Christoph Hellwig 24e6d5a59a mm: pass the vmem_altmap to arch_add_memory and __add_pages
We can just pass this on instead of having to do a radix tree lookup
without proper locking 2 levels into the callchain.

Heiko Carstens 8d306f53b6 s390/mm: fix off-by-one bug in 5-level page table handling
Martin Cermak reported that setting a uprobe doesn't work. Reason for
this is that the common uprobes code tries to get an unmapped area at
the last possible page within an address space.

This broke with commit 1aea9b3f92 ("s390/mm: implement 5 level pages
tables") which introduced an off-by-one bug which prevents to map
anything at the last possible page within an address space.

The check with the off-by-one bug however can be removed since with
commit 8ab867cb08 ("s390/mm: fix BUG_ON in crst_table_upgrade") the
necessary check is done at both call sites.

Greg Kroah-Hartman fec37202e8 s390: Remove redundant license text
Now that the SPDX tag is in all arch/s390/ files, that identifies the
license in a specific and legally-defined manner.  So the extra GPL text
wording in the remaining files can be removed as it is no longer needed
at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text.  And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

Greg Kroah-Hartman ac41aaeedc s390: mm: add SPDX identifiers to the remaining files
It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the arch/s390/mm/ files with the correct SPDX license
identifier based on the license text in the file itself.  The SPDX
identifier is a legally binding shorthand, which can be used instead of
the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

Hendrik Brueckner 544e8dd7a8 s390/cpum_sf: correctly set the PID and TID in perf samples
The hardware sampler creates samples that are processed at a later
point in time.  The PID and TID values of the perf samples that are
created for hardware samples are initialized with values from the
current task.  Hence, the PID and TID values are not correct and
perf samples are associated with wrong processes.

The PID and TID values are obtained from the Host Program Parameter
(HPP) field in the basic-sampling data entries.  These PIDs are
valid in the init PID namespace.  Ensure that the PIDs in the perf
samples are resolved considering the PID namespace in which the
perf event was created.

To correct the PID and TID values in the created perf samples,
a special overflow handler is installed.  It replaces the default
overflow handler and does not become effective if any other
overflow handler is used.  With the special overflow handler most
of the perf samples are associated with the right processes.
For processes, that are no longer exist, the association might
still be wrong.

Martin Schwidefsky 0aaba41b58 s390: remove all code using the access register mode
The vdso code for the getcpu() and the clock_gettime() call use the access
register mode to access the per-CPU vdso data page with the current code.

An alternative to the complicated AR mode is to use the secondary space
mode. This makes the vdso faster and quite a bit simpler. The downside is
that the uaccess code has to be changed quite a bit.

Which instructions are used depends on the machine and what kind of uaccess
operation is requested. The instruction dictates which ASCE value needs
to be loaded into %cr1 and %cr7.

The different cases:

* User copy with MVCOS for z10 and newer machines
  The MVCOS instruction can copy between the primary space (aka user) and
  the home space (aka kernel) directly. For set_fs(KERNEL_DS) the kernel
  ASCE is loaded into %cr1. For set_fs(USER_DS) the user space is already
  loaded in %cr1.

* User copy with MVCP/MVCS for older machines
  To be able to execute the MVCP/MVCS instructions the kernel needs to
  switch to primary mode. The control register %cr1 has to be set to the
  kernel ASCE and %cr7 to either the kernel ASCE or the user ASCE dependent
  on set_fs(KERNEL_DS) vs set_fs(USER_DS).

* Data access in the user address space for strnlen / futex
  To use "normal" instruction with data from the user address space the
  secondary space mode is used. The kernel needs to switch to primary mode,
  %cr1 has to contain the kernel ASCE and %cr7 either the user ASCE or the
  kernel ASCE, dependent on set_fs.

To load a new value into %cr1 or %cr7 is an expensive operation, the kernel
tries to be lazy about it. E.g. for multiple user copies in a row with
MVCP/MVCS the replacement of the vdso ASCE in %cr7 with the user ASCE is
done only once. On return to user space a CPU bit is checked that loads the
vdso ASCE again.

To enable and disable the data access via the secondary space two new
functions are added, enable_sacf_uaccess and disable_sacf_uaccess. The fact
that a context is in secondary space uaccess mode is stored in the
mm_segment_t value for the task. The code of an interrupt may use set_fs
as long as it returns to the previous state it got with get_fs with another
call to set_fs. The code in finish_arch_post_lock_switch simply has to do a
set_fs with the current mm_segment_t value for the task.

The lines with "lazy" refer to the state after a copy via the secondary
space with a delayed reload of %cr1 and %cr7.

There are three hardware address spaces that can cause a DAT exception,
primary, secondary and home space. The exception can be related to
four different fault types: user space fault, vdso fault, kernel fault,
and the gmap faults.

Dependent on the set_fs state and normal vs. sacf mode there are a number
of fault combinations:

1) user address space fault via the primary ASCE
2) gmap address space fault via the primary ASCE
3) kernel address space fault via the primary ASCE for machines with
   MVCOS and set_fs(KERNEL_DS)
4) vdso address space faults via the secondary ASCE with an invalid
   address while running in secondary space in problem state
5) user address space fault via the secondary ASCE for user-copy
   based on the secondary space mode, e.g. futex_ops or strnlen_user
6) kernel address space fault via the secondary ASCE for user-copy
   with secondary space mode with set_fs(KERNEL_DS)
7) kernel address space fault via the primary ASCE for user-copy
   with secondary space mode with set_fs(USER_DS) on machines without
   MVCOS.
8) kernel address space fault via the home space ASCE

Replace user_space_fault() with a new function get_fault_type() that
can distinguish all four different fault types.

