Mike Christie 95fe6c1a20 block, fs, mm, drivers: use bio set/get op accessors
This patch converts the simple bi_rw use cases in the block,
drivers, mm and fs code to set/get the bio operation using
bio_set_op_attrs/bio_op

These should be simple one or two liner cases, so I just did them
in one patch. The next patches handle the more complicated
cases in a module per patch.

Mike Christie 4e49ea4a3d block/fs/drivers: remove rw argument from submit_bio
This has callers of submit_bio/submit_bio_wait set the bio->bi_rw
instead of passing it in. This makes that use the same as
generic_make_request and how we set the other bio fields.

Jiri Kosina 09954bad44 floppy: refactor open() flags handling
In case /dev/fdX is open with O_NDELAY / O_NONBLOCK, floppy_open() immediately
succeeds, without performing any further media / controller preparations.
That's "correct" wrt. the NODELAY flag, but is hardly correct wrt. the rest
of the floppy driver, that is not really O_NONBLOCK ready, at all. Therefore
it's not too surprising, that subsequent attempts to work with the
filedescriptor produce bad results. Namely, syzkaller tool has been able
to livelock mmap() on the returned fd to keep waiting on the page unlock
bit forever.

Quite frankly, I have trouble defining what non-blocking behavior would be for
floppies. Is waiting ages for the driver to actually succeed reading a sector
blocking operation? Is waiting for drive motor to start blocking operation? How
about in case of virtualized floppies?

One option would be returning EWOULDBLOCK in case O_NDLEAY / O_NONBLOCK is
being passed to open(). That has a theoretical potential of breaking some
arcane and archaic userspace though.

Let's take a more conservative aproach, and accept the O_NDLEAY flag, and let
the driver behave as usual.

While at it, clean up a bit handling of !(mode & (FMODE_READ|FMODE_WRITE))
case and return EINVAL instead of succeeding as well.

Jiri Kosina a0c80efe59 floppy: fix lock_fdc() signal handling
floppy_revalidate() doesn't perform any error handling on lock_fdc()
result. lock_fdc() might actually be interrupted by a signal (it waits for
fdc becoming non-busy interruptibly). In such case, floppy_revalidate()
proceeds as if it had claimed the lock, but it fact it doesn't.

In case of multiple threads trying to open("/dev/fdX"), this leads to
serious corruptions all over the place, because all of a sudden there is
no critical section protection (that'd otherwise be guaranteed by locked
fd) whatsoever.

While at this, fix the fact that the 'interruptible' parameter to
lock_fdc() doesn't make any sense whatsoever, because we always wait
interruptibly anyway.

Most of the lock_fdc() callsites do properly handle error (and propagate
EINTR), but floppy_revalidate() and floppy_check_events() don't. Fix this.

Spotted by 'syzkaller' tool.

Rasmus Villemoes bcf4299e62 floppy: make local variable non-static
There's no reason for temparea to be static, since it's only used for
temporary sprintf output. It's not immediately obvious that the output
will always fit (in the worst case, the output including '\0' is
exactly 32 bytes), so save a future reader from worrying about that.

Christoph Hellwig 4246a0b63b block: add a bi_error field to struct bio
Currently we have two different ways to signal an I/O error on a BIO:

 (1) by clearing the BIO_UPTODATE flag
 (2) by returning a Linux errno value to the bi_end_io callback

The first one has the drawback of only communicating a single possible
error (-EIO), and the second one has the drawback of not beeing persistent
when bios are queued up, and are not passed along from child to parent
bio in the ever more popular chaining scenario.  Having both mechanisms
available has the additional drawback of utterly confusing driver authors
and introducing bugs where various I/O submitters only deal with one of
them, and the others have to add boilerplate code to deal with both kinds
of error returns.

So add a new bi_error field to store an errno value directly in struct
bio and remove the existing mechanisms to clean all this up.

     Philipp Reisner).

   - A use-after-free fix for null_blk from Ming Lei.

   - Also from Ming Lei is a performance patch for virtio-blk, which can
     net us a 3x win on kvm platforms where world notification is
     expensive.

   - Ming Lei also fixed a stall issue in virtio-blk, due to a race
     between queue start/stop and resource limits.

