The ZBC standard "piggy-backs" on many, but not all, of the facilities in
SBC. Add those ZBC mode pages (plus mode parameter block descriptors
(e.g. "WP")) and VPD pages in common with SBC. Add ZBC specific VPD page
for the host-managed ZBC device type (ptype=0x14).

/sys/modules/scsi_debug/version
and can thus be used by user space programs to alter the features they try
to use. Since the per_host_store and zbc/zone options are significant
additions, bump the version number to 1.89 .

This module has a lot of parameters and when searching for one, the author
prefers them in alphabetical order. This can lead to somewhat illogical
ordering (e.g. inq_product before inq_vendor). However it is not clear what
another sensible total logical ordering would be.

Many disks implement the SCSI PRE-FETCH commands. One use case might be a
disk-to-disk compare, say between disks A and B. Then this sequence of
commands might be used: PRE-FETCH(from B, IMMED), READ(from A), VERIFY
(BYTCHK=1 on B with data returned from READ). The PRE-FETCH (which returns
quickly due to the IMMED) fetches the data from the media into B's cache
which should speed the trailing VERIFY command. The next chunk of the
compare might be done in parallel, with A and B reversed.
The implementation tries to bring the specified range in main memory into
the cache(s) associated with this machine's CPU(s) using the
prefetch_range() function.

Previously the code did the work implied by the given SCSI command and
after that it waited for a timer based on the user specified command
duration to be exhausted before informing the mid-level that the command
was complete. For short command durations, the time to complete the work
implied by the SCSI command could be significant compared to the user
specified command duration.
For example a WRITE of 128 blocks (say 512 bytes each) on a machine that
can copy from main memory to main memory at a rate of 10 GB/sec will take
around 6.4 microseconds to do that copy. If the user specified a command
duration of 5 microseconds (ndelay=5000), should the driver do a further
delay of 5 microseconds after the copy or return immediately because 6.4 >
5 ?
The action prior to this patch was to always do the timer based
delay. After this patch, for ndelay values less than 1 millisecond, this
driver will complete the command immediately. And in the case where the
user specified delay was 7 microseconds, a timer delay of 600 nanoseconds
will be set ((7 - 6.4) * 1000).

The design of this driver is to do any ramdisk access on the same thread
that invoked the queuecommand() call. That is assumed to be user space
context. The command duration is implemented by setting the delay with a
high resolution timer. The hr timer's callback may well be in interrupt
context, but it doesn't touch the ramdisk. So try removing the
_irqsave()/_irqrestore() portion on the read-write lock that protects
ramdisk access.

With the addition of the per_host_store option, the ability to check
whether two different ramdisk images are the same or not becomes
practical. Prior to this patch VERIFY(10) always returned true (i.e. the
SCSI GOOD status) without checking. This option adds support for BYTCHK
equal to 0, 1 and 3. If the comparison fails, then a sense key of
MISCOMPARE is returned as per the T10 standards. Also add support for the
VERIFY(16) command.

The scsi_debug driver has always been restricted to using one ramdisk image
(or none) for its storage. This means that thousands of scsi_debug devices
can be created without exhausting the host machine's RAM. The downside is
that all scsi_debug devices share the same ramdisk image. This option
changes the way a following write to the add_host parameter (or an add_host
in the module/driver invocation) operates. For each new host that is
created while per_host_store is true, a new store (of dev-size_mb MiB) is
created and associated with all the LUs that belong to that new host. The
user (who will need root permissions) needs to take care not to exhaust all
the machine's available RAM.
One reason for doing this is to check that (partial) disk to disk copies
based on scsi_debug devices have actually copied accurately. To test this
the add_host=<n> parameter where <n> is 2 or greater can be used when the
scsi_debug module is loaded. Let us assume that /dev/sdb and /dev/sg1 are
the same scsi_debug device, while /dev/sdc and /dev/sg2 are the same
scsi_debug device. With per_host_store=1 add_host=2 they will have
different ramdisk images. Then the following pseudocode could be executed
to check if the sgh_dd copy worked:
dd if=/dev/urandom of=/dev/sdb
sgh_dd if=/dev/sg1 of=/dev/sg2 [plus option(s) to test]
cmp /dev/sdb /dev/sdc
If the cmp fails then the copy has failed (or some other mechanism wrote to
/dev/sdb or /dev/sdc in the interim).

