ANA log parsing invokes nvme_update_ana_state() per ANA group desc.
This updates the state of namespaces with nsids in desc->nsids[].
Both ctrl->namespaces list and desc->nsids[] array are sorted by nsid.
Hence nvme_update_ana_state() performs a single walk over ctrl->namespaces:
- if current namespace matches the current desc->nsids[n],
this namespace is updated, and n is incremented.
- the process stops when it encounters the end of either
ctrl->namespaces end or desc->nsids[]
In case desc->nsids[n] does not match any of ctrl->namespaces,
the remaining nsids following desc->nsids[n] will not be updated.
Such situation was considered abnormal and generated WARN_ON_ONCE.
However ANA log MAY contain nsids not (yet) found in ctrl->namespaces.
For example, lets consider the following scenario:
- nvme0 exposes namespaces with nsids = [2, 3] to the host
- a new namespace nsid = 1 is added dynamically
- also, a ANA topology change is triggered
- NS_CHANGED aen is generated and triggers scan_work
- before scan_work discovers nsid=1 and creates a namespace, a NOTICE_ANA
aen was issues and ana_work receives ANA log with nsids=[1, 2, 3]
Result: ana_work fails to update ANA state on existing namespaces [2, 3]
Solution:
Change the way nvme_update_ana_state() namespace list walk
checks the current namespace against desc->nsids[n] as follows:
a) ns->head->ns_id < desc->nsids[n]: keep walking ctrl->namespaces.
b) ns->head->ns_id == desc->nsids[n]: match, update the namespace
c) ns->head->ns_id >= desc->nsids[n]: skip to desc->nsids[n+1]
This enables correct operation in the scenario described above.
This also allows ANA log to contain nsids currently invisible
to the host, i.e. inactive nsids.

For RDMA transports, TOS is an extension of IB QoS to provide clients
the ability to segregate traffic flows for different type of data.
RDMA CM abstract it for ULPs using rdma_set_service_type().
Internally, each traffic flow is represented by a connection with all of
its independent resources like that of a normal connection, and is
differentiated by service type. In other words, there can be multiple qp
connections between an IP pair and each supports a unique service type.
One of the TOS usage is bandwidth management which allows setting bandwidth
limits for QoS classes, e.g. 80% bandwidth to controllers at QoS class A
and 20% to controllers at QoS class B.
Note: In addition to the TOS configuration, QOS must be configured on the
relevant HCA on the target (send RDMA commands) and initiator to effect
the traffic.

TOS is user-defined and needs to be configured via nvme-cli.
It must be set before initiating any traffic and once set the TOS
cannot be changed.

Simple polling support via socket busy_poll interface.
Although we do not shutdown interrupts but simply hammer
the socket poll, we can sometimes find completions faster
than the normal interrupt driven RX path.
We add per queue nr_cqe counter that resets every time
RX path is invoked such that .poll callback can return it
to stay consistent with the semantics.

Using socket specific read_sock() calls instead of directly calling
tcp_read_sock() helps lld module registered handlers if any, to be called
from nvme-tcp host.
This patch therefore replaces the tcp_read_sock() with socket specific
prot_ops.

One of the components in LiteON CL1 device has limitations that
can be encountered based upon boundary race conditions using the
nvme bus specific suspend to idle flow.
When this situation occurs the drive doesn't resume properly from
suspend-to-idle.
LiteON has confirmed this problem and fixed in the next firmware
version. As this firmware is already in the field, avoid running
nvme specific suspend to idle flow.

Commit 1b1031ca63 ("nvme: validate cntlid during controller initialisation")
introduced a validation for controllers with duplicate cntlid that runs
on nvme_init_subsystem(). The problem is that the validation relies on
ctrl->cntlid, and this value is assigned (from id_ctrl value) after the
call for nvme_init_subsystem() in nvme_init_identify() for non-fabrics
scenario. That leads to ctrl->cntlid always being 0 in case we have a
physical set of controllers in the same subsystem.
This patch fixes that by loading the discovered cntlid id_ctrl value into
ctrl->cntlid before the subsystem initialization, only for the non-fabrics
case. The patch was tested with emulated nvme devices (qemu) having two
controllers in a single subsystem. Without the patch, we couldn't make
it work failing in the duplicate check; when running with the patch, we
could see the subsystem holding both controllers.
For the fabrics case we see ctrl->cntlid has a more intricate relation
with the admin connect, so we didn't change that.

nvme_state_set_live() making a path available triggers requeue_work
in order to resubmit requests that ended up on requeue_list when no
paths were available.
This requeue_work may race with concurrent nvme_ns_head_make_request()
that do not observe the live path yet.
Such concurrent requests may by made by either:
- New IO submission.
- Requeue_work triggered by nvme_failover_req() or another ana_work.
A race may cause requeue_work capture the state of requeue_list before
more requests get onto the list. These requests will stay on the list
forever unless requeue_work is triggered again.
In order to prevent such race, nvme_state_set_live() should
synchronize_srcu(&head->srcu) before triggering the requeue_work and
prevent nvme_ns_head_make_request referencing an old snapshot of the
path list.