With these changes the futex atomic ops from the kernel and the
strnlen_user will get a little bit slower, as well as the old style
uaccess with MVCP/MVCS. All user accesses based on MVCOS will be as
fast as before. On the positive side, the user space vdso code is a
lot faster and Linux ceases to use the complicated AR mode.

Martin Schwidefsky c771320e93 s390/mm,kvm: improve detection of KVM guest faults
The identification of guest fault currently relies on the PF_VCPU flag.
This is set in guest_entry_irqoff and cleared in guest_exit_irqoff.
Both functions are called by __vcpu_run, the PF_VCPU flag is set for
quite a lot of kernel code outside of the guest execution.

Replace the PF_VCPU scheme with the PIF_GUEST_FAULT in the pt_regs and
make the program check handler code in entry.S set the bit only for
exception that occurred between the .Lsie_gmap and .Lsie_done labels.

Heiko Carstens ead7a22e9b s390: avoid undefined behaviour
At a couple of places smatch emits warnings like this:

    arch/s390/mm/vmem.c:409 vmem_map_init() warn:
        right shifting more than type allows

In fact shifting a signed type right is undefined. Avoid this and add
an unsigned long cast. The shifted values are always positive.

Heiko Carstens 978fa72e82 s390: remove named saved segment support
Remove the support to create a z/VM named saved segment (NSS). This
feature is not supported since quite a while in favour of jump labels,
function tracing and (now) CPU alternatives. All of these features
require to write to the kernel text section which is not possible if
the kernel is contained within an NSS.

Given that memory savings are minimal if kernel images are shared and
in addition updates of shared images are painful, the NSS feature can
be removed.

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Gerald Schaefer ba385c0594 s390/mm: fix write access check in gup_huge_pmd()
The check for the _SEGMENT_ENTRY_PROTECT bit in gup_huge_pmd() is the
wrong way around. It must not be set for write==1, and not be checked for
write==0. Fix this similar to how it was fixed for ptes long time ago in
commit 25591b0703 ("[S390] fix get_user_pages_fast").

One impact of this bug would be unnecessarily using the gup slow path for
write==0 on r/w mappings. A potentially more severe impact would be that
gup_huge_pmd() will succeed for write==1 on r/o mappings.

Christian Borntraeger fa41ba0d08 s390/mm: avoid empty zero pages for KVM guests to avoid postcopy hangs
Right now there is a potential hang situation for postcopy migrations,
if the guest is enabling storage keys on the target system during the
postcopy process.

For storage key virtualization, we have to forbid the empty zero page as
the storage key is a property of the physical page frame.  As we enable
storage key handling lazily we then drop all mappings for empty zero
pages for lazy refaulting later on.

This does not work with the postcopy migration, which relies on the
empty zero page never triggering a fault again in the future. The reason
is that postcopy migration will simply read a page on the target system
if that page is a known zero page to fault in an empty zero page.  At
the same time postcopy remembers that this page was already transferred
- so any future userfault on that page will NOT be retransmitted again
to avoid races.

If now the guest enters the storage key mode while in postcopy, we will
break this assumption of postcopy.

The solution is to disable the empty zero page for KVM guests early on
and not during storage key enablement. With this change, the postcopy
migration process is guaranteed to start after no zero pages are left.

As guest pages are very likely not empty zero pages anyway the memory
overhead is also pretty small.

While at it this also adds proper page table locking to the zero page
removal.

Claudio Imbrenda 1bab1c02af KVM: s390: expose no-DAT to guest and migration support
The STFLE bit 147 indicates whether the ESSA no-DAT operation code is
valid, the bit is not normally provided to the host; the host is
instead provided with an SCLP bit that indicates whether guests can
support the feature.

This patch:
* enables the STFLE bit in the guest if the corresponding SCLP bit is
  present in the host.
* adds support for migrating the no-DAT bit in the PGSTEs
* fixes the software interpretation of the ESSA instruction that is
  used when migrating, both for the new operation code and for the old
  "set stable", as per specifications.

Heiko Carstens 34ad7cdc1b s390/mm: prevent memory offline for memory blocks with cma areas
Memory blocks that contain areas for the contiguous memory allocator
(cma) should not be allowed to go offline. Otherwise this would render
cma completely useless.
This might make sense on other architectures where memory might be
taken offline due to hardware errors, but not on architectures which
support memory hotplug for load balancing.

The ESSA instruction has a new option that allows to tag pages that
are not used as a page table. Without the tag the hypervisor has to
assume that any guest page could be used in a page table inside the
guest. This forces the hypervisor to flush all guest TLB entries
whenever a host page table entry is invalidated. With the tag
the host can skip the TLB flush if the page is tagged as normal page.

When we enable storage keys for a guest lazily, we reset the ACC and F
values. That is correct assuming that these are 0 on a clear reset and
the guest obviously has not used any key setting instruction.

We also zero out the change and reference bit. This is not correct as
the architecture prefers over-indication instead of under-indication
for the keyless->keyed transition.

This patch fixes the behaviour and always sets guest change and guest
reference for all guest storage keys on the keyless -> keyed switch.

Punit Agrawal 7868a2087e mm/hugetlb: add size parameter to huge_pte_offset()
A poisoned or migrated hugepage is stored as a swap entry in the page
tables.  On architectures that support hugepages consisting of
contiguous page table entries (such as on arm64) this leads to ambiguity
in determining the page table entry to return in huge_pte_offset() when
a poisoned entry is encountered.

Let's remove the ambiguity by adding a size parameter to convey
additional information about the requested address.  Also fixup the
definition/usage of huge_pte_offset() throughout the tree.

