Many files include the filename at the beginning, serveral used a wrong one.

Replace all uses of kmem_cache_t with struct kmem_cache.
The patch was generated using the following script:

Brown. This patch is useful only after adding support for 64-bit block
numbers in the filesystem.

jbd and jbd2 currently use the same slab names which must be unique. The
patch below just renames jbd2's slabs.
