Bart Van Assche bbb427e342 blkcg: Unlock blkcg_pol_mutex only once when cpd == NULL
Unlocking a mutex twice is wrong. Hence modify blkcg_policy_register()
such that blkcg_pol_mutex is unlocked once if cpd == NULL. This patch
avoids that smatch reports the following error:

Glauber Costa 3932a86b4b cfq: fix starvation of asynchronous writes
While debugging timeouts happening in my application workload (ScyllaDB), I have
observed calls to open() taking a long time, ranging everywhere from 2 seconds -
the first ones that are enough to time out my application - to more than 30
seconds.

The problem seems to happen because XFS may block on pending metadata updates
under certain circumnstances, and that's confirmed with the following backtrace
taken by the offcputime tool (iovisor/bcc):

Inspecting my run with blktrace, I can see that the xfsaild kthread exhibit very
high "Dispatch wait" times, on the dozens of seconds range and consistent with
the open() times I have saw in that run.

Still from the blktrace output, we can after searching a bit, identify the
request that wasn't dispatched:

As we can see above, in this particular example CFQ took 15 seconds to dispatch
this request. Going back to the full trace, we can see that the xfsaild queue
had plenty of opportunity to run, and it was selected as the active queue many
times. It would just always be preempted by something else (example):

where cfq767 is the xfsaild queue and cfq1618 corresponds to one of the ScyllaDB
IO dispatchers.

The requests preempting the xfsaild queue are synchronous requests. That's a
characteristic of ScyllaDB workloads, as we only ever issue O_DIRECT requests.
While it can be argued that preempting ASYNC requests in favor of SYNC is part
of the CFQ logic, I don't believe that doing so for 15+ seconds is anyone's
goal.

Moreover, unless I am misunderstanding something, that breaks the expectation
set by the "fifo_expire_async" tunable, which in my system is set to the
default.

Looking at the code, it seems to me that the issue is that after we make
an async queue active, there is no guarantee that it will execute any request.

When the queue itself tests if it cfq_may_dispatch() it can bail if it sees SYNC
requests in flight. An incoming request from another queue can also preempt it
in such situation before we have the chance to execute anything (as seen in the
trace above).

This patch sets the must_dispatch flag if we notice that we have requests
that are already fifo_expired. This flag is always cleared after
cfq_dispatch_request() returns from cfq_dispatch_requests(), so it won't pin
the queue for subsequent requests (unless they are themselves expired)

Care is taken during preempt to still allow rt requests to preempt us
regardless.

Testing my workload with this patch applied produces much better results.
From the application side I see no timeouts, and the open() latency histogram
generated by systemtap looks much better, with the worst outlier at 131ms:

Sebastian Andrzej Siewior 97a32864e6 blk-mq: fixup "Convert to new hotplug state machine"
The "blk_mq_queue_reinit_dead()" just cleared the cpumask instead doing
a copy. Since we might never had an online callback we could end up with
a ZERO mask which in turn leads to crash as test robot demonstarted.

Two cases:

1) blk_mq_alloc_request() needlessly re-runs the queue, after
   calling into the tag allocation without NOWAIT set. We don't
   need to do that.

2) blk_mq_map_request() should just use blk_mq_run_hw_queue() with
   the async flag set to false.

Vivek Goyal 164c80ed84 blk-throttle: Extend slice if throttle group is not empty
Right now, if slice is expired, we start a new slice. If a bio is
queued, we keep on extending slice by throtle_slice interval (100ms).

This worked well as long as pending timer function got executed with-in
few milli seconds of scheduled time. But looks like with recent changes
in timer subsystem, slack can be much longer depending on the expiry time
of the scheduled timer.

commit 500462a9de ("timers: Switch to a non-cascading wheel")

This means, by the time timer function gets executed, it is possible the
delay from scheduled time is more than 100ms. That means current code
will conclude that existing slice has expired and a new one needs to
be started. New slice will be 100ms by default and that will not be
sufficient to meet rate requirement of group given the bio size and
bio will not be dispatched and we will start a new timer function to
wait. And when that timer expires, same process will repeat and we
will wait again and this can easily be an infinite loop.

Solve this issue by starting a new slice only if throttle gropup is
empty. If it is not empty, that means there should be an active slice
going on. Ideally it should not be expired but given the slack, it is
possible that it has expired.

Omar Sandoval 98d95416db sbitmap: randomize initial alloc_hint values
In order to get good cache behavior from a sbitmap, we want each CPU to
stick to its own cacheline(s) as much as possible. This might happen
naturally as the bitmap gets filled up and the alloc_hint values spread
out, but we really want this behavior from the start. blk-mq apparently
intended to do this, but the code to do this was never wired up. Get rid
of the dead code and make it part of the sbitmap library.

