Tom Lendacky 3e93467346 KVM: SVM: Fix build error when using -Werror=unused-but-set-variable
Commit 916e3e5f26 ("KVM: SVM: Do not use user return MSR support for
virtualized TSC_AUX") introduced a local variable used for the rdmsr()
function for the high 32-bits of the MSR value. This variable is not used
after being set and triggers a warning or error, when treating warnings
as errors, when the unused-but-set-variable flag is set. Mark this
variable as __maybe_unused to fix this.

Maxim Levitsky 3fdc6087df x86: KVM: SVM: refresh AVIC inhibition in svm_leave_nested()
svm_leave_nested() similar to a nested VM exit, get the vCPU out of nested
mode and thus should end the local inhibition of AVIC on this vCPU.

Failure to do so, can lead to hangs on guest reboot.

Raise the KVM_REQ_APICV_UPDATE request to refresh the AVIC state of the
current vCPU in this case.

Maxim Levitsky 2dcf37abf9 x86: KVM: SVM: add support for Invalid IPI Vector interception
In later revisions of AMD's APM, there is a new 'incomplete IPI' exit code:

"Invalid IPI Vector - The vector for the specified IPI was set to an
illegal value (VEC < 16)"

Note that tests on Zen2 machine show that this VM exit doesn't happen and
instead AVIC just does nothing.

Add support for this exit code by doing nothing, instead of filling
the kernel log with errors.

Also replace an unthrottled 'pr_err()' if another unknown incomplete
IPI exit happens with vcpu_unimpl()

(e.g in case AMD adds yet another 'Invalid IPI' exit reason)

Maxim Levitsky b65235f6e1 x86: KVM: SVM: always update the x2avic msr interception
The following problem exists since x2avic was enabled in the KVM:

svm_set_x2apic_msr_interception is called to enable the interception of
the x2apic msrs.

In particular it is called at the moment the guest resets its apic.

Assuming that the guest's apic was in x2apic mode, the reset will bring
it back to the xapic mode.

The svm_set_x2apic_msr_interception however has an erroneous check for
'!apic_x2apic_mode()' which prevents it from doing anything in this case.

As a result of this, all x2apic msrs are left unintercepted, and that
exposes the bare metal x2apic (if enabled) to the guest.
Oops.

Remove the erroneous '!apic_x2apic_mode()' check to fix that.

This fixes CVE-2023-5090

Sean Christopherson 8647c52e95 KVM: x86: Constrain guest-supported xfeatures only at KVM_GET_XSAVE{2}
Mask off xfeatures that aren't exposed to the guest only when saving guest
state via KVM_GET_XSAVE{2} instead of modifying user_xfeatures directly.
Preserving the maximal set of xfeatures in user_xfeatures restores KVM's
ABI for KVM_SET_XSAVE, which prior to commit ad856280dd ("x86/kvm/fpu:
Limit guest user_xfeatures to supported bits of XCR0") allowed userspace
to load xfeatures that are supported by the host, irrespective of what
xfeatures are exposed to the guest.

There is no known use case where userspace *intentionally* loads xfeatures
that aren't exposed to the guest, but the bug fixed by commit ad856280dd
was specifically that KVM_GET_SAVE{2} would save xfeatures that weren't
exposed to the guest, e.g. would lead to userspace unintentionally loading
guest-unsupported xfeatures when live migrating a VM.

Restricting KVM_SET_XSAVE to guest-supported xfeatures is especially
problematic for QEMU-based setups, as QEMU has a bug where instead of
terminating the VM if KVM_SET_XSAVE fails, QEMU instead simply stops
loading guest state, i.e. resumes the guest after live migration with
incomplete guest state, and ultimately results in guest data corruption.

Note, letting userspace restore all host-supported xfeatures does not fix
setups where a VM is migrated from a host *without* commit ad856280dd,
to a target with a subset of host-supported xfeatures.  However there is
no way to safely address that scenario, e.g. KVM could silently drop the
unsupported features, but that would be a clear violation of KVM's ABI and
so would require userspace to opt-in, at which point userspace could
simply be updated to sanitize the to-be-loaded XSAVE state.

Sean Christopherson 18164f66e6 x86/fpu: Allow caller to constrain xfeatures when copying to uabi buffer
Plumb an xfeatures mask into __copy_xstate_to_uabi_buf() so that KVM can
constrain which xfeatures are saved into the userspace buffer without
having to modify the user_xfeatures field in KVM's guest_fpu state.

KVM's ABI for KVM_GET_XSAVE{2} is that features that are not exposed to
guest must not show up in the effective xstate_bv field of the buffer.
Saving only the guest-supported xfeatures allows userspace to load the
saved state on a different host with a fewer xfeatures, so long as the
target host supports the xfeatures that are exposed to the guest.

KVM currently sets user_xfeatures directly to restrict KVM_GET_XSAVE{2} to
the set of guest-supported xfeatures, but doing so broke KVM's historical
ABI for KVM_SET_XSAVE, which allows userspace to load any xfeatures that
are supported by the *host*.

