Naoki Yanagimoto reported that configuration read on some hot-added
PCIe device returns invalid value. This patch fixes this problem.
According to the PCIe spec, software must wait for at least 1 second
to judge if the hot-added device is broken after Data Link Layer State
Changed Event. This patch changes pciehp driver to wait for 1 second
after the Data Link Layer State Changed Event is detected before
initiating a configuration access instead of 100 ms.

While it's declared static, etags points you to the wrong function
in drivers/acpi/dock.c and acpiphp_glue.c for example also makes
use of some (exported..) functions from this file.
If you trust etags and oversee the static declaration (what happened
to me) one gets totally confused...

Structs battery_file, acpi_dock_ops, file_operations,

are not changed in runtime. It is safe to make them const.
register_hotplug_dock_device() was altered to take const "ops" argument

powerpc has two different ways of matching PCI devices to their
corresponding OF node (if any) for historical reasons. The ppc64 one
does a scan looking for matching bus/dev/fn, while the ppc32 one does a
scan looking only for matching dev/fn on each level in order to be
agnostic to busses being renumbered (which Linux does on some
platforms).
This removes both and instead moves the matching code to the PCI core
itself. It's the most logical place to do it: when a pci_dev is created,
we know the parent and thus can do a single level scan for the matching
device_node (if any).
The benefit is that all archs now get the matching for free. There's one
hook the arch might want to provide to match a PHB bus to its device
node. A default weak implementation is provided that looks for the
parent device device node, but it's not entirely reliable on powerpc for
various reasons so powerpc provides its own.

The following patch sets the MaxPayload setting to match the parent
reading when inserting a PCIE card into a hotplug slot. On our system,
the upstream bridge is set to 256, but when inserting a card, the card
setting defaults to 128. As soon as I/O is performed to the card it
starts receiving errors since the payload size is too small.

If a device doesn't support power management (pm_cap == 0) but it is
acpi_pci_power_manageable() because there is a _PS0 method declared for
it and _EJ0 is also declared for the slot then nobody is going to set
current_state = PCI_D0 for this device. This is what I think it is
happening:

(SLOT_ENABLED | SLOT_POWEREDON) but it does not set the pci device
current state to PCI_D0.
So my proposed fix is also to set current_state = PCI_D0 in
register_slot.

If pcie_ports_disabled is set, pcie_port_service_register() returns
error code and select_detection_mode() should not attempt to
unregister dummy_driver and use dummy_slots. It should return
PCIEHP_DETECT_ACPI immediately instead.

While testing various randconfigs with ktest.pl, I hit the following panic:

Adding printks, I found that the loop that reads the ebda blocks
can move out of the mapped section.

The start of the iomap was at f7e44c00 and had a size of 5120,
making the end f7e46000. We start with an offset of 0x180 or
384, giving the first read at 0xf7e44d80. Reading that location
yields 65283, which is much bigger than the 5120 that was allocated
and makes the next read at f7e54b03 which is outside the mapped area.
Perhaps this is a bug in the driver, or buggy hardware, but this patch
is more about not crashing my box on start up and just giving a warning
if it detects this error.
This patch at least lets my box boot with just a warning.

All these files use the big kernel lock in a trivial
way to serialize their private file operations,
typically resulting from an earlier semi-automatic
pushdown from VFS.
None of these drivers appears to want to lock against
other code, and they all use the BKL as the top-level
lock in their file operations, meaning that there
is no lock-order inversion problem.
Consequently, we can remove the BKL completely,
replacing it with a per-file mutex in every case.
Using a scripted approach means we can avoid
typos.
These drivers do not seem to be under active
maintainance from my brief investigation. Apologies
to those maintainers that I have missed.

After commit 852972acff (ACPI: Disable
ASPM if the platform won't provide _OSC control for PCIe) control of
the PCIe Capability Structure is unconditionally requested by
acpi_pci_root_add(), which in principle may cause problems to
happen in two ways. First, the BIOS may refuse to give control of
the PCIe Capability Structure if it is not asked for any of the
_OSC features depending on it at the same time. Second, the BIOS may
assume that control of the _OSC features depending on the PCIe
Capability Structure will be requested in the future and may behave
incorrectly if that doesn't happen. For this reason, control of
the PCIe Capability Structure should always be requested along with
control of any other _OSC features that may depend on it (ie. PCIe
native PME, PCIe native hot-plug, PCIe AER).
Rework the PCIe port driver so that (1) it checks which native PCIe
port services can be enabled, according to the BIOS, and (2) it
requests control of all these services simultaneously. In
particular, this causes pcie_portdrv_probe() to fail if the BIOS
refuses to grant control of the PCIe Capability Structure, which
means that no native PCIe port services can be enabled for the PCIe
Root Complex the given port belongs to. If that happens, ASPM is
disabled to avoid problems with mishandling it by the part of the
PCIe hierarchy for which control of the PCIe Capability Structure
has not been received.
Make it possible to override this behavior using 'pcie_ports=native'
(use the PCIe native services regardless of the BIOS response to the
control request), or 'pcie_ports=compat' (do not use the PCIe native
services at all).
Accordingly, rework the existing PCIe port service drivers so that
they don't request control of the services directly.