One of the modifications made by commit d916b1be94 ("nvme-pci: use
host managed power state for suspend") was adding a pci_save_state()
call to nvme_suspend() so as to instruct the PCI bus type to leave
devices handled by the nvme driver in D0 during suspend-to-idle.
That was done with the assumption that ASPM would transition the
device's PCIe link into a low-power state when the device became
inactive. However, if ASPM is disabled for the device, its PCIe
link will stay in L0 and in that case commit d916b1be94 is likely
to cause the energy used by the system while suspended to increase.
Namely, if the device in question works in accordance with the PCIe
specification, putting it into D3hot causes its PCIe link to go to
L1 or L2/L3 Ready, which is lower-power than L0. Since the energy
used by the system while suspended depends on the state of its PCIe
link (as a general rule, the lower-power the state of the link, the
less energy the system will use), putting the device into D3hot
during suspend-to-idle should be more energy-efficient that leaving
it in D0 with disabled ASPM.
For this reason, avoid leaving NVMe devices with disabled ASPM in D0
during suspend-to-idle. Instead, shut them down entirely and let
the PCI bus type put them into D3.

When aborting in-flight request for recovering controller, we have
to make sure that queue's complete function is called on completed
request before moving on. Otherwise, for example, the warning of
WARN_ON_ONCE(qp->mrs_used > 0) in ib_destroy_qp_user() may be
triggered on nvme-rdma.
Fix this issue by using blk_mq_tagset_wait_completed_request.

Before aborting in-flight requests, all IO queues and their interrupts
have been shutdown. However, request's completion function may not be
done yet because it can be scheduled to run via IPI.
So don't abort one request if it is marked as completed, otherwise
we may abort one normal completed request.

This will always allow a subsequent nvme_remove() to set the state to

With multipath enabled, nvme_scan_work() can read from the device
(through nvme_mpath_add_disk()) and hang [1]. However, with fabrics,
once ctrl->state is set to NVME_CTRL_DELETING, the reads will hang
(see nvmf_check_ready()) and the mpath stack device make_request
will block if head->list is not empty. However, when the head->list
consistst of only DELETING/DEAD controllers, we should actually not
block, but rather fail immediately.
In addition, before we go ahead and remove the namespaces, make sure
to clear the current path and kick the requeue list so that the
request will fast fail upon requeuing.

When the user issues a command with side effects, we will end up freezing
the namespace request queue when updating disk info (and the same for
the corresponding mpath disk node).
However, we are not freezing the mpath node request queue,
which means that mpath I/O can still come in and block on blk_queue_enter
(called from nvme_ns_head_make_request -> direct_make_request).
This is a deadlock, because blk_queue_enter will block until the inner
namespace request queue is unfroze, but that process is blocked because
the namespace revalidation is trying to update the mpath disk info
and freeze its request queue (which will never complete because
of the I/O that is blocked on blk_queue_enter).
Fix this by freezing all the subsystem nsheads request queues before
executing the passthru command. Given that these commands are infrequent
we should not worry about this temporary I/O freeze to keep things sane.

When CONFIG_NVME_MULTIPATH is set, only the hidden gendisk associated
with the per-controller ns is run through revalidate_disk when a
rescan is triggered, while the visible blockdev never gets its size
(bdev->bd_inode->i_size) updated to reflect any capacity changes that
may have occurred.
This prevents online resizing of nvme block devices and in extension of
any filesystems atop that will are unable to expand while mounted, as
userspace relies on the blockdev size for obtaining the disk capacity
(via BLKGETSIZE/64 ioctls).
Fix this by explicitly revalidating the actual namespace gendisk in
addition to the per-controller gendisk, when multipath is enabled.

With this patch, set 'poll_queues > hard queues' will lead to 'nr_read_queues = 0'
in nvme_calc_irq_sets. Then poll_queues setting can fail since dev->tagset.nr_maps
equals to 2 and nvme_pci_map_queues will not do map for poll queues.

When freeing the subsystem after finding another match with
__nvme_find_get_subsystem(), use put_device() instead of
__nvme_release_subsystem() which calls kfree() directly.
Per the documentation, put_device() should always be used
after device_initialization() is called. Otherwise, leaks
like the one below which was detected by kmemleak may occur.
Once the call of __nvme_release_subsystem() is removed it no
longer makes sense to keep the helper, so fold it back
into nvme_release_subsystem().

preallocated sg list. This necessitates a large number of driver
changes because the previous guarantee that if a driver specified
SG_ALL as the size of its scatter list, it would get a non-chained
list and didn't need to bother with scatterlist iterators is now
broken and every driver *must* use scatterlist iterators.
This was broken out as a separate topic because we need to convert all
the drivers before pulling the trigger and unconverted drivers kept
being found, necessitating a rebase.

Current code allows the module to be unloaded even if there are
pending data structures, such as localports and controllers on
the localports, that have yet to hit their reference counting
to remove them.
Fix by having exit entrypoint explicitly delete every controller,
which in turn will remove references on the remoteports and localports
causing them to be deleted as well. The exit entrypoint, after
initiating the deletes, will wait for the last localport to be deleted
before continuing.

There was a few false alarms sighted on target side about wrong data
digest while performing high throughput load to XFS filesystem shared
through NVMoF TCP.
This flag tells the rest of the kernel to ensure that the data buffer
does not change while the write is in flight. It incurs a performance
penalty, so only enable it when it is actually needed, i.e. when we are
calculating data digests.
Although even with this change in place, ext2 users can steel experience
false positives, as ext2 is not respecting this flag. This may be apply
to vfat as well.

nvme_ns_remove() will first set the NVME_NS_REMOVING flag before removing
it from the list at the very last step.
So to avoid selecting a namespace in nvme_find_path() which is about to be
removed check the NVME_NS_REMOVING flag, too, when selecting a new path.

