Jens Axboe 600335205b ide: convert to blk-mq
ide-disk and ide-cd tested as working just fine, ide-tape and
ide-floppy haven't. But the latter don't require changes, so they
should work without issue.

Add helper function to insert a request from a work queue, since we
cannot invoke the blk-mq request insertion from IRQ context.

Hannes Reinecke fef912bf86 block: genhd: add 'groups' argument to device_add_disk
Update device_add_disk() to take an 'groups' argument so that
individual drivers can register a device with additional sysfs
attributes.
This avoids race condition the driver would otherwise have if these
groups were to be created with sysfs_add_groups().

Colin Ian King bb1215a2d0 ide: remove redundant variables queue_run_ms and left
Variable queue_run_ms is not being used and is redundant and hence can
be removed. Removing this also means that variable left can also be
removed.

Kees Cook 7a6873be1b ide-cd: Remove redundant sense buffer
This is already able to process the sense buffer, so remove the redundant
parsing during the failure path. This also fixes any possible stale values
since the prior code did not check the sense length.

Kees Cook e7d0748dd7 block: Switch struct packet_command to use struct scsi_sense_hdr
There is a lot of needless struct request_sense usage in the CDROM
code. These can all be struct scsi_sense_hdr instead, to avoid any
confusion over their respective structure sizes. This patch is a lot
of noise changing "sense" to "sshdr", but the final code is more
readable to distinguish between "sense" meaning "struct request_sense"
and "sshdr" meaning "struct scsi_sense_hdr".

Kees Cook 6396bb2215 treewide: kzalloc() -> kcalloc()
The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:

        kzalloc(a * b, gfp)

with:
        kcalloc(a * b, gfp)

as well as handling cases of:

        kzalloc(a * b * c, gfp)

with:

        kzalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

        kzalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

        kzalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The Coccinelle script used for this was:

Sebastian Andrzej Siewior 47b82e8818 ide: don't enable/disable interrupts in force threaded-IRQ mode
The interrupts are enabled/disabled so the interrupt handler can run
with enabled interrupts while serving the interrupt and not lose other
interrupts especially the timer tick.
If the system runs with force-threaded interrupts then there is no need
to enable the interrupts.

Sebastian Andrzej Siewior ce1e518190 ide: don't disable interrupts during kmap_atomic()
ide_pio_bytes() disables interrupts around kmap_atomic(). This is a
leftover from the old kmap_atomic() implementation which relied on fixed
mapping slots, so the caller had to make sure that the same slot could not
be reused from an interrupting context.

kmap_atomic() was changed to dynamic slots long ago and commit 1ec9c5ddc1
("include/linux/highmem.h: remove the second argument of k[un]map_atomic()")
removed the slot assignements, but the callers were not checked for now
redundant interrupt disabling.

Remove the conditional interrupt disable.

Sebastian Andrzej Siewior 56f0ddadce ide: Handle irq disabling consistently
ide_timer_expiry() disables interrupt at function entry when acquiring
hwif->lock. Before disabling the device interrupt it unlocks hwif->lock,
but interrupts stay disabled. After the call to disable_irq() interrupts
are disabled again, which is a pointless exercise.

After the device irq handler has been invoked with interrupts disabled,
hwif->lock is acquired again with spin_lock_irq() because the device irq
handler might have reenabled interrupts. This is not documented and
confusing for the casual reader.

Remove the redundant local_irq_disable() and add a comment which explains
why hwif->lock has to be reacquired with spin_lock_irq().

init_chipset_ali15x3() initializes the chipset during init with disabled
interrupts. There is no need to keep the interrupts disabled during
pci_dev_put().
Move the irq-restore before pci_dev_put() is invoked.

Side note: The same init is performed in
drivers/ata/pata_ali.c::ali_init_chipset() without disabled interrupts.
It looks that the same hardware is supported in the ATA land. Would it
make sense to remove this driver since it is supported in the other
subsystem?

ide_toggle_bounce did select various strange block bounce limits, including
not bouncing at all as soon as an iommu is present in the system.  Given
that the dma_map routines now handle any required bounce buffering except
for ISA DMA, and the ide code already must handle either ISA DMA or highmem
at least for iommu equipped systems we can get rid of the block layer
bounce limit setting entirely.

