Replace the current semi-arbitrary distribution of inline assemblies:
- Inline assemblies used by CIO go into ioasm.h
- Data definitions used by inline assemblies go into cio.h
Beyond cleaning up the current structure this is also required for
use of tracepoints in inline assemblies introduced by a follow-on
patch.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.
Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

To fetch a pending channel report word (crw) we use a kernel
thread which triggers stcrw and sleeps on a semaphore. The s390
machine check handler uses crw_handle_channel_report to handle
one crw if needed.
This patch replaces the semaphore with a waitqueue (to block the
kernel thread) and an atomic_t (to count the number of pending
requests).
By this we achieve the ability to force this thread to check for
a pending crw (independent on when it is triggered by the machine
check handler) and wait for this action to finish.

The crw_unregister_handler uses xchg + synchronize_sched when
unregistering a crw_handler.
This doesn't protect crw_collect_info to potentially jump to NULL since
it has unlocked code like this:
if (crw_handlers[i])
crw_handlers[i](NULL, NULL, 1);
So add a mutex which protects the crw handler array for changes.