When running a NVMe device that is attached to a addressing
challenged PCIe root port that requires bounce buffering, our
request sizes can easily overflow the swiotlb bounce buffer
size. Limit the maximum I/O size to the limit exposed by
the DMA mapping subsystem.

Modify nvme_alloc_sq_cmds() to call pci_free_p2pmem() to free the memory
it allocated using pci_alloc_p2pmem() in case pci_p2pmem_virt_to_bus()
returns null.
Makes sure not to call pci_free_p2pmem() if pci_alloc_p2pmem() returned
NULL, which can happen if CONFIG_PCI_P2PDMA is not configured.
The current implementation is not expected to leak since
pci_p2pmem_virt_to_bus() is expected to fail only if pci_alloc_p2pmem()
returns null. However, checking the return value of pci_alloc_p2pmem()
is more explicit.

Only request an IRQ mapping for read queues if at least one read queue
is being allocted, as nvme_pci_map_queues() will later on ignore the
unnecessary mapping request should nvme_dev_add() request such an IRQ
mapping even though no read queues are being allocated. However,
nvme_dev_add() can avoid making the request by checking the number of
read queues without assuming. This would bring it more in line with
nvme_setup_irqs() and nvme_calc_irq_sets().

nvme_trace_disk_name() is now already being invoked with the function
prototype in trace.h. We don't need to export this symbol at all.
The following patches are going to provide target-side trace feature
with the exactly same function with this so that this patch removes the
EXPORT_SYMBOL() for this function.

If the state change to NVME_CTRL_CONNECTING fails, the dmesg is going to
be like:

This patch removes the confusing assignment of the variable result at
the time of declaration and sets the value in error cases next to the
places where the actual error is happening.
Here we also set the result value to -ENODEV when we fail at the final
ctrl state transition in nvme_reset_work(). Without this assignment
result will hold 0 from nvme_setup_io_queue() and on failure 0 will be
passed to he nvme_remove_dead_ctrl() from final state transition.

queue_count_set() seems like that it has been provided to limit the
number of queue entries for write/poll queues. But, the
queue_count_set() has been doing nothing but a parameter check even it
has num_possible_cpus() which is nop.
This patch removes entire queue_count_ops from the write_queues and
poll_queues.

The nvme pci driver prepares its devices for power loss during suspend
by shutting down the controllers. The power setting is deferred to
pci driver's power management before the platform removes power. The
suspend-to-idle mode, however, does not remove power.
NVMe devices that implement host managed power settings can achieve
lower power and better transition latencies than using generic PCI power
settings. Try to use this feature if the platform is not involved with
the suspend. If successful, restore the previous power state on resume.

When a shared namespace is removed, we call blk_cleanup_queue()
when the device can still be accessed as the current path and this can
result in submission to a dying queue. Hence, direct_make_request()
called by our mpath device may fail (propagating the failure to userspace).
Instead, we want to failover this I/O to a different path if one exists.
Thus, before we cleanup the request queue, we make sure that the device is
cleared from the current path nor it can be selected again as such.
Fix this by:
- clear the ns from the head->list and synchronize rcu to make sure there is
no concurrent path search that restores it as the current path
- clear the mpath current path in order to trigger a subsequent path search
and sync srcu to wait for any ongoing request submissions
- safely continue to namespace removal and blk_cleanup_queue

When looking at console messages to troubleshoot, there are one
maybe two messages before creation of the controller is complete.
However, a lot of io takes place to reach that point. It's unclear
when things have started.
Add a message when the controller is attempting to create a new
association. Thus we know what controller, between what host and
remote port, and what NQN is being put into place for any
subsequent success or failure messages.

sg_alloc_table_chained() currently allows the caller to provide one
preallocated SGL and returns if the requested number isn't bigger than
size of that SGL. This is used to inline an SGL for an IO request.
However, scattergather code only allows that size of the 1st preallocated
SGL to be SG_CHUNK_SIZE(128). This means a substantial amount of memory
(4KB) is claimed for the SGL for each IO request. If the I/O is small, it
would be prudent to allocate a smaller SGL.
Introduce an extra parameter to sg_alloc_table_chained() and
sg_free_table_chained() for specifying size of the preallocated SGL.
Both __sg_free_table() and __sg_alloc_table() assume that each SGL has the
same size except for the last one. Change the code to allow both functions
to accept a variable size for the 1st preallocated SGL.

The reason for this crash is accessing an already freed ib_device for
performing dma_unmap during exit_request commands. The root cause for
that is that during re-connection all the queues are destroyed and
re-created (and the ib_device is reference counted by the queues and
freed as well) but the tagset stays alive and all the DMA mappings (that
we perform in init_request) kept in the request context. The original
commit fixed a different bug that was introduced during bonding (aka nic
teaming) tests that for some scenarios change the underlying ib_device
and caused memory leakage and possible segmentation fault. This commit
is a complementary commit that also changes the wrong DMA mappings that
were saved in the request context and making the request sqe dma
mappings dynamic with the command lifetime (i.e. mapped in .queue_rq and
unmapped in .complete). It also fixes the above crash of accessing freed
ib_device during destruction of the tagset.

