Currently the adapter firmware does not save outstanding I/O's log
information when an IOP reset is triggered. This is problematic when
trying to root cause and debug issues.
Fixed by adding sync command to trigger I/O log file save in the adapter
firmware before issuing an IOP reset.

The driver currently checks the SELF_TEST_FAILED first and then
KERNEL_PANIC next. Under error conditions(boot code failure) both
SELF_TEST_FAILED and KERNEL_PANIC can be set at the same time.
The driver has the capability to reset the controller on an KERNEL_PANIC,
but not on SELF_TEST_FAILED.
Fixed by first checking KERNEL_PANIC and then the others.

When the SMART family of controller panic (KERNEL_PANIC) , they do not
honor IOP resets. So better to skip it and directly perform a IWBR reset.

Currently driver checks the health status of the adapter once every 24
hours. When that happens the driver becomes dependent on the kernel to
figure out if the adapter is misbehaving. This might take some time
(when the adapter is idle). The driver currently has support to
restart/recover the controller when it fails, and decreasing the time
interval will help.
Fixed by decreasing check interval from 24 hours to 1 minute

During the IOP reset stress testing, it was found that the drives can be
marked offline when the adapter controller crashes and IO's are running
in parallel. When the controller does come back from the reset, the drive
that is marked offline is not exposed.
Fixed by removing and adding drives that are marked offline. In addition
invoke a scsi host bus rescan to capture any additional configuration
changes.

aac_command_thread checks on the health of controller periodically,
using aac_check_health. If the status is an error state KERNEL_PANIC or
anything else. The driver will attempt to restart the adapter, but the
response is not checked in aac_command_thread. This allows the periodic
sync to go thru and lead the driver to a hung state.
Fixed by terminating the periodic loop(intended per original design),
if the controller is not restored to a healthy state.

After controller shutdown, all sync fibs time out due to not knowing
about the switch to INT-x mode
Fixed by replacing aac_src_access_devreg() to aac_set_intx_mode() call.

aac_fib_map_free frees misaligned fib dma memory, additionally it does not
free up the whole memory.
Fixed by changing the code to free up the correct and full memory
allocation.

Arrconf management utility at times sends fibs with AdapterProcessed set
in its fibs. This causes the controller to panic and lockup.
Fixed by failing the commands that have AdapterProcessed set in its flag.

This issue showed up on a kdump debug(single CPU on powerkvm), when EEH
errors rendered the adapter unusable. The driver correctly detected the
issue and attempted to restart the controller, in doing so the driver
attempted to read the status registers of the controller. This triggered
additional eeh errors which continued for a good 6 minutes.
Fixed by returning without waiting when EEH error is reported.

The channel being used for raw srb commands is retrieved from the utility
sent fibs and is converted into physical channel id. The driver does not
need to to do this since the management utility sends the correct channel
id in the first place and in addition the driver sets inaccurate
information in the cmd sent to the firmware and gets an invalid response.
Fixed by using channel id from srb command.

aac_fib_send can return -ve error returns and hence rcode should be
signed. Currently the rcode >= 0 check is always true and -ve errors are
not being checked.

Moved the READ and WRITE switch cases to the top. Added a default
case to the switch case and replaced duplicate scsi result value with a
macro.
The idea is that since most of scsi commands we care about performance
wise are read or write, we need to process them first.
Internally the compiler (GCC) converts a switch case into either a jump
table or a bunch of if else conditions, so placing the often used read,
write cases at the top is an effort in optimization.

In aacraid's ioctl_send_fib() we do two fetches from userspace, one the
get the fib header's size and one for the fib itself. Later we use the
size field from the second fetch to further process the fib. If for some
reason the size from the second fetch is different than from the first
fix, we may encounter an out-of- bounds access in aac_fib_send(). We
also check the sender size to insure it is not out of bounds. This was
reported in https://bugzilla.kernel.org/show_bug.cgi?id=116751 and was
assigned CVE-2016-6480.

Only SRC-based adapters support the AifReqEvent function, so there is no
point in trying to activate it on older, non-SRC based adapters. Doing
so lead to crashes on older adapters.

When KDUMP is triggered the driver first talks to the firmware in INTX
mode, but the adapter firmware is still in MSIX mode. Therefore the first
driver command hangs since the driver is waiting for an INTX response and
firmware gives a MSIX response. If when the OS is installed on a RAID
drive created by the adapter KDUMP will hang since the driver does not
receive a response in sync mode.
Fixed by: Change the firmware to INTX mode if it is in MSIX mode before
sending the first sync command.

Currently driver completes double completed or spurious interrupted fibs.
This is not necessary and causes the SCSI mid layer to issue aborts and
resets, since completing a fib prematurely might trigger a race condition
resulting in the driver not calling the scsi_done callback.
Fixed by removing the call to fib complete.

