Calling kthread_run with a single name parameter causes it to be handled
as a format string. Many callers are passing potentially dynamic string
content, so use "%s" in those cases to avoid any potential accidents.

The capability field of v4l2_tuner should be ORed by the various subdevs
and by the main driver. In this case the stereo capability was dropped.

Convert the struct i2c_msg initialization to C99 format. This makes
maintaining and editing the code simpler. Also helps once other fields
like transferred are added in future.
