Introduce struct iommu_dev_data.use_vapic flag, which IOMMU driver
uses to determine if it should enable vAPIC support, by setting
the ga_mode bit in the device's interrupt remapping table entry.
Currently, it is enabled for all pass-through device if vAPIC mode
is enabled.

This patch enables support for the new 128-bit IOMMU IRTE format,
which can be used for both legacy and vapic interrupt remapping modes.
It replaces the existing operations on IRTE, which can only support
the older 32-bit IRTE format, with calls to the new struct amd_irt_ops.
It also provides helper functions for setting up, accessing, and
updating interrupt remapping table entries in different mode.

This patch introduces a new IOMMU driver parameter, amd_iommu_guest_ir,
which can be used to specify different interrupt remapping mode for
passthrough devices to VM guest:
* legacy: Legacy interrupt remapping (w/ 32-bit IRTE)
* vapic : Guest vAPIC interrupt remapping (w/ GA mode 128-bit IRTE)
Note that in vapic mode, it can also supports legacy interrupt remapping
for non-passthrough devices with the 128-bit IRTE.

There is a race condition in the AMD IOMMU init code that
causes requested unity mappings to be blocked by the IOMMU
for a short period of time. This results on boot failures
and IO_PAGE_FAULTs on some machines.
Fix this by making sure the unity mappings are installed
before all other DMA is blocked.

The IVRS in more recent AMD system usually contains multiple
IVHD block types (e.g. 0x10, 0x11, and 0x40) for each IOMMU.
The newer IVHD types provide more information (e.g. new features
specified in the IOMMU spec), while maintain compatibility with
the older IVHD type.
Having multiple IVHD type allows older IOMMU drivers to still function
(e.g. using the older IVHD type 0x10) while the newer IOMMU driver can use
the newer IVHD types (e.g. 0x11 and 0x40). Therefore, the IOMMU driver
should only make use of the newest IVHD type that it can support.
This patch adds new logic to determine the highest level of IVHD type
it can support, and use it throughout the to initialize the driver.
This requires adding another pass to the IVRS parsing to determine
appropriate IVHD type (see function get_highest_supported_ivhd_type())
before parsing the contents.

The IVHD header type 11h and 40h introduce the PCSup bit in
the EFR Register Image bit fileds. This should be used to
determine the IOMMU performance support instead of relying
on the PNCounters and PNBanks.
Note also that the PNCouters and PNBanks bits in the IOMMU
attributes field of IVHD headers type 11h are incorrectly
programmed on some systems.
So, we should not rely on it to determine the performance
counter/banks size. Instead, these values should be read
from the MMIO Offset 0030h IOMMU Extended Feature Register.

The AMD Family 15h Models 30h-3Fh (Kaveri) BIOS and Kernel Developer's
Guide omitted part of the BIOS IOMMU L2 register setup specification.
Without this setup the IOMMU L2 does not fully respect write permissions
when handling an ATS translation request.
The IOMMU L2 will set PTE dirty bit when handling an ATS translation with
write permission request, even when PTE RW bit is clear. This may occur by
direct translation (which would cause a PPR) or by prefetch request from
the ATC.
This is observed in practice when the IOMMU L2 modifies a PTE which maps a
pagecache page. The ext4 filesystem driver BUGs when asked to writeback
these (non-modified) pages.
Enable ATS write permission check in the Kaveri IOMMU L2 if BIOS has not.

The setup code for the performance counters in the AMD IOMMU driver
tests whether the counters can be written. It tests to setup a counter
for device 00:00.0, which fails on systems where this particular device
is not covered by the IOMMU.
Fix this by not relying on device 00:00.0 but only on the IOMMU being
present.

AMD IOMMU driver makes use of IOMMU PCI devices, so prevent binding other

Its a bit odd that debugfs_create_bool() takes 'u32 *' as an argument,
when all it needs is a boolean pointer.
It would be better to update this API to make it accept 'bool *'
instead, as that will make it more consistent and often more convenient.
Over that bool takes just a byte.
That required updates to all user sites as well, in the same commit
updating the API. regmap core was also using
debugfs_{read|write}_file_bool(), directly and variable types were
updated for that to be bool as well.

