The glue layer should directly write to mailbox register (present in
control module) instead of calling phy layer to write to mailbox
register. Writing to mailbox register notifies the core of events like
device connect/disconnect.
Currently writing to control module register is taken care in this
driver which will be removed once the control module driver is in place.

deprecated and in hardirq context the interrupt is disabled.

There is a possibility of QH overlay region having reference to a stale
qTD pointer during unlink.
Consider an endpoint having two pending qTD before unlink process begins.
The endpoint's QH queue looks like this.
qTD1 --> qTD2 --> Dummy
To unlink qTD2, QH is removed from asynchronous list and Asynchronous
Advance Doorbell is programmed. The qTD1's next qTD pointer is set to
qTD2'2 next qTD pointer and qTD2 is retired upon controller's doorbell
interrupt. If QH's current qTD pointer points to qTD1, transfer overlay
region still have reference to qTD2. But qtD2 is just unlinked and freed.
This may cause EHCI system error. Fix this by updating qTD next pointer
in QH overlay region with the qTD next pointer of the current qTD.

This patch adds two sysfs files for each usb hub port to allow userspace
to control the port power policy.
For an upcoming Intel xHCI roothub, this will translate into ACPI calls
to completely power off or power on the port. As a reminder, when these
ports are completely powered off, the USB host and device will see a
physical disconnect. All future USB device connections will be lost,
and the device will not be able to signal a remote wakeup.
The control sysfs file can be written to with two options:
"on" - port power must be on.
"off" - port must be off.
The state sysfs file reports usb port's power state:
"on" - powered on
"off" - powered off
"error" - can't get power state
For now, let userspace dictate the port power off policy. Future
patches may add an in-kernel policy.

Alan Stern pointed out that a USB port could potentially get powered off
when the attached USB device is in the middle of enumerating, due to
race conditions:

If that happens, we need to ensure the enumeration fails. If a call to
usb_get_descriptor() fails for a reason other than a Stall, return an
error. That should handle the case where the port is powered off.

Upcoming Intel systems will have an ACPI method to control whether a USB
port can be completely powered off. The implication of powering off a
USB port is that the device and host sees a physical disconnect, and
subsequent port connections and remote wakeups will be lost.
Add a new function, usb_acpi_power_manageable(), that can be used to
find whether the usb port has ACPI power resources that can be used to
power on and off the port on these machines. Also add a new function
called usb_acpi_set_power_state() that controls the port power via these
ACPI methods.
When the USB core calls into the xHCI hub driver to power off a port,
check whether the port can be completely powered off via this new ACPI
mechanism. If so, call into these new ACPI methods. Also use the ACPI
methods when the USB core asks to power on a port.

In the upcoming USB port power off patches, we need to know whether a
USB port can ever see a disconnect event. Often USB ports are internal
to a system, and users can't disconnect USB devices from that port.
Sometimes those ports will remain empty, because the OEM chose not to
connect an internal USB device to that port.
According to ACPI Spec 9.13, PLD indicates whether USB port is
user visible and _UPC indicates whether a USB device can be connected to
the USB port (we'll call this "connectible"). Here's a matrix of the
possible combinations:

USB_PORT_NOT_USED. When we get the USB port's acpi_handle, store the
state in connect_type in struct usb_port.

In the ACPI DSDT table, only usb root hub and usb ports are ACPI device
nodes. Originally, we bound the usb port's ACPI node to the usb device
attached to the port. However, we want to access those ACPI port
methods when the port is empty, and there's no usb_device associated
with that port.
Now that the usb port is a real device, we can bind the port's ACPI node
to struct usb_port instead.

The usb_device structure contains an array of usb_device "children".
This array is only valid if the usb_device is a hub, so it makes no
sense to store it there. Instead, store the usb_device child
in its parent usb_port structure.
Since usb_port is an internal USB core structure, add a new function to
get the USB device child, usb_hub_find_child(). Add a new macro,
usb_hub_get_each_child(), to iterate over all the children attached to a
particular USB hub.
Remove the printing the USB children array pointer from the usb-ip
driver, since it's really not necessary.

This patch turns each USB port on a hub into a new struct device. This
new device has the USB hub interface device as its parent. The port
devices are stored in a new structure (usb_port), and an array of
usb_ports are dynamically allocated once we know how many ports the USB
hub has.
Move the port_owner variable out of usb_hub and into this new structure.
A new file will be created in the hub interface sysfs directory, so
add documentation.

The kernel's version number is used as decimal in the bcdDevice field of
the RH descriptor. For kernel version v3.12 we would see 3.0c in lsusb.
I am not sure how important it is to stick with bcd values since this is
this way since we started git history and nobody complained (however back
then we reported only 2.6).

Apps which deal with devices which also have a kernel driver, need to do
the following:
1) Check which driver is attached, so as to not detach the wrong driver
(ie detaching usbfs while another instance of the app is using the device)
2) Detach the kernel driver
3) Claim the interface
Where moving from one step to the next for both 1-2 and 2-3 consists of
a (small) race window. So currently such apps are racy and people just live
with it.
This patch adds a new ioctl which makes it possible for apps to do this
in a race free manner. For flexibility apps can choose to:
1) Specify the driver to disconnect
2) Specify to disconnect any driver except for the one named by the app
3) Disconnect any driver
Note that if there is no driver attached, the ioctl will just act like the
regular claim-interface ioctl, this is by design, as returning an error for
this condition would open a new bag of race-conditions.

The Microchip vid:pid 04d8:000a is used for their CDC ACM
demo firmware application. This is a device with a single
function conforming to the CDC ACM specification and with
the intention of demonstrating CDC ACM class firmware and
driver interaction. The demo is used on a number of
development boards, and may also be used unmodified by
vendors using Microchip hardware.
Some vendors have re-used this vid:pid for other types of
firmware, emulating FTDI chips. Attempting to continue to
support such devices without breaking class based
applications that by matching on interface
class/subclass/proto being ff/ff/00. I have no information
about the actual device or interface descriptors, but this
will at least make the proper CDC ACM devices work again.
Anyone having details of the offending device's descriptors
should update this entry with the details.

Currently mx23 fails to enumerate a USB device:

Use a kernel workqueue to asynchronously delay the setting of
ENHOSTDISCONDETECT bit until after higher level hub connect/reset processing
is complete. Prematurely setting the bit prevents the connection
processing from completing and not setting it prevents disconnect from being
detected. No delay is needed for clearing of ENHOSTDISCONDETECT.
Successfully tested on mx23-olinuxino (micro, mini and maxi variants) and mx28evk.

