Sebastian Ott 8b2a7e609b s390/pci: remove pdev during unplug
The disable slot implementation on s390 currently just detaches the
pci function from the partition - without informing the pci layer.
Fix this by calling pci_stop_and_remove_bus_device prior to the
operation.

Sebastian Ott 92820a5f99 s390: remove virt_to_phys implementation
virt_to_phys on s390 currently uses the LRA instruction to translate
virtual to physical addresses. This creates an unnecessary overhead
and caused trouble with dma debugging code (when called with an
address pointing to a already unmapped page).
Just get rid of s390's implementation and use the one from
asm-generic/io.h .

Note: with this change virt_to_phys will no longer work on vmalloc'ed
addresses.

This broke the WWPN and LUN sysfs attributes for s390 reipl and dump
on panic.

Example:

$ echo 0x0123456701234567 > /sys/firmware/reipl/fcp/wwpn
$ cat /sys/firmware/reipl/fcp/wwpn
0x0001234567012345

So fix this and use format strings that work also with the
new sscanf implementation:

$ echo 0x012345670123456789 > /sys/firmware/reipl/fcp/wwpn
$ cat /sys/firmware/reipl/fcp/wwpn
0x0123456701234567

Thomas Huth 953ed88d10 KVM: s390: Reworked LCTL and LCTLG instructions
LCTL and LCTLG are also privileged instructions, thus there is no need for
treating them separately from the other instructions in priv.c. So this
patch moves these two instructions to priv.c, adds a check for supervisor
state and simplifies the "handle_eb" instruction decoding by merging the
two eb_handlers jump tables from intercept.c and priv.c into one table only.

When a guest calls the TPI instruction, the second operand address could
point to an invalid location. In this case the problem should be signaled
to the guest by throwing an access exception.

Thomas Huth 5087dfa6c8 KVM: s390: Privileged operation checks moved to instruction handlers
We need more fine-grained control about the point in time when we check
for privileged instructions, since the exceptions that can happen during
an instruction have a well-defined priority. For example, for the PFMF
instruction, the check for PGM_PRIVILEGED_OP must happen after the check
for PGM_OPERATION since the latter has a higher precedence - thus the
check for privileged operation must not be done in kvm_s390_handle_b9()
already.

The ath9k Kconfig conflict was a change of a Kconfig option name right
next to the deletion of another option.

The xen-netback conflict was overlapping changes involving the
handling of the notify list in xen_netbk_rx_action().

Batman conflict resolution provided by Antonio Quartulli, basically
keep everything in both conflict hunks.

The nl80211 conflict is a little more involved.  In 'net' we added a
dynamic memory allocation to nl80211_dump_wiphy() to fix a race that
Linus reported.  Meanwhile in 'net-next' the handlers were converted
to use pre and post doit handlers which use a flag to determine
whether to hold the RTNL mutex around the operation.

However, the dump handlers to not use this logic.  Instead they have
to explicitly do the locking.  There were apparent bugs in the
conversion of nl80211_dump_wiphy() in that we were not dropping the
RTNL mutex in all the return paths, and it seems we very much should
be doing so.  So I fixed that whilst handling the overlapping changes.

To simplify the initial returns, I take the RTNL mutex after we try
to allocate 'tb'.

Heiko Carstens 35b03aec91 s390/mem_detect: fix memory hole handling
With git commit 996b4a7d "s390/mem_detect: remove artificial kdump
memory types" the memory detection code got simplified.
As a side effect the array that describes memory chunks may now
contain empty (zeroed) entries.
All call sites can handle this except for

drivers/s390/char/zcore.c::zcore_memmap_open

which has a really odd user space interface. The easiest fix is to
change the memory hole handling code, so that no empty entries exist
before the last valid entry is reached.

The handle_epsw() function calculated the first register in the wrong way,
so that it always used r0 by mistake. Now the code uses the common helper
function for decoding the registers of rre functions instead to avoid such
mistakes.

