Rob Gardner a7c5724b5c sparc64: fix FP corruption in user copy functions
Short story: Exception handlers used by some copy_to_user() and
copy_from_user() functions do not diligently clean up floating point
register usage, and this can result in a user process seeing invalid
values in floating point registers. This sometimes makes the process
fail.

Long story: Several cpu-specific (NG4, NG2, U1, U3) memcpy functions
use floating point registers and VIS alignaddr/faligndata to
accelerate data copying when source and dest addresses don't align
well. Linux uses a lazy scheme for saving floating point registers; It
is not done upon entering the kernel since it's a very expensive
operation. Rather, it is done only when needed. If the kernel ends up
not using FP regs during the course of some trap or system call, then
it can return to user space without saving or restoring them.

The various memcpy functions begin their FP code with VISEntry (or a
variation thereof), which saves the FP regs. They conclude their FP
code with VISExit (or a variation) which essentially marks the FP regs
"clean", ie, they contain no unsaved values. fprs.FPRS_FEF is turned
off so that a lazy restore will be triggered when/if the user process
accesses floating point regs again.

The bug is that the user copy variants of memcpy, copy_from_user() and
copy_to_user(), employ an exception handling mechanism to detect faults
when accessing user space addresses, and when this handler is invoked,
an immediate return from the function is forced, and VISExit is not
executed, thus leaving the fprs register in an indeterminate state,
but often with fprs.FPRS_FEF set and one or more dirty bits. This
results in a return to user space with invalid values in the FP regs,
and since fprs.FPRS_FEF is on, no lazy restore occurs.

This bug affects copy_to_user() and copy_from_user() for NG4, NG2,
U3, and U1. All are fixed by using a new exception handler for those
loads and stores that are done during the time between VISEnter and
VISExit.

n.b. In NG4memcpy, the problematic code can be triggered by a copy
size greater than 128 bytes and an unaligned source address.  This bug
is known to be the cause of random user process memory corruptions
while perf is running with the callgraph option (ie, perf record -g).
This occurs because perf uses copy_from_user() to read user stacks,
and may fault when it follows a stack frame pointer off to an
invalid page. Validation checks on the stack address just obscure
the underlying problem.

Rob Gardner 833526941f sparc64: Perf should save/restore fault info
There have been several reports of random processes being killed with
a bus error or segfault during userspace stack walking in perf.  One
of the root causes of this problem is an asynchronous modification to
thread_info fault_address and fault_code, which stems from a perf
counter interrupt arriving during kernel processing of a "benign"
fault, such as a TSB miss. Since perf_callchain_user() invokes
copy_from_user() to read user stacks, a fault is not only possible,
but probable. Validity checks on the stack address merely cover up the
problem and reduce its frequency.

The solution here is to save and restore fault_address and fault_code
in perf_callchain_user() so that the benign fault handler is not
disturbed by a perf interrupt.

Rob Gardner 3f74306ac8 sparc64: Ensure perf can access user stacks
When an interrupt (such as a perf counter interrupt) is delivered
while executing in user space, the trap entry code puts ASI_AIUS in
%asi so that copy_from_user() and copy_to_user() will access the
correct memory. But if a perf counter interrupt is delivered while the
cpu is already executing in kernel space, then the trap entry code
will put ASI_P in %asi, and this will prevent copy_from_user() from
reading any useful stack data in either of the perf_callchain_user_X
functions, and thus no user callgraph data will be collected for this
sample period. An additional problem is that a fault is guaranteed
to occur, and though it will be silently covered up, it wastes time
and could perturb state.

In perf_callchain_user(), we ensure that %asi contains ASI_AIUS
because we know for a fact that the subsequent calls to
copy_from_user() are intended to read the user's stack.

Rob Gardner 1ca04a4ce0 sparc64: Don't set %pil in rtrap_nmi too early
Commit 28a1f53 delays setting %pil to avoid potential
hardirq stack overflow in the common rtrap_irq path.
Setting %pil also needs to be delayed in the rtrap_nmi
path for the same reason.

Khalid Aziz 82924e542f sparc64: Add ADI capability to cpu capabilities
Add ADI (Application Data Integrity) capability to cpu capabilities list.
ADI capability allows virtual addresses to be encoded with a tag in
bits 63-60. This tag serves as an access control key for the regions
of virtual address with ADI enabled and a key set on them. Hypervisor
encodes this capability as "adp" in "hwcap-list" property in machine
description.