The bcdDevice field is defined as
|Device release number in binary-coded decimal
in the USB 2.0 specification. We use this field to distinguish the UDCs
from each other. In theory this could be used on the host side to apply
certain quirks if the "special" UDC in combination with this gadget is
used. This hasn't been done as far as I am aware. In practice it would
be better to fix the UDC driver before shipping since a later release
might not need this quirk anymore.
There are some driver in tree (on the host side) which use the bcdDevice
field to figure out special workarounds for a given firmware revision.
This seems to make sense. Therefore this patch converts all gadgets
(except a few) to use the kernel version instead a random 2 or 3 plus
the UDC number. The few that don't report kernel's version are:
- webcam
This one reports always a version 0x10 so allow it to do so in future.
- nokia
This one reports always 0x211. The comment says that this gadget works
only if the UDC supports altsettings so I added a check for this.
- serial
This one reports 0x2400 + UDC number. Since the gadget version is 2.4
this could make sense. Therefore bcdDevice is 0x2400 here.
I also remove various gadget_is_<name> macros which are unused. The
remaining few macros should be moved to feature / bug bitfield.

Some gadgets provide custom entry here. Some may override it with an
etntry that is also created by composite if there was no value sumbitted
at all.
This patch removes all "custom manufacturer" strings which are the same
as these which are created by composite. Then it moves the creation of
the default manufacturer string to usb_composite_overwrite_options() in
case no command line argument has been used and the entry is still an
empty string.
By doing this we get rid of the global variable "composite_manufacturer"
in composite.

The index in usb_string array is defined by the gadget. The gadget can
choose which index entry it assigns for the serial number and which the
product name. The gadget has just to ensure that the descriptor contains
the proper string id which is assigned by composite.
If the composite layer knows the index of the "default" information
which will be overwritten by module parameters, it can be used later to
overwrite it.

After I moved the function from the header file to the c file I see:

That is almost a KiB which is removed from each user.
As Felipe pointed out, the function / usage is very dumb actually. This is
used for the following reasons:
- epautoconf ep hint (could provide a per-gadget callback)
- miss-features. currently the missing altsetting on pxa's and something
ZLP related on musbhdrc (looks like an optimisation which could be
implemented in musb itself if it is correct)
- unique BCD accross all UDCs. Not sure how important this is.

This was broken in 2e87edf49 ("usb: gadget: make g_printer use
composite").
The USB-strings were not setup properly and were not used. No function
was added which results in an empty USB config.
While fixing this, the interface number is now auto generated and not
hard coded to 0.

This is a partly revert of 4fffd6e5 ("usb: gadget: composite: make
module parameters accessible at runtime").
It is not possible to change the VID or other property for a gadget
right now. This change has been made for Anrdoid gadget which has this
functionality in its copy of the file. This function is executed currently
only once and most caller in tree are __init.

This patch moves USB_BUFSIZ into global header file as
USB_COMP_EP0_BUFSIZ. There is currently only one user (f_sourcesink)
besides composite which need it. Ideally f_sourcesink would have its
own ep0 buffer. Lets keep it that way it was for now.

epautoconf has two global variables which count the endpoint number of
last assigned endpoint.
This patch removes the global variable and keeps it as per (UDC) gadget.
While here, the ifdef is removed and now the in and outpoint are
enumerated unconditionally. The dwc3 for instance supports 32 endpoints
in total.

This patch removes the global variable composite in composite.c.
The private data which was saved there is now passed via an additional
argument to the bind() function in struct usb_gadget_driver.
Only the "old-style" UDC drivers have to be touched here, new style are
doing it right because this change is made in udc-core.

As it turns out, Sam's comment was better than I initially assumed. This
patch pushes as struct usb_composite_driver data structures into
__refdata section to avoid a section missmatch report from modpost
because the ->bind() can be marked __init. The only downside is that
modpost does not check between ->bind() and other member. However, it is
temporary.

save bind callback in struct usb_composite_driver"). The reason was to
avoid the section missmatch. The warning was shown because ->bind is
marked as __init becuase it is a one time init. The warning can be also
suppresed by whitelisting the variable i.e. rename it to lets say _probe.

All the PHY configuration other than VBUS, ID GND and OTG SRP are removed
from twl6030. The phy configurations are taken care by the dedicated
usb2 phy driver. So twl6030 is made as comparator driver for VBUS and
ID detection.

All phy related programming like enabling/disabling the clocks, powering
on/off the phy is taken care of by this driver. It is also used for OTG
related functionality like srp.
This also includes device tree support for usb2 phy driver and
the documentation with device tree binding information is updated.
Currently writing to control module register is taken care in this
driver which will be removed once the control module driver is in place.

If xfernotready is received and there is no request in request_list then
REQUEST_PENDING flag must be set, so that next request in ep queue is executed.
In case of isoc transfer, if xfernotready is already elapsed and even first
request has not been queued to request_list, then issue END TRANSFER, so that
you can receive xfernotready again and can have notion of current microframe.

Earlier we used to check for ep->ep.desc to figure out if this ep has
already been enabled and if so, abort.
Ido Shayevitz removed the usb_endpoint_descriptor from private udc
structure 5a6506f00 ("usb: gadget: Update at91_udc to use
usb_endpoint_descriptor inside the struct usb_ep") but did not fix up
the ep_enable condition because _now_ the member is always true and we
can't check if this ep is enabled twice.

I overlooked that the driver is overwritting the private data which is
used by the composite framework. The udc driver doesn't read it, it is

Populate the resources for xhci afresh instead of directly using the
*struct resource* of core. *resource* structure has parent, sibling,
child pointers which should be filled only by resource API's. By
directly using the *resource* pointer of core in xhci, these parent,
sibling, child pointers are already populated even before
*platform_device_add* causing side effects.

dma_controller_create() in this MUSB DMA driver only regards 0 as a wrong IRQ
number, despite platform_get_irq_byname() that it calls returns -ENXIO in that
case. It leads to calling request_irq() with a negative IRQ number, and when it
naturally fails, the following is printed to the console:
request_irq -6 failed!
and the DMA controller is not created.
Fix this function to filter out the error values as well as 0.

when using musb_urb_enqueue to submit three urbs to the same endpoint, when
hep->hcpriv is NULL, qh will be allocated when the first urb is completed.
When the IRQ completes the next two urbs, qh->hep->hcpriv will be set to NULL.
Now the second urb get musb->lock and executes musb_schedule(), but
next_urb(qh) is NULL, so musb_start_urb will Oops.

If USB2 host controller probes fine but USB3 does not then we don't
remove the USB controller properly and lock up the system while the HUB
code will try to enumerate the USB2 controller and access memory which
is no longer available in case the dummy_hcd was compiled as a module.
This is a problem since 448b6eb1 ("USB: Make sure to fetch the BOS desc
for roothubs.) if used in USB3 mode because dummy does not provide this
descriptor and explodes later.