Michal Hocko 3d79a728f9 mm, memory_hotplug: replace for_device by want_memblock in arch_add_memory
arch_add_memory gets for_device argument which then controls whether we
want to create memblocks for created memory sections.  Simplify the
logic by telling whether we want memblocks directly rather than going
through pointless negation.  This also makes the api easier to
understand because it is clear what we want rather than nothing telling
for_device which can mean anything.

This shouldn't introduce any functional change.

Michal Hocko f1dd2cd13c mm, memory_hotplug: do not associate hotadded memory to zones until online
The current memory hotplug implementation relies on having all the
struct pages associate with a zone/node during the physical hotplug
phase (arch_add_memory->__add_pages->__add_section->__add_zone).  In the
vast majority of cases this means that they are added to ZONE_NORMAL.
This has been so since 9d99aaa31f ("[PATCH] x86_64: Support memory
hotadd without sparsemem") and it wasn't a big deal back then because
movable onlining didn't exist yet.

Much later memory hotplug wanted to (ab)use ZONE_MOVABLE for movable
onlining 511c2aba8f ("mm, memory-hotplug: dynamic configure movable
memory and portion memory") and then things got more complicated.
Rather than reconsidering the zone association which was no longer
needed (because the memory hotplug already depended on SPARSEMEM) a
convoluted semantic of zone shifting has been developed.  Only the
currently last memblock or the one adjacent to the zone_movable can be
onlined movable.  This essentially means that the online type changes as
the new memblocks are added.

Let's simulate memory hot online manually

This is an awkward semantic because an udev event is sent as soon as the
block is onlined and an udev handler might want to online it based on
some policy (e.g.  association with a node) but it will inherently race
with new blocks showing up.

This patch changes the physical online phase to not associate pages with
any zone at all.  All the pages are just marked reserved and wait for
the onlining phase to be associated with the zone as per the online
request.  There are only two requirements

	- existing ZONE_NORMAL and ZONE_MOVABLE cannot overlap

	- ZONE_NORMAL precedes ZONE_MOVABLE in physical addresses

the latter one is not an inherent requirement and can be changed in the
future.  It preserves the current behavior and made the code slightly
simpler.  This is subject to change in future.

This means that the same physical online steps as above will lead to the
following state: Normal Movable

Implementation:
The current move_pfn_range is reimplemented to check the above
requirements (allow_online_pfn_range) and then updates the respective
zone (move_pfn_range_to_zone), the pgdat and links all the pages in the
pfn range with the zone/node.  __add_pages is updated to not require the
zone and only initializes sections in the range.  This allowed to
simplify the arch_add_memory code (s390 could get rid of quite some of
code).

devm_memremap_pages is the only user of arch_add_memory which relies on
the zone association because it only hooks into the memory hotplug only
half way.  It uses it to associate the new memory with ZONE_DEVICE but
doesn't allow it to be {on,off}lined via sysfs.  This means that this
particular code path has to call move_pfn_range_to_zone explicitly.

The original zone shifting code is kept in place and will be removed in
the follow up patch for an easier review.

Please note that this patch also changes the original behavior when
offlining a memory block adjacent to another zone (Normal vs.  Movable)
used to allow to change its movable type.  This will be handled later.

Michal Hocko 1b862aecfb mm, memory_hotplug: get rid of is_zone_device_section
Device memory hotplug hooks into regular memory hotplug only half way.
It needs memory sections to track struct pages but there is no
need/desire to associate those sections with memory blocks and export
them to the userspace via sysfs because they cannot be onlined anyway.

This is currently expressed by for_device argument to arch_add_memory
which then makes sure to associate the given memory range with
ZONE_DEVICE.  register_new_memory then relies on is_zone_device_section
to distinguish special memory hotplug from the regular one.  While this
works now, later patches in this series want to move __add_zone outside
of arch_add_memory path so we have to come up with something else.

Add want_memblock down the __add_pages path and use it to control
whether the section->memblock association should be done.
arch_add_memory then just trivially want memblock for everything but
for_device hotplug.

remove_memory_section doesn't need is_zone_device_section either.  We
can simply skip all the memblock specific cleanup if there is no
memblock for the given section.

This shouldn't introduce any functional change.

Hugh Dickins 1be7107fbe mm: larger stack guard gap, between vmas
Stack guard page is a useful feature to reduce a risk of stack smashing
into a different mapping. We have been using a single page gap which
is sufficient to prevent having stack adjacent to a different mapping.
But this seems to be insufficient in the light of the stack usage in
userspace. E.g. glibc uses as large as 64kB alloca() in many commonly
used functions. Others use constructs liks gid_t buffer[NGROUPS_MAX]
which is 256kB or stack strings with MAX_ARG_STRLEN.

This will become especially dangerous for suid binaries and the default
no limit for the stack size limit because those applications can be
tricked to consume a large portion of the stack and a single glibc call
could jump over the guard page. These attacks are not theoretical,
unfortunatelly.

Make those attacks less probable by increasing the stack guard gap
to 1MB (on systems with 4k pages; but make it depend on the page size
because systems with larger base pages might cap stack allocations in
the PAGE_SIZE units) which should cover larger alloca() and VLA stack
allocations. It is obviously not a full fix because the problem is
somehow inherent, but it should reduce attack space a lot.

One could argue that the gap size should be configurable from userspace,
but that can be done later when somebody finds that the new 1MB is wrong
for some special case applications.  For now, add a kernel command line
option (stack_guard_gap) to specify the stack gap size (in page units).

Implementation wise, first delete all the old code for stack guard page:
because although we could get away with accounting one extra page in a
stack vma, accounting a larger gap can break userspace - case in point,
a program run with "ulimit -S -v 20000" failed when the 1MB gap was
counted for RLIMIT_AS; similar problems could come with RLIMIT_MLOCK
and strict non-overcommit mode.

