Linus Torvalds 5df397dec7 mm: delay page_remove_rmap() until after the TLB has been flushed
When we remove a page table entry, we are very careful to only free the
page after we have flushed the TLB, because other CPUs could still be
using the page through stale TLB entries until after the flush.

However, we have removed the rmap entry for that page early, which means
that functions like folio_mkclean() would end up not serializing with the
page table lock because the page had already been made invisible to rmap.

And that is a problem, because while the TLB entry exists, we could end up
with the following situation:

 (a) one CPU could come in and clean it, never seeing our mapping of the
     page

 (b) another CPU could continue to use the stale and dirty TLB entry and
     continue to write to said page

resulting in a page that has been dirtied, but then marked clean again,
all while another CPU might have dirtied it some more.

End result: possibly lost dirty data.

This extends our current TLB gather infrastructure to optionally track a
"should I do a delayed page_remove_rmap() for this page after flushing the
TLB".  It uses the newly introduced 'encoded page pointer' to do that
without having to keep separate data around.

Note, this is complicated by a couple of issues:

 - we want to delay the rmap removal, but not past the page table lock,
   because that simplifies the memcg accounting

 - only SMP configurations want to delay TLB flushing, since on UP
   there are obviously no remote TLBs to worry about, and the page
   table lock means there are no preemption issues either

 - s390 has its own mmu_gather model that doesn't delay TLB flushing,
   and as a result also does not want the delayed rmap. As such, we can
   treat S390 like the UP case and use a common fallback for the "no
   delays" case.

 - we can track an enormous number of pages in our mmu_gather structure,
   with MAX_GATHER_BATCH_COUNT batches of MAX_TABLE_BATCH pages each,
   all set up to be approximately 10k pending pages.

   We do not want to have a huge number of batched pages that we then
   need to check for delayed rmap handling inside the page table lock.

Particularly that last point results in a noteworthy detail, where the
normal page batch gathering is limited once we have delayed rmaps pending,
in such a way that only the last batch (the so-called "active batch") in
the mmu_gather structure can have any delayed entries.

NOTE!  While the "possibly lost dirty data" sounds catastrophic, for this
all to happen you need to have a user thread doing either madvise() with
MADV_DONTNEED or a full re-mmap() of the area concurrently with another
thread continuing to use said mapping.

So arguably this is about user space doing crazy things, but from a VM
consistency standpoint it's better if we track the dirty bit properly even
when user space goes off the rails.

Linus Torvalds 7cc8f9c714 mm: mmu_gather: prepare to gather encoded page pointers with flags
This is purely a preparatory patch that makes all the data structures
ready for encoding flags with the mmu_gather page pointers.

The code currently always sets the flag to zero and doesn't use it yet,
but now it's tracking the type state along.  The next step will be to
actually start using it.

Jann Horn 2ba99c5e08 mm/khugepaged: fix GUP-fast interaction by sending IPI
Since commit 70cbc3cc78 ("mm: gup: fix the fast GUP race against THP
collapse"), the lockless_pages_from_mm() fastpath rechecks the pmd_t to
ensure that the page table was not removed by khugepaged in between.

However, lockless_pages_from_mm() still requires that the page table is
not concurrently freed.  Fix it by sending IPIs (if the architecture uses
semi-RCU-style page table freeing) before freeing/reusing page tables.

Linus Torvalds 7fb5e50831 mmu_gather: fix the CONFIG_MMU_GATHER_NO_RANGE case
Sudip reports that alpha doesn't build properly, with errors like

the cause being that We have this odd situation where some architectures
were never converted to the newer TLB flushing interfaces that have a
range for the flush.  Instead people left them alone, and we have them
select the MMU_GATHER_NO_RANGE config option to make the tlb header
files account for this.

Peter Zijlstra cleaned some of these nasty header file games up in
commits

  1e9fdf21a4 ("mmu_gather: Remove per arch tlb_{start,end}_vma()")
  18ba064e42 ("mmu_gather: Let there be one tlb_{start,end}_vma() implementation")

but tlb_update_vma_flags() was left alone, and then commit b67fbebd4c
("mmu_gather: Force tlb-flush VM_PFNMAP vmas") ended up removing only
_one_ of the two stale duplicate dummy inline functions.

This removes the other stale one.