The Number of Namespaces (nn) field in the identify controller data structure is
defined as u32 and the maximum allowed value in NVMe specification is
0xFFFFFFFEUL. This change fixes the possible overflow of the DIV_ROUND_UP()
operation used in nvme_scan_ns_list() by casting the nn to u64.

NVMe uses PRPs (or optionally unlimited SGLs) for data transfers and
has no specific limit for a single DMA segement. Limiting the size
will cause problems because the block layer assumes PRP-ish devices
using a virt boundary mask don't have a segment limit. And while this
is true, we also really need to tell the DMA mapping layer about it,
otherwise dma-debug will trip over it.

When the controller supports less queues than requested, we
should make sure that queue mapping does the right thing and
not assume that all queues are available. This fixes a crash
when the controller supports less queues than requested.
The rules are:
1. if no write queues are requested, we assign the available queues
to the default queue map. The default and read queue maps share the
existing queues.
2. if write queues are requested:
- first make sure that read queue map gets the requested
nr_io_queues count
- then grant the default queue map the minimum between the requested
nr_write_queues and the remaining queues. If there are no available
queues to dedicate to the default queue map, fallback to (1) and
share all the queues in the existing queue map.
Also, provide a log indication on how we constructed the different
queue maps.

We use the controller's reported maximum firmware activation time as our
timeout before resetting a controller for a failed activation notice,
but this value was never being read so we could only use the default
timeout. Copy the Identify Controller MTFA field to the corresponding
nvme_ctrl's mtfa field.

Unconditionally hide device pm latency tolerance when uninitializing
the controller to ensure all qos resources are released so that we're
not leaking this memory. This is safe to call if none were allocated in
the first place, or were previously freed.

Holding the SRCU critical section protecting the namespace list can
cause deadlocks when using the per-namespace admin passthrough ioctl to
delete as namespace. Release it earlier when performing per-controller
ioctls to avoid that.

If we can't get a namespace don't leak the SRCU lock. nvme_ioctl was
working around this, but nvme_pr_command wasn't handling this properly.
Just do what callers would usually expect.

We're trying to append known effects to the ones reported in the
controller's log. The original patch accomplished this, but something
went wrong when patch was merged causing the effects log to override
the known effects.

A controller with multiple namespaces may have multiple request_queues with
their own timeout work. If a controller fails with IO outstanding to
diffent namespaces, each request queue may attempt to handle it, so
ensure there is no previously scheduled timeout work executing prior to
starting controller initialization by synchronizing with each queue.

The reset_work waits for queued IO to complete before setting the
controller to live. If any of these times out and requeues, we won't be
able to restart the controller because the reset_work is already running.
Flush all entered requests to a failed completion if a timeout occurs
in the connecting state, and ensure the controller can't transition to
the live state after we've unblocked it from waiting for completions.

The reset state doesn't dispatch commands that it needs to wait for
anymore. If a timeout occurs in this state, the reset work is already
disabling the controller, so just reset the request's timer.

The CNTLID value is required to be unique, and we do rely on this

Life becomes a lot simpler if we just use the global
nvme_subsystems_lock to protect this list. Given that it is only
accessed during controller probing and removal that isn't a scalability
problem either.

This patch removes the tracing of the NVMe Async events out of the
switch so that it can trace all the events including the ones which
are not handled in the nvme_handle_aen_notice(). The events which
are not handled in the nvme_handle_aen_notice() such as
NVME_AER_NOTICE_DISC_CHANGED corresponding event identifier needs
to be added in the drivers/nvme/host/trace.h so that it can stringify
the AER .

A process holding an open reference to a removed disk prevents it
from completing deletion, so its name continues to exist. A subsequent
gendisk creation may have the same cntlid which risks collision when
using that for the name. Use the unique ctrl->instance instead.

When tearing down a controller the following warning is issued:
WARNING: CPU: 0 PID: 30681 at ../kernel/workqueue.c:2418 check_flush_dependency
This happens as the err_work workqueue item is scheduled on the
system workqueue (which has WQ_MEM_RECLAIM not set), but is flushed
from a workqueue which has WQ_MEM_RECLAIM set.
Fix this by providing an FC-NVMe specific workqueue.

In the past, before adding f41725bb ("nvme-rdma: Use mr pool") commit,
we needed a reference on the ib_device as long as the tagset
was alive, as the MRs in the request structures needed a valid ib_device.
Now, we allocate/deallocate MR pool per QP and consume on demand.
Also remove nvme_rdma_free_tagset function and use blk_mq_free_tag_set
instead, as it unneeded anymore.
This commit also fixes a memory leakage and possible segmentation fault.
When configuring the system with NIC teaming (aka bonding), we use 1
network interface to create an HA connection to the target side. In case
one connection breaks down, nvme-rdma driver will get notification from
rdma-cm layer that underlying address was change and will start error
recovery process. During this process, we'll reconnect to the target
via the second interface in the bond without destroying the tagset.
This will cause a leakage of the initial rdma device (ndev) and miscount
in the reference count of the new created rdma device (new ndev). In
the final destruction (or in another error flow), we'll get a warning
dump from the ib_dealloc_pd that we still have inflight MR's related to
that pd. This happens becasue of the miscount of the reference tag of
the rdma device and causing access violation to it's elements (some
queues are not destroyed yet).