Omar Sandoval 40aabb6746 sbitmap: push per-cpu last_tag into sbitmap_queue
Allocating your own per-cpu allocation hint separately makes for an
awkward API. Instead, allocate the per-cpu hint as part of the struct
sbitmap_queue. There's no point for a struct sbitmap_queue without the
cache, but you can still use a bare struct sbitmap.

Omar Sandoval 88459642cb blk-mq: abstract tag allocation out into sbitmap library
This is a generally useful data structure, so make it available to
anyone else who might want to use it. It's also a nice cleanup
separating the allocation logic from the rest of the tag handling logic.

The code is behind a new Kconfig option, CONFIG_SBITMAP, which is only
selected by CONFIG_BLOCK for now.

This should be a complete noop functionality-wise.

Jens Axboe 703fd1c0f1 blk-mq: account higher order dispatch
We currently account a '0' dispatch, and anything above that still falls
below the range set by BLK_MQ_MAX_DISPATCH_ORDER. If we dispatch more,
we don't account it.

Change the last bucket to be inclusive of anything above the range we
track, and have the sysfs file reflect that by including a '+' in the
output:

This allows drivers specify their own queue mapping by overriding the
setup-time function that builds the mq_map.  This can be used for
example to build the map based on the MSI-X vector mapping provided

Christoph Hellwig 7d7e0f90b7 blk-mq: remove ->map_queue
All drivers use the default, so provide an inline version of it.  If we
ever need other queue mapping we can add an optional method back,
although supporting will also require major changes to the queue setup
code.

This provides better code generation, and better debugability as well.

Christoph Hellwig bdd17e75cd blk-mq: only allocate a single mq_map per tag_set
The mapping is identical for all queues in a tag_set, so stop wasting
memory for building multiple.  Note that for now I've kept the mq_map
pointer in the request_queue, but we'll need to investigate if we can
remove it without suffering too much from the additional pointer chasing.
The same would apply to the mq_ops pointer as well.

Christoph Hellwig 4e68a01142 blk-mq: don't redistribute hardware queues on a CPU hotplug event
Currently blk-mq will totally remap hardware context when a CPU hotplug
even happened, which causes major havoc for drivers, as they are never
told about this remapping.  E.g. any carefully sorted out CPU affinity
will just be completely messed up.

The rebuild also doesn't really help for the common case of cpu
hotplug, which is soft onlining / offlining of cpus - in this case we
should just leave the queue and irq mapping as is.  If it actually
worked it would have helped in the case of physical cpu hotplug,
although for that we'd need a way to actually notify the driver.
Note that drivers may already be able to accommodate such a topology
change on their own, e.g. using the reset_controller sysfs file in NVMe
will cause the driver to get things right for this case.

With the rebuild removed we will simplify retain the queue mapping for
a soft offlined CPU that will work when it comes back online, and will
map any newly onlined CPU to queue 0 until the driver initiates
a rebuild of the queue map.

blk_mq_delay_kick_requeue_list() provides the ability to kick the
q->requeue_list after a specified time.  To do this the request_queue's
'requeue_work' member was changed to a delayed_work.

blk_mq_delay_kick_requeue_list() allows DM to defer processing requeued
requests while it doesn't make sense to immediately requeue them
(e.g. when all paths in a DM multipath have failed).

Jens Axboe 0e87e58bf6 blk-mq: improve warning for running a queue on the wrong CPU
__blk_mq_run_hw_queue() currently warns if we are running the queue on a
CPU that isn't set in its mask. However, this can happen if a CPU is
being offlined, and the workqueue handling will place the work on CPU0
instead. Improve the warning so that it only triggers if the batch cpu
in the hardware queue is currently online.  If it triggers for that
case, then it's indicative of a flow problem in blk-mq, so we want to
retain it for that case.

Jens Axboe e57690fe00 blk-mq: don't overwrite rq->mq_ctx
We do this in a few places, if the CPU is offline. This isn't allowed,
though, since on multi queue hardware, we can't just move a request
from one software queue to another, if they map to different hardware
queues. The request and tag isn't valid on another hardware queue.

This can happen if plugging races with CPU offlining. But it does
no harm, since it can only happen in the window where we are
currently busy freezing the queue and flushing IO, in preparation
for redoing the software <-> hardware queue mappings.

Jens Axboe 1eff9d322a block: rename bio bi_rw to bi_opf
Since commit 63a4cc2486, bio->bi_rw contains flags in the lower
portion and the op code in the higher portions. This means that
old code that relies on manually setting bi_rw is most likely
going to be broken. Instead of letting that brokeness linger,
rename the member, to force old and out-of-tree code to break
at compile time instead of at runtime.

No intended functional changes in this commit.

The name for a bdi of a gendisk is derived from the gendisk's devt.
However, since the gendisk is destroyed before the bdi it leaves a

Gabriel Krisman Bertazi 71f79fb317 blk-mq: Allow timeouts to run while queue is freezing
In case a submitted request gets stuck for some reason, the block layer
can prevent the request starvation by starting the scheduled timeout work.
If this stuck request occurs at the same time another thread has started
a queue freeze, the blk_mq_timeout_work will not be able to acquire the
queue reference and will return silently, thus not issuing the timeout.
But since the request is already holding a q_usage_counter reference and
is unable to complete, it will never release its reference, preventing
the queue from completing the freeze started by first thread.  This puts
the request_queue in a hung state, forever waiting for the freeze
completion.