Somebody braver than me should try to remove MMU_GATHER_NO_RANGE
entirely, but it requires fixing up the oddball architectures that use
it: alpha, m68k, microblaze, nios2 and openrisc.

The fixups should be fairly straightforward ("fix the build errors it
exposes by adding the appropriate range arguments"), but the reason this
wasn't done in the first place is that so few people end up working on
those architectures.  But it could be done one architecture at a time,
hint, hint.

Peter Zijlstra b67fbebd4c mmu_gather: Force tlb-flush VM_PFNMAP vmas
Jann reported a race between munmap() and unmap_mapping_range(), where
unmap_mapping_range() will no-op once unmap_vmas() has unlinked the
VMA; however munmap() will not yet have invalidated the TLBs.

Therefore unmap_mapping_range() will complete while there are still
(stale) TLB entries for the specified range.

Mitigate this by force flushing TLBs for VM_PFNMAP ranges.

Peter Zijlstra 18ba064e42 mmu_gather: Let there be one tlb_{start,end}_vma() implementation
Now that architectures are no longer allowed to override
tlb_{start,end}_vma() re-arrange code so that there is only one
implementation for each of these functions.

This much simplifies trying to figure out what they actually do.

Peter Zijlstra 1e9fdf21a4 mmu_gather: Remove per arch tlb_{start,end}_vma()
Scattered across the archs are 3 basic forms of tlb_{start,end}_vma().
Provide two new MMU_GATHER_knobs to enumerate them and remove the per
arch tlb_{start,end}_vma() implementations.

 - MMU_GATHER_NO_FLUSH_CACHE indicates the arch has flush_cache_range()
   but does *NOT* want to call it for each VMA.

 - MMU_GATHER_MERGE_VMAS indicates the arch wants to merge the
   invalidate across multiple VMAs if possible.

With these it is possible to capture the three forms:

  1) empty stubs;
     select MMU_GATHER_NO_FLUSH_CACHE and MMU_GATHER_MERGE_VMAS

  2) start: flush_cache_range(), end: empty;
     select MMU_GATHER_MERGE_VMAS

  3) start: flush_cache_range(), end: flush_tlb_range();
     default

Obviously, if the architecture does not have flush_cache_range() then
it also doesn't need to select MMU_GATHER_NO_FLUSH_CACHE.

Nadav Amit c9fe66560b mm/mprotect: do not flush when not required architecturally
Currently, using mprotect() to unprotect a memory region or uffd to
unprotect a memory region causes a TLB flush.  However, in such cases the
PTE is often not modified (i.e., remain RO) and therefore not TLB flush is
needed.

Add an arch-specific pte_needs_flush() which tells whether a TLB flush is
needed based on the old PTE and the new one.  Implement an x86
pte_needs_flush().

Always flush the TLB when it is architecturally needed even when skipping
a TLB flush might only result in a spurious page-faults by skipping the
flush.

Even with such conservative manner, we can in the future further refine
the checks to test whether a PTE is present by only considering the
architectural _PAGE_PRESENT flag instead of {pte|pmd}_preesnt().  For not
be careful and use the latter.

Steve Capper 697a1d44af tlb: hugetlb: Add more sizes to tlb_remove_huge_tlb_entry
tlb_remove_huge_tlb_entry only considers PMD_SIZE and PUD_SIZE when
updating the mmu_gather structure.

Unfortunately on arm64 there are two additional huge page sizes that
need to be covered: CONT_PTE_SIZE and CONT_PMD_SIZE. Where an end-user
attempts to employ contiguous huge pages, a VM_BUG_ON can be experienced
due to the fact that the tlb structure hasn't been correctly updated by
the relevant tlb_flush_p.._range() call from tlb_remove_huge_tlb_entry.

This patch adds inequality logic to the generic implementation of
tlb_remove_huge_tlb_entry s.t. CONT_PTE_SIZE and CONT_PMD_SIZE are
effectively covered on arm64. Also, as well as ptes, pmds and puds;
p4ds are now considered too.

Gustavo A. R. Silva 5224f79096 treewide: Replace zero-length arrays with flexible-array members
There is a regular need in the kernel to provide a way to declare
having a dynamically sized set of trailing elements in a structure.
Kernel code should always use “flexible array members”[1] for these
cases. The older style of one-element or zero-length arrays should
no longer be used[2].