Russell King (Oracle) c4dd854f74 cpu-hotplug: Provide prototypes for arch CPU registration
Provide common prototypes for arch_register_cpu() and
arch_unregister_cpu(). These are called by acpi_processor.c, with weak
versions, so the prototype for this is already set. It is generally not
necessary for function prototypes to be conditional on preprocessor macros.

Some architectures (e.g. Loongarch) are missing the prototype for this, and
rather than add it to Loongarch's asm/cpu.h, do the job once for everyone.

Since this covers everyone, remove the now unnecessary prototypes in
asm/cpu.h, and therefore remove the 'static' from one of ia64's
arch_register_cpu() definitions.

A virt scenario can be constructed where MMIO memory can be user memory.
When that happens, a race condition opens between when the hardware
raises the #VC and when the #VC handler gets to emulate the instruction.

If the MOVS is replaced with a MOVS accessing kernel memory in that
small race window, then write to kernel memory happens as the access
checks are not done at emulation time.

Disable MMIO emulation in user mode temporarily until a sensible use
case appears and justifies properly handling the race window.

We found that a panic can occur when a vsyscall is made while LBR sampling
is active. If the vsyscall is interrupted (NMI) for perf sampling, this
call sequence can occur (most recent at top):

    __insn_get_emulate_prefix()
    insn_get_emulate_prefix()
    insn_get_prefixes()
    insn_get_opcode()
    decode_branch_type()
    get_branch_type()
    intel_pmu_lbr_filter()
    intel_pmu_handle_irq()
    perf_event_nmi_handler()

Within __insn_get_emulate_prefix() at frame 0, a macro is called:

    peek_nbyte_next(insn_byte_t, insn, i)

Within this macro, this dereference occurs:

    (insn)->next_byte

Inspecting registers at this point, the value of the next_byte field is the
address of the vsyscall made, for example the location of the vsyscall
version of gettimeofday() at 0xffffffffff600000. The access to an address
in the vsyscall region will trigger an oops due to an unhandled page fault.

To fix the bug, filtering for vsyscalls can be done when
determining the branch type. This patch will return
a "none" branch if a kernel address if found to lie in the
vsyscall region.

Tom Lendacky 6bc6f7d9d7 x86/sev: Use the GHCB protocol when available for SNP CPUID requests
SNP retrieves the majority of CPUID information from the SNP CPUID page.
But there are times when that information needs to be supplemented by the
hypervisor, for example, obtaining the initial APIC ID of the vCPU from
leaf 1.

The current implementation uses the MSR protocol to retrieve the data from
the hypervisor, even when a GHCB exists. The problem arises when an NMI
arrives on return from the VMGEXIT. The NMI will be immediately serviced
and may generate a #VC requiring communication with the hypervisor.

Since a GHCB exists in this case, it will be used. As part of using the
GHCB, the #VC handler will write the GHCB physical address into the GHCB
MSR and the #VC will be handled.

When the NMI completes, processing resumes at the site of the VMGEXIT
which is expecting to read the GHCB MSR and find a CPUID MSR protocol
response. Since the NMI handling overwrote the GHCB MSR response, the
guest will see an invalid reply from the hypervisor and self-terminate.

Fix this problem by using the GHCB when it is available. Any NMI
received is properly handled because the GHCB contents are copied into
a backup page and restored on NMI exit, thus preserving the active GHCB
request or result.

Haitao Huang c6c2adcba5 x86/sgx: Resolves SECS reclaim vs. page fault for EAUG race
The SGX EPC reclaimer (ksgxd) may reclaim the SECS EPC page for an
enclave and set secs.epc_page to NULL. The SECS page is used for EAUG
and ELDU in the SGX page fault handler. However, the NULL check for
secs.epc_page is only done for ELDU, not EAUG before being used.

Fix this by doing the same NULL check and reloading of the SECS page as
needed for both EAUG and ELDU.

The SECS page holds global enclave metadata. It can only be reclaimed
when there are no other enclave pages remaining. At that point,
virtually nothing can be done with the enclave until the SECS page is
paged back in.

An enclave can not run nor generate page faults without a resident SECS
page. But it is still possible for a #PF for a non-SECS page to race
with paging out the SECS page: when the last resident non-SECS page A
triggers a #PF in a non-resident page B, and then page A and the SECS
both are paged out before the #PF on B is handled.

Hitting this bug requires that race triggered with a #PF for EAUG.

Jim Mattson 73554b29bd KVM: x86/pmu: Synthesize at most one PMI per VM-exit
When the irq_work callback, kvm_pmi_trigger_fn(), is invoked during a
VM-exit that also invokes __kvm_perf_overflow() as a result of
instruction emulation, kvm_pmu_deliver_pmi() will be called twice
before the next VM-entry.

Calling kvm_pmu_deliver_pmi() twice is unlikely to be problematic now that
KVM sets the LVTPC mask bit when delivering a PMI.  But using IRQ work to
trigger the PMI is still broken, albeit very theoretically.

E.g. if the self-IPI to trigger IRQ work is be delayed long enough for the
vCPU to be migrated to a different pCPU, then it's possible for
kvm_pmi_trigger_fn() to race with the kvm_pmu_deliver_pmi() from
KVM_REQ_PMI and still generate two PMIs.

