Pull percpu consistent-ops changes from Tejun Heo:
"Way back, before the current percpu allocator was implemented, static
and dynamic percpu memory areas were allocated and handled separately
and had their own accessors. The distinction has been gone for many
years now; however, the now duplicate two sets of accessors remained
with the pointer based ones - this_cpu_*() - evolving various other
operations over time. During the process, we also accumulated other
inconsistent operations.
This pull request contains Christoph's patches to clean up the
duplicate accessor situation. __get_cpu_var() uses are replaced with
with this_cpu_ptr() and __this_cpu_ptr() with raw_cpu_ptr().
Unfortunately, the former sometimes is tricky thanks to C being a bit
messy with the distinction between lvalues and pointers, which led to
a rather ugly solution for cpumask_var_t involving the introduction of
this_cpu_cpumask_var_ptr().
This converts most of the uses but not all. Christoph will follow up
with the remaining conversions in this merge window and hopefully
remove the obsolete accessors"

Replace duplicated code in all transports with a single wrapper in
virtio.c.
The only functional change is in virtio_mmio.c: if a buggy device sends
us an interrupt before driver is set, we previously returned IRQ_NONE,
now we return IRQ_HANDLED.
As this must not happen in practice, this does not look like a big deal.

The kernel used to contain two functions for length-delimited,
case-insensitive string comparison, strnicmp with correct semantics and
a slightly buggy strncasecmp. The latter is the POSIX name, so strnicmp
was renamed to strncasecmp, and strnicmp made into a wrapper for the new
strncasecmp to avoid breaking existing users.
To allow the compat wrapper strnicmp to be removed at some point in the
future, and to avoid the extra indirection cost, do
s/strnicmp/strncasecmp/g.

With this patch for kdump the s390 vector registers are stored into the
prepared save areas in the old kernel and into the REGSET_VX_LOW and
REGSET_VX_HIGH ELF notes for /proc/vmcore in the new kernel.
The NT_S390_VXRS_LOW note contains the lower halves of the first 16 vector
registers 0-15. The higher halves are stored in the floating point register
ELF note. The NT_S390_VXRS_HIGH contains the full vector registers 16-31.
The kernel provides a save area for storing vector register in case of
machine checks. A pointer to this save are is stored in the CPU lowcore
at offset 0x11b0. This save area is also used to save the registers for
kdump. In case of a dumped crashed kdump those areas are used to extract
the registers of the production system.
The vector registers for remote CPUs are stored using the "store additional
status at address" SIGP. For the dump CPU the vector registers are stored
with the VSTM instruction.
With this patch also zfcpdump stores the vector registers.

Move the nohz_delay bit from the s390_idle data structure to the
per-cpu flags. Clear the nohz delay flag in __cpu_disable and
remove the cpu hotplug notifier that used to do this.

Error recovery requests may not be cleaned up correctly so that other
needed erp requests can not be build because of insufficient memory.

Testing xmit_more support with netperf and connected UDP sockets,
I found strange dst refcount false sharing.
Current handling of IFF_XMIT_DST_RELEASE is not optimal.
Dropping dst in validate_xmit_skb() is certainly too late in case
packet was queued by cpu X but dequeued by cpu Y
The logical point to take care of drop/force is in __dev_queue_xmit()
before even taking qdisc lock.
As Julian Anastasov pointed out, need for skb_dst() might come from some
packet schedulers or classifiers.
This patch adds new helper to cleanly express needs of various drivers
or qdiscs/classifiers.
Drivers that need skb_dst() in their ndo_start_xmit() should call
following helper in their setup instead of the prior :

Instead of using a single bit, we use two bits, one being
eventually rebuilt in bonding/team drivers.
The other one, is permanent and blocks IFF_XMIT_DST_RELEASE being
rebuilt in bonding/team. Eventually, we could add something
smarter later.

Clear QUEUE_FLAG_ADD_RANDOM in all block drivers that set
QUEUE_FLAG_NONROT.
Historically, all block devices have automatically made entropy
contributions. But as previously stated in commit e2e1a148 ("block: add
sysfs knob for turning off disk entropy contributions"):
- On SSD disks, the completion times aren't as random as they
are for rotational drives. So it's questionable whether they
should contribute to the random pool in the first place.
- Calling add_disk_randomness() has a lot of overhead.
There are more reliable sources for randomness than non-rotational block
devices. From a security perspective it is better to err on the side of
caution than to allow entropy contributions from unreliable "random"
sources.

The event type masks can change asynchronously. These changes are reported
by SCLP to the OS by state-change events which are retrieved with the read
event data command. The SCLP driver has a request queue, there is a window
where the read event data request has not completed yet but the SCLP console
drivers are trying to queue output requests. As the masks are not updated
yet the requests are discarded.
The simplest fix is to queue the console requests independent of the
event type masks and rely on SCLP to return with an error code if a
specific event type is not available.

When qeth device is queried for ethtool data, hardware operation
is performed to extract the necessary information from the card.
If the card is not online at the moment (e.g. it is undergoing
recovery), this operation produces undesired effects like
temporarily freezing the system. This patch prevents execution
of the hardware query operation when the card is not online.
In such case, ioctl() operation returns error with errno ENODEV.

surfaced a bug in the 3215 device driver. In combination this
broke tab expansion for tty ouput.
The cause is an asymmetry in the behaviour of tty3215_ops->write
vs tty3215_ops->put_char. The put_char function scans for '\t'
but the write function does not.
As the driver has logic for the '\t' expansion remove XTABS
from c_oflag of the initial termios as well.

drives), plus an assortment of minor updates and bug fixes.