It is possible that the BIOS will not grant control of all _OSC
features requested via acpi_pci_osc_control_set(), so it is
recommended to negotiate the final set of _OSC features with the
query flag set before calling _OSC to request control of these
features.
To implement it, rework acpi_pci_osc_control_set() so that the caller
can specify the mask of _OSC control bits to negotiate and the mask
of _OSC control bits that are absolutely necessary to it. Then,
acpi_pci_osc_control_set() will run _OSC queries in a loop until
the mask of _OSC control bits returned by the BIOS is equal to the
mask passed to it. Also, before running the _OSC request
acpi_pci_osc_control_set() will check if the caller's required
control bits are present in the final mask.
Using this mechanism we will be able to avoid situations in which the
BIOS doesn't grant control of certain _OSC features, because they
depend on some other _OSC features that have not been requested.

Originally this was thought to have been required within the PCI
Hotplug specification for some legacy devices. However there is
no such requirement in the most recent revision. The check prevents
hotplug of not only display devices but also computational GPUs
which require serviceability.

There are devices out there which are PCI Hot-plug controllers with
compaq PCI IDs, but are not bridges, hence have pdev->subordinate
NULL. But cpqphp expects the pointer to be non-NULL.
Add a check to the probe function to avoid oopses like:

This ensures that the translations for unmapped IO mappings or
unmapped memory are properly removed from the MMU hash table
before such an unplug. Without this, the hypervisor refuses the
unplug operations due to those resources still being mapped by
the partition.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Per ACPI spec, _ERG method should be executed before device driver
gets control for hotpluged device. Firmware might do some configuration
there. See http://bugzilla.kernel.org/show_bug.cgi?id=10805. In this
machine, _REG method of docked device will configure cardbus bridge.

Constify struct sysfs_ops.
This is part of the ops structure constification
effort started by Arjan van de Ven et al.
Benefits of this constification:
* prevents modification of data that is shared
(referenced) by many other structure instances
at runtime
* detects/prevents accidental (but not intentional)
modification attempts on archs that enforce
read-only kernel data at runtime
* potentially better optimized code as the compiler
can assume that the const data cannot be changed
* the compiler/linker move const data into .rodata
and therefore exclude them from false sharing

No functional change; this converts loops that iterate from 0 to
PCI_BUS_NUM_RESOURCES through pci_bus resource[] table to use the
pci_bus_for_each_resource() iterator instead.
This doesn't change the way resources are stored; it merely removes
dependencies on the fact that they're in a table.

Move bus_size_bridges and assign resources out of pciehp_add_bridge()
and do them all together, one time, including slot bridge, to avoid to
calling assign resources several times when there are several bridges

Stanse found an ommitted pci_dev_put on one error path in
cpcihp_generic_init. The path is taken on !dev, but also when
dev->hdr_type != PCI_HEADER_TYPE_BRIDGE. However it omits to
pci_dev_put on the latter.
As it is fine to pass NULL to pci_dev_put, put it in there
uncoditionally.

ibmphp driver currently maps only 1KB of ebda memory area into kernel address
space during driver initialization. This causes kernel oops when the driver is
modprobe'd and it accesses memory beyond 1KB within ebda segment. The first
byte of ebda segment actually stores the length of the ebda region in
Kilobytes. Hence make use of the length parameter and map the entire ebda
region.

Move the max_bus_speed and cur_bus_speed into the pci_bus. Expose the
values through the PCI slot driver instead of the hotplug slot driver.
Update all the hotplug drivers to use the pci_bus instead of their own
data structures.

These enums must not overlap anyway, since we only have a single
pci_bus_speed_strings array. Use a single enum, and move it to

Callers (acpi_memhotplug.c, dock.c and others) check for the return
value of acpi_bus_add() and assume a valid device was returned in
case zero was returned.
Thus return -ENODEV if no device was found in acpi_bus_scan and
propagate this through acpi_bus_add and acpi_bus_start.
Also remove a confusing comment in acpiphp_glue.c, acpi_bus_scan
will and cannot invoke if acpi_bus_add returns no valid device.