Current lightnvm and pblk implementation does not care about NVMe max
data transfer size, which can be smaller than 64*K=256K. There are
existing NVMe controllers which NVMe max data transfer size is lower
that 256K (for example 128K, which happens for existing NVMe
controllers which are NVMe spec compliant). Such a controllers are not
able to handle command which contains 64 PPAs, since the the size of
DMAed buffer will be above the capabilities of such a controller.

If the bio is moved to a different queue via blk_steal_bios() and
the original queue is destroyed in nvme_remove_ns() we'll be ending
with a crash in bio_endio() as the mempool for the split bio bvecs
had already been destroyed.
So split the bio using the original queue (which will remain during the
lifetime of the bio) before sending it down to the underlying device.

an I/O tagset allocated which causes the teardown sequence to crash.
Make nvme_tcp_teardown_io_queues safe by not iterating inflight tags
if the tagset wasn't allocated.

Currently the FC-NVMe driver is leverating the SCSI FC transport class to
access the remote ports. Which means that all FC-NVMe remote ports will be
visible to the fc transport layer, but due to missing definitions the port
roles will always be 'unknown'. This patch adds the missing definitions to
the fc transport class to that the port roles are correctly displayed.

This patch fixes a long-standing bug that initialized the FC-NVME
cmnd iu CSN value to 1. Early FC-NVME specs had the connection starting
with CSN=1. By the time the spec reached approval, the language had
changed to state a connection should start with CSN=0. This patch
corrects the initialization value for FC-NVME connections.
Additionally, in reviewing the transport, the CSN value is assigned to
the new IU early in the start routine. It's possible that a later dma
map request may fail, causing the command to never be sent to the
controller. Change the location of the assignment so that it is
immediately prior to calling the lldd. Add a comment block to explain
the impacts if the lldd were to additionally fail sending the command.

nvme_cancel_request() is used in error handler, and it is always
reliable to cancel request synchronously, and avoids possible race
in which request may be completed after real hw queue is destroyed.
One issue is reported by our customer on NVMe RDMA, in which freed ib
queue pair may be used in nvme_rdma_complete_rq().

general iod fields, which can easily be done at the beginning of

A negative value for the cq_vector used to mean the queue is either
disabled or a polled queue. However, we have a queue enabled flag,
so the cq_vector had been serving double duty.
Don't overload the meaning of cq_vector. Use a flag specific to the
polled queues instead.

The cleanup_srcu_struct_quiesced() function was added because NVME
used WQ_MEM_RECLAIM workqueues and SRCU did not, which meant that
NVME workqueues waiting on SRCU workqueues could result in deadlocks
during low-memory conditions. However, SRCU now also has WQ_MEM_RECLAIM
workqueues, so there is no longer a potential for deadlock. Furthermore,
it turns out to be extremely hard to use cleanup_srcu_struct_quiesced()
correctly due to the fact that SRCU callback invocation accesses the
srcu_struct structure's per-CPU data area just after callbacks are
invoked. Therefore, the usual practice of using srcu_barrier() to wait
for callbacks to be invoked before invoking cleanup_srcu_struct_quiesced()
fails because SRCU's callback-invocation workqueue handler might be
delayed, which can result in cleanup_srcu_struct_quiesced() being invoked
(and thus freeing the per-CPU data) before the SRCU's callback-invocation
workqueue handler is finished using that per-CPU data. Nor is this a
theoretical problem: KASAN emitted use-after-free warnings because of
this problem on actual runs.
In short, NVME can now safely invoke cleanup_srcu_struct(), which
avoids the use-after-free scenario. And cleanup_srcu_struct_quiesced()
is quite difficult to use safely. This commit therefore removes
cleanup_srcu_struct_quiesced(), switching its sole user back to
cleanup_srcu_struct(). This effectively reverts the following pair
of commits:

A C2HData PDU with the SUCCESS flag set indicates that the I/O was
completed by the controller successfully and means that a subsequent
completion response capsule PDU will be ommitted.
If we see this flag, fisrt we check that LAST_PDU flag is set as well,
and then we complete the request when the data transfer (and data digest
verification if its on) is done.
While we're at it, reuse a bit of code with nvme_fail_request.

If:
- A successful connect has occurred with an io queue count greater than
zero and namespaces detected and running.
- An error or something occurs which causes a termination of the prior
association and then starts a reconnect,
- The reconnect then creates a new controller, but for whatever reason,
nvme_set_queue_count() results in io queue count set to zero. This
will skip io queue and tag set changes.
- But... the controller will transition to live, calling
nvme_start_ctrl, which calls nvme_start_queues(), which then releases
I/Os into the transport which then sends them to the driver.
As there are no queues, things eventually hit the driver looking for a
handle, which was cleared when the original controller was reset, and it
can't proceed. Worst case, things progress, but everything fails.
In the failing scenario, the nvme_set_features(NVME_FEAT_NUM_QUEUES)
command actually failed with a NVME_SC_INTERNAL error. For some reason,
although nvme_set_queue_count() saw the error and set io queue count to
zero, it doesn't return a failure status to the transport, which allows
the transport to continue using the controller.
Fix the problem by simply rejecting the new association if at least 1
I/O queue can't be created. The association reject will fail the
reconnect attempt and fall into the reconnect retry policy.

A recent change added a numa_node field to the nvme controller
and has the transport assign the node using dev_to_node().
However, fcloop registers with a NULL device struct, so the
dev_to_node() call oops.
Revise the assignment to assign no node when device struct is null.

