Move the guest enter/exit wrappers to kvm_host.h so that KVM can manage
its context tracking vs. vtime accounting without bleeding too many KVM
details into the context tracking code.

Sean Christopherson 6f922b89e5 sched/vtime: Move guest enter/exit vtime accounting to vtime.h
Provide separate helpers for guest enter vtime accounting (in addition to
the existing guest exit helpers), and move all vtime accounting helpers
to vtime.h where the existing #ifdef infrastructure can be leveraged to
better delineate the different types of accounting.  This will also allow
future cleanups via deduplication of context tracking code.

Opportunstically delete the vtime_account_kernel() stub now that all
callers are wrapped with CONFIG_VIRT_CPU_ACCOUNTING_NATIVE=y.

No functional change intended.

Wanpeng Li 88d8220bbf context_tracking: Move guest exit vtime accounting to separate helpers
Provide separate vtime accounting functions for guest exit instead of
open coding the logic within the context tracking code.  This will allow
KVM x86 to handle vtime accounting slightly differently when using
tick-based accounting.

Frederic Weisbecker 179a9cf792 context_tracking: Don't implement exception_enter/exit() on CONFIG_HAVE_CONTEXT_TRACKING_OFFSTACK
The typical steps with context tracking are:

1) Task runs in userspace
2) Task enters the kernel (syscall/exception/IRQ)
3) Task switches from context tracking state CONTEXT_USER to
   CONTEXT_KERNEL (user_exit())
4) Task does stuff in kernel
5) Task switches from context tracking state CONTEXT_KERNEL to
   CONTEXT_USER (user_enter())
6) Task exits the kernel

If an exception fires between 5) and 6), the pt_regs and the context
tracking disagree on the context of the faulted/trapped instruction.
CONTEXT_KERNEL must be set before the exception handler, that's
unconditional for those handlers that want to be able to call into
schedule(), but CONTEXT_USER must be restored when the exception exits
whereas pt_regs tells that we are resuming to kernel space.

This can't be fixed with storing the context tracking state in a per-cpu
or per-task variable since another exception may fire onto the current
one and overwrite the saved state. Also the task can schedule. So it
has to be stored in a per task stack.

This is how exception_enter()/exception_exit() paper over the problem:

5) Task switches from context tracking state CONTEXT_KERNEL to
   CONTEXT_USER (user_enter())
5.1) Exception fires
5.2) prev_state = exception_enter() // save CONTEXT_USER to prev_state
                                    // and set CONTEXT_KERNEL
5.3) Exception handler
5.4) exception_enter(prev_state) // restore CONTEXT_USER
5.5) Exception resumes
6) Task exits the kernel

The condition to live without exception_enter()/exception_exit() is to
forbid exceptions and IRQs between 2) and 3) and between 5) and 6), or if
any is allowed to trigger, it won't call into context tracking, eg: NMIs,
and it won't schedule. These requirements are met by architectures
supporting CONFIG_HAVE_CONTEXT_TRACKING_OFFSTACK and those can
therefore afford not to implement this hack.

Linus pointed out that compiler.h - which is a key header that gets included in every
single one of the 28,000+ kernel files during a kernel build - was bloated in:

  655389666643: ("vmlinux.lds.h: Create section for protection against instrumentation")

Linus noted:

 > I have pulled this, but do we really want to add this to a header file

Move these primitives into a new header: <linux/instrumentation.h>, and include that
header in the headers that make use of it.

Unfortunately one of these headers is asm-generic/bug.h, which does get included
in a lot of places, similarly to compiler.h. So the de-bloating effect isn't as
good as we'd like it to be - but at least the interfaces are defined separately.

No change to functionality intended.

Thomas Gleixner 0372007f5a context_tracking: Ensure that the critical path cannot be instrumented
context tracking lacks a few protection mechanisms against instrumentation:

 - While the core functions are marked NOKPROBE they lack protection
   against function tracing which is required as the function entry/exit
   points can be utilized by BPF.

 - static functions invoked from the protected functions need to be marked
   as well as they can be instrumented otherwise.

 - using plain inline allows the compiler to emit traceable and probable
   functions.

Fix this by marking the functions noinstr and converting the plain inlines
to __always_inline.

The NOKPROBE_SYMBOL() annotations are removed as the .noinstr.text section
is already excluded from being probed.

Cures the following objtool warnings:

 vmlinux.o: warning: objtool: enter_from_user_mode()+0x34: call to __context_tracking_exit() leaves .noinstr.text section

Standardize the naming on top of the vtime_accounting_enabled_*() base.
Also make it clear we are checking the vtime state of the
*current* CPU with this function. We'll need to add an API to check that
state on remote CPUs as well, so we must disambiguate the naming.

Frederic Weisbecker f83eeb1a01 sched/cputime: Rename vtime_account_system() to vtime_account_kernel()
vtime_account_system() decides if we need to account the time to the
system (__vtime_account_system()) or to the guest (vtime_account_guest()).

So this function is a misnomer as we are on a higher level than
"system". All we know when we call that function is that we are
accounting kernel cputime. Whether it belongs to guest or system time
is a lower level detail.