This code was transformed with the help of Coccinelle:

Will Deacon d8b450530b tlb: mmu_gather: Introduce tlb_gather_mmu_fullmm()
Passing the range '0, -1' to tlb_gather_mmu() sets the 'fullmm' flag,
which indicates that the mm_struct being operated on is going away. In
this case, some architectures (such as arm64) can elide TLB invalidation
by ensuring that the TLB tag (ASID) associated with this mm is not
immediately reclaimed. Although this behaviour is documented in
asm-generic/tlb.h, it's subtle and easily missed.

Introduce tlb_gather_mmu_fullmm() to make it clearer that this is for the
entire mm and WARN() if tlb_gather_mmu() is called with the 'fullmm'
address range.

Mike Rapoport ca15ca406f mm: remove unneeded includes of <asm/pgalloc.h>
Patch series "mm: cleanup usage of <asm/pgalloc.h>"

Most architectures have very similar versions of pXd_alloc_one() and
pXd_free_one() for intermediate levels of page table.  These patches add
generic versions of these functions in <asm-generic/pgalloc.h> and enable
use of the generic functions where appropriate.

In addition, functions declared and defined in <asm/pgalloc.h> headers are
used mostly by core mm and early mm initialization in arch and there is no
actual reason to have the <asm/pgalloc.h> included all over the place.
The first patch in this series removes unneeded includes of
<asm/pgalloc.h>

In the end it didn't work out as neatly as I hoped and moving
pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require
unnecessary changes to arches that have custom page table allocations, so
I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local
to mm/.

This patch (of 8):

In most cases <asm/pgalloc.h> header is required only for allocations of
page table memory.  Most of the .c files that include that header do not
use symbols declared in <asm/pgalloc.h> and do not require that header.

As for the other header files that used to include <asm/pgalloc.h>, it is
possible to move that include into the .c file that actually uses symbols
from <asm/pgalloc.h> and drop the include from the header file.

The process was somewhat automated using

	sed -i -E '/[<"]asm\/pgalloc\.h/d' \
                $(grep -L -w -f /tmp/xx \
                        $(git grep -E -l '[<"]asm/pgalloc\.h'))

where /tmp/xx contains all the symbols defined in
arch/*/include/asm/pgalloc.h.

Peter Zijlstra 0d6e24d430 asm-generic/tlb: provide MMU_GATHER_TABLE_FREE
As described in the comment, the correct order for freeing pages is:

 1) unhook page
 2) TLB invalidate page
 3) free page

This order equally applies to page directories.

Currently there are two correct options:

 - use tlb_remove_page(), when all page directores are full pages and
   there are no futher contraints placed by things like software
   walkers (HAVE_FAST_GUP).

 - use MMU_GATHER_RCU_TABLE_FREE and tlb_remove_table() when the
   architecture does not do IPI based TLB invalidate and has
   HAVE_FAST_GUP (or software TLB fill).

This however leaves architectures that don't have page based directories
but don't need RCU in a bind.  For those, provide MMU_GATHER_TABLE_FREE,
which provides the independent batching for directories without the
additional RCU freeing.

does two TLBIs when tlb->fullmm, because __tlb_reset_range() will not
clear tlb->end in that case.

Observe that any caller to __tlb_adjust_range() also sets at least one of
the tlb->freed_tables || tlb->cleared_p* bits, and those are
unconditionally cleared by __tlb_reset_range().

Change the condition for actually issuing TLBI to having one of those bits
set, as opposed to having tlb->end != 0.

Peter Zijlstra 0ed1325967 mm/mmu_gather: invalidate TLB correctly on batch allocation failure and flush
Architectures for which we have hardware walkers of Linux page table
should flush TLB on mmu gather batch allocation failures and batch flush.
Some architectures like POWER supports multiple translation modes (hash
and radix) and in the case of POWER only radix translation mode needs the
above TLBI.  This is because for hash translation mode kernel wants to
avoid this extra flush since there are no hardware walkers of linux page
table.  With radix translation, the hardware also walks linux page table
and with that, kernel needs to make sure to TLB invalidate page walk cache
before page table pages are freed.