Typically under error conditions, it is possible for aac_command_thread()
to miss the wakeup from kthread_stop() and go back to sleep, causing it
to hang aac_shutdown.
In the observed scenario, the adapter is not functioning correctly and so
aac_fib_send() never completes (or time-outs depending on how it was
called). Shortly after aac_command_thread() starts it performs
aac_fib_send(SendHostTime) which hangs. When aac_probe_one
/aac_get_adapter_info send time outs, kthread_stop is called which breaks
the command thread out of it's hang.
The code will still go back to sleep in schedule_timeout() without
checking kthread_should_stop() so it causes aac_probe_one to hang until
the schedule_timeout() which is 30 minutes.
Fixed by: Adding another kthread_should_stop() before schedule_timeout()

aac_fib_send has a special function case for initial commands during
driver initialization using wait < 0(pseudo sync mode). In this case,
the command does not sleep but rather spins checking for timeout.This
loop is calls cpu_relax() in an attempt to allow other processes/threads
to use the CPU, but this function does not relinquish the CPU and so the
command will hog the processor. This was observed in a KDUMP
"crashkernel" and that prevented the "command thread" (which is
responsible for completing the command from being timed out) from
starting because it could not get the CPU.
Fixed by replacing "cpu_relax()" call with "schedule()"

During EEH PCI hotplug activity kernel unloads and loads the driver,
causing character device to be unregistered(aac_remove_one).When the
driver is loaded back using aac_probe_one the character device needs
to be registered again for the AIF management tools to work.
Fixed by adding code to register character device in aac_probe_one if
it is unregistered in aac_remove_one.

while driver removal is in progress or PCI shutdown is invoked, driver
kills AIF aacraid thread, but IOCTL requests from the management tools
re-start AIF thread leading to IOP_RESET.
Fixed by setting adapter_shutdown flag when PCI shutdown is invoked.

aac_mutex was used to create protect the ioctl path for only the compat
path, it would be make more sense to place mutex in aac_do_ioctl, which
is the main ioctl function call that handles all ioctl commands.
Created new mutex ioctl_mutex in struct aac_dev to protect switch case
in aac_do_ioctl and removed aac_mutex from aac_cfg_ioctl and
aac_compat_do_ioctl

During EEH recovery number of online CPU's might change thereby changing
the number of MSIx vectors. Since each fib is allocated to a vector,
changes in the number of vectors causes fib to be sent thru invalid
vectors.In addition the correct number of MSIx vectors is not updated in
the INIT struct sent to the controller, when it is reinitialized.
Fixed by reassigning vectors to fibs based on the updated number of MSIx
vectors and updating the INIT structure before sending to controller.

aac_fib_map_free() calls pci_free_consistent() without checking that
dev->hw_fib_va is not NULL and dev->max_fib_size is not zero.If they are
indeed NULL/0, this will result in a hang as pci_free_consistent() will
attempt to invalidate cache for the entire 64-bit address space
(which would take a very long time).
Fixed by adding a check to make sure that dev->hw_fib_va and
dev->max_fib_size are not NULL and 0 respectively.

The driver utilizes an array of atomic variables to keep track of IO
submissions to each vector. To submit an IO multiple threads iterate
through the array to find a vector which has empty slots to send an
IO. The reading and updating of the variable is not atomic, causing race
conditions when a thread uses a full vector to submit an IO.
Fixed by mapping each FIB to a vector, the submission path then uses
said vector to submit IO thereby removing the possibly of a race
condition.The vector assignment is started from 1 since vector 0 is
reserved for the use of AIF management FIBS.If the number of MSIx
vectors is 1 (MSI or INTx mode) then all the fibs are allocated to
vector 0.

The method to allocate and free FIB's in the present code utilizes
spinlocks. Multiple IO's have to wait on the spinlock to acquire or free
fibs creating a performance bottleneck.
An alternative solution would be to use block layer tags to keep track
of the fibs allocated and freed. To this end aac_fib_alloc_tag was
created to utilize the blk layer tags to plug into the Fib pool.These
functions are used exclusively in the IO path. 8 fibs are reserved for
the use of AIF management software and utilize the previous spinlock
based implementations.

After getting the platform shutdown command "VM_CloseAll" response from the
firmware, driver was getting configuration IOCTL request from the upper layers
and it sends down to firmware. This causes firmware assert issue.
This patch fixes the firmware assert issue. During the shutdown, if driver
gets commands from the upper layer, driver sends error code to the upper
layers.

This patch fixes the IOP_RESET issue. Sending IOP_RESET command need to wait
for only 10 sec instead of 5 minutes in case of firmware does not response
IOP_RESET command. Disable interrupt before setup interrupt routine to
prevent spurious interrupts.

The Linux aacriad driver fails to detect the case of SG list count=0 on IOCTL
pass-through command and cause intermittent fault. The result is the Linux
aacriad driver send down IOCTL pass-through command with one not initialized
SG list to firmware when receiving SG list count =0 on pass-through command.