Heinz Graalfs b764bb1c50 KVM: s390,perf: Detect if perf samples belong to KVM host or guest
This patch is based on an original patch of David Hildenbrand.

The perf core implementation calls architecture specific code in order
to ask for specific information for a particular sample:

perf_instruction_pointer()
When perf core code asks for the instruction pointer, architecture
specific code must detect if a KVM guest was running when the sample
was taken. A sample can be associated with a  KVM guest when the PSW
supervisor state bit is set and the PSW instruction pointer part
contains the address of 'sie_exit'.
A KVM guest's instruction pointer information is then retrieved via
gpsw entry pointed to by the sie control-block.

perf_misc_flags()
perf code code calls this function in order to associate the kernel
vs. user state infomation with a particular sample. Architecture
specific code must also first detectif a KVM guest was running
at the time the sample was taken.

Christian Borntraeger d0321a24bf KVM: s390: Use common waitqueue
Lets use the common waitqueue for kvm cpus on s390. By itself it is
just a cleanup, but it should also improve the accuracy of diag 0x44
which is implemented via kvm_vcpu_on_spin. kvm_vcpu_on_spin has
an explicit check for waiting on the waitqueue to optimize the
yielding.

Christian Borntraeger 69d0d3a316 KVM: s390: guest large pages
This patch enables kvm to give large pages to the guest. The heavy
lifting is done by the hardware, the host only has to take care
of the PFMF instruction, which is also part of EDAT-1.

We also support the non-quiescing key setting facility if the host
supports it, to behave similar to the interpretation of sske.

Peter Oberparleiter 98f6d1a668 s390/sclp: fix new line detection
When printing multi-line text using sclp_print, line endings are not
correctly handled. The routine is expecting an EBCDIC new line character
as line terminator while the input text is encoded in ASCII format.

Fix this problem by modifying sclp_print to scan for ASCII new line
characters.

Martin Schwidefsky 9cc5c206d9 s390/dumpstack: fix address ranges for asynchronous and panic stack
git commit dc7ee00d47 "s390: lowcore stack pointer offsets"
introduced a regression in regard to show_stack(). The stack pointer
for the asynchronous and the panic stack in the lowcore now have an
additional offset applied to them. This offset needs to be taken into
account in the calculation for the low and high address for the stacks.

Christian Borntraeger 338679f7ba s390/pgtable: Fix guest overindication for change bit
When doing the transition invalid->valid in the host page table for
a guest, then the guest view of C/R is in the pgste. After validation
the view is pgste OR real key. We must zero out the real key C/R to
avoid guest over-indication for change (and reference).

Touching the real key is ok also for the host: The change bit is
tracked via write protection and the reference bit is also ok
because set_pte_at was called and  the page will be touched anyway
soon. Furthermore architecture defines reference as "substantially
accurate", over- and underindication are ok.

'buf[2]' is 2 bytes length, and sprintf() will append '\0' at the end
of string "?\n", so original implementation is memory overflow.

Need use strncpy() and strnlen() instead of sprintf().

Heiko Carstens 9acf73b7d0 s390/smp: lost IPIs on cpu hotplug
IPIs might be lost when a cpu gets brought offline:

When stop_machine executes its state machine there is a race window
for the state STOPMACHINE_DISABLE_IRQ where the to be brought offline
cpu might already have irqs disabled but a different cpu still may
have irqs enabled.
If the enabled cpu receives an interrupt and as a result sends an IPI
to the to be offlined cpu in its bottom halve context, the IPI won't
be noticed before the cpu is offline.

In fact the race window is much larger since there is no guarantee
when an IPI will be received.

To fix this check for enqueued but not yet received IPIs in the
cpu_disable() path and call the respective handlers before the cpu
is marked offline.

Michael Holzheu 576ebd7492 kernel: Fix s390 absolute memory access for /dev/mem
On s390 the prefix page and absolute zero pages are not correctly
returned when reading /dev/mem. The reason is that the s390 asm/io.h
file includes the asm-generic/io.h file which then defines
xlate_dev_mem_ptr() and therefore overwrites the s390 specific
version that does the correct swap operation for prefix and absolute
zero pages. The problem is a regression that was introduced with git
commit cd248341 (s390/pci: base support).