The ccw_device_start in raw3215_start_io can fail. raw3215_try_io
does not check if the request could be started and removes any
pending timer. This can leave the system in a hanging state.
Check for pending request after raw3215_start_io and start a
timer if necessary.

Kernel panic or a hanging device during format if an alias device is
set offline or I/O errors occur.
Omit the error recovery procedure for alias devices and do retries on
the base device with full erp.

Because of the embedded qdio_buffer array struct qeth_qdio_out_q is
quite large resulting in an order 4 allocation. This is likely to
fail at runtime and wastes a lot of memory since the actual size is
just about 34K.
Since there is no need for this buffer to be contiguous split it up
using qdio buffer helpers.

Users of qdio buffers employ different strategies to manage these
buffers. The qeth driver uses huge contiguous buffers which leads
to high order allocations with all their downsides.
This patch provides helpers to allocate, free, and reset arrays of
qdio buffers using non contiguous pages.

The size detection for 3270 terminals with the read-partition command is
broken. The raw3270_reset_device_cb function clears the init_data array,
but if raw3270_writesf_readpart has been called the read-partition command
is queued which needs the init_data array. In this case the size detection
will fail and the invalid command does funny things to the terminal.

device_schedule_callback. This function obtained an extra device
reference for its async work which is missing in the new implementation
and results in a "freeing memory with a lock still held" BUG. Fix

Currently we set the device name at the time we call device_add after
we receive the interrupt for the first I/O. When something is not working
as expected during that first I/O (e.g. we don't receive an interrupt) we
print a message including the device name which has not yet been
initialized.
Set the device name after calling device_initialize (prior to starting
the first I/O) so that we have the name present if some unexpected error
occurs during that first I/O.

When you want to use the HMC's ASCII console as console device for
a z/VM guest you have to specify console=ttyS1 on the kernel command
line. But it won't work until you specify conmode=sclp as well.
This behavior is inconsistent with the use of the ASCII console as
TTY device which works on z/VM without the need to specify a conmode.

On systems where a ccw based console device is used a lockdep false alarm
could be triggered when a device driver calls printk while holding a
subchannels lock (e.g. in it's irq handler). Since this is valid behavior
fix this by introducing a separate lock class for the console subchannels
lock.
The lockdep warning was revealed by "printk: enable interrupts before calling
console_trylock_for_printk()" which changed console_unlock() to be called with
lockdep enabled.

Change the visibility of the module parameters ap_domain_index and
ap_thread_flag for the owner and the members of the owners group in
sysfs.
Previously the parameters where invisible due to a value of zero

Currently there are two s390 kernel dump config options "CONFIG_ZFCPDUMP"
and "CONFIG_CRASH_DUMP". In order to keep things simple and because the
"CONFIG_ZFCPDUMP" option already has a dependency to "CONFIG_CRASH_DUMP"
remove the CONFIG_ZFCPDUMP option.

The original bootmem allocator is getting replaced by memblock. To
cover the needs of the s390 kdump implementation the physical memory
list is used.
With this patch the bootmem allocator and its bitmaps are completely
removed from s390.

The netlink conflict dealt with moving to netlink_capable() and

the now partially defunct TOS field. This patch adds two new options as
follows:
- prio_queueing_skb: bases priority queueing on skb-priority
- prio_queueing_vlan: uses the priority code point in the 802.1Q header

The default queue for queueing modes prio_queueing_prec and
prio_queueing_tos is supposed to be QETH_DEFAULT_QUEUE. However, neither
mode will reset the default default_out_queue value when enabled, leaving
a previously set value activate as the default.
E.g. enabling one of the fixed queueing modes, e.g. no_prio_queueing:0,
and successively switching to any of the prio_queueing_* modes will
leave default_out_queue at the previous (wrong) value 0.

When a device is lost, the common I/O layer calls the notification
handler with CIO_GONE: In that event, flag device_lost as true.
In case the device had been flagged as lost when the remove/offline callbacks
are called, call the new virtio_break_device() function prior to invoking
device_unregister(). This avoids hangs of I/O triggered via the device
unregistration callbacks.

Using a notification type mask for the store event information chsc
is unsupported on some firmware levels. Retry SEI with that mask set
to zero (which is the old way of requesting only channel subsystem
related events).

A kernel panic might occur when there is terminal input available via
the SCLP VT220 interface at an early time during the boot process.
The processing of terminal input requires prior initialization which is
done via an early_initcall function (init_workqueues) while the SCLP
VT220 driver registers for terminal input during a console_initcall
function (sclp_vt220_con_init). When there is terminal input available
via the SCLP interface between console_initcall and early_initcall, a
null pointer dereference occurs (system_wq is null).
Fix this problem by moving the registration for terminal input to a
device_initcall function (sclp_vt220_tty_init).