Remove the tagged argument from scsi_adjust_queue_depth, and just let it
handle the queue depth. For most drivers those two are fairly separate,
given that most modern drivers don't care about the SCSI "tagged" status
of a command at all, and many old drivers allow queuing of multiple
untagged commands in the driver.
Instead we start out with the ->simple_tags flag set before calling
->slave_configure, which is how all drivers actually looking at
->simple_tags except for one worke anyway. The one other case looks
broken, but I've kept the behavior as-is for now.
Except for that we only change ->simple_tags from the ->change_queue_type,
and when rejecting a tag message in a single driver, so keeping this
churn out of scsi_adjust_queue_depth is a clear win.
Now that the usage of scsi_adjust_queue_depth is more obvious we can
also remove all the trivial instances in ->slave_alloc or ->slave_configure
that just set it to the cmd_per_lun default.

Add a use_cmd_list flag in struct Scsi_Host to request keeping track of
all outstanding commands per device.
Default behaviour is not to keep track of cmd_list per sdev, as this may
introduce lock contention. (overhead is more on multi-node NUMA.), and
only enable it on the two drivers that need it.

The SCSI standard defines 64-bit values for LUNs, and large arrays
employing large or hierarchical LUN numbers become more and more
common.
So update the linux SCSI stack to use 64-bit LUN numbers.

This patch fixes kernel panic issue while booting into the kdump kernel.

Some host adapters do not pass commands through to the target disk
directly. Instead they provide an emulated target which may or may not
accurately report its capabilities. In some cases the physical device
characteristics are reported even when the host adapter is processing
commands on the device's behalf. This can lead to adapter firmware hangs
or excessive I/O errors.
This patch disables WRITE SAME for devices connected to host adapters
that provide an emulated target. Driver writers can disable WRITE SAME
by setting the no_write_same flag in the host adapter template.

It appears that driver runs into a problem here if fibsize is too small
because we allocate user_srbcmd with fibsize size only but later we
access it until user_srbcmd->sg.count to copy it over to srbcmd.
It is not correct to test (fibsize < sizeof(*user_srbcmd)) because this
structure already includes one sg element and this is not needed for
commands without data. So, we would recommend to add the following
(instead of test for fibsize == 0).

megaraid_sas, bfa, ipr) and a few bug fixes. Also of note is that the
Buslogic driver has been rewritten to a better coding style and 64 bit support
added. We also removed the libsas limitation on 16 bytes for the command size
(currently no drivers make use of this).

One of the customer had reported that the set of raid logical arrays will
become unavailable (I/O offline) after a long hours of IO stress test. The OS
wouldn`t be accessible afterwards and require a hard reset.
This driver patch has a fix for race condition between the doorbell and the
circular buffer. The driver is modified to do an extra read after clearing the
doorbell in case there had been a completion posted during the small timing
window.
With this fix, we ran IO stress for ~13 days. There were no IO failures.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

This may not fix all endian issues in this driver, but it does get the
driver working on PowerPC for a PMC SRC card. So it should at least fix

The loop that waited for syncronous fib commands was causing a CPU stall
when a timeout actually occured.
1) Switch to using a more accurate timeout mechanism.
2) Do not pace the loop with udelay(). Use cpu_relax() to allow for
scheduling to occur.

The patch 116046127d "[SCSI] aacraid: Added
Sync.mode to support series 7/8/9 controllers" removed an iounmap call from
aac_src_ioremap. Before that, the iounmap has been called twice with the same
value (dev->base and dev->regs.src.bar0) and the iounmap complained about it
(iounmap: bad address ...).
The proper solution is a change the paremeter from bar0 to bar1.
Fix this by adding a an iounmap(dev->regs.src.bar1) call.

Aacraid controller can hang on some nodes if kernel uses non-default
(powersave) ASPM policy. Controller hangs shortly after successful load and
hardware detection. Scsi error handler detects this hang and tries to restart
hardware but it does not help.
Initially it was noticed on RHEL6-based openVZ kernel after backporting
aacraid driver from mainline (RHEL6 kernel with original driver works well)

However seems this patch was not submitted to mainline. I've reproduced this
issue on vanilla 3.1.0 kernel booted with "pcie_aspm.policy=powersave" option,

This is just a cleanup, to silence static checker warnings. It
doesn't change how the code works.
buf[] can either be BUF_SIZE if this is called from sysfs, or it can
be 16 if it's called from aac_get_adapter_info() via
aac_get_serial_number(). We use the smaller limit here.
sizeof(dev->supplement_adapter_info.MfgPcbaSerialNo) is 12 so there
is actually no chance of hitting either limit.

Move the mid-layer's ->queuecommand() invocation from being locked
with the host lock to being unlocked to facilitate speeding up the
critical path for drivers who don't need this lock taken anyway.
The patch below presents a simple SCSI host lock push-down as an
equivalent transformation. No locking or other behavior should change
with this patch. All existing bugs and locking orders are preserved.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.
Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.
None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.
Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.