Rob Gardner cae9af6a82 sparc64: Don't restrict fp regs for no-fault loads
The function handle_ldf_stq() deals with no-fault ASI
loads and stores, but restricts fp registers to quad
word regs (ie, %f0, %f4 etc). This is valid for the
STQ case, but unnecessarily restricts loads, which
may be single precision, double, or quad. This results
in SIGFPE being raised for this instruction when the
source address is invalid:
	ldda [%g1] ASI_PNF, %f2
but not for this one:
	ldda [%g1] ASI_PNF, %f4
The validation check for quad register is moved to
within the STQ block so that loads are not affected
by the check.

An additional problem is that the calculation for freg
is incorrect when a single precision load is being
handled. This causes %f1 to be seen as %f32 etc,
and the incorrect register ends up being overwritten.
This code sequence demonstrates the problem:
	ldd [%g1], %f32		! g1 = valid address
	lda [%i3] ASI_PNF, %f1  ! i3 = invalid address
	std %f32, [%g1]
This is corrected by basing the freg calculation on
the load size.

David S. Miller d618382ba5 iommu-common: Fix error code used in iommu_tbl_range_{alloc,free}().
The value returned from iommu_tbl_range_alloc() (and the one passed
in as a fourth argument to iommu_tbl_range_free) is not a DMA address,
it is rather an index into the IOMMU page table.

Therefore using DMA_ERROR_CODE is not appropriate.

Use a more type matching error code define, IOMMU_ERROR_CODE, and
update all users of this interface.

BAR 15.  OF does report a MEM64 aperture, but before this patch,
pci_determine_mem_io_space() ignored it.

Add support for host bridge apertures with 64-bit PCI addresses.  Also
set IORESOURCE_MEM_64 for PCI device and bridge resources in PCI 64-bit
memory space.

Sparc doesn't actually print the device and bridge resources, but after
this patch, we should have the equivalent of this:

Sukadev Bhattiprolu 8f3e5684d3 perf/core: Drop PERF_EVENT_TXN
We currently use PERF_EVENT_TXN flag to determine if we are in the middle
of a transaction. If in a transaction, we defer the schedulability checks
from pmu->add() operation to the pmu->commit() operation.

Now that we have "transaction types" (PERF_PMU_TXN_ADD, PERF_PMU_TXN_READ)
we can use the type to determine if we are in a transaction and drop the
PERF_EVENT_TXN flag.

When PERF_EVENT_TXN is dropped, the cpuhw->group_flag on some architectures
becomes unused, so drop that field as well.

This is an extension of the Powerpc patch from Peter Zijlstra to s390,
Sparc and x86 architectures.

Sukadev Bhattiprolu fbbe070115 perf/core: Add a 'flags' parameter to the PMU transactional interfaces
Currently, the PMU interface allows reading only one counter at a time.
But some PMUs like the 24x7 counters in Power, support reading several
counters at once. To leveage this functionality, extend the transaction
interface to support a "transaction type".

The first type, PERF_PMU_TXN_ADD, refers to the existing transactions,
i.e. used to _schedule_ all the events on the PMU as a group. A second
transaction type, PERF_PMU_TXN_READ, will be used in a follow-on patch,
by the 24x7 counters to read several counters at once.

Extend the transaction interfaces to the PMU to accept a 'txn_flags'
parameter and use this parameter to ignore any transactions that are
not of type PERF_PMU_TXN_ADD.

    I was thinking the opposite approach and basically removing all uses
    of IORESOURCE_CACHEABLE from the kernel. There are only a handful of
    them.and we can probably replace them all with hardcoded
    ioremap_cached() calls in the cases they are actually useful.

All existing usages of IORESOURCE_CACHEABLE call ioremap() instead of
ioremap_nocache() if the resource is cacheable, however ioremap() is
uncached by default. Clearly none of the existing usages care about the
cacheability. Particularly devm_ioremap_resource() never worked as
advertised since it always fell back to plain ioremap().

Clean this up as the new direction we want is to convert
ioremap_<type>() usages to memremap(..., flags).

Viresh Kumar ff4aea45c7 sparc/time: Migrate to new 'set-state' interface
Migrate sparc drivers to the new 'set-state' interface provided by
clockevents core, the earlier 'set-mode' interface is marked obsolete
now.