Rename this function to vtime_account_kernel(). This will clarify things
and avoid too many underscored vtime_account_system() versions.

Greg Kroah-Hartman b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   and resulted in the first patch in this series.

   If that file was a */uapi/* path one, it was "GPL-2.0 WITH

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

Linus Torvalds 221bb8a46e - ARM: GICv3 ITS emulation and various fixes. Removal of the old
VGIC implementation.
 
 - s390: support for trapping software breakpoints, nested virtualization
 (vSIE), the STHYI opcode, initial extensions for CPU model support.
 
 - MIPS: support for MIPS64 hosts (32-bit guests only) and lots of cleanups,
 preliminary to this and the upcoming support for hardware virtualization
 extensions.
 
 - x86: support for execute-only mappings in nested EPT; reduced vmexit
 latency for TSC deadline timer (by about 30%) on Intel hosts; support for
 more than 255 vCPUs.
 
 - PPC: bugfixes.
 
 The ugly bit is the conflicts.  A couple of them are simple conflicts due
 to 4.7 fixes, but most of them are with other trees. There was definitely
 too much reliance on Acked-by here.  Some conflicts are for KVM patches
 where _I_ gave my Acked-by, but the worst are for this pull request's
 patches that touch files outside arch/*/kvm.  KVM submaintainers should
 probably learn to synchronize better with arch maintainers, with the
 latter providing topic branches whenever possible instead of Acked-by.
 This is what we do with arch/x86.  And I should learn to refuse pull
 requests when linux-next sends scary signals, even if that means that
 submaintainers have to rebase their branches.
 
 Anyhow, here's the list:
 
 - arch/x86/kvm/vmx.c: handle_pcommit and EXIT_REASON_PCOMMIT was removed
 by the nvdimm tree.  This tree adds handle_preemption_timer and
 EXIT_REASON_PREEMPTION_TIMER at the same place.  In general all mentions
 of pcommit have to go.
 
 There is also a conflict between a stable fix and this patch, where the
 stable fix removed the vmx_create_pml_buffer function and its call.
 
 - virt/kvm/kvm_main.c: kvm_cpu_notifier was removed by the hotplug tree.
 This tree adds kvm_io_bus_get_dev at the same place.
 
 - virt/kvm/arm/vgic.c: a few final bugfixes went into 4.7 before the
 file was completely removed for 4.8.
 
 - include/linux/irqchip/arm-gic-v3.h: this one is entirely our fault;
 this is a change that should have gone in through the irqchip tree and
 pulled by kvm-arm.  I think I would have rejected this kvm-arm pull
 request.  The KVM version is the right one, except that it lacks
 GITS_BASER_PAGES_SHIFT.
 
 - arch/powerpc: what a mess.  For the idle_book3s.S conflict, the KVM
 tree is the right one; everything else is trivial.  In this case I am
 not quite sure what went wrong.  The commit that is causing the mess

Paolo Bonzini 2e9d1e150a x86/entry: Avoid interrupt flag save and restore
Thanks to all the work that was done by Andy Lutomirski and others,
enter_from_user_mode() and prepare_exit_to_usermode() are now called only with
interrupts disabled.  Let's provide them a version of user_enter()/user_exit()
that skips saving and restoring the interrupt flag.

On an AMD-based machine I tested this patch on, with force-enabled
context tracking, the speed-up in system calls was 90 clock cycles or 6%,
measured with the following simple benchmark:

Paolo Bonzini d0e536d893 context_tracking: avoid irq_save/irq_restore on guest entry and exit
guest_enter and guest_exit must be called with interrupts disabled,
since they take the vtime_seqlock with write_seq{lock,unlock}.
Therefore, it is not necessary to check for exceptions, nor to
save/restore the IRQ state, when context tracking functions are
called by guest_enter and guest_exit.

Split the body of context_tracking_entry and context_tracking_exit
out to __-prefixed functions, and use them from KVM.

Rik van Riel has measured this to speed up a tight vmentry/vmexit
loop by about 2%.

Frederic Weisbecker fafe870f31 context_tracking: Inherit TIF_NOHZ through forks instead of context switches
TIF_NOHZ is used by context_tracking to force syscall slow-path
on every task in order to track userspace roundtrips. As such,
it must be set on all running tasks.

It's currently explicitly inherited through context switches.
There is no need to do it in this fast-path though. The flag
could simply be set once for all on all tasks, whether they are
running or not.

Lets do this by setting the flag for the init task on early boot,
and let it propagate through fork inheritance.

While at it, mark context_tracking_cpu_set() as init code, we
only need it at early boot time.

Rik van Riel 126a6a5424 kvm,rcu,nohz: use RCU extended quiescent state when running KVM guest
The host kernel is not doing anything while the CPU is executing
a KVM guest VCPU, so it can be marked as being in an extended
quiescent state, identical to that used when running user space
code.

The only exception to that rule is when the host handles an
interrupt, which is already handled by the irq code, which
calls rcu_irq_enter and rcu_irq_exit.