"PPC and ARM do not have much going on this time. Most of the cool
stuff, instead, is in s390 and (after a few releases) x86.
ARM has some caching fixes and PPC has transactional memory support in
guests. MIPS has some fixes, with more probably coming in 3.16 as
QEMU will soon get support for MIPS KVM.
For x86 there are optimizations for debug registers, which trigger on
some Windows games, and other important fixes for Windows guests. We
now expose to the guest Broadwell instruction set extensions and also
Intel MPX. There's also a fix/workaround for OS X guests, nested
virtualization features (preemption timer), and a couple kvmclock

This patch adds a timeout option for queued requests and introduces
sclp_sync_request_timeout() to use this timer. With this, blocking the
system too long, e.g. during an SE reboot, can be avoided in critical
situations like CPU and memory hotplug.
Since there is no way to cancel a running request, this timeout only
applies to queued requests that have not yet been started.

del_timer() does not wait for a possible running callback to
complete. So the call side might free request and the associated
objects while on another cpu the timer handler runs.

If the 3270 device is detached the initial reset device request will
stays pending until the device is operational. A second reset device
call will reuse the same request structure which will cause an oops.
Add a check to see if the reset device request is already pending
and do nothing in this case.

This patch adds a parameter 'auto_update' to the con3270
driver, causing the 'auto_update' feature to be disabled
if unset.
The 'auto_update' feature will cause the con3270 driver
to switch to the console view whenever new system messages
are displayed, which makes working on the 3270 terminal
awkward.

The current code would always return 0 for VLAN-encapsulated IP traffic.
One notable side effect was that VLAN traffic would never get prioritized
on OSD and OSX devices when priority queueing modes prio_queueing_tos or
prio_queueing_prec were enabled.

Limit the number of bits to the maximum number of cpus a machine
can have.
possible_cpu_mask typically will have more bits set than a machine
may physically have. This results in wasted memory during per-cpu
memory allocations, if the possible mask contains more cpus than
physically possible for a given configuration.

The architectures that override cputime_t (s390, ppc) don't provide
any version of nsecs_to_cputime(). Indeed this cputime_t implementation
by backend only happens when CONFIG_VIRT_CPU_ACCOUNTING_NATIVE=y under
which the core code doesn't make any use of nsecs_to_cputime().
At least for now.
We are going to make a broader use of it so lets provide a default
version with a per usecs granularity. It should be good enough for most
usecases.

One fix for virtio-ccw, fixing a problem introduced with
"virtio_ccw: fix vcdev pointer handling issues" and noticed just
after it went into git.
During set offline processing virtio_grab_drvdata() incorrectly
calls dev_set_drvdata() to remove the virtio_ccw_device from the
parent ccw_device's driver data. This is wrong and ends up in a
hang during virtio_ccw_reset(), as the interrupt handler still
has need of the virtio_ccw_device.
A new field 'going_away' is introduced in struct virtio_ccw_device
to control the usage of the ccw_device's driver data pointer in
virtio_grab_drvdata().

Implement the new CCW_CMD_SET_IND_ADAPTER command and try to enable
adapter interrupts for every device on the first startup. If the host
does not support adapter interrupts, fall back to normal I/O interrupts.
virtio-ccw adapter interrupts use the same isc as normal I/O subchannels
and share a summary indicator for all devices sharing the same indicator
area.
Indicator bits for the individual virtqueues may be contained in the same
indicator area for different devices.

The interrupt handler virtio_ccw_int_handler() using the vcdev pointer
is protected by the ccw_device lock. Resetting the pointer within the
ccw_device structure should be done when holding this lock.
Also resetting the vcdev pointer (under the ccw_device lock) prior to
freeing the vcdev pointer memory removes a critical path.

To guarantee that a qdio ccw_device no longer touches the
qdio memory shared with Linux, the qdio ccw_device should
be offline when freeing the qdio memory. Thus this patch
postpones freeing of qdio memory.

Currently we return a negative block count if SCLP returns HSA block
count zero. The reason is that we subtract one for the header page.
So fix this and correctly return zero block count if SCLP returns zero.

The maximum usable buffer size of the s390 debug feature (when using
the sprintf_view) is 11 * sizeof(long) (1 pointer for the format
string + 10 arguments). When a larger buffer size is specified the
additional memory is unused and wasted per debug entry. So reducing
the buffer size to its maximum (or to the actual buffer size used)
will make more precious debug feature space usable.
For pci_msg, chsc_msg, and cio_crw we use the additional usable dbf
space to reduce the number of allocated pages.

Interrupts which happen on ccw consoles prior to their registration
with the driver core are not accounted to the respective device
driver. Fix this by setting the proper interrupt class during
initialization of ccw consoles.

Drivers for ccw consoles use ccw_device_probe_console to receive
an initialized ccw device which is already enabled for interrupts.
After that the device driver does the initialization of its private
data. This can race with unsolicited interrupts which can happen
once the device is enabled for interrupts.
Split ccw_device_probe_console into ccw_device_create_console and
ccw_device_enable_console and reorder the initialization of the ccw
console drivers.
While at it mark these functions as __init.

ccw consoles are in use before they can be properly registered with
the driver core. For devices which are in use by a device driver we
rely on the ccw_device's pointer to the driver callbacks to be valid.
For ccw consoles this pointer is NULL until they are registered later
during boot and we dereferenced this pointer. This worked by
chance on 64 bit builds (cdev->drv was NULL but the optional callback
cdev->drv->path_event was also NULL by coincidence) and was unnoticed
until we received reports about boot failures on 31 bit systems.
Fix it by initializing the driver pointer for ccw consoles.