Instead of keeping gap inside the stack vma, maintain the stack guard
gap as a gap between vmas: using vm_start_gap() in place of vm_start
(or vm_end_gap() in place of vm_end if VM_GROWSUP) in just those few
places which need to respect the gap - mainly arch_get_unmapped_area(),
and and the vma tree's subtree_gap support for that.

Heiko Carstens a752598254 s390: rename struct psw_bits members
Rename a couple of the struct psw_bits members so it is more obvious
for what they are good. Initially I thought using the single character
names from the PoP would be sufficient and obvious, but admittedly
that is not true.

The current implementation is not easy to use, if one has to look into
the source file to figure out which member represents the 'per' bit
(which is the 'r' member).

Therefore rename the members to sane names that are identical to the
uapi psw mask defines:

r -> per
i -> io
e -> ext
t -> dat
m -> mcheck
w -> wait
p -> pstate

Heiko Carstens 8bb3fdd686 s390: rename psw_bits enums
The address space enums that must be used when modifying the address
space part of a psw with the psw_bits() macro can easily be confused
with the psw defines that are used to mask and compare directly the
mask part of a psw.
We have e.g. PSW_AS_PRIMARY vs PSW_ASC_PRIMARY.

To avoid confusion rename the PSW_AS_* enums to PSW_BITS_AS_*.

In addition also rename the PSW_AMODE_* enums, so they also follow the
same naming scheme: PSW_BITS_AMODE_*.

Heiko Carstens 60c497014e s390/mm: use correct address space when enabling DAT
Right now the kernel uses the primary address space until finally the
switch to the correct home address space will be done when the idle
PSW will be loaded within psw_idle().

Correct this and simply use the home address space when DAT is enabled
for the first time.

This doesn't really fix a bug, but fixes odd behavior.

Heiko Carstens 1366def38b s390/pageattr: avoid unnecessary page table splitting
The kernel page table splitting code will split page tables even for
features the CPU does not support. E.g. a CPU may not support the NX
feature.
In order to avoid this, remove those bits from the flags parameter
that correlate with unsupported CPU features within __set_memory(). In
addition add an early exit if the flags parameter does not have any
bits set afterwards.

Janosch Frank 2fa5ed7d87 s390/mm: Remove double gaddr calculation when notifying
ptep_notify and gmap_shadow_notify both need a guest address and
therefore retrieve them from the available virtual host address.

As they operate on the same guest address, we can calculate it once
and then pass it on. As a gmap normally has more than one shadow gmap,
we also do not recalculate for each of them any more.

Michal Hocko faee35a57b s390: get rid of superfluous __GFP_REPEAT
__GFP_REPEAT has a rather weak semantic but since it has been introduced
around 2.6.12 it has been ignored for low order allocations.

page_table_alloc then uses the flag for a single page allocation. This
means that this flag has never been actually useful here because it has
always been used only for PAGE_ALLOC_COSTLY requests.

An earlier attempt to remove the flag 10d58bf297 ("s390: get rid of
superfluous __GFP_REPEAT") has missed this one but the situation is very
same here.

Janosch Frank 2e4d88009f KVM: s390: Fix guest migration for huge guests resulting in panic
While we can technically not run huge page guests right now, we can
setup a guest with huge pages. Trying to migrate it will trigger a
VM_BUG_ON and, if the kernel is not configured to panic on a BUG, it
will happily try to work on non-existing page table entries.

With this patch, we always return "dirty" if we encounter a large page
when migrating. This at least fixes the immediate problem until we
have proper handling for both kind of pages.

Dominik Dingel 54397bb0bb s390/mm: use _SEGMENT_ENTRY_EMPTY in the code
_SEGMENT_ENTRY_INVALID denotes the invalid bit in a segment table
entry whereas _SEGMENT_ENTRY_EMPTY means that the value of the whole
entry is only the invalid bit, as the entry is completely empty.

Therefore we use _SEGMENT_ENTRY_INVALID only to check and set the
invalid bit with bitwise operations. _SEGMENT_ENTRY_EMPTY is only used
to check for (un)equality.

messages.

Paul Gortmaker ff24b07abb s390: mm: Audit and remove any unnecessary uses of module.h
Historically a lot of these existed because we did not have
a distinction between what was modular code and what was providing
support to modules via EXPORT_SYMBOL and friends.  That changed
when we forked out support for the latter into the export.h file.

This means we should be able to reduce the usage of module.h
in code that is obj-y Makefile or bool Kconfig.  The advantage
in doing so is that module.h itself sources about 15 other headers;
adding significantly to what we feed cpp, and it can obscure what
headers we are effectively using.

Since module.h was the source for init.h (for __init) and for
export.h (for EXPORT_SYMBOL) we consider each change instance
for the presence of either and replace as needed.  An instance
where module_param was used without moduleparam.h was also fixed,
as well as an implict use of asm/elf.h header.

Martin Schwidefsky 57d7f939e7 s390: add no-execute support
Bit 0x100 of a page table, segment table of region table entry
can be used to disallow code execution for the virtual addresses
associated with the entry.

There is one tricky bit, the system call to return from a signal
is part of the signal frame written to the user stack. With a
non-executable stack this would stop working. To avoid breaking
things the protection fault handler checks the opcode that caused
the fault for 0x0a77 (sys_sigreturn) and 0x0aad (sys_rt_sigreturn)
and injects a system call. This is preferable to the alternative
solution with a stub function in the vdso because it works for
vdso=off and statically linked binaries as well.