The problem reported by one of the customer was when a logical array
is deleted(from the SDK, from the GUI, from arcconf) then the
corresponding physical device (/dev/sdb, for example) is not removed
from the Linux namespace. So you end up with a "dead" device
entry. And some of the linux tools go slightly wonky.
Solution:
---------
Based on the notification from FW, the driver calls
"scsi_remove_device" for the DELETED drive. This call not only informs
the scsi device status to the SCSI mid layer and also it will remove
corresponding scsi device entries from the Linux sysfs.

Problem description:
--------------------
The issue reported by one of the customer was able to read LBA beyond
the array reported size with "sg_read" utility. If N is the last block
address reported, then should not be able to read past N,
i.e. N+1. But in their case, reported last LBA=143134719. So should
not have been able to read with LBA=143134720, but it is read without
failure, which means reported size to the OS is not correct and is
less than the actual last block address.
Solution:
---------
Firmware layer exposes lesser container capacity than the actual
one. It exposes [Actual size - Spitfire space(10MB)] to the OS, IO's
to the 10MB should be prohibited from the Linux driver. Driver checks
LBA boundary, if its greater than the array reported size then sets
sensekey to HARDWARE_ERROR and sends the notification to the MID
layer.

There are two conditions for ATA pass thru command that falls into
'SRB_STATUS_ERROR' condition.
1. When the "CC" bit is set by the host in ATA pass-through CDB
- Even for the successful completion, SCSI target shall generate
check condition.
- Driver returns a result code of SAM_STAT_CHECK_CONDITION, with a
driver byte of DID_OK to the mid layer.
Below is the snippet of existing code which fills a result code
of SAM_STAT_CHECK_CONDITION:

The default driver setting is "expose_physicals=0", which means raw
physical drives are not exposed to OS. If the user wants to expose
connected physical drives, enable "expose_physicals" module parameter.
With the new JBOD firmware, physical drives are not available for
"expose_physicals>0". In function "aac_expose_phy_device", modified
to reset the appropriate bit in the first byte of inquiry data. This
fix exposes the connected physical drives.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

These particular problems were reported by Cisco and SAP and customers
as well. Cisco reported on RHEL4 U6 and SAP reported on SLES9 SP4 and
SLES10 SP2. We added these fixes on RHEL4 U6 and gave a private build
to IBM and Cisco. Cisco and IBM tested it for more than 15 days and
they reported that they did not see the issue so far. Before the fix,
Cisco used to see the issue within 5 days. We generated a patch for
SLES9 SP4 and SLES10 SP2 and submitted to Novell. Novell applied the
patch and gave a test build to SAP. SAP tested and reported that the
build is working properly.
We also tested in our lab using the tools "dishogsync", which is IO
stress tool and the tool was provided by Cisco.
Issue1: File System going into read-only mode
Root cause: The driver tends to not free the memory (FIB) when the
management request exits prematurely. The accumulation of such
un-freed memory causes the driver to fail to allocate anymore memory
(FIB) and hence return 0x70000 value to the upper layer, which puts
the file system into read only mode.
Fix details: The fix makes sure to free the memory (FIB) even if the
request exits prematurely hence ensuring the driver wouldn't run out
of memory (FIBs).
Issue2: False Raid Alert occurs
When the Physical Drives and Logical drives are reported as deleted or
added, even though there is no change done on the system
Root cause: Driver IOCTLs is signaled with EINTR while waiting on
response from the lower layers. Returning "EINTR" will never initiate
internal retry.
Fix details: The issue was fixed by replacing "EINTR" with
"ERESTARTSYS" for mid-layer retries.

This patch modifies scsi_host_template->change_queue_depth so that
it takes an argument indicating why it is being called. This will be
used so that if a LLD needs to do some extra processing when
handling queue fulls or later ramp ups, it can do so.
This is a simple port of the drivers setting a change_queue_depth
callback. In the patch I just have these LLDs adjust the queue depth
if the user was requesting it.

We need to check the address that pci_alloc_consistent() returns since
it might fail.
When pci_alloc_consistent() fails, some IOMMUs set the dma_handle
argument to zero. So we can't use fibptr->hw_fib_pa directly here.

Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.
Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

kernel BUG at arch/i386/lib/usercopy.c:872
under a 2.6.18-8.el5 kernel. Traced it to a garbage-in/garbage-out
ioctl condition in the aacraid driver.
Adaptec's special ioctl scb passthrough needs to check the validity of
the individual scatter gather count fields to the maximum the adapter
supports. Doing so will have the side effect of preventing
copy_from_user() from bugging out while populating the dma buffers.
This is a hardening effort, issue was triggered by an errant version
of the management tools and thus the BUG should not be seen in the
field.