Add a new command line option (e.g. random=1) and sysfs attribute that
causes subsequent command completion times to be between the current
command delay setting and 0. A uniformly distributed 32 bit, kernel
provided integer is used for this purpose.
Since the existing 'delay' whose units are jiffies (typically milliseconds)
and 'ndelay' (units: nanoseconds) options (and sysfs attributes) span a
range greater than 32 bits, some scaling is required.
The purpose of this patch is to widen the range of testing cases that are
visited in long running tests. Put simply: rarely struct race conditions
are more likely to be found when this facility is used.
The default is the previous case in which all command completions were
roughly equal to (if not, slightly longer) than the value given by the
'delay' or 'ndelay' settings (or their defaults). This option's default is
equivalent to setting 'random=0' .

Smatch complains that the "path_data->handle" variable is user controlled.
It comes from iscsi_set_path() so that seems possible. It's harmless to
add a limit check.
The qedi->ep_tbl[] array has qedi->max_active_conns elements (which is
always ISCSI_MAX_SESS_PER_HBA (4096) elements). The array is allocated in
the qedi_cm_alloc_mem() function.

In case scsi_setup_fs_cmnd() fails we're not freeing the sgtables allocated
by scsi_init_io(), thus we leak the allocated memory.
Free the sgtables allocated by scsi_init_io() in case scsi_setup_fs_cmnd()
fails.
Technically scsi_setup_scsi_cmnd() does not suffer from this problem as it
can only fail if scsi_init_io() fails, so it does not have sgtables
allocated. But to maintain symmetry and as a measure of defensive
programming, free the sgtables on scsi_setup_scsi_cmnd() failure as well.
scsi_mq_free_sgtables() has safeguards against double-freeing of memory so
this is safe to do.
While we're at it, rename scsi_mq_free_sgtables() to scsi_free_sgtables().

Commit ed830385a2 ("scsi: ibmvfc: Avoid loss of all paths during SVC node
reboot") introduced a regression where when the client resets or re-enables
its CRQ with the hypervisor there is a chance that if the server side
doesn't issue its INIT handshake quick enough the client can issue an
Implicit Logout prior to doing an NPIV Login. The server treats this
scenario as a protocol violation and closes the CRQ on its end forcing the
client through a reset that gets the client host state and next host action
out of agreement leading to a BUG assert.

The write performance of TLC NAND is considerably lower than SLC NAND.
Using SLC NAND as a WriteBooster Buffer enables the write request to be
processed with lower latency and improves the overall write performance.
Adds support for shared-buffer mode WriteBooster.
WriteBooster enable: SW enables it when clocks are scaled up, thus it's
enabled only in high load conditions.
WriteBooster disable: SW will disable the feature, when clocks are scaled
down. Thus writes would go as normal writes.
To keep the endurance of the WriteBooster Buffer at a maximum, this
load-based toggling is adopted.

scsi_host_block() calls scsi_internal_device_block() for each scsi_device and
scsi_internal_device_block() calls blk_mq_quiesce_queue() for each LUN.
Since synchronize_rcu() is called from blk_mq_quiesce_queue(), this can cause
substantial slowdowns on systems with many LUNs.
Use scsi_internal_device_block_nowait() to implement scsi_host_block() so it
is sufficient to run synchronize_rcu() once. This is safe since SCSI does not
set the BLK_MQ_F_BLOCKING flag.

VENTURA_SERIES, each set of 16 reply queues (0 - 15, 16 - 31,..) need to be
within the same 4 GB boundary. Driver uses limitation of VENTURA_SERIES to
manage INVADER_SERIES as well. The driver is allocating the DMA able
memory for RDPQs accordingly.
1) At driver load, set DMA mask to 64 and allocate memory for RDPQs
2) Check if allocated resources for RDPQ are in the same 4GB range
3) If #2 is true, continue with 64 bit DMA and go to #6
4) If #2 is false, then free all the resources from #1
5) Set DMA mask to 32 and allocate RDPQs
6) Proceed with driver loading and other allocations

In NPIV environment, a NPIV host may use a queue pair created by base host
or other NPIVs, so the check for a queue pair created by this NPIV is not
correct, and can cause an abort to fail, which in turn means the NVME
command not returned. This leads to hang in nvme_fc layer in
nvme_fc_delete_association() which waits for all I/Os to be returned, which
is seen as hang in the application.