To fix the problem add "#ifndef xlate_dev_mem_ptr" in asm-generic/io.h
and "#define xlate_dev_mem_ptr" in asm/io.h. This ensures that the
s390 version is used. For completeness also add the "#ifndef"
construct for xlate_dev_kmem_ptr().

Christian Borntraeger f8b5ff2cff s390: fix gmap_ipte_notifier vs. software dirty pages
On heavy paging load some guest cpus started to loop in gmap_ipte_notify.
This was visible as stalled cpus inside the guest. The gmap_ipte_notifier
tries to map a user page and then made sure that the pte is valid and
writable. Turns out that with the software change bit tracking the pte
can become read-only (and only software writable) if the page is clean.
Since we loop in this code, the page would stay clean and, therefore,
be never writable again.
Let us just use fixup_user_fault, that guarantees to call handle_mm_fault.

Christian Borntraeger 49b99e1e0d s390/kvm: Provide a way to prevent reentering SIE
Lets provide functions to prevent KVM from reentering SIE and
to kick cpus out of SIE. We cannot use the common kvm_vcpu_kick code,
since we need to kick out guests in places that hold architecture
specific locks (e.g. pgste lock) which might be necessary on the
other cpus - so no waiting possible.

So lets provide a bit in a private field of the sie control block
that acts as a gate keeper, after we claimed we are in SIE.
Please note that we do not reuse prog0c, since we want to access
that bit without atomic ops.

Christian Borntraeger 617e164c35 s390: disable pfmf for clear page instruction
Wit the introduction of large pages Linux also used pfmf for page
clearing. The current implementation is not ideal, though:
- currently we set usage intent=0, but cleared pages are often used
directly after the clearing
- z/VM does not yet provide EDAT
- KVM does have to intercept PFMF even for resident pages

Lets just the mvcl loop in all cases until we have a well defined
pattern were pfmf is besser.

If the size of the opcode to be printed is larger than "len" we'll
see an overflow of an unsigned long value, which means that the
while loop within print_fn_code() will loop quite a long time until
there is the next chance for an exit.
So add an early exit check.

Heiko Carstens e5d709bb5f s390/memory hotplug: provide memory_block_size_bytes() function
Commit 0c2c99b1b "memory hotplug: Allow memory blocks to span
multiple memory sections" introduced a weak memory_block_size_bytes()
function which can be used to set the size of a memory block as
seen in sysfs.
Provide an s390 specific override which makes sure that each
memory block has at least a size of 256MB or the increment size of
of a memory increment, whatever is larger.
This way we can make sure that the number of memory sysfs objects
doesn't explode for very large memory configurations.

Heiko Carstens df1bd59c5c s390/mem_detect: limit memory detection loop to "mem=" parameter
The current memory detection loop will detect all present memory of
a machine. This is true even if the user specified the "mem=" parameter
on the kernel command line.
This can be a problem since the memory detection may cause a fully
populated host page table for the guest, even for those parts of the
memory that the guest will never use afterwards.

So fix this and only detect memory up to a user supplied "mem=" limit
if specified.

Heiko Carstens 67b5c3eeb4 s390/kdump,bootmem: fix bootmem allocator bitmap size
When in kdump mode the kernel may access only the first couple of
megabytes for execution, the rest contains the dump. However
the size of the bitmap used by the bootmem allocator was calculated
for the whole amount of memory of the machine.

For very large machines this can lead to the situation that the kdump
kernel will not come up because not enough memory is available.

So fix this and calculate the size of the bitmap only for the piece
of memory that the kdump kernel actually uses.

Call reserve_oldmem() before setup_memory_end() so that the memory_chunk
array already has been updated with respect to oldmem chunks.
Afterwards setup_memory_end() will ignore those chunks.