Performing a Channel-Path configure on operation on a Channel-Path ID
(CHPID) does not trigger a scan for subchannels that might have become
available through that CHPID. As a result, some subchannels and
associated I/O devices might be missing. Fix this by adding the missing
scan.
This problem was introduced by commit c820de39, "[S390] cio: Rework
css driver.", but wasn't noticed earlier because the machine usually
also generates a Channel-Report-Word when the first CHPID of a
subchannel is configured on, resulting in a separate scan for that
subchannel. The problem only becomes apparent when this first CHPID is
not working properly and additional working CHPIDs are subsequently
configured on without any effect on the availability of the affected
subchannel.

driver-core now supports synchrnous self-deletion of attributes and
the asynchrnous removal mechanism is scheduled for removal. Use it
instead of device_schedule_callback().
* Conversions in arch/s390/pci/pci_sysfs.c and
drivers/s390/block/dcssblk.c are straightforward.
* drivers/s390/cio/ccwgroup.c is a bit more tricky because
ccwgroup_notifier() was (ab)using device_schedule_callback() to
purely obtain a process context to kick off ungroup operation which
may block from a notifier callback.
Rename ccwgroup_ungroup_callback() to ccwgroup_ungroup() and make it
take ccwgroup_device * instead. The new function is now called
directly from ccwgroup_ungroup_store().
ccwgroup_notifier() chain is updated to explicitly bounce through
ccwgroup_device->ungroup_work. This also removes possible failure
from memory pressure.

The modify subchannel wrapper cio_commit_config can fail when
(unexpected) status is pending on the subchannel.
Callers of cio_commit_config (that operated on enabled subchannels)
needed to do error handling for that case (clear the unexpected
status with test subchannel and retry). This error handling is
missing in some code paths and caused online setting of devices to
fail.
Fix this for all callers by moving the error handling inside
cio_commit_config.

I observed that there are for_each macros that do an extra memory access
beyond the defined area.
Normally this does not cause problems.
But, this can cause exceptions. For example: if the area is allocated at
the end of a page and the next page is not accessible.
For correctness, I suggest changing the arguments of the 'for loop' like
others 'for_each' do in the kernel.

During parsing of the sizes array the pointer to the particular
string is lost. Keep it by using an extra pointer to store the end
position of the parsed string. Keeping these parameters accessible
can be helpful for debugging purposes and for userspace reading
the parameters at runtime via sysfs. Also this will ensure that the
memory is freed at module unload time.

A command reject for a ccw may happen if we run on a host not supporting
a certain feature. We want to be able to handle this as special case of
command failure, so let's split this off from the generic -EIO error code.

The tty3270_alloc_screen function is called from tty3270_install with
swapped arguments, the number of columns instead of rows and vice versa.
The number of rows is typically smaller than the number of columns which
makes the screen array too big but the individual cell arrays for the
lines too small. Creating lines longer than the number of rows will
clobber the memory after the end of the cell array.
The fix is simple, call tty3270_alloc_screen with the correct argument
order.

The early sclp detect functions gather the available SCLP facility
information. The sclp_early_read_info_sccb_valid indicates whether the
early sclp request was valid. However, one external reference to it
checks for particular sclp facility bits and this should be sufficient.
Another occurance is in the sclp_get_ipl_info() function that is called
later. Because all information are available at the early stage, save
the ipl information when detecting the sclp facilities. Hence, no more
checks for sclp_early_read_info_sccb_valid are required.

The deactivation and freeing of the tty view of the 3270 device
can race with a tty3270_update invocation via the update timer.
To fix this move the del_timer_sync call for the update timer from
tty3270_free_view to tty3270_free prior to the tty3270_free_screen
call.

Subchannel looping function for_each_subchannel_staged() allocates a
subchannel-ID-bitmap to efficiently iterate over the list of known
and unknown subchannels. Since this function is also used to iterate
over known-subchannels only, optimize that case by not requiring the
ID-bitmap allocation and falling back to simple bus_for_each_dev()
looping.

The CIO layer scans for newly available I/O devices by performing a scan
of available subchannels using the Store Subchannel (STSCH) instruction.
Performing too many STSCH instructions in a tight loop can cause high
Hypervisor overhead which can negatively impact the performance of the
virtual machine as a whole.
A subchannel scan is triggered for example during a hardware event that
indicates that a channel path has become available. It is also triggered
by the DASD device driver for each device that is set online.
This patch reduces the number of STSCH instructions being performed by
delaying the start of the actual subchannel scan by 1 second. Multiple
scan requests that are scheduled during this time will be merged into a
single scan loop.
The trade-off consists of a short delay that is introduced between
the time that the event is processed and a newly available device
becoming usable. This delay should be acceptable since it only
affects devices that have not been in use before.

The CIO layer scans for newly available I/O devices by performing a scan
of available subchannels using the Store Subchannel (STSCH) instruction.
This processing can take a significant amount of time during which no
other task can run on the same CPU (unless CONFIG_PREEMPT has been
enabled). As a result, scheduling latencies for other tasks are
increased noticeably, especially on a single-CPU system.
Fix this problem by explicitly allowing other tasks to be scheduled
each time a subchannel has been processed.

