ide-tape: simplify read/write functions

Impact: cleanup

idetape_chrdev_read/write() functions are unnecessarily complex when
everything can be handled in a single loop.  Collapse
idetape_add_chrdev_read/write_request() into the rw functions and
simplify the implementation.
