Alex Dewar 90ca6333fd VMCI: check return value of get_user_pages_fast() for errors
In a couple of places in qp_host_get_user_memory(),
get_user_pages_fast() is called without properly checking for errors. If
e.g. -EFAULT is returned, this negative value will then be passed on to
qp_release_pages(), which expects a u64 as input.

Fix this by only calling qp_release_pages() when we have a positive
number returned.

Linus Torvalds 0da522107e compat_ioctl: remove most of fs/compat_ioctl.c
As part of the cleanup of some remaining y2038 issues, I came to
fs/compat_ioctl.c, which still has a couple of commands that need support
 for time64_t.

In completely unrelated work, I spent time on cleaning up parts of this
file in the past, moving things out into drivers instead.

After Al Viro reviewed an earlier version of this series and did a lot
more of that cleanup, I decided to try to completely eliminate the rest
of it and move it all into drivers.

This series incorporates some of Al's work and many patches of my own,
but in the end stops short of actually removing the last part, which is
the scsi ioctl handlers. I have patches for those as well, but they need
more testing or possibly a rewrite.

Stefano Garzarella b1bba80a43 vsock/vmci: register vmci_transport only when VMCI guest/host are active
To allow other transports to be loaded with vmci_transport,
we register the vmci_transport as G2H or H2G only when a VMCI guest

To do that, this patch adds a callback registered in the vmci driver
that will be called when the host or guest becomes active.
This callback will register the vmci_transport in the VSOCK core.

Arnd Bergmann 1832f2d8ff compat_ioctl: move more drivers to compat_ptr_ioctl
The .ioctl and .compat_ioctl file operations have the same prototype so
they can both point to the same function, which works great almost all
the time when all the commands are compatible.

One exception is the s390 architecture, where a compat pointer is only
31 bit wide, and converting it into a 64-bit pointer requires calling
compat_ptr(). Most drivers here will never run in s390, but since we now
have a generic helper for it, it's easy enough to use it consistently.

I double-checked all these drivers to ensure that all ioctl arguments
are used as pointers or are ignored, but are not interpreted as integer
values.

The cause for the bug is that when the "delayed" doorbell is invoked, it
takes a reference on the doorbell entry and schedules work that is
supposed to run the appropriate code and drop the doorbell entry
reference. The code ignores the fact that if the work is already queued,
it will not be scheduled to run one more time. As a result one of the
references would not be dropped. When the code waits for the reference
to get to zero, during balloon reset or module removal, it gets stuck.

Fix it. Drop the reference if schedule_work() indicates that the work is
already queued.

Note that this bug got more apparent (or apparent at all) due to
commit ce664331b2 ("vmw_balloon: VMCI_DOORBELL_SET does not check status").

Vishnu DASA 1c2eb5b285 VMCI: Fix integer overflow in VMCI handle arrays
The VMCI handle array has an integer overflow in
vmci_handle_arr_append_entry when it tries to expand the array. This can be
triggered from a guest, since the doorbell link hypercall doesn't impose a
limit on the number of doorbell handles that a VM can create in the
hypervisor, and these handles are stored in a handle array.

In this change, we introduce a mandatory max capacity for handle
arrays/lists to avoid excessive memory usage.

Ira Weiny 73b0140bf0 mm/gup: change GUP fast to use flags rather than a write 'bool'
To facilitate additional options to get_user_pages_fast() change the
singular write parameter to be gup_flags.

This patch does not change any functionality. New functionality will
follow in subsequent patches.

Some of the get_user_pages_fast() call sites were unchanged because they
already passed FOLL_WRITE or 0 for the write parameter.

NOTE: It was suggested to change the ordering of the get_user_pages_fast()
arguments to ensure that callers were converted. This breaks the current
GUP call site convention of having the returned pages be the final
parameter. So the suggestion was rejected.

