perf/hwbp: Simplify the perf-hwbp code, fix documentation

Annoyingly, modify_user_hw_breakpoint() unnecessarily complicates the
modification of a breakpoint - simplify it and remove the pointless
local variables.

Also update the stale Docbook while at it.
