Pull SCSI target updates from Nicholas Bellinger:
 "It's been a busy development cycle for target-core in a number of
  different areas.

  The fabric API usage for se_node_acl allocation is now within
  target-core code, dropping the external API callers for all fabric
  drivers tree-wide.

  There is a new conversion to RCU hlists for se_node_acl and
  se_portal_group LUN mappings, that turns fast-past LUN lookup into a
  completely lockless code-path.  It also removes the original
  hard-coded limitation of 256 LUNs per fabric endpoint.

  The configfs attributes for backends can now be shared between core
  and driver code, allowing existing drivers to use common code while
  still allowing flexibility for new backend provided attributes.

  The highlights include:

   - Merge sbc_verify_dif_* into common code (sagi)
   - Remove iscsi-target support for obsolete IFMarker/OFMarker
     (Christophe Vu-Brugier)
   - Add bidi support in target/user backend (ilias + vangelis + agover)
   - Move se_node_acl allocation into target-core code (hch)
   - Add crc_t10dif_update common helper (akinobu + mkp)
   - Handle target-core odd SGL mapping for data transfer memory
     (akinobu)
   - Move transport ID handling into target-core (hch)
   - Move task tag into struct se_cmd + support 64-bit tags (bart)
   - Convert se_node_acl->device_list[] to RCU hlist (nab + hch +
     paulmck)
   - Convert se_portal_group->tpg_lun_list[] to RCU hlist (nab + hch +
     paulmck)
   - Simplify target backend driver registration (hch)
   - Consolidate + simplify target backend attribute implementations
     (hch + nab)
   - Subsume se_port + t10_alua_tg_pt_gp_member into se_lun (hch)
   - Drop lun_sep_lock for se_lun->lun_se_dev RCU usage (hch + nab)
   - Drop unnecessary core_tpg_register TFO parameter (nab)
   - Use 64-bit LUNs tree-wide (hannes)

Nicholas Bellinger bc0c94b140 target: Drop unnecessary core_tpg_register TFO parameter
This patch drops unnecessary target_core_fabric_ops parameter usage
for core_tpg_register() during fabric driver TFO->fabric_make_tpg()
se_portal_group creation callback execution.

Instead, use the existing se_wwn->wwn_tf->tf_ops pointer to ensure
fabric driver is really using the same TFO provided at module_init
time.

Also go ahead and drop the forward TFO declarations tree-wide, and
handling the special case for iscsi-target discovery TPG.

Bart Van Assche ba92999252 target: Minimize SCSI header #include directives
Only include SCSI initiator header files in target code that needs
these header files, namely the SCSI pass-through code and the tcm_loop
driver. Change SCSI_SENSE_BUFFERSIZE into TRANSPORT_SENSE_BUFFER in
target code because the former is intended for initiator code and the
latter for target code. With this patch the only initiator include
directives in target code that remain are as follows:

Nicholas Bellinger 403edd78a2 target: Convert se_tpg->acl_node_lock to ->acl_node_mutex
This patch converts se_tpg->acl_node_lock to struct mutex, so that
->acl_node_acl walkers in core_clear_lun_from_tpg() can block when
calling core_disable_device_list_for_node().

It also updates core_dev_add_lun() to hold ->acl_node_mutex when
calling core_tpg_add_node_to_devs() to build ->lun_entry_hlist
for dynamically generated se_node_acl.

Bart Van Assche 649ee05499 target: Move task tag into struct se_cmd + support 64-bit tags
Simplify target core and target drivers by storing the task tag
a.k.a. command identifier inside struct se_cmd.

For several transports (e.g. SRP) tags are 64 bits wide.
Hence add support for 64-bit tags.

(Fix up usb-gadget to use 16-bit tags - HCH + bart)

Christoph Hellwig 2650d71e24 target: move transport ID handling to the core

Christoph Hellwig c7d6a80392 target: refactor init/drop_nodeacl methods
By always allocating and adding, respectively removing and freeing
the se_node_acl structure in core code we can remove tons of repeated
code in the init_nodeacl and drop_nodeacl routines.  Additionally
this now respects the get_default_queue_depth method in this code
path as well.