This also enables us to implement callbacks for new states of clockevent
devices, for example: ONESHOT_STOPPED.

We weren't doing anything which switching to few clockevent modes and so
their callbacks aren't implemented.

Peter Zijlstra 76b235c6bc jump_label: Rename JUMP_LABEL_{EN,DIS}ABLE to JUMP_LABEL_{JMP,NOP}
Since we've already stepped away from ENABLE is a JMP and DISABLE is a
NOP with the branch_default bits, and are going to make it even worse,
rename it to make it all clearer.

This way we don't mix multiple levels of logic attributes, but have a
plain 'physical' name for what the current instruction patching status
of a jump label is.

This is a first step in removing the naming confusion that has led to
a stream of avoidable bugs such as:

Processes are getting killed (sigbus or segv) while walking userspace
callchains when using perf. In some instances I have seen ufp = 0x7ff
which does not seem like a proper stack address.

This patch adds a function to run validity checks against the address
before attempting the copy_from_user. The checks are copied from the
x86 version as a start point with the addition of a 4-byte alignment
check.

Akinobu Mita 8c07a308ec sparc: use for_each_sg()
This replaces the plain loop over the sglist array with for_each_sg()
macro which consists of sg_next() function calls.  Since sparc does select
ARCH_HAS_SG_CHAIN, it is necessary to use for_each_sg() in order to loop
over each sg element.  This also help find problems with drivers that do
not properly initialize their sg tables when CONFIG_DEBUG_SG is enabled.

Xunlei Pang 460ea8d70d sparc: time: Replace update_persistent_clock() with CONFIG_RTC_SYSTOHC
On Sparc systems, update_persistent_clock() uses RTC drivers to do
the job, it makes more sense to hand it over to CONFIG_RTC_SYSTOHC.

In the long run, all the update_persistent_clock() should migrate to
proper class RTC drivers if any and use CONFIG_RTC_SYSTOHC instead.

sparc: Resolve conflict between sparc v9 and M7 on usage of bit 9 of TTE

Bit 9 of TTE is CV (Cacheable in V-cache) on sparc v9 processor while
the same bit 9 is MCDE (Memory Corruption Detection Enable) on M7
processor. This creates a conflicting usage of the same bit. Kernel
sets TTE.cv bit on all pages for sun4v architecture which works well
for sparc v9 but enables memory corruption detection on M7 processor
which is not the intent. This patch adds code to determine if kernel
is running on M7 processor and takes steps to not enable memory
corruption detection in TTE erroneously.

Eric Snowberg f0c1a11737 sparc64: pci slots information is not populated in sysfs
Add PCI slot numbers within sysfs for PCIe hardware.  Larger
PCIe systems with nested PCI bridges and slots further
down on these bridges were not being populated within sysfs.
This will add ACPI style PCI slot numbers for these systems
since the OF 'slot-names' information is not available on
all PCIe platforms.

Rusty Russell e4afa120c9 cpumask: remove __first_cpu / __next_cpu

10 threads results in a severe performance drop when TSO is disabled,
indicating a weakness in the software that can be avoided by using
the scalable IOMMU arena DMA allocation.

Baseline numbers before this patch:
   with default settings (TSO enabled) :    9-9.5 Gbps
   Disable TSO using ethtool- drops badly:  2-3 Gbps.

After this patch, iperf client with 10 threads, can give a
throughput of at least 8.5 Gbps, even when TSO is disabled.

Michal Hocko f91e8d6d76 sparc: clarify __GFP_NOFAIL allocation
Commit 920c3ed741 ("[SPARC64]: Add basic infrastructure for MD
add/remove notification") has added __GFP_NOFAIL for the allocation
request but it hasn't mentioned why is this strict requirement really
needed.  The code was handling an allocation failure and propagated it
properly up the callchain so it is not clear why it is needed.

Dave has clarified the intention when I tried to remove the flag as not
being necessary:

: It is a serious failure.

: If we miss an MDESC update due to this allocation failure, the update
: is not an event which gets retransmitted so we will lose the updated
: machine description forever.
:
: We really need this allocation to succeed.

So add a comment to clarify the nofail flag and get rid of the failure
check because __GFP_NOFAIL allocation doesn't fail.