For some nvme command, when issued by the nvme core layer, there
is an internal buffer which can cause blk_rq_payload_bytes() to
return a non-zero value yet there is no actual/real command payload
and sg list. An example is the WRITE ZEROES command.
To address this, when making choices on whether to dma map an sgl,
use blk_rq_nr_phys_segments() instead of blk_rq_payload_bytes().
When there is a sgl, blk_rq_payload_bytes() will return the amount
of data to be transferred by the sgl.

In case nvme_alloc_ns fails after we initialize ns_head but before we
add the ns to the controller namespaces list we need to explicitly put
the ns_head reference because when we teardown the controller we
won't find it, causing us to leak a dangling subsystem eventually.

Implement a simple round-robin I/O policy for multipathing. Path
selection is done in two rounds, first iterating across all optimized
paths, and if that doesn't return any valid paths, iterate over all
optimized and non-optimized paths. If no paths are found, use the
existing algorithm. Also add a sysfs attribute 'iopolicy' to switch
between the current NUMA-aware I/O policy and the 'round-robin' I/O
policy.

The NVME PCI driver contains a tedious mechanism for interrupt
allocation, which is necessary to adjust the number and size of interrupt
sets to the maximum available number of interrupts which depends on the
underlying PCI capabilities and the available CPU resources.
It works around the former short comings of the PCI and core interrupt
allocation mechanims in combination with interrupt sets.
The PCI interrupt allocation function allows to provide a maximum and a
minimum number of interrupts to be allocated and tries to allocate as
many as possible. This worked without driver interaction as long as there
was only a single set of interrupts to handle.
With the addition of support for multiple interrupt sets in the generic
affinity spreading logic, which is invoked from the PCI interrupt
allocation, the adaptive loop in the PCI interrupt allocation did not
work for multiple interrupt sets. The reason is that depending on the
total number of interrupts which the PCI allocation adaptive loop tries
to allocate in each step, the number and the size of the interrupt sets
need to be adapted as well. Due to the way the interrupt sets support was
implemented there was no way for the PCI interrupt allocation code or the
core affinity spreading mechanism to invoke a driver specific function
for adapting the interrupt sets configuration.
As a consequence the driver had to implement another adaptive loop around
the PCI interrupt allocation function and calling that with maximum and
minimum interrupts set to the same value. This ensured that the
allocation either succeeded or immediately failed without any attempt to
adjust the number of interrupts in the PCI code.
The core code now allows drivers to provide a callback to recalculate the
number and the size of interrupt sets during PCI interrupt allocation,
which in turn allows the PCI interrupt allocation function to be called
in the same way as with a single set of interrupts. The PCI code handles
the adaptive loop and the interrupt affinity spreading mechanism invokes
the driver callback to adapt the interrupt set configuration to the
current loop value. This replaces the adaptive loop in the driver
completely.
Implement the NVME specific callback which adjusts the interrupt sets
configuration and remove the adaptive allocation loop.

The interrupt affinity spreading mechanism supports to spread out
affinities for one or more interrupt sets. A interrupt set contains one
or more interrupts. Each set is mapped to a specific functionality of a
device, e.g. general I/O queues and read I/O queus of multiqueue block
devices.
The number of interrupts per set is defined by the driver. It depends on
the total number of available interrupts for the device, which is
determined by the PCI capabilites and the availability of underlying CPU
resources, and the number of queues which the device provides and the
driver wants to instantiate.
The driver passes initial configuration for the interrupt allocation via
a pointer to struct irq_affinity.
Right now the allocation mechanism is complex as it requires to have a
loop in the driver to determine the maximum number of interrupts which
are provided by the PCI capabilities and the underlying CPU resources.
This loop would have to be replicated in every driver which wants to
utilize this mechanism. That's unwanted code duplication and error
prone.
In order to move this into generic facilities it is required to have a
mechanism, which allows the recalculation of the interrupt sets and
their size, in the core code. As the core code does not have any
knowledge about the underlying device, a driver specific callback will
be added to struct affinity_desc, which will be invoked by the core
code. The callback will get the number of available interupts as an
argument, so the driver can calculate the corresponding number and size
of interrupt sets.
To support this, two modifications for the handling of struct irq_affinity
are required:
1) The (optional) interrupt sets size information is contained in a
separate array of integers and struct irq_affinity contains a
pointer to it.
This is cumbersome and as the maximum number of interrupt sets is small,
there is no reason to have separate storage. Moving the size array into
struct affinity_desc avoids indirections and makes the code simpler.
2) At the moment the struct irq_affinity pointer which is handed in from
the driver and passed through to several core functions is marked
'const'.
With the upcoming callback to recalculate the number and size of
interrupt sets, it's necessary to remove the 'const'
qualifier. Otherwise the callback would not be able to update the data.
Implement #1 and store the interrupt sets size in 'struct irq_affinity'.
No functional change.

A surprise removal may fail to tear down request queues if it is racing
with the initial asynchronous probe. If that happens, the remove path
won't see the queue resources to tear down, and the controller reset
path may create a new request queue on a removed device, but will not
be able to make forward progress, deadlocking the pci removal.
Protect setting up non-blocking resources from a shutdown by holding the
same mutex, and transition to the CONNECTING state after these resources
are initialized so the probe path may see the dead controller state
before dispatching new IO.