Fix amd_iommu_detect() to return positive value on success, like
intended, and not zero. This will not change anything in the end
as AMD IOMMU disable swiotlb and properly associate itself with

in-memory structures and not through the MMIO register region.
The ARM SMMUv3 also supports IO demand paging for PCI devices

the driver yet.
* Lots of cleanups and device-tree support for the Exynos IOMMU
driver. This is part of the effort to bring Exynos DRM support
upstream.
* Introduction of default domains into the IOMMU core code. The
rationale behind this is to move functionalily out of the
IOMMU drivers to common code to get to a unified behavior
between different drivers.
The patches here introduce a default domain for iommu-groups
(isolation groups). A device will now always be attached to a
domain, either the default domain or another domain handled by
the device driver. The IOMMU drivers have to be modified to
make use of that feature. So long the AMD IOMMU driver is
converted, with others to follow.
* Patches for the Intel VT-d drvier to fix DMAR faults that
happen when a kdump kernel boots. When the kdump kernel boots
it re-initializes the IOMMU hardware, which destroys all
mappings from the crashed kernel. As this happens before
the endpoint devices are re-initialized, any in-flight DMA
causes a DMAR fault. These faults cause PCI master aborts,
which some devices can't handle properly and go into an
undefined state, so that the device driver in the kdump kernel
fails to initialize them and the dump fails.
This is now fixed by copying over the mapping structures (only
context tables and interrupt remapping tables) from the old
kernel and keep the old mappings in place until the device
driver of the new kernel takes over. This emulates the the
behavior without an IOMMU to the best degree possible.

Commit 7fa1c842ca "iommu/irq_remapping: Change variable
disable_irq_remap to be static" returns unconditionally success from
the irq remapping prepare callback if the iommu can be initialized.
The change assumed that iommu_go_to_state(IOMMU_ACPI_FINISHED) returns
a failure if irq remapping is not enabled, but thats not the case.
The function returns success when the iommu is initialized to the
point which is required for remapping to work. The actual state of the
irq remapping feature is reflected in the status variable
amd_iommu_irq_remap, which is not considered in the return value.
The fix is simple: If the iommu_go_to_state() returns success,
evaluate the remapping state amd_iommu_irq_remap and reflect it in the
return value.

When the device id for an IOAPIC is overridden on the kernel

AMD-Vi support for IOMMU sysfs. This allows us to associate devices
with a specific IOMMU device and examine the capabilities and features
of that IOMMU. The AMD IOMMU is hosted on and actual PCI device, so
we make that device the parent for the IOMMU class device. This
initial implementaiton exposes only the capability header and extended
features register for the IOMMU.

set_device_exclusion_range(u16 devid, struct ivmd_header *m) enables
exclusion range for ONE device. IOMMU does not translate the access
to the exclusion range from the device.
The device is specified by input argument 'devid'. But 'devid' is not
passed to the actual set function set_dev_entry_bit(), instead
'm->devid' is passed. 'm->devid' does not specify the exact device
which needs enable the exclusion range. 'm->devid' represents DeviceID
field of IVMD, which has different meaning depends on IVMD type.
The caller init_exclusion_range() sets 'devid' for ONE device. When
m->type is equal to ACPI_IVMD_TYPE_ALL or ACPI_IVMD_TYPE_RANGE,
'm->devid' is not equal to 'devid'.
This patch fixes 'm->devid' to 'devid'.

In reality, the spec can only support 16-bit PASID since
INVALIDATE_IOTLB_PAGES and COMPLETE_PPR_REQUEST commands only allow 16-bit
PASID. So, we updated the PASID_MASK accordingly and invoke BUG_ON
if the hardware is reporting PASmax more than 16-bit.
Besides, max PASID is defined as ((2^(PASmax+1)) - 1). The current does not
determine this correctly.