For firmware that supports the feature(s), add the ability to start or
stop an array using the associated SCSI commands, to automatically
manage the spin-up of an array on new I/O reporting back the
appropriate check conditions and actions in cooperation with the
normal timeout mechanisms and enable the blackout period management in
the Firmware associated with the background spin-down of the arrays
when the Firmware times out and deems the arrays as idle.

As JBOD devices (really just Simple Single Drive Volumes exported to
the SCSI channel) are managed, they fail to update correctly when the
driver triggers a SCSI scan. In addition, the ability to change
multiple arrays or JBODs at the same time was resulting in dropped
scans, set up a mechanism to issue a list of single target scans on a
single configuration change notification from the Firmware.
Performed some additional sundry cosmetic code style cleanups.

None of these files use any of the functionality promised by
asm/semaphore.h. It's possible that they rely on it dragging in some
unrelated header file, but I can't build all these files, so we'll have
fix any build failures as they come up.

When aacraid spoofs READ_CAPACITY_16, it assumes that the data length
in the sg list is equal to allocation length in cdb. But sg can put
any value in scb so the driver needs to check both the data length in
the sg list and allocation length in cdb.
If allocation length is larger than the response length that the
driver expects, it clears the data buffer in the sg list to zero but
it doesn't need to do. Just setting resid is fine.

Some sysfs problems reported. The serial number on late model
controllers was truncated. Non-DASD devices (tapes and CDROMs) were
showing up as JBOD in the level report on the physical channel.

Added support for MSI utilizing the aacraid.msi=1 parameter. This
patch adds some localized or like-minded janitor fixes. Since the
default is disabled, there is no impact on the code paths unless the
customer wishes to experiment with the MSI performance.

Point taken regarding the aacraid driver. Dropped the VALID bit, and
then did some cleanup/simplification of the set_sense procedure and
the associated parameters. Mike did some preliminary tests when the
VALID bit was dropped before the 'Re: [PATCH] [SCSI] sd: make error
handling more robust' patches came on the scene. The change in the
SCSI subsystem does make this enclosed aacraid patch unnecessary, so
this aacraid patch is merely post battle ground cleanup. If the
simplification is an issue, repugnant, too much for a back-port to the
stable trees or clouds the point, this patch could be happily
distilled down to:

With the sg table code, every SCSI driver is now either chain capable
or broken (or has sg_tablesize set so chaining is never activated), so
there's no need to have a check in the host template.
Also tidy up the code by moving the scatterlist size defines into the
SCSI includes and permit the last entry of the scatterlist pools not
to be a power of two.

Alan noticed the lack of locking surrounding the driver's dealings
with the fib context managed by the trio of ioctls that are used by
the RAID management applications to retrieve Adapter Initiated FIBs. I
merely expanded the fib lock to include the fib context. There have
been no field reports of any issues generally because the applications
are relatively static and do not come and go often enough to stress
this area. I bloated this patch a little with some space junk.

The cards being added are supported in a limited sense already through
family matching, but we needed to add some functionality to the driver
to expose selectively the physical drives. These Physical drives are
specifically marked to not be part of any array and thus are declared
JBODs (Just a Bunch Of Drives) for generic SCSI access.
We report that this is the second patch in a set of two, but merely
depends on the stand-alone functionality of the first patch which adds
in that case the ability to report a driver feature flag via sysfs. We
leverage that functionality by reporting that this driver now supports
this new JBOD feature for the controller so that the array management
applications may react accordingly and guide the user as they manage
the controller.

Feature enhancement, adding a 'flags' entry that will reside in the
host controller's tree, with a newline separated list of arbitrary
ascii named features that indicate whether the combination of driver
and controller has support for said feature. Breaking from the
one-line output typical of sysfs entries, newline was added to tailor
for grep, or simple gets line by line string match within an
application. I added one for a compiler time check for existence of
debug print output, one for an optional manifest defined enhanced
status reporting in the logs, and one for runtime reporting whether
the controller and driver supports arrays larger than 2TB. Adaptec's
storage management software uses the last flag to determine whether to
make available the creation of arrays larger than 2TB, otherwise a
warning is posted.

I was amazed at how much embedded space was present in the aacraid
driver source files. Just selected five files from the set to clean up
for now and the attached patch swelled to 73K in size!

Actually there are several but one is trivially fixed
1. FSACTL_GET_NEXT_ADAPTER_FIB ioctl does not lock dev->fib_list
but needs to
2. Ditto for FSACTL_CLOSE_GET_ADAPTER_FIB
3. It is possible to construct an attack via the SRB ioctls where
the user obtains assorted elevated privileges. Various approaches are
possible, the trivial ones being things like writing to the raw media
via scsi commands and the swap image of other executing programs with
higher privileges.
So the ioctls should be CAP_SYS_RAWIO - at least all the FIB manipulating
ones. This is a bandaid fix for #3 but probably the ioctls should grow
their own capable checks. The other two bugs need someone competent in that
driver to fix them.

