Scott Teel e863d68e48 [SCSI] hpsa: rescan devices on ioaccel2 error
Allow driver to schedule a rescan whenever a request fails on the ioaccel2 path.
This eliminates the possibility of driver getting stuck in non-ioaccel mode.

IOaccel mode (HP SSD Smart Path) is disabled by driver upon error detection.
Driver relied on idea that request would be retried through normal path, and a
subsequent error would occur on that path, and be processed by controller
firmware.  As part of that process, controller disables ioaccel mode and later
reinstates it, signalling driver to change modes.

In some error cases, the error will not duplicate on the standard path,
so the driver could get stuck in non-ioaccel mode.
To avoid that, we allow driver to request a rescan during the next run of the
rescan thread.

Allow SSD Smart Path for a controller to be disabled by
the user, regardless of settings in controller firmware
or array configuration.

To disable:     echo 0 > /sys/class/scsi_host/host<id>/acciopath_status
To re-enable:   echo 1 > /sys/class/scsi_host/host<id>/acciopath_status
To check state: cat /sys/class/scsi_host/host<id>/acciopath_status

Stephen M. Cameron 76438d087f [SCSI] hpsa: poll controller to detect device change event
For shared SAS configurations, hosts need to poll Smart Arrays
periodically in order to be able to detect configuration changes
such as logical drives being added or removed from remote hosts.
A register on the controller indicates when such events have
occurred, and the driver polls the register via a workqueue
and kicks off a rescan of devices if such an event is detected.
Additionally, changes to logical drive raid offload eligibility
are autodetected in this way.

Stephen M. Cameron 283b4a9b98 [SCSI] hpsa: add ioaccell mode 1 RAID offload support.
This enables sending i/o's destined for RAID logical drives
which can be serviced by a single physical disk down a different,
faster i/o path directly to physical drives for certain logical
volumes on SSDs bypassing the Smart Array RAID stack for a
performance improvement.

Stephen M. Cameron 2f6ae5cd24 [SCSI] hpsa: only allow REQ_TYPE_FS to use fast path
When commands sent down the "fast path" fail, they must be re-tried down the
normal RAID path.  We do this by kicking i/o's back to the scsi mid layer with
a DID_SOFT_ERROR status, which causes them to be retried.  This won't work for
SG_IO's and other non REQ_TYPE_FS i/o's which could get kicked all the way back
to the application, which may have no idea that the command needs resubmitting
and likely no way to resubmit it in such a way the that driver can recognize it
as a resubmit and send it down the normal RAID path.  So we just always send
non REQ_TYPE_FS i/o's down the normal RAID path, never down the "fast path".

directly and it will be faster.

Matt Gates a93aa1fe00 [SCSI] hpsa: use extended report luns command for HP SSD SmartPath
There is an extended report luns command which contains
additional information about physical devices.  In particular
we need to get the physical device handle so we can use an
alternate i/o path for fast physical devices like SSDs so
we can speed up certain i/o's by bypassing the RAID stack
code in the controller firmware.

Suresh Thiagarajan 2b01d816f5 [SCSI] pm80xx: Spinlock fix
spin_lock_irqsave for the HBA lock is called in one function where flag
is local to that function. Another function is called from the first
function where lock has to be released using spin_unlock_irqrestore for
calling task_done of libsas. In the second function also flag is declared
and used. For calling task_done there is no need to enable the irq. So
instead of using spin_lock_irqsave and spin_unlock_irqrestore, spin_lock
and spin_unlock is used now. This also avoids passing the flags across all
the functions where HBA lock is being used. Also removed redundant code.

Hannes Reinecke eee0f03a54 [SCSI] hpsa: fixup MSI-X registration
Commit 254f796b9f updated
the driver to use 16 MSI-X vectors, despite the fact that
older controllers would provide only 4.
This was causing MSI-X registration to drop down to INTx
mode. But as the controller support performant mode, the
initialisation will become confused and cause the machine
to stall during boot.

This patch fixes up the MSI-X registration to re-issue
the pci_enable_msix() call with the correct number of
MSI-X vectors. With that the hpsa driver continues to
works on older controllers like the P200.

the eh_deadline mechanism is pointless and will set the

James Bottomley f2495e228f [SCSI] dual scan thread bug fix
In the highly unusual case where two threads are running concurrently through
the scanning code scanning the same target, we run into the situation where
one may allocate the target while the other is still using it.  In this case,
because the reap checks for STARGET_CREATED and kills the target without
reference counting, the second thread will do the wrong thing on reap.