Today, upon an MPI failure AEN, on top of collecting an MPI dump, a regular
firmware dump is also taken and then chip reset. This is disruptive to IOs
and not required. Make the firmware dump collection, followed by chip
reset, optional (not done by default).
Firmware dump buffer and MPI dump buffer are independent of each
other with this change and each can have dump that was taken at two
different times for two different issues. The MPI dump is saved in a
separate buffer and is retrieved differently from firmware dump.
To collect full dump on MPI failure AEN, a module parameter is
introduced:

There are only two callers of blk_rq_map_sg/__blk_rq_map_sg that set
the dma_pad value in the queue. Move the handling into those callers
instead of burdening the common code, and move the ->extra_len field
from struct request to struct scsi_cmnd.

Don't burden the common block code with with specifics of the libata DMA
draining mechanism. Instead move most of the code to the scsi midlayer.
That also means the nr_phys_segments adjustments in the blk-mq fast path
can go away entirely, given that SCSI never looks at nr_phys_segments
after mapping the request to a scatterlist.

qlt_free_session_done() tries to post async PRLO / LOGO, and waits for the
completion of these async commands. If UNLOADING is set, this is doomed to
timeout, because the async logout command will never complete.
The only way to avoid waiting pointlessly is to fail posting these commands
in the first place if the driver is in UNLOADING state. In general,
posting any command should be avoided when the driver is UNLOADING.
With this patch, "rmmod qla2xxx" completes without noticeable delay.

The purpose of the UNLOADING flag is to avoid port login procedures to
continue when a controller is in the process of shutting down. It makes
sense to set this flag before starting session teardown.
Furthermore, use atomic test_and_set_bit() to avoid the shutdown being run
multiple times in parallel. In qla2x00_disable_board_on_pci_error(), the
test for UNLOADING is postponed until after the check for an already
disabled PCI board.

The APIs scsi_host_block()/scsi_host_unblock() were recently added by
commit 2bb955840c ("scsi: core: add scsi_host_(block,unblock) helper
function") and so far the APIs are only used by: commit 3d3ca53b16
("scsi: aacraid: use scsi_host_(block,unblock) to block I/O").
However, from reading the code, I think the APIs don't really work for
aacraid, because, in the resume path of hibernation, when aac_suspend() ->
scsi_host_block() is called, scsi_device_quiesce() has set the state to
SDEV_QUIESCE, so aac_suspend() -> scsi_host_block() returns -EINVAL.
Fix the issue by allowing the state change.

This reverts commit 7e70aa789d.
Now that we have the patches ("blk-mq: In blk_mq_dispatch_rq_list()
"no budget" is a reason to kick") and ("blk-mq: Rerun dispatching in
the case of budget contention") we should no longer need the fix in
the SCSI code. Revert it, resolving conflicts with other patches that
have touched this code.
With this revert (and the two new patches) I can run the script that
was in commit 7e70aa789d ("scsi: core: run queue if SCSI device
queue isn't ready and queue is idle") in a loop with no failure. If I
do this revert without the two new patches I can easily get a failure.

The MFW may make a call to qed and then to qedf for protocol statistics
while the function is still probing. If this happens it's possible that
some members of the struct qedf_ctx may not be fully initialized which can
result in a NULL pointer dereference or general protection fault.
To prevent this, add a new flag call QEDF_PROBING and set it when the
__qedf_probe() function is active. Then in the qedf_get_protocol_tlv_data()
function we can check if the function is still probing and return
immediantely before any uninitialized structures can be touched.

If a port is brought down for an extended period of time, the fipvlan
counter gets exhausted and the driver will fall back to default VLAN 1002
and call fcoe_ctlr_link_up to log in. However, the switch will discard the
FLOGI attempt because the VLAN is now different.
Keep track of the number of FLOGI attempts and if a threshold of
QEDF_FLOGI_RETRY_CNT is exceeded, perform a context soft reset.