The CIO layer processes hardware events that indicate that a channel
path has become available by performing a scan of available subchannels
using the Store Subchannel (STSCH) instruction. Performing too many
STSCH instructions in a tight loop can cause high Hypervisor overhead
which can negatively impact the performance of the virtual machine as
a whole.
This patch reduces the number of STSCH instructions performed while
processing a resource accessibility event and while varying a CHPID
online.
In both cases, Linux first performs a STSCH instruction on each unused
subchannel to see if the subchannel has become available. If the STSCH
instruction indicates that the subchannel is available, a full
evaluation of this subchannel is scheduled. Since the full evaluation
includes performing a STSCH instruction, the initial STSCH is
unnecessary and can be removed.

(7b50da53f6) replaced the sclp_event_mask_early()
with sclp_set_event_mask(). The early_event_mask_sccb variable is no longer
initialized but is still used in sclp_has_linemode() and sclp_has_vt220().
Replace early_event_mask_sccb with the sccb_early variable in both
functions.

After the call to del_gendisk, the gendisk still holds a reference to
its request_queue. We must not modify the gendisks queue pointer
before the put_disk call, or the gendisk_release function cannot
release the reference and the memory for the request_queue structure
is lost.

More prep work for immutable biovecs - with immutable bvecs drivers
won't be able to use the biovec directly, they'll need to use helpers
that take into account bio->bi_iter.bi_bvec_done.
This updates callers for the new usage without changing the
implementation yet.

An I/O request that does not read or write full blocks cannot be
translated into a correct CCW or TCW program and should be rejected
right away. In particular the code that creates TCW requests will not
notice this problem and create broken TCWs that will be rejected by
the hardware.

Stop hiding scm_block's dependency to the eadm subchannel driver
(by using functions provided by the eadm subchannel instead of
wrappers provided by the scm bus).
This will help userspace recognizing module dependencies (e.g. for
building a ramdisk). As a side effect we can get rid of some code
reimplementing refcounting between those modules.

Unloading the fs3270 kernel module does not remove the created
"3270/tub" device. Reloading the module then causes a sysfs warning:
"sysfs: cannot create duplicate filename '/devices/virtual/3270/3270!tub'".
Call device_destroy() in the module exit function to solve this issue.

Pull networking updates from David Miller:
1) The addition of nftables. No longer will we need protocol aware
firewall filtering modules, it can all live in userspace.
At the core of nftables is a, for lack of a better term, virtual
machine that executes byte codes to inspect packet or metadata
(arriving interface index, etc.) and make verdict decisions.
Besides support for loading packet contents and comparing them, the
interpreter supports lookups in various datastructures as
fundamental operations. For example sets are supports, and
therefore one could create a set of whitelist IP address entries
which have ACCEPT verdicts attached to them, and use the appropriate
byte codes to do such lookups.
Since the interpreted code is composed in userspace, userspace can
do things like optimize things before giving it to the kernel.
Another major improvement is the capability of atomically updating
portions of the ruleset. In the existing netfilter implementation,
one has to update the entire rule set in order to make a change and
this is very expensive.
Userspace tools exist to create nftables rules using existing
netfilter rule sets, but both kernel implementations will need to
co-exist for quite some time as we transition from the old to the
new stuff.
Kudos to Patrick McHardy, Pablo Neira Ayuso, and others who have
worked so hard on this.
2) Daniel Borkmann and Hannes Frederic Sowa made several improvements
to our pseudo-random number generator, mostly used for things like
UDP port randomization and netfitler, amongst other things.
In particular the taus88 generater is updated to taus113, and test
cases are added.
3) Support 64-bit rates in HTB and TBF schedulers, from Eric Dumazet
and Yang Yingliang.
4) Add support for new 577xx tigon3 chips to tg3 driver, from Nithin
Sujir.
5) Fix two fatal flaws in TCP dynamic right sizing, from Eric Dumazet,
Neal Cardwell, and Yuchung Cheng.
6) Allow IP_TOS and IP_TTL to be specified in sendmsg() ancillary
control message data, much like other socket option attributes.

12) Allow stackability of segmentation offloads to, in particular, allow

various hash functions in the inet hashtables, TCP fast open, as

operation is "net_get_random_once()" which uses static keys.
Hannes even extended this to ipv4/ipv6 fragmentation handling and
our generic flow dissector.
14) The generic driver layer takes care now to set the driver data to
NULL on device removal, so it's no longer necessary for drivers to
explicitly set it to NULL any more. Many drivers have been cleaned
up in this way, from Jingoo Han.
15) Add a BPF based packet scheduler classifier, from Daniel Borkmann.
16) Improve CRC32 interfaces and generic SKB checksum iterators so that
SCTP's checksumming can more cleanly be handled. Also from Daniel
Borkmann.
17) Add a new PMTU discovery mode, IP_PMTUDISC_INTERFACE, which forces
using the interface MTU value. This helps avoid PMTU attacks,

changed the semantics of elevator_init() in a way that now enforces to hold

The while loop only peeks at the top request in the queue but does
not yet consume it. Since we only handle fs requests, we need to
dequeue and complete all other request command types with error
just in case we would ever receive such an unforeseen request.