Fix this by reference counting even creates and doing the STARGET_CREATED
check in the final put.

James Bottomley e63ed0d7a9 [SCSI] fix our current target reap infrastructure
This patch eliminates the reap_ref and replaces it with a proper kref.
On last put of this kref, the target is removed from visibility in
sysfs.  The final call to scsi_target_reap() for the device is done from
__scsi_remove_device() and only if the device was made visible.  This
ensures that the target disappears as soon as the last device is gone
rather than waiting until final release of the device (which is often
too long).

Dan Carpenter b2a72ec32d [SCSI] qla2xxx: fix error handling of qla2x00_mem_alloc()
qla2x00_mem_alloc() returns 1 on success and -ENOMEM on failure.  On the
one hand the caller assumes non-zero is success but on the other hand
the caller also assumes that it returns an error code.

I've fixed it to return zero on success and a negative error code on
failure.  This matches the documentation as well.

Dan Williams 2193b1b16c [SCSI] isci: fix needless ata reset escalations
isci is needlessly tying libata's hands by returning
SAM_STAT_CHECK_CONDITION to some ata errors.  Instead, prefer
SAS_PROTO_RESPONSE to let libata (via sas_ata_task_done()) disposition
the device-to-host fis.

For example isci is triggering an HSM Violation where AHCI is showing a
simple media error for the same bus condition:

Dan Williams 3af74a3c07 [SCSI] libsas: introduce scmd_dbg() to quiet false positive "timeout" messages
libsas sometimes short circuits timeouts to force commands into error
recovery.  It is misleading to log that the command timed-out in
sas_scsi_timed_out() when in fact it was just queued for error handling.
It's also redundant in the case of a true timeout as libata eh will
detect and report timeouts via it's AC_ERR_TIMEOUT facility.

Given that some environments consider "timeout" errors to be indicative
of impending device failure demote the sas_scsi_timed_out() timeout
message to be disabled by default.  This parallels ata_scsi_timed_out().

The problem has been identified to be a change in the scsi_remove_device
path where a call to the pm_runtime_set_memalloc_noio was added when
del_gendisk is called in this path.  Note that the new pm routine
attempts to cycle through all parent devices from the FC target device
to set the memalloc_noio flag.  Because of this new change, a dependency
was created between the FC target device and the parent netdev device
in the destroy path.

In order to synchronized the destroy paths, bnx2fc has been modified
to flush all destroy workqueues in the NETDEV_UNREGISTER return path.

Jens Axboe 89f8b33ca1 block: remove old blk_iopoll_enabled variable
This was a debugging measure to toggle enabled/disabled
when testing. But for real production setups, it's not
safe to toggle this setting without either reloading
drivers of quiescing IO first. Neither of which the toggle
enforces.

Additionally, it makes drivers deal with the conditional
state.

Remove it completely. It's up to the driver whether iopoll
is enabled or not.

Arnd Bergmann eff9cf8d6e [SCSI] atari_scsi: Fix sleep_on race
sleep_on is known broken and going away. The atari_scsi driver is one of
two remaining users in the falcon_get_lock() function, which is a rather
crazy piece of code. This does not attempt to fix the driver's locking
scheme in general, but at least prevents falcon_get_lock from going to
sleep when no other thread holds the same lock or tries to get it,
and we no longer schedule with irqs disabled.

Lukasz Dorau c59053a23d [SCSI] isci: correct erroneous for_each_isci_host macro
In the first place, the loop 'for' in the macro 'for_each_isci_host'
(drivers/scsi/isci/host.h:314) is incorrect, because it accesses
the 3rd element of 2 element array. After the 2nd iteration it executes
the instruction:
        ihost = to_pci_info(pdev)->hosts[2]
(while the size of the 'hosts' array equals 2) and reads an
out of range element.

In the second place, this loop is incorrectly optimized by GCC v4.8
(see http://marc.info/?l=linux-kernel&m=138998871911336&w=2).
As a result, on platforms with two SCU controllers,
the loop is executed more times than it can be (for i=0,1 and 2).