It happens often while I'm preparing a patch for a block driver that
I'm wondering: is a definition of SECTOR_SIZE and/or SECTOR_SHIFT
available for this driver? Do I have to introduce definitions of these
constants before I can use these constants? To avoid this confusion,
move the existing definitions of SECTOR_SIZE and SECTOR_SHIFT into the
<linux/blkdev.h> header file such that these become available for all
block drivers. Make the SECTOR_SIZE definition in the uapi msdos_fs.h
header file conditional to avoid that including that header file after
<linux/blkdev.h> causes the compiler to complain about a SECTOR_SIZE
redefinition.

Note: the SECTOR_SIZE / SECTOR_SHIFT / SECTOR_BITS definitions have
not been removed from uapi header files nor from NAND drivers in
which these constants are used for another purpose than converting
block layer offsets and sizes into a number of sectors.

Linus Walleij d1ed7c5586 leds: Extends disk trigger for reads and writes
This adds two new disk triggers for triggering on reads
and writes respectively, named "disk-read" and "disk-write".

The use case comes from working on the D-Link DNS-313 NAS
box. This features an RGB LED for disk activity. with
these two triggers I can couple the green LED to read
activity and the red LED to write activity, which gives
the appropriate user feedback about what is happening
on the disk. When tested it gave exactly the feedback
desired.

The in-kernel interface is simply changed to pass a bool
indicating if the activity is write activity and update
each trigger (and the composite "disk-activity" trigger)
depending on what is passed in.

This task tried to open the cdrom device, the sr_block_open function
acquired the global sr_mutex lock. The call to check_disk_change()
then saw an event flag indicating a possible media change and tried
to flush any cached data for the device.
As part of the flush, it tried to acquire the super_block->s_umount
lock associated with the cdrom device.
This was the same super_block as created and locked by the previous task.

The first task acquires the s_umount lock and then the sr_mutex_lock;
the second task acquires the sr_mutex_lock and then the s_umount lock.

This patch fixes the issue by moving check_disk_change() out of
cdrom_open() and let the caller take care of it.

Sinan Kaya 8c016394e8 sl82c105: deprecate pci_get_bus_and_slot()
pci_get_bus_and_slot() is restrictive such that it assumes domain=0 as
where a PCI device is present. This restricts the device drivers to be
reused for other domain numbers.

Getting ready to remove pci_get_bus_and_slot() function in favor of
pci_get_domain_bus_and_slot().

Replace pci_get_bus_and_slot() with pci_get_domain_bus_and_slot()
and extract the domain number from struct pci_dev.

Bart Van Assche 4fc930896d ide: Make ide_cdrom_prep_fs() initialize the sense buffer pointer
The changes introduced through commit 82ed4db499 assume that the
sense buffer pointer in struct scsi_request is initialized for all
requests - passthrough and filesystem requests. Hence make sure
that that pointer is initialized for filesystem requests. Remove
the memset() call that clears .cmd because the scsi_req_init()
call in ide_initialize_rq() already initializes the .cmd.

     seconds boundary which originates from the PC CMOS RTC to all RTCs.

Borislav Petkov fbc3edf7d7 drivers/ide-cd: Handle missing driver data during status check gracefully
The 0day bot reports the below failure which happens occasionally, with
their randconfig testing (once every ~100 boots).  The Code points at
the private pointer ->driver_data being NULL, which hints at a race of
sorts where the private driver_data descriptor has disappeared by the
time we get to run the workqueue.

So let's check that pointer before we continue with issuing the command
to the drive.

This fix is of the brown paper bag nature but considering that IDE is
long deprecated, let's do that so that random testing which happens to
enable CONFIG_IDE during randconfig builds, doesn't fail because of
this.

Besides, failing the TEST_UNIT_READY command because the drive private
data is gone is something which we could simply do anyway, to denote
that there was a problem communicating with the device.