The guest_enter and guest_exit functions already switch vtime
accounting independent of context tracking. Leave those calls
where they are, instead of moving them into the context tracking
code.

Rik van Riel 3aab4f50bf context_tracking: Generalize context tracking APIs to support user and guest
Generalize the context tracking APIs to support various nature of
contexts. This is performed by splitting out the mechanism from
context_tracking_user_enter and context_tracking_user_exit into
context_tracking_enter and context_tracking_exit.

The nature of the context we track is now detailed in a ctx_state
parameter pushed to these APIs, allowing the same functions to not just
track kernel <> user space switching, but also kernel <> guest transitions.

But leave the old functions in order to avoid breaking ARM, which calls
these functions from assembler code, and cannot easily use C enum
parameters.

Current context tracking symbols are designed to express living state.
As such they are prefixed with "IN_": IN_USER, IN_KERNEL.

Now we are going to use these symbols to also express state transitions
such as context_tracking_enter(IN_USER) or context_tracking_exit(IN_USER).
But while the "IN_" prefix works well to express entering a context, it's
confusing to depict a context exit: context_tracking_exit(IN_USER)
could mean two things:
	1) We are exiting the current context to enter user context.
	2) We are exiting the user context
We want 2) but the reviewer may be confused and understand 1)

So lets disambiguate these symbols and rename them to CONTEXT_USER and
CONTEXT_KERNEL.

Frederic Weisbecker e7358b3bc0 context_tracking: Split low level state headers
We plan to use the context tracking static key on inline
vtime APIs. For this we need to include the context tracking
headers from those of vtime.

However vtime headers need to stay low level because they are
included in hardirq.h that mostly contains standalone
definitions. But context_tracking.h includes sched.h for
a few task_struct references, therefore it wouldn't be sensible
to include it from vtime.h

To solve this, lets split the context tracking headers and move
out the pure state definitions that only require a few low level
headers. We can safely include that small part in vtime.h later.

Now what we want in practice is to enable the tracking on full
dynticks CPUs only. In order to perform this, we just need to pass
our full dynticks CPU range selection from the full dynticks
subsystem to the context tracking.

This way we can spare the overhead of RCU user extended quiescent
state and vtime maintainance on the CPUs that are outside the
full dynticks range. Just keep in mind the raw context tracking
itself is still necessary everywhere.

Frederic Weisbecker 2d854e5738 context_tracing: Fix guest accounting with native vtime
1) If context tracking is enabled with native vtime accounting (which
combo is useless except for dev testing), we call vtime_guest_enter()
and vtime_guest_exit() on host <-> guest switches. But those are stubs
in this configurations. As a result, cputime is not correctly flushed
on kvm context switches.

2) If context tracking runs but is disabled on some CPUs, those
CPUs end up calling __guest_enter/__guest_exit which in turn
call vtime_account_system(). We don't want to call this because we
run in tick based accounting for these CPUs.

Refactor the guest_enter/guest_exit code such that all combinations
finally work.

Frederic Weisbecker 521921bad1 kvm: Move guest entry/exit APIs to context_tracking
The kvm_host.h header file doesn't handle well
inclusion when archs don't support KVM.

This results in build crashes for such archs when they
want to implement context tracking because this subsystem
includes kvm_host.h in order to implement the
guest_enter/exit APIs but it doesn't handle KVM off case.

To fix this, move the guest_enter()/guest_exit()
declarations and generic implementation to the context
tracking headers. These generic APIs actually belong to
this subsystem, besides other domains boundary tracking
like user_enter() et al.

KVM now properly becomes a user of this library, not the
other buggy way around.

Frederic Weisbecker 6c1e0256fa context_tracking: Restore correct previous context state on exception exit
On exception exit, we restore the previous context tracking state based on
the regs of the interrupted frame. Iff that frame is in user mode as
stated by user_mode() helper, we restore the context tracking user mode.

However there is a tiny chunck of low level arch code after we pass through
user_enter() and until the CPU eventually resumes userspace.
If an exception happens in this tiny area, exception_enter() correctly
exits the context tracking user mode but exception_exit() won't restore
it because of the value returned by user_mode(regs).

As a result we may return to userspace with the wrong context tracking
state.

To fix this, change exception_enter() to return the context tracking state
prior to its call and pass this saved state to exception_exit(). This restores
the real context tracking state of the interrupted frame.

(May be this patch was suggested to me, I don't recall exactly. If so,
sorry for the missing credit).

Frederic Weisbecker 56dd9470d7 context_tracking: Move exception handling to generic code
Exceptions handling on context tracking should share common
treatment: on entry we exit user mode if the exception triggered
in that context. Then on exception exit we return to that previous
context.

Generalize this to avoid duplication across archs.

Create a new subsystem that probes on kernel boundaries
to keep track of the transitions between level contexts
with two basic initial contexts: user or kernel.

This is an abstraction of some RCU code that use such tracking
to implement its userspace extended quiescent state.

We need to pull this up from RCU into this new level of indirection
because this tracking is also going to be used to implement an "on
demand" generic virtual cputime accounting. A necessary step to
shutdown the tick while still accounting the cputime.