During suspend, if the link is put to off, it would require a full
initialization during resume. This patch resets and restores both the host
and the card during initialization, otherwise host-only reset and restore
would fail occasionally.

While parsing the iSCSI TLV data to MFW request, a newline was added to the
firmware boot target iqnname string. Because of this, we were getting the
following error even after the boot target was successfully logged in:

The commit 6d1368e8f9 ("scsi: qedf: fixup locking in
qedf_restart_rport()") introduced the lock. Though the lock protects
only the fc_rport_create() call. Thus, we can move the mutex unlock up

This is a batch of changes that didn't make it in the initial pull
request because the lpfc series had to be rebased to redo an incorrect
split. It's basically driver updates to lpfc, target, bnx2fc and ufs
with the rest being minor updates except the sr_block_release one
which fixes a use after free introduced by the removal of the global
mutex in the first patch set.

The original patch was to resolve the lldd being able to be unloaded
while being used to talk to the boot device of the system. However, the
end result of the original patch is that any driver unload while a nvme
controller is live via the lldd is now being prohibited. Given the module
reference, the module teardown routine can't be called, thus there's no
way, other than manual actions to terminate the controllers.

When scaling gear, use ufshcd_config_pwr_mode() instead of
ufshcd_change_power_mode() so that vops_pwr_change_notify(PRE_CHANGE)
can be utilized to allow vendors use customized settings before

Generic protection fault type kernel panic is observed when user performs
soft (ordered) HBA unplug operation while IOs are running on drives
connected to HBA.
When user performs ordered HBA removal operation, the kernel calls PCI
device's .remove() call back function where driver is flushing out all the
outstanding SCSI IO commands with DID_NO_CONNECT host byte and also unmaps
sg buffers allocated for these IO commands.
However, in the ordered HBA removal case (unlike of real HBA hot removal),
HBA device is still alive and hence HBA hardware is performing the DMA
operations to those buffers on the system memory which are already unmapped
while flushing out the outstanding SCSI IO commands and this leads to
kernel panic.
Don't flush out the outstanding IOs from .remove() path in case of ordered
removal since HBA will be still alive in this case and it can complete the
outstanding IOs. Flush out the outstanding IOs only in case of 'physical
HBA hot unplug' where there won't be any communication with the HBA.
During shutdown also it is possible that HBA hardware can perform DMA
operations on those outstanding IO buffers which are completed with
DID_NO_CONNECT by the driver from .shutdown(). So same above fix is applied
in shutdown path as well.
It is safe to drop the outstanding commands when HBA is inaccessible such
as when permanent PCI failure happens, when HBA is in non-operational
state, or when someone does a real HBA hot unplug operation. Since driver
knows that HBA is inaccessible during these cases, it is safe to drop the
outstanding commands instead of waiting for SCSI error recovery to kick in
and clear these outstanding commands.

Currently ufshcd driver assumes that bInitPowerMode parameter is not
changed by any vendors thus device power mode can be set as "Active" during
initialization.
According to UFS JEDEC specification, device power mode shall be "Active"
after HW Reset is triggered if the bInitPowerMode parameter in Device
Descriptor is default value.
By above description, we can set device power mode as "Active" after device
reset is triggered by vendor's callback. With this change, the link startup
performance can be improved in some cases by not setting link_startup_again
as true in ufshcd_link_startup().

If the daemon is restarted or crashes while logging out of a session, the
unbind session event sent by the kernel is not processed and is lost. When
the daemon starts again, the session can't be unbound because the daemon is
waiting for the event message. However, the kernel has already logged out
and the event will not be resent.
When iscsid restart is complete, logout session reports error:

The default lun queue depth by the driver has been 30 for many years.
However, this value, when used with more recent hardware, has actually
throttled some tests that concentrate io on a lun.
Increase the default lun queue depth to 64.
Queue full handling, reported by the target, remains in effect and
unchanged.

Driver received a SCSI completion after it posted the cleanup request. This
leads to a problem that one ref count wasn't released leading to
flush_active_ios to get struck. The callback from libfc never returned and
other ports were not processed leading to APD.
Decrease the refcnt as well as try to complete if something is waiting for
completion.

