in the same manner
as any other page of memory.

The caller will not undo a mapping which failed and therefore the
override will not be removed.
This is especially bad in the case of GNTMAP_contains_pte mapping type
mappings where m2p_add_override will destroy the kernel mapping of the
page.
This was observed via a failure of map_grant_pages in gntdev_mmap (due
to userspace using a bad grant reference), which left the page in
question unmapped (because it was a GNTMAP_contains_pte mapping) which
led to a crash later on.

We only did this for PV guests that are xen_initial_domain() but
there is not reason not to do this for other cases. The other
case is only exercised when you pass in a PCI device to a PV guest
_and_ the device in question.

There are three cases which we need to care about, PV guest, PV domain
0 and HVM guest.
The PV guest case is simple since it has no access to ACPI or real
APICs and therefore has no GSIs therefore we simply dynamically
allocate all IRQs. The potentially interesting case here is PIRQ type
event channels associated with passed through PCI devices. However
even in this case the guest has no direct interaction with the
physical GSI since that happens in the PCI backend.
The PV domain 0 and HVM guest cases are actually the same. In domain 0
case the kernel sees the host ACPI and GSIs (although it only sees the
APIC indirectly via the hypervisor) and in the HVM guest case it sees
the virtualised ACPI and emulated APICs. In these cases we start
allocating dynamic IRQs at nr_irqs_gsi so that they cannot clash with
any GSI.
Currently xen_allocate_irq_dynamic starts at nr_irqs and works
backwards looking for a free IRQ in order to (try and) avoid clashing
with GSIs used in domain 0 and in HVM guests. This change avoids that
although we retain the behaviour of allowing dynamic IRQs to encroach
on the GSI range if no suitable IRQs are available since a future IRQ
clash is deemed preferable to failure right now.

This is neater than open-coded calls to irq_alloc_desc_at and
irq_free_desc.
No intended behavioural change.
Note that we previously were not checking the return value of
irq_alloc_desc_at which would be failing for GSI<NR_IRQS_LEGACY
because the core architecture code has already allocated those for
us. Hence the additional check against NR_IRQS_LEGACY in
xen_allocate_irq_gsi.

With this patch, we diligently set regions that will be used by the
balloon driver to be INVALID_P2M_ENTRY and under the ownership
of the balloon driver. We are OK using the __set_phys_to_machine
as we do not expect to be allocating any P2M middle or entries pages.
The set_phys_to_machine has the side-effect of potentially allocating
new pages and we do not want that at this stage.
We can do this because xen_build_mfn_list_list will have already
allocated all such pages up to xen_max_p2m_pfn.
We also move the check for auto translated physmap down the
stack so it is present in __set_phys_to_machine.

The PV xenbus control/shutdown node is written by the toolstack as a
request to the guest to perform a particular action (shutdown, reboot,
suspend etc). The guest is expected to acknowledge that it will
complete a request by clearing the control node.
Previously it would acknowledge any request, even if it did not know
what to do with it. Specifically in the case where CONFIG_PM_SLEEP is
not enabled the kernel would acknowledge a suspend request even though
it was not actually going to do anything.
Instead make the kernel only acknowledge requests if it is actually
going to do something with it. This will improve the toolstack's
ability to diagnose and deal with failures.

Otherwise we fail to properly suspend/resume all of the emulated devices.
Something between 2.6.38-rc2 and rc3 appears to have exposed this
issue, but it's always been wrong not to do this.

If an already-mapped area of the device was mapped into userspace a
second time, a hypercall was incorrectly made to remap the memory
again. Avoid the hypercall on later mmap calls, and fail the mmap call
if a writable mapping is attempted on a read-only range.

In paravirtualized domains, mn_invl_page or mn_invl_range_start can
unmap a segment of a mapped region without unmapping all pages. When
the region is later released, the pages will be unmapped twice, leading
to an incorrect -EINVAL return.

The is_mapped flag used to be set at the completion of the map operation,
but was not checked in all error paths. Use map->vma instead, which will
now be cleared if the initial grant mapping fails.