Enabling power fault detected event notification in current pciehp
might cause power fault interrupt storm on some machines. On those
machines. On those machines, power fault detected bit in the slot
status register was set again immediately when it is cleared in the
interrupt service routine, and next power fault detected interrupt was
notified again. Therefore, disable power fault detected event
notification for now.
This patch also removes unnecessary handling for power fault cleared
event because this event is not supported by PCIe spec.

Use pci_pcie_cap() instead of pci_find_capability() to get PCIe capability
offset in pciehp driver. This avoids unnecessary search in PCI
configuration space. This patch also removes 'cap_base' field in
struct controller, that was used to hold PCIe capability offset by
pciehp itself.

This patch moves PCI I/O APIC support from acpiphp to a separate driver.
Like pciehp and shpchp, acpiphp handles PCI hotplug, i.e., addition and
removal of PCI adapters. But in addition, acpiphp handles some ACPI
hotplug, such as the addition of new host bridges, and the I/O APIC
support was tangled up with that.
I don't think the I/O APIC support needs to be in acpiphp; PCI I/O APICs
usually appear as a function on a PCI host bridge, and we'll enumerate the
APIC before any of the devices behind the bridge that use it.
As far as I know, nobody actually uses I/O APIC hotplug. It depends on
acpi_register_ioapic(), which is only implemented for ia64, and I don't
think any vendors have supported I/O chassis hotplug yet.

Current pciehp returns successfully on read/write failure with dummy
state values. It should return error instead.
With this patch, pciehp no longer uses hotplug_slot_info data
structure. So this also removes hotplug_slot_info related code. But
note that it still allocates hotplug_slot_info because it is required
by pci hotplug core.

Current pciehp driver creates 'attention' and 'latch' files even if
the controller doesn't support them. In this case, the contents of
those files are meaningless and unpredictable. Those files should be
created only if the controller has the corresponding capabilities.

As a followup to 71a082efc9, it's conceivable
that some vendors may expose PCI hotplug functionality through both vendor
mechanisms and ACPI. The native mechanism will generally be a superset of
any functionality provided via ACPI, so the acpiphp driver should always
be initialised after any others. Change the link order such that acpiphp
will not be initialised until any other statically linked drivers have had
an opportunity to claim the hardware.

Commit 15b8dd53f5 changed info->hardware_id from a static array to
a pointer. If hardware_id is non-NULL, it points to a NULL-terminated
string, so we don't need to terminate it explicitly. However, it may
be NULL; in that case, we *can't* add a NULL terminator.
This causes a NULL pointer dereference oops for devices without _HID.

Unlike the previous acpiphp-specific code, pci_configure_slot() programs
PCIe settings when an _HPX method provides them, so acpiphp-managed PCIe
devices can now be configured.

program_fw_provided_values().
Unlike the previous SHPC-specific code, pci_configure_slot() programs PCIe
settings when an _HPX method provides them, so if it's possible to have an
SHPC-managed PCIe device, it can now be configured.

This patch adds a new pci_configure_slot() function that programs the
PCI bus characteristics for a newly-added device. This is based on
code in pciehp_pci.c, but should be generic enough to be used by pciehp,
shpchp, and acpiphp.
The hotplug_params struct and the program_hpp_typeX() functions are based
on the ACPI definitions, but they aren't really ACPI-specific, and there's
no alternate implementation, so I don't see the need to abstract them yet.

We always look up hotplug_params with decode_hpp() immediately before using
them, so we don't need to save them in the acpiphp_bridge struct. This
patch follows the example of program_fw_provided_values() in pciehp_pci.c
and shpchp_pci.c by just keeping the params on the stack while we need them.

decode_hpp() looks up hotplug PCI parameters from ACPI and saves them
in the acpiphp_bridge structure. These parameters (in bridge->hpp) are
only used by the acpiphp_set_hpp_values() -> program_hpp() path. In
that path, we always call decode_hpp() before program_hpp(), so there's
no need to do it in init_bridge_misc().

struct pci_bus arg to an acpi_handle, but every time we use this
interface, we already have the handle available.
So let's just use the handle instead of converting back and forth.

Instead of constantly evaluating _ADR and _SEG over and over again,
let's simplify our lives by using:
acpi_pci_find_root() for root bridges
acpi_get_pci_dev() for p2p bridges
This change eliminates some copy 'n paste code and also allows us
to simplify some internal interfaces.