Due to an internal limit associated with the AdapterTypeText field,
SMC required a product ID that overloaded the combined vendor and
product ID. A decision was made to ship the SMC products without a
vendor string dropping the defacto space that used to delineate vendor
and product to boot. To correct this, we needed to adjust the code in
the driver to parse out the vendor and product strings for the
adapter. We match of 'AOC' in the AdapterTypeText, if so we set the
vendor to SMC and place the entire AdapterTypeText into the product
field.
This only affects the cosmetic presentation of the Adapter vendor and
product in the logs and in sysfs.

Added support to respond to enclosure service events
(controller AIFs) to add, online or offline physical targets
reported to sg. Also added online and offlining of arrays.
Removed an automatic variable definition in a sub block that
hid an earlier definition, determined to be inert as the
sub-block use did not interfere. Bumped the driver versioning
to stamp the addition of this feature.

The adapter queue is divided up equally to all the arrays to prevent
command starvation to any individual array. On the other hand,
physical targets are only granted a queue depth of one each. The code
prior to this patch used to deal with the incremental discovery of
targets, but the driver knows how many arrays are present prior to the
scan so this knowledge is used to generate a better estimate for the
queue depth.
Remove the capability of 'physical=0' from preventing access to the
class of adapters that have the RAID/SCSI mode of operation since none
of the physicals on the SCSI channel are candidates ever for an array.
As always, the user can override this default queue depth policy by
making the appropriate adjustments utilizing sysfs.

In experiments in the lab we managed to trigger an Adapter firmware
panic (BlinkLED) coincidentally while several pass-through ioctl
command from the management software were outstanding on a bug only
present on a class of RAID Adapters that require a hardware reset
rather than a commanded reset. The net result was an attempt to time
out the management software command as if it came from the SCSI layer
resulting in an OS panic.
Adapters that use commanded reset, management commands are returned
failed by the Adapter correctly. The adapter firmware panic that
resulted in this condition was also resolved, and there were no
adapters in the field with this specific firmware bug so we do not
expect any field reports. This is a rare or unlikely corner condition,
and no reports have ever been forwarded from the field.

Big endian systems issues discovered in the aacraid driver. Somewhat
reverses a patch from November 7th of last year that removed swap
operations because they formerly were being assigned to an u8 array
when they should have been assigned to an le32 array.
This patch is largely inert for any little endian processor
architecture. It resolves a bug in delivering the BlinkLED AIF event
to registered applications when the adapter or associated hardware was
reset due to ill health. A rare corner case occurrence, also largely
unnoticed by any as it was a new (untested!) feature.

aacraid.cache parameter, Disable Queue Flush commands:
bit 0 - Disable FUA in WRITE SCSI commands
bit 1 - Disable SYNCHRONIZE_CACHE SCSI command
bit 2 - Disable only if Battery not protecting adapter supplied Cache
e.g.: aacraid.cache=7 will disable the FUA and SYNCHRONIZE_CACHE
commands if the adapter has reported that it's cache is battery backed
up.
This parameter permits experimentation with tradeoffs between
performance and caching policy.

As reported in http://bugzilla.kernel.org/show_bug.cgi?id=3D9133 it was
discovered that the PERC line of controllers lacked a key 64 bit
ScatterGather capable SCSI pass-through function. The adapters are still
capable of 64 bit ScatterGather I/O commands, but these two can not be
mixed. This problem was exacerbated by the introduction of the SCSI
Generic access to the DASD physical devices.
The fix for users before this patch is applied is aacraid.dacmode=3D0 on
the kernel command line to disable 64 bit I/O.
The enclosed patch introduces a new adapter quirk and tries to limp
along by enabling pass-through in situations where memory is 32 bit
addressable on 64 bit machines, or disable the pass-through functions
altogether. I expect that the check for 32 bit addressable memory to be
controversial in that it can be incorrect in non-Dell non-Intel systems
that PERC would never be installed under, the alternative is to disable
pass-through in all cases which could be reported as another regression.
Pass-through is used for SCSI Generic access to the physical devices, or
for the management applications to properly function.
In systems where this patch has disabled pass-through because it is
unsupportable in combination with I/O performance, the user can choose
to enable pass-through by turning off dacmode (aacraid.dacmode=3D0) or
limiting the discovered kernel memory (mem=3D4G) with an associated loss
in runtime performance. If we chose instead to turn off 64 bit dacmode
for the adapters with this quirk, then this would be reported as another
regression.

Some of our vendors have requested that our adapters ignore the hardware
reset attempts during recovery and have enforced this with changes in
Adapter Firmware. Some of our customers have requested the option to be
able to reset the adapter under adverse adapter failure, we even had a
few defects reported here considering it a regression that the Adapter
could not be reset. This patch addresses this dichotomy. The user can
force the adapter to be reset if it supports the IOP_RESET_ALWAYS
command, in cases where the adapter has been programmed to ignore the
reset, by setting the aacraid.check_reset parameter to a value of -1.
The driver will not reset an Adapter that does not support the reset
command(s).
This patch also fixes and cleans up some of the logic associated with
resetting the adapter.