Dan Williams ddfadd7736 [SCSI] isci: fix reset timeout handling
Remove an erroneous BUG_ON() in the case of a hard reset timeout.  The
reset timeout handler puts the port into the "awaiting link-up" state.
The timeout causes the device to be disconnected and we need to be in
the awaiting link-up state to re-connect the port.  The BUG_ON() made
the incorrect assumption that resets never timeout and we always
complete the reset in the "resetting" state.

Testing this patch also uncovered that libata continues to attempt to
reset the port long after the driver has torn down the context.  Once
the driver has committed to abandoning the link it must indicate to
libata that recovery ends by returning -ENODEV from
->lldd_I_T_nexus_reset().

Chad Dupuis f324777ea8 [SCSI] qla2xxx: Fix multiqueue MSI-X registration.
This fixes requesting of the MSI-X vectors for the base response queue.
The iteration in the for loop in qla24xx_enable_msix() was incorrect.
We should only iterate of the first two MSI-X vectors and not the total
number of MSI-X vectors that have given to the driver for this device
from pci_enable_msix() in this function.

Dr. Greg Wettstein 6f58c780e5 qla2xxx: Fix kernel panic on selective retransmission request
A selective retransmission request (SRR) is a fibre-channel
protocol control request which provides support for requesting
retransmission of a data sequence in response to an issue such as
frame loss or corruption.  These events are experienced
infrequently in fibre-channel based networks which makes
it difficult to test and assess codepaths which handle these
events.

We were fortunate enough, for some definition of fortunate, to
have a metro-area single-mode SAN link which, at 10 GBPS
sustained load levels, would consistently generate SRR's in
a SCST based target implementation using our SCST/in-kernel
Qlogic target interface driver.  In response to an SRR the
in-kernel Qlogic target driver immediately panics resulting
in a catastrophic storage failure for serviced initiators.

The culprit was a debug statement in the qla_target.c file which
does not verify that a pointer to the SCSI CDB is not null.
The unchecked pointer dereference results in the kernel panic
and resultant system failure.

The other two references to the SCSI CDB by the SRR handling code
use a ternary operator to verify a non-null pointer is being
acted on.  This patch simply adds a similar test to the implicated
debug statement.

This patch is a candidate for any stable kernel being maintained
since it addresses a potentially catastrophic event with
minimal downside.

Nicholas Bellinger 7474f52a82 tcm_qla2xxx: Perform configfs depend/undepend for base_tpg
This patch performs configfs_depend_item() during TPG enable for
base_tpg (eg: non-NPIV) ports, and configfs_undepend_item() during
TPG disable for base_tpg.

This is done to ensure that any attempt to configfs rmdir a base_tpg
with active NPIV ports will fail with -EBUSY, until all associated
NPIV ports have been explicitly shutdown and base_tpg disabled.

Note that the actual configfs_[un]depend_item() is done from seperate
process context, as these are not intended to be called directly
from configfs callbacks.

Nicholas Bellinger ddb95145a3 qla2xxx: Fix qlt_lport_register base_vha callback race
This patch closes a race between qlt_lport_register() and
tcm_qla2xxx callback logic by holding qla_tgt_mutex before
making the callback.

In order for this to work, the qlt_add_target() and
qlt_remove_target() code has been changed to avoid the
accessing qla_tgt_mutex + list_[add,del] for NPIV enabled
ports.

This bug introduced in v3.14-rc1 code with commit 49a47f2.

Bart Van Assche 93079162bf scsi_transport_srp: Fix a race condition
The rport timers must be stopped before the SRP initiator destroys the
resources associated with the SCSI host. This is necessary because
otherwise the callback functions invoked from the SRP transport layer
could trigger a use-after-free. Stopping the rport timers before
invoking scsi_remove_host() can trigger long delays in the SCSI error
handler if a transport layer failure occurs while scsi_remove_host()
is in progress. Hence move the code for stopping the rport timers from
srp_rport_release() into a new function and invoke that function after
scsi_remove_host() has finished. This patch fixes the following

The current behavior of the SRP transport layer when a transport layer
error is encountered is to block SCSI command processing only if
fast_io_fail_tmo != off.  The current behavior of the FC transport
layer when a transport layer error is encountered is to block SCSI
command processing no matter which value fast_io_fail_tmo has been set
to.  Make the behavior of the SRP transport layer consistent with that
of the FC transport layer to avoid confusion.