There is a very old quirk for the intel E7502 E7320 and E7525 memory
controller hubs that disables usage of msi interrupts on pcie hotplug
bridges of those devices, and disables changing the affinity of irqs.
Today all we have to do to disable msi on a specific device is to set
dev->no_msi, which is much more straightforward than the previous
logic.
The re-running of this fixup after pci hotplug happens below these
devices is totally bogus. All of the state we change is pure software
state and we don't change the hardware at all. Which means hotplug on
the lower devices doesn't have a chance to change this state. So we
can safely remove the special case from the pciehp driver and the pcie
portdriver.
I suspect the special case was someone's expermental debug code that
slipped in. Certainly it isn't mentioned in commit

We cannot simply call acpi_get_pci_dev() on any random ACPI handle
and hope that it works, because a PCI root bridge may not have
an associated struct pci_dev.
This is allowed per the PCI specification, and is referred to as a
non-materialized bridge.
So, depending on the type of PCI bridge that the handle points to,
use the appropriate interface to return the struct pci_bus correctly.

Completed a major update for the acpi_get_object_info external interface.
Changes include:
- Support for variable, unlimited length HID, UID, and CID strings
- Support Processor objects the same as Devices (HID,UID,CID,ADR,STA, etc.)
- Call the _SxW power methods on behalf of a device object
- Determine if a device is a PCI root bridge
- Change the ACPI_BUFFER parameter to ACPI_DEVICE_INFO.
These changes will require an update to all callers of this interface.

By the pci slot changes, callbacks of attributes under slot directory
(/sys/bus/pci/slots) had been changed to get the pointer to struct
pci_slot instead of struct hotplug_slot. So the path_show() that
assumes the parameter is a pointer to struct hotplug_slot seems
broken.

There is no way to interact with a physical PCI slot without
sysfs, so encode the dependency and prevent this build error:

Create symbolic link to hotplug driver module in the PCI slot
directory (/sys/bus/pci/slots/<SLOT#>). In the past, we need to load
hotplug drivers one by one to identify the hotplug driver that handles
the slot, and it was very inconvenient especially for trouble shooting.
With this change, we can easily identify the hotplug driver.

Current has_foo() functions in pci_hotplug_core.c returns 0 if the
"foo" property is true. It would cause misunderstanding. In addition,
the error code of those functions is never checked, so this patch
changes those functions' error code to 'bool' and return true if the
property "foo" is true.

The EMI support in pciehp is obviously broken. It is implemented using
struct hotplug_slot_attribute, but sysfs_ops for pci_slot_ktype is NOT
for struct hotplug_slot_attribute, but for struct pci_slot_attribute.
This bug had been there for a long time, maybe it was introduced when
PCI slot framework was introduced. The reason why this bug didn't
cause any problem is maybe the EMI support is not tested at all
because of lack of test environment.
As described above, the EMI support in pciehp seems not to be tested
at all. So this patch removes EMI support from pciehp, instead of
fixing the bug.

Convert uses of pci_find_slot to modern API.
In the conversion sites, we end up calling pci_dev_put() right away.
This may seem like it misses the entire point of doing something like
pci_get_bus_and_slot(), since we drop the reference so soon, but it turns
out we don't actually do much with the returned pci_dev.
I plan on untangling cpqphp further, but clearly cpqphp never worried too
much about a properly refcounted pci_dev anyway. For now, this conversion
seems reasonable, as it gets rid of the last in-tree caller of pci_find_slot.

Instead of making multiple calls to pcibios_get_irq_routing_table, let's
just do it once and save the answer.
The reason we were making multiple calls is because we liked to calculate
its length and perform some loop over it. Instead of open-coding the length
calculation every time, provide it in an inline helper function.
Finally, since pci_print_IRQ_route() is used only for debug, let's only
do it when cpqhp_debug is set.

An oops can occur if a user attempts to use both PCI logical
hotplug and the ACPI physical hotplug driver (acpiphp) in this
sequence, where $slot/address == $device.
In other words, if acpiphp has claimed a PCI device, and that
device is logically removed, then acpiphp may oops when it
attempts to access it again.

The root cause of this oops is that the logical remove ("echo 1 >
/sys/bus/pci/devices/$device/remove") destroyed the pci_dev. The
pci_dev struct itself wasn't deallocated because acpiphp kept a
reference, but some of its fields became invalid.
acpiphp doesn't have any real reason to keep a pointer to a
pci_dev around. It can always derive it using pci_get_slot().
If a logical remove destroys the pci_dev, acpiphp won't find it
and is thus prevented from causing mischief.

according to Ingo, io_apic irq-setup related functions have too many
parameters with a repetitive signature.

If a logical hot unplug (remove) is performed on a bridge claimed
by acpiphp and then acpiphp is unloaded, we will encounter an oops.
This is because acpiphp will access the bridge's subordinate bus,
which was released by the user's prior hot unplug.
The solution is to grab a reference on the subordinate PCI bus.
This will prevent the bus from release until acpiphp is unloaded.