So the ioctls should be CAP_SYS_RAWIO - at least all the FIB manipulating
ones. This is a bandaid fix for #3 but probably the ioctls should grow
their own capable checks. The other two bugs need someone competent in that
driver to fix them.

Got a panic in the threading code on an older kernel when the Adapter
failed to load properly and driver shut down apparently before any
threading had started, can not dupe. Expect that this may be relevant in
the latest kernel, but not sure. This patch does no harm, and should
alleviate the possibility of this panic.

Use unsigned long or uintptr_t instead. There are several places misusing
ptrdiff_t; fixed.

This option is true if a low-level driver can support sg

If the driver fails to allocate the contiguous (DMAable) memory for
system reasons, we fail to load the instance, but then we try to free
the <nul> allocation in the cleanup code and we get a panic in
pci_free_consistent(). This is reported against an older kernel, hope
this is relevant for latest/greatest.

The patch is *much* smaller than the description. I am attempting to
answer to those that want to understand an issue that was reported in
May this year.
If a Sunrise Lake based card that requires an alternate reset mechanism
is set up to ignore the commanded IOP_RESET it reports 0x00000010
(IOP_RESET ignored) instead of 0x3803000F (use alternate reset mechanism
to reset all cores), and thus the reset platform function decides to
switch to IOP_RESET_ALWAYS because the reset platform function
parameters indicate that we *need* to reset the card. IOP_RESET_ALWAYS
then responds with the 0x3803000F return code, but alas we treat this as
an error instead of using the alternate reset mechanism (put a 0x03 into
the register offset 0x38). The reset fails, but the fact that the
IOP_RESET_ALWAYS command was issued has put the card in a purposeful
shutdown state in preparation for the alternate hardware reset to be
applied. Yuck.
IOP_RESET is ignored in internal production cards, typically to ensure
that we catch all adapter lockup issues without the driver progressing
further, so this would not appear to be a field issue and thus this
patch was destined to be only in the internal Adaptec source tree.
IOP_RESET_ALWAYS is reserved for
kexec/kdump/FirmwareUpdate/AutomatedTestFrames so we did not function as
expected in any case. Also in the past we have had OEMs specifically
request that cards not be resetable after a BlinkLED/FirmwareAssert for
one reason or another and To head off the possibility that the Sunrise
Lake based cards would suffer a similar fate, we propose the enclosed
fix.
Yinghai Lu of SUN had a pre-production card with IOP_RESET disabled when
he reported an issue to the linux kernel list back in May regarding a
kexec problem resulting from this reset being ignore. His fix was to
update the Firmware to one that did not ignore the IOP_RESET. Previous
kernels did not attempt to reset the adapter and that is why it surfaced
as a regression in his hands.

Customer running an application that issues SYNCHRONIZE_CACHE calls
directly noticed the broad stroke of the current implementation in the
aacraid driver resulting in multiple applications feeding I/O to the
storage causing the issuing application to stall for long periods of
time. By only waiting for the current WRITE commands, rather than all
commands, to complete; and those that are in range of the
SYNCHRONIZE_CACHE call that would associate more tightly with the
issuing application before telling the Firmware to flush it's dirty
cache, we managed to reduce the stalling. The Firmware itself still
flushes all the dirty cache associated with the array ignoring the
range, it just does so in a more timely manner.

On the SCSI layer ioctl path there is no implicit permissions check for
ioctls (and indeed other drivers implement unprivileged ioctls). aacraid
however allows all sorts of very admin only things to be done so should
check.

Report VPD inquiry page 0x80 with an unique array creation serial
number (CUID). When an array is created, the metadata stored on the
physical drives gets an unique serial number. This serial number
remains constant through array morphing or migration to other
controllers. This patch is a forward port and modification to survive
morphing and migration operations, of a similar piece of
(un-attributed author) code added to the SLES10 SP1 aacraid driver.
To test the results of the patch, observe that /dev/disk/by-id/
entries will show up for the arrays resulting from the udev rules.
Also, as per the udev rules, 'scsi_id -g -x -a -s /block/sd? -d
/dev/sd?' will report the ID_SERIAL as constructed from the inquiry
data.
It was reported to me that the 'ADPT' leading the serial number was bad
form, that the inquiry vendor field was enough to differentiate the
storage uniquely. Subsequent search found that another Adaptec AAC based
driver reported the 8 hex serial number only without such adornments, so
dropped ADPT to match. Resubmitting the patch with this alteration.

Incorrect dma mask was used for blinkled (firmware assert) recovery or
user initiated reset during initialization portion. Ensure that all
callers of aac_fib_map_free null out the fib allocation references to
prevent multiple free. Although serious sounding, no reports of these
problems have surfaced...

During an Adapter Initiated scan request, the query disk ioctl reports a
value of 2 rather than 1 for the valid field. This presents a problem
for some legacy management applications.