KVM could set the mask bit using an atomic operation, but that'd just be
piling on unnecessary code to workaround what is effectively a hack.  The
*only* reason KVM uses IRQ work is to ensure the PMI is treated as a wake
event, e.g. if the vCPU just executed HLT.

Remove the irq_work callback for synthesizing a PMI, and all of the
logic for invoking it. Instead, to prevent a vcpu from leaving C0 with
a PMI pending, add a check for KVM_REQ_PMI to kvm_vcpu_has_events().

Jim Mattson a16eb25b09 KVM: x86: Mask LVTPC when handling a PMI
Per the SDM, "When the local APIC handles a performance-monitoring
counters interrupt, it automatically sets the mask flag in the LVT
performance counter register."  Add this behavior to KVM's local APIC
emulation.

Failure to mask the LVTPC entry results in spurious PMIs, e.g. when
running Linux as a guest, PMI handlers that do a "late_ack" spew a large
number of "dazed and confused" spurious NMI warnings.

Roman Kagan b29a2acd36 KVM: x86/pmu: Truncate counter value to allowed width on write
Performance counters are defined to have width less than 64 bits.  The
vPMU code maintains the counters in u64 variables but assumes the value
to fit within the defined width.  However, for Intel non-full-width
counters (MSR_IA32_PERFCTRx) the value receieved from the guest is
truncated to 32 bits and then sign-extended to full 64 bits.  If a
negative value is set, it's sign-extended to 64 bits, but then in
kvm_pmu_incr_counter() it's incremented, truncated, and compared to the
previous value for overflow detection.

That previous value is not truncated, so it always evaluates bigger than
the truncated new one, and a PMI is injected.  If the PMI handler writes
a negative counter value itself, the vCPU never quits the PMI loop.

Turns out that Linux PMI handler actually does write the counter with
the value just read with RDPMC, so when no full-width support is exposed
via MSR_IA32_PERF_CAPABILITIES, and the guest initializes the counter to
a negative value, it locks up.

This has been observed in the field, for example, when the guest configures
atop to use perfevents and runs two instances of it simultaneously.

To address the problem, maintain the invariant that the counter value
always fits in the defined bit width, by truncating the received value
in the respective set_msr methods.  For better readability, factor the
out into a helper function, pmc_write_counter(), shared by vmx and svm
parts.

Zen 4 systems running buggy microcode can hit a WARN_ON() in the PMI
handler, as shown below, several times while perf runs. A simple
`perf top` run is enough to render the system unusable:

This happens because the Performance Counter Global Status Register
(PerfCntGlobalStatus) has one or more bits set which are considered
reserved according to the "AMD64 Architecture Programmer’s Manual,
Volume 2: System Programming, 24593":

To make this less intrusive, warn just once if any reserved bit is set
and prompt the user to update the microcode. Also sanitize the value to
what the code is handling, so that the overflow events continue to be
handled for the number of counters that are known to be sane.

for Zen 4 processors in order to avoid such issues with reserved bits:

Tom Lendacky 916e3e5f26 KVM: SVM: Do not use user return MSR support for virtualized TSC_AUX
When the TSC_AUX MSR is virtualized, the TSC_AUX value is swap type "B"
within the VMSA. This means that the guest value is loaded on VMRUN and
the host value is restored from the host save area on #VMEXIT.

Since the value is restored on #VMEXIT, the KVM user return MSR support
for TSC_AUX can be replaced by populating the host save area with the
current host value of TSC_AUX. And, since TSC_AUX is not changed by Linux
post-boot, the host save area can be set once in svm_hardware_enable().
This eliminates the two WRMSR instructions associated with the user return
MSR support.

Tom Lendacky e0096d01c4 KVM: SVM: Fix TSC_AUX virtualization setup
The checks for virtualizing TSC_AUX occur during the vCPU reset processing
path. However, at the time of initial vCPU reset processing, when the vCPU
is first created, not all of the guest CPUID information has been set. In
this case the RDTSCP and RDPID feature support for the guest is not in
place and so TSC_AUX virtualization is not established.

This continues for each vCPU created for the guest. On the first boot of
an AP, vCPU reset processing is executed as a result of an APIC INIT
event, this time with all of the guest CPUID information set, resulting
in TSC_AUX virtualization being enabled, but only for the APs. The BSP
always sees a TSC_AUX value of 0 which probably went unnoticed because,
at least for Linux, the BSP TSC_AUX value is 0.

Move the TSC_AUX virtualization enablement out of the init_vmcb() path and
into the vcpu_after_set_cpuid() path to allow for proper initialization of
the support after the guest CPUID information has been set.

With the TSC_AUX virtualization support now in the vcpu_set_after_cpuid()
path, the intercepts must be either cleared or set based on the guest
CPUID input.

Sean Christopherson 0df9dab891 KVM: x86/mmu: Stop zapping invalidated TDP MMU roots asynchronously
Stop zapping invalidate TDP MMU roots via work queue now that KVM
preserves TDP MMU roots until they are explicitly invalidated.  Zapping
roots asynchronously was effectively a workaround to avoid stalling a vCPU
for an extended during if a vCPU unloaded a root, which at the time
happened whenever the guest toggled CR0.WP (a frequent operation for some
guest kernels).

