Boris Ostrovsky dc6416f1d7 xen/x86: Call cpu_startup_entry(CPUHP_AP_ONLINE_IDLE) from xen_play_dead()
This call has always been missing from xen_play dead() but until
recently this was rather benign. With new cpu hotplug framework
(commit 8df3e07e7f ("cpu/hotplug: Let upcoming cpu bring itself fully up").
however this call is required, otherwise a hot-plugged CPU will not
be properly brough up (by never calling cpuhp_online_idle())

Boris Ostrovsky ed6069be72 xen/apic: Provide Xen-specific version of cpu_present_to_apicid APIC op
Currently Xen uses default_cpu_present_to_apicid() which will always
report BAD_APICID for PV guests since x86_bios_cpu_apic_id is initialised
to that value and is never updated.

With commit 1f12e32f4c ("x86/topology: Create logical package id"), this
op is now called by smp_init_package_map() when deciding whether to call
topology_update_package_map() which sets cpu_data(cpu).logical_proc_id.
The latter (as topology_logical_package_id(cpu)) may be used, for example,
by cpu_to_rapl_pmu() as an array index. Since uninitialized
logical_package_id is set to -1, the index will become 64K which is
obviously problematic.

While RAPL code (and any other users of logical_package_id) should be
careful in their assumptions about id's validity, Xen's
cpu_present_to_apicid op should still provide value consistent with its
own xen_apic_read(APIC_ID).

 "This tree adds a new kernel build-time object file validation feature
  (ONFIG_STACK_VALIDATION=y): kernel stack frame correctness validation.
  It was written by and is maintained by Josh Poimboeuf.

  The motivation: there's a category of hard to find kernel bugs, most
  of them in assembly code (but also occasionally in C code), that
  degrades the quality of kernel stack dumps/backtraces.  These bugs are
  hard to detect at the source code level.  Such bugs result in
  incorrect/incomplete backtraces most of time - but can also in some
  rare cases result in crashes or other undefined behavior.

  The build time correctness checking is done via the new 'objtool'
  user-space utility that was written for this purpose and which is
  hosted in the kernel repository in tools/objtool/.  The tool's (very
  simple) UI and source code design is shaped after Git and perf and
  shares quite a bit of infrastructure with tools/perf (which tooling
  infrastructure sharing effort got merged via perf and is already
  upstream).  Objtool follows the well-known kernel coding style.

  Objtool does not try to check .c or .S files, it instead analyzes the
  resulting .o generated machine code from first principles: it decodes
  the instruction stream and interprets it.  (Right now objtool supports
  the x86-64 architecture.)

  From tools/objtool/Documentation/stack-validation.txt:

   "The kernel CONFIG_STACK_VALIDATION option enables a host tool named
    objtool which runs at compile time.  It has a "check" subcommand
    which analyzes every .o file and ensures the validity of its stack
    metadata.  It enforces a set of rules on asm code and C inline
    assembly code so that stack traces can be reliable.

    Currently it only checks frame pointer usage, but there are plans to
    add CFI validation for C files and CFI generation for asm files.

    For each function, it recursively follows all possible code paths
    and validates the correct frame pointer state at each instruction.

    It also follows code paths involving special sections, like
    .altinstructions, __jump_table, and __ex_table, which can add
    alternative execution paths to a given instruction (or set of
    instructions).  Similarly, it knows how to follow switch statements,
    for which gcc sometimes uses jump tables."

  When this new kernel option is enabled (it's disabled by default), the
  tool, if it finds any suspicious assembly code pattern, outputs
  warnings in compiler warning format:

  ... so that scripts that pick up compiler warnings will notice them.
  All known warnings triggered by the tool are fixed by the tree, most
  of the commits in fact prepare the kernel to be warning-free.  Most of
  them are bugfixes or cleanups that stand on their own, but there are
  also some annotations of 'special' stack frames for justified cases
  such entries to JIT-ed code (BPF) or really special boot time code.

  There are two other long-term motivations behind this tool as well:

   - To improve the quality and reliability of kernel stack frames, so
     that they can be used for optimized live patching.

   - To create independent infrastructure to check the correctness of
     CFI stack frames at build time.  CFI debuginfo is notoriously
     unreliable and we cannot use it in the kernel as-is without extra
     checking done both on the kernel side and on the build side.

  The quality of kernel stack frames matters to debuggability as well,
  so IMO we can merge this without having to consider the live patching
  or CFI debuginfo angle"

Andy Lutomirski b7a584598a x86/iopl/64: Properly context-switch IOPL on Xen PV
On Xen PV, regs->flags doesn't reliably reflect IOPL and the
exit-to-userspace code doesn't change IOPL.  We need to context
switch it manually.

I'm doing this without going through paravirt because this is
specific to Xen PV.  After the dust settles, we can merge this with
the 32-bit code, tidy up the iopl syscall implementation, and remove
the set_iopl pvop entirely.

 "What's wrong with the current cpu hotplug infrastructure?

   - Asymmetry

     The hotplug notifier mechanism is asymmetric versus the bringup and
     teardown.  This is mostly caused by the notifier mechanism.