In case of ep0 out, if length is not aligned to maxpacket size then we use
dwc->ep_bounce_addr for dma transfer and not request->dma. Since, we have
alreday done memcpy from dwc->ep0_bounce to request->buf, so we do not need to
issue cache sync function. In fact, cache sync function will bring wrong data
in request->buf from request->dma in this scenario.
So, cache sync function must not be executed in case of ep0 bounced.

The "Low Performance USB Block driver" has been removed which a user of
libusual. Now we have only the usb-storage driver as the only driver in
tree. This makes libusual needless.
This patch removes libusal, fixes up all users. The usual-table is now
linked into usb-storage.
usb_usual.h remains in public include directory because some staging
users seem to need it.

If the number of ports present on the SoC/board is not the maximum
and that the platform data is not filled with all data, there is
an easy way to mess the PIO setup for this interface.
This quick fix addresses mis-configuration in USB host platform data
that is common in at91 boards since commit 0ee6d1e (USB: ohci-at91:
change maximum number of ports) that did not modified the associatd
board files.

When IORESOURCE_IRQ_SHAREABLE is set, the irq handler may be called
even if the interupt of the USB module doesn't happen. So, it may

Commit b69cc67205 added support for the E-861. After acquiring a C-867, I
realised that every Physik Instrumente's device has a different PID. They are
listed in the Windows device driver's .inf file. So here are all PIDs for the
current (and probably future) USB devices from Physik Instrumente.
Compiled, but only actually tested on the E-861 and C-867.

For non PCI-based stacks, this function call
usb_disable_xhci_ports(to_pci_dev(hcd->self.controller));
made from xhci_shutdown is not applicable.
Ideally, we wouldn't have any PCI-specific code on
a generic driver such as the xHCI stack, but it looks
like we should just stub usb_disable_xhci_ports() out
for non-PCI devices.

This patch is intended to work around a known issue on the
SN65LVPE502CP USB3.0 re-driver that can delay the negotiation
between a device and the host past the usual handshake timeout.
If that happens on the first insertion, the host controller
port will enter in Compliance Mode and NO port status event will
be generated (as per xHCI Spec) making impossible to detect this
event by software. The port will remain in compliance mode until
a warm reset is applied to it.
As a result of this, the port will seem "dead" to the user and no
device connections or disconnections will be detected.
For solving this, the patch creates a timer which polls every 2
seconds the link state of each host controller's port (this
by reading the PORTSC register) and recovers the port by issuing a
Warm reset every time Compliance mode is detected.
If a xHC USB3.0 port has previously entered to U0, the compliance
mode issue will NOT occur only until system resumes from
sleep/hibernate, therefore, the compliance mode timer is stopped
when all xHC USB 3.0 ports have entered U0. The timer is initialized
again after each system resume.
Since the issue is being caused by a piece of hardware, the timer
will be enabled ONLY on those systems that have the SN65LVPE502CP
installed (this patch uses DMI strings for detecting those systems)
therefore making this patch to act as a quirk (XHCI_COMP_MODE_QUIRK
has been added to the xhci stack).
This patch applies for these systems:
Vendor: Hewlett-Packard. System Models: Z420, Z620 and Z820.
This patch should be backported to kernels as old as 3.2, as that was
the first kernel to support warm reset. The kernels will need to
contain both commit 10d674a82e "USB: When
hot reset for USB3 fails, try warm reset" and commit
8bea2bd37d "usb: Add support for root hub
port status CAS". The first patch add warm reset support, and the
second patch modifies the USB core to issue a warm reset when the port
is in compliance mode.

My test platform (Intel DX79SI) boots reliably under BIOS, but frequently
crashes when booting via UEFI. I finally tracked this down to the xhci
handoff code. It seems that reads from the device occasionally just return
0xff, resulting in xhci_find_next_cap_offset generating a value that's
larger than the resource region. We then oops when attempting to read the
value. Sanity checking that value lets us avoid the crash.
I've no idea what's causing the underlying problem, and xhci still doesn't
actually *work* even with this, but the machine at least boots which will
probably make further debugging easier.
This should be backported to kernels as old as 2.6.31, that contain the
commit 66d4eadd8d "USB: xhci: BIOS handoff
and HW initialization."

Driver for the "USB20D" / "USBD" block on BCM6328, BCM6368, BCM6816,

The hardware block was designed to support networking applications

With a previous patch to enable the EHCI/XHCI port switching, it switches
all the available ports.
The assumption is not correct because the BIOS may expect some ports
not switchable by the OS.
There are two more registers that contains the information of the switchable
and non-switchable ports.
This patch adds the checking code for the two register so that only the
switchable ports are altered.
This patch should be backported to kernels as old as 3.0, that contain
commit ID 69e848c209 "Intel xhci: Support

On Intel Panther Point chipset USB 3.0 devices show up as
high-speed devices on powerup, but after an s3 cycle they are
correctly recognized as SuperSpeed. At powerup switch the port
to xHCI so that USB 3.0 devices are correctly recognized.

This commit removes USB_GADGET_DUALSPEED and USB_GADGET_SUPERSPEED
Kconfig options. Since now kernel allows many UDC drivers to be
compiled, those options may turn to no longer be valid. For
instance, if someone decides to build UDC that supports super
speed and UDC that supports high speed only, the latter will be
"assumed" to support super speed since USB_GADGET_SUPERSPEED will
be selected by the former.
The test of whether CONFIG_USB_GADGET_*SPEED was defined was just
an optimisation which removed otherwise dead code (ie. if UDC is
not dual speed, there is no need to handle cases that can happen
if speed is high). This commit removes those checks.

This patch (as1599) fixes dummy-hcd to make it update the appropriate
driver pointer when a new gadget driver is bound or unbound. Without
this change, the gadget driver's name doesn't appear in dev_printk
output.

The lpc32xx_udc driver supported only one controller by defining a global
static struct for it. This patch enables multiple instances of the controller
by dynamic allocation of the struct at probe(). A static struct is kept as a
template on initialization since it does some complex preset, reflecting fixed
hardware endpoint structure.

When connection is established non-ss mode, endpoint.maxburst is correctly set
to 0, this means that current code will set maxburst bitfield on DEPCFG's
parameter 0 to the highest possible value (0x0f) which is wrong.
Even though this hasn't caused any issues so far (HW seems to ignore that when
not running in SS mode) we want to make sure maxburst bitfield is only set to
anything other than zero if we're running on SuperSpeed mode. In order to
achieve that, let's check for gadget's operating speed before setting maxburst
bitfield when issuing DEPCFG command.