While a clever hack, zapping roots via an unbound worker had subtle,
unintended consequences on host scheduling, especially when zapping
multiple roots, e.g. as part of a memslot.  Because the work of zapping a
root is no longer bound to the task that initiated the zap, things like
the CPU affinity and priority of the original task get lost.  Losing the
affinity and priority can be especially problematic if unbound workqueues
aren't affined to a small number of CPUs, as zapping multiple roots can
cause KVM to heavily utilize the majority of CPUs in the system, *beyond*
the CPUs KVM is already using to run vCPUs.

When deleting a memslot via KVM_SET_USER_MEMORY_REGION, the async root
zap can result in KVM occupying all logical CPUs for ~8ms, and result in
high priority tasks not being scheduled in in a timely manner.  In v5.15,
which doesn't preserve unloaded roots, the issues were even more noticeable
as KVM would zap roots more frequently and could occupy all CPUs for 50ms+.

Consuming all CPUs for an extended duration can lead to significant jitter
throughout the system, e.g. on ChromeOS with virtio-gpu, deleting memslots
is a semi-frequent operation as memslots are deleted and recreated with
different host virtual addresses to react to host GPU drivers allocating
and freeing GPU blobs.  On ChromeOS, the jitter manifests as audio blips
during games due to the audio server's tasks not getting scheduled in
promptly, despite the tasks having a high realtime priority.

Deleting memslots isn't exactly a fast path and should be avoided when
possible, and ChromeOS is working towards utilizing MAP_FIXED to avoid the
memslot shenanigans, but KVM is squarely in the wrong.  Not to mention
that removing the async zapping eliminates a non-trivial amount of
complexity.

Note, one of the subtle behaviors hidden behind the async zapping is that
KVM would zap invalidated roots only once (ignoring partial zaps from
things like mmu_notifier events).  Preserve this behavior by adding a flag
to identify roots that are scheduled to be zapped versus roots that have
already been zapped but not yet freed.

Add a comment calling out why kvm_tdp_mmu_invalidate_all_roots() can
encounter invalid roots, as it's not at all obvious why zapping
invalidated roots shouldn't simply zap all invalid roots.

Saurabh Sengar 14058f72cf x86/hyperv: Remove hv_vtl_early_init initcall
There has been cases reported where HYPERV_VTL_MODE is enabled by mistake,
on a non Hyper-V platforms. This causes the hv_vtl_early_init function to
be called in an non Hyper-V/VTL platforms which results the memory
corruption.

Remove the early_initcall for hv_vtl_early_init and call it at the end of
hyperv_init to make sure it is never called in a non Hyper-V platform by
mistake.

Saurabh Sengar f2a55d08d7 x86/hyperv: Restrict get_vtl to only VTL platforms
When Linux runs in a non-default VTL (CONFIG_HYPERV_VTL_MODE=y),
get_vtl() must never fail as its return value is used in negotiations
with the host. In the more generic case, (CONFIG_HYPERV_VTL_MODE=n) the
VTL is always zero so there's no need to do the hypercall.

Make get_vtl() BUG() in case of failure and put the implementation under
"if IS_ENABLED(CONFIG_HYPERV_VTL_MODE)" to avoid the call altogether in
the most generic use case.

made '__x86_return_thunk' a placeholder value.  All code setting
X86_FEATURE_RETHUNK also changes the value of 'x86_return_thunk'.  So
the optimization at the beginning of apply_returns() is dead code.

Also, before the above-mentioned commit, the optimization actually had a
bug It bypassed __static_call_fixup(), causing some raw returns to
remain unpatched in static call trampolines.  Thus the 'Fixes' tag.

Sandipan Das 23d2626b84 perf/x86/amd/core: Fix overflow reset on hotplug
Kernels older than v5.19 do not support PerfMonV2 and the PMI handler
does not clear the overflow bits of the PerfCntrGlobalStatus register.
Because of this, loading a recent kernel using kexec from an older
kernel can result in inconsistent register states on Zen 4 systems.

The PMI handler of the new kernel gets confused and shows a warning when
an overflow occurs because some of the overflow bits are set even if the
corresponding counters are inactive. These are remnants from overflows
that were handled by the older kernel.

During CPU hotplug, the PerfCntrGlobalCtl and PerfCntrGlobalStatus
registers should always be cleared for PerfMonV2-capable processors.
However, a condition used for NB event constaints applicable only to
older processors currently prevents this from happening. Move the reset
sequence to an appropriate place and also clear the LBR Freeze bit.

Sean Christopherson 50107e8b2a KVM: x86/mmu: Open code leaf invalidation from mmu_notifier
The mmu_notifier path is a bit of a special snowflake, e.g. it zaps only a
single address space (because it's per-slot), and can't always yield.
Because of this, it calls kvm_tdp_mmu_zap_leafs() in ways that no one
else does.