The Dell PERC2/QC erroneously was listed as having the 31 bit limit
quirk on the interface allocations, removing the reference to repair
this oversight. Also, the 2 quad pci address (family) match catch-all
also retained the 31 bit limit and the 34 SG limit quirks in a paranoid
move. Now, many years later, we find that none of the Adapters that did
trigger with the family match had such quirks; these quirks are all
limited to the 4 quad pci address matches to select legacy adapters
already populated.

This patch is more like a spelling correction than a fix. It was
discovered that if we had a busy status return from the Adapter for the
SCSI srb command to a physical component, that we returned
DID_NO_CONNECT rather than what one would expect DID_BUS_BUSY.

Add the ability for an application to issue a hardware reset to the
adapter via sysfs. Typical uses include restarting the adapter after it
has been flashed. Bumped revision number for the driver and added a
feature to periodically check the adapter's health (check_interval),
update the adapter's concept of time (update_interval) and block
checking/resetting of the adapter (check_reset).

Sundry cleanups:
1) Use kzalloc instead of kmalloc.
2) Make sure probe worked before recalling the SCSI command to finalize
processing.
3) _aac_probe_container2 and _aac_probe_container1 return value goes
unused, change return to void.
4) Use a lower depth pointer reference to pick up the driver instance
variable.
5) Although effectively unused except to fake for scsicmd validity, set
the scsi_done in probe code to aac_probe_container_callback1 instead of
the less valid dummy reference to _aac_probe_container1.
6) SCp.phase is set in aac_valid_context, drop setting up this value in
caller when unnecessary.
7) take container target id at the beginning, rather than referencing
scmd_id() to pick it up.
There should be no side effects or functionality changes.

Moves quiesce, thread and interrupt shutdown into aacraid drivers'
.shutdown handler. This fix to the aac_shutdown handler will remove the
superfluous reset of the adapter during a (clean) kexec.
This fix may mitigate the active investigation 'kexec and aacraid
broken' but it is unlikely to affect the root cause (issue likely
present in both kexec and kdump). This patch reduces the chance the
problem will occur with a kexec. The fix for root cause is currently
expected to be the minimum value check to the aacraid.startup_timeout
driver variable after an adapter reset within aacraid_commit_reset.patch
submitted on 05/22/2007 and awaiting testing by Yinghai to confirm.

Under some conditions associated with the unclean transition to kdump,
the aacraid adapters will view the array as foreign and not export it to
prevent access and data manipulation. The solution is to submit a commit
configuration to export the devices since this is a expected behavior
when transitioning to a kdump kernel.
This patch adds the aacraid.reset_devices flag and when either this or
the global reset_devices flag is set, ensures that a commit config is
issued and extends the startup_timeout if it is set less than 5 minutes.

Back in the beginning of last year we disabled mode page 8 and mode page
3f requests through device quirk bits instead of enhancing the driver to
respond to these mode pages because there was no apparent added value.
The Firmware that supports the new communication commands supports the
ability to force a write around of the adapter cache on a command by
command basis. In the attached patch we enable mode page 8 and 3f and
spoof the results as needed in order to *convince* the layers above to
submit writes with the FUA (Force Unit Attention) bit set if the file
system or application requires it, if the Firmware supports the write
through, or instead to submit a SYNCHRONIZE_CACHE if the Firmware does
not. The added value here is for file systems that benefit from this
functionality and for clustering or redundancy scenarios.
Caveats: By convince, we are responding with a minimal short 3 byte
content mode page 8, with only the data the SCSI layer needs and that we
can fill confidently. Applications that require the customarily larger
mode page 8 results may be confused by this(?). The FUA, or the
SYNCHRONIZE_CACHE only affect the cache on the controller. Our firmware
by default ensure that the underlying physical drives of the array have
their cache turned off so normally this is not a problem.
This attached patch is against current scsi-misc-2.6 and was unit tested
on RHEL5. Since this is a feature enhancement, it should not be
considered for any current stabilization efforts.

As discussed in the bugzilla outlined below, we have an sa based
(Mustang) RAID adapter on the system, a Dell PERC2/QC. Affected
controllers are HP NetRAID, Adaptec AAC-364, Dell PERC2/QC or Adaptec
5400S. This problem coincides with the introduction of the adapter_comm
and adapter_deliver platform functions (Message [PATCH 1/4] aacraid:
rework communication support code, January 23 2007, which initially
migrated to 2.6.21)
The panic occurs with an uninitialized adapter_deliver platform function
pointer. The enclosed patch, unmodified as tested by Rainer, solves the
problem.

The kexec patch introduced a superfluous (and otherwise inert) reset of
some adapters. The register can have a hardware default value that has
zeros for the undefined interrupts. This patch refines the test of the
interrupt enable register to focus on only the interrupts that affect
the driver in order to detect if an incomplete shutdown of the Adapter
had occurred (kdump).