Many source files in the tree are missing licensing information, which
 makes it harder for compliance tools to determine the correct license.
 
 By default all files without license information are under the default
 license of the kernel, which is GPL version 2.
 
 Update the files which contain no license information with the 'GPL-2.0'
 SPDX license identifier.  The SPDX identifier is a legally binding
 shorthand, which can be used instead of the full boiler plate text.
 
 This patch is based on work done by Thomas Gleixner and Kate Stewart and
 Philippe Ombredanne.
 
 How this work was done:
 
 Patches were generated and checked against linux-4.14-rc6 for a subset of
 the use cases:
  - file had no licensing information it it.
  - file was a */uapi/* one with no licensing information in it,
  - file was a */uapi/* one with existing licensing information,
 
 Further patches will be generated in subsequent months to fix up cases
 where non-standard license headers were used, and references to license
 had to be inferred by heuristics based on keywords.
 
 The analysis to determine which SPDX License Identifier to be applied to
 a file was done in a spreadsheet of side by side results from of the
 output of two independent scanners (ScanCode & Windriver) producing SPDX
 tag:value files created by Philippe Ombredanne.  Philippe prepared the
 base worksheet, and did an initial spot review of a few 1000 files.
 
 The 4.13 kernel was the starting point of the analysis with 60,537 files
 assessed.  Kate Stewart did a file by file comparison of the scanner

 by lawyers working with the Linux Foundation.
 
 Kate also obtained a third independent scan of the 4.13 code base from
 FOSSology, and compared selected files where the other two scanners
 disagreed against that SPDX file, to see if there was new insights.  The
 Windriver scanner is based on an older version of FOSSology in part, so

 Thomas did random spot checks in about 500 files from the spreadsheets
 for the uapi headers and agreed with SPDX license identifier in the
 files he inspected. For the non-uapi files Thomas did random spot checks
 in about 15000 files.
 
 In initial set of patches against 4.14-rc6, 3 files were found to have
 copy/paste license identifier errors, and have been fixed to reflect the
 correct identifier.
 
 Additionally Philippe spent 10 hours this week doing a detailed manual
 inspection and review of the 12,461 patched files from the initial patch
 version early this week with:
  - a full scancode scan run, collecting the matched texts, detected
    license ids and scores
  - reviewing anything where there was a license detected (about 500+
    files) to ensure that the applied SPDX license was correct
  - reviewing anything where there was no detection but the patch license
    was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
    SPDX license was correct
 
 This produced a worksheet with 20 files needing minor correction.  This
 worksheet was then exported into 3 different .csv files for the
 different types of files to be modified.
 
 These .csv files were then reviewed by Greg.  Thomas wrote a script to
 parse the csv files and add the proper SPDX tag to the file, in the
 format that the file expected.  This script was further refined by Greg
 based on the output to detect more types of files automatically and to
 distinguish between header and source .c files (which need different
 comment types.)  Finally Greg ran the script using the .csv files to

Arnd Bergmann c091fbe9a2 block: fix CDROM dependency on BLK_DEV
After the cdrom cleanup, I get randconfig warnings for some configurations:

warning: (BLK_DEV_IDECD && BLK_DEV_SR) selects CDROM which has unmet direct dependencies (BLK_DEV)

This adds an explicit BLK_DEV dependency for both drivers. The other
drivers that select 'CDROM' already have this and don't need a change.

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Bart Van Assche 2a750166a5 block: Rework drivers/cdrom/Makefile
Instead of referring from inside drivers/cdrom/Makefile to all the
drivers that use this driver, let these drivers select the cdrom
driver. This change makes the cdrom build code follow the approach
that is used for most other drivers, namely refer from the higher
layers to the lower layer instead of from the lower layer to the
higher layers.

Since we split the scsi_request out of struct request, while the
standard prep_rq_fn builds 10 byte cmds, it missed to invoke
scsi_req_init() to initialize certain fields of a scsi_request

members of struct scsi_request).