A complete re-implementation of fakephp is necessary if it is to
present its former interface (pre-2.6.27, when it broke). The
reason is that PCI hotplug drivers call pci_hp_register(), which
enforces the rule that only one /sys/bus/pci/slots/ file may be
created per physical slot.
The change breaks the old fakephp's assumption that it could
create a file per function. So we re-implement fakephp to avoid
using the standard PCI hotplug API so that we can restore the old
fakephp user interface.
It puts entries in /sys/bus/pci/slots with the names of all PCI
devices/functions, exactly symmetrical to what is shown in
/sys/bus/pci/devices. Each slots/ entry has a "power" attribute,
which works the same way as the fakephp driver's power attribute
has worked.
There are a few improvements over old fakephp, which couldn't handle
PCI devices being added or removed via a means outside of
fakephp's knowledge. If a device was added another way, old fakephp
didn't notice and didn't create the fake slot for it. If a
device was removed another way, old fakephp didn't delete the fake
slot for it (and accessing the stale slot caused an oops).
The new implementation overcomes these limitations. As a
consequence, removing a bridge with other devices behind it now
works as well, which is something else old fakephp couldn't do
previously.
This duplicates a tiny bit of the code in the PCI core that does
this same function. Re-using that code ends up being more
complex than duplicating it, and it makes code in the PCI core
more ugly just to support this legacy fakephp interface
compatibility layer.

Current acpi_get_hp_hw_control_from_firmware() has a assumption that
pci_bus->self is NULL on a PCI root bus. But it might not be true on
some platforms. Because of this wrong assumption, current
acpi_get_hp_hw_control_from_firmware() might cause endless loop. We
must check pci_bus->parent instead.

pci_bus->self is NULL on the root pci bus. But it might not true on
some platforms. Because of this wrong assumption, current
acpi_get_hp_params_from_firmware() might cause endless loop. We must
check pci_bus->parent instead.

Current pciehp disables software notification of adapter presence
changed event and MRL changed event when slot is turned off. Because
of this, there is no way to detect those events on empty slots in the
current pciehp implementation.
According to the past discussion(*), this behavior was introduced to
prevent endless loop that could happen if pcie_isr() runs after power
fault is detected on a certain platform whose stickey power-fault bit
remains on till the slot is powered on again.

I think this endless loop can be avoided using one bit flag that
indicates power fault had been detected, instead of disabling software
notification of adapter present changed event and MRL changed event.
With this patch, we can enable software notification mechanism of
presence changed and MRL changed event on the empty slots again.

Fix possible endless loop in pcie_isr.
Currently, pcie_isr() (interrupt service routine of pciehp) can end up in an
endless loop if the Slot Status register is set again immediately after being
cleared. According to the past discussion (see below URL) this case can happen
if the power fault detected bit is set during handling.

The PCI Express port driver uses 'struct pcie_port_service_id' for
matching port service devices and drivers, but this structure
contains fields that duplicate information from the port device
itself (vendor, device, subvendor, subdevice) and fields that are not
used by any existing port service driver (class, class_mask,
drvier_data). Also, both existing port service drivers (AER and
PCIe HP) don't even use the vendor and device fields for device
matching. Therefore 'struct pcie_port_service_id' can be removed
altogether and the only useful members of it (port_type, service) can
be introduced directly into the port service device and port service
driver structures. That simplifies the code quite a bit and reduces
its size.

Some hardware exposes PCIE slots in such a way that they can be claimed
by either the acpiphp or pciehp driver. pciehp is the preferred driver
if the firmware allows the OS to claim control via the _OSC method so
should be loaded first - if it fails to bind (either due to a missing
_OSC method or the firmware refusing to hand off control) then we can
fall back to acpiphp or a vendor-specific driver.
This patch simply changes the link order to ensure that pciehp will be
initialised before acpiphp if both are statically built into the kernel.

For PCI devices, pci_bus_assign_resources() must be called to set up the
pci_device->resource array before pci_bus_add_devices() can be called, else
attempts to load drivers results in BAR collision errors where there are none.
This is not done in fakephp, so devices can be "unplugged" but scanning the
parent bus won't bring the devices back due to resource unallocation. Move the
pci_bus_add_device-calling logic into pci_rescan_bus and preface it with a call
to pci_bus_assign_resources so that we only have to (re)allocate resources once
per bus where a new device is found.

