Mike Rapoport e31cf2f4ca mm: don't include asm/pgtable.h if linux/mm.h is already included
Patch series "mm: consolidate definitions of page table accessors", v2.

The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once.  For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.

Most of these definitions are actually identical and typically it boils
down to, e.g.

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.

For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.

These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.

This patch (of 12):

The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g.  pte_alloc() and
pmd_alloc().  So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.

The include statements in such cases are remove with a simple loop:

Gustavo A. R. Silva 3c2faf61fb char: mspec: Replace zero-length array with flexible-array member
The current codebase makes use of the zero-length array language
extension to the C90 standard, but the preferred mechanism to declare
variable-length types such as these ones is a flexible array member[1][2],
introduced in C99:

struct foo {
        int stuff;
        struct boo array[];
};

By making use of the mechanism above, we will get a compiler warning
in case the flexible array does not occur last in the structure, which
will help us prevent some kind of undefined behavior bugs from being
inadvertently introduced[3] to the codebase from now on.

Also, notice that, dynamic memory allocations won't be affected by
this change:

"Flexible array members have incomplete type, and so the sizeof operator
may not be applied. As a quirk of the original implementation of
zero-length arrays, sizeof evaluates to zero."[1]

This issue was found with the help of Coccinelle.

Souptick Joarder 3eb87d4e5a char: mspec: change return type to vm_fault_t
Use new return type vm_fault_t for the fault handler
in struct vm_operations_struct.  For now, this is just
documenting that the function returns a VM_FAULT value
rather than an errno.  Once all instances are converted,
vm_fault_t will become a distinct type.

This driver failed to handle any error returned by
vm_insert_pfn. Use the new vmf_insert_pfn function
to return the correct value.

Elena Reshetova f7d88d24c5 drivers, char: convert vma_data.refcnt from atomic_t to refcount_t
refcount_t type and corresponding API should be
used instead of atomic_t when the variable is used as
a reference counter. This allows to avoid accidental
refcounter overflows that might lead to use-after-free
situations.

Jan Kara 1a29d85eb0 mm: use vmf->address instead of of vmf->virtual_address
Every single user of vmf->virtual_address typed that entry to unsigned
long before doing anything with it so the type of virtual_address does
not really provide us any additional safety.  Just use masked
vmf->address which already has the appropriate type.

Tetsuo Handa 1d5cfdb076 tree wide: use kvfree() than conditional kfree()/vfree()
There are many locations that do

  if (memory_was_allocated_by_vmalloc)
    vfree(ptr);
  else
    kfree(ptr);

but kvfree() can handle both kmalloc()ed memory and vmalloc()ed memory
using is_vmalloc_addr().  Unless callers have special reasons, we can
replace this branch with kvfree().  Please check and reply if you found
problems.

A long time ago, in v2.4, VM_RESERVED kept swapout process off VMA,
currently it lost original meaning but still has some effects:

Thus VM_RESERVED can be replaced with VM_IO or pair VM_DONTEXPAND | VM_DONTDUMP.

remap_pfn_range() and io_remap_pfn_range() set VM_IO|VM_DONTEXPAND|VM_DONTDUMP.
remap_vmalloc_range() set VM_DONTEXPAND | VM_DONTDUMP.

Tony Luck a119365586 [IA64] Redefine ATOMIC_INIT and ATOMIC64_INIT to drop the casts
The following build error occured during a ia64 build with
swap-over-NFS patches applied.

This is identical to a parisc build error. Fengguang Wu, Mel Gorman
and James Bottomley did all the legwork to track the root cause of
the problem. This fix and entire commit log is shamelessly copied
from them with one extra detail to change a dubious runtime use of
ATOMIC_INIT() to atomic_set() in drivers/char/mspec.c

Dave Anglin says:

The above line contains two compound literals.  It also uses a designated
initializer to initialize the field enabled.  A compound literal is not a
constant expression.

The location of the above statement isn't fully clear, but if a compound
literal occurs outside the body of a function, the initializer list must
consist of constant expressions.

Arnd Bergmann 6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

Nick Piggin 2f98735c9c vm audit: add VM_DONTEXPAND to mmap for drivers that need it
Drivers that register a ->fault handler, but do not range-check the
offset argument, must set VM_DONTEXPAND in the vm_flags in order to
prevent an expanding mremap from overflowing the resource.

I've audited the tree and attempted to fix these problems (usually by
adding VM_DONTEXPAND where it is not obvious).

Cliff Wickman 4191ba26da mspec: handle shrinking virtual memory areas
The shrinking of a virtual memory area that is mmap(2)'d to a memory
special file (device drivers/char/mspec.c) can cause a panic.

If the mapped size of the vma (vm_area_struct) is very large, mspec allocates
a large vma_data structure with vmalloc(). But such a vma can be shrunk by
an munmap(2).  The current driver uses the current size of each vma to
deduce whether its vma_data structure was allocated by kmalloc() or vmalloc().
So if the vma was shrunk it appears to have been allocated by kmalloc(),
and mspec attempts to free it with kfree().  This results in a panic.

This patch avoids the panic (by preserving the type of the allocation) and
also makes mspec work correctly as the vma is split into pieces by the
munmap(2)'s.

All vma's derived from such a split vma share the same vma_data structure that
represents all the pages mapped into this set of vma's.  The mpec driver
must be made capable of using the right portion of the structure for each
member vma.  In other words, it must index into the array of page addresses
using the portion of the array that represents the current vma. This is
enabled by storing the vma group's vm_start in the vma_data structure.

The shared vma_data's are not protected by mm->mmap_sem in the fork() case
so the reference count is left as atomic_t.

 1) Uninline vma_wants_writenotify(). It's pretty huge anyway.
 2) Add back fs.h or less bloated headers (err.h) to files that need it.

As result, on x86_64 allyesconfig, fs.h dependencies cut down from 3929 files
rebuilt down to 3444 (-12.3%).

that, it serves no purpose in 2.6, other than to mess up the locked_vm

Many struct file_operations in the kernel can be "const".  Marking them const
moves these to the .rodata section, which avoids false sharing with potential
dirty data.  In addition it'll catch accidental writes at compile time to
these shared resources.

Jes Sorensen 1a4b0fc503 [PATCH] mspec driver build fix
Fix MSPEC driver to build for non SN2 enabled configs as the driver should
work in cached and uncached modes (no fetchop) on these systems.  In
addition make MSPEC select IA64_UNCACHED_ALLOCATOR, which is required for
it and move it to arch/ia64/Kconfig to avoid warnings on non ia64
architectures running allmodconfig.  Once the Kconfig code is fixed, we can
move it back.