Replace direct inclusions of <acpi/acpi.h>, <acpi/acpi_bus.h> and
<acpi/acpi_drivers.h>, which are incorrect, with <linux/acpi.h>
inclusions and remove some inclusions of those files that aren't
necessary.
First of all, <acpi/acpi.h>, <acpi/acpi_bus.h> and <acpi/acpi_drivers.h>
should not be included directly from any files that are built for
CONFIG_ACPI unset, because that generally leads to build warnings about
undefined symbols in !CONFIG_ACPI builds. For CONFIG_ACPI set,
<linux/acpi.h> includes those files and for CONFIG_ACPI unset it
provides stub ACPI symbols to be used in that case.
Second, there are ordering dependencies between those files that always
have to be met. Namely, it is required that <acpi/acpi_bus.h> be included
prior to <acpi/acpi_drivers.h> so that the acpi_pci_root declarations the
latter depends on are always there. And <acpi/acpi.h> which provides
basic ACPICA type declarations should always be included prior to any other
ACPI headers in CONFIG_ACPI builds. That also is taken care of including
<linux/acpi.h> as appropriate.

PCI core will initialize device MSI/MSI-X capability in
pci_msi_init_pci_dev(). So device driver should use
pci_dev->msi_cap/msix_cap to determine whether the device
support MSI/MSI-X instead of using
pci_find_capability(pci_dev, PCI_CAP_ID_MSI/MSIX). Access
to PCIe device config space again will consume more time.

Add functionality to check the availability of the AMD IOMMU Performance
Counters and export this functionality to other core drivers, such as in this
case, a perf AMD IOMMU PMU. This feature is not bound to any specific AMD
family/model other than the presence of the IOMMU with P-C enabled.
The AMD IOMMU P-C support static counting only at this time.

and an extension to the IOMMU group interface. On the x86 side this

In the current interrupt handling scheme, there are as many threads as
the number of IOMMUs. Each thread is created and assigned to an IOMMU at
the time of registering interrupt handlers (request_threaded_irq).
When an IOMMU HW generates an interrupt, the irq handler (top half) wakes up
the corresponding thread to process event and PPR logs of all IOMMUs
starting from the 1st IOMMU.
In the system with multiple IOMMU,this handling scheme complicates the
synchronization of the IOMMU data structures and status registers as
there could be multiple threads competing for the same IOMMU while
the other IOMMU could be left unhandled.
To simplify, this patch is proposing a different interrupt handling scheme
by having each thread only managing interrupts of the corresponding IOMMU.
This can be achieved by passing the struct amd_iommu when registering the
interrupt handlers. This structure is unique for each IOMMU and can be used
by the bottom half thread to identify the IOMMU to be handled instead
of calling for_each_iommu. Besides this also eliminate the needs to lock
the IOMMU for processing event and PPR logs.

When the IVRS entries for IOAPIC and HPET are overridden on
the kernel command line, a problem detected in the check
function might not be a firmware bug anymore. So disable
the firmware bug reporting if the user provided valid
ivrs_ioapic or ivrs_hpet entries on the command line.

Add two new kernel commandline parameters ivrs_ioapic and
ivrs_hpet to override the Id->DeviceId mapping from the IVRS
ACPI table. This can be used to work around broken BIOSes to
get interrupt remapping working on AMD systems.

When dma_ops are initialized the unity mappings are
created. The init_device_table_dma() function makes sure DMA
from all devices is blocked by default. This opens a short
window in time where DMA to unity mapped regions is blocked
by the IOMMU. Make sure this does not happen by initializing
the device table after dma_ops.

The IOMMU may stop processing page translations due to a perceived lack
of credits for writing upstream peripheral page service request (PPR)
or event logs. If the L2B miscellaneous clock gating feature is enabled
the IOMMU does not properly register credits after the log request has
completed, leading to a potential system hang.
BIOSes are supposed to disable L2B micellaneous clock gating by setting
L2_L2B_CK_GATE_CONTROL[CKGateL2BMiscDisable](D0F2xF4_x90[2]) = 1b. This
patch corrects that for those which do not enable this workaround.