Currently a host kick error is silently ignored and not reflected in
the virtqueue of a particular virtio device.
Changing the notify API for guest->host notification seems to be one
prerequisite in order to be able to handle such errors in the context
where the kick is triggered.
This patch changes the notify API. The notify function must return a
bool return value. It returns false if the host notification failed.

Using strcpy() is wrong, because it also copies the terminating null
byte, but in this case the extra copied null byte will be overwritten
right after the strcpy(), so there is no real problem here.
Use strncpy() to fix the warning.

find_first_bit_left() and friends have nothing to do with the normal
LSB0 bit numbering for big endian machines used in Linux (least
significant bit has bit number 0).
Instead they use MSB0 bit numbering, where the most signficant bit has
bit number 0. So rename find_first_bit_left() and friends to
find_first_bit_inv(), to avoid any confusion.
Also provide inv versions of set_bit, clear_bit and test_bit.
This also removes the confusing use of e.g. set_bit() in airq.c which
uses a "be_to_le" bit number conversion, which could imply that instead
set_bit_le() could be used. But that is entirely wrong since the _le
bitops variant uses yet another bit numbering scheme.

get_inbound_buffer_frontier() makes use of the return value of atomic_sub()
which shouldn't work, since atomic_sub() is supposed to return void.
This only works on s390 because atomic_sub() gets mapped to atomic_sub_return()
with a define without changing it's return value to void.
So use atomic_sub_return() instead of atomic_sub() in qeth code before fixing
atomic ops.

The result of the store-clock-fast (STCKF) instruction is a bit fuzzy.
It can happen that the value stored on one CPU is smaller than the value
stored on another CPU, although the order of the stores is the other
way around. This can cause deltas of get_tod_clock() values to become
negative when they should not be.
We need to be more careful with store-clock-fast, this patch partially
reverts git commit e4b7b4238e666682555461fa52eecd74652f36bb "time:
always use stckf instead of stck if available". The get_tod_clock()
function now uses the store-clock-extended (STCKE) instruction.
get_tod_clock_fast() can be used if the fuzziness of store-clock-fast
is acceptable e.g. for wait loops local to a CPU.

The prefix command is used instead of a define extent to make use of
PAV alias devices during format. On some older storage servers the
prefix command may not be available and the IO request will fail.
Check for availability of prefix command and use define extent if

To detect a line mode console we need a message event type
for the receive mask and a command event type for the send mask.

hardirqs. Which means that all current architectures have been
converted and that CONFIG_GENERIC_HARDIRQS can be removed. Martin
prepared a patch for that already (see genirq branch), but the best
time to merge that is probably at the end of the merge window / begin

Another patch converts s390 to software referenced bits instead of
relying on the reference bit in the storage key. Therefore s390
doesn't use storage keys anymore, except for kvm.

Enabling the data router support by default
can increase performance in certain situations.

When a recovery cqr is cleaned up, copy the start time, stop time,
and start device to the original cqr. These times are needed later
when the original request is finalized and counted in the DASD
statistics.

By popular demand, this patch brings back a couple of sysfs attributes
removed by commit 663e0890e3
"[SCSI] zfcp: remove access control tables interface".
The content has been irrelevant for years, but the files must be
there forever for whatever user space tools that may rely on them.
Since these files always return a constant value, a new stripped
down show-macro was required. Otherwise build warnings would have
been introduced.

Apparently, the ref_count for some scsi_device drops down to zero,
triggering device removal through execute_in_process_context(), while
the lldd error recovery thread iterates through a scsi device list.
Unfortunately, execute_in_process_context() decides to immediately
execute that device removal function, instead of scheduling asynchronous
execution, since it detects process context and thinks it is safe to do
so. But almost all calls to shost_for_each_device() in our lldd are
inside spin_lock_irq, even in thread context. Obviously, schedule()
inside spin_lock_irq sections is a bad idea.
Change the lldd to use the proper iterator function,
__shost_for_each_device(), in combination with required locking.
Occurences that need to be changed include all calls in zfcp_erp.c,
since those might be executed in zfcp error recovery thread context
with a lock held.
Other occurences of shost_for_each_device() in zfcp_fsf.c do not
need to be changed (no process context, no surrounding locking).
The problem was introduced in Linux 2.6.37 by commit
b62a8d9b45

This patch adds wait_event_interruptible_lock_irq_timeout(), which is a
straight-forward descendant of wait_event_interruptible_timeout() and
wait_event_interruptible_lock_irq().
The zfcp driver used to call wait_event_interruptible_timeout()
in combination with some intricate and error-prone locking. Using
wait_event_interruptible_lock_irq_timeout() as a replacement
nicely cleans up that locking.
This rework removes a situation that resulted in a locking imbalance
in zfcp_qdio_sbal_get():

It was introduced by commit c2af7545aa
"[SCSI] zfcp: Do not wait for SBALs on stopped queue", which had a new
code path related to ZFCP_STATUS_ADAPTER_QDIOUP that took an early exit
without a required lock being held. The problem occured when a
special, non-SCSI I/O request was being submitted in process context,
when the adapter's queues had been torn down. In this case the bug
surfaced when the Fibre Channel port connection for a well-known address
was closed during a concurrent adapter shut-down procedure, which is a
rare constellation.
This patch also fixes these warnings from the sparse tool (make C=1):

The ECKD protocol supports reading of tracks with arbitrary format as
raw track images. The DASD device driver supports this in its
raw_track_access mode. In this mode it maps each track to sixteen 4096
byte sectors and rejects all requests that are not properly aligned to
this mapping.
An application that wants to use a DASD in raw_track_access mode will
usually use direct I/O to make sure that properly aligned requests are
directly submitted to the driver. However, applications that are not
aware of this mode, e.g. udev, will encounter I/O errors.
To make the use without direct I/O possible and avoid this kind of
alignment errors, we now pad unaligned read requests with a dummy
page, so that we can always read full tracks. Please note that
writing is still only possible for full track images that are properly
aligned.