Heiko Carstens 00de54c803 s390/mem_detect: fix memory type of first block
Fix a long-standing but currently irrelevant bug: the memory detection
code performs a tprot instruction on address zero to figure out if the
first memory chunk is readable or writable. Due to low address
protection the result is "read-only". If the memory detection code
would actually care, it would have to ignore the first memory
increment, but it adds the memory increment to writable memory anyway.

If memblock debugging is enabled this leads to an extra rather
surprising call which registers memory. To avoid this get rid of the
first misleading tprot call and simply assume that the first memory
increment is writable. Otherwise we wouldn't have reached the memory
detection code anyway.

The s390 specific memory detection code does not call memblock_add,
which would generate debug output if memblock=debug is specified on
the kernel command line. Instead it directly calls memblock_add_range,
which doesn't generate any debug output.
To have a chance to debug early memblock related bugs add an s390
specific memblock_dbg call and a (missing) memblock_dump_all call.

Johannes Weiner 6d75f366b9 lib: radix-tree: check accounting of existing slot replacement users
The bug in khugepaged fixed earlier in this series shows that radix tree
slot replacement is fragile; and it will become more so when not only
NULL<->!NULL transitions need to be caught but transitions from and to
exceptional entries as well.  We need checks.

Re-implement radix_tree_replace_slot() on top of the sanity-checked
__radix_tree_replace().  This requires existing callers to also pass the
radix tree root, but it'll warn us when somebody replaces slots with
contents that need proper accounting (transitions between NULL entries,
real entries, exceptional entries) and where a replacement through the
slot pointer would corrupt the radix tree node counts.

Heiko Carstens 9e427365af s390: convert remaining bootmem allocations to memblock
Get rid of all remaining alloc_bootmem calls and use memblock_alloc
instead everywhere.  This way we get rid of the inconsistent mixture
of alloc_bootmem and memblock_alloc usages.

Two of the alloc_bootmem_low calls within arch/s390/kernel/setup.c are
replaced with memblock_alloc calls that don't enforce that the
allocated memory is below 2GB. This restriction was never necessary.

Martin Schwidefsky c360192bf4 s390/preempt: move preempt_count to the lowcore
Convert s390 to use a field in the struct lowcore for the CPU
preemption count. It is a bit cheaper to access a lowcore field
compared to a thread_info variable and it removes the depencency
on a task related structure.

bloat-o-meter on the vmlinux image for the default configuration
(CONFIG_PREEMPT_NONE=y) reports a small reduction in text size:

add/remove: 0/0 grow/shrink: 18/578 up/down: 228/-5448 (-5220)

A larger improvement is achieved with the default configuration
but with CONFIG_PREEMPT=y and CONFIG_DEBUG_PREEMPT=n:

Gerald Schaefer 4a65429457 s390/mm: fix zone calculation in arch_add_memory()
Standby (hotplug) memory should be added to ZONE_MOVABLE on s390. After
commit 199071f1 "s390/mm: make arch_add_memory() NUMA aware",
arch_add_memory() used memblock_end_of_DRAM() to find out the end of
ZONE_NORMAL and the beginning of ZONE_MOVABLE. However, commit 7f36e3e5
"memory-hotplug: add hot-added memory ranges to memblock before allocate
node_data for a node." moved the call of memblock_add_node() before
the call of arch_add_memory() in add_memory_resource(), and thus changed
the return value of memblock_end_of_DRAM() when called in
arch_add_memory(). As a result, arch_add_memory() will think that all
memory blocks should be added to ZONE_NORMAL.

Fix this by changing the logic in arch_add_memory() so that it will
manually iterate over all zones of a given node to find out which zone
a memory block should be added to.

Paul Gortmaker dcc096c540 s390: migrate exception table users off module.h and onto extable.h
These files were only including module.h for exception table
related functions.  We've now separated that content out into its
own file "extable.h" so now move over to that and avoid all the
extra header content in module.h that we don't really need to compile
these files.

Heiko Carstens 4d81aaa53c s390/pageattr: handle numpages parameter correctly
Both set_memory_ro() and set_memory_rw() will modify the page
attributes of at least one page, even if the numpages parameter is
zero.

The author expected that calling these functions with numpages == zero
would never happen. However with the new 444d13ff10 ("modules: add
ro_after_init support") feature this happens frequently.

Therefore do the right thing and make these two functions return
gracefully if nothing should be done.

 The ugly bit is the conflicts.  A couple of them are simple conflicts due
 to 4.7 fixes, but most of them are with other trees. There was definitely
 too much reliance on Acked-by here.  Some conflicts are for KVM patches
 where _I_ gave my Acked-by, but the worst are for this pull request's
 patches that touch files outside arch/*/kvm.  KVM submaintainers should
 probably learn to synchronize better with arch maintainers, with the
 latter providing topic branches whenever possible instead of Acked-by.
 This is what we do with arch/x86.  And I should learn to refuse pull
 requests when linux-next sends scary signals, even if that means that
 submaintainers have to rebase their branches.
 
 Anyhow, here's the list:
 
 - arch/x86/kvm/vmx.c: handle_pcommit and EXIT_REASON_PCOMMIT was removed
 by the nvdimm tree.  This tree adds handle_preemption_timer and
 EXIT_REASON_PREEMPTION_TIMER at the same place.  In general all mentions
 of pcommit have to go.
 
 There is also a conflict between a stable fix and this patch, where the
 stable fix removed the vmx_create_pml_buffer function and its call.
 
 - virt/kvm/kvm_main.c: kvm_cpu_notifier was removed by the hotplug tree.
 This tree adds kvm_io_bus_get_dev at the same place.
 
 - virt/kvm/arm/vgic.c: a few final bugfixes went into 4.7 before the
 file was completely removed for 4.8.
 