Linus Torvalds 96d4f267e4 Remove 'type' argument from access_ok() function
Nobody has actually used the type (VERIFY_READ vs VERIFY_WRITE) argument
of the user address range verification function since we got rid of the
old racy i386-only code to walk page tables by hand.

It existed because the original 80386 would not honor the write protect
bit when in kernel mode, so you had to do COW by hand before doing any
user access. But we haven't supported that in a long time, and these
days the 'type' argument is a purely historical artifact.

A discussion about extending 'user_access_begin()' to do the range
checking resulted this patch, because there is no way we're going to
move the old VERIFY_xyz interface to that model. And it's best done at
the end of the merge window when I've done most of my merges, so let's
just get this done once and for all.

This patch was mostly done with a sed-script, with manual fix-ups for
the cases that weren't of the trivial 'access_ok(VERIFY_xyz' form.

There were a couple of notable cases:

 - csky still had the old "verify_area()" name as an alias.

 - the iter_iov code had magical hardcoded knowledge of the actual
values of VERIFY_{READ,WRITE} (not that they mattered, since nothing
   really used it)

 - microblaze used the type argument for a debug printout

but other than those oddities this should be a total no-op patch.

I tried to fix up all architectures, did fairly extensive grepping for
access_ok() uses, and the changes are trivial, but I may have missed
something. Any missed conversion should be trivially fixable, though.

David Howells aa563d7bca iov_iter: Separate type from direction and use accessor functions
In the iov_iter struct, separate the iterator type from the iterator
direction and use accessor functions to access them in most places.

Convert a bunch of places to use switch-statements to access them rather
then chains of bitwise-AND statements. This makes it easier to add further
iterator types. Also, this can be more efficient as to implement a switch
of small contiguous integers, the compiler can use ~50% fewer compare
instructions than it has to use bitwise-and instructions.

Further, cease passing the iterator type into the iterator setup function.
The iterator function can set that itself. Only the direction is required.

Kees Cook 6da2ec5605 treewide: kmalloc() -> kmalloc_array()
The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:

        kmalloc(a * b, gfp)

with:
        kmalloc_array(a * b, gfp)

as well as handling cases of:

        kmalloc(a * b * c, gfp)

with:

        kmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kmalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

The Coccinelle script used for this was:

// Fix redundant parens around sizeof().

Linus Torvalds a9a08845e9 vfs: do bulk POLL* -> EPOLL* replacement
This is the mindless scripted replacement of kernel use of POLL*
variables as described by Al, done by this script:

for V in IN OUT PRI ERR RDNORM RDBAND WRNORM WRBAND HUP RDHUP NVAL MSG; do
        L=`git grep -l -w POLL$V | grep -v '^t' | grep -v /um/ | grep -v '^sa' | grep -v '/poll.h$'|grep -v '^D'`
        for f in $L; do sed -i "-es/^\([^\"]*\)\(\<POLL$V\>\)/\\1E\\2/" $f; done
    done

with de-mangling cleanups yet to come.

NOTE! On almost all architectures, the EPOLL* constants have the same
values as the POLL* constants do. But they keyword here is "almost".
For various bad reasons they aren't the same, and epoll() doesn't
actually work quite correctly in some cases due to this on Sparc et al.

The next patch from Al will sort out the final differences, and we
should be all done.

In the following patches we are going to remove various headers
from sched.h and other headers that sched.h includes.

To make those patches build cleanly prepare the scene by adding
dependencies to various files that learned to rely on those
to-be-removed dependencies.

These changes all make sense standalone: they add a header for
a data type that a particular .c or .h file is using.

Jorgen Hansen eb94cd68ab VMCI: Doorbell create and destroy fixes
This change consists of two changes:

1) If vmci_doorbell_create is called when neither guest nor
host personality as been initialized, vmci_get_context_id
will return VMCI_INVALID_ID. In that case, we should fail
   the create call.
2) In doorbell destroy, we assume that vmci_guest_code_active()
has the same return value on create and destroy. That may not
be the case, so we may end up with the wrong refcount.
Instead, destroy should check explicitly whether the doorbell
is in the index table as an indicator of whether the guest
code was active at create time.