Change wording for the cpu capabiity changed message:
If such an event occurs it only means that a cpu capability *may* have
changed. A cpu capability change event may also occur for other reasons.
Also change the severity of the message from warning to informational.
If such an event happens user space should into /proc/sysinfo and verify
if some capability values changed, if that is of interest.

With the introduction of PCI it became apparent that s390 should
convert to generic hardirqs as too many drivers do not have the
correct dependency for GENERIC_HARDIRQS. On the architecture
level s390 does not have irq lines. It has external interrupts,
I/O interrupts and adapter interrupts. This patch hard-codes all
external interrupts as irq #1, all I/O interrupts as irq #2 and
all adapter interrupts as irq #3. The additional information from
the lowcore associated with the interrupt is stored in the
pt_regs of the interrupt frame, where the interrupt handler can
pick it up. For PCI/MSI interrupts the adapter interrupt handler
scans the relevant bit fields and calls generic_handle_irq with
the virtual irq number for the MSI interrupt.

The zcrypt device driver has been split into base/bus module, api-module,
card modules and message type modules. The base module has been renamed
from z90crypt to ap.
A module alias (with the well-known z90crypt identifier) will be introduced
that enable users to use their existing way to load the zcrypt device driver.

Fix various weird constructions of strncpy(dst, src, strlen(src)).
Length limits should be about the space available in the destination,
not repurposed as a method to either always include or always exclude a
trailing NULL byte. Either the NULL should always be copied (using
strlcpy), or it should not be copied (using something like memcpy).
Readable code should not depend on the weird behavior of strncpy when it
hits the length limit. Better to avoid the anti-pattern entirely.

Whenever a DASD request encounters a timeout we might
need to abort all outstanding requests on this or
even other devices.
This is especially useful if one wants to fail all
devices on one side of a RAID10 configuration, even
though only one device exhibited an error.
To handle this I've introduced a new device flag
DASD_FLAG_ABORTIO.
This flag is evaluated in __dasd_process_request_queue()
and will invoke blk_abort_request() for all
outstanding requests with DASD_CQR_FLAGS_FAILFAST set.
This will cause any of these requests to be aborted
immediately if the blk_timeout function is activated.
The DASD_FLAG_ABORTIO is also evaluated in
__dasd_process_request_queue to abort all
new request which would have the
DASD_CQR_FLAGS_FAILFAST bit set.
The flag can be set with the new ioctls 'BIODASDABORTIO'
and removed with 'BIODASDALLOWIO'.

This patch adds a 'timeout' attibute to the DASD driver.
When set to non-zero, the blk_timeout function will
be enabled with the timeout specified in the attribute.
Setting 'timeout' to '0' will disable block timeouts.

The DASD driver is using FASTFAIL as an equivalent to the
transport errors in SCSI. And the 'steal lock' function maps
roughly to a reservation error. So we should be returning the
appropriate error codes when completing a request.

Whenever a request has been aborted internally by the driver
there is no sense data to be had. And printing lots of messages
stalls the system, so better to print out a short one-liner.

This patch implements generic block layer timeout handling
callbacks for DASDs. When the timeout expires the respective
cqr is aborted.
With this timeout handler time-critical request abort
is guaranteed as the abort does not depend on the internal
state of the various DASD driver queues.

Originally the DASD device tasklet would process the entries on
the ccw_queue until the first non-final request was found.
Which was okay as long as all requests have the same retries and
expires parameter.
However, as we're now allowing to modify both it is possible to
have requests _after_ the first request which already have expired.
So we need to check all requests in the device tasklet.

Using static data for fields which are accessed by HW will fail if
the driver is build as a module (since this would be vmalloc'ed
memory). This Bug was revealed via
"s390: remove virt_to_phys implementation" - the old virt_to_phys
implementation would have translated the address but it was not
guaranteed that the memory was contiguous.
Fix it by putting the data on the stack.

There are three users of adapter interrupts: AP, QDIO and PCI. Each
registers a single adapter interrupt with independent ISCs. Define
a "struct airq" with the interrupt handler, a pointer and a mask for
the local summary indicator and the ISC for the adapter interrupt
source. Convert the indicator array with its fixed number of adapter
interrupt sources per ISE to an array of hlists. This removes the
limitation to 32 adapter interrupts per ISC and allows for arbitrary
memory locations for the local summary indicator.