More details in commit d86564a2f0 ("mm/tlb, x86/mm: Support invalidating
TLB caches for RCU_TABLE_FREE")

The changes to sparc are to make sure we keep the old behavior since we
are now removing HAVE_RCU_TABLE_NO_INVALIDATE.  The default value for
tlb_needs_table_invalidate is to always force an invalidate and sparc can
avoid the table invalidate.  Hence we define tlb_needs_table_invalidate to
false for sparc architecture.

Aneesh Kumar K.V 864edb758c powerpc/mm/book3s64/radix: Flush the full mm even when need_flush_all is set
With the previous patch, we should now not be using need_flush_all for
powerpc. But then make sure we force a PID tlbie flush with RIC=2 if
we ever find need_flush_all set. Also don't reset it after a mmu
gather flush.

Nadav Amit 5932c9fd19 mm/tlb: Provide default nmi_uaccess_okay()
x86 has an nmi_uaccess_okay(), but other architectures do not.
Arch-independent code might need to know whether access to user
addresses is ok in an NMI context or in other code whose execution
context is unknown.  Specifically, this function is needed for
bpf_probe_write_user().

Add a default implementation of nmi_uaccess_okay() for architectures
that do not have such a function.

Make issuing a TLB invalidate for page-table pages the normal case.

The reason is twofold:

 - too many invalidates is safer than too few,
 - most architectures use the linux page-tables natively
   and would thus require this.

Make it an opt-out, instead of an opt-in.

When an architecture does not have (an efficient) flush_tlb_range(),
but instead always uses full TLB invalidates, the current generic
tlb_flush() is sub-optimal, for it will generate extra flushes in
order to keep the range small.

But if we cannot do range flushes, that is a moot concern. Optionally
provide this simplified default.

Provide a generic tlb_flush() implementation that relies on
flush_tlb_range(). This is a little awkward because flush_tlb_range()
assumes a VMA for range invalidation, but we no longer have one.

Audit of all flush_tlb_range() implementations shows only vma->vm_mm
and vma->vm_flags are used, and of the latter only VM_EXEC (I-TLB

Therefore, track VM_EXEC and VM_HUGETLB in two more bits, and create a
'fake' VMA.

This allows architectures that have a reasonably efficient
flush_tlb_range() to not require any additional effort.

Peter Zijlstra e7fd28a706 asm-generic/tlb, arch: Provide generic VIPT cache flush
The one obvious thing SH and ARM want is a sensible default for
tlb_start_vma(). (also: https://lkml.org/lkml/2004/1/15/6 )

Avoid all VIPT architectures providing their own tlb_start_vma()
implementation and rely on architectures to provide a no-op
flush_cache_range() when it is not relevant.

This patch makes tlb_start_vma() default to flush_cache_range(), which
should be right and sufficient. The only exceptions that I found where
(oddly):

  - m68k-mmu
  - sparc64
  - unicore

Those architectures appear to have flush_cache_range(), but their
current tlb_start_vma() does not call it.

No change in behavior intended.

Will Deacon a6d60245d6 asm-generic/tlb: Track which levels of the page tables have been cleared
It is common for architectures with hugepage support to require only a
single TLB invalidation operation per hugepage during unmap(), rather than
iterating through the mapping at a PAGE_SIZE increment. Currently,
however, the level in the page table where the unmap() operation occurs
is not stored in the mmu_gather structure, therefore forcing
architectures to issue additional TLB invalidation operations or to give
up and over-invalidate by e.g. invalidating the entire TLB.

Ideally, we could add an interval rbtree to the mmu_gather structure,
which would allow us to associate the correct mapping granule with the
various sub-mappings within the range being invalidated. However, this
is costly in terms of book-keeping and memory management, so instead we
approximate by keeping track of the page table levels that are cleared
and provide a means to query the smallest granule required for invalidation.

Some architectures require different TLB invalidation instructions
depending on whether it is only the last-level of page table being
changed, or whether there are also changes to the intermediate
(directory) entries higher up the tree.

Add a new bit to the flags bitfield in struct mmu_gather so that the
architecture code can operate accordingly if it's the intermediate
levels being invalidated.

Nicholas Piggin fd1102f0aa mm: mmu_notifier fix for tlb_end_vma
The generic tlb_end_vma does not call invalidate_range mmu notifier, and
it resets resets the mmu_gather range, which means the notifier won't be
called on part of the range in case of an unmap that spans multiple
vmas.