 - include/linux/irqchip/arm-gic-v3.h: this one is entirely our fault;
 this is a change that should have gone in through the irqchip tree and
 pulled by kvm-arm.  I think I would have rejected this kvm-arm pull
 request.  The KVM version is the right one, except that it lacks
 GITS_BASER_PAGES_SHIFT.
 
 - arch/powerpc: what a mess.  For the idle_book3s.S conflict, the KVM
 tree is the right one; everything else is trivial.  In this case I am
 not quite sure what went wrong.  The commit that is causing the mess
 (fd7bacbca4, "KVM: PPC: Book3S HV: Fix TB corruption in guest exit
 path on HMI interrupt", 2016-05-15) touches both arch/powerpc/kernel/
 and arch/powerpc/kvm/.  It's large, but at 396 insertions/5 deletions
 I guessed that it wasn't really possible to split it and that the 5
 deletions wouldn't conflict.  That wasn't the case.
 
 - arch/s390: also messy.  First is hypfs_diag.c where the KVM tree
 moved some code and the s390 tree patched it.  You have to reapply the
 relevant part of commits 6c22c98637, plus all of e030c1125e, to
 arch/s390/kernel/diag.c.  Or pick the linux-next conflict
 resolution from http://marc.info/?l=kvm&m=146717549531603&w=2.
 Second, there is a conflict in gmap.c between a stable fix and 4.8.

Gerald Schaefer bc29b7ac1d s390/mm: clean up pte/pmd encoding
The hugetlbfs pte<->pmd conversion functions currently assume that the pmd
bit layout is consistent with the pte layout, which is not really true.

The SW read and write bits are encoded as the sequence "wr" in a pte, but
in a pmd it is "rw". The hugetlbfs conversion assumes that the sequence
is identical in both cases, which results in swapped read and write bits
in the pmd. In practice this is not a problem, because those pmd bits are
only relevant for THP pmds and not for hugetlbfs pmds. The hugetlbfs code
works on (fake) ptes, and the converted pte bits are correct.

There is another variation in pte/pmd encoding which affects dirty
prot-none ptes/pmds. In this case, a pmd has both its HW read-only and
invalid bit set, while it is only the invalid bit for a pte. This also has
no effect in practice, but it should better be consistent.

This patch fixes both inconsistencies by changing the SW read/write bit
layout for pmds as well as the PAGE_NONE encoding for ptes. It also makes
the hugetlbfs conversion functions more robust by introducing a
move_set_bit() macro that uses the pte/pmd bit #defines instead of
constant shifts.

David Hildenbrand f045402984 s390/mm: fix gmap tlb flush issues
__tlb_flush_asce() should never be used if multiple asce belong to a mm.

As this function changes mm logic determining if local or global tlb
flushes will be neded, we might end up flushing only the gmap asce on all
CPUs and a follow up mm asce flushes will only flush on the local CPU,
although that asce ran on multiple CPUs.

The missing tlb flushes will provoke strange faults in user space and even
low address protections in user space, crashing the kernel.

Michal Hocko 10d58bf297 s390: get rid of superfluous __GFP_REPEAT
__GFP_REPEAT has a rather weak semantic but since it has been introduced
around 2.6.12 it has been ignored for low order allocations.

page_table_alloc then uses the flag for a single page allocation.  This
means that this flag has never been actually useful here because it has
always been used only for PAGE_ALLOC_COSTLY requests.

Nested virtualization will have to enable own gmaps. Current code
would enable the wrong gmap whenever scheduled out and back in,
therefore resulting in the wrong gmap being enabled.

This patch reenables the last enabled gmap, therefore avoiding having to
touch vcpu->arch.gmap when enabling a different gmap.

We can easily support real-space designation just like EDAT1 and EDAT2.
So guest2 can provide for guest3 an asce with the real-space control being
set.

We simply have to allocate the biggest page table possible and fake all
levels.

There is no protection to consider. If we exceed guest memory, vsie code
will inject an addressing exception (via program intercept). In the future,
we could limit the fake table level to the gmap page table.

As the top level page table can never go away, such gmap shadows will never
get unshadowed, we'll have to come up with another way to limit the number
of kept gmap shadows.

- ACCF-Validity Control in RTTE
- Access-Control Bits in RTTE
- Fetch-Protection Bit in RTTE
- Common-Region Bit in RTTE

Just like for EDAT1, all bits might be dropped and there is no guaranteed
that they are active.

but it might not be so for us. Therefore we have to create fake page tables.

We can easily add that to our existing infrastructure. The invalidation
mechanism will make sure that fake page tables are removed when the parent
table (sgt table entry) is changed.

As EDAT1 also introduced protection on all page table levels, we have to
also shadow these correctly.

We don't have to care about:
- ACCF-Validity Control in STE
- Access-Control Bits in STE
- Fetch-Protection Bit in STE
- Common-Segment Bit in STE

As all bits might be dropped and there is no guaranteed that they are
active ("unpredictable whether the CPU uses these bits", "may be used").
Without using EDAT1 in the shadow ourselfes (STE-format control == 0),
simply shadowing these bits would not be enough. They would be ignored.

Please note that we are using the "fake" flag to make this look consistent
with further changes (EDAT2, real-space designation support) and don't let
the shadow functions handle fc=1 stes.

In the future, with huge pages in the host, gmap_shadow_pgt() could simply
try to map a huge host page if "fake" is set to one and indicate via return
value that no lower fake tables / shadow ptes are required.

Before any thread is allowed to use a gmap_shadow, it has to be fully
initialized. However, for invalidation to work properly, we have to
register the new gmap_shadow before we protect the parent gmap table.

Because locking is tricky, and we have to avoid duplicate gmaps, let's
introduce an initialized field, that signalizes other threads if that
gmap_shadow can already be used or if they have to retry.