Bit 10 in Link Status register used to be defined as Training Error in
the PCI Express 1.0a specification. But it was removed by Training Error
ECN and is no longer defined. So pciehp must ignore the value read from

Some ACPI related PCI hotplug code can be shared among PCI hotplug
drivers. This patch introduces the following functions in
drivers/pci/hotplug/acpi_pcihp.c to share the code, and changes
acpiphp and pciehp to use them.
- int acpi_pci_detect_ejectable(struct pci_bus *pbus)
This checks if the specified PCI bus has ejectable slots.
- int acpi_pci_check_ejectable(struct pci_bus *pbus, acpi_handle handle)
This checks if the specified handle is ejectable ACPI PCI slot. The
'pbus' parameter is needed to check if 'handle' is PCI related ACPI
object.
This patch also introduces the following inline function in
include/linux/pci-acpi.h, which is useful to get ACPI handle of the
PCI bridge from struct pci_bus of the bridge's secondary bus.
- static inline acpi_handle acpi_pci_get_bridge_handle(struct pci_bus *pbus)
This returns ACPI handle of the PCI bridge which generates PCI bus
specified by 'pbus'.

ACPI based hot-pluggable PCIe slot detection logic was added to
prevent the problem non hot-pluggable PCIe slot was detected as
hot-pluggable. The slot detection logic can be selected through
'pciehp_detect_mode', but it would be better if it is selected
automatically.
This patch adds 'auto' option for 'pciehp_detect_mode'. When it is
specified, pciehp judges which 'acpi' or 'pcie' should be used. It
seems that the physical slot number is duplicated among some slots on
most of the platforms with the above-mentioned problem. So 'auto' mode
uses this information to judge which 'acpi' or 'pcie' should be
used. That is, if duplicated physical slot numbers are detected,
'acpi' mode is used. This method is not perfect, but it's realistic.

There is a problem that some non hot-pluggable PCIe slots are detected
as hot-pluggable by pciehp on some platforms. The immediate cause of
this problem is that hot-plug capable bit in the Slot Capabilities
register is set even for non hot-pluggable slots on those platforms.
It seems a BIOS/hardware problem, but we need workaround about that.
Some of those platforms define hot-pluggable PCIe slots on ACPI
namespace properly, while hot-plug capable bit in the Slot
Capabilities register is set improperly. So using ACPI namespace
information in pciehp to detect PCIe hot-pluggable slots would be a
workaround.
This patch adds 'pciehp_detect_mode' module option. When 'acpi' is
specified, pciehp uses ACPI namespace information to detect PCIe
hot-pluggable slots.

This patch changes cpqphp to use interrupt pin values just as they
come from PCI config space, i.e., 1=INTA, ..., 4=INTD.
pcibios_set_irq_routing() takes pin arguments in the range 0=INTA, ...,
3=INTD, so we'll adjust the pin just before calling it.

According to section 6.3.6 of the ACPI spec, the presence of an _RMV
method that evaluates to 1 is sufficient to indicate that a slot is
removable without needing an eject method. This patch refactors the
ejectable slot detection code a little in order to flag these slots as
ejectable and register them. Acpihp then binds to the expresscard slot
on my HP test machine.

pci_get_slot does a pci_dev_get, so pci_dev_put needs to be called in an
error case.
An alterative would be to move the test_and_set_bit before the call to
pci_get_slot.

This patch is part of a larger patch series which will remove
the "char bus_id[20]" name string from struct device. The device
name is managed in the kobject anyway, and without any size
limitation, and just needlessly copied into "struct device".
To set and read the device name dev_name(dev) and dev_set_name(dev)
must be used. If your code uses static kobjects, which it shouldn't
do, "const char *init_name" can be used to statically provide the
name the registered device should have. At registration time, the
init_name field is cleared, to enforce the use of dev_name(dev) to
access the device name at a later time.
We need to get rid of all occurrences of bus_id in the entire tree
to be able to enable the new interface. Please apply this patch,
and possibly convert any remaining remaining occurrences of bus_id.

The pciehp driver waits for 1000 msec after turning power off to make
sure the power has been completely removed. But this 1000 msec wait is
not needed if a slot doesn't implement power control because software
cannot control the power. Power will be automatically removed at adapter
removal time on such a slot

Fix trivial conflict in drivers/char/Makefile as per Paul's directions
I happened to notice that the ibmphp hotplug driver does something
rather silly in its init routine. It purposely calls module_put so as
to underflow its module ref count to avoid being removed from the
kernel. This is bad practice, and wrong, since it provides a window for
subsequent module_gets to reset the refcount to zero, allowing an unload
to race in and cause all sorts of mysterious panics. If the module is
unsafe to load, simply omitting the module_exit parameter is sufficient
to prevent the kernel from allowing the unload.

Certain HP machines require the full 64 bits of _SUN as allowed
by the ACPI spec. Without this change, we get name collisions in
the lower 32 bits of the _SUN returned by firmware.

The pseries PCI hotplug code has a number of issues, ranging from
incorrect resource setup to crashes, depending on what is added,
when, whether it contains a bridge, etc etc....
This fixes a whole bunch of these, while actually simplifying the code
a bit, using more generic code in the process and factoring out common
code between adding of a PHB, a slot or a device.