   - Largely undocumented dependencies

     While some notifiers use explicitely defined notifier priorities,
     we have quite some notifiers which use numerical priorities to
     express dependencies without any documentation why.

   - Control processor driven

     Most of the bringup/teardown of a cpu is driven by a control
     processor.  While it is understandable, that preperatory steps,
     like idle thread creation, memory allocation for and initialization
     of essential facilities needs to be done before a cpu can boot,
     there is no reason why everything else must run on a control
     processor.  Before this patch series, bringup looks like this:

       Control CPU                     Booting CPU

       do preparatory steps
       kick cpu into life

                                       do low level init

       sync with booting cpu           sync with control cpu

       bring the rest up

   - All or nothing approach

     There is no way to do partial bringups.  That's something which is
     really desired because we waste e.g.  at boot substantial amount of
     time just busy waiting that the cpu comes to life.  That's stupid
     as we could very well do preparatory steps and the initial IPI for
     other cpus and then go back and do the necessary low level
     synchronization with the freshly booted cpu.

   - Minimal debuggability

     Due to the notifier based design, it's impossible to switch between
     two stages of the bringup/teardown back and forth in order to test
     the correctness.  So in many hotplug notifiers the cancel
     mechanisms are either not existant or completely untested.

   - Notifier [un]registering is tedious

     To [un]register notifiers we need to protect against hotplug at
     every callsite.  There is no mechanism that bringup/teardown
     callbacks are issued on the online cpus, so every caller needs to
     do it itself.  That also includes error rollback.

  What's the new design?

     The base of the new design is a symmetric state machine, where both
     the control processor and the booting/dying cpu execute a well
     defined set of states.  Each state is symmetric in the end, except
     for some well defined exceptions, and the bringup/teardown can be
     stopped and reversed at almost all states.

     So the bringup of a cpu will look like this in the future:

       Control CPU                     Booting CPU

       do preparatory steps
       kick cpu into life

                                       do low level init

       sync with booting cpu           sync with control cpu

                                       bring itself up

     The synchronization step does not require the control cpu to wait.
     That mechanism can be done asynchronously via a worker or some
     other mechanism.

     The teardown can be made very similar, so that the dying cpu cleans
     up and brings itself down.  Cleanups which need to be done after
     the cpu is gone, can be scheduled asynchronously as well.

  There is a long way to this, as we need to refactor the notion when a
  cpu is available.  Today we set the cpu online right after it comes
  out of the low level bringup, which is not really correct.

  The proper mechanism is to set it to available, i.e. cpu local
  threads, like softirqd, hotplug thread etc. can be scheduled on that
  cpu, and once it finished all booting steps, it's set to online, so
  general workloads can be scheduled on it.  The reverse happens on
  teardown.  First thing to do is to forbid scheduling of general
  workloads, then teardown all the per cpu resources and finally shut it
  off completely.

  This patch series implements the basic infrastructure for this at the
  core level.  This includes the following:

   - Basic state machine implementation with well defined states, so
     ordering and prioritization can be expressed.

   - Interfaces to [un]register state callbacks

     This invokes the bringup/teardown callback on all online cpus with
     the proper protection in place and [un]installs the callbacks in
     the state machine array.

     For callbacks which have no particular ordering requirement we have
     a dynamic state space, so that drivers don't have to register an
     explicit hotplug state.

     If a callback fails, the code automatically does a rollback to the
     previous state.

   - Sysfs interface to drive the state machine to a particular step.

     This is only partially functional today.  Full functionality and
     therefor testability will be achieved once we converted all
     existing hotplug notifiers over to the new scheme.

   - Run all CPU_ONLINE/DOWN_PREPARE notifiers on the booting/dying
     processor:

     In a previous step of this work we've done a full tree mechanical
     conversion of all hotplug notifiers to the new scheme.  The balance
     is a net removal of about 4000 lines of code.

     This is not included in this series, as we decided to take a
     different approach.  Instead of mechanically converting everything
     over, we will do a proper overhaul of the usage sites one by one so
     they nicely fit into the symmetric callback scheme.

     I decided to do that after I looked at the ugliness of some of the
     converted sites and figured out that their hotplug mechanism is
     completely buggered anyway.  So there is no point to do a
     mechanical conversion first as we need to go through the usage
     sites one by one again in order to achieve a full symmetric and
     testable behaviour"

ELF spec is unclear about whether .bss must me cleared by the loader.
Currently the domain builder does it when loading the guest but because
it is not (or rather may not be) guaranteed we should zero it out
explicitly.

Thomas Gleixner fc6d73d674 arch/hotplug: Call into idle with a proper state
Let the non boot cpus call into idle with the corresponding hotplug state, so
the hotplug core can handle the further bringup. That's a first step to
convert the boot side of the hotplugged cpus to do all the synchronization
with the other side through the state machine. For now it'll only start the
hotplug thread and kick the full bringup of the cpu.

xen_irq_enable_direct(), xen_restore_fl_direct(), and check_events() are
callable non-leaf functions which don't honor CONFIG_FRAME_POINTER,

Ouyang Zhaowei (Charles) 6a1f513776 x86/xen: don't reset vcpu_info on a cancelled suspend
On a cancelled suspend the vcpu_info location does not change (it's
still in the per-cpu area registered by xen_vcpu_setup()).  So do not
call xen_hvm_init_shared_info() which would make the kernel think its
back in the shared info.  With the wrong vcpu_info, events cannot be
received and the domain will hang after a cancelled suspend.