   - A small batch of fixes for xen-blk{back,front} from Olaf Hering and
     Valentin Priescu"

 "It's a big(ish) round this time, lots of development effort has gone
  into blk-mq in the last 3 months.  Generally we're heading to where
  3.16 will be a feature complete and performant blk-mq.  scsi-mq is
  progressing nicely and will hopefully be in 3.17.  A nvme port is in
  progress, and the Micron pci-e flash driver, mtip32xx, is converted
  and will be sent in with the driver pull request for 3.16.

  This pull request contains:

   - Lots of prep and support patches for scsi-mq have been integrated.
     All from Christoph.

   - API and code cleanups for blk-mq from Christoph.

   - Lots of good corner case and error handling cleanup fixes for
     blk-mq from Ming Lei.

   - A flew of blk-mq updates from me:

     * Provide strict mappings so that the driver can rely on the CPU
       to queue mapping.  This enables optimizations in the driver.

     * Provided a bitmap tagging instead of percpu_ida, which never
       really worked well for blk-mq.  percpu_ida relies on the fact
       that we have a lot more tags available than we really need, it
       fails miserably for cases where we exhaust (or are close to
       exhausting) the tag space.

     * Provide sane support for shared tag maps, as utilized by scsi-mq

     * Various fixes for IO timeouts.

     * API cleanups, and lots of perf tweaks and optimizations.

   - Remove 'buffer' from struct request.  This is ancient code, from
     when requests were always virtually mapped.  Kill it, to reclaim
     some space in struct request.  From me.

   - Remove 'magic' from blk_plug.  Since we store these on the stack
     and since we've never caught any actual bugs with this, lets just
     get rid of it.  From me.

   - Only call part_in_flight() once for IO completion, as includes two
     atomic reads.  Hopefully we'll get a better implementation soon, as
     the part IO stats are now one of the more expensive parts of doing
     IO on blk-mq.  From me.

   - File migration of block code from {mm,fs}/ to block/.  This
     includes bio.c, bio-integrity.c, bounce.c, and ioprio.c.  From me,
     from a discussion on lkml.

  That should describe the meat of the pull request.  Also has various
  little fixes and cleanups from Dave Jones, Shaohua Li, Duan Jiong,
  Fengguang Wu, Fabian Frederick, Randy Dunlap, Robert Elliott, and Sam

Jiri Kosina 6314a108ec floppy: do not corrupt bio.bi_flags when reading block 0
Commit 41a55b4de3 ("floppy: silence warning during disk test") caused
bio.bi_flags being overwritten, and its initialization to BIO_UPTODATE
in bio_init() to be lost.

This was unnoticed until 7b7b68bba5 ("floppy: bail out in open() if
drive is not responding to block0 read"), because the error value wasn't
checked for in the bio completion callback.

Now we are actually looking at the error, and the loss of BIO_UPTODATE
causes EIO to be wrongly passed to the callback, which confuses the
FD_OPEN_SHOULD_FAIL_BIT logic.

Fix this by not destroying previous value of bi_flags when setting
BIO_QUIET.

Jens Axboe b4f42e2831 block: remove struct request buffer member
This was used in the olden days, back when onions were proper
yellow. Basically it mapped to the current buffer to be
transferred. With highmem being added more than a decade ago,
most drivers map pages out of a bio, and rq->buffer isn't
pointing at anything valid.

Convert old style drivers to just use bio_data().

For the discard payload use case, just reference the page
in the bio.

Tejun Heo 75ddb38f09 floppy: don't use PREPARE_[DELAYED_]WORK
PREPARE_[DELAYED_]WORK() are being phased out.  They have few users
and a nasty surprise in terms of reentrancy guarantee as workqueue
considers work items to be different if they don't have the same work
function.

floppy has been multiplexing floppy_work and fd_timer with multiple
work functions.  Introduce floppy_work_workfn() and fd_timer_workfn()
which invoke floppy_work_fn and fd_timer_fn respectively and always
use the two functions as the work functions and update the users to
set floppy_work_fn and fd_timer_fn instead of overriding work
functions using PREPARE_[DELAYED_]WORK().

It would probably be best to route this with other related updates
through the workqueue tree.