The following patch fixes the regression in 2.6.27 that causes kernel
NULL pointer dereference at cpqphp driver probe time. This patch should
be backported to the .27 stable series.
Seems to have been introduced by
f46753c5e3.
The root cause of this problem seems that cpqphp driver calls
pci_hp_register() wrongly. In current implementation, cpqphp driver
passes 'ctrl->pci_dev->subordinate' as a second parameter for
pci_hp_register(). But because hotplug slots and it's hotplug controller
(exists as a pci funcion) are on the same bus, it should be
'ctrl->pci_dev->bus' instead.

This patch adds polling mechanism for Data Link Layer Link Active bit
after turning power on, instead of waiting for 1000 msec. This reduces
reduce the unnecessary long wait.

The PCI core now manages slot names on behalf of slot detection
and slot hotplug drivers, including the handling of duplicate
slot names.
We can use the fakephp driver to help test the new functionality.
Add a 'dup_slots' module param to force fakephp to create multiple
slots with the same name. We can then verify that the PCI core
correctly renamed the slots.

Now that the PCI core manages the 'name' for each individual
hotplug driver, and all drivers (except rpaphp) have been converted
to use hotplug_slot_name(), there is no need for the PCI hotplug
core to drag around its own copy of name either.

rpaphp tends to use slot->name directly everywhere, and doesn't
ever need slot->hotplug_slot->name.
struct hotplug_slot->name is going away, so convert rpaphp directly
manipulate its own slot->name everywhere, and don't bother touching
slot->hotplug_slot->name.

Prevent callers of pci_create_slot() from registering slots with
duplicate names. This condition occurs most often when PCI hotplug
drivers are loaded on platforms with broken firmware that assigns
identical names to multiple slots.
We now rename these duplicate slots on behalf of the user.
If firmware assigns the name N to multiple slots, then:
The first registered slot is assigned N
The second registered slot is assigned N-1
The third registered slot is assigned N-2
etc.
This is the permanent fix mentioned in earlier commits d6a9e9b4 and
167e782e (shpchp/pciehp: Rename duplicate slot name...).
We take advantage of the new 'hotplug' parameter in pci_create_slot()
to prevent a slot create/rename race between hotplug drivers and
detection drivers.
Scenario A:
hotplug driver detection driver
-------------- ----------------
pci_create_slot(hotplug=set)
pci_create_slot(hotplug=NULL)
The hotplug driver creates the slot with its desired name, and then
releases the semaphore. Now, the detection driver tries to create
the same slot, but it already exists. We don't care about renaming,
so return the existing slot.
Scenario B:
hotplug driver detection driver
-------------- ----------------
pci_create_slot(hotplug=NULL)
pci_create_slot(hotplug=set)
The detection driver creates the slot with name "X". Then the hotplug
driver tries to create the same slot, but wants the name "Y" instead.
We detect that we're trying to create the same slot and that we also
want a rename, so rename the slot to "Y" and return.
Scenario C:
hotplug driver hotplug driver
-------------- ----------------
pci_create_slot(hotplug=set)
pci_create_slot(hotplug=set)
Two separate hotplug drivers are attempting to claim the slot and
are passing valid hotplug_slot args to pci_create_slot(). We detect
that the slot already has a ->hotplug callback, prevent a rename,
and return -EBUSY.

Slot detection drivers can co-exist with hotplug drivers. The names
of the detected/claimed slots may be different depending on module
load order.
For legacy reasons, we need to allow hotplug drivers to override
the slot name if a detection driver is loaded first (and they find
the same slots).
Creating and overriding slot names should be an atomic operation,
otherwise you get a locking nightmare as various drivers race to
call pci_create_slot().
pci_create_slot() is already serialized by grabbing the pci_bus_sem.
We update the API and add a 'hotplug' param, which is:
set if the caller is a hotplug driver
NULL if the caller is a detection driver
pci_create_slot() does not actually use the 'hotplug' parameter in this
patch. A later patch will add the logic that uses it.

Update pci_hp_register() to take a const char *name parameter.
The motivation for this is to clean up the individual hotplug
drivers so that each one does not have to manage its own name.
The PCI core should be the place where we manage the name.
We update the interface and all callsites first, in a
"no functional change" manner, and clean up the drivers later.

The commit 356a9d6f3dd283f83861adf1ac909879f0e66411 (PCI: fix hotplug
get_##name return value problem) doesn't seem to be merged properly.
Because of this, PCI hotplug no longer works (Read/Write PCI hotplug
files always returns -ENODEV).
This patch fixes wrong check of try_module_get() return value check in
get_##name().