If a controller supports the NS Change Notification, the namespace
scan_work is automatically triggered after attaching a new namespace.
Occasionally the namespace scan_work may append the new namespace to the
list before the admin command effects handling is completed. The effects
handling unfreezes namespaces, but if it unfreezes the newly attached
namespace, its request_queue freeze depth will be off and we'll hit the
warning in blk_mq_unfreeze_queue().
On the next namespace add, we will fail to freeze that queue due to the
previous bad accounting and deadlock waiting for frozen.
Fix that by preventing scan work from altering the namespace list while
command effects handling needs to pair freeze with unfreeze.

Allow write zeroes operations (REQ_OP_WRITE_ZEROES) on the block
device, if the device supports an optional command bit set for write
zeroes. Add support to setup write zeroes command. Set maximum possible
write zeroes sectors in one write zeroes command according to
nvme write zeroes command definition.
This patch was posted as a part of block-write-zeroes support
implementation (https://patchwork.kernel.org/patch/9454859/),
but did not make into mainline kernel as it got reverted due to
failure on the Linus's machine.
In this patch in order to be more cautious, we use NVMe controller's
maximum hardware sector size which is calculated based on the
controller's MDTS (Maximum Data Transfer Size) field to calculate
the maximum sectors for the write zeroes request.

Bit 6 in the ANACAP field is used to indicate that the ANA group ID
doesn't change while the namespace is attached to the controller.
There is an optimisation in the code to only allocate space
for the ANA group header, as the namespace list won't change and
hence would not need to be refreshed.
However, this optimisation was never carried over to the actual
workflow, which always assumes that the buffer is large enough
to hold the ANA header _and_ the namespace list.
So drop this optimisation and always allocate enough space.

If the device supports less queues than provided (if the device has less
completion vectors), we might hit a bug due to the fact that we ignore
that in nvme_rdma_map_queues (we override the maps nr_queues with user
opts).
Instead, keep track of how many default/read/poll queues we actually
allocated (rather than asked by the user) and use that to assign our
queue mappings.

the create_ctrl flow needs to fail and cleanup on its own)
2. We might also hang if we get a disconnet on a queue while the
controller is already deleting. This racy flow can cause the controller
disable/shutdown admin command to hang.
We cannot complete a timed out request from the timeout handler without
mutual exclusion from the teardown flow (e.g. nvme_rdma_error_recovery_work).
So we serialize it in the timeout handler and teardown io and admin
queues to guarantee that no one races with us from completing the
request.

When -ENOSPC is returned from pci_alloc_irq_vectors_affinity(),
we still try to allocate multiple irq vectors again, so irq queues
covers the admin queue actually. But we don't consider that, then
number of the allocated irq vector may be same with sum of
io_queues[HCTX_TYPE_DEFAULT] and io_queues[HCTX_TYPE_READ], this way
is obviously wrong, and finally breaks nvme_pci_map_queues(), and
warning from pci_irq_get_affinity() is triggered.
IRQ queues should cover admin queues, this patch makes this
point explicitely in nvme_calc_io_queues().
We got severl boot failure internal report on aarch64, so please
consider to fix it in v4.20.

If a device provides an NQN it is expected to be globally unique.
Unfortunately some firmware revisions for Intel 760p/Pro 7600p devices did
not satisfy this requirement. In these circumstances if a system has >1
affected device then only one device is enabled. If this quirk is enabled
then the device supplied subnqn is ignored and we fallback to generating
one as if the field was empty. In this case we also suppress the version
check so we don't print a warning when the quirk is enabled.

When nvme_init_identify() fails the ANA log buffer is deallocated

There is an out of bounds array access in nvme_cqe_peding().
When enable irq_thread for nvme interrupt, there is racing between the
nvmeq->cq_head updating and reading.
nvmeq->cq_head is updated in nvme_update_cq_head(), if nvmeq->cq_head
equals nvmeq->q_depth and before its value set to zero, nvme_cqe_pending()
uses its value as an array index, the index will be out of bounds.

When passed with nr_poll_queues setup additional queues with cq polling
context IB_POLL_DIRECT (no interrupts) and make sure to set
QUEUE_FLAG_POLL on the connect_q. In addition add the third queue
mapping for polling queues.
nvmf connect on this queue is polled for like all other requests so make
nvmf_connect_io_queue poll for polling queues.

Pass poll bool to indicate that we need it to poll. This prepares us for
polling support in nvmf since connect is an I/O that will be queued
and has to be polled in order to complete. If poll is passed,
we call nvme_execute_rq_polled which sends the requests and polls
for its completion.

When boxes are run near (or to) OOM, we have a problem with the discard
page allocation in nvme. If we fail allocating the special page, we
return busy, and it'll get retried. But since ordering is honored for
dispatch requests, we can keep retrying this same IO and failing. Behind
that IO could be requests that want to free memory, but they never get
the chance.
Allocate a fixed discard page per controller for a safe fallback, and use
that if the initial allocation fails.

This patch implements the NVMe over TCP host driver. It can be used to
connect to remote NVMe over Fabrics subsystems over good old TCP/IP.
The driver implements the TP 8000 of how nvme over fabrics capsules and
data are encapsulated in nvme-tcp pdus and exchaged on top of a TCP byte
stream. nvme-tcp header and data digest are supported as well.
To connect to all NVMe over Fabrics controllers reachable on a given taget
port over TCP use the following command:
nvme connect-all -t tcp -a $IPADDR
This requires the latest version of nvme-cli with TCP support.