When IORESOURCE_IRQ_SHAREABLE is set, the irq handler may be called
even if the interupt of the USB module doesn't happen. So, it may
clear the interrupt flags by mistake. This patch fixes it.

usb_gadget_remove_driver() runs through a four-step sequence to shut down
the gadget driver. For the case of a composite gadget + at91 UDC, this
would look like:

usb_gadget_disconnect(udc->gadget); // at91_pullup(gadget, 0)
udc->driver->unbind(udc->gadget); // composite_unbind()
usb_gadget_udc_stop(udc->gadget, udc->driver); // at91_stop()
The UDC driver can receive SETUP packets from the host up until the
point when usb_gadget_disconnect() returns. On rare occasions, the
gadget driver may see this sequence:
udc->driver->disconnect(udc->gadget); // composite_disconnect()
udc->driver->setup(udc->gadget, &ctrl); // composite_setup()
udc->driver->unbind(udc->gadget); // composite_unbind()
Some gadget drivers, such as composite, assume this will never happen
and crash as a result.
The fix is to quiesce the UDC hardware (via usb_gadget_disconnect)
before running the gadget driver through the disconnect/unbind sequence.

The timeout values were 1000 and timeout issue occured many times on my
s3c6410 Soc based board (mostly when booting whith USB cable not
connected). This patch increase the values to 10000 to guarantee the
success of reset.
Having set timeout to 10000, I printed the remained timeout values
which could cause timeout issue before this change (tested several
times).
the first timeout value remained:
timeout = 8079

timeout = 8081
the second timeout value remained:
timeout = 7940
timeout = 7945
timeout = 7940
timeout = 7938
Seeing from above values, I think the value 10000 is big enough.

Fix boot up hang when enable udc without otg enabled.
The root cause is that the clock will be shut down when probe routine is
finished because of clock gating. When a gadget driver is registered at
this time, it will call mv_udc_start which in turn will call
mv_udc_vbus_session. If there is no cable attached at the boot up time,
the vbus is low, so it will call stop_activity path without clock
enabled which will cause system hang then.
Actually, we need't go this path when clock is disabled, what we need to
do is just jump out.

build_dtd() can be called when hold a spinlock, but GFP_KERNEL may cause
dma_pool_alloc() sleep, So we need use GFP_ATOMIC instead of GFP_KERNEL.
But using GFP_ATOMIC may cause failure when allocating memory, add error
handler to handle it.

flush[_delayed]_work_sync() are now spurious. Mark them deprecated
and convert all users to flush[_delayed]_work().
If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.
This patch doesn't make any functional difference.

If a device specifies zero endpoints in its interface descriptor,
the kernel oopses in acm_probe(). Even though that's clearly an
invalid descriptor, we should test wether we have all endpoints.
This is especially bad as this oops can be triggered by just
plugging a USB device in.

This structure needs to always stick around, even if CONFIG_HOTPLUG
is disabled, otherwise we can oops when trying to probe a device that
was added after the structure is thrown away.

Bjorn's latest patchset does break Gobi 1K and 2K because on both
devices as it claims usb interface 0. That's because usbif 0 is not
handled in the switch statement, and thus the if0 gets claimed when it
should not. So let's just make things even simpler yet, and handle both
the 1K and 2K+ cases separately. This patch should not affect the new
Sierra device support, because those devices are matched via
interface-specific matching and thus should never hit the composite
code.

If usb_create_hcd() fails here then we dereference "whc" inside the call
to whc_clean_up() before it has been set. The compiler would have
warned about this if we hadn't initialized all the pointers to NULL at
the start of the function. I've cleaned that up as well.

In case some gadget driver tries to enable an endpoint
which is already enabled, we print a nice WARN so we
can track broken gadget drivers. The only problem is that
we're printing the WARN when we already changed endpoint's
name, which would result in endpoints named as:
ep1in-bulk-bulk-bulk-bulk-bulk-bulk-bulk
To prevent that, we will continue to print the WARN,
but do so before changing endpoint's name and return
early.

When called during resume_irqs, omap2430_musb_set_vbus() is run with
interrupts disabled, In that case 'jiffies' never changes so the loop
can loop forever.
So impose a maximum loop count and add an 'mdelay' to ensure we wait
a reasonable amount of time for bit to be cleared.
This fixes a hang on resume.

Currently we have no way to assign tty->port while performing tty
installation. There are two ways to provide the link tty_struct =>
tty_port. Either by calling tty_port_install from tty->ops->install or
tty_port_register_device called instead of tty_register_device when
the device is being set up after connected.
In this patch we modify most of the drivers to do the latter. When the
drivers use tty_register_device and we have tty_port already, we
switch to tty_port_register_device. So we have the tty_struct =>
tty_port link for free for those.

This patch fixes an issue introduced by patch:
72c973d usb: gadget: add usb_endpoint_descriptor to struct usb_ep
Without this patch we see a kworker taking 100% CPU, after this sequence:
- Connect gadget to a windows host
- load g_ether
- ifconfig up <ip>; ifconfig down; ifconfig up
- ping <windows host>
The "ifconfig down" results in calling eth_stop(), which will call
usb_ep_disable() and, if the carrier is still ok, usb_ep_enable():

The ep should stay enabled, but will not, as ep_disable set the desc
pointer to NULL, therefore the subsequent ep_enable will fail. This leads
to permanent rescheduling of the eth_work() worker as usb_ep_queue()
(called by the worker) will fail due to the unconfigured endpoint.
We fix this issue by saving the ep descriptors and re-assign them before
usb_ep_enable().

Existing implementation of tegra_ehci_remove() calls
usb_put_hcd(hcd) first and then iounmap(hcd->regs).
usb_put_hcd() implementation calls hcd_release()
which frees up memory allocated for hcd.
As iounmap is trying to unmap hcd->regs, after hcd
getting freed up, warning messages were observed during
unload of USB.
Hence fixing it.

We cannot unconditionally access any usb-serial port specific
data from the interface driver. Both supending and resuming
may happen after the port has been removed and portdata is
freed.
Treat ports with no portdata as closed ports to avoid a NULL
pointer dereference on resume. No need to kill URBs for
removed ports on suspend, avoiding the same NULL pointer
reference there.

Some usb-serial drivers may access port data in their suspend/
resume functions. Such drivers must always verify the validity
of the data as both suspend and resume can be called both before
usb_serial_device_probe and after usb_serial_device_remove.
But the port data may be invalidated during port_probe and
port_remove. This patch prevents the race against suspend and
resume by disabling suspend while port_probe or port_remove is
running.

The usb message must be saved also in case the USB endpoint is not a
control endpoint (i.e., "endpoint 0"), otherwise in some circumstances
we don't have a payload in case of error.
The patch has been created by tracing with usbmon the different error
messages generated by this driver with respect to the ehci-hcd driver.