Copy the interrupt parameters from the lowcore to the pt_regs structure
in entry[64].S and reduce the arguments of the low level interrupt handler
to the pt_regs pointer only. In addition move the test-pending-interrupt
loop from do_IRQ to entry[64].S to make sure that interrupt information
is always delivered via pt_regs.

the /dev/chsc character device can only be accessed by one

Add a kernel parameter to be able to specify the number of pages to be
used as output buffer by the line-mode sclp driver and the vt220 sclp
driver. The current number of output pages is 6, if the service element
is unavailable the boot messages alone can fill up the output buffer.
If this happens the system blocks until the service element is working
again. For a large LPAR with many devices it is sensible to have the
ability to increase the output buffer size. To help to debug this
situation add a counter for the page-pool-empty situation and make it
available as a sclp driver attribute.
To avoid the system to stall until the service element works again
add another kernel parameter to allow to drop output buffers.

When the initial MTU size is changed prior to any activity on the device
(e.g. by attaching a z/VM vNIC already configured in Linux to a guestLAN),
we call dev_kfree_skb_irq(NULL) which results in a kernel panic.
Adding a proper check for NULL pointers to address this issue.

blkt settings (or LAN idle settings) for an OSA Express card
determine when and how often an OSA Express card tells the
operating system about new incoming packets. The semantic of
these settings has changed starting with OSA Express3. Currently
the qeth standard settings apply to OSA Express2 and older
generations of OSA Express cards, while new generations of OSA
Express cards require extra coding of their reasonable default.
To cover future OSA Express generations the qeth default standard
blkt setting is now the desired setting for OSA generations
starting with OSA Express3, while the fixed set of older OSA
Express cards receives its blkt settings explicitly.

"[SCSI] zfcp: Use status_read_buf_num provided by FCP channel"
started using a value returned by the channel but only evaluated the value
if the fabric link is up.
Commit 8d88cf3f3b
"[SCSI] zfcp: Update status read mempool"
introduced mempool resizings based on the above value.
On setting an FCP device online for the very first time since boot, a new
zeroed adapter object is allocated. If the link is down, the number of
status read requests remains zero. Since just the config data exchange is
incomplete, we proceed with adapter open recovery. However, we
unconditionally call mempool_resize with adapter->stat_read_buf_num == 0 in
this case.
This causes a kernel message "kernel BUG at mm/mempool.c:131!" in process
"zfcperp<FCP-device-bus-ID>" with last function mempool_resize in Krnl PSW
and zfcp_erp_thread in the Call Trace.
Don't evaluate channel values which are invalid on link down. The number of
status read requests is always valid, evaluated, and set to a positive
minimum greater than zero. The adapter open recovery can proceed and the
channel has status read buffers to inform us on a future link up event.
While we are not aware of any other code path that could result in mempool
resize attempts of size zero, we still also initialize the number of status
read buffers to be posted to a static minimum number on adapter object
allocation.

This patch removes an interface that was used to manage access control
tables within the HBA. The patch consequently removes the handling
for conditions related to those access control tables, too.
That initiator-based access control feature was only needed until the
introduction of NPIV and was withdrawn with z10 years ago.
It's time to cleanup the corresponding device driver code.

So far, we could only increase the s390dbf log level after an FCP
device has been initially set online for it to create the dbf entries
required to adjust the level.
Introduce zfcp.dbflevel as counterpart to the already existing
zfcp.dbfsize to enable debugging of e.g. setting an FCP device online.

"[SCSI] zfcp: support for hardware data router"
reduced the initial block queue limits in the scsi_host_template to the
absolute minimum and adjusted them later on. However, the adjustment was
too late for the BSG devices of Scsi_Host and fc_host.
Therefore, ioctl(..., SG_IO, ...) with request or response size > 4kB to a
BSG device of an fc_host or a Scsi_Host fails with EINVAL. As a result,
users of such ioctl such as HBA_SendCTPassThru() in libzfcphbaapi return
with error HBA_STATUS_ERROR.
Initialize the block queue limits in zfcp_scsi_host_template to the
greatest common denominator (GCD).
While we cannot exploit the slightly enlarged maximum request size with
data router, this should be neglectible. Doing so also avoids running into
trouble after live guest relocation (LGR) / migration from a data router
FCP device to an FCP device that does not support data router. In that
case, zfcp would figure out the new limits on adapter recovery, but the
fc_host and Scsi_Host (plus in fact all sdevs) still exist with the old and
now too large queue limits.
It should also OK, not to use half the size as in the DIX case, because
fc_host and Scsi_Host do not transport FCP requests including SCSI commands
using protection data.

FCP device remains in status ERP_FAILED when device is switched online
or adapter recovery is triggered while link to SAN is down.
When Exchange Configuration Data command returns the FSF status
FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE it aborts the exchange process.
The only retries are done during the common error recovery procedure
(i.e. max. 3 retries with 8sec sleep between) and remains in status
ERP_FAILED with QDIO down.
This commit reverts the commit 0df138476c
(zfcp: Fix adapter activation on link down).
When FSF status FSF_EXCHANGE_CONFIG_DATA_INCOMPLETE is received the
adapter recovery will be finished without any retries. QDIO will be
up now and status changes such as LINK UP will be received now.

When a path is gone and dasd_generic_path_event is called with a
PE_PATH_GONE event, we must assume that any I/O request on that
subchannel is still running. This is unlike the dasd_generic_notify
handler and the CIO_NO_PATH event, which implies that the subchannel
has been cleared.
If dasd_generic_path_event finds that the path has been the last
usable path, it must not call dasd_generic_last_path_gone (which would
reset the state of running requests), but just set the
DASD_STOPPED_DC_WAIT bit.
