The SCLP interface to query, configure and deconfigure CPUs actually
operates on cores. For a machine without the multi-threading faciltiy
a CPU and a core are equivalent but starting with System z13 a core
can have multiple hardware threads, also referred to as logical CPUs.
To avoid confusion replace the word 'cpu' with 'core' in the SCLP
interface. Also replace MAX_CPU_ADDRESS with SCLP_MAX_CORES.
The core-id is an 8-bit field, the maximum thread id is in the range
0-31. The theoretical limit for the CPU address is therefore 8191.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

The store-cpu-id instruction has a minimum alignment of 8. Reflect
that in the definition of struct cpuid.

Merge cpuid.h header file into cpu.h.
While at it convert from typedef to struct declaration and also
convert cio code to use proper lowcore structure instead of casts.

All definition in cpu.h have to do with cputime accounting. Move
them to cputime.h and remove the header file.

Distinguish the cputime of the idle process where idle is actually using
cpu cycles from the cputime where idle is sleeping on an enabled wait psw.
The former is accounted as system time, the later as idle time.