ARM64 seems to be the only arch I could see that has notifiers and uses
the generic tlb_end_vma.  I have not actually tested it.

[ Catalin and Will point out that ARM64 currently only uses the
  notifiers for KVM, which doesn't use the ->invalidate_range()
  callback right now, so it's a bug, but one that happens to
  not affect them.  So not necessary for stable.  - Linus ]

Rik van Riel 2ff6ddf19c x86/mm/tlb: Leave lazy TLB mode at page table free time
Andy discovered that speculative memory accesses while in lazy
TLB mode can crash a system, when a CPU tries to dereference a
speculative access using memory contents that used to be valid
page table memory, but have since been reused for something else
and point into la-la land.

The latter problem can be prevented in two ways. The first is to
always send a TLB shootdown IPI to CPUs in lazy TLB mode, while
the second one is to only send the TLB shootdown at page table
freeing time.

The second should result in fewer IPIs, since operationgs like
mprotect and madvise are very common with some workloads, but
do not involve page table freeing. Also, on munmap, batching
of page table freeing covers much larger ranges of virtual
memory than the batching of unmapped user pages.

Nicholas Piggin a90744bac5 mm: allow arch to supply p??_free_tlb functions
The mmu_gather APIs keep track of the invalidated address range
including the span covered by invalidated page table pages.  Ranges
covered by page tables but not ptes (and therefore no TLBs) still need
to be invalidated because some architectures (x86) can cache
intermediate page table entries, and invalidate those with normal TLB
invalidation instructions to be almost-backward-compatible.

Architectures which don't cache intermediate page table entries, or
which invalidate these caches separately from TLB invalidation, do not
require TLB invalidation range expanded over page tables.

Allow architectures to supply their own p??_free_tlb functions, which
can avoid the __tlb_adjust_range.

Minchan Kim 99baac21e4 mm: fix MADV_[FREE|DONTNEED] TLB flush miss problem
Nadav reported parallel MADV_DONTNEED on same range has a stale TLB
problem and Mel fixed it[1] and found same problem on MADV_FREE[2].

Quote from Mel Gorman:
 "The race in question is CPU 0 running madv_free and updating some PTEs
  while CPU 1 is also running madv_free and looking at the same PTEs.
  CPU 1 may have writable TLB entries for a page but fail the pte_dirty
  check (because CPU 0 has updated it already) and potentially fail to
  flush.

  Hence, when madv_free on CPU 1 returns, there are still potentially
  writable TLB entries and the underlying PTE is still present so that a
  subsequent write does not necessarily propagate the dirty bit to the
  underlying PTE any more. Reclaim at some unknown time at the future
  may then see that the PTE is still clean and discard the page even
  though a write has happened in the meantime. I think this is possible
  but I could have missed some protection in madv_free that prevents it
  happening."

This patch aims for solving both problems all at once and is ready for
other problem with KSM, MADV_FREE and soft-dirty story[3].

TLB batch API(tlb_[gather|finish]_mmu] uses [inc|dec]_tlb_flush_pending
and mmu_tlb_flush_pending so that when tlb_finish_mmu is called, we can
catch there are parallel threads going on.  In that case, forcefully,
flush TLB to prevent for user to access memory via stale TLB entry
although it fail to gather page table entry.

I confirmed this patch works with [4] test program Nadav gave so this
patch supersedes "mm: Always flush VMA ranges affected by zap_page_range
v2" in current mmotm.

NOTE:

This patch modifies arch-specific TLB gathering interface(x86, ia64,
s390, sh, um).  It seems most of architecture are straightforward but
s390 need to be careful because tlb_flush_mmu works only if
mm->context.flush_mm is set to non-zero which happens only a pte entry
really is cleared by ptep_get_and_clear and friends.  However, this
problem never changes the pte entries but need to flush to prevent
memory access from stale tlb.

Minchan Kim 56236a5955 mm: refactor TLB gathering API
This patch is a preparatory patch for solving race problems caused by
TLB batch.  For that, we will increase/decrease TLB flush pending count
of mm_struct whenever tlb_[gather|finish]_mmu is called.

Before making it simple, this patch separates architecture specific part
and rename it to arch_tlb_[gather|finish]_mmu and generic part just
calls it.

It shouldn't change any behavior.