Nicholas Bellinger 49a47f2caf qla2xxx: Configure NPIV fc_vport via tcm_qla2xxx_npiv_make_lport
This patch changes qla2xxx qlt_lport_register() code to accept
target_lport_ptr + npiv_wwpn + npiv_wwnn parameters, and updates
tcm_qla2xxx to use the new tcm_qla2xxx_lport_register_npiv_cb()
callback for invoking fc_vport_create() from configfs context
via tcm_qla2xxx_npiv_make_lport() code.

In order for this to work, the qlt_lport_register() callback is
now called without holding qla_tgt_mutex, as the fc_vport creation
process will call qlt_vport_create() -> qlt_add_target(), which
already expects to acquire it.

It enforces /sys/kernel/config/target/qla2xxx_npiv/$NPIV_WWPN
naming in the following format:

     $PHYSICAL_WWPN@$NPIV_WWPN:$NPIV_WWNN

and assumes the $PHYSICAL_WWPN in question has already had been

These interfaces:

  pcibios_resource_to_bus(struct pci_dev *dev, *bus_region, *resource)
  pcibios_bus_to_resource(struct pci_dev *dev, *resource, *bus_region)

took a pci_dev, but they really depend only on the pci_bus.  And we want to
use them in resource allocation paths where we have the bus but not a
device, so this patch converts them to take the pci_bus instead of the
pci_dev:

  pcibios_resource_to_bus(struct pci_bus *bus, *bus_region, *resource)
  pcibios_bus_to_resource(struct pci_bus *bus, *resource, *bus_region)

In fact, with standard PCI-PCI bridges, they only depend on the host
bridge, because that's the only place address translation occurs, but
we aren't going that far yet.

Bodo Stroesser 769989a4a0 [SCSI] st: fix enlarge_buffer
This patch removes a bug in enlarge_buffer() that can make a
read or write fail under special conditions.

After changing TRY_DIRECT_IO to 0 and ST_MAX_SG to 32 in
st_options.h, a program that writes a first block of 128k and
than a second bigger block (e.g. 256k) fails. The second write
returns errno EOVERFLOW, as enlarge_buffer() checks the sg list
and detects that it already is full.
As enlarge_buffer uses different page allocation orders
depending on the size of the buffer needed, the check does not
make sense.

Ren Mingxin bb3b621a33 [SCSI] Set the minimum valid value of 'eh_deadline' as 0
The former minimum valid value of 'eh_deadline' is 1s, which means
the earliest occasion to shorten EH is 1 second later since a
command is failed or timed out. But if we want to skip EH steps
ASAP, we have to wait until the first EH step is finished. If the
duration of the first EH step is long, this waiting time is
excruciating. So, it is necessary to accept 0 as the minimum valid
value for 'eh_deadline'.

According to my test, with Hannes' patchset 'New EH command timeout
handler' as well, the minimum IO time is improved from 73s
(eh_deadline = 1) to 43s(eh_deadline = 0) when commands are timed
out by disabling RSCN and target port.

Hannes Reinecke 76ad3e5956 [SCSI] Unlock accesses to eh_deadline
32bit accesses are guaranteed to be atomic, so we can remove
the spinlock when checking for eh_deadline. We only need to
make sure to catch any updates which might happened during
the call to time_before(); if so we just recheck with the
correct value.

Hannes Reinecke e494f6a728 [SCSI] improved eh timeout handler
When a command runs into a timeout we need to send an 'ABORT TASK'
TMF. This is typically done by the 'eh_abort_handler' LLDD callback.

Conceptually, however, this function is a normal SCSI command, so
there is no need to enter the error handler.

This patch implements a new scsi_abort_command() function which
invokes an asynchronous function scsi_eh_abort_handler() to
abort the commands via the usual 'eh_abort_handler'.

If abort succeeds the command is either retried or terminated,
depending on the number of allowed retries. However, 'eh_eflags'
records the abort, so if the retry would fail again the
command is pushed onto the error handler without trying to
abort it (again); it'll be cleared up from SCSI EH.

James Bottomley 2451079bc2 [SCSI] Fix erratic device offline during EH
Commit 18a4d0a22e
(Handle disk devices which can not process medium access commands)
was introduced to offline any device which cannot process medium
access commands.
However, commit 3eef6257de
(Reduce error recovery time by reducing use of TURs) reduced
the number of TURs by sending it only on the first failing
command, which might or might not be a medium access command.
So in combination this results in an erratic device offlining
during EH; if the command where the TUR was sent upon happens
to be a medium access command the device will be set offline,
if not everything proceeds as normal.