Kirill A. Shutemov 09cbfeaf1a mm, fs: get rid of PAGE_CACHE_* and page_cache_{get,release} macros
PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.

This promise never materialized. And unlikely will.

We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE. And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.

Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.

Let's stop pretending that pages in page cache are special. They are
not.

The changes are pretty straight-forward:

 - <foo> << (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} -> PAGE_{SIZE,SHIFT,MASK,ALIGN};

 - page_cache_get() -> get_page();

 - page_cache_release() -> put_page();

This patch contains automated changes generated with coccinelle using
script below. For some reason, coccinelle doesn't patch header files.
I've called spatch for them manually.

The only adjustment after coccinelle is revert of changes to
PAGE_CAHCE_ALIGN definition: we are going to drop it later.

There are few places in the code where coccinelle didn't reach. I'll
fix them manually in a separate patch. Comments and documentation also
will be addressed with the separate patch.

Jorgen Hansen f42a0fd13b VMCI: Use 32bit atomics for queue headers on X86_32
This change restricts the reading and setting of the head and tail
pointers on 32bit X86 to 32bit for both correctness and
performance reasons. On uniprocessor X86_32, the atomic64_read
may be implemented as a non-locked cmpxchg8b. This may result in
updates to the pointers done by the VMCI device being overwritten.
On MP systems, there is no such correctness issue, but using 32bit
atomics avoids the overhead of the locked 64bit operation. All this
is safe because the queue size on 32bit systems will never exceed
a 32bit value.

Greg Kroah-Hartman f368ed6088 char: make misc_deregister a void function
With well over 200+ users of this api, there are a mere 12 users that
actually checked the return value of this function. And all of them
really didn't do anything with that information as the system or module
was shutting down no matter what.

So stop pretending like it matters, and just return void from
misc_deregister(). If something goes wrong in the call, you will get a
WARNING splat in the syslog so you know how to fix up your driver.
Other than that, there's nothing that can go wrong.

Jorgen Hansen aa6467f190 VMCI: Guard against overflow in queue pair allocation
The current maximum size of a queue in a queue pair is 128 MB. If
we increase that in the future, the queue pair allocation routines
may run into overflow issues. This change adds additional checks
to guard against this.

Ensure that the size filled in by userland in the datagram header
matches the size of the buffer passed down in the IOCTL. Note that we

Davidlohr Bueso bf1361211d drivers/vmw_vmci: Show correct get_user_pages_fast upon failure
As of 240ddd495a (vmw_vmci: Convert driver to use get_user_pages_fast())
we no longer user get_user_pages(), thus update the warning.

Also convert to pr_debug, which is a more appropriate level of logging.

Linus Torvalds 4ba63072b9 Char / Misc patches for 3.20-rc1
Here's the big char/misc driver update for 3.20-rc1.

Lots of little things in here, all described in the changelog. Nothing
major or unusual, except maybe the binder selinux stuff, which was all
acked by the proper selinux people and they thought it best to come
 through this tree.

All of this has been in linux-next with no reported issues for a while.

Jorgen Hansen a1d88436d5 VMCI: Fix two UVA mapping bugs
(this is a resend of this patch. Originally sent last year, but post
 appears to have been lost)

This change fixes two bugs in the VMCI host driver related to mapping
the notify boolean from user space into kernel space:
- the actual UVA was rounded up to the next page boundary - resulting
in memory corruption in the calling process whenever notifications
would be signalled. This has been fixed by just removing the
PAGE_ALIGN part, since get_user_pages_fast can figure this out on
  its own
- the mapped page wasn't stored anywhere, so it wasn't unmapped and
put back when a VMCI context was destroyed. Fixed this by
  remembering the page.