Let's properly return errors using ERR_PTR() instead of simply returning
NULL, so a caller can properly react on the error.

We have to unlock sg->guest_table_lock in order to call
gmap_protect_rmap(). If we sleep just before that call, another VCPU
might pick up that shadowed page table (while it is not protected yet)
and use it.

In order to avoid these races, we have to introduce a third state -
"origin set but still invalid" for an entry. This way, we can avoid
another thread already using the entry before the table is fully protected.
As soon as everything is set up, we can clear the invalid bit - if we
had no race with the unshadowing code.

We really want to avoid manually handling protection for nested
virtualization. By shadowing pages with the protection the guest asked us
for, the SIE can handle most protection-related actions for us (e.g.
special handling for MVPG) and we can directly forward protection
exceptions to the guest.

PTEs will now always be shadowed with the correct _PAGE_PROTECT flag.
Unshadowing will take care of any guest changes to the parent PTE and
any host changes to the host PTE. If the host PTE doesn't have the
fitting access rights or is not available, we have to fix it up.

2. When the gmap is freed and therefore the top level pages are freed.

Martin Schwidefsky 4be130a084 s390/mm: add shadow gmap support
For a nested KVM guest the outer KVM host needs to create shadow
page tables for the nested guest. This patch adds the basic support
to the guest address space (gmap) code.

For each guest address space the inner KVM host creates, the first
outer KVM host needs to create shadow page tables. The address space
is identified by the ASCE loaded into the control register 1 at the
time the inner SIE instruction for the second nested KVM guest is
executed. The outer KVM host creates the shadow tables starting with
the table identified by the ASCE on a on-demand basis. The outer KVM
host will get repeated faults for all the shadow tables needed to
run the second KVM guest.

While a shadow page table for the second KVM guest is active the access
to the origin region, segment and page tables needs to be restricted
for the first KVM guest. For region and segment and page tables the first
KVM guest may read the memory, but write attempt has to lead to an
unshadow.  This is done using the page invalid and read-only bits in the
page table of the first KVM guest. If the first guest re-accesses one of
the origin pages of a shadow, it gets a fault and the affected parts of
the shadow page table hierarchy needs to be removed again.

PGSTE tables don't have to be shadowed, as all interpretation assist can't
deal with the invalid bits in the shadow pte being set differently than
the original ones provided by the first KVM guest.

The current gmap pte notifier forces a pte into to a read-write state.
If the pte is invalidated the gmap notifier is called to inform KVM
that the mapping will go away.

Extend this approach to allow read-write, read-only and no-access
as possible target states and call the pte notifier for any change
to the pte.

This mechanism is used to temporarily set specific access rights for
a pte without doing the heavy work of a true mprotect call.

Heiko Carstens 6c22c98637 s390: avoid extable collisions
We have some inline assemblies where the extable entry points to a
label at the end of an inline assembly which is not followed by an
instruction.

On the other hand we have also inline assemblies where the extable
entry points to the first instruction of an inline assembly.

If a first type inline asm (extable point to empty label at the end)
would be directly followed by a second type inline asm (extable points
to first instruction) then we would have two different extable entries
that point to the same instruction but would have a different target
address.

This can lead to quite random behaviour, depending on sorting order.

I verified that we currently do not have such collisions within the
kernel. However to avoid such subtle bugs add a couple of nop
instructions to those inline assemblies which contain an extable that
points to an empty label.

Heiko Carstens d07a980c1b s390: add proper __ro_after_init support
On s390 __ro_after_init is currently mapped to __read_mostly which
means that data marked as __ro_after_init will not be protected.

Reason for this is that the common code __ro_after_init implementation
is x86 centric: the ro_after_init data section was added to rodata,
since x86 enables write protection to kernel text and rodata very
late. On s390 we have write protection for these sections enabled with
the initial page tables. So adding the ro_after_init data section to
rodata does not work on s390.

In order to make __ro_after_init work properly on s390 move the
ro_after_init data, right behind rodata. Unlike the rodata section it
will be marked read-only later after all init calls happened.

This s390 specific implementation adds new __start_ro_after_init and
__end_ro_after_init labels. Everything in between will be marked
read-only after the init calls happened. In addition to the
__ro_after_init data move also the exception table there, since from a
practical point of view it fits the __ro_after_init requirements.

Martin Schwidefsky 64f31d5802 s390/mm: simplify the TLB flushing code
ptep_flush_lazy and pmdp_flush_lazy use mm->context.attach_count to
decide between a lazy TLB flush vs an immediate TLB flush. The field
contains two 16-bit counters, the number of CPUs that have the mm
attached and can create TLB entries for it and the number of CPUs in
the middle of a page table update.

The __tlb_flush_asce, ptep_flush_direct and pmdp_flush_direct functions
use the attach counter and a mask check with mm_cpumask(mm) to decide
between a local flush local of the current CPU and a global flush.

For all these functions the decision between lazy vs immediate and
local vs global TLB flush can be based on CPU masks. There are two
masks:  the mm->context.cpu_attach_mask with the CPUs that are actively
using the mm, and the mm_cpumask(mm) with the CPUs that have used the
mm since the last full flush. The decision between lazy vs immediate
flush is based on the mm->context.cpu_attach_mask, to decide between
local vs global flush the mm_cpumask(mm) is used.

With this patch all checks will use the CPU masks, the old counter
mm->context.attach_count with its two 16-bit values is turned into a
single counter mm->context.flush_count that keeps track of the number
of CPUs with incomplete page table updates. The sole user of this
counter is finish_arch_post_lock_switch() which waits for the end of
all page table updates.