This was observed while running IO to a NVMe device at the same time we
toggled the CPU hotplug code. Eventually, once a request got stuck
requiring a timeout during a queue freeze, we saw the CPU Hotplug
notification code get stuck inside blk_mq_freeze_queue_wait, as shown in
the trace below.

The fix is to allow the timeout work to execute in the window between
dropping the initial refcount reference and the release of the last
reference, which actually marks the freeze completion.  This can be
achieved with percpu_refcount_tryget, which does not require the counter
to be alive.  This way the timeout work can do it's job and terminate a
stuck request even during a freeze, returning its reference and avoiding
the deadlock.

Allowing the timeout to run is just a part of the fix, since for some
devices, we might get stuck again inside the device driver's timeout
handler, should it attempt to allocate a new request in that path -
which is a quite common action for Abort commands, which need to be sent
after a timeout.  In NVMe, for instance, we call blk_mq_alloc_request
from inside the timeout handler, which will fail during a freeze, since
it also tries to acquire a queue reference.

I considered a similar change to blk_mq_alloc_request as a generic
solution for further device driver hangs, but we can't do that, since it
would allow new requests to disturb the freeze process.  I thought about
creating a new function in the block layer to support unfreezable
requests for these occasions, but after working on it for a while, I
feel like this should be handled in a per-driver basis.  I'm now
experimenting with changes to the NVMe timeout path, but I'm open to
suggestions of ways to make this generic.

Paolo Valente 20bd723ec6 block: add missing group association in bio-cloning functions
When a bio is cloned, the newly created bio must be associated with
the same blkcg as the original bio (if BLK_CGROUP is enabled). If
this operation is not performed, then the new bio is not associated
with any group, and the group of the current task is returned when
the group of the bio is requested.

Depending on the cloning frequency, this may cause a large
percentage of the bios belonging to a given group to be treated
as if belonging to other groups (in most cases as if belonging to
the root group). The expected group isolation may thereby be broken.

This commit adds the missing association in bio-cloning functions.

Ross Zwisler 99a01cdf9d block: remove BLK_DEV_DAX config option
The functionality for block device DAX was already removed with commit
acc93d30d7 ("Revert "block: enable dax for raw block devices"")

However, we still had a config option hanging around that was always
disabled because it depended on CONFIG_BROKEN.  This config option was
introduced in commit 03cdadb040 ("block: disable block device DAX by
default")

This change reverts that commit, removing the dead config option.

Tahsin Erdogan 72ef799b3f block: do not merge requests without consulting with io scheduler
Before merging a bio into an existing request, io scheduler is called to
get its approval first. However, the requests that come from a plug
flush may get merged by block layer without consulting with io
scheduler.

In case of CFQ, this can cause fairness problems. For instance, if a
request gets merged into a low weight cgroup's request, high weight cgroup
now will depend on low weight cgroup to get scheduled. If high weigt cgroup
needs that io request to complete before submitting more requests, then it
will also lose its timeslice.

Following script demonstrates the problem. Group g1 has a low weight, g2
and g3 have equal high weights but g2's requests are adjacent to g1's
requests so they are subject to merging. Due to these merges, g2 gets
poor disk time allocation.

Christoph Hellwig 4613c5f1df scsi/osd: open code blk_make_request
I wish the OSD code could simply use blk_rq_map_* helpers like
everyone else, but the complex nature of deciding if we have
DATA IN and/or DATA OUT buffers might make this impossible
(at least for a mere human like me).

But using blk_rq_append_bio at least allows sharing the setup code
between request with or without dat a buffers, and given that this
is the last user of blk_make_request it allows getting rid of that
somewhat awkward interface.

Christoph Hellwig 0c4de0f33b block: ensure bios return from blk_get_request are properly initialized
blk_get_request is used for BLOCK_PC and similar passthrough requests.
Currently we always need to call blk_rq_set_block_pc or an open coded
version of it to allow appending bios using the request mapping helpers
later on, which is a somewhat awkward API.  Instead move the
initialization part of blk_rq_set_block_pc into blk_get_request, so that
we always have a safe to use request.

Christoph Hellwig e950fdf71c block: introduce BLKDEV_DISCARD_ZERO to fix zeroout
Currently blkdev_issue_zeroout cascades down from discards (if the driver
guarantees that discards zero data), to WRITE SAME and then to a loop
writing zeroes.  Unfortunately we ignore run-time EOPNOTSUPP errors in the
block layer blkdev_issue_discard helper to work around DM volumes that
may have mixed discard support underneath.