Heiko Carstens d009f4d886 s390/mem_detect: fix lockdep irq tracing
When disabling and enabling interrupts we must tell lockdep.
So use local_irq_save()/restore() to disable and enable interrupts.
The DAT disabling/enabling get handled separately now.
Note: we may not call trace_hardirqs_on() with DAT disabled, since
the generic code may access vmalloc'ed data structures.

Stephen Boyd 446f24d119 Kconfig: consolidate CONFIG_DEBUG_STRICT_USER_COPY_CHECKS
The help text for this config is duplicated across the x86, parisc, and
s390 Kconfig.debug files.  Arnd Bergman noted that the help text was
slightly misleading and should be fixed to state that enabling this
option isn't a problem when using pre 4.4 gcc.

To simplify the rewording, consolidate the text into lib/Kconfig.debug
and modify it there to be more explicit about when you should say N to
this config.

Also, make the text a bit more generic by stating that this option
enables compile time checks so we can cover architectures which emit
warnings vs.  ones which emit errors.  The details of how an
architecture decided to implement the checks isn't as important as the
concept of compile time checking of copy_from_user() calls.

While we're doing this, remove all the copy_from_user_overflow() code
that's duplicated many times and place it into lib/ so that any
architecture supporting this option can get the function for free.

Tejun Heo a43cb95d54 dump_stack: unify debug information printed by show_regs()
show_regs() is inherently arch-dependent but it does make sense to print
generic debug information and some archs already do albeit in slightly
different forms.  This patch introduces a generic function to print debug
information from show_regs() so that different archs print out the same
information and it's much easier to modify what's printed.

show_regs_print_info() prints out the same debug info as dump_stack()
does plus task and thread_info pointers.

* Archs which didn't print debug info now do.

  alpha, arc, blackfin, c6x, cris, frv, h8300, hexagon, ia64, m32r,
  metag, microblaze, mn10300, openrisc, parisc, score, sh64, sparc,
  um, xtensa

* Already prints debug info.  Replaced with show_regs_print_info().
  The printed information is superset of what used to be there.

  arm, arm64, avr32, mips, powerpc, sh32, tile, unicore32, x86

* s390 is special in that it used to print arch-specific information
  along with generic debug info.  Heiko and Martin think that the
  arch-specific extra isn't worth keeping s390 specfic implementation.
  Converted to use the generic version.

Note that now all archs print the debug info before actual register
dumps.

Tejun Heo 196779b9b4 dump_stack: consolidate dump_stack() implementations and unify their behaviors
Both dump_stack() and show_stack() are currently implemented by each
architecture.  show_stack(NULL, NULL) dumps the backtrace for the
current task as does dump_stack().  On some archs, dump_stack() prints
extra information - pid, utsname and so on - in addition to the
backtrace while the two are identical on other archs.

The usages in arch-independent code of the two functions indicate
show_stack(NULL, NULL) should print out bare backtrace while
dump_stack() is used for debugging purposes when something went wrong,
so it does make sense to print additional information on the task which
triggered dump_stack().

There's no reason to require archs to implement two separate but mostly
identical functions.  It leads to unnecessary subtle information.

This patch expands the dummy fallback dump_stack() implementation in
lib/dump_stack.c such that it prints out debug information (taken from
x86) and invokes show_stack(NULL, NULL) and drops arch-specific
dump_stack() implementations in all archs except blackfin.  Blackfin's
dump_stack() does something wonky that I don't understand.

Debug information can be printed separately by calling
dump_stack_print_info() so that arch-specific dump_stack()
implementation can still emit the same debug information.  This is used
in blackfin.

This patch brings the following behavior changes.

* On some archs, an extra level in backtrace for show_stack() could be
  printed.  This is because the top frame was determined in
  dump_stack() on those archs while generic dump_stack() can't do that
  reliably.  It can be compensated by inlining dump_stack() but not
  sure whether that'd be necessary.