This patch moves the check to the final test, eliminating
this problem.

Paul Gortmaker 1a4049ddfd [SCSI] aci7xxx_old: delete decade+ obsolete driver
After getting warnings in an allyesconfig build[1] from this
driver, I decided to remind myself just how old it was, and
whether it warranted fixing.  In the Kconfig help text, I found:

  "This driver will eventually be phased out entirely"

Going back to the history archive, I see the line was added[2]
in Feb 2002, when we moved from v2.4.2.1 ---> v2.4.2.2

So, with over a decade of notification, and multiple major releases
since then, I think we can justify removing this.  Currently we have
people wasting time building it during routine testing, and then
wasting more time re-researching the known reported warnings, only to
find that nobody really is willing to integrate the fixes[3] for it.

A quick search didn't seem to indicate any active user base for it.
If someone happens to have a quirky _old_ card that the eleven year
old "new" driver doesn't work with, then it is entirely reasonable
that they stick with a kernel version that predates this removal.

Chad Dupuis f3ddac1918 [SCSI] qla2xxx: Disable adapter when we encounter a PCI disconnect.
If we become disconnected from the PCI bus/PCIe fabric, there can be long delays
in register reads which can cause erroneous decisions to be made and cause a
soft lockup if a lock is held too long. As a preventative measure, check for a
disconnection (register reads that return -1) and then disable the board if we
find ourselves in this condition. For now, check in our interrupt handlers and
the per adapter one second timer.

Stephen M. Cameron 396883e292 [SCSI] hpsa: prevent stalled i/o
If a fifo full condition is encountered, i/o requests will stack
up in the h->reqQ queue.  The only thing which empties this queue
is start_io, which only gets called when new i/o requests come in.
If none are forthcoming, i/o in h->reqQ will be stalled.

To fix this, whenever fifo full condition is encountered, this
is recorded, and the interrupt handler examines this to see
if a fifo full condition was recently encountered when a
command completes and will call start_io to prevent i/o's in
h->reqQ from getting stuck.

I've only ever seen this problem occur when running specialized
test programs that pound on the the CCISS_PASSTHRU ioctl.

Stephen M. Cameron 0390f0c0df [SCSI] hpsa: cap CCISS_PASSTHRU at 20 concurrent commands.
Cap CCISS_BIG_PASSTHRU as well.  If an attempt is made
to exceed this, ioctl() will return -1 with errno == EAGAIN.

This is to prevent a userland program from exhausting all of
pci_alloc_consistent memory.  I've only seen this problem when
running a special test program designed to provoke it.  20
concurrent commands via the passthru ioctls (not counting SG_IO)
should be more than enough.

Vijaya Mohan Guvva dcaf9aed99 [SCSI] bfa: Chinook quad port 16G FC HBA claim issue
Bfa driver crash is observed while pushing the firmware on to chinook
quad port card due to uninitialized bfi_image_ct2 access which gets
initialized only for CT2 ASIC based cards after request_firmware().
For quard port chinook (CT2 ASIC based), bfi_image_ct2 is not getting
initialized as there is no check for chinook PCI device ID before
request_firmware and instead bfi_image_cb is initialized as it is the
default case for card type check.

This patch includes changes to read the right firmware for quad port chinook.

Stephen M. Cameron 8500923947 [SCSI] hpsa: add 5 second delay after doorbell reset
The hardware guys tell us that after initiating a software
reset via the doorbell register we need to wait 5 seconds before
attempting to talk to the board *at all*.  This means that we
cannot watch the board to verify it transitions from "ready" to
to "not ready" then back "ready", since this transition will
most likely happen during those 5 seconds (though we can still
verify the reset happens by watching the "driver version" field
get cleared.)

Aaron Lu 6627b38fd9 [SCSI] sr: use block layer runtime PM
Migrate sr to make use of block layer runtime PM. Accordingly, the
SCSI bus layer runtime PM callback is simplified as all SCSI drivers
implementing runtime PM now use the block layer's request-based
mechanism.

Note that due to the device will be polled by kernel at a constant
interval, if the autosuspend delay is set longer than the polling
interval then the device will never suspend.