Lightly tested using qemu.

Jiri Kosina 7b7b68bba5 floppy: bail out in open() if drive is not responding to block0 read
In case reading of block 0 during open() fails, it is not the right thing
to let open() succeed.

Fix this by introducing FD_OPEN_SHOULD_FAIL_BIT flag, and setting it in
case the bio callback encounters an error while trying to read block 0.

As a bonus, this works around certain broken userspace (blkid), which is
not able to properly handle read()s returning IO errors. Hence be nice to
those, and bail out during open() already; if block 0 is not readable,
read()s are not going to provide any meaningful data anyway.

Kent Overstreet 7988613b0e block: Convert bio_for_each_segment() to bvec_iter
More prep work for immutable biovecs - with immutable bvecs drivers
won't be able to use the biovec directly, they'll need to use helpers
that take into account bio->bi_iter.bi_bvec_done.

This updates callers for the new usage without changing the
implementation yet.

Kent Overstreet 4f024f3797 block: Abstract out bvec iterator
Immutable biovecs are going to require an explicit iterator. To
implement immutable bvecs, a later patch is going to add a bi_bvec_done
member to this struct; for now, this patch effectively just renames
things.

Al Viro db2a144bed block_device_operations->release() should return void
The value passed is 0 in all but "it can never happen" cases (and those
only in a couple of drivers) *and* it would've been lost on the way
out anyway, even if something tried to pass something meaningful.

Herton Ronaldo Krzesinski b54e1f8889 floppy: don't call alloc_ordered_workqueue inside the alloc_disk loop
Since commit 070ad7e ("floppy: convert to delayed work and single-thread
wq"), we end up calling alloc_ordered_workqueue multiple times inside
the loop, which shouldn't be intended. Besides the leak, other side
effect in the current code is if blk_init_queue fails, we would end up
calling unregister_blkdev even if we didn't call yet register_blkdev.

Just moved the allocation of floppy_wq before the loop, and adjusted the
code accordingly.

Tejun Heo e7c2f96744 workqueue: use mod_delayed_work() instead of __cancel + queue
Now that mod_delayed_work() is safe to call from IRQ handlers,
__cancel_delayed_work() followed by queue_delayed_work() can be
replaced with mod_delayed_work().

Most conversions are straight-forward except for the following.

* net/core/link_watch.c: linkwatch_schedule_work() was doing a quite
  elaborate dancing around its delayed_work.  Collapse it such that
  linkwatch_work is queued for immediate execution if LW_URGENT and
  existing timer is kept otherwise.

Linus Torvalds dab058fd5f floppy: cancel any pending fd_timeouts before adding a new one
In commit 070ad7e793 ("floppy: convert to delayed work and
single-thread wq") the 'fd_timeout' timer was converted to a delayed
work.  However, the "del_timer(&fd_timeout)" was lost in the process,
and any previous pending timeouts would stay active when we then
re-queued the timeout.

This resulted in the floppy probe sequence having a (stale) 20s timeout
rather than the intended 3s timeout, and thus made booting with the
floppy driver (but no actual floppy controller) take much longer than it
should.

Of course, there's little reason for most people to compile the floppy
driver into the kernel at all, which is why most people never noticed.

Canceling the delayed work where we used to do the del_timer() fixes the
issue, and makes the floppy probing use the proper new timeout instead.
The three second timeout is still very wasteful, but better than the 20s
one.

Jiri Kosina bfa10b8c98 floppy: remove floppy-specific O_EXCL handling
Block layer now handles O_EXCL in a generic way for block devices.

The semantics is however different for floppy and all other block devices,
as floppy driver contains its own O_EXCL handling.

The semantics for all-but-floppy bdevs is "there can be at most one O_EXCL
open of this file", while for floppy bdev the semantics is "if someone has
the bdev open with O_EXCL, noone else can open it".

There is actual userspace-observable change in behavior because of this
since commit e525fd89d3 ("block: make blkdev_get/put() handle exclusive
access") -- on kernels containing this commit, mount of /dev/fd0 causes
the fd0 block device be claimed with _EXCL, preventing subsequent
open(/dev/fd0).