This patch intoroduces a new BLKDEV_DISCARD_ZERO flag to
blkdev_issue_discard that indicates we are called for zeroing operation.
This allows both to ignore the EOPNOTSUPP hack and actually consolidating
the discard_zeroes_data check into the function.

For 4K LBA or very large disks, atari_partition can easily get tricked
into thinking it has found an Atari partition table.  Depending on the
data in the disk, it ends up creating partitions with awkward lengths.

We saw logs like this while playing with fio.

just bail out early, before even attempting to read sector 0.

Commit 9a7f38c42c (cfq-iosched: Convert from jiffies to nanoseconds)
could result in charging just 1 ns to a cgroup submitting IO instead of 1
jiffie we always charged before. It is arguable what is the right amount
to change but for now lets retain the old behavior of always charging at
least one jiffie.

Jan Kara 149321a611 cfq-iosched: Fix regression in bonnie++ rewrite performance
Commit 9a7f38c42c (cfq-iosched: Convert from jiffies to nanoseconds)
broke the condition for detecting starved sync IO in
cfq_completed_request() because rq->start_time remained in jiffies but
we compared it with nanosecond values. This manifested as a regression
in bonnie++ rewrite performance because we always ended up considering
sync IO starved and thus never increased async IO queue depth.

Since rq->start_time is used in a lot of places, converting it to ns
values would be non-trivial. So just revert the condition in CFQ to use
comparison with jiffies. This will lead to suboptimal results if
cfq_fifo_expire[1] will ever come close to 1 jiffie but so far we are
relatively far from that with the storage used with CFQ (the default
value is 128 ms).

Jan Kara 93fdf1478a cfq-iosched: Convert slice_resid from u64 to s64
slice_resid can be both positive and negative. Commit 9a7f38c42c
(cfq-iosched: Convert from jiffies to nanoseconds) converted it from
long to u64. Although this did not introduce any functional regression
(the operations just overflow and the result was fine), it is certainly
wrong and could cause issues in future. So convert the type to more
appropriate s64.

Jan Kara 9828c2c6c1 block: Convert fifo_time from ulong to u64
Currently rq->fifo_time is unsigned long but CFQ stores nanosecond
timestamp in it which would overflow on 32-bit archs. Convert it to u64
to avoid the overflow. Since the rq->fifo_time is unioned with struct
call_single_data(), this does not change the size of struct request in
any way.

We have to slightly fixup block/deadline-iosched.c so that comparison
happens in the right types.

Jens Axboe b8269db456 cfq-iosched: temporarily boost queue priority for idle classes
If we're queuing REQ_PRIO IO and the task is running at an idle IO
class, then temporarily boost the priority. This prevents livelocks
due to priority inversion, when a low priority task is holding file
system resources while attempting to do IO.

An example of that is shown below. An ioniced idle task is holding
the directory mutex, while a normal priority task is trying to do
a directory lookup.

Omar Sandoval 52b9c330c6 blk-mq: actually hook up defer list when running requests
If ->queue_rq() returns BLK_MQ_RQ_QUEUE_OK, we use continue and skip
over the rest of the loop body. However, dptr is assigned later in the
loop body, and the BLK_MQ_RQ_QUEUE_OK case is exactly the case that we'd
want it for.

NVMe isn't actually using BLK_MQ_F_DEFER_ISSUE yet, nor is any other
in-tree driver, but if the code's going to be there, it might as well
work.

Convert all time-keeping in CFQ IO scheduler from jiffies to nanoseconds
so that we can later make the intervals more fine-grained than jiffies.
One jiffie is several miliseconds and even for today's rotating disks
that is a noticeable amount of time and thus we leave disk unnecessarily
idle.

We currently set REQ_WRITE/WRITE for all non READ IOs
like discard, flush, writesame, etc. In the next patches where we
no longer set up the op as a bitmap, we will not be able to
detect a operation direction like writesame by testing if REQ_WRITE is
set.

This patch converts the drivers and cgroup to use the
op_is_write helper. This should just cover the simple
cases. I did dm, md and bcache in their own patches
because they were more involved.

Ming Lin c7de572630 blk-mq: clear q->mq_ops if init fail
blk_mq_init_queue() calls blk_mq_init_allocated_queue(), but q->mq_ops
was not cleared when blk_mq_init_allocated_queue() fails.
Then blk_cleanup_queue() calls blk_mq_free_queue() which will crash because:
- q->all_q_node is not added to all_q_list yet
- q->tag_set is NULL
- hctx was not setup yet or already freed

Fixed it by clearing q->mq_ops on error path.

blkdev_dax_capable() is similar to bdev_dax_supported(), but needs
to remain as a separate interface for checking dax capability of
a raw block device.

Rename and relocate blkdev_dax_capable() to keep them maintained
consistently, and call bdev_direct_access() for the dax capability
check.

There is no change in the behavior.