Iterate manually over the leafs in response to an mmu_notifier
invalidation, instead of invoking kvm_tdp_mmu_zap_leafs().  Drop the
@can_yield param from kvm_tdp_mmu_zap_leafs() as its sole remaining
caller unconditionally passes "true".

There are several ways a thread's shadow stacks can get unmapped. This
can happen on exit or exec, as well as error handling in exec or clone.
The task struct already keeps track of the thread's shadow stack. Use the
size variable to keep track of if the shadow stack has already been freed.

When an attempt to double unmap the thread shadow stack is caught, warn
about it and abort the operation.

stack is cleaned up in exit_thread() in copy_process(). So the logic in
copy_thread() is unneeded, and also will not handle failures that happen

In addition, since there is a second attempt to unmap the same shadow
stack, there is a race where an newly mapped region could get unmapped.

So remove the logic in copy_thread() and rely on exit_thread() to handle
clone failure.

Rick Edgecombe 331955600d x86/shstk: Handle vfork clone failure correctly
Shadow stacks are allocated automatically and freed on exit, depending
on the clone flags. The two cases where new shadow stacks are not
allocated are !CLONE_VM (fork()) and CLONE_VFORK (vfork()). For
!CLONE_VM, although a new stack is not allocated, it can be freed normally
because it will happen in the child's copy of the VM.

However, for CLONE_VFORK the parent and the child are actually using the
same shadow stack. So the kernel doesn't need to allocate *or* free a
shadow stack for a CLONE_VFORK child. CLONE_VFORK children already need
special tracking to avoid returning to userspace until the child exits or
execs. Shadow stack uses this same tracking to avoid freeing CLONE_VFORK
shadow stacks.

However, the tracking is not setup until the clone has succeeded
(internally). Which means, if a CLONE_VFORK fails, the existing logic will
not know it is a CLONE_VFORK and proceed to unmap the parents shadow stack.
This error handling cleanup logic runs via exit_thread() in the
bad_fork_cleanup_thread label in copy_process(). The issue was seen in
the glibc test "posix/tst-spawn3-pidfd" while running with shadow stack
using currently out-of-tree glibc patches.

Fix it by not unmapping the vfork shadow stack in the error case as well.
Since clone is implemented in core code, it is not ideal to pass the clone
flags along the error path in order to have shadow stack code have
symmetric logic in the freeing half of the thread shadow stack handling.

Instead use the existing state for thread shadow stacks to track whether
the thread is managing its own shadow stack. For CLONE_VFORK, simply set
shstk->base and shstk->size to 0, and have it mean the thread is not
managing a shadow stack and so should skip cleanup work. Implement this
by breaking up the CLONE_VFORK and !CLONE_VM cases in
shstk_alloc_thread_stack() to separate conditionals since, the logic is
now different between them. In the case of CLONE_VFORK && !CLONE_VM, the
existing behavior is to not clean up the shadow stack in the child (which
should go away quickly with either be exit or exec), so maintain that
behavior by handling the CLONE_VFORK case first in the allocation path.

This new logioc cleanly handles the case of normal, successful
CLONE_VFORK's skipping cleaning up their shadow stack's on exit as well.
So remove the existing, vfork shadow stack freeing logic. This is in
deactivate_mm() where vfork_done is used to tell if it is a vfork child
that can skip cleaning up the thread shadow stack.

IBPB.  The current code doesn't allow for that.

Also, cpu_has_ibpb_brtype_microcode() has some surprising side effects
and the setting of these feature bits really doesn't belong in the
mitigation code anyway.  Move it to earlier.

Justin Stitt 0fc6ff5a0f xen/efi: refactor deprecated strncpy
`strncpy` is deprecated for use on NUL-terminated destination strings [1].

`efi_loader_signature` has space for 4 bytes. We are copying "Xen" (3 bytes)
plus a NUL-byte which makes 4 total bytes. With that being said, there is
currently not a bug with the current `strncpy()` implementation in terms of
buffer overreads but we should favor a more robust string interface
either way.

A suitable replacement is `strscpy` [2] due to the fact that it guarantees
NUL-termination on the destination buffer while being functionally the
same in this case.

Juergen Gross 49147beb0c x86/xen: allow nesting of same lazy mode
When running as a paravirtualized guest under Xen, Linux is using
"lazy mode" for issuing hypercalls which don't need to take immediate
effect in order to improve performance (examples are e.g. multiple
PTE changes).

There are two different lazy modes defined: MMU and CPU lazy mode.
Today it is not possible to nest multiple lazy mode sections, even if
they are of the same kind. A recent change in memory management added
nesting of MMU lazy mode sections, resulting in a regression when
running as Xen PV guest.

Technically there is no reason why nesting of multiple sections of the
same kind of lazy mode shouldn't be allowed. So add support for that
for fixing the regression.