The vunmap_pte_range() function calls ptep_get_and_clear() without any
locking. ptep_get_and_clear() uses ptep_xchg_lazy()/ptep_flush_direct()
for the page table update. ptep_flush_direct requires that preemption
is disabled, but without any locking this is not the case. If the kernel
preempts the task while the attach_counter is increased an endless loop
in finish_arch_post_lock_switch() will occur the next time the task is
scheduled.

Add explicit preempt_disable()/preempt_enable() calls to the relevant
functions in arch/s390/mm/pgtable.c.

Heiko Carstens 0ccb32c983 s390/mm: align swapper_pg_dir to 16k
The segment/region table that is part of the kernel image must be
properly aligned to 16k in order to make the crdte inline assembly
work.
Otherwise it will calculate a wrong segment/region table start address
and access incorrect memory locations if the swapper_pg_dir is not
aligned to 16k.

Therefore define BSS_FIRST_SECTIONS in order to put the swapper_pg_dir
at the beginning of the bss section and also align the bss section to
16k just like other architectures did.

Heiko Carstens 37cd944c8d s390/pgtable: add mapping statistics
Add statistics that show how memory is mapped within the kernel
identity mapping. This is more or less the same like git
commit ce0c0e50f9 ("x86, generic: CPA add statistics about state
of direct mapping v4") for x86.

I also intentionally copied the lower case "k" within DirectMap4k vs
the upper case "M" and "G" within the two other lines. Let's have
consistent inconsistencies across architectures.

The output of /proc/meminfo now contains these additional lines:

DirectMap4k:        2048 kB
DirectMap1M:     3991552 kB
DirectMap2G:     4194304 kB

The implementation on s390 is lockless unlike the x86 version, since I
assume changes to the kernel mapping are a very rare event. Therefore
it really doesn't matter if these statistics could potentially be
inconsistent if read while kernel pages tables are being changed.

Heiko Carstens e8a97e42dc s390/pageattr: allow kernel page table splitting
set_memory_ro() and set_memory_rw() currently only work on 4k
mappings, which is good enough for module code aka the vmalloc area.

However we stumbled already twice into the need to make this also work
on larger mappings:
- the ro after init patch set
- the crash kernel resize code

Therefore this patch implements automatic kernel page table splitting
if e.g. set_memory_ro() would be called on parts of a 2G mapping.
This works quite the same as the x86 code, but is much simpler.

In order to make this work and to be architecturally compliant we now
always use the csp, cspg or crdte instructions to replace valid page
table entries. This means that set_memory_ro() and set_memory_rw()
will be much more expensive than before. In order to avoid huge
latencies the code contains a couple of cond_resched() calls.

The current code only splits page tables, but does not merge them if
it would be possible.  The reason for this is that currently there is
no real life scenarion where this would really happen. All current use
cases that I know of only change access rights once during the life
time. If that should change we can still implement kernel page table
merging at a later time.

Usually segment and region tables are 16k aligned due to the way the
buddy allocator works.  This is not true for the vmem code which only
asks for a 4k alignment. In order to be consistent enforce a 16k
alignment here as well.

This alignment will be assumed and therefore is required by the
pageattr code.

Christian Borntraeger 1c343f7b0e KVM: s390/mm: Fix CMMA reset during reboot
commit 1e133ab296 ("s390/mm: split arch/s390/mm/pgtable.c") factored
out the page table handling code from __gmap_zap and  __s390_reset_cmma
into ptep_zap_unused and added a simple flag that tells which one of the
function (reset or not) is to be made. This also changed the behaviour,
as it also zaps unused page table entries on reset.
Turns out that this is wrong as s390_reset_cmma uses the page walker,
which DOES NOT take the ptl lock.

The most simple fix is to not do the zapping part on reset (which uses
the walker)

Aleksa has reported incorrect si_errno value when stracing task which
received SIGSEGV:
[pid 20799] --- SIGSEGV {si_signo=SIGSEGV, si_code=SEGV_MAPERR, si_errno=2510266, si_addr=0x100000000000000}

The reason seems to be that do_sigsegv is not initializing siginfo
structure defined on the stack completely so it will leak 4B of
the previous stack content. Fix it simply by initializing si_errno
to 0 (same as do_sigbus does already).

The identity mapping is suboptimal for the last 2GB frame. The mapping
will be established with a mix of 4KB and 1MB mappings instead of a
single 2GB mapping.

This happens because of a off-by-one bug introduced with
commit 50be634507 ("s390/mm: Convert bootmem to memblock").

Currently the identity mapping looks like this:

Gerald Schaefer 723cacbd9d s390/mm: fix asce_bits handling with dynamic pagetable levels
There is a race with multi-threaded applications between context switch and
pagetable upgrade. In switch_mm() a new user_asce is built from mm->pgd and
mm->context.asce_bits, w/o holding any locks. A concurrent mmap with a
pagetable upgrade on another thread in crst_table_upgrade() could already
have set new asce_bits, but not yet the new mm->pgd. This would result in a
corrupt user_asce in switch_mm(), and eventually in a kernel panic from a
translation exception.

Fix this by storing the complete asce instead of just the asce_bits, which
can then be read atomically from switch_mm(), so that it either sees the
old value or the new value, but no mixture. Both cases are OK. Having the
old value would result in a page fault on access to the higher level memory,
but the fault handler would see the new mm->pgd, if it was a valid access
after the mmap on the other thread has completed. So as worst-case scenario
we would have a page fault loop for the racing thread until the next time
slice.

Also remove dead code and simplify the upgrade/downgrade path, there are no
upgrades from 2 levels, and only downgrades from 3 levels for compat tasks.
There are also no concurrent upgrades, because the mmap_sem is held with
down_write() in do_mmap, so the flush and table checks during upgrade can
be removed.