Christoph Hellwig 9ac8928e6a target: simplify the target template registration API
Instead of calling target_fabric_configfs_init() +
target_fabric_configfs_register() / target_fabric_configfs_deregister()
target_fabric_configfs_free() from every target driver, rewrite the API
so that we have simple register/unregister functions that operate on
a const operations vector.

This patch also fixes a memory leak in several target drivers. Several
target drivers namely called target_fabric_configfs_deregister()
without calling target_fabric_configfs_free().

A large part of this patch is based on earlier changes from
Bart Van Assche <bart.vanassche@sandisk.com>.

(v2: Add a new TF_CIT_SETUP_DRV macro so that the core configfs code
can declare attributes as either core only or for drivers)

The use of "rcu_assign_pointer()" is NULLing out the pointer.
According to RCU_INIT_POINTER()'s block comment:
"1.   This use of RCU_INIT_POINTER() is NULLing out the pointer"
it is better to use it instead of rcu_assign_pointer() because it has a
smaller overhead.

Nicholas Bellinger 6dbe7f4e97 tcm_fc: Generate TASK_SET_FULL status for response failures
This patch changes ft_queue_status() to set SAM_STAT_TASK_SET_FULL
status upon lport->tt.seq_send( failure, and return -EAGAIN to notify
target-core to attempt to requeue the response.

It also does the same for a fc_frame_alloc() failures, in order to
signal the initiator that it should try to reduce it's current
queue_depth, to lower the number of outstanding I/Os on the wire.

Nicholas Bellinger b3e5fe1688 tcm_fc: Generate TASK_SET_FULL status for DataIN failures
This patch changes ft_queue_data_in() to set SAM_STAT_TASK_SET_FULL
status upon a lport->tt.seq_send() failure, where it will now stop
sending subsequent DataIN, and immediately attempt to send the
response with exception status.

Sending a response with SAM_STAT_TASK_SET_FULL status is useful in
order to signal the initiator that it should try to reduce it's
current queue_depth, to lower the number of outstanding I/Os on
the wire.

Also, add a check to skip sending DataIN if TASK_SET_FULL status
has already been set due to a response lport->tt.seq_send()
failure, that has asked target-core to requeue a response.

Nicholas Bellinger ed8ec8f707 tcm_fc: Fix free-after-use regression in ft_free_cmd
This patch fixes a free-after-use regression in ft_free_cmd(), where
ft_sess_put() is called with cmd->sess after percpu_ida_free() has
already released the tag.

Fix this bug by saving the ft_sess pointer ahead of percpu_ida_free(),
and pass it directly to ft_sess_put().

The regression was originally introduced in v3.13-rc1 commit:

These functions are not adding or deleting an lport. They are adding a
wwn that may match with an lport that is present on the system.

Renaming ft_del_lport also means we won't have functions named
both ft_del_lport and ft_lport_del any more.

Rename struct ft_lport_acl to ft_lport_wwn. "acl" is associated with
something different in LIO terms. Really, ft_lport_wwn is the
fabric-specific wrapper for the struct se_wwn.

Rename "lacl" local variables to "ft_wwn" as well.

Rename list_heads used as list members to make it clear they're nodes, not
heads.

Rename lport_node to ft_wwn_node.

Rename ft_lport_list to ft_wwn_list

Andy Grover d242c1d7d3 target/tcm_fc: Limit to 1 TPG per wwn
tcm_fc doesn't support multiple TPGs per wwn. For proof, see
ft_lport_find_tpg. Enforce this in the code.

Replace ft_lport_wwn.tpg_list with a single pointer. We can't fold ft_tpg
into ft_lport_wwn because they can have different lifetimes.

Andy Grover 2c42be2dd4 target/tcm_fc: Fix use-after-free of ft_tpg
ft_del_tpg checks tpg->tport is set before unlinking the tpg from the
tport when the tpg is being removed. Set this pointer in ft_tport_create,
or the unlinking won't happen in ft_del_tpg and tport->tpg will reference
a deleted object.