There are several functions involved for performing the functionality
of evtchn_do_upcall():

- __xen_evtchn_do_upcall() doing the real work
- xen_hvm_evtchn_do_upcall() just being a wrapper for
  __xen_evtchn_do_upcall(), exposed for external callers
- xen_evtchn_do_upcall() calling __xen_evtchn_do_upcall(), too, but
  without any user

Simplify this maze by:

- removing the unused xen_evtchn_do_upcall()
- removing xen_hvm_evtchn_do_upcall() as the only left caller of
  __xen_evtchn_do_upcall(), while renaming __xen_evtchn_do_upcall() to
  xen_evtchn_do_upcall()

The code calling ima_free_kexec_buffer() runs long after the memblock
allocator has already been torn down, potentially resulting in a use
after free in memblock_isolate_range().

With KASAN or KFENCE, this use after free will result in a BUG
from the idle task, and a subsequent kernel panic.

Switch ima_free_kexec_buffer() over to memblock_free_late() to avoid
that bug.

Kirill A. Shutemov f530ee95b7 x86/boot/compressed: Reserve more memory for page tables
The decompressor has a hard limit on the number of page tables it can
allocate. This limit is defined at compile-time and will cause boot
failure if it is reached.

The kernel is very strict and calculates the limit precisely for the
worst-case scenario based on the current configuration. However, it is
easy to forget to adjust the limit when a new use-case arises. The
worst-case scenario is rarely encountered during sanity checks.

In the case of enabling 5-level paging, a use-case was overlooked. The
limit needs to be increased by one to accommodate the additional level.
This oversight went unnoticed until Aaron attempted to run the kernel
via kexec with 5-level paging and unaccepted memory enabled.

Update wost-case calculations to include 5-level paging.

To address this issue, let's allocate some extra space for page tables.
128K should be sufficient for any use-case. The logic can be simplified
by using a single value for all kernel configurations.

Ricardo Neri 108af4b4bd x86/sched: Restore the SD_ASYM_PACKING flag in the DIE domain
Commit 8f2d6c41e5 ("x86/sched: Rewrite topology setup") dropped the
SD_ASYM_PACKING flag in the DIE domain added in commit 044f0e27de
("x86/sched: Add the SD_ASYM_PACKING flag to the die domain of hybrid
processors"). Restore it on hybrid processors.

The die-level domain does not depend on any build configuration and now
x86_sched_itmt_flags() is always needed. Remove the build dependency on
CONFIG_SCHED_[SMT|CLUSTER|MC].

It was reported that under certain circumstances GCC emits ENDBR
instructions for _THIS_IP_ usage. Specifically, when it appears at the
start of a basic block -- but not elsewhere.

Since _THIS_IP_ is never used for control flow, these ENDBR
instructions are completely superfluous. Override the _THIS_IP_
definition for x86_64 to avoid this.

Less ENDBR instructions is better.

Steve Wahl 5290e88ba2 x86/platform/uv: Use alternate source for socket to node data
The UV code attempts to build a set of tables to allow it to do
bidirectional socket<=>node lookups.

But when nr_cpus is set to a smaller number than actually present, the
cpu_to_node() mapping information for unused CPUs is not available to
build_socket_tables(). This results in skipping some nodes or sockets
when creating the tables and leaving some -1's for later code to trip.
over, causing oopses.

The problem is that the socket<=>node lookups are created by doing a
loop over all CPUs, then looking up the CPU's APICID and socket. But
if a CPU is not present, there is no way to start this lookup.

Instead of looping over all CPUs, take CPUs out of the equation
entirely. Loop over all APICIDs which are mapped to a valid NUMA node.
Then just extract the socket-id from the APICID.

This avoid tripping over disabled CPUs.

Ard Biesheuvel aba7e066c7 efi/x86: Ensure that EFI_RUNTIME_MAP is enabled for kexec
CONFIG_EFI_RUNTIME_MAP needs to be enabled in order for kexec to be able
to provide the required information about the EFI runtime mappings to
the incoming kernel, regardless of whether kexec_load() or
kexec_file_load() is being used. Without this information, kexec boot in
EFI mode is not possible.

The CONFIG_EFI_RUNTIME_MAP option is currently directly configurable if
CONFIG_EXPERT is enabled, so that it can be turned on for debugging
purposes even if KEXEC is not enabled. However, the upshot of this is
that it can also be disabled even when it shouldn't.

So tweak the Kconfig declarations to avoid this situation.

Jack Wang 3d7d72a34e x86/sgx: Break up long non-preemptible delays in sgx_vepc_release()
On large enclaves we hit the softlockup warning with following call trace:

	xa_erase()
	sgx_vepc_release()
	__fput()
	task_work_run()
	do_exit()

The latency issue is similar to the one fixed in:

  8795359e35 ("x86/sgx: Silence softlockup detection when releasing large enclaves")

The test system has 64GB of enclave memory, and all is assigned to a single VM.
Release of 'vepc' takes a longer time and causes long latencies, which triggers
the softlockup warning.

Add cond_resched() to give other tasks a chance to run and reduce
latencies, which also avoids the softlockup detector.

Thomas Huth 659df86a7b x86: Remove the arch_calc_vm_prot_bits() macro from the UAPI
The arch_calc_vm_prot_bits() macro uses VM_PKEY_BIT0 etc. which are
not part of the UAPI, so the macro is completely useless for userspace.