Mike Snitzer 0ef5a50c16 block: make bio_inc_remaining() interface accessible again
Commit 326e1dbb57 ("block: remove management of bi_remaining when
restoring original bi_end_io") made bio_inc_remaining() private to bio.c
because the only use-case that made sense was confined to the
bio_chain() interface.

Since that time DM thinp went on to use bio_chain() in its relatively
complex implementation of async discard support.  That implementation,
even when converted over to use the new async __blkdev_issue_discard()
interface, depends on deferred completion of the original discard bio --
which is most appropriately implemented using bio_inc_remaining().

DM thinp foolishly duplicated bio_inc_remaining(), local to dm-thin.c as
__bio_inc_remaining(), so re-exporting bio_inc_remaining() allows us to
put an end to that foolishness.

All said, bio_inc_remaining() should really only be used in conjunction
with bio_chain().  It isn't intended for generic bio reference counting.

Mike Snitzer bbd848e0fa block: reinstate early return of -EOPNOTSUPP from blkdev_issue_discard
Commit 38f2525533 ("block: add __blkdev_issue_discard") incorrectly
disallowed the early return of -EOPNOTSUPP if the device doesn't support
discard (or secure discard).  This early return of -EOPNOTSUPP has
always been part of blkdev_issue_discard() interface so there isn't a
good reason to break that behaviour -- especially when it can be easily
reinstated.

The nuance of allowing early return of -EOPNOTSUPP vs disallowing late
return of -EOPNOTSUPP is: if the overall device never advertised support
for discards and one is issued to the device it is beneficial to inform
the caller that discards are not supported via -EOPNOTSUPP.  But if a
device advertises discard support it means that at least a subset of the
device does have discard support -- but it could be that discards issued
to some regions of a stacked device will not be supported.  In that case
the late return of -EOPNOTSUPP must be disallowed.

Jens Axboe 93e9d8e836 block: add ability to flag write back caching on a device
Add an internal helper and flag for setting whether a queue has
write back caching, or write through (or none). Add a sysfs file
to show this as well, and make it changeable from user space.

This will replace the (awkward) blk_queue_flush() interface that
drivers currently use to inform the block layer of write cache state
and capabilities.

Kirill A. Shutemov 09cbfeaf1a mm, fs: get rid of PAGE_CACHE_* and page_cache_{get,release} macros
PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} macros were introduced *long* time
ago with promise that one day it will be possible to implement page
cache with bigger chunks than PAGE_SIZE.

This promise never materialized.  And unlikely will.

We have many places where PAGE_CACHE_SIZE assumed to be equal to
PAGE_SIZE.  And it's constant source of confusion on whether
PAGE_CACHE_* or PAGE_* constant should be used in a particular case,
especially on the border between fs and mm.

Global switching to PAGE_CACHE_SIZE != PAGE_SIZE would cause to much
breakage to be doable.

Let's stop pretending that pages in page cache are special.  They are
not.

The changes are pretty straight-forward:

 - <foo> << (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - <foo> >> (PAGE_CACHE_SHIFT - PAGE_SHIFT) -> <foo>;

 - PAGE_CACHE_{SIZE,SHIFT,MASK,ALIGN} -> PAGE_{SIZE,SHIFT,MASK,ALIGN};

 - page_cache_get() -> get_page();

 - page_cache_release() -> put_page();

This patch contains automated changes generated with coccinelle using
script below.  For some reason, coccinelle doesn't patch header files.
I've called spatch for them manually.

The only adjustment after coccinelle is revert of changes to
PAGE_CAHCE_ALIGN definition: we are going to drop it later.

There are few places in the code where coccinelle didn't reach.  I'll
fix them manually in a separate patch.  Comments and documentation also
will be addressed with the separate patch.

Thomas Gleixner 897bb0c7f1 blk-mq: Use proper cpumask iterator
queue_for_each_ctx() iterates over per_cpu variables under the assumption that
the possible cpu mask cannot have holes. That's wrong as all cpumasks can have
holes. In case there are holes the iteration ends up accessing uninitialized
memory and crashing as a result.

Replace the macro by a proper for_each_possible_cpu() loop and drop the unused
macro blk_ctx_sum() which references queue_for_each_ctx().

San Mehat 0d9c51a6e1 block: partition: add partition specific uevent callbacks for partition info
This patch has been carried in the Android tree for quite some time and
is one of the few patches required to get a mainline kernel up and
running with an exsiting Android userspace.  So I wanted to submit it
for review and consideration if it should be merged.

For partitions, add new uevent parameters 'PARTN' which specifies the
partitions index in the table, and 'PARTNAME', which specifies PARTNAME
specifices the partition name of a partition device.