The Intel desktop boards DH77EB and DH77DF have a hardware issue that
can be worked around by BIOS. If the USB ports are switched to xHCI on
shutdown, the xHCI host will send a spurious interrupt, which will wake
the system. Some BIOS will work around this, but not all.
The bug can be avoided if the USB ports are switched back to EHCI on
shutdown. The Intel Windows driver switches the ports back to EHCI, so
change the Linux xHCI driver to do the same.
Unfortunately, we can't tell the two effected boards apart from other
working motherboards, because the vendors will change the DMI strings
for the DH77EB and DH77DF boards to their own custom names. One example
is Compulab's mini-desktop, the Intense-PC. Instead, key off the
Panther Point xHCI host PCI vendor and device ID, and switch the ports
over for all PPT xHCI hosts.
The only impact this will have on non-effected boards is to add a couple
hundred milliseconds delay on boot when the BIOS has to switch the ports
over from EHCI to xHCI.
This patch should be backported to kernels as old as 3.0, that contain
the commit 69e848c209 "Intel xhci: Support
EHCI/xHCI port switching."

dwc3_stop_active_transfer has been called from two places. After each
calling of this function , we should allow 100 us delay to synchronize
with interconnect.
It would be better if we put this delay in that function only, rather
than into calling routine of this function.

This patch fixes a particularly nasty bug that was revealed by the ring
expansion patches. The bug has been present since the very beginning of
the xHCI driver history, and could have caused general protection faults
from bad memory accesses.
The first thing to note is that a Set TR Dequeue Pointer command can
move the dequeue pointer to a link TRB, if the canceled or stalled
transfer TD ended just before a link TRB. The function to increment the
dequeue pointer, inc_deq, was written before cancellation and stall
support was added. It assumed that the dequeue pointer could never
point to a link TRB. It would unconditionally increment the dequeue
pointer at the start of the function, check if the pointer was now on a
link TRB, and move it to the top of the next segment if so.
This means that if a Set TR Dequeue Point command moved the dequeue
pointer to a link TRB, a subsequent call to inc_deq() would move the
pointer off the segment and into la-la-land. It would then read from
that memory to determine if it was a link TRB. Other functions would
often call inc_deq() until the dequeue pointer matched some other
pointer, which means this function would quite happily read all of
system memory before wrapping around to the right pointer value.
Often, there would be another endpoint segment from a different ring
allocated from the same DMA pool, which would be contiguous to the
segment inc_deq just stepped off of. inc_deq would eventually find the
link TRB in that segment, and blindly move the dequeue pointer back to
the top of the correct ring segment.
The only reason the original code worked at all is because there was
only one ring segment. With the ring expansion patches, the dequeue
pointer would eventually wrap into place, but the dequeue segment would
be out-of-sync. On the second TD after the dequeue pointer was moved to
a link TRB, trb_in_td() would fail (because the dequeue pointer and
dequeue segment were out-of-sync), and this message would appear:

When we encounter an xHCI host that needs the XHCI_TRUST_TX_LENGTH
quirk, the xHCI driver ends up spewing messages about the quirk into
dmesg every time a short packet occurs. Change the xHCI driver to
rate-limit such warnings.

Gary reports that with recent kernels, he notices more xHCI driver
warnings:
xhci_hcd 0000:03:00.0: WARN Successful completion on short TX: needs XHCI_TRUST_TX_LENGTH quirk?
We think his Etron xHCI host controller may have the same buggy behavior
as the Fresco Logic xHCI host. When a short transfer is received, the
host will mark the transfer as successfully completed when it should be
marking it with a short completion.
Fix this by turning on the XHCI_TRUST_TX_LENGTH quirk when the Etron
host is discovered. Note that Gary has revision 1, but if Etron fixes
this bug in future revisions, the quirk will have no effect.
This patch should be backported to kernels as old as 2.6.36, that
contain a backported version of commit

for Fresco Logic host."

The NEC/Renesas 720201 xHCI host controller does not complete its reset
within 250 milliseconds. In fact, it takes about 9 seconds to reset the
host controller, and 1 second for the host to be ready for doorbell
rings. Extend the reset and CNR polling timeout to 10 seconds each.
This patch should be backported to kernels as old as 2.6.31, that
contain the commit 66d4eadd8d "USB: xhci:
BIOS handoff and HW initialization."

Do not rely on any hints from gadget drivers and use DMA mode 1
whenever we expect data of at least the endpoint's packet size and
have not yet received a short packet.
The last packet if short is always transferred using DMA mode 0.
This patch fixes USB throughput issues in mass storage mode for

In case of USB bulk transfer, when himem page
is received, the usb_sg_init function sets the
urb transfer buffer to NULL. When such URB
transfer is handled, kernel crashes in PIO mode.
Handle this by mapping the highmem buffer in PIO mode.

Fixes endpoint starvation issue when more than one bulk QH is
multiplexed on the reserved bulk TX endpoint.
This patch sets the NAK timeout interval for such QHs, and when
a timeout triggers the next QH will be scheduled.
This scheme doesn't work for devices which are connected to a
high to full speed tree (transaction translator) as there is
no NAK timeout interrupt from the musb controller from such
devices.

In rxstate(), improper types are given to 'fifo_count' and 'len' variables, and
these variables are not used in accordance to their names (up to the certain
point), i.e. 'len' to hold the size of a packet in the RX FIFO, and 'fifo_count'
to hold a difference between 'request->length' and 'request->actual'...
Interchange the variables up to the point where their use starts to make sense
again.

We are overwriting the resource->name to "mc" so that musb_core.c
can understand it but this is also changing the platform device's
resource->name as the "name" address remains same.
Fixing the same by changing the resource->name field of local
structure only.

commit 68d3e66 (usb: dwc3: ep0: fix for possible early
delayed_status) added handling for early delayed status,
but the current code only works because so far delayed
status will always be on the IN direction.
This patch makes the code more robust by making sure that
we can handle all directions properly.

Due to the late Silicon limitation found, we are
now pre-starting DATA phase's TRBs. If, still, we
get XferNotReady(DATA) we will ignore it unless
we're getting it for the wrong direction.
In that case we must keep the error case handling
plus add a ENDTRANSFER command to forcefully end
the Data TRB we started previously, then continue
to SetStall and so on.