On some systems the BIOS puts the wrong device-id for the
IO-APIC into the IVRS table. The result is that interrupt
remapping is not working for the IO-APIC irqs. This usually
means a kernel panic at boot because the timer is not
working.
Fix this kernel panic by disabling interrupt remapping if
this problem is discovered in the IVRS table.

Do not deinitialize the AMD IOMMU driver completly when
interrupt remapping is already in use but the initialization
of the DMA layer fails for some reason. Make sure the IOMMU

When the IOMMU is enabled very early (as with irq-remapping)
some devices are still in BIOS hand. When dma is blocked
early this can cause lots of IO_PAGE_FAULTs. So delay the
DMA initialization and do it right before the dma_ops are
initialized.
To be secure, block all interrupts by default when irq-remapping is
enabled in the system. They will be reenabled on demand
later. Without blocking interrupts by default devices can
issue arbitrary interrupts by sending special DMA packets to
the CPU that look like MSI messages. This is especially
dangerous when a device is assigned to a KVM guest because
the guest can then DoS the host.

When the IOAPIC information provided in the IVRS table is
not correct or not complete the system may not boot at all
when interrupt remapping is enabled. So check if this
information is correct and print out a firmware bug message
when it is not.

This step makes it very easy to keep track about the current
intialization state of the iommu driver. With this change we
can initialize the IOMMU hardware to a point where it can
remap interrupts and later resume the initializion to enable
dma remapping.

This function will be called before the PCI subsystem is
initialized. Therefore dev_name doen't work and IOMMU
information can't be printed to the klog as before. Move the
code to print that information to a later point where PCI
initializtion has already happened.

For interrupt remapping the relevant IOMMU initialization
needs to run earlier at boot when the PCI subsystem is not
yet initialized. To support that this patch splits the parts
of IOMMU initialization which need PCI accesses out of the
initial setup path so that this can be done later.

write_file_bool() modifies 32 bits of data, so "amd_iommu_unmap_flush"
needs to be 32 bits as well or we'll corrupt memory. Fortunately it
looks like the data is aligned with a gap after the declaration so this
is harmless in production.

The iommu_shutdown callback is not initialized when the AMD
IOMMU driver runs in passthrough mode. Fix that by moving
the callback initialization before the check for
passthrough mode.

At some point pci_get_bus_and_slot started to enable
interrupts. Since this function is used in the
amd_iommu_resume path it will enable interrupts on resume
which causes a warning. The fix will use a cached pointer
to the root-bridge to re-enable the IOMMU in case the BIOS
is broken.

For interrupt remapping the enablement of the IOMMU MSI
interrupt needs to be deferred because the IOMMU itself will
be initialized before the io-apics are up and running. So
the code to setup the MSI is moved seperated from the
hardware-setup routine now.

Unfortunatly the interrupts for the event log and the
peripheral page-faults are only enabled at boot but not
re-enabled at resume. Fix that.

When ioremap_nocache fails in iommu initialization the code
calls release_mem_region immediatly. But the function is
called again when the propagates into the upper init
functions leading to a double-free. Fix that.

The hardware-initializtion part of the AMD IOMMU driver is
split out into a seperate function. This function can now be
called either from amd_iommu_init() itself or any other
place if the hardware needs to be ready earlier. This will
be used to implement interrupt remapping for AMD.

To protect the command buffer from hanging when a device
does not respond to an IOTLB invalidation, set a timeout of
1s for outstanding IOTLB invalidations.

If the device starts to use IOMMUv2 features the dma handles
need to stay valid. The only sane way to do this is to use a
identity mapping for the device and not translate it by the
iommu. This is implemented with this patch. Since this lifts
the device-isolation there is also a new kernel parameter
which allows to disable that feature.

In mixed IOMMU setups this flag inidicates whether an IOMMU
supports the v2 features or not. This patch also adds a
global flag together with a function to query that flag from
other code. The flag shows if at least one IOMMUv2 is in the
system.