Filesystem goes to read-only after continuous error injection because RQE
was handled in deferred context, leading to mismatch between CQE and RQE.
Specifically, this patch makes the following changes:
- Process the RQE with CQE in interrupt context, before putting it into
the work queue.
- Producer and consumer indices are also updated in the interrupt context
to guarantee the the order of processing.

Once fail happens during suspend and resume flow if the desired low power
link state is H8, link recovery is required for MediaTek UFS controller.
For resume flow, since power and clocks are already enabled before invoking
vendor's resume callback, simply using ufshcd_link_recovery() inside
callback is fine.
For suspend flow, the device power enters low power mode or is disabled
before suspend callback, thus ufshcd_link_recovery() can not be directly
used in vendor callback. One solution is to set the link to off state and
then ufshcd_host_reset_and_restore() will be executed by ufshcd_suspend().

During code review, identified dss feature that was a prototype only and
was never productized in SLI3. They shouldn't be there and prevents reuse
of the command areas.
Remove any code in the driver to deal with dss, including code to deal with
fips, which is associated with the dss feature.

The cpu io statistics were capped by a hard define limit of 128. This
effectively was a max number of CPUs, not an actual CPU count, nor actual
CPU numbers which can be even larger than both of those values. This made
stats off/misleading and on large CPU count systems, wrong.
Fix the stats so that all CPUs can have a stats struct. Fix the looping
such that it loops by hdwq, finds CPUs that used the hdwq, and sum the
stats, then display.

The AER interfaces to clear error status registers were a confusing mess:
- pci_cleanup_aer_uncorrect_error_status() cleared non-fatal errors
from the Uncorrectable Error Status register.
- pci_aer_clear_fatal_status() cleared fatal errors from the
Uncorrectable Error Status register.
- pci_cleanup_aer_error_status_regs() cleared the Root Error Status
register (for Root Ports), the Uncorrectable Error Status register,
and the Correctable Error Status register.
Rename them to make them consistent:

During target side fault injections, it is possible to hit the
NLP_WAIT_FOR_UNREG case in lpfc_nvme_remoteport_delete. A prior commit
fixed a rebind and delete race condition, but called lpfc_nlp_put
unconditionally. This triggered a deletion and the crash.
Fix by movng nlp_put to inside the NLP_WAIT_FOR_UNREG case, where the nlp
will be being unregistered/removed. Leave the reference if the flag isn't
set.

The lpfc_sli4_wq_release() routine iterates for each interim value when
updating the wq consuemr index. This wastes cycles and possibly confuses
things as thevalue itterates (and the modulo logic is being applied).
There's no reason for this. Just set it to the value from the hw.

Injecting EEH on a 32GB card is causing kernel oops
The pci error handler is doing an IO flush and the offline code is also
doing an IO flush. When the 1st flush is complete the hdwq is destroyed
(freed), yet the second flush accesses the hdwq and crashes.
Added a check in lpfc_sli4_fush_io_rings to check both the HBA_IOQ_FLUSH
flag and the hdwq pointer to see if it is already set and not already
freed.

SCSI layer sends driver IOs with more s/g segments than driver can handle.
This results in "Too many sg segments from dma_map_sg. Config 64, seg_cnt
219" error messages from the lpfc_scsi_prep_dma_buf_s3() routine.
The was due to use the driver using individual templates for pport and
vport, host reset enabled or not, nvme vs scsi, etc. In the end, there was
a combination for a vport that didn't match the pport.
Rather than enumerating more templates and more discretionary assignments,
revert to a base template that is copied to a template specific to the
pport/vport. Then, based on role, attributes and sli type, modify the
fields that are different for that port. Added a log message to
lpfc_create_port to validate values.

In lpfc_nvmet_prep_fcp_wqe() the line "rsp->sg_cnt = 0" is modifying the
transport's data structure. This may result in the transport believing the
s/g list was already freed, thus may not unmap/free it properly. Lpfc
driver should not modify the transport data structure.
The zeroing of the sg_cnt is to avoid use of the transport's sgl in a
subsequent loop where the driver builds the necessary requests for the
adapter firmware to complete the IO.
Change LLDD to use a local copy of the transport sg_cnt when building
requests to be passed to the adapter fw.