* Most archs didn't use to print debug info on dump_stack().  They do
  now.

v2: CPU number added to the generic debug info as requested by s390
    folks and dropped the s390 specific dump_stack().  This loses %ksp
    from the debug message which the maintainers think isn't important
    enough to keep the s390-specific dump_stack() implementation.

    dump_stack_print_info() is moved to kernel/printk.c from
    lib/dump_stack.c.  Because linkage is per objecct file,
    dump_stack_print_info() living in the same lib file as generic
    dump_stack() means that archs which implement custom dump_stack()
    - at this point, only blackfin - can't use dump_stack_print_info()
    as that will bring in the generic version of dump_stack() too.  v1
    The v1 patch broke build on blackfin due to this issue.  The build
    breakage was reported by Fengguang Wu.

Johannes Weiner 0aad818b2d sparse-vmemmap: specify vmemmap population range in bytes
The sparse code, when asking the architecture to populate the vmemmap,
specifies the section range as a starting page and a number of pages.

This is an awkward interface, because none of the arch-specific code
actually thinks of the range in terms of 'struct page' units and always
translates it to bytes first.

In addition, later patches mix huge page and regular page backing for
the vmemmap.  For this, they need to call vmemmap_populate_basepages()
on sub-section ranges with PAGE_SIZE and PMD_SIZE in mind.  But these
are not necessarily multiples of the 'struct page' size and so this unit
is too coarse.

Just translate the section range into bytes once in the generic sparse
code, then pass byte ranges down the stack.

Gerald Schaefer 106c992a5e mm/hugetlb: add more arch-defined huge_pte functions
Commit abf09bed3c ("s390/mm: implement software dirty bits")
introduced another difference in the pte layout vs.  the pmd layout on
s390, thoroughly breaking the s390 support for hugetlbfs.  This requires
replacing some more pte_xxx functions in mm/hugetlbfs.c with a
huge_pte_xxx version.

This patch introduces those huge_pte_xxx functions and their generic
implementation in asm-generic/hugetlb.h, which will now be included on
all architectures supporting hugetlbfs apart from s390.  This change
will be a no-op for those architectures.

Sebastian Ott 1c21351b72 s390/pci: use pci_scan_root_bus
The pci config space accessors on s390 are (now) smart enough to
figure out if a pci function is available. So instead of calling
pci_create_root_bus and then pci_scan_single_device for each
available function just call pci_scan_root_bus and let the pci core
do the scanning (via config reads on all possible functions) and
device creation.

Martin Schwidefsky 616498813b s390: system call path micro optimization
Add a pointer to the system call table to the thread_info structure.
The TIF_31BIT bit is set or cleared by SET_PERSONALITY exactly once
for the lifetime of a process. With the pointer to the correct system
call table in thread_info the system call code in entry64.S path can
drop the check for TIF_31BIT which saves a couple of instructions.

s390's statfs[64] definition:

The bug is caused by an implicit sign extension within the stat tool:

out_uint_x (pformat, prefix_len, statfsbuf->f_type);

where the format finally will be "%lx".
A similar problem can be found in the 'tail' tool.
s390 is the only architecture which has an int type f_type member in
struct statfs[64]. Other architectures have either unsigned ints or
long values, so that the problem doesn't occur there.

Therefore change the type of the f_type member to unsigned int, so
that we get zero extension instead of sign extension when assignment to
a long value happens.

This patch changes the s390 uapi struct stafs[64] definition in the kernel
to contain only unsigned values.
This was true for 32 bit builds anyway, since we use the generic uapi
header file in that case. So lets not include conditionally the generic
uapi header file but have the s390 implementation completely independent.

Also fix the types of struct compat_stafs to match reality and move the
definition of struct compat_statfs64 to asm/compat.h since it is not part
of the api.

Gerald Schaefer 186f50fa56 s390/pci: return correct dma address for offset > PAGE_SIZE
For offset > PAGE_SIZE, s390_dma_map_pages() will issue a warning
and return a wrong dma address.

This patch removes the warning and fixes the dma return address
calculation.