We uncovered a limitation of this core WRT to the
Link Layer Compliance Suite's TD7.06.
On that test, host will start a GetDescriptor(DEVICE)
standard request, but it will do so only on the
SETUP phase, meaning there will *NOT* be any DATA or
STATUS phases.
The idea of the test is to verify robustness of the
IP WRT framing errors, so the test will send a
sequence of different SETUP_DPs each with a different
framing error and the Suite expects us to be able to
receive all SETUP_DPs with no timeouts.
This core, has the ability to tell us which phase the
host is expecting before we start it. Whenever we
receive a TP or DP when no transfers are cached on
the internal IP's caches, the IP will generate a
XferNotReady event with status informing us (in case
of physical ep0/ep1) if it's related to DATA or STATUS
phases - SETUP phase is expected to be prestarted.
Because we're always waiting for XferNotReady
events for DATA and STATUS phases, we will never
be able to know that the Host wants to start another
SETUP phase instead, which will render us "not
compliant" with TD7.06.
In order to "fix" the problem we must not rely
on XferNotReady events for the DATA phase and try
to always pre-start DATA transfers on physical
endpoints 0 and 1. If host goes back to SETUP phase
from DATA phase we will receive a XferComplete for
that phase with TRB's status set to SETUP_PENDING,
which is only useful for printing a debugging log as
the core expects us to still go through to the STATUS
phase, initiate a CONTROL_STATUS TRB just so it
completes right away and, only then, we go back to
the pending SETUP phase.
SNPS has decided to modify the programming model of
the core so that on-demand DATA phases will not be
supported anymore. Note that this limitation does not
affect 2-stage transfers, meaning that if TD7.06 would
start a 2-stage transfer instead of a 3-stage transfer,
we would receive a "fake" XferNotReady(STATUS) which
would complete right after being initiated with
SETUP_PENDING status.
Other endpoints are also not affected, so we can still
use on-demand transfers on Bulk/Isoc/Interrupt endpoints.

Databook doesn't say we should stall if we
get XferNotReady(STATUS) while we're expecting
something else.
Instead of stalling and restarting, tests have
proven that ignoring the event is far more
effective.
This problem has been caught while rewriting
ep0 handling in order we pass Link Layer TD7.6.

The skb->pfmemalloc flag gets set to true iff during the slab allocation
of data in __alloc_skb that the the PFMEMALLOC reserves were used. If
page splitting is used, it is possible that pages will be allocated from
the PFMEMALLOC reserve without propagating this information to the skb.
This patch propagates page->pfmemalloc from pages allocated for fragments
to the skb.
It works by reintroducing and expanding the skb_alloc_page() API to take
an skb. If the page was allocated from pfmemalloc reserves, it is
automatically copied. If the driver allocates the page before the skb, it
should call skb_propagate_pfmemalloc() after the skb is allocated to
ensure the flag is copied properly.
Failure to do so is not critical. The resulting driver may perform slower
if it is used for swap-over-NBD or swap-over-NFS but it should not result
in failure.

The default 10 microsecond delay for the controller to come out of
halt in dbgp_ehci_startup is too short, so increase it to 1 millisecond.
This is based on emperical testing on various USB debug ports on
modern machines such as a Lenovo X220i and an Ivybridge development
platform that needed to wait ~450-950 microseconds.

Fix up mostly trivial conflicts, with the exception of
drivers/usb/host/ehci-omap.c in particular, which had some
re-organization of the reset sequence (commit 1a49e2ac9651: "EHCI:
centralize controller initialization") that clashed with commit
2761a63945 ("mfd: USB: Fix the omap-usb EHCI ULPI PHY reset fix
issues").
In particular, commit 2761a63945 moved the usb_add_hcd() to the
*middle* of the reset sequence, which clashes fairly badly with the
reset sequence re-organization (although it could have been done inside
the new omap_ehci_init() function).
I left that part of commit 2761a63945 just undone.

The clk patches added code to get and enable clocks in the
respective driver probe functions. If the probe function failed
for some reason after enabling the clock, the clock was not
disabled again in many cases.

The most important feature of this patch set is the new async infrastructure
that makes sure async_synchronize_full() synchronizes all domains and allows
us to remove all the hacks (like having scsi_complete_async_scans() in the
device base code) and means that the async infrastructure will "just work" in
future. The rest is assorted driver updates (aacraid, bnx2fc, virto-scsi,
megaraid, bfa, lpfc, qla2xxx, qla4xxx) plus a lot of infrastructure work in
sas and FC.

1) Remove the ipv4 routing cache. Now lookups go directly into the FIB
trie and use prebuilt routes cached there.
No more garbage collection, no more rDOS attacks on the routing
cache. Instead we now get predictable and consistent performance,
no matter what the pattern of traffic we service.
This has been almost 2 years in the making. Special thanks to
Julian Anastasov, Eric Dumazet, Steffen Klassert, and others who
have helped along the way.
I'm sure that with a change of this magnitude there will be some
kind of fallout, but such things ought the be simple to fix at this

Most Logitech UVC webcams (both early models that don't advertise UVC
compatibility and newer UVC-advertised devices) require the RESET_RESUME
quirk. Instead of listing each and every model, match the devices based
on the UVC interface information.

When a whole class of devices (possibly from a specific vendor, or
across multiple vendors) require a quirk, explictly listing all devices
in the class make the quirks table unnecessarily large. Fix this by
allowing matching devices based on interface information.

We want it to be possible for target_submit_cmd() to return errors up
to its fabric module callers. For now just update the prototype to
return an int, and update all callers to handle non-zero return values
as an error.
This is immediately useful for tcm_qla2xxx to fix a long-standing active
I/O session shutdown race, but tcm_fc, usb-gadget, and sbp-target the
fabric maintainers need to check + ACK that handling a target_submit_cmd()
failure due to session shutdown does not introduce regressions
(nab: Respin against for-next after initial NACK + update docbook comment +
fix double se_cmd init in exception path for usb-gadget)

Now that we don't have tty->termios tied to drivers->tty we can untangle
the logic here. In addition we can push the removal logic out of the
destructor path.
At that point we can think about sorting out tty_port and console and all
the other ugly hangovers.

The MC7710 in QMI mode does not support the vendor
specific USB requests used by this driver. The most
noticable effect of this is a 5 second delay as each
serial port is probed, caused by the set_power_state

The QMI mode of the Sierra Wireless MC7710 is close to
a Gobi device, and also identified as one by the
Windows drivers provided by Sony. The serial interfaces
are the same as for any other Gobi module, but the USB
interface layout is different:
0: DM/DIAG (also present in bootloader mode)
2: NMEA
3: AT-capable modem port
8: QMI/net
19: QMI/net (not always present)
20: QMI/net (not always present)
Note in particular that the NMEA and AT ports are reversed
compared to a Gobi 2k+ device, and that the DM port appears
as a QDL port in bootloader mode using the same device ID.
The Sony driver also document two new devices with standard
Gobi 2k+ layout (1199:68a5, 1199:68a9) having a QDL mode
(1199:68a4, 1199:68a8). Adding these as well.
Lenovo Windows drivers document the USB interface layout
for a few additional Sierra Wireless devices. Adding these
while at it:
- MC7770 (1199:901b) with standard Gobi 2k+ layout
- MC7700 (0f3d:68a2) with the same layout as MC7710
- MC7750 (114f:68a2) with the same layout as MC7710
- EM7700 (1199:901c) with the same layout as MC7710

No need to verify the interface layout when doing
interface number based matching. We can safely trust
the device ID table in this case.
This allows the driver to support any USB interface
layout for non-Gobi 1k/2k+ devices.