Android's userspace uses this for creating device node links from the
partition name and number, ie:

    /dev/block/platform/soc/by-name/system

    /dev/block/platform/soc/by-num/p1

Christoph Hellwig 4d6af73d9e block: support large requests in blk_rq_map_user_iov
This patch adds support for larger requests in blk_rq_map_user_iov by
allowing it to build multiple bios for a request.  This functionality
used to exist for the non-vectored blk_rq_map_user in the past, and
this patch reuses the existing functionality for it on the unmap side,
which stuck around.  Thanks to the iov_iter API supporting multiple
bios is fairly trivial, as we can just iterate the iov until we've
consumed the whole iov_iter.

Mike Snitzer 6acfe68bac dm: fix excessive dm-mq context switching
Request-based DM's blk-mq support (dm-mq) was reported to be 50% slower
than if an underlying null_blk device were used directly.  One of the
reasons for this drop in performance is that blk_insert_clone_request()
was calling blk_mq_insert_request() with @async=true.  This forced the
use of kblockd_schedule_delayed_work_on() to run the blk-mq hw queues
which ushered in ping-ponging between process context (fio in this case)
and kblockd's kworker to submit the cloned request.  The ftrace
function_graph tracer showed:

  ...

Fixing blk_insert_clone_request()'s blk_mq_insert_request() call to
_not_ use kblockd to submit the cloned requests isn't enough to
eliminate the observed context switches.

In addition to this dm-mq specific blk-core fix, there are 2 DM core
fixes to dm-mq that (when paired with the blk-core fix) completely
eliminate the observed context switching:

1)  don't blk_mq_run_hw_queues in blk-mq request completion

    Motivated by desire to reduce overhead of dm-mq, punting to kblockd
    just increases context switches.

    In my testing against a really fast null_blk device there was no benefit
    to running blk_mq_run_hw_queues() on completion (and no other blk-mq
    driver does this).  So hopefully this change doesn't induce the need for
    yet another revert like commit 621739b00e !

2)  use blk_mq_complete_request() in dm_complete_request()

    blk_complete_request() doesn't offer the traditional q->mq_ops vs
    .request_fn branching pattern that other historic block interfaces
    do (e.g. blk_get_request).  Using blk_mq_complete_request() for
    blk-mq requests is important for performance.  It should be noted
    that, like blk_complete_request(), blk_mq_complete_request() doesn't
    natively handle partial completions -- but the request-based
    DM-multipath target does provide the required partial completion
    support by dm.c:end_clone_bio() triggering requeueing of the request
    via dm-mpath.c:multipath_end_io()'s return of DM_ENDIO_REQUEUE.

dm-mq fix #2 is _much_ more important than #1 for eliminating the
context switches.
Before: cpu          : usr=15.10%, sys=59.39%, ctx=7905181, majf=0, minf=475
After:  cpu          : usr=20.60%, sys=79.35%, ctx=2008, majf=0, minf=472

With these changes multithreaded async read IOPs improved from ~950K
to ~1350K for this dm-mq stacked on null_blk test-case.  The raw read
IOPs of the underlying null_blk device for the same workload is ~1950K.

Mika Westerberg d07ab6d114 block: Add blk_set_runtime_active()
If block device is left runtime suspended during system suspend, resume
hook of the driver typically corrects runtime PM status of the device back
to "active" after it is resumed. However, this is not enough as queue's
runtime PM status is still "suspended". As long as it is in this state
blk_pm_peek_request() returns NULL and thus prevents new requests to be
processed.

Add new function blk_set_runtime_active() that can be used to force the
queue status back to "active" as needed.

Ming Lei 6684167216 blk-mq: mark request queue as mq asap
Currently q->mq_ops is used widely to decide if the queue
is mq or not, so we should set the 'flag' asap so that both
block core and drivers can get the correct mq info.

For example, commit 868f2f0b720(blk-mq: dynamic h/w context count)
moves the hctx's initialization before setting q->mq_ops in
blk_mq_init_allocated_queue(), then cause blk_alloc_flush_queue()
to think the queue is non-mq and don't allocate command size
for the per-hctx flush rq.

This patches should fix the problem reported by Sasha.

Hannes Reinecke 2d99b55d37 bio: return EINTR if copying to user space got interrupted
Commit 35dc248383 introduced a check for
current->mm to see if we have a user space context and only copies data
if we do. Now if an IO gets interrupted by a signal data isn't copied
into user space any more (as we don't have a user space context) but
user space isn't notified about it.

This patch modifies the behaviour to return -EINTR from bio_uncopy_user()
to notify userland that a signal has interrupted the syscall, otherwise
it could lead to a situation where the caller may get a buffer with
no data returned.

This can be reproduced by issuing SG_IO ioctl()s in one thread while
constantly sending signals to it.

Keith Busch 868f2f0b72 blk-mq: dynamic h/w context count
The hardware's provided queue count may change at runtime with resource
provisioning. This patch allows a block driver to alter the number of
h/w queues available when its resource count changes.

The main part is a new blk-mq API to request a new number of h/w queues
for a given live tag set. The new API freezes all queues using that set,
then adjusts the allocated count prior to remapping these to CPUs.

The bulk of the rest just shifts where h/w contexts and all their
artifacts are allocated and freed.