Unloading the driver resulted in a call to del_timer_sync for the
cpuhp_poll_timer. However the call to setup the timer had never been made,
so the timer structures used by lockdep checking were not initialized.
Unconditionally call setup_timer for the cpuhp_poll_timer during driver

When processing the completion of a "Reg Rpi" login mailbox command in
lpfc_sli_def_mbox_cmpl, a call may be made to lpfc_unreg_login. The vpi is
extracted from the completing mailbox context and passed as an input for
the next. However, the vpi stored in the mailbox command context is an
absolute vpi, which for SLI4 represents both base + offset. When used with
a non-zero base component, (function id > 0) this results in an
out-of-range access beyond the allocated phba->vpi_ids array.
Fix by subtracting the function's base value to get an accurate vpi number.

We were setting bActiveICCLevel attribute for UFS device only once but the
type of this attribute has changed from persistent to volatile since UFS
device specification v2.1. This attribute is set to the default value after
power cycle or hardware reset event. It isn't safe to rely on prefetched
data (only used for bActiveICCLevel attribute now). Hence this change
removes the code related to data prefetching and set this parameter on
every attempt to probe the UFS device.

Correct race condition where ioaccel is re-enabled before the raid_map is
updated. For RAID_1, RAID_1ADM, and RAID 5/6 there is a BUG_ON called which
is bad.
- Change event thread to disable ioaccel only. Send all requests down the
RAID path instead.
- Have rescan thread handle offload_enable.
- Since there is only one rescan allowed at a time, turning
offload_enabled on/off should not be racy. Each handler queues up a
rescan if one is already in progress.
- For timing diagram, offload_enabled is initially off due to a change
(transformation: splitmirror/remirror), ...

Block layer RPM is enabled for the genernal UFS SCSI devices when they are
probed by their driver. However block layer RPM is not enabled for UFS
well-known SCSI devices.
As UFS SCSI devices have their corresponding BSG char devices, accessing a
BSG char device via IOCTL may send requests to its corresponding SCSI
device through its request queue. If BSG IOCTL sends a request to a
well-known SCSI device when HBA is not runtime active, due to block layer
RPM not being enabled for the well-known SCSI devices, the HBA, which is at
the top of a SCSI device's parent chain, will not be resumed.
This change enables block layer RPM for the well-known SCSI devices so that
block layer can handle RPM for the well-known SCSI devices just like for
the general SCSI devices.

Currently, the frequency that devfreq provides the driver always leads the
clocks to be scaled up. Hence, round the clock-rate to the nearest
frequency before deciding to scale.

As a part of sysfs reading of descriptors/attributes/flags, query commands
should only be executed when hba's power runtime status is active. To
guarantee this, add pm_runtime_get/put_sync() to those paths where query
commands are sent.

MediaTek platform and UFS controller can dynamically customize the delay
for host enabling according to different scenarios.
For example, if UniPro enters lower-power mode, such delay can be
minimized, otherwise longer delay shall be expected.

Reduce the waiting period between each HCE (Host Controller Enable) polling
from 5 ms to 1 ms. Also increase the maximum polling times to make "total
polling time" roughly the same.
This change could make HCE initialization faster to improve latency of
ufshcd initialization, error recovery, and resume behaviors.

Currently a 1 ms delay is applied before polling CONTROLLER_ENABLE
bit. This delay may not be required or can be changed in different
controllers. Make the delay as a changeable value in struct ufs_hba to
allow it customized by vendors.

In ufshcd_disable_tx_lcc(), if ufshcd_dme_get() or ufshcd_dme_peer_get()
get fail, uninitialized variable "tx_lanes" may be used as unexpected lane
ID for DME configuration.

If an iSCSI connection happens to fail while the daemon isn't running (due
to a crash or for another reason), the kernel failure report is not
received. When the daemon restarts, there is insufficient kernel state in
sysfs for it to know that this happened. open-iscsi tries to reopen every
connection, but on different initiators, we'd like to know which
connections have failed.
There is session->state, but that has a different lifetime than an iSCSI
connection, so it doesn't directly reflect the connection state.

If 'dma_map_single()' fails, the ref counted 'shpnt' will be decremented
twice because 'scsi_host_put()' is called in the if block, and in the error
handling path.

