Many drivers (21 to be exact) create connectors that are always
connected (for instance to an LVDS or DSI panel). Instead of forcing
them to implement a dummy .detect() handler, make the callback optional
and consider the connector as always connected in that case.

We get 1 warning when building kernel with W=1:

Only really needed for fbdev emulation at 8bpp. And bochs doesn't do
that. And either way bochs only does 32bit rgb, so this is all pretty
much wasted dead code.
The only consideration is that we need to not set up any gamma size
either.

Previously a negative pipe caused a special case to be triggered for
drivers that didn't have proper VBLANK support. The trigger for this
special case is now independent of the pipe, so the correct CRTC index
can now be stored in events.

Implement crtc page_flip callback for bochsdrm. The qemu stdvga has no
vblank signaling, so we have to fake it. We do so by instantly calling
drm_send_vblank_event. Tested with kmscon.

Just a bit of OCD cleanup on headers - this function isn't the core
interface any more but just a helper for drivers who haven't yet
transitioned to universal planes. Put the declaration at the right
spot and sprinkle necessary #includes over all drivers.
Maybe this helps to encourage driver maintainers to do the switch.

Now that CRTC's have a primary plane, there's no need to track the
framebuffer in the CRTC. Replace all references to the CRTC fb with the
primary plane's fb.

DRM driver for (virtual) vga cards using the bochs dispi
interface, such as the qemu standard vga (qemu -vga std).
Don't bother supporting anything but 32bpp for now, even
though the virtual hardware is able to do that.
Known issue: mmap(/dev/fb0) doesn't work.