Currently pblk only check the size of I/O metadata and does not take
into account if this metadata is in a separate buffer or interleaved
in a single metadata buffer.
In reality only the first scenario is supported, where second mode will
break pblk functionality during any IO operation.
This patch prevents pblk to be instantiated in case device only
supports interleaved metadata.

Currently lightnvm and pblk uses single DMA pool, for which the entry
size always is equal to PAGE_SIZE. The contents of each entry allocated
from the DMA pool consists of a PPA list (8bytes * 64), leaving
56bytes * 64 space for metadata. Since the metadata field can be bigger,
such as 128 bytes, the static size does not cover this use-case.
This patch adds support for I/O metadata above 56 bytes by changing DMA
pool size based on device meta size and allows pblk to use OOB metadata
>=16B.

Currently the geometry of an OCSSD is enumerated using a two step
approach:
First, nvm_register is called, the OCSSD identify command is issued,
and second the geometry sos and csecs values are read either from the
OCSSD identify if it is a 1.2 drive, or from the NVMe namespace data
structure if it is a 2.0 device.
This patch recombines it into a single step, such that nvm_register can
use the csecs and sos fields independent of which version is used. This
enables one to dynamically size the lightnvm subsystem dma pool.

Guenter reported an boot hang issue on HPPA after we default to 0 poll
queues. We have two issues in the queue count calculations:
1) We don't separate the poll queues from the read/write queues. This is
important, since the former doesn't need interrupts.
2) The adjust logic is broken.
Adjust the poll queue count before doing nvme_calc_io_queues(). The poll
queue count is only limited by the IO queue count we were able to get
from the controller, not failures in the IRQ allocation loop. This
leaves nvme_calc_io_queues() just adjusting the read/write queue map.

Pull in v4.20-rc6 to resolve the conflict in NVMe, but also to get the
two corruption fixes. We're going to be overhauling the direct dispatch

A controller may have an internal state that is not able to successfully
process commands for a short duration. In such states, an immediate
command requeue is expected to fail. The driver may exceed its max
retry count, which permanently ends the command in failure when the same
command would succeed after waiting for the controller to be ready.
NVMe ratified TP 4033 provides a delay hint in the completion status
code for failed commands. Implement the retry delay based on the command
completion status and the controller's requested delay.
Note that requeued commands are handled per request_queue, not per
individual request. If multiple commands fail, the controller should
consistently report the desired delay time for retryable commands in
all CQEs, otherwise the requeue list may be kicked too soon.

As for now, we don't care about sq_head pointer updates anyway, so
at least allow the controller to micro-optimize by omiting this update.
Note that we will probably need to support it when a controller
that requires this comes along.

If the controller supports traffic based keep alive, we restart the keep
alive timer if any admin or io commands was completed during the kato
period. This prevents a possible starvation of keep alive commands in
the presence of heavy traffic as in such case, we already have a health
indication from the host perspective.
Only set a comp_seen indicator in case the controller supports keep
alive to minimize the overhead for pci controllers.

Instead of directly poking into the struct device add a new numa_node
field to struct nvme_ctrl. This allows fabrics drivers where ctrl->dev
is a virtual device to support NUMA affinity as well.
Also expose the field as a sysfs attribute, and populate it for the
RDMA and FC transports.

In function nvme_setup_cmd() we call command specific setup function
for flush, rw, and discard. Instead of calling memset in each function
lets call it once in the parent function.
This is purely code cleanup patch and it does not change any existing
functionality.

Delete operations are seeing NULL pointer references in call_timer_fn.
Tracking these back, the timer appears to be the keep alive timer.
nvme_keep_alive_work() which is tied to the timer that is cancelled
by nvme_stop_keep_alive(), simply starts the keep alive io but doesn't
wait for it's completion. So nvme_stop_keep_alive() only stops a timer
when it's pending. When a keep alive is in flight, there is no timer
running and the nvme_stop_keep_alive() will have no affect on the keep
alive io. Thus, if the io completes successfully, the keep alive timer
will be rescheduled. In the failure case, delete is called, the
controller state is changed, the nvme_stop_keep_alive() is called while
the io is outstanding, and the delete path continues on. The keep
alive happens to successfully complete before the delete paths mark it
as aborted as part of the queue termination, so the timer is restarted.
The delete paths then tear down the controller, and later on the timer
code fires and the timer entry is now corrupt.
Fix by validating the controller state before rescheduling the keep
alive. Testing with the fix has confirmed the condition above was hit.

The ->poll_fn has been stale for a while, as a lot of places check for mq
ops. But there is no real point in it anyway, as we don't even use
the multipath code for subsystems without multiple ports, which is usually
what we do high performance I/O to. If it really becomes an issue we
should rework the nvme code to also skip the multipath code for any
private namespace, even if that could mean some trouble when rescanning.

Now that we can't poll regular, interrupt driven I/O queues there
is almost nothing that can race with an interrupt. The only
possible other contexts polling a CQ are the error handler and
queue shutdown, and both are so far off in the slow path that
we can simply use the big hammer of disabling interrupts.
With that we can stop taking the cq_lock for normal queues.

We have three places that can poll for I/O completions on a normal
interrupt-enabled queue. All of them are in slow path code, so
consolidate them to a single helper that uses spin_lock_irqsave and
removes the fast path cqe_pending check.