The number of max h/w contexts is capped to the number of possible cpus
since there is no use for more than that. As such, all pre-allocated
memory for pointers need to account for the max possible rather than
the initial number of queues.

A side effect of this is that the blk-mq will proceed successfully as
long as it can allocate at least one h/w context. Previously it would
fail request queue initialization if less than the requested number
was allocated.

Roman Pen 39a169b62b block: fix module reference leak on put_disk() call for cgroups throttle
get_disk(),get_gendisk() calls have non explicit side effect: they
increase the reference on the disk owner module.

The following is the correct sequence how to get a disk reference and
to put it:

    disk = get_gendisk(...);

    owner = disk->fops->owner;
    put_disk(disk);
    module_put(owner);

fs/block_dev.c is aware of this required module_put() call, but f.e.
blkg_conf_finish(), which is located in block/blk-cgroup.c, does not put
a module reference.  To see a leakage in action cgroups throttle config
can be used.  In the following script I'm removing throttle for /dev/ram0
(actually this is NOP, because throttle was never set for this device):

When a storage device rejects a WRITE SAME command we will disable write
same functionality for the device and return -EREMOTEIO to the block
layer. -EREMOTEIO will in turn prevent DM from retrying the I/O and/or
failing the path.

Yiwen Jiang discovered a small race where WRITE SAME requests issued
simultaneously would cause -EIO to be returned. This happened because
any requests being prepared after WRITE SAME had been disabled for the
device caused us to return BLKPREP_KILL. The latter caused the block
layer to return -EIO upon completion.

To overcome this we introduce BLKPREP_INVALID which indicates that this
is an invalid request for the device. blk_peek_request() is modified to
return -EREMOTEIO in that case.

Jan Kara 3984aa5520 cfq-iosched: Allow parent cgroup to preempt its child
Currently we don't allow sync workload of one cgroup to preempt sync
workload of any other cgroup. This is because we want to achieve service
separation between cgroups. However in cases where cgroup preempting is
ancestor of the current cgroup, there is no need of separation and
idling introduces unnecessary overhead. This hurts for example the case
when workload is isolated within a cgroup but journalling threads are in
root cgroup. Simple way to demostrate the issue is using:

dbench4 -c /usr/share/dbench4/client.txt -t 10 -D /mnt 1

on ext4 filesystem on plain SATA drive (mounted with barrier=0 to make
difference more visible). When all processes are in the root cgroup,
reported throughput is 153.132 MB/sec. When dbench process gets its own
blkio cgroup, reported throughput drops to 26.1006 MB/sec.

Fix the problem by making check in cfq_should_preempt() more benevolent
and allow preemption by ancestor cgroup. This improves the throughput
reported by dbench4 to 48.9106 MB/sec.

Jan Kara a257ae3e48 cfq-iosched: Allow sync noidle workloads to preempt each other
The original idea with preemption of sync noidle queues (introduced in
commit 718eee0579 "cfq-iosched: fairness for sync no-idle queues") was
that we service all sync noidle queues together, we don't idle on any of
the queues individually and we idle only if there is no sync noidle
queue to be served. This intention also matches the original test:

	if (cfqd->serving_type == SYNC_NOIDLE_WORKLOAD
	   && new_cfqq->service_tree == cfqq->service_tree)
		return true;

However since at that time cfqq->service_tree was not set for idling
queues, this test was unreliable and was replaced in commit e4a229196a
"cfq-iosched: fix no-idle preemption logic" by:

	    cfqq_type(new_cfqq) == SYNC_NOIDLE_WORKLOAD &&
	    new_cfqq->service_tree->count == 1)

That was a reliable test but was actually doing something different -
now we preempt sync noidle queue only if the new queue is the only one
busy in the service tree.

These days cfq queue is kept in service tree even if it is idling and
thus the original check would be safe again. But since we actually check
that cfq queues are in the same cgroup, of the same priority class and
workload type (sync noidle), we know that new_cfqq is fine to preempt
cfqq. So just remove the service tree check.

Dan Williams 9f4736fe7c block: revert runtime dax control of the raw block device
Dynamically enabling DAX requires that the page cache first be flushed
and invalidated.  This must occur atomically with the change of DAX mode
otherwise we confuse the fsync/msync tracking and violate data
durability guarantees.  Eliminate the possibilty of DAX-disabled to
DAX-enabled transitions for now and revisit this for the next cycle.

Ming Lei d0e5fbb01a block: fix bio splitting on max sectors
After commit e36f62042880(block: split bios to maxpossible length),
bio can be splitted in the middle of a vector entry, then it
is easy to split out one bio which size isn't aligned with block
size, especially when the block size is bigger than 512.

This patch fixes the issue by making the max io size aligned
to logical block size.

Keith Busch e36f620428 block: split bios to max possible length
This splits bio in the middle of a vector to form the largest possible
bio at the h/w's desired alignment, and guarantees the bio being split
will have some data.