David Ahern 31aaa98c24 sparc: Touch NMI watchdog when walking cpus and calling printk
With the increase in number of CPUs calls to functions that dump
output to console (e.g., arch_trigger_all_cpu_backtrace) can take
a long time to complete. If IRQs are disabled eventually the NMI
watchdog kicks in and creates more havoc. Avoid by telling the NMI
watchdog everything is ok.

does the same. Passing PERF_EF_RELOAD to sparc_pmu_start means the call
to sparc_perf_event_set_period can be removed as well.

incorrect; the PCI core should not change device resources while a driver
is managing the device.

Remove pci_bus_add_devices() from pci_scan_root_bus() and do it after any
resource assignment in the callers.

Note that ARM's pci_common_init_dev() already called pci_bus_add_devices()
after pci_scan_root_bus(), so we only need to remove the first call:

John Stultz 3142f76022 clocksource, sparc32: Convert to using clocksource_register_hz()
While cleaning up some clocksource code, I noticed the
time_32 implementation uses the clocksource_hz2mult()
helper, but doesn't use the clocksource_register_hz()
method.

I don't believe the Sparc clocksource is a default
clocksource, so we shouldn't need to self-define
the mult/shift pair.

So convert the time_32.c implementation to use
clocksource_register_hz().

A long running project has been to clean up remaining uses
of clocksource_register(), replacing it with the simpler
clocksource_register_khz/hz() functions.

However, there are a few cases where we need to self-define
our mult/shift values, so switch the function to a more
obviously internal __clocksource_register() name, and
consolidate much of the internal logic so we don't have
duplication.

Rob Gardner 53eb251697 sparc: semtimedop() unreachable due to comparison error
A bug was reported that the semtimedop() system call was always
failing eith ENOSYS.

Since SEMCTL is defined as 3, and SEMTIMEDOP is defined as 4,
the comparison "call <= SEMCTL" will always prevent SEMTIMEDOP
from getting through to the semaphore ops switch statement.

This is corrected by changing the comparison to "call <= SEMTIMEDOP".

For instrumenting global variables KASan will shadow memory backing memory
for modules.  So on module loading we will need to allocate memory for
shadow and map it at address in shadow that corresponds to the address
allocated in module_alloc().

__vmalloc_node_range() could be used for this purpose, except it puts a
guard hole after allocated area.  Guard hole in shadow memory should be a
problem because at some future point we might need to have a shadow memory
at address occupied by guard hole.  So we could fail to allocate shadow
for module_alloc().

Now we have VM_NO_GUARD flag disabling guard page, so we need to pass into
__vmalloc_node_range().  Add new parameter 'vm_flags' to
__vmalloc_node_range() function.

Andy Lutomirski f56141e3e2 all arches, signal: move restart_block to struct task_struct
If an attacker can cause a controlled kernel stack overflow, overwriting
the restart block is a very juicy exploit target.  This is because the
restart_block is held in the same memory allocation as the kernel stack.

Moving the restart block to struct task_struct prevents this exploit by
making the restart_block harder to locate.

Note that there are other fields in thread_info that are also easy
targets, at least on some architectures.

It's also a decent simplification, since the restart code is more or less
identical on all architectures.

     For complex interrupt routing scenarios where more than one
     interrupt related chip is involved we had no proper representation
     in the generic interrupt infrastructure so far.  That made people
     implement rather ugly constructs in their nested irq chip
     implementations.  The main offenders are x86 and arm/gic.

     To distangle that mess we have now hierarchical irqdomains which
     seperate the various interrupt chips and connect them via the
     hierarchical domains.  That keeps the domain specific details
     internal to the particular hierarchy level and removes the
     criss/cross referencing of chip internals.  The resulting hierarchy
     for a complex x86 system will look like this:

   - Support for generic MSI interrupt domain handling

     We have more and more non PCI related MSI interrupts, so providing
     a generic infrastructure for this is better than having all
     affected architectures implementing their own private hacks.

   - Support for PCI-MSI interrupt domain handling, based on the generic
     MSI support.

     This part carries the pci/msi branch from Bjorn Helgaas pci tree to
     avoid a massive conflict.  The PCI/MSI parts are acked by Bjorn.

Thomas Gleixner 280510f106 PCI/MSI: Rename mask/unmask_msi_irq treewide
The PCI/MSI irq chip callbacks mask/unmask_msi_irq have been renamed
to pci_msi_mask/unmask_irq to mark them PCI specific. Rename all usage
sites. The conversion helper functions are kept around to avoid
conflicts in next and will be removed after merging into mainline.