Bring things back into shape, i.e.  make it possible, analogically to
other block devices, to mount the floppy and open() it afterwards --
remove the floppy-specific handling and let the generic bdev code O_EXCL
handling take over.

(scheduled_work) and timers (fd_timeout, fd_timer). Due to slowness
of the actual floppy devices, those races are never (at least to my
knowledge) triggered on a bare floppy metal. However on virtualized
(emulated) floppy drives, which are of course magnitudes faster
than the real ones, these races trigger reliably. They usually exhibit

This patch converts all the timers and bottom halfs to be processed in a single
workqueue. This aproach has been already discussed back in 2010 if I remember
correctly, and Acked by Linus [1], but it then never made it to the tree.

This all is based on original idea and code of Stephen Hemminger.  I have
ported original Stepen's code to the current state of the floppy driver, and
performed quite some testing (on real hardware), which didn't reveal any issues
(this includes not only writing and reading data, but also formatting
(unfortunately I didn't find any Double-Density disks any more)). Ability to
handle errors properly (supplying known bad floppies) has also been verified.

Len Brown f6365201d8 x86: Remove the ancient and deprecated disable_hlt() and enable_hlt() facility
The X86_32-only disable_hlt/enable_hlt mechanism was used by the
32-bit floppy driver. Its effect was to replace the use of the
HLT instruction inside default_idle() with cpu_relax() - essentially
it turned off the use of HLT.

This workaround was commented in the code as:

 "disable hlt during certain critical i/o operations"

 "This halt magic was a workaround for ancient floppy DMA
  wreckage. It should be safe to remove."

H. Peter Anvin additionally adds:

 "To the best of my knowledge, no-hlt only existed because of
  flaky power distributions on 386/486 systems which were sold to
  run DOS.  Since DOS did no power management of any kind,
  including HLT, the power draw was fairly uniform; when exposed
  to the much hhigher noise levels you got when Linux used HLT
  caused some of these systems to fail.

  They were by far in the minority even back then."

Alan Cox further says:

 "Also for the Cyrix 5510 which tended to go castors up if a HLT
  occurred during a DMA cycle and on a few other boxes HLT during
  DMA tended to go astray.

  Do we care ? I doubt it. The 5510 was pretty obscure, the 5520
  fixed it, the 5530 is probably the oldest still in any kind of
  use."

So, let's finally drop this.

Vivek Goyal 3f9a5aabd0 floppy: Cleanup disk->queue before caling put_disk() if add_disk() was never called
add_disk() takes gendisk reference on request queue. If driver failed during
initialization and never called add_disk() then that extra reference is not
taken. That reference is put in put_disk(). floppy driver allocates the
disk, allocates queue, sets disk->queue and then relizes that floppy
controller is not present. It tries to tear down everything and tries to
put a reference down in put_disk() which was never taken.

In such error cases cleanup disk->queue before calling put_disk() so that
we never try to put down a reference which was never taken in first place.

Al Viro ff01bb4832 fs: move code out of buffer.c
Move invalidate_bdev, block_sync_page into fs/block_dev.c.  Export
kill_bdev as well, so brd doesn't have to open code it.  Reduce
buffer_head.h requirement accordingly.

Removed a rather large comment from invalidate_bdev, as it looked a bit
obsolete to bother moving.  The small comment replacing it says enough.

Carsten Emde 6c4867f646 floppy: use del_timer_sync() in init cleanup
When no floppy is found the module code can be released while a timer
function is pending or about to be executed.

Len Brown 3b70b2e5fc x86 idle floppy: deprecate disable_hlt()
Plan to remove floppy_disable_hlt in 2012, an ancient
workaround with comments that it should be removed.

This allows us to remove clutter and a run-time branch
from the idle code.

WARN_ONCE() on invocation until it is removed.

Tejun Heo 9fd097b149 block: unexport DISK_EVENT_MEDIA_CHANGE for legacy/fringe drivers
In-kernel disk event polling doesn't matter for legacy/fringe drivers
and may lead to infinite event loop if ->check_events() implementation
generates events on level condition instead of edge.

Now that block layer supports suppressing exporting unlisted events,
simply leaving disk->events cleared allows these drivers to keep the
internal revalidation behavior intact while avoiding weird
interactions with userland event handler.