As of version 2.0, ACPI can return 64-bit integers. The current
acpi_evaluate_integer only supports 64-bit integers on 64-bit platforms.
Change the argument to take a pointer to an acpi_integer so we support
64-bit integers on all platforms.

io_mem = ioremap (ebda_seg<<4, 65000);
it assumes that the EBDA is 65000 bytes. But BIOS EBDA pointers are
at most 1K large.
_if_ the Rio code truly extends upon the customary EBDA size it needs
to iounmap() this memory and ioremap() it larger, once it knows it from
the generic descriptors that a Rio system is around.

dock's uevent reported itself, not ata. It might be difficult to find an
ata device just according to a dock. This patch introduces docking ops
for each device in a dock. when docking, dock driver can send device
specific uevent. This should help dock station too (not just bay)

Commit fe99740cac (construct one
fakephp slot per PCI slot) introduced a regression, causing a
deadlock when removing a PCI device.
We also never actually removed the device from the PCI core.
So we:
- remove the device from the PCI core
- do not directly call remove_slot() to prevent deadlock

introduces the shpchp_slot_with_bus module parameter, which was intended
to help work around broken firmware that assigns the same name to multiple
slots.
Commit b3bd307c62 (shpchp: add message about
shpchp_slot_with_bus option) tells the user to use the above parameter
in the event of a name collision.
This approach is sub-optimal because it requires too much work from
the user.
Instead, let's rename the slot on behalf of the user. If firmware
assigns the name N to multiple slots, then:
The first registered slot is assigned N
The second registered slot is assigned N-1
The third registered slot is assigned N-2
The Mth registered slot becomes N-M
In the event we overflow the slot->name parameter, we report an
error to the user.
This is a temporary fix until the entire PCI core can be reworked
such that individual drivers no longer have to manage their own
slot names.

_OSC should be ran on a root bridge instead of the device itself. Do
this before touching OSHP since PCI fw specs states that _OSC should be
preferred over OSHP (however if the device has OSHP but not _OSC -- not
a root bridge -- it's not).

Fixed up conflicts in various files (arch/x86/kernel/setup_64.c,

Proper memory barriers have been added to order accesses
to ->cmd_busy, so volatile declaration for cmd_busy can
be removed.

During the development of the physical PCI slot patch series, Gary Hade
kept on reporting strange oopses due to interactions between pci_slot
and acpiphp.

find_root_bridges() unconditionally installs
handle_hotplug_event_bridge() as an ACPI_SYSTEM_NOTIFY handler for all
root bridges.
However, during module cleanup, remove_bridge() will only remove the
notify handler iff the root bridge had a hot-pluggable slot directly
underneath. That is:
root bridge -> hotplug slot
But, if the topology looks like either of the following:
root bridge -> non-hotplug slot
root bridge -> p2p bridge -> hotplug slot
Then we currently do not remove the notify handler from that root
bridge.
This can cause a kernel oops if we modprobe acpiphp later and it gets
loaded somewhere else in memory. If the root bridge then receives a
hotplug event, it will then attempt to call a stale, non-existent notify
handler and we blow up.

Current pciehp driver saves its private data pointer into pci_dev
structure using pci_set_drvdata()/pci_get_drvdata(). But because
pciehp is not a pci device driver but a PCI Express service driver, it
should save its private data pointer into pcie_device structure using
set_service_data()/get_service_data().

Currently, pciehp driver enables command completed interrupt as follows.
(1) Don't enable at initialization.
(2) Enable command completed interrupt whenever pciehp issues a
command, if the command doesn't attempt to disable the interrupt.
(3) Disable command completed interrupt at driver unloading.
Once we enable command completed interrupt, we don't need to re-enable
it for every command. So we can simplify above steps as follows:
(1) Enable command completed interrupt at initialization.
(2) No special sequence for command completed interrupt.
(3) Disable command completed interrupt at driver unloading.

Current pciehp driver's intialization sequence is as follows:
(1) initialize controller data structure
(2) install interrupt handler
(3) enable software notification
(4) initialize controller specific slot data structure
(5) initialize generic slot data structure and register it to pci hotplug core
The interrupt handler of pciehp assumes that controller specific slot
data structure is already initialized. However, it is installed at (2)
before initializing controller specific slot data structure at
(4). Because of this, pciehp driver cannot handle the following cases
properly.
- If devices that shares IRQ with pciehp raise interrupts between (2) and (4).
- If hotplug events (e.g. MRL open) happen between (3) and (4).
We already have a workaround for this problem ("pciehp: fix NULL
dereference in interrupt handler: dbd79aed1a).
But we still need fundamental fix.
This patch fix the problem by changing the initilization sequence as follows:
(1) initialize controller data structure
(2) initialize controller specific slot data structure
(3) install interrupt handler
(4) enable software notification
(5) initialize generic slot data structure and register it to pci hotplug core