There is no reason to expose SATA_PMP config option when no SATA
host drivers are enabled. To fix it add SATA_HOST config option,
make all SATA host drivers select it and finally make SATA_PMP
config options depend on it.
This also serves as preparation for the future changes which
optimize libata core code size on PATA only setups.

Some USB bridge devices will return a default set of characteristics during
initialization. And then, once an attached drive has spun up, substitute
the actual parameters reported by the drive. According to the SCSI spec,
the device should return a UNIT ATTENTION in case any reported parameters
change. But in this case the change is made silently after a small window
where default values are reported.
Commit a83da8a450 ("scsi: sd: Optimal I/O size should be a multiple of
physical block size") validated the reported optimal I/O size against the
physical block size to overcome problems with devices reporting nonsensical
transfer sizes. However, this validation did not account for the fact that
aforementioned devices will return default values during a brief window
during spin-up. The subsequent change in reported characteristics would
invalidate the checking that had previously been performed.
Unset a previously configured optimal I/O size should the sanity checking
fail on subsequent revalidate attempts.

struct partition is the on-disk format of a MSDOS partition table entry.
Move it out of genhd.h into a new msdos_partition.h header and give it
a msdos_ prefix to avoid confusion.
Also move the magic number from block/partitions/msdos.h to the new
header so that it can be used by the SCSI drivers looking at the DOS
partition tables.

Call scsi_bios_ptable from scsi_partsize instead of requiring boilerplate
code in the callers. Also switch the calling convention to match that
of the ->bios_param instances calling this function, and use true/false
for the return value instead of the weird -1 convention.

Added the sysfs attribute for non fatal log so that management utility can
get the non fatal dump from driver. The non-fatal error is an error
condition or abnormal behavior detected by the host, or detected and
reported by the controller to the host.The non-fatal error does not stop
the controller firmware and enables it to still respond to host requests.
A typical example of a non-fatal error is an I/O timeout or an unusual
error notification from the controller. Since the firmware is operational,
the error dump information is pushed to host memory (by firmware) upon
request from the host.

1) Move the instance tracking down after we think the instance is good to
go. Avoids having a use-after free.
2) There are goto targets for trying to cleanup if the hw fails to
initialize, but there's some overlap depending on who thinks they own
the sub-structures.

In pm80xx driver, the command mpi_set_phy_profile_req is sent by host
during boot to configure the phy profile such as analog setting page, rate
control page. However, the tag is not freed when its response is
received. As a result, 16 tags are missing for each HBA after boot. When
NCQ is enabled with queue depth 16, it needs at least, 15 * 16 = 240 tags
for each HBA to achieve the best performance. In current pm80xx driver with
setting CCB_MAX = 256, the total number of tags in each HBA is 255 for data
IO. Hence, without returning those tags to the pool after boot, some device
will finally be forced to non-ncq mode by ATA layer due to excessive errors
(i.e. LLDD cannot allocate tag for queued task).

A kexec reboot causes the controller fw to assert. This assertion shows up
in two ways, the controller doesn't show up as ready and an interrupt is
waiting as soon as the handler is registered. To resolve this added below
fix:
- Split the interrupt handling setup into two parts, setup and request.
- If the controller ready register indicates not-ready, but that the not
readiness is only on the IOC units we can still try a reset to bring the
system back to the pre-reboot state.

Increasing the per-request size maximum (max_sectors_kb) runs into the
per-device DMA scatter gather list limit (max_segments) for users of the io
vector system calls (eg, readv and writev). This is because the kernel
combines io vectors into DMA segments when possible, but it doesn't work
for our user because the vectors in the buffer cache get scrambled. This
change bumps the advertised max scatter gather length to 528 to cover 2M w/
x86's 4k pages and some extra for the user checksum. It trims the size of
some of the tables we don't care about and exposes all of the command slots
upstream to the SCSI layer. Also reduced the PM8001_MAX_CCB to 256 as
pm8001 driver has memory limit depend on machine capability. If we increase
the sg length, we need to trade-off it by decreasing PM8001_MAX_CCB.
PM8001_MAX_CCB = 256 does not have any influence on normal use

I/Os could be passed down while the device FC SCSI device is being deleted.
This would result in unnecessary delay of I/O and driver messages (when
extended logging is set).