This patch (as1589) resolves some unlikely races involving system
shutdown or controller death in ehci-hcd:
Shutdown races with both root-hub resume and controller
resume.
Controller death races with root-hub suspend.
A new bitflag is added to indicate that the controller has been shut
down (whether for system shutdown or because it died). Tests are
added in the suspend and resume pathways to avoid reactivating the
controller after any sort of shutdown.

This patch (as1588) adjusts the locking in ehci-hcd's various halt,
shutdown, and suspend/resume pathways. We want to hold the spinlock
while writing device registers and accessing shared variables, but not
while polling in a loop.
In addition, there's no need to call ehci_work() at times when no URBs

This patch (as1587) simplifies ehci-hcd's scan_isoc() routine by
eliminating some local variables, declaring boolean-valued values as
bool rather than unsigned, changing variable names to make more sense,
and so on.
The logic at the end of the routine is cut down significantly. The
scanning doesn't have to catch up all the way to where the hardware
is; it merely has to catch up to where the hardware was when the last
interrupt occurred. If the hardware has made more progress since then
and issued another interrupt, a rescan will catch up to it.

This patch (as1586) replaces the kernel timer used by ehci-hcd as an
I/O watchdog with an hrtimer event.
Unlike in the current code, the watchdog event is now always enabled
whenever any isochronous URBs are active. This will prevent bugs
caused by the periodic schedule wrapping around with no completion
interrupts; the watchdog handler is guaranteed to scan the isochronous
transfers at least once during each iteration of the schedule. The
extra overhead will be negligible: one timer interrupt every 100 ms.

This patch (as1585) fixes a bug in ehci-hcd's scheme for scanning
interrupt QHs.
Currently a single routine takes care of scanning everything on the
periodic schedule. Whenever an interrupt occurs, it scans all
isochronous and interrupt URBs scheduled for frames that have elapsed
since the last scan.
This has two disadvantages. The first is relatively minor: An
interrupt QH is likely to end up getting scanned multiple times,
particularly if the last scan was not fairly recent. (The current
code avoids this by maintaining a periodic_stamp in each interrupt
QH.)
The second is more serious. The periodic schedule wraps around. If
the last scan occurred during frame N, and the next scan occurs when
the schedule has gone through an entire cycle and is back at frame N,
the scanning code won't look at any frames other than N. Consequently
it won't see any QHs that completed during frame N-1 or earlier.
The patch replaces the entire frame-based approach for scanning
interrupt QHs with a new routine using a list-based approach, the same
as for async QHs. This has a slight disadvantage, because it means
that all interrupt QHs have to be scanned every time. But it is more
robust than the current approach.

This patch (as1584) fixes a minor bug that has been present in
ehci-hcd since the beginning.
Scanning the schedules for URB completions is single-threaded. If a
completion interrupt occurs while an URB is being given back, the
interrupt handler realizes that a scan is in progress on another CPU
and avoids starting a new one.
This means that completion events can be lost. If an URB completes
after it has been scanned but while a scan is still in progress, the
driver won't notice and won't rescan the completed URB.
The patch fixes the problem by adding a new flag to indicate that
another scan is needed after the current scan is done. The flag gets
set whenever a completion interrupt occurs while a scan is in
progress. The rescan will see the completion, thus preventing it from
getting lost.

This patch (as1582) changes ehci-hcd's strategy for unlinking async
QHs. Currently the driver never unlinks more than one QH at a time.
This can be inefficient and cause unnecessary delays, since a QH
cannot be reused while it is waiting to be unlinked.
The new strategy unlinks all the waiting QHs at once. In practice the
improvement won't be very big, because it's somewhat uncommon to have
two or more QHs waiting to be unlinked at any time. But it does
happen, and in any case, doing things this way makes more sense IMO.
The change requires the async unlinking code to be refactored
slightly. Now in addition to the routines for starting and ending an
unlink, there are new routines for unlinking a single QH and starting
an IAA cycle. This approach is needed because there are two separate
paths for unlinking async QHs:
When a transfer error occurs or an URB is cancelled, the QH
must be unlinked right away;
When a QH has been idle sufficiently long, it is unlinked
to avoid consuming DMA bandwidth uselessly.
In the first case we want the unlink to proceed as quickly as
possible, whereas in the second case we can afford to batch several
QHs together and unlink them all at once. Hence the division of
labor.

This patch (as1580) makes ehci_iso_stream structures behave more like
QHs, in that they will remain allocated until their isochronous
endpoint is disabled. This will come in useful in the future, when
periodic bandwidth gets allocated as an altsetting is installed rather
than on-the-fly.
For now, the change to the ehci_iso_stream lifetimes means that each
structure is always deallocated at exactly one spot in
ehci_endpoint_disable() and never used again. As a result, it is no
longer necessary to use reference counting on these things, and the
patch removes it.

This patch (as1579) adds an hrtimer event to handle deallocation of
iTDs and siTDs in ehci-hcd.
Because of the frame-oriented approach used by the EHCI periodic
schedule, the hardware can continue to access the Transfer Descriptor
for isochronous (or split-isochronous) transactions for up to a
millisecond after the transaction completes. The iTD (or siTD) must
not be reused before then.
The strategy currently used involves putting completed iTDs on a list
of cached entries and every so often returning them to the endpoint's
free list. The new strategy reduces overhead by putting completed
iTDs back on the free list immediately, although they are not reused
until it is safe to do so.
When the isochronous endpoint stops (its queue becomes empty), the
iTDs on its free list get moved to a global list, from which they will
be deallocated after a minimum of 2 ms. This delay is what the new
hrtimer event is for.
Overall this may not be a tremendous improvement over the current
code, but to me it seems a lot more clear and logical. In addition,
it removes the need for each iTD to keep a reference to the
ehci_iso_stream it belongs to, since the iTD never needs to be moved
back to the stream's free list from the global list.

EHCI controller. When a controller dies, it doesn't necessarily stop
running right away. The new event polls at 1-ms intervals to see when
all activity has safely stopped. This replaces a busy-wait polling
loop in the current code.

This patch (as1577) adds hrtimer support for unlinking interrupt QHs
in ehci-hcd. The current code relies on a fixed delay of either 2 or
55 us, which is not always adequate and in any case is totally bogus.
Thanks to internal caching, the EHCI hardware may continue to access
an interrupt QH for more than a millisecond after it has been unlinked.
In fact, the EHCI spec doesn't say how long to wait before using an
unlinked interrupt QH. The patch sets the delay to 9 microframes
minimum, which ought to be adequate.