This patch sets tpg->tport in ft_tport_create, because that's what
ft_del_tpg checks, and is the only way to get back to the tport to
clear tport->tpg.

The bug was occuring when:

- lport created, tport (our per-lport, per-provider context) is
  allocated.
  tport->tpg = NULL
- tpg created
- a PRLI is received. ft_tport_create is called, tpg is found and
  tport->tpg is set
- tpg removed. ft_tpg is freed in ft_del_tpg. Since tpg->tport was not
  set, tport->tpg is not cleared and points at freed memory
- Future calls to ft_tport_create return tport via first conditional,
  instead of searching for new tpg by calling ft_lport_find_tpg.
  tport->tpg is still invalid, and will access freed memory.

Nicholas Bellinger e70beee783 target: Pass in transport supported PI at session initialization
In order to support local WRITE_INSERT + READ_STRIP operations for
non PI enabled fabrics, the fabric driver needs to be able signal
what protection offload operations are supported.

This is done at session initialization time so the modes can be
signaled by individual se_wwn + se_portal_group endpoints, as well
as optionally across different transports on the same endpoint.

For iser-target, set TARGET_PROT_ALL if the underlying ib_device
has already signaled PI offload support, and allow this to be
exposed via a new iscsit_transport->iscsit_get_sup_prot_ops()
callback.

For loopback, set TARGET_PROT_ALL to signal SCSI initiator mode
operation.

For all other drivers, set TARGET_PROT_NORMAL to disable fabric
level PI.

Nicholas Bellinger 131e6abc67 target: Add TFO->abort_task for aborted task resources release
Now that TASK_ABORTED status is not generated for all cases by
TMR ABORT_TASK + LUN_RESET, a new TFO->abort_task() caller is
necessary in order to give fabric drivers a chance to unmap
hardware / software resources before the se_cmd descriptor is
released via the normal TFO->release_cmd() codepath.

This patch adds TFO->aborted_task() in core_tmr_abort_task()
in place of the original transport_send_task_abort(), and
also updates all fabric drivers to implement this caller.

The fabric drivers that include changes to perform cleanup
via ->aborted_task() are:

  - iscsi-target
  - iser-target
  - srpt
  - tcm_qla2xxx

The fabric drivers that currently set ->aborted_task() to
NOPs are:

  - loopback
  - tcm_fc
  - usb-gadget
  - sbp-target
  - vhost-scsi

For the latter five, there appears to be no additional cleanup
required before invoking TFO->release_cmd() to release the
se_cmd descriptor.

Kent Overstreet 6f6b5d1ec5 percpu_ida: Make percpu_ida_alloc + callers accept task state bitmask
This patch changes percpu_ida_alloc() + callers to accept task state
bitmask for prepare_to_wait() for code like target/iscsi that needs
it for interruptible sleep, that is provided in a subsequent patch.

It now expects TASK_UNINTERRUPTIBLE when the caller is able to sleep
waiting for a new tag, or TASK_RUNNING when the caller cannot sleep,
and is forced to return a negative value when no tags are available.

v2 changes:
  - Include blk-mq + tcm_fc + vhost/scsi + target/iscsi changes
  - Drop signal_pending_state() call

  - Only call prepare_to_wait() + finish_wait() when != TASK_RUNNING

Nicholas Bellinger 5f544cfac9 tcm_fc: Convert to per-cpu command map pre-allocation of ft_cmd
This patch converts tcm_fc to use transport_init_session_tags()
pre-allocation logic for struct ft_cmd descriptors using per-cpu
session tag pooling in order to effectively avoid memory allocation
+ release for each received I/O.

It adds percpu_ida_alloc() in ft_recv_cmd() to obtain an tag and
locate ft_cmd from se_sess->sess_cmd_map[], and percpu_ida_free()
in ft_free_cmd() to release the tag based upon se_cmd->map_tag id.