It is also hidden behind the CONFIG_X86_INTEL_MEMORY_PROTECTION_KEYS
config switch which we shouldn't expose to userspace. Thus let's move
this macro into a new internal header instead.

Kan Liang 6f7f984fa8 perf/x86/uncore: Correct the number of CHAs on EMR
Starting from SPR, the basic uncore PMON information is retrieved from
the discovery table (resides in an MMIO space populated by BIOS). It is
called the discovery method. The existing value of the type->num_boxes
is from the discovery table.

On some SPR variants, there is a firmware bug that makes the value from the
discovery table incorrect. We use the value from the
SPR_MSR_UNC_CBO_CONFIG MSR to replace the one from the discovery table:

   38776cc45e ("perf/x86/uncore: Correct the number of CHAs on SPR")

Unfortunately, the SPR_MSR_UNC_CBO_CONFIG isn't available for the EMR
XCC (Always returns 0), but the above firmware bug doesn't impact the
EMR XCC.

Don't let the value from the MSR replace the existing value from the
discovery table.

Thomas Gleixner 3f874c9b2a x86/smp: Don't send INIT to non-present and non-booted CPUs
Vasant reported that kexec() can hang or reset the machine when it tries to
park CPUs via INIT. This happens when the kernel is using extended APIC,
but the present mask has APIC IDs >= 0x100 enumerated.

As extended APIC can only handle 8 bit of APIC ID sending INIT to APIC ID
0x100 sends INIT to APIC ID 0x0. That's the boot CPU which is special on
x86 and INIT causes the system to hang or resets the machine.

Prevent this by sending INIT only to those CPUs which have been booted
once.

Kees Cook feec5e1f74 kbuild: Show marked Kconfig fragments in "help"
Currently the Kconfig fragments in kernel/configs and arch/*/configs
that aren't used internally aren't discoverable through "make help",
which consists of hard-coded lists of config fragments. Instead, list
all the fragment targets that have a "# Help: " comment prefix so the
targets can be generated dynamically.

Add logic to the Makefile to search for and display the fragment and
comment. Add comments to fragments that are intended to be direct targets.

  protections against modification. When executing a CALL instruction,

Dave Hansen c9f4c45c8e x86/speculation: Mark all Skylake CPUs as vulnerable to GDS
The Gather Data Sampling (GDS) vulnerability is common to all Skylake
processors.  However, the "client" Skylakes* are now in this list:

which means they are no longer included for new vulnerabilities here:

or in other GDS documentation.  Thus, they were not included in the
original GDS mitigation patches.

Mark SKYLAKE and SKYLAKE_L as vulnerable to GDS to match all the
other Skylake CPUs (which include Kaby Lake).  Also group the CPUs
so that the ones that share the exact same vulnerabilities are next
to each other.

Last, move SRBDS to the end of each line.  This makes it clear at a
glance that SKYLAKE_X is unique.  Of the five Skylakes, it is the
only "server" CPU and has a different implementation from the
clients of the "special register" hardware, making it immune to SRBDS.

This makes the diff much harder to read, but the resulting table is
worth it.

I very much appreciate the report from Michael Zhivich about this
issue.  Despite what level of support a hardware vendor is providing,
the kernel very much needs an accurate and up-to-date list of
vulnerable CPUs.  More reports like this are very welcome.

Sean Christopherson 0e3223d8d0 KVM: x86/mmu: Use dummy root, backed by zero page, for !visible guest roots
When attempting to allocate a shadow root for a !visible guest root gfn,
e.g. that resides in MMIO space, load a dummy root that is backed by the
zero page instead of immediately synthesizing a triple fault shutdown
(using the zero page ensures any attempt to translate memory will generate
a !PRESENT fault and thus VM-Exit).

Unless the vCPU is racing with memslot activity, KVM will inject a page
fault due to not finding a visible slot in FNAME(walk_addr_generic), i.e.
the end result is mostly same, but critically KVM will inject a fault only
*after* KVM runs the vCPU with the bogus root.

Waiting to inject a fault until after running the vCPU fixes a bug where
KVM would bail from nested VM-Enter if L1 tried to run L2 with TDP enabled
and a !visible root.  Even though a bad root will *probably* lead to
shutdown, (a) it's not guaranteed and (b) the CPU won't read the
underlying memory until after VM-Enter succeeds.  E.g. if L1 runs L2 with
a VMX preemption timer value of '0', then architecturally the preemption
timer VM-Exit is guaranteed to occur before the CPU executes any
instruction, i.e. before the CPU needs to translate a GPA to a HPA (so
long as there are no injected events with higher priority than the
preemption timer).

If KVM manages to get to FNAME(fetch) with a dummy root, e.g. because
userspace created a memslot between installing the dummy root and handling
the page fault, simply unload the MMU to allocate a new root and retry the
instruction.  Use KVM_REQ_MMU_FREE_OBSOLETE_ROOTS to drop the root, as
invoking kvm_mmu_free_roots() while holding mmu_lock would deadlock, and
conceptually the dummy root has indeeed become obsolete.  The only
difference versus existing usage of KVM_REQ_MMU_FREE_OBSOLETE_ROOTS is
that the root has become obsolete due to memslot *creation*, not memslot
deletion or movement.