This patch (as1576) adds hrtimer support for managing ehci-hcd's
async schedule. Just as with the earlier change to the periodic
schedule management, two new hrtimer events take care of everything.
One event polls at 1-ms intervals to see when the Asynchronous
Schedule Status (ASS) flag matches the Asynchronous Schedule Enable
(ASE) value; the schedule's state must not be changed until it does.
The other event delays for 15 ms after the async schedule becomes
empty before turning it off.
The new events replace a busy-wait poll and a kernel timer usage.
They also replace the rather illogical method currently used for
indicating the async schedule should be turned off: attempting to
unlink the dedicated QH at the head of the async list.

This patch (as1575) removes special code added for status polling of
the EHCI controller in PS3 systems. While the controller is running,
the polling is now carried out by an hrtimer handler. When the
controller is suspending or stopping, we use the same polling routine
as the old code -- but in neither case do we need to conclude that the
controller has died if the polling goes on for too long.
As a result the entire handshake_on_error_set_halt() routine is now
unused, so it is removed from the driver.

This patch (as1573) adds hrtimer support for managing ehci-hcd's
periodic schedule. There are two issues to deal with.
First, the schedule's state (on or off) must not be changed until the
hardware status has caught up with the current command. This is
handled by an hrtimer event that polls at 1-ms intervals to see when
the Periodic Schedule Status (PSS) flag matches the Periodic Schedule
Enable (PSE) value.
Second, the schedule should not be turned off as soon as it becomes
empty. Turning the schedule on and off takes time, so we want to wait
until the schedule has been empty for a suitable period before turning
it off. This is handled by an hrtimer event that gets set to expire
10 ms after the periodic schedule becomes empty.
The existing code polls (for up to 1125 us and with interrupts
disabled!) to check the status, and doesn't implement a delay before
turning off the schedule. Furthermore, if the polling fails then the
driver decides that the controller has died. This has caused problems
for several people; some controllers can take 10 ms or more to turn
off their periodic schedules.
This patch fixes these issues. It also makes the "broken_periodic"
workaround unnecessary; there is no longer any danger of turning off
the periodic schedule after it has been on for less than 1 ms.

This patch (as1572) begins the conversion of ehci-hcd over to using
high-resolution timers rather than old-fashioned low-resolution kernel
timers. This reduces overhead caused by timer roundoff on systems
where HZ is smaller than 1000. Also, the new timer framework
introduced here is much more logical and easily extended than the
ad-hoc approach ehci-hcd currently uses for timers.
An hrtimer structure is added to ehci_hcd, along with a bitflag array
and an array of ktime_t values, to keep track of which timing events
are pending and what their expiration times are.
Only the infrastructure for the timing operations is added in this
patch. Later patches will add routines for handling each of the
various timing events the driver needs. In some cases the new hrtimer
handlers will replace the existing handlers for ehci-hcd's kernel
timers; as this happens the old timers will be removed. In other
cases the new timing events will replace busy-wait loops.

This patch (as1571) adds a new state for ehci-hcd's root hubs:
EHCI_RH_STOPPING. This value is used at times when the root hub is
being stopped and we don't know whether or not the hardware has
finished all its DMA yet.
Although the purpose may not be apparent, this distinction will come
in useful later on. Future patches will avoid actions that depend on
the root hub being operational (like turning on the async or periodic
schedules) when they see the state is EHCI_RH_STOPPING.

This patch (as1570) adds a pointer for the end of ehci-hcd's
async-unlink list. The list (which is actually a queue) is singly
linked, so having a pointer to its end makes adding new entries easier
-- there's no longer any need to scan through the whole list.
In principle it could be changed to a standard doubly-linked list. It
turns out that doing so actually makes the code less clear, so I'm
leaving it as is.

This patch (as1569) renames the ehci->reclaim list in ehci-hcd. The
word "reclaim" is used in the EHCI specification to mean something
quite different, and "unlink_next" is more descriptive of the list's
purpose anyway.
Similarly, the "reclaim" field in the ehci_stats structure is renamed
"iaa", which is more meaningful (to experts, anyway) and is a better
match for the "lost_iaa" field.

Currently, EHCI initialization turns off the controller (in case it
was left running by the firmware) before setting up the ehci_hcd data
structure. This patch (as1565) reverses that order.
Although it doesn't matter now, it will matter later on when future
additions to ehci_halt() will want to acquire a spinlock that gets
initialized by ehci_init().

USB 3.0 devices can optionally support Latency Tolerance Messaging
(LTM). Add a new sysfs file in the device directory to show whether a
device is LTM capable. This file will be present for both USB 2.0 and
USB 3.0 devices.

USB 3.0 devices may optionally support a new feature called Latency
Tolerance Messaging. If both the xHCI host controller and the device
support LTM, it should be turned on in order to give the system hardware
a better clue about the latency tolerance values of its PCI devices.
Once a Set Feature request to enable LTM is received, the USB 3.0 device
will begin to send LTM updates as its buffers fill or empty, and it can
tolerate more or less latency.
The USB 3.0 spec, section C.4.2 says that LTM should be disabled just
before the device is placed into suspend. Then the device will send an
updated LTM notification, so that the system doesn't think it should
remain in an active state in order to satisfy the latency requirements
of the suspended device.
The Set and Clear Feature LTM enable command can only be sent to a
configured device. The device will respond with an error if that
command is sent while it is in the Default or Addressed state. Make
sure to check udev->actconfig in usb_enable_ltm() and usb_disable_ltm(),
and don't send those commands when the device is unconfigured.
LTM should be enabled once a new configuration is installed in
usb_set_configuration(). If we end up sending duplicate Set Feature LTM
Enable commands on a switch from one installed configuration to another
configuration, that should be harmless.
Make sure that LTM is disabled before the device is unconfigured in
usb_disable_device(). If no drivers are bound to the device, it doesn't
make sense to allow the device to control the latency tolerance of the
xHCI host controller.

Some xHCI host controllers may have optional support for Latency
Tolerance Messaging (LTM). This allows USB 3.0 devices that support LTM
to pass information about how much latency they can tolerate to the xHC.
A PCI xHCI host will use this information to update the PCI Latency
Tolerance Request (LTR) info. The goal of this is to gather latency
information for the system, to enable hardware-driven C states, and the
shutting down of PLLs.

When a user runs `echo 0 > bConfigurationValue` for a USB 3.0 device,
usb_disable_device() is called. This function disables all drivers,
deallocates interfaces, and sets the device configuration value to 0
(unconfigured).
With the new scheme to ensure that unconfigured devices have LPM
disabled, usb_disable_device() must call usb_unlocked_disable_lpm() once
it unconfigures the device.
This commit should be backported to kernels as old as 3.5, that contain
the commit 8306095fd2 "USB: Disable USB
3.0 LPM in critical sections."