Coccinelle assisted conversion. No functional change.

The problem is that pbm->pci_bus->self is NULL.

This code is trying to go through the standard PCI config space
interfaces to read the PCI controller's PCI_STATUS register.

This doesn't work, because we more often than not do not enumerate
the PCI controller as a bonafide PCI device during the OF device
node scan.  Therefore bus->self remains NULL.

Existing common code for PSYCHO and PSYCHO-like PCI controllers
handles this properly, by doing the config space access directly.

Do the same here, pbm->pci_ops->{read,write}().

Further investigation showed that compiling only per_cpu_patch() with
an older compiler fixes the boot.

Detailed analysis showed that the function is not being miscompiled by
gcc-4.9, but it is using a different register allocation ordering.

With the gcc-4.9 compiled function, something during the code patching
causes some of the %i* input registers to get corrupted.  Perhaps
we have a TLB miss path into the firmware that is deep enough to
cause a register window spill and subsequent restore when we get
back from the TLB miss trap.

Let's plug this up by doing two things:

1) Stop using the firmware stack for client interface calls into
   the firmware.  Just use the kernel's stack.

2) As soon as we can, call into a new function "start_early_boot()"
   to put a one-register-window buffer between the firmware's
   deepest stack frame and the top-most initial kernel one.

David S. Miller 84bd6d8b9c sparc64: Fix corrupted thread fault code.
Every path that ends up at do_sparc64_fault() must install a valid
FAULT_CODE_* bitmask in the per-thread fault code byte.

Two paths leading to the label winfix_trampoline (which expects the
FAULT_CODE_* mask in register %g4) were not doing so:

1) For pre-hypervisor TLB protection violation traps, if we took
   the 'winfix_trampoline' path we wouldn't have %g4 initialized
   with the FAULT_CODE_* value yet.  Resulting in using the
   TLB_TAG_ACCESS register address value instead.

2) In the TSB miss path, when we notice that we are going to use a
   hugepage mapping, but we haven't allocated the hugepage TSB yet, we
   still have to take the window fixup case into consideration and
   in that particular path we leave %g4 not setup properly.

Errors on this sort were largely invisible previously, but after
commit 4ccb927289 ("sparc64: sun4v TLB
error power off events") we now have a fault_code mask bit
(FAULT_CODE_BAD_RA) that triggers due to this bug.

FAULT_CODE_BAD_RA triggers because this bit is set in TLB_TAG_ACCESS
(see #1 above) and thus we get seemingly random bus errors triggered
for user processes.

Pull percpu consistent-ops changes from Tejun Heo:
 "Way back, before the current percpu allocator was implemented, static
  and dynamic percpu memory areas were allocated and handled separately
  and had their own accessors.  The distinction has been gone for many
  years now; however, the now duplicate two sets of accessors remained
  with the pointer based ones - this_cpu_*() - evolving various other
  operations over time.  During the process, we also accumulated other
  inconsistent operations.

  This pull request contains Christoph's patches to clean up the
  duplicate accessor situation.  __get_cpu_var() uses are replaced with
  with this_cpu_ptr() and __this_cpu_ptr() with raw_cpu_ptr().

  Unfortunately, the former sometimes is tricky thanks to C being a bit
  messy with the distinction between lvalues and pointers, which led to
  a rather ugly solution for cpumask_var_t involving the introduction of
  this_cpu_cpumask_var_ptr().

  This converts most of the uses but not all.  Christoph will follow up
  with the remaining conversions in this merge window and hopefully
  remove the obsolete accessors"

David S. Miller d195b71bad sparc64: Kill unnecessary tables and increase MAX_BANKS.
swapper_low_pmd_dir and swapper_pud_dir are actually completely
useless and unnecessary.

We just need swapper_pg_dir[].  Naturally the other page table chunks
will be allocated on an as-needed basis.  Since the kernel actually
accesses these tables in the PAGE_OFFSET view, there is not even a TLB
locality advantage of placing them in the kernel image.

Use the hard coded vmlinux.ld.S slot for swapper_pg_dir which is
naturally page aligned.

Increase MAX_BANKS to 1024 in order to handle heavily fragmented
virtual guests.

Even with this MAX_BANKS increase, the kernel is 20K+ smaller.