It also uses a TCM_FC_DEFAULT_TAGS value of 512, that puts the
per se_sess->sess_cmd_map allocation at ~360K on 64-bit.

Joern Engel 11fee8a751 target: remove unused codes from enum tcm_tmrsp_table
Three have been checked for but were never set.  Remove the dead code.
Also renumbers the remaining ones to a) get rid of the holes after the
removal and b) avoid a collision between TMR_FUNCTION_COMPLETE==0 and
the uninitialized case.  If we failed to set a code, we should rather
fall into the default case then return success.

Sasha Levin b67bfe0d42 hlist: drop the node parameter from iterators
I'm not sure why, but the hlist for each entry iterators were conceived

        list_for_each_entry(pos, head, member)

The hlist ones were greedy and wanted an extra parameter:

        hlist_for_each_entry(tpos, pos, head, member)

Why did they need an extra pos parameter? I'm not quite sure. Not only
they don't really need it, it also prevents the iterator from looking
exactly like the list iterator, which is unfortunate.

Besides the semantic patch, there was some manual work required:

 - Fix up the actual hlist iterators in linux/list.h
 - Fix up the declaration of other iterators based on the hlist ones.
 - A very small amount of places were using the 'node' parameter, this
 was modified to use 'obj->member' instead.
 - Coccinelle didn't handle the hlist_for_each_entry_safe iterator
 properly, so those had to be fixed up manually.

Yi Zou 9f4ad44b26 target/tcm_fc: fix the lockdep warning due to inconsistent lock state
The lockdep warning below is in theory correct but it will be in really weird
rare situation that ends up that deadlock since the tcm fc session is hashed
based the rport id. Nonetheless, the complaining below is about rcu callback
that does the transport_deregister_session() is happening in softirq, where
transport_register_session() that happens earlier is not. This triggers the
lockdep warning below. So, just fix this to make lockdep happy by disabling
the soft irq before calling transport_register_session() in ft_prli.

BTW, this was found in FCoE VN2VN over two VMs, couple of create and destroy
would get this triggered.

Roland Dreier 9c58b7ddd7 target: Simplify fabric sense data length handling
Every fabric driver has to supply a se_tfo->set_fabric_sense_len()
method, just so iSCSI can return an offset of 2.  However, every fabric
driver is already allocating a sense buffer and passing it into the
target core, either via transport_init_se_cmd() or target_submit_cmd().

So instead of having iSCSI pass the start of its sense buffer into the
core and then later tell the core to skip the first 2 bytes, it seems
easier for iSCSI just to do the offset of 2 when it passes the sense
buffer into the core.  Then we can drop the se_tfo->set_fabric_sense_len()
everywhere, and just add a couple of lines of code to iSCSI to set the
sense data length to the beginning of the buffer right before it sends
it over the network.

Mark Rustad 1fa8f45041 tcm_fc: Avoid debug overhead when not debugging
Stop doing a pile of work related to debugging messages when
the ft_debug_logging flag is not set. Use unlikely to add the
check in a way that the check can be inlined without inlining the
whole thing.

Roland Dreier d6dfc868bc target: Allow for target_submit_cmd() returning errors
We want it to be possible for target_submit_cmd() to return errors up
to its fabric module callers.  For now just update the prototype to
return an int, and update all callers to handle non-zero return values
as an error.

This is immediately useful for tcm_qla2xxx to fix a long-standing active
I/O session shutdown race, but tcm_fc, usb-gadget, and sbp-target the
fabric maintainers need to check + ACK that handling a target_submit_cmd()
failure due to session shutdown does not introduce regressions

(nab: Respin against for-next after initial NACK + update docbook comment +
      fix double se_cmd init in exception path for usb-gadget)

Christoph Hellwig b8b22533fe tcm_fc: Offload WRITE I/O backend submission to tpg workqueue
Defer the write processing to the internal to be able to use
target_execute_cmd.  I'm not even entirely sure the calling code requires
this due to the convoluted structure in libfc, but let's be safe for now.