Sean Christopherson b5b359ac30 KVM: x86/mmu: Disallow guest from using !visible slots for page tables
Explicitly inject a page fault if guest attempts to use a !visible gfn
as a page table.  kvm_vcpu_gfn_to_hva_prot() will naturally handle the
case where there is no memslot, but doesn't catch the scenario where the
gfn points at a KVM-internal memslot.

Letting the guest backdoor its way into accessing KVM-internal memslots
isn't dangerous on its own, e.g. at worst the guest can crash itself, but
disallowing the behavior will simplify fixing how KVM handles !visible
guest root gfns (immediately synthesizing a triple fault when loading the
root is architecturally wrong).

Sean Christopherson 2c6d4c27b9 KVM: x86/mmu: Harden TDP MMU iteration against root w/o shadow page
Explicitly check that tdp_iter_start() is handed a valid shadow page
to harden KVM against bugs, e.g. if KVM calls into the TDP MMU with an
invalid or shadow MMU root (which would be a fatal KVM bug), the shadow
page pointer will be NULL.

Opportunistically stop the TDP MMU iteration instead of continuing on
with garbage if the incoming root is bogus.  Attempting to walk a garbage
root is more likely to caused major problems than doing nothing.

Harden kvm_mmu_new_pgd() against NULL pointer dereference bugs by sanity
checking that the target root has an associated shadow page prior to
dereferencing said shadow page.  The code in question is guaranteed to
only see roots with shadow pages as fast_pgd_switch() explicitly frees the
current root if it doesn't have a shadow page, i.e. is a PAE root, and
that in turn prevents valid roots from being cached, but that's all very
subtle.

Add a dedicated helper for converting a root hpa to a shadow page in
anticipation of using a "dummy" root to handle the scenario where KVM
needs to load a valid shadow root (from hardware's perspective), but
the guest doesn't have a visible root to shadow.  Similar to PAE roots,
the dummy root won't have an associated kvm_mmu_page and will need special
handling when finding a shadow page given a root.

Opportunistically retrieve the root shadow page in kvm_mmu_sync_roots()
*after* verifying the root is unsync (the dummy root can never be unsync).

Sean Christopherson 338068b5be KVM: x86/mmu: Drop infrastructure for multiple page-track modes
Drop "support" for multiple page-track modes, as there is no evidence
that array-based and refcounted metadata is the optimal solution for
other modes, nor is there any evidence that other use cases, e.g. for
access-tracking, will be a good fit for the page-track machinery in
general.

E.g. one potential use case of access-tracking would be to prevent guest
access to poisoned memory (from the guest's perspective).  In that case,
the number of poisoned pages is likely to be a very small percentage of
the guest memory, and there is no need to reference count the number of
access-tracking users, i.e. expanding gfn_track[] for a new mode would be
grossly inefficient.  And for poisoned memory, host userspace would also
likely want to trap accesses, e.g. to inject #MC into the guest, and that
isn't currently supported by the page-track framework.

A better alternative for that poisoned page use case is likely a
variation of the proposed per-gfn attributes overlay (linked), which
would allow efficiently tracking the sparse set of poisoned pages, and by
default would exit to userspace on access.

Sean Christopherson e998fb1a30 KVM: x86/mmu: Use page-track notifiers iff there are external users
Disable the page-track notifier code at compile time if there are no
external users, i.e. if CONFIG_KVM_EXTERNAL_WRITE_TRACKING=n.  KVM itself
now hooks emulated writes directly instead of relying on the page-track
mechanism.

Provide a stub for "struct kvm_page_track_notifier_node" so that including
headers directly from the command line, e.g. for testing include guards,
doesn't fail due to a struct having an incomplete type.

Bury the declaration of the page-track helpers that are intended only for
internal KVM use in a "private" header.  In addition to guarding against
unwanted usage of the internal-only helpers, dropping their definitions
avoids exposing other structures that should be KVM-internal, e.g. for
memslots.  This is a baby step toward making kvm_host.h a KVM-internal
header in the very distant future.

Yan Zhao b83ab124de KVM: x86: Add a new page-track hook to handle memslot deletion
Add a new page-track hook, track_remove_region(), that is called when a
memslot DELETE operation is about to be committed.  The "remove" hook
will be used by KVMGT and will effectively replace the existing
track_flush_slot() altogether now that KVM itself doesn't rely on the
"flush" hook either.

The "flush" hook is flawed as it's invoked before the memslot operation
is guaranteed to succeed, i.e. KVM might ultimately keep the existing
memslot without notifying external page track users, a.k.a. KVMGT.  In
practice, this can't currently happen on x86, but there are no guarantees
that won't change in the future, not to mention that "flush" does a very
poor job of describing what is happening.

Pass in the gfn+nr_pages instead of the slot itself so external users,
i.e. KVMGT, don't need to exposed to KVM internals (memslots).  This will
help set the stage for additional cleanups to the page-track APIs.

Opportunistically align the existing srcu_read_lock_held() usage so that
the new case doesn't stand out like a sore thumb (and not aligning the
new code makes bots unhappy).