Adding the rtc platform device in non-privileged Xen PV guests causes
an IRQ conflict because these guests do not have legacy PIC and may
allocate irqs in the legacy range.

In a single VCPU Xen PV guest we should have:

But hvc_console cannot get its interrupt because it is already in use
by rtc0 and the console does not work.

We can avoid this problem by realizing that unprivileged PV guests (both
Xen and lguests) are not supposed to have rtc_cmos device and so
adding it is not necessary.

Privileged guests (i.e. Xen's dom0) do use it but they should not have
irq conflicts since they allocate irqs above legacy range (above
gsi_top, in fact).

Instead of explicitly testing whether the guest is privileged we can
extend pv_info structure to include information about guest's RTC
support.

Boris Ostrovsky 91e2eea98f x86/xen: Avoid fast syscall path for Xen PV guests
After 32-bit syscall rewrite, and specifically after commit:

  5f310f739b ("x86/entry/32: Re-implement SYSENTER using the new C path")

... the stack frame that is passed to xen_sysexit is no longer a
"standard" one (i.e. it's not pt_regs).

Since we end up calling xen_iret from xen_sysexit we don't need
to fix up the stack and instead follow entry_SYSENTER_32's IRET
path directly to xen_iret.

We can do the same thing for compat mode even though stack does
not need to be fixed. This will allow us to drop usergs_sysret32
paravirt op (in the subsequent patch)

Boris Ostrovsky 20f36e0380 xen/x86/pvh: Use HVM's flush_tlb_others op
Using MMUEXT_TLB_FLUSH_MULTI doesn't buy us much since the hypervisor
will likely perform same IPIs as would have the guest.

More importantly, using MMUEXT_INVLPG_MULTI may not to invalidate the
guest's address on remote CPU (when, for example, VCPU from another guest
is running there).

Boris Ostrovsky de0afc9bde xen: Resume PMU from non-atomic context
Resuming PMU currently triggers a warning from ___might_sleep() (assuming
CONFIG_DEBUG_ATOMIC_SLEEP is set) when xen_pmu_init() allocates GFP_KERNEL
page because we are in state resembling atomic context.

Move resuming PMU to xen_arch_resume() which is called in regular context.
For symmetry move suspending PMU to xen_arch_suspend() as well.

Juergen Gross d6ccc3ec95 x86/paravirt: Remove paravirt ops pmd_update[_defer] and pte_update_defer
pte_update_defer can be removed as it is always set to the same
function as pte_update. So any usage of pte_update_defer() can be
replaced by pte_update().

pmd_update and pmd_update_defer are always set to paravirt_nop, so they
can just be nuked.

Boris Ostrovsky 5fdf5d37f4 x86/xen: Avoid fast syscall path for Xen PV guests
After 32-bit syscall rewrite, and specifically after commit:

  5f310f739b ("x86/entry/32: Re-implement SYSENTER using the new C path")

... the stack frame that is passed to xen_sysexit is no longer a
"standard" one (i.e. it's not pt_regs).

Since we end up calling xen_iret from xen_sysexit we don't need
to fix up the stack and instead follow entry_SYSENTER_32's IRET
path directly to xen_iret.

We can do the same thing for compat mode even though stack does
not need to be fixed. This will allow us to drop usergs_sysret32
paravirt op (in the subsequent patch)

Zhenzhong Duan abed7d0710 xen: fix the check of e_pfn in xen_find_pfn_range
On some NUMA system, after dom0 up, we see below warning even if there are
enough pfn ranges that could be used for remapping:
"Unable to find available pfn range, not remapping identity pages"

Fix it to avoid getting a memory region of zero size in xen_find_pfn_range.

David Vrabel 98dd166ea3 x86/xen/p2m: hint at the last populated P2M entry
With commit 633d6f17cd (x86/xen: prepare
p2m list for memory hotplug) the P2M may be sized to accomdate a much
larger amount of memory than the domain currently has.

When saving a domain, the toolstack must scan all the P2M looking for
populated pages.  This results in a performance regression due to the
unnecessary scanning.

Instead of reporting (via shared_info) the maximum possible size of
the P2M, hint at the last PFN which might be populated.  This hint is
increased as new leaves are added to the P2M (in the expectation that
they will be used for populated entries).

Malcolm Crossley 64c98e7f49 x86/xen: Do not clip xen_e820_map to xen_e820_map_entries when sanitizing map
Sanitizing the e820 map may produce extra E820 entries which would result in
the topmost E820 entries being removed. The removed entries would typically
include the top E820 usable RAM region and thus result in the domain having
signicantly less RAM available to it.

Fix by allowing sanitize_e820_map to use the full size of the allocated E820
array.