The criteria for splitting is changed from the max sectors to the h/w's
optimal sector alignment if it is provided. For h/w that advertise their
block storage's underlying chunk size, it's a big performance win to not
submit commands that cross them. If sector alignment is not provided,
this patch uses the max sectors as before.

This addresses the performance issue commit d380561113 attempted to
fix, but was reverted due to splitting logic error.

Vishal Verma 99e6608c9e block: Add badblock management for gendisks
NVDIMM devices, which can behave more like DRAM rather than block
devices, may develop bad cache lines, or 'poison'. A block device
exposed by the pmem driver can then consume poison via a read (or
write), and cause a machine check. On platforms without machine
check recovery features, this would mean a crash.

The block device maintaining a runtime list of all known sectors that
have poison can directly avoid this, and also provide a path forward
to enable proper handling/recovery for DAX faults on such a device.

Use the new badblock management interfaces to add a badblocks list to
gendisks.

Dan Williams ac34f15e0c block: fix del_gendisk() vs blkdev_ioctl crash
When tearing down a block device early in its lifetime, userspace may
still be performing discovery actions like blkdev_ioctl() to re-read
partitions.

The nvdimm_revalidate_disk() implementation depends on
disk->driverfs_dev to be valid at entry.  However, it is set to NULL in
del_gendisk() and fatally this is happening *before* the disk device is
deleted from userspace view.

There's no reason for del_gendisk() to clear ->driverfs_dev.  That
device is the parent of the disk.  It is guaranteed to not be freed
until the disk, as a child, drops its ->parent reference.

We could also fix this issue locally in nvdimm_revalidate_disk() by
using disk_to_dev(disk)->parent, but lets fix it globally since
->driverfs_dev follows the lifetime of the parent.  Longer term we
should probably just add a @parent parameter to add_disk(), and stop
carrying this pointer in the gendisk.

Dan Williams 5a023cdba5 block: enable dax for raw block devices
If an application wants exclusive access to all of the persistent memory
provided by an NVDIMM namespace it can use this raw-block-dax facility
to forgo establishing a filesystem.  This capability is targeted
primarily to hypervisors wanting to provision persistent memory for
guests.  It can be disabled / enabled dynamically via the new BLKDAXSET
ioctl.

Jens Axboe 6126eb2483 Revert "block: Split bios on chunk boundaries"
This reverts commit d380561113.

If we end up splitting on the first segment, we don't adjust
the sector count. That results in hitting a BUG() with attempting
to split 0 sectors.

As this is just a performance issue and not a regression since
4.3 release, let's just rever this change. That gives us more
time to test a real fix for 4.5, which would be marked for
stable anyway.

We currently only have an inline/sync helper to restart a stopped
queue. If drivers need an async version, they have to roll their
own. Add a generic helper instead.

Keith Busch d380561113 block: Split bios on chunk boundaries
For h/w that advertise their block storage's underlying chunk size, it's
a big performance win to not submit commands that cross them. This patch
uses that criteria if it is provided. If it is not provided, this patch
uses the max sectors as before.

Junichi Nomura 23688bf4f8 block: ensure to split after potentially bouncing a bio
blk_queue_bio() does split then bounce, which makes the segment
counting based on pages before bouncing and could go wrong. Move
the split to after bouncing, like we do for blk-mq, and the we
fix the issue of having the bio count for segments be wrong.

Christoph Hellwig 287922eb0b block: defer timeouts to a workqueue
Timer context is not very useful for drivers to perform any meaningful abort
action from.  So instead of calling the driver from this useless context
defer it to a workqueue as soon as possible.

Note that while a delayed_work item would seem the right thing here I didn't
dare to use it due to the magic in blk_add_timer that pokes deep into timer
internals.  But maybe this encourages Tejun to add a sensible API for that to
the workqueue API and we'll all be fine in the end :)

Contains a major update from Keith Bush:

"This patch removes synchronizing the timeout work so that the timer can
 start a freeze on its own queue. The timer enters the queue, so timer
 context can only start a freeze, but not wait for frozen."

Tejun Heo 0b98f0c042 Merge branch 'master' into for-4.4-fixes
The following commit which went into mainline through networking tree

  3b13758f51 ("cgroups: Allow dynamically changing net_classid")

conflicts in net/core/netclassid_cgroup.c with the following pending
fix in cgroup/for-4.4-fixes.

  1f7dd3e5a6 ("cgroup: fix handling of multi-destination migration from subtree_control enabling")

The former separates out update_classid() from cgrp_attach() and
updates it to walk all fds of all tasks in the target css so that it
can be used from both migration and config change paths.  The latter
drops @css from cgrp_attach().

Resolve the conflict by making cgrp_attach() call update_classid()
with the css from the first task.  We can revive @tset walking in
cgrp_attach() but given that net_cls is v1 only where there always is
only one target css during migration, this is fine.
